Abstract:Purpose Intravenous sedation has been well accepted to allow dental restoration in uncooperative children while avoiding aspiration and laryngospasm; however, intravenous anesthetics such as propofol may lead to undesired effects such as respiratory depression and delayed recovery. The use of the bispectral index system (BIS), a monitoring system reflective of the hypnotic state, is con-troversial in the reduction in the risk of respiratory adverse events (RAEs), recovery time, the in-travenous drug dosage, and post-procedural events. The aim of the study is to evaluate whether BIS is advantageous in pediatric dental procedures. Methods A total of 206 cases, aged 2–8 years, receiving dental procedures under deep sedation with propofol using target-controlled infusion (TCI) technique were enrolled in the study. BIS level was not monitored in 93 children whereas it was for 113 children, among which BIS values were maintained between 50–65. Physiological variables and adverse events were recorded. Statistical analysis was conducted using Chi-square, Mann Whitney U, Independent Samples t and Wilcoxon signed tests, with a p value of <0.05 considered to be statistically significant. Results Although no statistical significance in the post-discharge events and total amount of propofol used was observed, a clear significance was identified in periprocedural adverse events (hypoxia, apnea, and recurrent cough, all p value < 0.05) and discharge time (63.4 ± 23.2 vs. 74.5 ± 24.0 min, p value < 0.001) between these two groups. Conclusions The application of BIS in combination with TCI may be beneficial for young children undergoing deep sedation for dental procedures.

[Effect of femoral shaft fracture for growth of the lower extremity in children].

Abstract:On the material of 15 children aged from 1.5 to 15 years (mean 6.6), who were treated conservatively the influence of fracture of the femoral shaft for growth of the lower extremity was assessed. In the follow-up from 1 to 14 years (mean 6.2) 2 patients had shortened of the fractured limb 5 mm, 5 patients had equal extremity and 8 had lengthened limb from 5 mm to 15 mm, at the same time 6 had overgrowth 10 mm and more. The overgrowth of the limb was observed only within of the femur, the legs were equal in every case. The overgrowth was remarked in the cases of spiral fracture with overriding, in fracture of the opposite site to dominant hand, in boys and in the cases which were treated by direct traction.

Abstract:Background. In intensive care, monitoring the depth of anesthesia during surgical procedures is a key element in the success of the medical operation and postoperative recovery. However, despite the development of anesthesia thanks to technological and pharmacological advances, its side effects such as underdose or overdose of hypnotics remain a major problem. Observation and monitoring must combine clinical observations (loss of consciousness and reactivity) with tools for real-time measurement of changes in the depth of anesthesia. Methodology. In this work, we will develop a noninvasive method for calculating, monitoring, and controlling the depth of general anesthesia during surgery. The objective is to reduce the effects of pharmacological usage of hypnotics and to ensure better quality recovery. Thanks to the overall activity of sets of neurons in the brain, we have developed a BIS technique based on bispectral analysis of the electroencephalographic signal EEG. Discussion. By collecting the electrical voltages from the brain, we distinguish light sleep from deep sleep according to the values of the BIS indicator (ranging from 0 : sleep to 100 : wake) and also control it by acting on the dosage of propofol and sevoflurane. We showed that the BIS value must be maintained during the operation and the anesthesia at a value greater than 60. Conclusion. This study showed that the BIS technology led to an optimization of the anesthetic management, the adequacy of the hypnotic dosage, and a better postoperative recovery.

Abstract:Chlorination of 1,2,3,4-tetracyclohexyl-cyclo-tetraphosphine (2) by PhICl(2) or PCl(5) in the presence of Me(3)SiOTf or GaCl(3) provides a stepwise approach to salts of the first cyclo-phosphino-chlorophosphonium cations [Cy(4)P(4)Cl](+) ([19](+)) and [Cy(4)P(4)Cl(2)](2+) ([20](2+)). The analogous iodo derivative [Cy(4)P(4)I](+) ([17](+)) is obtained as the tetraiodogallate salt from reaction of 2 with I(2) in the presence of GaI(3). Bispectral index monitoring of sedation depth during endoscopy: a meta-analysis with trial sequential analysis of randomized controlled trials.

Abstract:INTRODUCTION The Bispectral Index (BIS) provides an objective measure of the level of sedation and general anesthesia. We performed this meta-analysis and trial sequential analysis (TSA) of randomized clinical trials to clarify whether BIS monitoring is helpful in enhancing intraprocedual safety, shortening procedure duration or promoting recovery during sedation for endoscopic procedures. EVIDENCE ACQUISITION PubMed, EMBASE, CINAHL, the Cochrane Central Register of Controlled Trials [CENTRAL]) databases, reference lists of articles as well as relevant articles from "Google Scholar" were searched until May 31st, 2018. Randomized controlled trials (RCTs) comparing BIS with clinical signs for titration of sedation depth during endoscopy were screened and identified if they reported one of the following outcome measures: intraprocedual safety (hemodynamic stability and cardiorespiratory complications such as hypoxia, hypertension/hypotension, and bradycardia/tachycardia), procedure duration, recovery time and patient/endoscopist's satisfaction. EVIDENCE SYNTHESIS Twelve studies with 13 RCTs recruiting 1372 patients were identified with great inter-trial heterogeneity. Meta-analysis found that BIS monitoring of sedation depth was associated with lower incidences of intraprocedural hypoxia (P=0.009) compared with clinical signs which was not confirmed by TSA. Meta-analysis and TSA found that the endoscopic procedure duration (P=0.143), recovery time (P=0.083), satisfaction scores from both the cases (P=0.085) and endoscopists (P=0.125) and the incidences of hypertension/ hypotension (P=0.639) or heart rates (P=0.201) were similar between BIS and control group. CONCLUSIONS More high-quality large-sampled RCTs are needed to confirm whether BIS monitoring for endoscopy sedation helps reduce intraprocedural hypoxia. BIS monitoring fails to shorten procedure duration, promote recovery or boost satisfaction among patients and endoscopists.

Abstract:Background Assessment of hypnotic depth has been deeply concerned by anesthetists all the time.Bispectral index (BIS) monitoring is now the most widely used approach to quantitively measure the hypnotic component of anesthesia.With its extensive using,the value of BIS monitoring receives better understanding.Objective To review recent advances in clinical application of BIS monitoring.Content Using BIS monitoring to guide anaesthetic administration allows optimization of drug delivery according to the needs of individual patients.This helps to avoid both too light anesthesia which may lead to intraoperative awareness and too deep anaesthesia which may give rise to delayed emergence.These are particularly important for patients with chronic liver disease.Besides,recent studies indicate that BIS-guided anesthesia helps to accelerate cognitive function recovery.BIS monitoring is also found to be useful for patients at high risk of global cerebral ischemia.For these patients,a sudden reduction of BIS value may suggest the occurrence of cerebral hypoperfusion.Furthermore,BIS monitoring may be helpful in estimating prognosis of patients after cardiac arrest and cardiopulmonary resuscitation.However,the optimal duration of BIS monitoring and the optimal cutoff point of BIS value for prognostic estimation in these patients are still unclear.Trend Using BIS monitoring helps to avoid intraoperative awareness and too deep anaesthesia,which is correlated with faster recovery of cognitive function.BIS is also found to be very useful in patients with chronic liver disease,recognition of occurance of cerebral hypoperfusion and prognosis of patients after cardiac arrest and cardiopulmonary resuscitation.
Bispectral Index Monitoring Reduces the Dosage of Propofol and Adverse Events in Sedation for Endobronchial Ultrasound

Abstract:BACKGROUND: Current guidelines recommend monitoring the anesthetic depth of sedation during respiratory endoscopy by using clinical scales despite their subjective nature and the potential change in the level of sedation caused by frequent stimulation. Monitoring by means of the bispectral index (BIS) has shown its utility in reducing the use of drugs and their adverse events in general anesthesia, but evidence in prolonged sedation is insufficient.OBJECTIVE: Our objective was to evaluate BIS in patients undergoing endobronchial ultrasound (EBUS).METHODS: A randomized cohort study of 90 patients with mediastinal lymph node involvement and/or lung or mediastinal lesions for whom EBUS was indicated, comparing the modified observer's assessment of alertness/sedation scale clinical evaluation (n = 45) versus the BIS evaluation (n = 45) of sedation with propofol-remifentanil, was conducted in order to evaluate the clinical parameters, doses used, adverse events, and tolerance of the procedure.RESULTS: We found a shorter waking time and a significantly lower dose of total propofol in the BIS group. Significantly fewer overall adverse events were recorded in the BIS group and included desaturation, hypotension, and bradypnea. Tolerance was better in the BIS group. No significant differences were found in terms of cough, memory of the procedure, or the level of difficulty of EBUS on the part of the pulmonologists.CONCLUSIONS: BIS monitoring of sedation in EBUS makes it possible to reduce the dosage of propofol, thereby shortening the waking time and reducing adverse events. This form of monitoring should be taken into consideration in the future for systematic use in prolonged sedation, as in the case of EBUS.

Abstract:OBJECTIVE:To evaluate bispectral index (BIS) technology in post-operative patients under sedation undergoing mechanical ventilation according to Ramsay sedation scales in intensive care unit (ICU).METHODS:Thirty post-operative patients on mechanical ventilation were sedated with midazolam or propofol intravenously. All patients were evaluated for their levels of sedation by using BIS and the Ramsay scale simultaneously every 5 minutes with blind method. The total differences of median BIS in each sedation scale were compared. The BIS measurements and the corresponding Ramsay scale measurements were selected for analyzing the correlations. The sensitivity of BIS and Youden index were calculated, and the sensitivity and specificity of BIS were identified.RESULTS:The median BIS corresponding to each Ramsay scale decreased significantly along with the increasing of sedation depth. There were significant differences of median BIS in each Ramsay sedation scale (P<0.001). BIS was negatively correlated with Ramsay scale, and the r value was -0.794 (P<0.01). When the patient was optimally sedated (Ramsay scale 2-5 scores), the corresponding median BIS 95% confident interval (CI) were 61-84. The sensitivity and Youden index of BIS value with optimal or under sedation were the highest when BIS=81. The 95% CI of median BIS were 48-58 when Ramsay scale was 6 scores.CONCLUSION:BIS measurements evaluated in clinically relevant ranges compare favorably with Ramsay scale. BIS may be best used to identify and prevent over or under sedation of mechanical ventilated patients in the ICU.

What’s new in... Capnography Monitoring for Dental Conscious Sedation: A Clinical Review

Abstract:Capnography monitoring during conscious sedation is not currently required for dentistry in Britain and Ireland. Other countries have introduced guidelines and standards requiring capnography monitoring for procedural sedation. This review highlights the variability of procedural sedation including the setting, the position on the sedation continuum, and the routine use of supplemental oxygen. Specific research is required for conscious sedation in a dental setting to support standards and guidelines with regard to capnography monitoring. The Academy of Medical Royal Colleges and their Faculties emphasise that each specialty must produce its own guidance for the use of sedative techniques.1 Clinical practice guidelines for the monitoring and safe practice of sedation vary by specialty and institution. Standards are generally set from the best available evidence based research. There is a growing body of literature that recognises the potential additional value of capnography (ETCO2) monitoring during procedural sedation in different settings and for different sedation techniques.2-5 In these studies, capnography reduced the incidence of hypoxaemia during procedural sedation. A meta-analysis published by Waugh et al. (2010) concluded that end-tidal carbon dioxide monitoring is an important addition in detecting respiratory depression during procedural sedation.6 A more recent systematic review by Conway et al. (2016) concluded that patients monitored with capnography in addition to standard monitoring had a reduced risk of hypoxaemia compared to those with only standard monitoring.7 However, it has to be noted that both the Waugh and Conway reviews contained substantial statistical heterogenicity which is likely to affect the quality of the evidence. As research evidence for capnography monitoring from the medical settings studied became available, new standards for capnography monitoring were introduced in several countries (Table 1).
The Impact of BIS-Monitored Depth Regulation on Prognosis: Improving Outcomes for Patients with Chronic DoCs

Abstract:Abstract Background: The value of the bispectral index (BIS) for monitoring anesthesia in patients with disorder of consciousness (DoC) has not yet been determined. Objective: To investigate the effect of BIS-monitored depth regulation on the prognosis of patients with chronic DoCs undergoing spinal cord stimulation (SCS) implantation. Methods: Ninety-two 92 patients with DoC who underwent SCS implantation under general anesthesia were followed up for three months after surgery. Among the cohort, 83 patients met the inclusion criteria and were divided into the BIS (n = 45) and non-BIS (n = 38) groups. Relevant information was collected, along with patient characteristics. Coma recovery scale-revised (CRS-R) scores and outcomes were assessed preoperatively and three months postoperatively (CRS-R3m). Results: In the BIS group, the CRS-R3m score was higher than the preoperative CRS-R score; however, in the non-BIS group, there was no significant difference between the scores. The CRS-R3m score in the BIS group was higher than that in the non-BIS group. Consciousness improvement was evaluated in a multifactor logistic regression analysis, which revealed that dexmedetomidine was a high-risk factor for consciousness improvement. During follow-up, two patients in the BIS group experienced a decrease in consciousness, while two patients in the non-BIS group died. Conclusion: For patients with DoC undergoing SCS implantation under general anesthesia, intraoperative BIS monitoring to control the depth of general anesthesia sedation contributes to better postoperative outcomes. It is advisable to avoid the use of dexmedetomidine during the perioperative period, because it affects patient outcomes.
Anesthesia Depth Monitoring with Bispectral Index in Spinal Cord Stimulation Device Implantation for Chronic Disorders of Consciousness: A Retrospective Study

Abstract:Abstract Objective: To assess whether maintaining an appropriate depth of anesthesia using the Bispectral Index (BIS) could enhance outcomes following spinal cord stimulation (SCS) device implantation in patients with chronic disorders of consciousness (DoC). Methods: A total of 103 patients with DoC who underwent SCS implantation were reviewed between January 2019 and December 2021, of whom 83 met the inclusion and exclusion criteria. Patients were categorized into the BIS group (n = 45) and the non-BIS group (n = 38) based on whether BIS monitoring was used during the operation. Relevant data, such as disease course, cause, anesthesia, and operation time, were collected. Preoperative Coma Recovery Scale—Revised (CRS-R(preoperative)) score, postoperative CRS-R(24h), and postoperative CRS-R(3m) changes were recorded. Results: The CRS-R(3m) score was significantly higher in the BIS group than that in the non-BIS group (preoperative), with a statistically significant difference (p < 0.05). Furthermore, in the CRS-R(24h), the BIS group demonstrated a higher score than the non-BIS group, with a statistically significant difference noted (X2 = 8.787, p = 0.004). When assessing the improvement in consciousness in the multivariate logistic regression analysis model, it was revealed that the thalamus independently influenced the enhancement of consciousness (p < 0.05). Throughout the follow-up, one patient in the BIS group experienced a decline in consciousness from a minimally consciousness state (MCS)- to-vegetative state, whereas two patients in the non-BIS group passed away during the follow-up. Conclusion: For patients with DoC undergoing SCS implantation under general anesthesia, employing BIS monitoring during surgery to regulate the depth of sedation under general anesthesia can reduce propofol dosage and influence the brain network. Patients can benefit from improved hearing, as observed in the CRS-R(24h). We recommend using BIS to monitor the depth of anesthesia in patients with DoC to enhance overall patient outcomes.

Abstract:Abstract Objective: The objective of this study was to observe the effect of Bispectral Index (BIS)-monitored depth regulation of general anesthesia on the prognosis of patients with chronic disorders of consciousness (DOC) undergoing Spinal Cord Stimulation (SCS) implantation. Methods: A total of 92 DOC patients who underwent SCS implantation under general anesthesia were followed up for 3 months after surgery. Among them, 83 patients met the inclusion criteria and were divided into the BIS group (n=67) and non-BIS group (n=16). Patient characteristics, such as gender, age, height, etiology, medical history, anesthetic drugs used, BIS utilization, blood gas parameters, cortisol levels, and output, were recorded. Coma Recovery Scale-Revised (CRS-R) scores and outcomes were assessed before surgery, 3 days post-surgery, and 3 months post-surgery. Results: A total of 83 patients were included, with 67 patients in the BIS monitoring group and 16 patients in the non-BIS monitoring group. The causes of DOC included traumatic brain injury (TBI) in 33 cases, cerebrovascular disease (CVD) in 45 cases, and ischemia and anoxia (IAA) in 5 cases. There was no statistical difference in CRS-R 3d scores and CRS-R preoperative score between the BIS monitoring group and the non-BIS monitoring group before surgery (P > 0.05). However, the CRS-R 3m scores at 3 months post-surgery were significantly higher than the CRS-R preoperative score in both the BIS group and the non-BIS group (P < 0.05). There was no significant difference in the diagnoses between preoperative and 3 months post-surgery assessments in both groups (P > 0.05). In DOC patients with thalamic injury, BIS monitoring was correlated with the extent of thalamic injury (AUC=0.823, P=0.002). Similarly, in DOC patients with brain stem injury, BIS monitoring was correlated with the extent of brain stem injury (AUC=0.691, P=0.048). Blood gas analysis revealed that glucose levels were higher in the BIS group compared to the non-BIS group, and this difference was statistically significant (P < 0.05). In the BIS group, postoperative glucose levels were higher than preoperative levels (t=2.361, P =0.021). The use of dexmedetomidine as part of general anesthesia was found to be a risk factor associated with no improvement in CRS-R 3d scores (r=1.409, P=0.033). A positive correlation was observed between BIS awake and CRS-R preoperative scores, with a correlation coefficient of 0.47 (P =0.00). In the BIS group, 47.77% of patients showed improved consciousness, 50% remained unchanged, and 2.98% experienced decreased consciousness. In the non-BIS monitoring group, 31.11% showed improved consciousness, 44.35% remained unchanged while 69% experienced decreased consciousness. Conclusion: In patients with chronic disorders of consciousness (DOC) undergoing Spinal Cord Stimulation (SCS) implantation under general anesthesia, the utilization of Bispectral Index (BIS) monitoring to regulate the depth of sedation has been shown to improve postoperative outcomes. This approach is also correlated with the preoperative Coma Recovery Scale-Revised (CRS-R) score. To ensure the accuracy of the CRS-R assessment, it is recommended to use propofol, sevoflurane, and remifentanil for anesthesia maintenance, thereby avoiding the potential effect of dexmedetomidine on postoperative CRS-R 3d scores.

Abstract:Abstract Objective: To assess whether maintaining an appropriate depth of anesthesia using the Bispectral Index (BIS) could enhance outcomes following spinal cord stimulation (SCS) device implantation in patients with chronic disorders of consciousness (DoC). Methods: A total of 103 patients with DoC who underwent SCS implantation were reviewed between January 2019 and December 2021, of whom 83 met the inclusion and exclusion criteria. Patients were categorized into the BIS group (n = 45) and the non-BIS group (n = 38) based on whether BIS monitoring was used during the operation. Relevant data, such as disease course, cause, anesthesia, and operation time, were collected. Preoperative Coma Recovery Scale—Revised (CRS-R(preoperative)) score, postoperative CRS-R(24h), and postoperative CRS-R(3m) changes were recorded. Results: The CRS-R(3m) score was significantly higher in the BIS group than that in the non-BIS group (preoperative), with a statistically significant difference (p < 0.05). Furthermore, in the CRS-R(24h), the BIS group demonstrated a higher score than the non-BIS group, with a statistically significant difference noted (X2 = 8.787, p = 0.004). When assessing the improvement in consciousness in the multivariate logistic regression analysis model, it was revealed that the thalamus independently influenced the enhancement of consciousness (p < 0.05). Throughout the follow-up, one patient in the BIS group experienced a decline in consciousness from a minimally consciousness state (MCS)- to-vegetative state, whereas two patients in the non-BIS group passed away during the follow-up. Conclusion: For patients with DoC undergoing SCS implantation under general anesthesia, employing BIS monitoring during surgery to regulate the depth of sedation under general anesthesia can reduce propofol dosage and influence the brain network. Patients can benefit from improved hearing, as observed in the CRS-R(24h). We recommend using BIS to monitor the depth of anesthesia in patients with DoC to enhance overall patient outcomes.
Can the Bispectral Index Monitor Quantify Altered Level of Consciousness in Emergency Department Patients?

Abstract:OBJECTIVES: A daily part of emergency medicine practice includes assessing patients with altered levels of consciousness (ALOC). The authors hypothesized that a bispectral index monitor (BIS), a processed electroencephalographic monitor traditionally used to monitor patients under anesthesia, would represent an objective quantification of impairment of consciousness. They compared the BIS score with the Glasgow Coma Scale score (GCS) in emergency department (ED) patients with ALOC.METHODS: The authors performed a convenience sampling of ED adults presenting with ALOC (GCS <or= 14). Patients with abnormal baseline mental status were excluded, as were those who were unable to tolerate the forehead BIS leads. The blinded BIS value was recorded after the treating physician assigned a GCS to the patient. The BIS and GCS measurements were then correlated.RESULTS: Data were obtained for 38 patients (20 male, 18 female). The median age was 42 years (range 14 to 93 years). Despite being statistically significant (p = 0.0165), the correlation between the GCS and the BIS score was only moderate (Spearman's rho = 0.387) and displayed wide variability. For example, when the GCS was between 3 and 5, the corresponding BIS scores ranged from 47 to 98. When the GCS was between 12 and 14, the corresponding BIS scores ranged from 56 to 98. Receiver operating characteristic curves for BIS at each GCS threshold demonstrated low discriminatory power (areas under the curve range 0.61 to 0.73).CONCLUSIONS: BIS monitoring does not reliably correlate with GCS in ED patients with ALOC, and does not appear to have potential to accurately quantify impairment of consciousness in this setting.

Abstract:Continuous cerebrovascular reactivity monitoring in both neurocritical and intra-operative care has gained extensive interest in recent years, as it has documented associations with long-term outcomes (in neurocritical care populations) and cognitive outcomes (in operative cohorts). This has sparked further interest into the exploration and evaluation of methods to achieve an optimal cerebrovascular reactivity measure, where the individual patient is exposed to the lowest insult burden of impaired cerebrovascular reactivity. Recent literature has documented, in neural injury populations, the presence of a potential optimal sedation level in neurocritical care, based on the relationship between cerebrovascular reactivity and quantitative depth of sedation (using bispectral index (BIS)) – termed BISopt. The presence of this measure outside of neural injury patients has yet to be proven.

Abstract:Background. The bispectral index (BIS) and the rapidly extracted auditory evoked potentials index (A-line ARX Index or AAI) have been proposed as methods to measure the depth of sedation. A prospective study was designed to assess the performance of both these methods for measuring the depth of sedation induced by propofol or midazolam under epidural block.Methods. Thirty-two ASA I and II adult patients undergoing elective gynaecological surgery under low-thoracolumbar epidural block were studied. Eighteen patients received propofol (Group P: 20 mg bolus every 3 min) and 14 received midazolam (Group M: 0.5 mg bolus every 5 min) until an observer's assessment of alertness/sedation (OAA/S) scale score of 1 was achieved. AAI and BIS were monitored for different OAA/S scores.Results. AAI and BIS decreased and increased following the changes on the patients' OAA/S scores and correlated with sedation significantly. During the onset phase, the coefficients of Spearman's rank correlation for AAI and BIS were respectively 0.958 and 0.898 (P<0.001) for Group P, and 0.973 and 0.945 (P<0.001) for Group M. During the recovery phase in Group P, the coefficients were respectively 0.946 and 0.702 (P<0.001). Linear regression analysis showed that both AAI and BIS were linearly related to the OAA/S scores. The coefficients of Spearman's rank correlation and linear regression for AAI were all greater than those for BIS (P<0.05).Conclusions. Both AAI and BIS correlated well with the depth of sedation induced by propofol or midazolam under epidural block. AAI may be more valuable when monitoring depth of sedation.
