Abstract:Primary care in the UK has evolved significantly to enhance accessibility, continuity, and comprehensiveness of care, addressing the growing complexity of patient needs. Its characteristic GP-centred model has transitioned to a more integrated and multidisciplinary approach, including preventive care, chronic disease management, and mental health support. As the first port of call, the goal of any nation's primary care system is to be responsive to the prevailing health needs of its local communities. Globally, primary care is crucial to ensuring accessible, non-fragmented, and patient-centred care.1 Researchers in the field of primary care are therefore responsible for providing sufficient evidence to achieve this goal, through rigorous research underpinned with a contextual lens. Primary care researchers add to knowledge of current advances in medical research but move a step further to ensure its applicability to the communities and individuals for whom these advances have been designed.2 To achieve this, research teams must have adequately qualified academics, and more importantly, adequate representation of populations whose circumstances are being explored. Individuals who identify as Black (that is, descended from Black African, Black Caribbean, and other Black origin) represent approximately 4.0% of the total population of England and Wales, but London for instance, has around 13.3% of its population identifying as Black, indicating significant regional variations.3 However, despite the growing proportion of Black students entering undergraduate and postgraduate education, the representation of Black individuals in UK higher education remains disproportionately low. Abstract:The Higher Education attainment gap between BAME students and their White peers is well documented. The cause of this gap is multifactorial, and there is a need to understand contributing factors to support the design of meaningful interventions. This study aimed to probe student and staff understanding of what contributes to the attainment gap and to collect feedback on how to reduce it. Qualitative data were collected from 110 STEM students (95% BAME and 75% female) and 20 staff (70% BAME and 80% female) from Universities in the West Midlands and London via one-to-one interviews and focus groups. Questions were developed across themes of support, inclusivity, and development. Transcripts were subjected to inductive and deductive thematic analysis. Key findings included: the need for cultural awareness and representation within student support services and pastoral care provision and tailored support for students who were the first in their families to attend university. Students also felt the academic staff disproportionally represent their backgrounds, leading to a sense of not belonging amongst BAME students. BAME staff felt like tokens for diversity and reported having higher workloads than White staff, the social drinking culture felt isolating, participants felt that all staff should engage with cultural/religious training and diversity of academic staff should be improved through inclusive recruitment practices and mentoring. The results highlight the need for access to academic and pastoral support that is culturally sensitive to all backgrounds.

Abstract:The participation of Black educators in the UK's education system has been a source of much debate in recent years. Research indicates having a teaching force that better represents society is critical because of the character, ubiquity, pervasiveness, duration and importance of teaching as a social activity. However, to date, many of the existing studies have taken place in primary, secondary and higher education contexts. The primary purpose of this paper is to draw upon concepts of identity to make Black educator identity visible in the Early Childhood Education and Care (ECEC) context. Secondly, this paper aims to contribute to recent developments around mobilising Black studies as an academic discipline by seeking to explore how Black ECEC educators construct their identity through their professional practice. This paper draws on Critical Race Theory and narrative analysis methods to illustrate the experiences of Black ECEC educators. While this paper does not generalise to the experience of all Black educators, it does highlight a much under-researched area and advocates the need for counter-narratives to challenge normative unracialised experiences.

Abstract:Background: Research funding disparities contribute to clinical academic workforce inequalities. Hence, our study explores the association between student demographics and research grant application rates and outcomes among UK medical students. Methods: This is a national multicentre cross-sectional survey of UK medical students in the 2020-21 academic year. Multiple zero-inflated negative binomial regression and generalized linear model (binomial distribution; logit link) were utilized to investigate the association between student demographics, number of grant applications submitted, and successful grant applications (yes or no). P-values less than a Bonferroni-corrected significance level of 0.05/36 = 0.0014 were considered to be statistically significant. Results: A total of 1528 students participated from 36 medical schools. One hundred fifty-one respondents (9.9%) had applied for research grants. Black students submitted applications 2.90 times more often than white students [Incident rate ratio (IRR): 2.90, 95% confidence interval (CI): 1.37-6.16], with no ethnic disparity in the odds of successful applications. Gender did not influence application rates significantly (P = .248), but women were 4.61 times more likely to secure a grant than men [odds ratio: 4.61, 95% CI: 2.04-10.4]. Being a PubMed-indexed author was associated with increased grant application submission rates [IRR: 3.61, 95% CI: 2.20- 5.92] while conducting more research was associated with greater odds of securing a grant [odds ratio: 1.42, 95% CI: 1.17- 1.73]. Conclusion: Although black students submitted more applications, ethnicity did not influence success rates. Gender did not influence application rates, but women were more successful. These findings underscore the need for strategies supporting women and underrepresented students for continued academic achievement after graduation. Key messages: What is already known on this topic Research funding for post-PhD researchers is believed to be a major driver of gender and ethnic inequalities in the clinical academic workforce.Students who receive research grants are more likely to receive postgraduate research grants.What this study adds Black students applied for more research grants than white students, but there were no ethnic differences in the odds of securing a grant.There were no gender differences in the research grant application rates. However, female students had greater odds of securing research grants compared to male students.How this study might affect research, practice or policy Medical schools should incorporate grant writing skills into the undergraduate research curriculum. Also, to sustain women's academic success post medical school, the NIHR and affiliates should provide research award extensions and childcare support for women when required.

Abstract:The number of GPs working in England has crept up in the past year, a rise almost entirely accounted for by an increase in GP trainees. In March 2019 England’s GPs totalled 44 847, a rise of 350 (0.8%) on the same month in 2018. The full time equivalent count, which allows for some GPs working only part time, showed a similar increase, rising from 34 424 to 34 736 (0.9%).1 But closer examination of the data released by NHS Digital shows that the headcount of fully qualified GPs fell by 494 year on year, while the number of trainees increased by 760. Organisational perspectives on addressing differential attainment in postgraduate medical education: a qualitative study in the UK

Abstract:Objectives To explore how representatives from organisations with responsibility for doctors in training perceive risks to the educational progression of UK medical graduates from black and minority ethnic groups (BME UKGs), and graduates of non-UK medical schools (international medical graduates (IMGs)). To identify the barriers to and facilitators of change. Design Qualitative semistructured individual and group interview study. Setting Postgraduate medical education in the UK. Participants Individuals with roles in examinations and/or curriculum design from UK medical Royal Colleges. Employees of NHS Employers. Results Representatives from 11 medical Royal Colleges (n=29) and NHS Employers (n=2) took part (55% medically qualified, 61% male, 71% white British/Irish, 23% Asian/Asian British, 6% missing ethnicity). Risks were perceived as significant, although more so for IMGs than for BME UKGs. Participants based significance ratings on evidence obtained largely through personal experience. A lack of evidence led to downgrading of significance. Participants were pessimistic about effecting change, two main barriers being sensitivities around race and the isolation of interventions. Participants felt that organisations should acknowledge problems, but felt concerned about being transparent without a solution; and talking about race with trainees was felt to be difficult. Participants mentioned 63 schemes aiming to address differential attainment, but these were typically local or specialty-specific, were not aimed at BME UKGs and were largely unevaluated. Participants felt that national change was needed, but only felt empowered to effect change locally or within their specialty. Conclusions Representatives from organisations responsible for training doctors perceived the risks faced by BME UKGs and IMGs as significant but difficult to change. Strategies to help organisations address these risks include: increased openness to discussing race (including ethnic differences in attainment among UKGs); better sharing of information and resources nationally to empower organisations to effect change locally and within specialties; and evaluation of evidence-based interventions.

Race, representativity, and the (im)probability of being a Black African professional psychology graduate: an institutional case study

Abstract:South African Journal of Psychology, Ahead of Print. The process of transforming South African psychology requires several coordinated initiatives. One initiative likely to unlock the transformation process in exponential ways is through attaining race-based representativity in the South African psychology workforce. Using graduation data, this article reports on the pace of racial transformation and representativity among professional psychology graduates from the University of KwaZulu-Natal. Since its inception in 2004, the University of KwaZulu-Natal has made concerted efforts to transform the racial, gender, and socio-economic diversity of its student and staff body. The institution has produced at least 469 professional psychology master's degree graduates in clinical, counselling, educational, industrial, and research psychology during this time. However, only 43.9% of these professional psychology graduates have been Black African, while the average year-on-year increase in Black African graduates was only 9.7% between 2005 and 2020. A forecasting model predicts that the University of KwaZulu-Natal is only likely to achieve national race-based representativity among its professional psychology graduates in the 2026 graduation cohort, and provincial representativity in the 2028 cohort. This article discusses why race-based representativity remains foundational in transforming professional psychology, and how and why the pace of racial transformation among professional psychology graduates at the University of KwaZulu-Natal has been relatively slow, despite transformational efforts and successes at an institutional level.

Abstract:Evidence acknowledges inequalities to progression and achievement for black, Asian and minority ethnic students within higher education, as well as barriers for promotion of black, Asian and minority ethnic staff within the NHS. In the UK, legislation and regulatory guidance requires students studying undergraduate midwifery to undertake their programme across both these institutions. To understand the experiences of black, Asian and minority ethnic students studying undergraduate midwifery at a UK university. This qualitative study used semi-structured interviews with five student midwives who identified as black, Asian or minority ethnic from a university, conducted by a peer researcher. Thematic analysis was used to analyse anonymised transcript data. Three key themes were identified: 'invisibility', 'emerging visibility' and 'managing visibility'. Participants experienced a monocultural focus in the curriculum and in practice and were exposed to racist behaviours, causing them to modify behaviours. A need for diverse teaching materials and cultural inclusivity across institutions was identified to help combat outdated systemic Eurocentric practices and support the implementation of recently published midwifery standards.

Abstract:Black and minority ethnic (BME) students are less likely to achieve a first or upper-class second degree than White students. This evaluation investigated the experiences of BME Health and Social Care students at a London university of the hidden curriculum, thematically analysed through a critical race lens. Four overarching themes were identified, (1) super-visible ethnicity, (2) the negative depiction of BME lived experiences, (3) self-depreciation and self-confidence and (4) separation. Recommendations at an institution-level are made in the areas of staff diversity, language support, student registration experiences and canteen costs. Recommendations at a course-level are made in the areas of co-creation of the curriculum, student belonging, academic and literacy skill development. The recommendations are underpinned by a critical pedagogy, including culturally sensitive teaching strategies and the development of meaningful staff-student partnerships. The recommendations made may be applicable to other Higher Education Institutions with a diverse student cohort where the attainment gap is prevalent.
‘Race’ and academic performance in international higher education: Black Africans in the UK

Abstract:Much UK higher education research routinely pinpoints the racialized nature of academic performance, but it often fails to even consider if racism contributes to such a pervasive racial or ethnic disparity. While much research in the area often focuses on comparing the attainment of home white and ethnic minority students, little attention is given to the experiences of black African international students (BAIS), particularly in UK higher education. Using semi-structured qualitative interviews, this study documents how ‘race’ shapes academic performance and achievement by exploring the experiences of 21 BAIS undergraduates studying in ten different universities in England. Factors identified, inter alia, include racism and discrimination, and the analysis challenges the narrative of assessment as neutral and objective technology that rewards merit, and lifts the voices of BAIS which are normally silent in the literature about international student experience.

Abstract:Faculty hiring and retention determine the composition of the US academic workforce and directly shape educational outcomes 1 , careers 2 , the development and spread of ideas 3 and research priorities 4,5 . However, hiring and retention are dynamic, reflecting societal and academic priorities, generational turnover and efforts to diversify the professoriate along gender 6,7,8 , racial 9 and socioeconomic 10 lines. A comprehensive study of the structure and dynamics of the US professoriate would elucidate the effects of these efforts and the processes that shape scholarship more broadly. Here we analyse the academic employment and doctoral education of tenure-track faculty at all PhD-granting US universities over the decade 2011–2020, quantifying stark inequalities in faculty production, prestige, retention and gender. Our analyses show universal inequalities in which a small minority of universities supply a large majority of faculty across fields, exacerbated by patterns of attrition and reflecting steep hierarchies of prestige. We identify markedly higher attrition rates among faculty trained outside the United States or employed by their doctoral university. Our results indicate that gains in women's representation over this decade result from demographic turnover and earlier changes made to hiring, and are unlikely to lead to long-term gender parity in most fields. These analyses quantify the dynamics of US faculty hiring and retention, and will support efforts to improve the organization, composition and scholarship of the US academic workforce.

Abstract:Aim. To explore how Black African international students adjust to their new university environment and gain insight into how they feel supported to engage in physical activity (PA). Background. People from ethnic minority backgrounds have poor physical activity levels compared with white populations in the UK. The Black population is known to have the lowest PA levels among the ethnic minority groups in the UK. This trend is suggested to be the case among university students, but no research has examined this. Ethnic minority students, including international students, encounter adaptation difficulties such as cultural barriers and social isolation which affect their university experiences and health behaviours such as physical activity. Methodology. This was a qualitative method, with in‐depth semistructured interviews conducted with five African students (two males and three females) aged between 21 and 40 years from a university in the West Midlands. The transcribed interviews were analysed using Braun and Clarke's thematic analysis. Results/Findings. Culture and lifestyle of physical activity; knowledge, experience, and self‐motivation; the effect of various social groups, activities, and services at the university; and weather variations and physical settings were the identified themes. The key finding of the study is that respondents engaged in PA to a greater extent at the British university than they did in their home countries. This was due to a combination of factors, including low‐cost gym memberships and events and a general lack of racial or gender‐based discrimination. Conclusions/Recommendations. Participants expressed satisfaction with the cultural and social support systems at their university, such as sports clubs and societies, and mental health support services that encouraged them to learn about and engage more in physical activity. Although some barriers, such as weather variations, were mentioned, various indoor options coupled with cheaper gymnasiums motivated students to remain or become active. Understanding the experiences of minority student groups in physical activity could help in reviewing current provisions and extending them to a wider population.

Abstract:The academic disciplines of Science, Technology, Engineering and Mathematics (STEM) have long suffered from a lack of diversity. While in recent years there has been some progress in addressing the underrepresentation of women in STEM subjects, other characteristics that have the potential to impact on equality of opportunity have received less attention. In this study, we surveyed 188 early career scientists (ECRs), defined as within 10 years of completing their PhD, in the fields of ecology, evolutionary biology, behaviour, and related disciplines. We examined associations between ethnicity, age, sexual orientation, sex, socioeconomic background, and disability, with measures of career progression, namely publication record, number of applications made before obtaining a postdoc, type of contract, and number of grant applications made. We also queried respondents on perceived barriers to progression and potential ways of overcoming them. Our key finding was that socioeconomic background and ethnicity were associated with measures of career progression. While there was no difference in the number of reported first‐authored papers on PhD completion, ethnic minority respondents reported fewer other‐authored papers. In addition, ECRs from a lower socioeconomic background were more likely to report being in teaching and research positions, rather than research‐only positions, the latter being perceived as more prestigious by some institutions. We discuss our findings in the context of possible inequality of opportunity. We hope that this study will stimulate wider discussion and help to inform strategies to address the underrepresentation of minority groups in the fields of ecology and evolution, and STEM subjects more widely.

Abstract:Background There is a knowledge gap about the experiences that affect the mental health of Black university students in the UK. Current research is focused on understanding the continuation, attainment and progression gap between Black students and non-Black students. It is essential to know more about the interactions between personal and institutional factors on the mental health of Black students to explain the inequalities in their experiences and outcomes across the university lifecycle. The current study set out to thematically synthesise articles that explore the experiences that affect the mental health and mental well-being of Black university students in the UK. Methods This study is a qualitative thematic synthesis of a literature review. We developed search strategies for four online databases (PubMed, Social Science Premium Collection via ProQuest, Open Access Theses and Dissertations, and Open Grey) covering January 2010 to July 2020. This search was combined with a manual search of reference lists and related citations. All articles in English addressing mental health and mental well-being experiences among Black university students studying at a UK university were included. Critical Appraisal Skills Programme Checklist was used to assess bias. A thematic synthesis was conducted using Braun and Clarke (2006)’s six-step guide to develop descriptive themes and analytical constructs. Results Twelve articles were included. Several themes were identified as affecting the mental health of Black university students in the UK: academic pressure, learning environment, Black gendered experience, isolation and alienation, culture shock, racism and support. Discussion This review provides an appraisal of the factors affecting the mental health and mental well-being of Black students at UK universities, which need to be addressed by higher education policy-makers and key decision-makers. Further research is needed about the mental health experiences of Black university students in relation to Black identities, suicidality, mental health language, the physical environment, and racism and other institutional factors.

Predictors and Socio-Demographic Disparities in STEM Degree Outcomes: A UK Longitudinal Study using Hierarchical Logistic Regression

Abstract:Socio-demographic disparities in STEM degree outcomes impact the diversity of the UK's future workforce, particularly in fields essential for innovation and growth. Despite the importance of institution-level, longitudinal analyses in understanding degree awarding gaps, detailed multivariate and hierarchical analyses remain limited within the UK context. This study addresses this gap by using a multivariate binary logistic model with random intercepts for STEM subjects to analyse predictors of first-class degree outcomes using a nine-year dataset (2014 to 2022) from a research-intensive Russell Group university. We find that prior academic attainment, ethnicity, gender, socioeconomic status, disability, age, and course duration are significant predictors of achieving a first-class degree, with Average Marginal Effects calculated to provide insight into probability differences across these groups. Key findings reveal that Black students face a significantly lower likelihood of achieving first-class degrees compared to White students, with an average 16 percent lower probability, while students graduating from 4-year degree programmes have an average 24 percent higher probability of achieving a first-class degree relative to those on 3-year programmes. Although male students received a higher proportion of first-class degrees overall, our multivariate hierarchical model shows higher odds for female students, underscoring the importance of model choice when quantifying awarding gaps. Baseline odds for first-class outcomes rose considerably from 2016, peaking in 2021, indicating possible grade inflation during the COVID-19 pandemic. Interaction effects between socio-demographic variables and graduation year indicate stability in ethnicity, disability, and socioeconomic awarding gaps but reveal a declining advantage for female students over time.

Abstract:Background: Equity redress in the higher education and health sectors is a global discourse that seeks to address the inequalities caused by past discrimination practices. The apartheid regime in South Africa fragmented both the higher education and the health sectors, creating White and male dominated systems. Consequently, Black Africans and females were under-represented in these sectors. Furthermore, the provision of higher education including medical training was unequal between the different populations. As democracy was established in South Africa in 1994, it is necessary to assess whether transformation in population affinity and sex of postgraduate students in the higher education and health sector has occurred, as these individuals are crucial for providing the future academic workforce and also healthcare to the public. Methods: The demographic profile of postgraduate students graduating in a health sciences facility in South Africa over the period 2008-2017 was retrospectively assessed. Survival analysis models were used to investigate the time taken to graduate. Log-rank tests were used to compare the completion rates. Results: More females (53.3%) than males (41.9%) completed their postgraduate degree over the period 2008-2017 (p˂0.0001). In relation to population affinity, more White students (56.4%) than Black African students (40.8%) completed their degrees overall (p˂0.0001). Conclusion: While transformation occurred in the sex of graduating students over the ten year period, the same change has not occurred with regards to population affinity. The under-representation of Black African graduates is a major setback for efforts to diversify the South African higher education and health sectors. Transformation of the demographic profile of postgraduate students at South African institutions is vital for developing individuals who will contribute to equitable redress of academic staff in the higher education sector and also of the healthcare workforce. Diversified health personnel including highly skilled clinician scientists will aid in improving the provision of health care to communities particularly the underpriviledged rural areas, and also assist in training the next generation of healthcare staff. The challenges identified in this study may assist other countries where adequate transformation of the education and health sectors has not occurred.
Trends Among Women in Academic Medicine Faculty Ranks

Abstract:Introduction: Similar proportions of women and men have entered medical school since 2003. However, career advancement and promotion for women continues to be fraught with disparities and inequalities. Building on current literature, this study explores the rates of change of female faculty in faculty ranks over the last 10 years to gain a more comprehensive view of the faculty trends of women in academic medicine. Methods: Using the Faculty Administrative Management Online User System database, counts by gender and faculty rank at each Association of American Medical Colleges (AAMC) academic medical school were obtained. Statistical analysis was done using generalized estimating equations modeling to assess rates of change for each gender from 2012 to 2021. Results: Higher proportions of female faculty are concentrated at the Instructor and Assistant Professor level and lower proportions at the Associate Professor and Professor rank compared to male faculty. Over the study period, female faculty showed increased rate change compared to male faculty of 1.007 (95% confidence interval [CI]: 1.002–1.012) for Associate Professor rank and 1.012 (95% CI: 1.007–1.016) for Professor rank. At the Instructor and Assistant Professor levels, female faculty decreased at a relative rate of 0.980 (95% CI: 0.969–0.990) and 0.995 (95% CI: 0.992–0.997) each year, respectively. Conclusion: Female faculty continue to be concentrated at the junior faculty rank. Rate changes at the senior faculty rank for female faculty have slightly improved over the last 10 years compared to male faculty. However, this improvement is minimal, and work is still needed to achieve true gender equity in academic medicine.
