Abstract:The purpose of this work is to give a contribution to the understanding of the COVID-19 contagion in Italy. To this end, we developed a modified Susceptible-Infected-Recovered (SIR) model for the contagion, and we used official data of the pandemic up to March 30th, 2020 for identifying the parameters of this model. The non standard part of our approach resides in the fact that we considered as model parameters also the initial number of susceptible individuals, as well as the proportionality factor relating the detected number of positives with the actual (and unknown) number of infected individuals. Identifying the contagion, recovery and death rates as well as the mentioned parameters amounts to a non-convex identification problem that we solved by means of a two-dimensional grid search in the outer loop, with a standard weighted least-squares optimization problem as the inner step.

Abstract:In late December 2019, a novel strand of Coronavirus (SARS-CoV-2) causing a severe, potentially fatal respiratory syndrome (COVID-19) was identified in Wuhan, Hubei Province, China and is causing outbreaks in multiple world countries, soon becoming a pandemic. Italy has now become the most hit country outside of Asia: on March 16, 2020, the Italian Civil Protection documented a total of 27980 confirmed cases and 2158 deaths of people tested positive for SARS-CoV-2. In the context of an emerging infectious disease outbreak, it is of paramount importance to predict the trend of the epidemic in order to plan an effective control strategy and to determine its impact. This paper proposes a new epidemic model that discriminates between infected individuals depending on whether they have been diagnosed and on the severity of their symptoms. The distinction between diagnosed and non-diagnosed is important because non-diagnosed individuals are more likely to spread the infection than diagnosed ones, since the latter are typically isolated, and can explain misperceptions of the case fatality rate and of the seriousness of the epidemic phenomenon. Being able to predict the amount of patients that will develop life-threatening symptoms is important since the disease frequently requires hospitalisation (and even Intensive Care Unit admission) and challenges the healthcare system capacity. We show how the basic reproduction number can be redefined in the new framework, thus capturing the potential for epidemic containment. Simulation results are compared with real data on the COVID-19 epidemic in Italy, to show the validity of the model and compare different possible predicted scenarios depending on the adopted countermeasures.

Abstract:An epidemic disease caused by a new coronavirus has spread in Northern Italy with a strong contagion rate. We implement an SEIR model to compute the infected population and the number of casualties of this epidemic. The example may ideally regard the situation in the Italian Region of Lombardy, where the epidemic started on February 24, but by no means attempts to perform a rigorous case study in view of the lack of suitable data and the uncertainty of the different parameters, namely, the variation of the degree of home isolation and social distancing as a function of time, the initial number of exposed individuals and infected people, the incubation and infectious periods, and the fatality rate. First, we perform an analysis of the results of the model by varying the parameters and initial conditions (in order for the epidemic to start, there should be at least one exposed or one infectious human). Then, we consider the Lombardy case and calibrate the model with the number of dead individuals to date (May 5, 2020) and constrain the parameters on the basis of values reported in the literature. The peak occurs at day 37 (March 31) approximately, with a reproduction ratio R0 of 3 initially, 1.36 at day 22, and 0.8 after day 35, indicating different degrees of lockdown. The predicted death toll is approximately 15,600 casualties, with 2.7 million infected individuals at the end of the epidemic. The incubation period providing a better fit to the dead individuals is 4.25 days, and the infectious period is 4 days, with a fatality rate of 0.00144/day [values based on the reported (official) number of casualties]. The infection fatality rate (IFR) is 0.57%, and it is 2.37% if twice the reported number of casualties is assumed. However, these rates depend on the initial number of exposed individuals. If approximately nine times more individuals are exposed, there are three times more infected people at the end of the epidemic and IFR = 0.47%. If we relax these constraints and use a wider range of lower and upper bounds for the incubation and infectious periods, we observe that a higher incubation period (13 vs. 4.25 days) gives the same IFR (0.6 vs. 0.57%), but nine times more exposed individuals in the first case. Other choices of the set of parameters also provide a good fit to the data, but some of the results may not be realistic. Therefore, an accurate determination of the fatality rate and characteristics of the epidemic is subject to knowledge of the precise bounds of the parameters. Besides the specific example, the analysis proposed in this work shows how isolation measures, social distancing, and knowledge of the diffusion conditions help us to understand the dynamics of the epidemic. Hence, it is important to quantify the process to verify the effectiveness of the lockdown.

Spatial spread of COVID-19 outbreak in Italy using multiscale kinetic transport equations with uncertainty

Abstract:In this paper we introduce a space-dependent multiscale model to describe the spatial spread of an infectious disease under uncertain data with particular interest in simulating the onset of the COVID-19 epidemic in Italy. While virus transmission is ruled by a SEIAR type compartmental model, within our approach the population is given by a sum of commuters moving on a extra-urban scale and non commuters interacting only on the smaller urban scale. A transport dynamic of the commuter population at large spatial scales, based on kinetic equations, is coupled with a diffusion model for non commuters at the urban scale. Thanks to a suitable scaling limit, the kinetic transport model used to describe the dynamics of commuters, within a given urban area coincides with the diffusion equations that characterize the movement of non-commuting individuals. Because of the high uncertainty in the data reported in the early phase of the epidemic, the presence of random inputs in both the initial data and the epidemic parameters is included in the model. A robust numerical method is designed to deal with the presence of multiple scales and the uncertainty quantification process. In our simulations, we considered a realistic geographical domain, describing the Lombardy region, in which the size of the cities, the number of infected individuals, the average number of daily commuters moving from one city to another, and the epidemic aspects are taken into account through a calibration of the model parameters based on the actual available data. The results show that the model is able to describe correctly the main features of the spatial expansion of the first wave of COVID-19 in northern Italy.

Modelling the second wave of COVID-19 infections in France and Italy via a Stochastic SEIR model

Abstract:COVID-19 has forced quarantine measures in several countries across the world. These measures have proven to be effective in significantly reducing the prevalence of the virus. To date, no effective treatment or vaccine is available. In the effort of preserving both public health as well as the economical and social textures, France and Italy governments have partially released lockdown measures. Here we extrapolate the long-term behavior of the epidemics in both countries using a Susceptible-Exposed-Infected-Recovered (SEIR) model where parameters are stochastically perturbed to handle the uncertainty in the estimates of COVID-19 prevalence. Our results suggest that uncertainties in both parameters and initial conditions rapidly propagate in the model and can result in different outcomes of the epidemics leading or not to a second wave of infections. Using actual knowledge, asymptotic estimates of COVID-19 prevalence can fluctuate of order of ten millions units in both countries.

Abstract:We introduce an extended generalised logistic growth model for discrete outcomes, in which a network structure can be specified to deal with spatial dependence and time dependence is dealt with using an Auto-Regressive approach. A major challenge concerns the specification of the network structure, crucial to consistently estimate the canonical parameters of the generalised logistic curve, e.g. peak time and height. Parameters are estimated under the Bayesian framework, using the {\texttt{ Stan}} probabilistic programming language. The proposed approach is motivated by the analysis of the first and second wave of COVID-19 in Italy, i.e. from February 2020 to July 2020 and from July 2020 to December 2020, respectively. We analyse data at the regional level and, interestingly enough, prove that substantial spatial and temporal dependence occurred in both waves, although strong restrictive measures were implemented during the first wave. Accurate predictions are obtained, improving those of the model where independence across regions is assumed.
Identification of a time-varying SIR Model for Covid-19

Abstract:Throughout human history, epidemics have been a constant presence. Understanding their dynamics is essential to predict scenarios and make substantiated decisions. Mathematical models are powerful tools to describe an epidemic behavior. Among the most used, the compartmental ones stand out, dividing population into classes with well-defined characteristics. One of the most known is the $SIR$ model, based on a set of differential equations describing the rates of change of three categories over time. These equations take into account parameters such as the disease transmission rate and the recovery rate, which both change over time. However, classical models use constant parameters and can not describe the behavior of a disease over long periods. In this work, it is proposed a $SIR$ model with time-varying transmission rate parameter with a method to estimate this parameter based on an optimization problem, which minimizes the sum of the squares of the errors between the model and historical data. Additionally, based on the infection rates determined by the algorithm, the model's ability to predict disease activity in future scenarios was also investigated. Epidemic data released by the government of the State of Rio Grande do Sul in Brazil was used to evaluate the models, where the models shown a very good forecasting ability, resulting in errors for predicting the total number of accumulated infected persons of 0.13% for 7 days ahead and 0.6% for 14 days ahead.

Abstract:We study the datafor the cumulative as well as daily number of cases in the Covid-19 outbreak in China. The cumulative data can be fit to an empirical form obtained from a Susceptible-Infected-Removed (SIR) model studied on an Euclidean network previously. Plotting the number of cases against the distance from the epicenter for both China and Italy, we find an approximate power law variation with an exponent $\sim 1.85$ showing strongly that the spatial dependence plays a key role, a factor included in the model. We report here that the SIR model on the Eucledean network can reproduce with a high accuracy the data for China for given parameter values, and can also predict when the epidemic, at least locally, can be expected to be over.

Abstract:International Journal of Biomathematics, Ahead of Print. This paper handles with a time-space SIR model in two regions which have a common interface. The model is defined as coupled reaction–diffusion system with a robin boundary in interface to predict the immigration between the two regions. As known, the immigration has an influence on the spread of disease, this is caused by immigrants who could carry the disease from their areas to other places. For this reason, this paper deals with the existence of solutions to SIR model with disease spread between different regions by the Fadeo–Galerkin method and validate this result by the numerical simulations.

Abstract:We consider the SIR epidemiological model applied to the evolution of COVID-19 with two approaches. In the first place we fit a global SIR model, with time delay, and constant parameters throughout the outbreak, including the contagion rate. The contention measures are reflected on an effective reduced susceptible population $N_{eff}$. In the second approach we consider a time-dependent contagion rate that reflects the contention measures either through a step by step fitting process or by following an exponential decay. In this last model the population is considered the one of the country $N$. In the linear region of the differential equations, when the total population $N$ is large the predictions are independent of $N$. We apply these methodologies to study the spread of the pandemic in Argentina, Brazil, Colombia, Mexico, and South Africa for which the infection peaks are yet to be reached. In all of these cases we provide estimates for the reproduction and recovery rates. The scenario for a time varying contagion rate is optimistic, considering that reasonable measures are taken such that the reproduction factor $R_0$ decreases exponentially. The measured values for the recovery rate $\gamma$ are reported finding a universality of this parameter over various countries. We discuss the correspondence between the global SIR with effective population $N_{eff}$ and the evolution of the time local SIR.

Abstract:Calibration of a SIR (Susceptibles-Infected-Recovered) model with official international data for the COVID-19 pandemics provides a good example of the difficulties inherent the solution of inverse problems. Inverse modeling is set up in a framework of discrete inverse problems, which explicitly considers the role and the relevance of data. Together with a physical vision of the model, the present work addresses numerically the issue of parameters calibration in SIR models, it discusses the uncertainties in the data provided by international authorities, how they influence the reliability of calibrated model parameters and, ultimately, of model predictions.

Abstract:Abstract Since the outbreak of COVID-19, many COVID-19 research studies have proposed different models for predicting the trend of COVID-19. Among them, the prediction model based on mathematical epidemiology (SIR) is the most widely used, but most of these models are adapted in special situations based on various assumptions. In this study, a general adapted time-window based SIR prediction model is proposed, which is characterized by introducing a time window mechanism for dynamic data analysis and using machine learning method predicts the basic reproduction number and the exponential growth rate of the epidemic. We analyzed COVID-19 data from February to July 2020 in seven countries–––China, South Korea, Italy, Spain, Brazil, Germany and France, and the numerical results showed that the framework can effectively measure the real-time changes of the parameters during the epidemic, and error rate of predicting the number of COVID-19 infections in a single day is within 5%.

Abstract:This article describes a simple Susceptible Infected Recovered (SIR) model fitting with COVID-19 data for the month of march 2020 in New York (NY) state. The model is a classical SIR, but is non-autonomous; the rate of susceptible people becoming infected is adjusted over time in order to fit the available data. The death rate is also secondarily adjusted. Our fitting is made under the assumption that due to limiting number of tests, a large part of the infected population has not been tested positive. In the last part, we extend the model to take into account the daily fluxes between New Jersey (NJ) and NY states and fit the data for both states. Our simple model fits the available data, and illustrates typical dynamics of the disease: exponential increase, apex and decrease. The model highlights a decrease in the transmission rate over the period which gives a quantitative illustration about how lockdown policies reduce the spread of the pandemic. The coupled model with NY and NJ states shows a wave in NJ following the NY wave, illustrating the mechanism of spread from one attractive hot spot to its neighbor. }

Abstract:In Italy, 128,948 confirmed cases and 15,887 deaths of people who tested positive for SARS-CoV-2 were registered as of 5 April 2020. Ending the global SARS-CoV-2 pandemic requires implementation of multiple population-wide strategies, including social distancing, testing and contact tracing. We propose a new model that predicts the course of the epidemic to help plan an effective control strategy. The model considers eight stages of infection: susceptible (S), infected (I), diagnosed (D), ailing (A), recognized (R), threatened (T), healed (H) and extinct (E), collectively termed SIDARTHE. Our SIDARTHE model discriminates between infected individuals depending on whether they have been diagnosed and on the severity of their symptoms. The distinction between diagnosed and non-diagnosed individuals is important because the former are typically isolated and hence less likely to spread the infection. This delineation also helps to explain misperceptions of the case fatality rate and of the epidemic spread. We compare simulation results with real data on the COVID-19 epidemic in Italy, and we model possible scenarios of implementation of countermeasures. Our results demonstrate that restrictive social-distancing measures will need to be combined with widespread testing and contact tracing to end the ongoing COVID-19 pandemic.

Abstract:Background: Coronavirus Disease 2019 (COVID-19) is currently a global public health threat. Outside of China, Italy is one of the countries suffering the most with the COVID-19 epidemic. It is important to predict the epidemic trend of the COVID-19 epidemic in Italy to help develop public health strategies. Methods: We used time-series data of COVID-19 from Jan 22 2020 to Apr 02 2020. An infectious disease dynamic extended susceptible-infected-removed (eSIR) model, which covers the effects of different intervention measures in dissimilar periods, was applied to estimate the epidemic trend in Italy. The basic reproductive number was estimated using Markov Chain Monte Carlo methods and presented using the resulting posterior mean and 95% credible interval (CI). Hunan, with a similar total population number to Italy, was used as a comparative item. Results: In the eSIR model, we estimated that the mean of basic reproductive number for COVID-19 was 4.34 (95% CI, 3.04-6.00) in Italy and 3.16 (95% CI, 1.73-5.25) in Hunan. There would be a total of 182 051 infected cases (95%CI:116 114-274 378) under the current country blockade and the endpoint would be Aug 05 in Italy. Conclusion: Italy's current strict measures can efficaciously prevent the further spread of COVID-19 and should be maintained. Necessary strict public health measures should be implemented as soon as possible in other European countries with a high number of COVID-19 cases. The most effective strategy needs to be confirmed in further studies.

Ambiguity in the use of SIR models to fit epidemic incidence data

Abstract:When fitting a multi-parameter model to a data set, computer algorithms may suggest that a range of parameters provide equally reasonable fits, making the parameter estimation difficult. Here, we prove this fact for an SIR model. We say a set of parameter values is a good fit to outbreak data if the solution has the data's three most significant characteristics: the standard deviation, the mean time, and the total number of cases. In our model, in addition to the "basic reproduction number" $R_0$, three other parameters need to be estimated to fit a solution to outbreak data. We will show that those parameters can be chosen so that each gives a linear transformation of a solution's incidence data. As a result, we show that for every choice of $R_0>1$, there is a good fit for each outbreak. We also illustrate our results by providing the least square best fits of the New York City and London data sets of the Omicron variant of COVID-19. Furthermore, we show how versions of the SIR model with $N$ compartments have far more good fits- - indeed a high dimensional set of good fits -- for each target -- showing that more complicated models may have an even greater problem in overparametrizing outbreak characteristics.

Abstract:The COVID-19 epidemic has required countries to implement different containment strategies to limit its spread, like strict or weakened national lockdown rules and the application of age-stratified vaccine prioritization strategies. These interventions have in turn modified the age-dependent patterns of social contacts. In our recent paper, starting from the available age-structured real data at the national level, we identified, for the Italian case, specific virulence parameters for a two-age-structured COVID-19 epidemic compartmental model (under 60, and 60 years and over) in six different diseases transmission scenarios under concurrently adopted feedback interventions. An interpretation of how each external scenario modifies the age-dependent patterns of social contacts and the spread of COVID-19 disease has been accordingly provided. In this paper, which can be viewed as a sequel to the previous one, we mainly apply the same general methodology therein (involving the same dynamic model) to new data covering the three subsequent additional scenarios: (i) a mitigated coordinated intermittent regional action in conjunction with the II vaccination phase; (ii) a super-attenuated coordinated intermittent regional action in conjunction with the II vaccination phase; and (iii) a last step towards normality in conjunction with the start of the III vaccination phase. As a new contribution, we show how meaningful updated information can be drawn out, once the identification of virulence parameters, characterizing the two age groups within the latest three different phases, is successfully carried out. Nevertheless, differently from our previous paper, the global optimization procedure is carried out here with the number of susceptible individuals in each scenario being left free to change, to account for reinfection and immunity due to vaccination. Not only do the slightly different estimates we obtain for the previous scenarios not impact any of the previous considerations (and thus illustrate the robustness of the procedure), but also, and mainly, the new results provide a meaningful picture of the evolution of social behaviors, along with the goodness of strategic interventions.

Abstract:In this note we analyze the temporal dynamics of the coronavirus disease 2019 outbreak in China, Italy and France in the time window $22/01-09/03/2020$. A first analysis of simple day-lag maps points to some universality in the epidemic spreading, suggesting that simple mean-field models can be meaningfully used to gather a quantitative picture of the epidemic spreading, and notably the height and time of the peak of confirmed infected individuals. The analysis of the same data within a simple susceptible-(confirmed) infected-recovered-deaths model indicates that the kinetic parameter that describes the rate of recovery seems to be the same, irrespective of the country, while the infection and death rates appear to be more variable. The model places the peak in Italy around March 20$^{\rm th}$ 2020, with a maximum number of confirmed infected individuals of about 16,000.

The pandemic alternates periods of high and low infections, in a way that sheds a light on the nature of mathematical model that can be used for reliable predictions. The main hypothesis of the model presented here is that the oscillatory behavior is a structural feature of the outbreak, even without postulating a time-dependence of the coefficients. As such, it should be reflected by the presence of limit cycles as asymptotic solutions. This stems from the introduction of (i) a non-linear waning immunity based on the concept of immunity booster (already used for other pathologies); (ii) a fine description of the compartments with a discrimination between individuals infected/vaccinated for the first time, and individuals already infected/vaccinated, undergoing to new infections/doses. We provide a proof-of-concept that our novel model is capable of reproducing long-term oscillatory behavior of many infectious diseases, and, in particular, the periodic nature of the waves of infection. Periodic solutions are inherent to the model, and achieved without changing parameter values in time. This may represent an important step in the long-term modeling of COVID-19 and similar diseases, as the natural, unforced behavior of the solution shows the qualitative characteristics observed during the COVID-19 pandemic.
