Abstract:Back pain is a worldwide health problem, adding a tremendous burden to modern societies. However, little information on back health is available in China, even though a quarter of the world's population is Chinese. To enhance knowledge in this area, we designed and initiated the Hangzhou Lumbar Spine Study, which is a cross-sectional study of a general sample of mainland Chinese with focusing on disc degeneration, Modic changes, endplate lesions, and back pain. The study consists of a structured questionnaire to measure back pain history and lifetime exposure to suspected risk factors, magnetic resonance imaging of the lumbar spine, bone mineral density study of the spine and hip, and DNA sample analysis. Here we briefly introduce the study methodology, report the test-retest reliability of the questionnaire, and describe the cohort profile to date. Since May 2014, 301 randomly selected subjects (male/female, 122/179; mean age, 51.0 years; range, 20-87 years) have been recruited. Tests-retests of the questionnaire, completed by 40 participants, revealed good reliability. To our knowledge, the Hangzhou Lumbar Spine Study is the first population-based epidemiological study conducted to characterize lumbar spinal phenotypes and back pain, their interaction, and their associations with lifetime environmental exposure, in mainland Chinese. Epidemiological information obtained from a reliable questionnaire, magnetic resonance (MR) imaging data, dual energy X-ray absorptiometry (DXA) measurements, and DNA analysis may serve as a valuable reference for future studies on back health, particularly for mainland Chinese.

Abstract:Background: The incidence of cervical, thoracic and lumbar spinal canal stenosis is low. It is difficult to identify the main focus and responsible segment, and it is also difficult to select the sequence of staging surgery. We report a patient with triple stenosis. Case Presentation: In this paper, we introduced a 61-year-old female patient with cervical, thoracic and lumbar spinal canal stenosis who had previously undergone “lumbar discectomy” in the outer hospital. The postoperative effect was not good and the symptoms were poor. The diagnosis was “cervical spinal stenosis and lumbar postoperative surgery”. The staged spinal canal decompression operation and Duhuo Jisheng Decoction (DHJSD) treatment were conducted in our hospital. After three months of follow-up, the functional and imaging results were satisfactory. Conclusions: The main focus and responsible spinal segment should be determined by the comprehensive analysis of medical history, signs, and images. Surgery combined with Chinese herbal medicine DHJSD therapy may be an effective treatment for this kind of disease.

Abstract:Background: Multiple minimally invasive spine approaches and techniques have been developed in recent years. While the disease processes affecting the spinal motion-segment (SMS) have remained largely the same, surgical treatment options have changed radically and not necessarily in an organized fashion. This is inevitable given the rapid evolution of the technology. The current image-based diagnostic techniques, also evolving, have helped us appreciate the disease Patho anatomy in minute detail. A comprehensive classification method accounting for all anatomical participants in the spinal motion-segment pathology, tailored to treatment options, is necessary. Out of the many valid options, a spine surgeon should be able to choose a single surgical approach that is most appropriate for the Patho anatomy of his/her patient’s disease. We feel that our classification system will help the spine surgeon make that important decision consistently, with minimal risk of leaving behind a significant lesion, or disrupting a structure which is not a participant in the disease process. Furthermore, universal acceptance of this classification system will make it easier for spine surgeons to communicate with each other and meaningfully compare the results of the various surgical approaches. Purpose of the Study: To develop a comprehensive, treatment-orientated classification of lumbar spinal motion-segment disease. Materials and Methods: Contributors to spinal motion-segment disease - intervertebral disc, facet joint, ligamentum flavum and mal-alignment were identified. The degrees of abnormalities in each of these entities were coded, and the codes were entered in a table from which the possible combinations of pathologic processes were generated. Study of 57 lumbar MRI images (217 spinal motion-segments) was carried out to determine the prevalence of various combinations of the motion-segment disease. Pre- and post-operative MRI-based spinal motion-segment classifications were performed to evaluate the clinical application of this classification system in 15 patients. Results: This classification presents 494 possible combinations of the spinal motion-segment disease. Many of the combinations are only theoretical possibilities without clinical significance. Normal motion-segments, D0 A0 L0 F0 , represented 33.3% of the total motion-segments; D1 A0 L0 F0 was 8.8%, representing bulging disc, normal alignment, ligamentum flavum and facet joint. D2 A0 L0 F2 was 6.9% representing intraannular disc herniation, normal alignment, mildly thickened ligamentum flavum, and hypertrophied superior articular process of the facet joint. 6.4% was D1 A0 L1 F3 representing bulging disc, mildly hypertrophic ligamentum and hypertrophied facet joint. Clinical application of the classification revealed: Accurate anatomic classification; immediate post-operative classification changes which correlate with patient’s symptoms; pre-operative, immediate post-operative and late post-operative classifications which correlate with patient’s symptoms and accurately demonstrate post-operative remodeling of the motion-segment, especially after disc surgery, and accurate; and anatomic documentation of pre- and post op classifications of interlaminar endoscopic decompression. Conclusion: A treatment-orientated, standardized classification of spinal motion-segment disease is necessary considering current multiple treatment options and availability of sophisticated pre-operative imaging techniques. Such a classification will allow standardization of treatment options for various combinations of the pathological processes. With the emergence of new technologies surgical options can be upgraded based on a standardized classification. This in turn will help minimize confusion for those who want to learn and facilitate growth in the minimally invasive technology. The preliminary results of clinical application of the classification showed it to be a very accurate patho-anatomic representation; immediate post-operative classification change reflected clinical improvement post-operatively; and precise representation of post-operative remodeling of the motion-segment one to two years post-operatively. The precision of this classification allows accurate communication regarding the pathology, between providers across the globe, and more accurate comparison of results of different surgical interventions.

Abstract:Novel classification based on sagittal plane and posterior view. Study Design Retrospective clinical study. Objective The purpose of this study was to establish a novel classification of migrated lumbar disk herniation (LDH) based on magnetic resonance imaging and provide appropriate treatment strategies for each type through algorithms. Summary of Background Data Full‐endoscopic lumbar discectomy is a surgical technique that has been developed rapidly in recent years. For migrated LDH, few surgeons currently classify it with multiplanar positioning, and there is no consensus on the choice of treatment strategy. Therefore, we established a new multiplanar classification criteria that can localize the lesions more accurately than previous studies. Methods A total of 263 eligible patients from March 2017 to March 2022 were included. Protrusions for each patient were located based on our classification and the surgical approach was selected according to our algorithms. The clinical symptoms of all patients before surgery, and at 1 day, 1 month, 3 months, 6 months, and 12 months after surgery were collected. Evaluations were performed using visual analogue scale (VAS), Oswestry Disability Index (ODI) and modified MacNab criteria. We used the chi‐squared test, one‐way analysis of variance (ANOVA), and t‐test to compare perioperative results and postoperative 3‐month, 6‐month, and 12‐month follow‐up results. We feel that our classification system will help the spine surgeon make that important decision consistently, with minimal risk of leaving behind a significant lesion or disrupting an otherwise normal structure of the spinal motion segment. Furthermore, we feel such a comprehensive classification will help surgeons and other caregivers to standardize treatment approaches to the various presentations of disc disease, and apply the evolving technology in an organized fashion. Purpose To develop a comprehensive, treatment-orientated classification of the lumbar disc disease. Materials and Methods The literature was reviewed for the classification of disc disease. The morphology of the disc disease, the topography of the disc lesion, and the symptom-complex produced by the disc lesion are identified and graded. The features so identified and graded are placed in a matrix. The combinations of the anatomical features and symptoms are then computed as shown in the matrix. The MRI database held in the office was studied to determine the most frequent combinations of the disc disease and symptom complex. Results A total of 494 combinations were identified, but most have no clinical relevance. The retrospective study of the clinical data and MRI studies of 93 patients (50 male and 43 female) revealed the most affected motion-segment was L5-S1 (male = 19.3%, and female = 23.8%). The most common patho-anatomy is a globally bulging disc (T3L1), representing 37.6% of the total. The second most common combination is a degenerated disc with central, intra-annular tear T4L1), representing 20.4% of the total. At 11.8%, globally bulging with severe axial pain and moderate radicular pain represented the most common patho-anatomic/clinical classification (T3L1B4R2). The most frequent top 10 patho-anatomic/clinical classifications represented 15.5% of the total. Conclusion In light of the multiple surgical options for excision of the herniated lumbar disc, including the conventional and minimally invasive, and the fact that the imaging technology allows spine surgeons to see in great detail, the disease status of each of the components of the spinal motion segment, it is imperative to develop comprehensive classification systems which take account of the unique characteristics of the disease entity and guide treatment strategies. The classification system presented here is fairly complex, but the software technology will be utilized for the classification system along with the most appropriate treatment approach.

Abstract:Lower lumbosacral vertebrae are the important part of spinal column,and are essential for spinal supporting,moving,protecting and weighting.Recently,lower lumbosacral vertebral fractures are becoming more and more common with the development of the industry and transportation.Lower lumbosacral vertebrae are surrounded by important ligaments,blood vessels and nervous tissue.The position and function of lower lumbosacral vertebrae are especially significant,therefore,the fractures in this area are extremely complicated,which can affect the normal physical function of spine.The optimal classification for lower lumbosacral vertebral fractures plays an important role in guiding clinical diagnosis and treatment.With the improvement of examination methods and the development of technology,further understanding concerning the diagnosis and treatment of lower lumbosacral vertebral fractures was achieved.The optimal classification for lower lumbosacral fractures remains controversial,which results in enormous trouble for clinical decision making.Based on understanding of thoracolumbar fractures,the thoracolumbar injury classification system and severity score (TLICS),widely accepted in clinic,owns good validation and reliability.However,because of the specificality of lower lumbosacral vertebral location and function,it is revealed that there is certain limitation in the process of clinical application of TLICS.The lumbosacral injury classification system and severity score (LSICS) based on TLICS is presently a novel and comprehensive classification system for evaluating lower lumbosacral injury severity and guiding clinical decision making,which is certainly unknown to us.Problems are still existed when LSICS newly proposed is applied to guiding and treating low lumbosacral vertebral fractures.Related problems and the progress of applying thoracolumbar vertebral fractures severity score to lower lumbosacral vertebral fractures are reviewed.The current problems of low lumbosacral vertebral fractures classification are summarized.Also,TLICS and LSICS are briefly introduced.The injury morphology of thoracolumbar vertebral fractures and lower lumbosacral vertebral fractures is described,and the grading rules are compared.The calculation methods of composite injury severity score are illustrated.The limitation of TLICS and the advantages of LSICS are analysed.The clinical application of LSICS in guiding clinical decision making of low lumbosacral vertebral fractures is discussed.
Diagnostic Value of the Nerve Root Sedimentation Sign for Lumbar Spinal Stenosis

Abstract:Objective To evaluate the diagnostic value of the nerve root sedimentation sign for lumbar spinal stenosis and improve morphological classification.Methods This is a retrospective radiologic study.From November 2012 to November 2013,511 patients who carried out MR examination due to backache together with intermittent claudication were selected.Cross-section MRI images at the narrowest layer of canalis spinalis were chosen.The patients were further divided into 7 groups,A1,A2,A3,A4,B,C,and D based on Schizas.The dural sac cross-sectional area (DSCA) of each group was measured.The groups without difference were merged.A new method to grade nerve root sedimentation was developed.Then,the correlation and consistency of nerve root sedimentation dichotomy,Schizas classification,new nerve root sedimentation classification and DSCA classification were compared.The differences of patients' Oswestry disability index (ODI) which adopted new classification method were compared.Results Results show there are no statistical significance between groups A1 and A2,groups A4 and B,or groups C and D in DSCA.After the merger,new nerve root sedimentation classification includes negative,a,b,and c.The correlation between nerve root sedimentation dichotomy and DSCA method is rs=0.579,and the consistency is κ=0.254.The correlation between new nerve root sedimentation classification method and DSCA method is rs=0.722,and the consistency is κ=0.381.The scores improve compared with the first two classification methods.ODI score difference of each group which adopted new nerve root sedimentation classification method has statistical signiftcance (F=17.972,P=0.000).Pairwise comparison shows only group c differs significantly from other groups (P＜0.05).Conclusion The new nerve root sedimentation classification method is more effective than nerve root sedimentation dichotomy and Schizas grading.It reflects the severity degree of patients'clinical symptoms to some extent.
A Lumbar Classification of Scoliosis in the Adult Patient: Preliminary Approach

Abstract:STUDY DESIGN: Retrospective consecutive clinical review of 98 patients.OBJECTIVE: To create a preliminary approach to a clinically important classification of scoliosis in adult patients.SUMMARY OF BACKGROUND DATA: There is currently no accepted classification of scoliosis in adults. High prevalence rates of scoliosis in the elderly and recent studies of health impact support the need for a clinically relevant classification.METHODS: A total of 98 adult patients with scoliosis with a 2-year minimum treatment/follow-up were included. Patients were classified into one of 3 types of deformity based on the degree of lordosis (L1-S1) and frontal plane endplate obliquity of L3 on standing radiographs: type I = lordosis > 55 degrees, L3 obliquity < 15 degrees; type II = lordosis 35 degrees-55 degrees, L3 obliquity 15 degrees-25 degrees; and type III = lordosis < 35 degrees, L3 obliquity > 25 degrees.RESULTS: Curve patterns included thoracic, thoracolumbar, lumbar, thoracic, and lumbar (mean Cobb angle 30 degrees, standard deviation 19 degrees). Cobb angle revealed no correlation to visual analog pain score (VAS) or general health (36-Item Short-Form Health Survey). Significant correlation between endplate obliquity L3, L1-S1 lordosis and VAS was noted (P < 0.05). Mean pain scores of classified patients were: type I, VAS = 27.7; type II, VAS = 43.3; and type III, VAS = 47.1 (type I vs. III, P < 0.05). Surgical rates (failed minimum 3-month conservative care, including bracing, physical therapy, and pharmacological treatment) by group were: type I, 0%; type II, 9%; and type III, 22.7% (P = 0.002).CONCLUSIONS: A simple classification of adult scoliosis was developed based on frontal and sagittal plane standing radiographs. With increasing type (from I to III), self-reported pain and disability increased. This result was reflected in the treatment approach as well, with surgical rates increasing from types I to III. Further refinement is important to develop an all inclusive and sufficiently descriptive system.

Classification of Coronal Trunk Hnbalance in Degenerative Lumbar Scoliosis and Its Influence on Osteotomy Strategy

Abstract:Objective To propose a classification system of degenerative lumbar scoliosis based on frontal trunk imbalance and to investigate the correction outcomes following the osteotomy strategy according to this system. Methods From October 2000 to October 2006, thirty-six patients with degenerative lumbar scoliosis were treated in our hospital. It included 13 males and 23 females with a mean age of 60.1 years (ranged 49-73 years). Based on the pre-operative posteroanterior standing X-ray films, all patients were classified according to frontal truncal balance (the distance between C7 plumb line to center sacral vertical line): Type A, the distance between C7 plumb line and central sacral vertical line is less than 3 era; Type B, C7 plumb line shifts more than 3 cm to the concave side of the lumbar curve; and Type C, C7 plumb line shifts more than 3 cm to the convex side. Results All patients received posterior 3-D instrumentation correction with osteotomy and were followed up for 28 months (ranged 12-60 months). According to the classification system, there were 10 cases with Type A, 20 cases with Type B, 6 cases with Type C. For patients with Type A and Type B, the osteotomies were performed from convex side and the post-operative average angle were 22° (correction rate of 58%). For patients with Type C curve, the osteotomies were performed from distal end area of concave side and the post-operative average angle were 26° (correction rate of 40%). The average distance between C7 plumb line and center vertical sacral line was 0.6 cm, 1.0 cm and i.2 cm, respectively. There was no significant loss of correction in all types and no death and infection occurred during the followups. Conclusion This classification system for degenerative lumbar scoliosis was established on the basis of frontal trunk imbalance. According to this system, options of the osteotomy level and the osteotomy approach could be decided with satisfactory outcomes.
Interobserver and Intraobserver Reproducibility and Reliability of the Huashan Clinical Classification System for Hirayama Disease

Abstract:Purpose: The Huashan clinical classification system for Hirayama disease has recently been proposed and has been found useful for diagnosis and treatment. So far, however, there has been little in-depth evaluation of its reliability. Thus, this study aimed to assess the reproducibility and reliability of the system. Methods: Patients diagnosed with Hirayama disease between 2019 and 2020 were recruited. Seven spine surgeons from four different institutions, including an experienced group of three and an inexperienced group of four, were trained as observers of the Huashan clinical classification system for Hirayama disease, and these surgeons classified the recruited patients using the system. Then, 2 months later, they repeated the classification on the same patients in a different order. The interobserver and intraobserver agreement between the results was analyzed using percentage agreement and weighted kappa (κ) statistics. Results: A total of 60 patients were included in the analysis. For all the observers, experienced observers, and inexperienced observers, the agreement percentages were, respectively, 78.5% (κ = 0.76), 80.0% (κ = 0.78), and 78.9% (κ = 0.77), indicating substantial interobserver reproducibility. For distinguishing typical (Types I and II) and atypical (Type III) Hirayama disease among the different groups of observers, the percentage agreement ranged from 95.6 to 98.9% (κ = 0.74–0.92), indicating substantial to nearly perfect reproducibility. For suggesting conservative treatment (Types I and III) or surgery (Type II), the percentage agreement ranged from 93.3 to 96.4% (κ = 0.81–0.90), indicating nearly perfect reproducibility. As for intraobserver agreement, the percentage agreement ranged from 68.3 to 81.7% (κ = 0.65–0.79), indicating substantial reliability. Conclusion: The Huashan clinical classification system for Hirayama disease was easy to learn and apply in a clinical environment, showing excellent reproducibility and reliability. Therefore, it would be promising to apply and promote this system for the precise and individualized future treatment of Hirayama disease.

[Reconsideration of lumbar spinal stenosis].

Abstract:People's understanding of lumbar spinal stenosis has become more and more comprehensive and reasonable, however, there are still many controversies about the concepts of "central lumbar canal" and "lateral lumbar spinal canal", and there is no unified standard at present. In this paper, we redefine and differentiate the two concepts. We believe that some kinds of central canal stenosis caused by bilateral recess stenosis can be completely solved by bilateral percutaneous endoscopic transforaminal discectomy. At the same time, the concept of "lumbar lateral recess" is ambiguous. We redefine it as "lateral lumbar spinal canal" and propose "West China Hospital classification" to guide surgical decision-making, which has been widely recognized and applied.

Abstract:Purposes To propose a new lumbar degenerative staging system using the current radiological classification system. Methods A cross-sectional analysis of retrospective databases between January 2018 and December 2022 was performed. 410 Patients for Modic changes, paravertebral muscle fat infiltration, disc degeneration, articular process degeneration, vertebral endplate degeneration and other structures and disc displacement, Spondylolisthesis, and Stenosis, and grouped patients according to stage were assessed. Visual analogue scale (VAS), Japanese Orthopaedic Association (JOA), and Oswestry Disability Index (ODI) scores were used to assess LBP strength, neurological function, and quality of life, respectively. Results The lumbar degeneration staging system consists of 8 variables, which can be divided into three steps: early, middle and late, and the correlation between each variable is strong (p<0.05). The later the staging, the worse the JOA, VAS and ODI scores. Conclusions Patients with later stages have worse clinical scores. This staging system recommends a uniform classification to assess lumbar degeneration.

A novel classification of congenital cervicothoracic scoliosis: identification of coronal subtypes and their prognostic significance

Abstract:Objective: To propose a novel classification system for stratifying coronal curve patterns in congenital cervicothoracic scoliosis with hemivertebrae (CTS-HV). Methods: Type A: regional cervicothoracic deformity only disturbing the balance of head-neck-shoulder complex; Type B: cervicothoracic deformity with significant trunk tilt to the convex side; Type C: cervicothoracic deformity with a significant compensatory thoracic curve. The reliability and reproducibility were assessed via the Kappa test. The differences among different subtypes in deformity parameters and bony structures were compared to identify the causative factors predisposing to different subtypes. Results: 98 patients were classified into Type A (47 cases), Type B (31 cases), and Type C (20 cases). The Kappa test showed excellent reliability (Kappa value = 0.847) and reproducibility (Kappa value = 0.881). The proportions of Klippel-Feil syndrome in Types B (71.0%) and C (85.0%) were significantly higher than in Type A (46.8%; all P < 0.05). Type A (66.0%) and Type B (71.0%) predominantly had their hemivertebra (HV) at T3 or T4, while Type C (75%) mostly had HV at T1 or T2. Type B exhibited the most severe trunk tilt, head shift, neck tilt, head tilt, and coronal balance distance (all P < 0.05). Type C had the lowest T1 tilt and first rib angle despite the greatest cervicothoracic Cobb angle (all P < 0.05). Conclusions: This novel reliable classification allows a better understanding of structural diversity and different coronal compensatory mechanisms for the natural progression of CTS-HV. It can contribute to determining the individualized treatment strategy and standardizing academic communication for this rare clinical entity.

Abstract:Lumbar intervertebral disc herniation is one of the most common causes of lumbocrural pain with its uncertain causes and varied clinical manifestations which marked by the specific symptoms and signs. Effective individualized therapy scheme depends on the correct and timely multi-level diagnosis of this disease. The paper systematically proposed the multilevel diagnosis, including qualitative diagnosis, level diagnosis, diagnosis by exclusion,combined diagnosis, classification diagnosis, syndrome differentitation in different stages, and disease severity grading diagnosis etc. The content and methods of the multi-level diagnosis on each individual case has great significance to formulate the individualized therapy scheme and to promote the clinical research of LIDH.

Abstract:STUDY DESIGN:Retrospective study.OBJECTIVE:Lumbar magnetic resonance imaging (MRI) findings are believed to be associated with low back pain (LBP). This study sought to develop a new predictive classification system for low back pain.METHOD:Normal subjects with repeated lumbar MRI scans were retrospectively enrolled. A new classification system, based on the radiological features on MRI, was developed using an unsupervised clustering method.RESULTS:One hundred and fifty-nine subjects were included. Three distinguishable clusters were identified with unsupervised clustering that were significantly correlated with LBP (P = .017). The incidence of LBP was highest in cluster 3 (57.14%), nearly twice the incidence in cluster 1 (30.11%). There were obvious differences in the sagittal parameters among the 3 clusters. Cluster 3 had the smallest intervertebral height. Based on follow-up findings, 27% of subjects changed clusters. More subjects changed from cluster 1 to clusters 2 or 3 (14.5%) than changed from cluster 2 or cluster 3 to cluster 1 (5%). Participation in sport was more frequent in subjects who changed from cluster 3 to cluster 1.CONCLUSION:Using an unsupervised clustering method, we developed a new classification system comprising 3 clusters, which were significantly correlated with LBP. The prediction of LBP is independent of age and better than that based on individual sagittal parameters derived from MRI. A change in cluster during follow-up may partially predict lumbar degeneration. This study provides a new system for the prediction of LBP that should be useful for its diagnosis and treatment.
Classification and Surgical Options of Adult De Novo Degenerative Lumbar Scoliosis

Abstract:[Objective]To explore a clinical classification system for the guideline of adult de novo degenerative lumbar scoliosis (ADDLS) . [Methods]Totally 168 cases were diagnosed as ADDLS and underwent surgery during 1994 and 2004. ADDLS were classified into three types based on vertebral rotation, olisthesis and coronal and/or sagittal balance on full -length standing spine plain anteroposterior radiographs; type Ⅰ, minimal or no rotation; type Ⅱ, rotatory olisthesis; type Ⅲ, rotatory olisthesis and coronal or sagittal imbalance ( C_7 PL >4 cm) . Three subtypes were further classified based on the patients clinical symptoms and signs (back pain and radiclular pain) ; A, back pain without radicular symptoms; B, radicular symptoms (from the lumbosacral hemicurve proved with SNRB) ± back pain; C, radicular symptoms (from the major curve proved with SNRB) ± back pain. [Results]There were Ⅰ A in 3 cases, Ⅰ B 28 cases,Ⅰ C 6 cases, ⅡA 21 cases,Ⅱ B 45 cases,ⅡC 39 cases, ⅢA 12 cases, ⅢB 6 cases,ⅢC 8 cases respectively. Type Ⅰ A were basically treated with PLIF OR TLIF; Type Ⅰ B and type Ⅰ C were treated with decompression; type Ⅱ A, ⅡB and ⅡC were treated with decompression and PLF; type Ⅲ A,ⅢB and ⅢC were treated with combined ALIF and PLF. [Conclusion]This classification is simple and practical, especially providing the guideline for surgical options of ADDLS.
