Recent advances in elastography have made possible the imaging of viscoelastic moduli within individual muscles [1, 2]. We have investigated a new approach, supersonic shear imaging (SSI), which combines high frame-rate (5kHz) ultrasound with acoustic radiation force induction of shear plane waves transients [3]. The elastic moduli can then be deduced from the propagation of these waves, and the viscous moduli from their attenuation.

Abstract:The aim of this study was to evaluate the arterial and venous blood flow in women who underwent upper limb axillary dissection surgery for the treatment of breast cancer. Sixty women were divided into two groups: group 1 (G1)—30 women who underwent breast surgery with axillary dissection level II or III (55.6 ± 8.6 years); group 2 (G2)—control, 30 women with no breast cancer (57.4 ± 7.0 years). Blood flow profile was evaluated by a continuous wave ultrasound Doppler device (Nicolet Vascular Versalab SE®) with an 8 MHz probe. Axillary, brachial arteries and veins, arm circumference, volumes, and the ankle-brachial index (ABI) were examined. Wilcoxon test and Mann–Whitney tests were applied to analyze blood flow velocity intra-group and between G1 and G2, respectively. The G1 results showed no lymphedema and no peripheral arterial disease (ABI > 0.9). Moreover, the mean blood flow velocity of the vessels ipsilateral to the surgery was significantly higher than the contralateral ones for all vessels examined (P < 0.05). The mean velocity of blood flow of the vessels contralateral to surgery was significantly higher than the axillary artery in G2 (P < 0.05). It can be concluded that women who underwent axillary dissection due to breast cancer showed probable stenosis in the arterial and venous axillary and brachial vessels of the upper limb ipsilateral to the surgery, confirmed by the increase of blood flow velocity, and such obstruction might affect the limb contralateral to the operation site.

Elongation of intercostal nerve cutaneous branches for breast and nipple neurotization during breast reconstruction after mastectomy for breast cancer: case-control study

Abstract:Background: To restore sensation after breast reconstruction, a modified surgical approach was employed by identifying the cut fourth intercostal lateral cutaneous branch, elongating it with intercostal nerve grafts, and coapting it to the innervating nerve of the flap or by using direct neurotization of the spared nipple/skin. Methods: This was a retrospective case-control study including 56 patients who underwent breast neurotization surgery. Breast operations included immediate reconstruction after nipple-sparing mastectomy (36 patients), skin-sparing mastectomy (8 patients), and delayed reconstruction with nipple preservation (7 patients) or without nipple preservation (5 patients). Patients who underwent breast reconstruction without neurotization were included as the non-neurotization negative control group. The contralateral normal breasts were included as positive controls. Results: The mean(s.d.) monofilament test values were 0.07(0.10) g for the positive control breasts and 179.13(143.31) g for the breasts operated on in the non-neurotization group. Breasts that underwent neurotization had significantly better sensation after surgery, with a mean(s.d.) value of 35.61(92.63) g (P < 0.001). The mean(s.d.) sensory return after neurotization was gradual; 138.17(143.65) g in the first 6 months, 59.55(116.46) g at 7-12 months, 14.54(62.27) g at 13-18 months, and 0.37(0.50) g at 19-24 months after surgery. Two patients had accidental rupture of the pleura, which was repaired uneventfully. One patient underwent re-exploration due to a lack of improvement 1.5 years after neurotization. Conclusion: Using the lateral cutaneous branch of the intercostal nerve as the innervating stump and elongating it with intercostal nerve grafts is a suitable technique to restore sensation after mastectomy. This method effectively innervates reconstructed breasts and spares the nipple/skin with minimal morbidity.
Relationship between lymphedema after breast cancer treatment and biophysical characteristics of the affected tissue

Abstract:The treatment of breast cancer is often complicated by lymphedema of the upper limbs. Standard lymphedema evaluation methodologies are not able to measure tissue fibrosis. The ultrasound aspects related to tissue microstructures of lymphedema are neglected in clinical evaluations. The objective of this study was to identify and measure the degree of impairment, topography, and biophysical alterations of subcutaneous lymphedema tissue secondary to the treatment of breast cancer by ultrasonography. Forty-two women at a mean age of 58 (±9.7) years, with unilateral lymphedema due to breast cancer treatment, were evaluated. The upper limbs were divided into affected (affected by lymphedema) and control (contralateral limb). Each limb was subdivided into seven areas, defined by perimetry, evaluated in pairs. The biophysical characteristics thickness, entropy, and echogenicity were evaluated by ultrasonography. The results showed a significant difference in the echogenicity and thickness variables between the affected and unaffected upper limb, in all the extent of the upper limb, while entropy showed no significant difference. The findings indicate that the data presented were consistent both in identifying and measuring the degree of impairment and biophysical changes in the subcutaneous tissue of lymphedema secondary to the treatment of breast cancer.

[Value of somatosensory evoked potentials in thoraco- brachial outlet syndrome]

Abstract:Introduction: Brachial plexus involvement in symptoms of thoracic outlet syndrome (TOS) is often difficult to assess from clinical data. Conventional EMG and nerve conduction studies (NCS) do not seem reliable to all authors. For this reason, our investigations of this syndrome were complemented by study of somatosensory evoked potentials (SEP) in order to compare the results of these different techniques. Patients and methods: Ten patients were studied, all of whom had prominent vascular symptoms which led to their consulting a vascular surgeon. Only one had hand wasting without hypoesthesia. None had cervical rib or cervical spine anomaly. In all cases, diagnosis was confirmed by arteriography or phlebography. Operations were decided on clinical data and results of vascular investigations. Patients were tested with conventional motor and sensitive NCS F-wave studies. Needle EMG was performed in abductor pollicis brevis, first dorsal interosseus or abductor digitiminimi. Their SEP were performed as for controls. Ten controls were studied whose SEP were obtained at Erb's point (N9) and C2 cervical spine level (N13) after percutaneous stimulation of median and ulnar nerves at the wrist on both sides. The criterion of abnormality was the mean of controls + 2.5 SD for latencies. Amplitude was considered as low when it was less than 50% of the contralateral one. Results: For 2 patients EMG, NCS and SEP were abnormal. One had hand wasting and denervation in hand muscles as well as slowed median and ulnar sensory conduction with low amplitude responses. SEP at Erb's point were slightly delayed after ulnar stimulation. No cervical response was obtained after ulnar stimulation. The second one had normal responses at Erb's point but delayed responses at the cervical level. In addition, N13 amplitude after ulnar stimulation was low. Four patients had normal EMG, NCS and SEP. Two patients had normal EMG and NCS, but their SEP was questionable since latencies were normal, even though amplitude was low after median and ulnar stimulation. This was not considered this to be abnormal since it was bilateral. For the remaining 2 patients (F.1), EMG and NCS and Erb's point SEP were normal, but C2 median and ulnar responses were delayed in one case and C2 ulnar response amplitude was very low on one side only in the other. In conclusion, SEP were abnormal for 4 patients out of 10 but gave more information than conventional EMG and NCS for only 2 patients. SEP abnormalities prevailed after ulnar stimulation.

Abstract:Purpose: This study aims to map the point of emergence of the lateral cutaneous branch of the fourth intercostal nerve in both sexes, addressing gaps in the current literature. The findings are intended to support surgeons in restoring breast function by providing precise anatomical data, regardless of the specific surgical technique employed. Methods: An anatomical study was conducted using 50 sides from 25 formalin-preserved full-body donors, with equal representation of male and female subjects. Measurements were taken to determine the fascial point of emergence of the lateral cutaneous branch of the fourth intercostal nerve relative to the anterior axillary line and the transverse line. Multiple linear regression models were applied to evaluate the influence of variables such as sex, height, and side. Results: Our findings indicate that the fascial point of emergence of the lateral cutaneous branch of the fourth intercostal nerve is consistently located within 20 to 24 mm relative to the transverse line in 37 (74%) cases, and within 4 to 10 mm relative to the anterior axillary line in 39 (78%) cases, regardless of sex, height, or side. The multiple linear regression analysis showed no significant correlation between these variables and the position of the point of emergence (p > 0.05). Conclusion: These results provide surgeons with a practical tool for nerve coaptation during breast reconstruction, showing that sex and height do not affect the anatomical localization of the point of emergence. Precise knowledge of this point can improve surgical accuracy and enhance sensory recovery, leading to better patient outcomes.
Analysis and relationship between the volume of upper limb lymphoedema and pressure pain threshold, neural range of motion, pain intensity, kinesiophobia, pain hypervigilance and catastrophizing in breast cancer survivors

Abstract:Background: Lymphedema of the upper limbs and persistent pain are frequent sequelae after surgical treatment of breast cancer. Aim: The aim of this paper was to analyze the upper limb volume, pressure pain threshold, neural range of motion, pain intensity, kinesiophobia, pain hypervigilance and catastrophizing in patients with and without lymphoedema after breast cancer surgery. Secondly, we aimed to investigated the association between upper limb volume and these variables. Design: Descriptive observational study. Setting: Faculty of Health Sciences of the University of Granada. Population: Fifty-eight post-surgical breast cancer survivors, 29 with upper limb lymphoedema and 29 without lymphoedema. Methods: We measured upper limb volume (perimetric method). Also, pressure pain thresholds were assessed with a digital algometer, neural range of motion (neurodynamic test for radial, ulnar and median nerves), pain intensity (visual analogue scale), kinesiophobia, pain hypervigilance and catastrophizing (validated tests). To detect differences between the groups for the measurement variables we performed a t-test for independent samples analysis. A simple linear regression analysis adjusting for age and body mass index was performed to check the association among upper limb volume and pain variables in the group with lymphoedema. Results: The analysis showed that lymphoedema group had lower pressure pain threshold bilaterally in the masseter (origin P≤0.036; insertion P≤0.046), temporalis (insertion P≤0.021), suboccipitalis (P≤0.036); second (P≤0.014), third (P≤0.001) and tenth rib (P≤0.001); affected side of the temporalis (origin P=0.025); temporomandibular joint (P=0.024); neural range of motion in the median nerve (P=0.047), ulnar (P=0.042) on the affected side and radial (P=0.039) on the unaffected side; and greater kinesiophobia (P=0.042). Linear regression analysis only showed a significant association between upper limb volume and neural range of motion in the radial nerve (P=0.020) in the lymphedema group. No significant associations were obtained for the rest of variables. Conclusions: These findings suggest that the presence of lymphoedema may contribute to an increased level of generalized mechanosensitivity and fear to movement in this population. Clinical rehabilitation impact: Upper limb lymphedema can lead to heightened mechanosensitivity and movement-related fear in breast cancer survivors. Therefore, fast track rehabilitation approach should be focus in screening and rehabilitation methods for detection and control this sequalae.
Do medical procedures in the arm increase the risk of lymphoedema after axillary surgery? A review

Abstract:Lymphoedema of the arm is a potentially serious consequence of any axillary procedure performed during the management of breast cancer. In an attempt to reduce its incidence and severity, patients are instructed to avoid venepunctures and blood pressure measurements on the treated arm. These precautions are not possible in some patients and attempts to adhere to them can cause discomfort, anxiety and stress for both patients and their health-care workers. The strength with which these recommendations are made is in contrast to the level of evidence underpinning them. This paper reviews this evidence regarding the safety, or lack thereof, of blood pressure monitoring and intravenous puncture in women who have had axillary surgery. With this evidence generally being anecdotal in nature, there appears to be no rigorous evidence-based support for the risk-reduction behaviours of avoiding blood pressure monitoring and venepuncture in the affected arm in the prevention of lymphoedema after axillary procedure. A clinical trial was proposed to investigate whether such avoidance measures were valuable, but failed during its inception. There remains a need for research from prospective trials on this controversial topic to determine the most appropriate patient recommendations that should be provided after axillary procedure regarding the risks for development of lymphoedema.
Lymphoedema and reduced shoulder function as indicators of quality of life after axillary lymph node dissection for invasive breast cancer

Abstract:Abstract Background The aim was to explore measurements of arm circumference and shoulder abduction as indicators of quality of life after axillary lymph node dissection for invasive breast cancer. Methods Differences in arm circumference and shoulder abduction were measured in 465 consecutive women who underwent axillary lymph node dissection. These women received a treatment-specific questionnaire on the severity of physical disability and the effects on their daily life and well-being. Results The questionnaire was returned by 400 women (86 per cent). Of these 400, only the 332 women who did not receive axillary radiotherapy were included in the analysis. Their mean time since axillary lymph node dissection was 4·2 (range 0·3–28) years. For 86 patients (26 per cent) there was a difference in arm circumference of 2 cm or more, or a difference in abduction of 20° or more. These patients found it more difficult to do household chores, were more likely to have given up hobbies, felt more disabled and were more likely to be treated by a physiotherapist. However, complaints also occurred among the women with smaller differences in arm circumference and shoulder abduction, although the frequency and severity of their complaints were similar to those in women without swelling of the arm or without restricted shoulder abduction. Conclusion Measuring arm circumference and shoulder abduction during control visits identifies only some of the women whose daily life and well-being is affected by the side-effects of axillary lymph node dissection.

Abstract:The clinical picture of an acute frosted periphlebitis has been reported in a limited number of otherwise healthy patients, and in one case of human immunodeficiency virus (HIV) infection. The disease is usually bilateral and highly sensitive to steroids. The case of a 26-year-old, HIV-seropositive woman who developed unilateral diffuse perivenous sheathing in the course of recurrent cytomegalovirus retinitis is reported. The appearance of the retinovasculitis was identical to that described as an acute frosted periphlebitis, and the disease proved to be highly sensitive to a combined treatment of steroids and ganciclovir. The clinical findings in this case support the possibility that the appearance of an acute frosted periphlebitis can occur in response to several different stimuli, including cytomegalovirus.
Analysis of Different Lymphedema Assessment Tools in Women with Breast Cancer After Mastectomy

Abstract:Background: Lymphedema is a common complication after mastectomy in women with breast cancer. Several methods have been described to assess and diagnose lymphedema, one of the most studied being the perimeter and ultrasonography. However, the reliability of these methods and the correlation between them are still controversial. The aim of this study was to analyze the reliability of cytometry and ultrasound imaging in the assessment of lymphedema after mastectomy in women with breast cancer and to study the correlation between them. Methods and Results: A cross-sectional study was conducted in 29 women with mastectomy after breast cancer. Lymphedema in the arm was measured both with cytometry and ultrasonography. Reliability was calculated with intraclass correlation coefficient. The correlation between the two methods was carried out with the Pearson correlation coefficient. Both cytometry (M1: α = 0.999, ICC = 0.996; M2: = α = 0.998, ICC = 0.994) and ultrasonography (M1: α = 0.992, ICC = 0.976; M2: = α = 0.991, ICC = 0.973) are reliable methods to assess lymphedema in the arm. No significant correlation was found between them (p > 0.05). Conclusions: Cytometry and ultrasonography appear to be adequate for the measurement of edema in women with breast cancer after mastectomy. However, for an accurate measurement of lymphedema, these measurements should not be used interchangeably.
Reproducibility of Thermography for Measuring Skin Temperature of Upper Limbs in Breast Cancer Survivors

Abstract:Background/Objectives: Breast cancer-related lymphedema (BCRL) is a chronic condition that has early diagnosis as a critical component for proper treatment. Thermography, a non-invasive imaging method, is considered a promising complementary tool for the diagnosis and monitoring of BCRL, especially in subclinical stages. The present study aimed to evaluate the intra- and inter-examiner reproducibility of thermography for measuring the skin temperature of the upper limbs (UL) of women with and without BCRL. Methods: This study, conducted with women who underwent a unilateral mastectomy, assessed BCRL using indirect volumetry. Maximum, minimum, and mean skin temperatures were measured in five regions of interest (ROI) of each UL (C1, C2, C3, C4, and Cup) in four different postures. Reproducibility measures were assessed using an intraclass correlation coefficient, 95% confidence interval, and coefficient of variation. Results: The sample comprised 30 women; 14 were diagnosed with BCRL. A total of 120 thermograms were recorded in different postures, and 3600 ROI were analyzed in the UL with and without BRCL. The intraclass correlation coefficient of the analyses indicated intra- and inter-examiner reproducibility from good to excellent (0.82 to 1.00) for all skin temperatures evaluated (maximum, minimum, and mean). The coefficient of variation for all measures was below 10%, indicating low variability. Conclusions: Our findings demonstrate that thermography shows good-to-excellent reproducibility across multiple postures and regions of interest, reinforcing its potential as a non-invasive and reliable method for assessing lymphedema in breast cancer survivors. This study establishes a foundation for incorporating thermography into clinical practice for early BCRL detection, particularly in subclinical stages, thus improving patient management and outcomes.

Abstract:Background/Objectives: Infrared thermography (IRT) is an imaging technique used in clinical practice to detect changes in skin temperature caused by several dysfunctions, including breast cancer-related lymphedema (BCRL). Thus, the present study aimed to assess the reproducibility and accuracy of IRT in diagnosing BCRL. Methods: This cross-sectional study included participants who underwent a unilateral mastectomy and used indirect volumetry for lymphedema detection. IRT analysis was recorded in four positions, analyzing maximum, mean, and minimum temperatures, as well as the temperature differences between the upper limbs. The analysis encompassed reliability, agreement, accuracy, and the establishment of cut-off points for sensitivity and specificity. A total of 88 upper limbs were included; 176 thermograms were captured, and 1056 regions of interest were analyzed. Results: IRT presented excellent intra- and inter-rater reproducibility and reliability with excellent intraclass correlation coefficient values (0.99 to 1.00). In addition, this assessment reached a sensitivity of 85% and a specificity of 56%; the cut-off point considered a temperature difference of −0.45 °C. Conclusions: IRT was a reliable and reproducible assessment, and the temperature difference between the upper limbs evidenced moderate accuracy. Thus, IRT is recommended as a complementary technique for detecting BCRL.

The Clinical Usefulness of Lymphedema Measurement Technique Using Ultrasound

Abstract:Background: We previously invented a new technique to measure the cross-sectional area of soft tissue of a limb (ΔCSA) with lymphedema using ultrasonography. The measurement correlated strongly with both circumference and volumetry in normal subjects. The purpose of this study was to measure the reliability and accuracy of the method in patients with lymphedema. Methods and Results: Ultrasonography was performed on both arms of 69 female patients diagnosed with stage ≥1 lymphedema related to advanced breast cancer. At 10 cm above elbow (AE) and below elbow (BE), soft-tissue thicknesses at various locations were measured by two examiners. Subcutaneous tissue stiffness was also obtained by measuring thickness differences of soft tissue when applying minimal and maximal pressure to the skin (compliance) and its ratio to the initial thickness (compliance ratio). ΔCSA showed a strong positive correlation with circumference (r = 0.758 to 0.951), and a moderate negative correlation with Z at 5 Hz (r = -0.326 to -0.486). Intra- and interclass coefficients of all ultrasonography measurements were moderate to excellent (0.623-0.990). Compliance measured at 10 cm BE on the lesion side was significantly higher than on the normal side (p < 0.001), and compliance measured at 10 cm AE showed no difference between the two sides (p = 0.653). Conversely, compliance ratios measured at 10 cm AE and BE on the lesion side were significantly lower than on the normal side (p < 0.001). Conclusion: Thus, ΔCSA using ultrasonography could be a particularly viable option for determining status in lymphedema patients.

Abstract:The lateral antebrachial cutaneous nerve (LACN) is the sensory branch of the musculocutaneous nerve and usually innervates the lateral aspect of the forearm. Isolated lesions are rare, of varied etiology and generally underdiagnosed. We present a retrospective descriptive study of electromyography performed at the General University Hospital of Castellón in the last 20 years with isolated NCAL lesion. We identified 11 cases (8 men and 3 women), average age 44 years (15-73 years). 73% were referred from traumatology. Only one patient was correctly guided in the application. 63.6% of cases noted hypoesthesia extending to the wrist and 18.2% to the thumb. The electromyographic study showed severe axonal involvement in 3 patients and moderate in 8. The symptoms were observed associated with surgery in 4 patients, manipulation of the elbow flexure in 4 cases and bicipital tendonitis in the rest. Four patients had a poor clinical outcome (3 with severe axonal involvement and 1 with moderate involvement). Isolated involvement of the NCAL is a rare and underdiagnosed alteration. It is important to suspect it in patients with hypoesthesia in the forearm, including the radial edge of the wrist or thumb, especially if it is associated with manipulations around the elbow flexure or bicipital tendonitis. Electromyography is useful in confirming the diagnosis, ruling out other differential diagnoses, and predicting prognosis. Knowing the location of this nerve during manipulations on the arm and placing patients in an appropriate posture during surgeries can help minimize cases.
Analysis on the questionnaire of arm complications in breast cancer patients after operation

Abstract:Objective To post-operatively investigate the incidence of the arm complications in breast cancer patients.Methods A total of 452 patients with breast cancer,including 191 patients with total axillary lymph node dissection and 261 patients with partial axillary lymph node dissection,were recruited from January 2001 to January 2007 in breast center of Beijing Cancer Hospital.The circumferences of upper arm and forearm on both ipsilateral and contralateral sides in breast cancer patients post-operatively were acquired through questionnaires.Results Among 452 breast cancer patients in this study,incidences of upper arm and forearm edema after axillary dissection were 14.4％ (65/452)and 7.3％ (33/452)in total and partial axillary dissections respectively.Incidences of upper arm edema were 14.6％ (28/191) and 14.2％ (37/261) in total and partial axillary dissections respectively (P ＞ 0.05) ; incidences of forearm edema were 6.3％ (12/191) and 8.0％ (21/261) in total and partial axillary dissections respectively (P ＞ 0.05) ; there were 17.8％ (34/191) and 19.5 ％ (51/261) in total and partial axillary dissections respectively (P ＞ 0.05) regarding combing malfunction (abduction and supination) ; 9.9％ (19/191) and 8.8％ (23/261) in total and partial axillary dissections were incapable of fastening hooks from back (afterward-stretching,intorsion).Conclusion The range of axillary lymph node dissection is expanded in the group of total axillary lymph node dissection,which does not increase the incidence of upper limb edema and shoulder joint dysfunction.

Abstract:Abstract OBJECTIVE: To evaluate the prevalence of sensitivity changes in patients during the cycle of chemotherapy with taxanes and the impacts on quality of life. DESIGN: A prospective, observational, Approved study was conducted at the Oncomastology outpatient clinic of Hospital São Paulo- Unifesp, in the period from November 2018 to February 2022, where 102 women with breast cancer were selected and 79 patients who underwent chemotherapy with paclitaxel were included. METHODS: Collection of demographic and clinical information. Used stesiometer that assessed sensitivity in dermatos and myotomes in the following body regions: arms, hands, legs and feet. FACT/GOG-Ntx Questionnaire analyzed the quality of life and chemotherapy-induced peripheral neuropathy of the patients before starting chemotherapy. All participants were informed and signed an informed consent form. RESULTS: The mean age was 49 years, mean BMI was 27.95Kg/m2, NICC (Invasive Carcinoma not special) represented 89.9% of the sample, 88.6% had Neoadjuvant Chemotherapy, regarding the size of the tumor 45.6% presented from 2 to 5 cm, of the pre existing diseases Diabetes mellitus 15.2% and Systemic Arterial Hypertension 30.4%. The initial moment in relation to sensitivity alterations, of the arm regions presented (right C6 (HC 1.05-1.23) left C6 (HC 1.05- 1.23), and legs (right L5 (HC 1.14 -1.55) left L5 (HC1.22 - 1.52) and right S1 (HC 1.21-1.47) left S1 (HC 1.21-1.48). The conversion of the Estesiometer colors into numbers followed as follows, Green (1.0): normal sensitivity, Blue (2.0): decreased sensitivity. The FACT/Taxane (TOI) quality of life Questionnaire (30 items) its scores 0-120,(average 91.44 points) demonstrates in its response impairment in more than 80 % in the quality of life in the domains. Physical Well-Being-PWB (0-28 points) (mean 20.28/HC 18.88-21.66), Functional Well-Being-FWB (0-28 points) (mean16.00/HC14.22-17.22) and in the specific questions about chemotherapy-induced peripheral neuropathy (CPIN), TaxS (0-64 points) (mean 55.16/HC 52.74-57.26) reduced scores. CONCLUSION: It is concluded that mean age, overweight, tumors between 02 and 05 cm were the most prevalent among patients diagnosed with breast cancer. At the beginning of chemotherapy containing paclitaxel, sensitivity changes were not present characterizing arthralgia, myalgia and the hand-foot syndrome (CPIN). The quality of life evaluated by FACIT/Taxane, showed a reduction of 80% in the scores of the domains Physical Well-Being-PWB, Functional Well-Being-FWB and general quality of life, that is, the quality of life of patients from the time of diagnosis is already very compromised. Abstract:OBJECTIVE The aim of this study was to assess the use of atomoxetine and olanzapine in combination to treat attention-deficit/hyperactivity disorder (ADHD) and comorbid disruptive behaviors in children and adolescents 10-18 years of age. METHODS Eleven subjects ages 10-18 received open-label atomoxetine and olanzapine for a 10 week treatment period. Patients were assessed at baseline, 2 weeks, 4 weeks, 6 weeks, and 10 weeks (posttreatment). ADHD improvement was measured through the ADHD Rating Scale (ADHD-RS) (Investigator and Parent ratings). Aggression was measured through the Modified Overt Aggression Scale (MOAS). RESULTS The combined use of atomoxetine and olanzapine resulted in statistically significant improvement in ADHD symptoms and overt aggression from baseline to posttreatment. As evidenced by a 33% reduction in symptoms on the ADHD-RS-I and the MOAS, 73% of patients were considered responders to ADHD treatment, whereas 55% responded to treatment for aggression. Both medications were generally well tolerated. Olanzapine treatment was associated with significant weight gain. Patients gained, on average, 3.9 kg. throughout the treatment period. CONCLUSIONS These data provide initial evidence that combination use of atomoxetine and olanzapine for the treatment of ADHD and comorbid disruptive behaviors was effective in reducing ADHD symptoms and aggressive behavior in a 10 week treatment period.

For the last two years the traditional paper file has been replaced by more than 130 terminals distributed throughout this establishment. Additional functions may be added (statistics, internal information, etc...).

Abstract:Background: The need for axillary dissection (AD) is declining, but it is still essential for many patients with nodal involvement who risk developing breast-cancer-related lymphedema (BCRL) with lifelong consequences. Previous nonrandomized studies found axillary reverse mapping and selective axillary dissection (ARM-SAD) a safe and feasible way to preserve the arm's lymphatic drainage. Methods: The present two-arm prospective randomized clinical trial was held at a single comprehensive cancer center to ascertain whether ARM-SAD can reduce the risk of BCRL, compared with standard AD, in patients with node-positive breast cancer. Whatever the type of breast surgery or adjuvant treatments planned, 130 patients with nodal involvement met our inclusion criteria: 65 were randomized for AD and 65 for ARM-SAD. Twelve months after surgery, a physiatrist assessed patients for BCRL and calculated the excess volume of the operated arm. Lymphoscintigraphy was used to assess drainage impairment. Self-reports of any impairment were also recorded. Results: The difference in the incidence of BCRL between the two groups was 21% (95% CI, 3-37; p = .03). A significantly lower rate of BCRL after ARM-SAD was confirmed by a multimodal analysis that included the physiatrist's findings, excess arm volume, and lymphoscintigraphic findings, but this was not matched by a significant difference in patients' self-reports. Conclusions: Our findings encourage a change of surgical approach when AD is still warranted. ARM-SAD may be an alternative to standard AD to reduce the treatment-related morbidity.
