Abstract:Acute radiation dermatitis (RD) is one of the major toxicities in patients with breast cancer (BC) receiving radiation therapy (RT), which significantly affects the patient's treatment tolerance and quality of life. However, to differentiate the degree of severe RD could be challenging due to the variations related to current clinical methods used to evaluate the skin toxicity symptoms. A test that facilitates identification of patients who are likely to develop severe RD could have a personalized therapeutic implication. In this study, we developed, for the first time to our knowledge, a radiothermomics-enabled predictive model to synergistically integrate planning 4DCT radiomics signatures with surface skin temperature distribution imaged with thermography for early prediction of severe RD. A total of 180 patients with BC receiving RT were included in this study. All patients received 4DCT scan and 1766 radiomics features were extracted from PTV and the surface skin tissue region on PTV (PTV-Skin) regions, respectively. The surface skin temperature distribution features of the treated breast were also derived from the thermography images taken during the course of RT. RD and non-RD group classes were labeled within the patients based on the cut-off value of CTCAE ≥ 2. The patients were randomly partitioned for model training and validation. The Mann-Whitney test was applied to assess the differences of individual 4DCT radiomics and thermography features between RD and non-RD groups in the training cohort. The 4-fold cross validation method and the least absolute shrinkage and selection operator (LASSO) regression were employed to select optimal features, which were then used to build radiomics and thermography signatures separately. A radiothermomics model was constructed to combine demographic and clinical characteristics, radiomics and thermography signatures using the multivariable logistic regression method. The utility of the model constructed in predicting severe RD was evaluated by the ROC analysis in the validation cohort. Two radiomics features from PTV-Skin region, eight radiomics features from PTV region, one clinical factor and one thermography feature show significant differences between RD and non-RD groups in the training cohort. The individual radiomics signatures built from these optimal radiomics features in PTV-skin, PTV and combined PTV-skin-PTV regions show significantly different performances in predicting severe RD with AUC values of 0.792, 0.665 and 0.863, respectively. Furthermore, the radiothermomics model constructed showed an improved predictive value with an AUC value of 0.917 in the validation cohort. The radiothermomics model depicting the correlation of PTV and skin temperature with severe RD could be used to accurately predict the severe RD in patients with BC receiving RT, which could potentially improve the effectiveness of RT for BC from a precision treatment perspective.
