Abstract:Human health and safety are seriously threatened by tuberculosis (TB), a chronic infectious illness that is caused by human infection with Mycobacterium tuberculosis and can be classified as either pulmonary or extrapulmonary. [ 1 ] TB is susceptible to the population, has a long duration of illness, is complex, and its pathogenesis is associated with immune regulation and a variety of cytokines. The most common extrapulmonary tuberculosis among them is called TB pleurisy (TP), and its primary clinical sign is called TB pleural effusion (TPE). [ 2 ] Most patients with benign pleural effusion (PE), after receiving timely diagnosis and treatment, can see a significant reduction or even complete subsidence of pleural fluid (PF). However, there exists a portion of patients with PE whose condition is difficult to obtain a clear diagnosis through conventional testing methods, and this diagnostic uncertainty may lead to a delay in treatment, which may seriously affect the prognosis of the patient, or worse, may lead to the death of the patient as a result. [ 3 ] Traditional diagnostic methods are not only invasive but also expensive, and these problems need to be addressed. Therefore, new laboratory examination techniques and testing indexes are particularly important in order to help make an effective diagnosis of TPE in a wider and more timely manner and to detect, diagnose, and treat the disease as early as possible. [ 4 ] In recent years, the development and application of immunological examination methods have provided new perspectives and tools in the clinic, and it has been confirmed that TB is closely associated with Th cell immunity in the human immune system. [ 5 ] The body's immunological stability is largely dependent on the balance of Th1 and Th2, the 2 primary functional cell types. Th1 cells produce interferon-gamma (IFN-γ) after activation, which promotes the immune response, has antiviral and macrophage activation functions, and is of discriminatory value in the diagnosis of TPE. [ 6 ] Interleukin-33 (IL-33) is critical in Th1-mediated immune response. Interleukin-35 (IL-35) is involved in T-cell immunity and can be used as a new biomarker for adjunctive diagnosis of TPE. [ 7 , 8 ] There is no study to investigate the effect of combined assay (CA) of IL-35, IL-33, and IFN-γ on the diagnosis of TPE, so it is particularly important to comprehensively analyze their concentrations. Thus, the concentrations of IL-35, IL-33, and IFN-γ are important for the diagnosis of TPE, but no study exists that analyzes the effect of CA of the 3. Therefore, the study discusses the diagnostic effect of TPE by the concentration content of the 3 CAs.
