Abstract:ObjectivesExtracorporeal membrane oxygenation (ECMO) has become an important life support technique during lung transplantation. We aimed to develop a rat model for lung transplantation using venoarterial (VA) ECMO support.MethodsAdult male Sprague-Dawley rats weighing 400 to 450 g were used in this study. ECMO circuits were created by obtaining venous access from the femoral vein with subsequent extracorporeal oxygen exchange, which was then returned to the circulatory system through the left carotid artery (ie, VA-ECMO). Simultaneously, the donor lungs were retrieved and immersed in cold, low-potassium dextran lung preservation solution. Orthotopic left lung transplantation supported by VA-ECMO was performed. Thereafter, a respiratory failure rat model was constructed using ventilation with a hypoxic and hypercapnic gas mixture, consisting of 6% oxygen, 8% carbon dioxide, and 86% nitrogen, before lung transplantation. Similarly, left lung transplantation supported by VA-ECMO was performed in rats with respiratory failure. Arterial blood gas levels were measured at designated time points throughout the experiment.ResultsWe found that VA-ECMO provided sufficient oxygenation and carbon dioxide removal to allow for smooth left lung transplantation in healthy rats and those with respiratory failure.ConclusionsWe established a rat model for lung transplantation using VA-ECMO. Left lung transplantation using VA-ECMO support is also feasible and safe in rat models of respiratory failure. These models provide efficient and economical models for translational medicine for lung transplantation using ECMO. Moreover, it will be invaluable to evaluate the physiological and pathophysiological roles of ECMO during lung transplantation.
Experimental Study of Ex Vivo Liposome-mediated Gene Transfer in Orthotopic Rat Left Lung Transplantation

Abstract:Objective To determine the feasibility of ex vivo liposome-mediated gene transfer ( Lac-Z gene ) to lung isografts and to get expression. Methods An orthotopic rat left lung transplant model was developed with the use of a modification of the “cuff” technique. 18 SD rats were divided into three groups randomly. Gene transfer group ( group 1 ): PSV-β-gal/ liposome ( Lipofectin R○ ) complex was transfered into the donor left lung via the left pulmonary vein route at the time of lung harvesting. Liposome group ( group 2 ) and Control group ( group 3 ) : Grafts underwent the same procedure but received liposome and 0 9% saline solution respectively. Donor lungs were harvested one day after transplantation. The transgene expression of Lac-Z gene was detected by histochemical staining. Results Transgene expression of β-gal in group 1 was detected in bronchial epithelial cells and alveolar epithelial cells. But no expression of β-gal was found in groups 2 and 3. Conclusion Ex vivo plasmid vector/liposome complex-mediated gene transfer to lung isografts via the left pulmonary vein route is feasible. This study provides the scientific proof to the feasibility of functional gene therapy of complications such as ischemia-reperfusion injury, rejection and infection after rat lung transplantation.
Establishment and Modification of Isolated Lung Transplantation and Reperfusion Model in Rats

Abstract:OBJECTIVE To establish and modify of the isolated lung transplantation and reperfusion model in rats with a relatively less-invasive way.METHODS Angiocatheters were inserted into femoral artery and jugular vein of the recipient rat to establish the controlled cross-circulatory.The venous cannula was connected to the pulmonary artery of the ex vivo lung block after preservation and cold storage to provide inflow,and Pulmonary effluent blood from the lung block was collected and returned to the support rat via the femoral artery for detoxification and deoxygenation.Each support rat was used to perfuse one lung block for at least one hour.The inflow sample to each lung block was analyzed for pH,pO2,pCO2,and Hb to follow alterations in support rat physiology.RESULTS There were no statistical differences in pH,PvO2,PvCO2 or Hb(P>0.05).CONCLUSION The support rat can deliver constant systemic venous blood at stable physiologic parameters to the ex vivo lung block and this animal model can be used in research of lung transplantation and ischemic-reperfusion injury.
Short-term Ex Vivo Tissue Culture Models Help Study Human Lung Infectionsa Review

Abstract:Most studies on human lung infection have been performed using animal models, formalin or other fixed tissues, and in vitro cultures of established cell lines. However, the experimental data and results obtained from these studies may not completely represent the complicated molecular events that take place in intact human lung tissue in vivo. The newly developed ex vivo short-term tissue culture model can mimic the in vivo microenvironment of humans and allow investigations of different cell types that closely interact with each other in intact human lung tissues. Therefore, this kind of model may be a promising tool for future studies of different human lung infections, owing to its special advantages in providing more realistic events that occur in vivo. In this review, we have summarized the preliminary applications of this novel short-term ex vivo tissue culture model, with a particular emphasis on its applications in some common human lung infections.
Experimental Models of Ischemic Lung Damage for the Study of Therapeutic Reconditioning During Ex Vivo Lung Perfusion

Abstract:Background: Ex vivo lung perfusion (EVLP) may allow therapeutic reconditioning of damaged lung grafts before transplantation. This study aimed to develop relevant rat models of lung damage to study EVLP therapeutic reconditioning for possible translational applications. Methods: Lungs from 31 rats were exposed to cold ischemia (CI) or warm ischemia (WI), inflated at various oxygen fractions (FiO2), followed by 3 h EVLP. Five groups were studied as follow: (1) C21 (control): 3 h CI (FiO2 0.21); (2) C50: 3 h CI (FiO2 0.5); (3) W21: 1 h WI, followed by 2 h CI (FiO2 0.21); (4) W50: 1 h WI, followed by 2 h CI (FiO2 0.5); and (5) W2h: 2 h WI, followed by 1 h CI (FiO2 0.21). Following 3 h EVLP, we measured static pulmonary compliance (SPC), pulmonary vascular resistance, lung weight gain (edema), oxygenation capacity (differential partial pressure of oxygen), and protein carbonyls in lung tissue (oxidative stress), as well as lactate dehydrogenase (LDH, lung injury), nitrotyrosine (nitro-oxidative stress), interleukin-6 (IL-6, inflammation), and proteins (permeability edema) in bronchoalveolar lavage (BAL). Perivascular edema was quantified by histology. Results: No significant alterations were noted in C21 and C50 groups. W21 and W50 groups had reduced SPC and disclosed increased weight gain, BAL proteins, nitrotyrosine, and LDH. These changes were more severe in the W50 group, which also displayed greater oxidative stress. In contrast, both W21 and W50 showed comparable perivascular edema and BAL IL-6. In comparison with the other WI groups, W2h showed major weight gain, perivascular edema, SPC reduction, drop of differential partial pressure of oxygen, and massive increases of BAL LDH and proteins but comparable increase of IL-6 and biomarkers of oxidative stress. Conclusions: These models of lung damage of increasing severity might be helpful to evaluate new strategies for EVLP therapeutic reconditioning. A model combining 1 h WI and inflation at FiO2 of 0.5 seems best suited for this purpose by reproducing major alterations of clinical lung ischemia-reperfusion injury.

Abstract:Perfusion, Ahead of Print. Background: Extracorporeal life support echniques as an Adjunct to Advanced Cardiac Life Support is usually suitable for complex heart surgery such as cardiopulmonary bypass (CPB). Cerebral perfusion is a clinically feasible neuroprotective strategy; however, the lack of a reliable small animal model.Methods: Based on the rat model of ECLS we evaluate the effects of ECLS-CP using HE staining, Nissl staining, TUNEL staining and ELISA.Result: We found that ECLS combined with the cerebral perfusion model did not cause brain injury and immune inflammation. There was no difference between the two by a left carotid artery or right carotid artery CP.Conclusion: These experimental results can provide the experimental basis for selecting blood vessels for ECLS patients and clinical CP to offers a trustworthy animal model for future exploration of applying brain perfusion strategies during ECLS-CP.

Abstract:OBJECTIVE To pursue a simple method to establish isolated lung ischemia reperfusion injury model on piglets. METHODS Twelve piglets of 4-5 weeks old were randomly divided into two groups, i.e., ischemia reperfusion group and continuous perfusion group. After anesthesia and tracheal intubation, the heart was exposed by median sternotomy. Pulmonary, left atrium and right atrium cannulation was made to establish the isolated lung reperfusion model. RESULTS The decrement of lung compliance and the increment of wet to dry lung weight (W/D) ratio occurred more significantly in ischemia reperfusion group. More severe lung injury in ischemia reperfusion group was detected via light and electric microscope examination. CONCLUSION Isolated lung ischemic reperfusion model could be uneventfully established in piglets. The advantages of this model is that it is much closer to clinical settings. The establishment of this animal model facilitates the research on mechanism and prevention of ischemia reperfusion lung injury during open heart surgery.
Establishment of porcine isolated lung ischemia perfusion model and continuous perfusion model using roller pump

Abstract:AIM:To observe if the ischemia-reperfusion (IR) injury to the lung is more serious than the continuous perfusion injury to the lung by establishing a new isolated lung perfusion model in piglets using a roller pump. METHODS: Eighteen piglets were randomly divided into control group, IR group and nonpulsatile continuous perfusion (NCP) group, with 6 in each group. Three catheters were respectively inserted into the right atrium,left atrium and main pulmonary artery. The whole blood was drained into the resevior through the right atrium catheter before the lung perfusion. The perfusion solution in reservior was pumped into the main pulmonary artery by a roller pump, and the solution was drained back to the reservoir from the left atrium. In control group, the lung was perfused by the heart at a physiological state for 120 min. In IR group, the lung perfusion started after 90 min ischemia and the perfusion lasted 30 min at the flow of 80 mL/kg·min. In NCP group, the lungs were continuously perfused by the roller pump at the same flow for 120 min. The lung compliance, the contents of TNF-α and IL-6 in the perfusate were measured at the time points of 0 min and 120 min. The lung wet to dry weight ratio (W/D) was determined after perfusion. Histological and ultrastructural changes of the lung were also examined after experiment. RESULTS: In IR group, compared to the parameters at 0 min, the lung compliance was lower (P<0.05), and the contents of TNF-α and IL-6 were higher (P<0.05). At 120 min, the lung compliance and the W/D in NCP group were significantly better than those in IR group (P<0.05). The contents of TNF-α (P<0.05) in IR group were significantly higher than that in NCP group. It also showed milder lung injury in NCP group through light and electron microscope examination. CONCLUSION: The results bears out that the continuous perfusion injury is milder than the ischemia-perfusion injury to the lung, and the convenient operation indicates this model's reliability and feasibility.

Abstract:Objective:To build up and refine rat allograft orthotopic left lung transplantation model. Methods:One hundred male Sprague-Dawley rats,weight ranging from 200 to 250 grams,were selected and randomized into 50 pairs for donors and recipients. After general anesthesia with intubation,donor left lungs were harvested individually after perfusion with LPD solution through left main pulmonary artery,then were immersed into prepared fresh LPD solution. Muscle-sparing incision was adopted in recipient lung operations. Blunt dissection was carried out during dissect hilum of left lung. Cuff techniques were introduced in anastomosis of bronchus,pulmonary artery,and pulmonary vein. Results:All cuffs of the donor lungs were accomplished successfully. In all the 50 transplantations,we yielded a 90 percent success rate. Time for harvesting and cuff techniques was (25±2) min,and anastomosis time was (30±4) min. The reasons of 5 failed cases were as follows:disrupture of pulmonary vein in 3,disrupture of pulmonary artery in 1,and fatal bleeding in 1. Conclusion:Rat lung transplantation model was successfully built up and refined on some techniques. The animal model could be duplicated easily and the success rate was high.

Abstract:BACKGROUND:To establish a stable ex vivo lung perfusion (EVLP) model and determine the preservation effect of the EVLP technique on donor lungs in vitro.METHODS:EVLP was performed on nonacceptable human donor and porcine lungs, and during perfusion, the oxygenation index was assessed and blood gas analysis was performed.RESULTS:After 4 h of EVLP of nonacceptable human donor and porcine lungs, lung function parameters remained stable, and lung energy metabolism was improved to a certain extent.CONCLUSIONS:EVLP can suitably maintain donor lungs in vitro for transplantation and is an effective method for ex vivo preservation of donor lungs.

Abstract:The rat model of cardiopulmonary bypass (CPB) is becoming more and more widely used because of its easy operation and low costs. With the aid of a mini ventilator, the blood was drained down into a reservoir from the right atrium through a jugular vein cannula, then the blood infused into the arterial system through a femoral or tail artery cannula after being drived by a peristaltic pump, oxygenated by a membrane oxygenator and warmed or cooled by a heat exchanger. Thus the CPB circuit was formed. Blood priming, total priming reduction and enough bypass flow were the key points in establishing the CPB model in rats.
A Model of Orthotopic Right Lung Transplantation in Rats

Abstract:Objective To report the preliminary experience with the techniques of orthotopic right lung transplantation in rats, and to evaluate an application of the model in the study on ischemia-reperfusion injury after experimental lung transplantation. Methods Pure breed SD rats (weighing 250～270 g) were chosen. Under an operation microscope (10) for both donor and recipient procedures, 12 consecutive orthotopic right lung transplantations in rats were performed. Under the general anes-thesia and mechanical ventilation, middle line laparothoracotomy was used to expose the heart and bi-lateral lungs. Heparin was given through the inferior vena cava. After opening the right ventricular outflow tract and exsanguinating through the left auricle, a perfusion catheter was placed in the pul-monary artery. Following lung perfusion, the donor lung was retrieved, and properly preserved. The techniques of general anesthesia and mechanical ventilation for the recipient were the same as the do-nor. Right posterior thoracotomy through the fourth intercostal space was performed. Pulmonary ves-sels and bronchus were isolated, clamped and excised as far from the clamps as possible. Donor lung was implanted, all bronchial and vessel end-to-end anastomoses were hand-sewn with 9-0 single strand running suture. The recipients" intraoperative and postoperative conditions were recorded. Results The mean cold ischemic time of the graft was (131.25 ± 20.24) min; mean time of lung transplanta-tion was (91.25 ± 15.97) min. A total of 8 recipients survived for the entire 24 h observation period. Success rate of right lung transplantation was 66.6%. Furthermore, 6 recipients survived for more than 4 weeks. Conclusion Rat right lung transplant model is a new option for ischemia-reperfusion in-jury study after lung transplantation.
Low-Volume Ex Situ Lung Perfusion System for Single Lung Application in a Small Animal Model Enables Optimal Compliance With " Reduction" in 3R Principles of Animal Research

Abstract:Ex situ lung perfusion (ESLP) is used for organ reconditioning, repair, and re-evaluation prior to transplantation. Since valid preclinical animal models are required for translationally relevant studies, we developed a 17 mL low-volume ESLP for double- and single-lung application that enables cost-effective optimal compliance "reduction" of the 3R principles of animal research. In single-lung mode, ten Fischer344 and Lewis rat lungs were subjected to ESLP and static cold storage using STEEN or PerfadexPlus. Key perfusion parameters, thermal lung imaging, blood gas analysis (BGA), colloid oncotic pressure (COP), lung weight gain, histological work-up, and cytokine analysis were performed. Significant differences between perfusion solutions but not between the rat strains were detected. Most relevant perfusion parameters confirmed valid ESLP with homogeneous lung perfusion, evidenced by uniform lung surface temperature. BGA showed temperature-dependent metabolic activities with differences depending on perfusion solution composition. COP is not decisive for pulmonary oedema and associated weight gain, but possibly rather observed chemokine profile and dextran sensitivity of rats. Histological examination confirmed intact lung architecture without infarcts or hemorrhages due to optimal organ procurement and single-lung application protocol using our in-house-designed ESLP system.
A Split-Lung Ex Vivo Perfusion Model for Time- and Cost-Effective Evaluation of Therapeutic Interventions to the Human Donor Lung

Abstract:With the ongoing shortage of donor lungs, ex vivo lung perfusion (EVLP) offers the opportunity for objective assessment and potential therapeutic repair of marginal organs. There is a need for robust research on EVLP interventions to increase the number of transplantable organs. The use of human lungs, which have been declined for transplant, for these studies is preferable to animal organs and is indeed essential if clinical translation is to be achieved. However, experimental human EVLP is time-consuming and expensive, limiting the rate at which promising interventions can be assessed. A split-lung EVLP model, which allows stable perfusion and ventilation of two single lungs from the same donor, offers advantages scientifically, financially and in time to yield results. Identical parallel circuits allow one to receive an intervention and the other to act as a control, removing inter-donor variation between study groups. Continuous hemodynamic and airway parameters are recorded and blood gas, perfusate, and tissue sampling are facilitated. Pulmonary edema is assessed directly using ultrasound, and indirectly using the lung tissue wet:dry ratio. Evans blue dye leaks into the tissue and can quantify vascular endothelial permeability. The split-lung ex vivo perfusion model offers a cost-effective, reliable platform for testing therapeutic interventions with relatively small sample sizes.

Liquid Ventilation Reconditions Isolated Rat Lungs Following Ischemia-Reperfusion Injury

Abstract:Lung transplantation remains the only curative treatment for end-stage pulmonary disease. Lung ischemia-reperfusion injury (IRI) is a major contributor to primary allograft dysfunction and donor organ nonutilization. The alveolar macrophage is a key inflammatory mediator in IRI. Ex vivo lung perfusion (EVLP) has been investigated to rehabilitate lungs before transplant but has failed to provide significant improvements after IRI. We hypothesized that liquid ventilation (LV) could be utilized for ex vivo lung reconditioning in a rat IRI model. We compared EVLP with LV in an isolated ex vivo rat lung with an aqueous ventilant using quantitative physiological and immunological parameters. We observed improved physiological parameters and mechanical clearance of alveolar macrophages and cytokines halting the propagation of the inflammatory response in IRI. While the wide applicability to large animal or human transplantation have yet to be explored, these findings represent a method for lung reconditioning in the setting of significant IRI that could widen the lung organ donation pool and limit morbidity and mortality associated with ischemia-induced primary graft dysfunction.

Abstract:Objective To evaluate pulmonary vascular permeability by using an improved situ-isolated lung perfusion rat model.Methods Based on traditional rat isolated lung perfusion methods,we established the improved situ-isolated lung perfusion rat model and Lipopolysaccharide(LPS)induced lung injury model.The content of Evans blue(EB)in lung was tested and the albumin permeability-surface area(PS)was caculated to quantitatively evaluate pulmonary vascular permeability.Results PS values of normal control and lung injury group were 5.37±0.94,25.00±8.8×10-2 cm3(/min.g),respectively.Conclusion The present evaluation method of pulmonary vascular permeability is simple,accurate positioning and reliable.
Establishment of a rat lung transplantation model with non-heart beating donor

Abstract:BACKGROUND: A large number of experiments have shown that the donor lung tissue and pulmonary vascular function can be stored under appropriate conditions. When the donor heart beating stops, the pulmonary gas exchange system can be tolerant to the warm ischemia for 60 minutes. OBJECTIVE: To establish a simple and effective rat lung transplantation model with non-heart beating donor. METHODS: Sixty Sprague-Dawley rats were randomly divided into heart beating donor group, non-heart beating donor-ischemia 30 minutes group and non-heart beating donor-ischemia 60 minutes group, there were 20 rats in each group. The lungs of the donor rats in the heart beating donor group were obtained and stored in 4 ℃ low potassium dextran solution for 4 hours; rats in the non-heart beating donor-ischemia 30 minutes group were bloodletting sacrificed, and the lungs of the rats were maintained ventilation for 30 minutes, and then performed with cold preservation after hot ischemia for 30 minutes; the treatment method in the non-heart beating donor-ischemia 60 minutes group was similar to that in the heart beating donor-ischemia 30 minutes group, the difference was that the time of hot ischemia. The peak airway pressure and pulmonary venous blood oxygen partial pressure was measured at 1, 15, 30, 45 and 60 minutes after the connection of the circulation loop in each group. After perfusion, the wet weight and dry weight ratio and the malondialdehyde level were detected. RESULTS AND CONCLUSION: There was no significant difference of all the indexes between heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P > 0.05). The peak airway pressure in each group was increased gradually along with the extension of time, but the peak airway pressure in non-heart beating donor-ischemia 60 minutes group was higher than that in the heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P < 0.05). The pulmonary blood oxygen partial pressure in each group was gradually decreased with the extension of time, and the pulmonary blood oxygen partial pressure non-heart beating donor-ischemia 60 minutes groupwas lower than that in heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P < 0.05). The wet/dry weight ratio and the malondialdehyde level in non-heart beating donor-ischemia 60 minutes group were higher than those in heart beating donor group and non-heart beating donor-ischemia 30 minutes group (P < 0.05). Non-heart beating donor-ischemia 30 minutes is a safe and effective method for lung transplantation.
Application of Roller Pump in Establishing a Piglet Model of Lung Perfusion

Abstract:Objective In order to evaluate if ischemia-reperfusion injury to the lung is more serious than the continuous perfusion injury to the lung, we decided to establish a new isolated lung model of piglets by using a roller pump. Methods Twelve piglets were randomly divided into control and experimental groups. Three catheters were inserted into the right atrium,left atrium and main pulmonary artery,respectively. The whole blood was drained into the reservoir through the right atrium catheter before the lung perfusion. The perfusion solution in reservoir was pumped into the main pulmonary artery by a roller pump, and the solution was drained back to the reservoir from the left atrium. In the experimental group, the lungs were continuously perfused by the roller pump with a flow of 80 mL/kg/min for 120 minutes. In the control group, the lungs perfusion started after 90 minutes ceasation and the perfusion lasted 30 minutes at the same flow. The lung compliance and the content of TNF-α and IL-6 in the perfusate were measured at the beginning and at the end of the experiment. The lung wet to dry weight ratio (W/D) was calculated after perfusion. Ultrastructural changes of the lungs were also examined after experiment. Results The lung compliance was 6.14±1.17 mL/cm H_2O in the experimental group and 7.89±0.94 mL/cm H_2O in the control group (P=0.017). The W/D ratio was 5.18±0.97 in the control group and 3.84±1.08 in the experimental group (P=0.048), indicating that the W/D ratio in the control group was significantly better than that in the experimental group. The content of TNF-α was 0.80±0.26 ng/mL in the experimental group and 0.52±0.15 ng/mL in the control group (P=0.044), showing the TNF-α content was significantly higher in the experimental group. Milder lung injury was observed in the experimental group by electron microscopic examination. Conclusion The results demonstrate that a continuous perfusion may cause less severe lung injury than the ischemia-reperfusion lung injury. The operation described in this study is convenient to establish the piglet model with high reliability and feasibility.

Abstract:Objective To establish rat orthotopic left lung allograft transplantation model by the improved "cuff-like" vessel and bronchial anastomosis technique. Methods Thirty-six male Sprague-Dawley rats were randomly allocated to donors and recipients. The donor right hilus was clipped for 5 mins(clipping test), then the blood samples were collected from carotid artery for arterial blood gas analysis as baseline. The graft's oxygenation function was also evaluated after 2-hour reperfusion. Dynamic compliances(Cdyn), resistances of airway(Raw) and peak inspiratory pressures(PIP) were monitored before the operation and after 2-hour reperfusion. Results After 2-hour reperfusion, Cdyn, Raw and PIP were worse than related baselines, but there weren't significant differences. The PaO_ 2 , OI, Qs/Qt, and A-aO_ 2 were significantly worse than related baseline values(P<0.01). This improved model makes the operation easier than ever. Conclusion This simpler model may be used to study lung transplantation.
