Abstract:Background: Several studies have demonstrated that uremic patients who have preserved left ventricular ejection fraction (LVEF) could still have the potential for systolic dysfunction. The aim of this study was to assess the differences between the left ventricular (LV) myocardial function in hemodialysis and nondialysis uremic patients based on three-dimensional speckle-tracking echocardiography.Methods: The study population consisted of 35 maintenance hemodialysis patients (the hemodialysis group), 30 uremic patients who were hospitalized for the creation of a primary arteriovenous fistula (the nondialysis group), and 32 healthy volunteers. All of the patients had normal left ventricular ejection fractions (i.e., 55% or greater). Three-dimensional speckle tracking echocardiography was performed to assess the left ventricle's global three-dimensional strain, regional longitudinal strain, circumferential strain, and radial strain.Results: The left ventricular regional longitudinal strain, radial strain, circumferential strain, and global three-dimensional strain were significantly decreased in the nondialysis patients compared with the other two groups (all, P<0.001). However, the three-dimensional strain and the regional longitudinal strain were lower in the hemodialysis patients than in the controls (P<0.01). In the hemodialysis patients and the control group, the longitudinal strain, circumferential strain, and radial strain were higher at the apical level than they were at the basal level and midlevels. A multivariate linear regression analysis showed that the blood urea nitrogen and creatinine levels were independently associated with the values of the global three-dimensional strain (beta = -0.217, P = 0.000; beta = -0.243, P = 0.011, respectively) and the longitudinal strain (beta = -0.154, P = 0.032; beta = -0.188, P = 0.029, respectively).Conclusions: Three-dimensional speckle-tracking echocardiography may detect myocardial dysfunction in patients with uremia who have preserved LVEF. The global three-dimensional strain and the regional longitudinal strain appear to be superior in hemodialysis patients compared with nondialysis patients.

Abstract:Introduction Hemodialysis (HD) patients are under observably elevated cardiovascular mortality. Cardiac dysfunction is closely related to death caused by cardiovascular diseases (CVD). In the general population, repetitive myocardial ischemia induced left ventricular (LV) dysfunction may progress to irreversible loss of contraction step by step, and finally lead to cardiac death. In HD patients, to remove water and solute accumulated from 48 or 72 hours of interdialysis period in a 4‐hour HD session will induce myocardial ischemia. In this study, we evaluated the prevalence and potential risk factors associated with HD‐induced LV systolic dysfunction and provide some evidences for clinical strategies.

Abstract:Hemodialysis (HD)-induced myocardial stunning, characterized by transient left ventricular systolic dysfunction during HD, has been reported to be common and associated with a poor prognosis. However, the pathophysiology is not fully understood. Hemodialysis-Related Acute Brain Injury Demonstrated by Application of Intradialytic Magnetic Resonance Imaging and Spectroscopy

Abstract:Significance Statement Hemodialysis (HD) results in reduced brain blood flow, and HD-related circulatory stress and regional ischemia are associated with brain injury over time. However, studies to date have not provided definitive direct evidence of acute brain injury during a HD treatment session. Using intradialytic magnetic resonance imaging (MRI) and spectroscopy to examine HD‐associated changes in brain structure and neurochemistry, the authors found that multiple white (WM) tracts had diffusion imaging changes characteristic of cytotoxic edema, a consequence of ischemic insult and a precursor to fixed structural WM injury. Spectroscopy showed decreases in prefrontal N-acetyl aspartate (NAA) and choline concentrations consistent with energy deficit and perfusion anomaly. This suggests that one HD session can cause brain injury and that studies of interventions that mitigate this treatment's effects on the brain are warranted. Background Hemodialysis (HD) treatment-related hemodynamic stress results in recurrent ischemic injury to organs such as the heart and brain. Short-term reduction in brain blood flow and long-term white matter changes have been reported, but the basis of HD-induced brain injury is neither well-recognized nor understood, although progressive cognitive impairment is common. Methods We used neurocognitive assessments, intradialytic anatomical magnetic resonance imaging, diffusion tensor imaging, and proton magnetic resonance spectroscopy to examine the nature of acute HD-associated brain injury and associated changes in brain structure and neurochemistry relevant to ischemia. Data acquired before HD and during the last 60 minutes of HD (during maximal circulatory stress) were analyzed to assess the acute effects of HD on the brain. Results We studied 17 patients (mean age 63±13 years; 58.8% were male, 76.5% were White, 17.6% were Black, and 5.9% were of Indigenous ethnicity). We found intradialytic changes, including the development of multiple regions of white matter exhibiting increased fractional anisotropy with associated decreases in mean diffusivity and radial diffusivity—characteristic features of cytotoxic edema (with increase in global brain volumes). We also observed decreases in proton magnetic resonance spectroscopy–measured N-acetyl aspartate and choline concentrations during HD, indicative of regional ischemia. Conclusions This study demonstrates for the first time that significant intradialytic changes in brain tissue volume, diffusion metrics, and brain metabolite concentrations consistent with ischemic injury occur in a single dialysis session. These findings raise the possibility that HD might have long-term neurological consequences. Further study is needed to establish an association between intradialytic magnetic resonance imaging findings of brain injury and cognitive impairment and to understand the chronic effects of HD-induced brain injury. Abstract:In patients with chronic kidney disease (CKD), reverse left ventricular (LV) remodelling, including reduction in LV mass, can be observed following long-term haemodialysis (HD) and has been attributed to regression of LV hypertrophy. However, LV mass can vary in response to changes in myocyte volume, edema, or fibrosis. The aims of this study were to investigate the acute changes in structural (myocardial mass and biventricular volumes) and tissue characterization parameters (native T1 and T2) following HD using cardiovascular magnetic resonance (CMR). Twenty-five stable HD patients underwent non-contrast CMR including volumetric assessment and native T1 and T2 mapping immediately pre- and post-HD. The mean time between the first and second scan was 9.1 ± 1.1 hours and mean time from completion of dialysis to the second scan was 3.5 ± 1.3 hours. Post-HD, there was reduction in LV mass (pre-dialysis 98.9 ± 36.9 g/m2 vs post-dialysis 93.3 ± 35.8 g/m2, p = 0.003), which correlated with change in body weight (r = 0.717, p < 0.001). Both native T1 and T2 reduced significantly following HD (Native T1: pre-dialysis 1085 ± 43 ms, post-dialysis 1072 ± 43 ms; T2: pre-dialysis 53.3 ± 3.0 ms, post-dialysis 51.8 ± 3.1 ms, both p < 0.05). These changes presumably reflect acute reduction in myocardial water content rather than regression of LV hypertrophy. CMR with multiparametric mapping is a promising tool to assess the cardiac changes associated with HD.

Abstract:Objective To detect the acute effects of a single hemodialysis(HD)session on left ventricular(LV)function in patients with end-stage renal disease(ESRD)on maintenance HD therapy using pulsed-wave tissue Doppler imaging(PW-TDI).Methods Fifty-seven patients underwent echocardiography 1 h prior to and 1 h following regular HD.Two-dimensional and Doppler echocardiography were used to analyze conventional LV structural and functional parameters.Systolic and diastolic mitral annular velocities assessed by PW-TDI.Results After HD,the size of LV significantly reduced.Parameters of LV systolic function-ejection fraction and systolic mitral annular velocity were significantly improved.As for LV diastolic function,conventional Doppler parameters were substantially decreased after HD,while PW-TDI diastolic velocities did not change significantly.And the ratio of E/Em also decreased after a single HD session.Conclusion A single HD session can improve the global LV systolic function.The LV diastolic function can be evaluated accurately with PW-DTE in the different LV preload conditions.
Impact of Hemodialysis on Left Ventricular-Arterial Coupling in End-Stage Renal Disease Patients

Abstract:Introduction: As a key determinant of cardiovascular performance, vascular-arterial coupling (VAC) has been reported to be a predictor of clinical outcomes in various clinical scenarios. However, few studies have explored how acute fluid removal during hemodialysis (HD) impacts the interaction between cardiac function and the arterial system. Methods: We recruited 317 HD patients from an established renal dialysis unit for this cross-sectional study and a total of 285 were included in the final analyses. We measured left ventricle end-systolic elastance (Ees), the effective arterial elastance (Ea), and VAC before and after HD using noninvasive echocardiographic measurements. We also compared echocardiographic and hemodynamic parameters in ventriculo-arterial coupling and ventriculo-arterial uncoupling patients. Results: HD significantly altered partial ventricular and vascular function parameters such as blood pressure, left ventricular end-diastolic volume, stroke volume, left ventricular ejection fraction, and systemic vascular resistance index. Ea increased following HD from 3.5 & PLUSMN; 1.4 to 4.2 & PLUSMN; 1.8 mm Hg/mL (p < 0.0001), Ees increased following HD from 7.9 & PLUSMN; 5.5 to 9.2 & PLUSMN; 6.9 mm Hg/mL (p = 0.04), whereas VAC did not markedly alter as a result of HD. Ventriculo-arterial uncoupling was found to be related to abnormal cardiac structure and worse systolic function. Conclusions: VAC obtained from echocardiography is likely to be load-independent and useful as a reliable index for stratifying the risk of cardiovascular diseases in HD patients. Further investigations on larger patient cohorts are needed to further validate our findings.

Abstract:BACKGROUND:Right ventricular (RV) dysfunction is a major cause of death in patients undergoing maintenance hemodialysis (MHD). We used 3-dimensional speckle-tracking echocardiography (3DSTE) to evaluate long-term impacts of MHD on RV function.HYPOTHESIS:In this study, RV dysfunction in MHD patients will be revealed and studied in depth by 3DSTE.METHODS:Echocardiography was performed on 110 consecutively enrolled individuals: 30 controls and 80 patients with MHD. Conventional echocardiographic parameters and 3DSTE parameters were obtained and compared between groups. Univariate and multivariate logistic regression analysis identified independent predictors of intradialytic hypotension (IDH).RESULTS:Compared with the control group, RV end-diastolic volume (RVEDV) was markedly enlarged (46.1 ± 11.8 mL/m2 vs 42.3 ± 8.6 mL/m2 ; P = 0.047), whereas RV ejection fraction (RVEF) was significantly lower in the MHD group (50.6% ± 5.8% vs 55.2% ± 3.7%; P < 0.001). RV global, septal, and lateral wall longitudinal strains were also decreased in the MHD group (-18.2 ± 3.6 vs -22.6 ± 4.3%; -13.1 ± 3.8 vs -17.5 ± 5.5%; and -23.4 ± 4.7 vs -27.7 ± 4.0%, respectively; all P < 0.001). RVEF (odds ratio [OR]: 0.72, 95% confidence interval [CI]: 0.51 to 1.01, P = 0.038) and history of diabetes (OR: 11.14, 95% CI: 1.16 to 106.71, P = 0.036) were 2 independent predictors of IDH. Ultrafiltration rate was an independent factor associated with RVEF (β = -0.01, 95% CI: -0.019 to 0.001, P = 0.039).CONCLUSIONS:RVEF by 3DSTE could be an important predictor of IDH in MHD patients, and lower ultrafiltration rate was protective for RVEF. 3DSTE may have potential in RV evaluation and risk stratification in MHD patients.

Abstract:Background/Objectives: Hemodialysis-induced myocardial stunning (HIMS) is a frequent complication in patients undergoing maintenance hemodialysis, characterized by transient left ventricular dysfunction due to ischemic episodes. Mitochondrial dysfunction and fluctuations in key ions such as potassium (K+) and calcium (Ca2+) are implicated in the pathogenesis of HIMS. This study aims to investigate the role of mitochondrial dysfunction and the protective potential of mitochondrial ATP-sensitive potassium channels (mitoKATP) in mitigating HIMS. Methods: A 5/6 nephrectomy rat model was established to mimic chronic kidney disease and the subsequent HIMS. The effects of mitoKATP channel modulators were evaluated by administering diazoxide (DZX), a mitoKATP opener, and 5-hydroxydecanoate (5-HD), a mitoKATP blocker, before hemodialysis. Mitochondrial function was assessed by measuring membrane potential, ATP synthase activity, and intramitochondrial Ca2+ levels. Myocardial function was evaluated using speckle tracking echocardiography. Results: Rats undergoing hemodialysis exhibited significant reductions in left ventricular strain and synchrony. DZX administration significantly improved mitochondrial function and reduced myocardial strain compared to controls. Conversely, 5-HD worsened mitochondrial swelling and disrupted myocardial function. Higher K+ and Ca2+ concentrations in the dialysate were associated with improved mitochondrial energy metabolism and myocardial strain. Conclusions: Mitochondrial dysfunction and ion imbalances during hemodialysis are key contributors to HIMS. The activation of mitoKATP channels provides mitochondrial protection and may serve as a potential therapeutic strategy to mitigate HIMS.

Reproducibility of Heart Rate Variability Revealed by Repeated Measurements during and after Hemodialysis

Abstract:Background/aims: Trajectory of heart rate variability (HRV) represents a noninvasive real-time measure of autonomous nervous system (ANS) and carries the capability of providing new insights into the hemodynamic compensation reserve during hemodialysis (HD). However, studies on HRV reproducibility during HD are scarce and did not refer to different reading periods. In this observational study, we aimed to establish the best suited and most reliable and reproducible HRV index in routine HD treatments including different reading rates. Methods: HRV was characterized by standardized mathematical variation expressions of R/R' intervals: SD of all R/R' intervals (ms), square root of the root mean square of the sum of all differences between adjacent R/R' intervals (ms), percentage of consecutive R/R' intervals that differ by >50 ms (%), low-frequency spectral analysis HRV (LF, expressing sympathetic activity), and high-frequency HRV (HF, expressing parasympathetic activity). To compare robustness of these HRV indices during HD procedures, we compared HRV indices means between different HD sessions and controlled for association with clinical parameters. Results: In 72 HD treatments of 34 patients, we detected the highest reproducibility (89%) of HRV measures when analyzing the low-frequency to high-frequency (LF/HF) ratio in long-term (3 h) readings. Long-term LF/HF was able to discriminate -between patients with and without heart failure NYHA classes ≥3 (p = 0.009) and type 2 diabetes (p = 0.023). We were unable to study relationships between ANS and intradialytic complications because they did not appear in our cohort. Short-term readings of HRV indices did not show any significance of pattern change during HD. Conclusion: In summary, our data provide evidence for high robustness of long-term LF/HF in analyzing HRV in HD patients using future automated monitoring systems. For short-term analysis, mathematical real-time analysis must evolve.

Abstract:Objective To evaluate the impacts of single session of hemodialysis on right ventricular(RV) function by three dimensional speckle tracking echocardiography (3DSTE).Methods 3DSTE was performed on 14 MHD patients (≥6 month) with intra dialytic hypotension (IDH) and 14 age-and sex-matched patients without IDH (non-IDH) before and immediately after one hemodialysis session (≤30 min).3DSTE RV volume and systolic function parameters were obtained and compared before and after hemodialysis.Results Compared with non-IDH group,IDH group had lower RV ejection fraction (RVEF) [(45.9±2.6)％ vs (52.0±4.3)％,P＜0.001].After one session of hemodialysis,RV longitudinal strains increased in the non IDH group (all P＜0.05),while RV systolic parameters decreased in the IDH group (all P＜0.01).Conclusions One session of hcmodialysis may have different effects on RV function in non-IDH and IDH patients.3DSTE have important value in the RV function evaluation,follow-up and risk stratification in MHD patients.

Abstract:ABSTRACT Hemodialysis (HD) is a life-sustaining therapy as well as an intermittent and repetitive stress condition for the patient. In ridding the blood of unwanted substances and excess fluid from the blood, the extracorporeal procedure simultaneously induces persistent physiological changes that adversely affect several organs. Dialysis patients experience this systemic stress condition usually thrice weekly and sometimes more frequently depending on the treatment schedule. Dialysis-induced systemic stress results from multifactorial components that include treatment schedule (i.e. modality, treatment time), hemodynamic management (i.e. ultrafiltration, weight loss), intensity of solute fluxes, osmotic and electrolytic shifts and interaction of blood with components of the extracorporeal circuit. Intradialytic morbidity (i.e. hypovolemia, intradialytic hypotension, hypoxia) is the clinical expression of this systemic stress that may act as a disease modifier, resulting in multiorgan injury and long-term morbidity. Thus, while lifesaving, HD exposes the patient to several systemic stressors, both hemodynamic and non-hemodynamic in origin. In addition, a combination of cardiocirculatory stress, greatly conditioned by the switch from hypervolemia to hypovolemia, hypoxemia and electrolyte changes may create pro-arrhythmogenic conditions. Moreover, contact of blood with components of the extracorporeal circuit directly activate circulating cells (i.e. macrophages–monocytes or platelets) and protein systems (i.e. coagulation, complement, contact phase kallikrein–kinin system), leading to induction of pro-inflammatory cytokines and resulting in chronic low-grade inflammation, further contributing to poor outcomes. The multifactorial, repetitive HD-induced stress that globally reduces tissue perfusion and oxygenation could have deleterious long-term consequences on the functionality of vital organs such as heart, brain, liver and kidney. In this article, we summarize the multisystemic pathophysiological consequences of the main circulatory stress factors. Strategies to mitigate their effects to provide more cardioprotective and personalized dialytic therapies are proposed to reduce the systemic burden of HD.

Despite current limitations, on-line kilo-voltage cone-beam computed tomography (CBCT) also has the potential to identify changes in target volume geometry and the location of important organs at risk. These capabilities could be enhanced by further technical improvements in CBCT, including image quality.

Abstract:Background: Myocardial pathology is common in patients undergoing hemodialysis. To explore the effects of differing aspects of dialysis treatment on its evolution, we examined the impact of change in markers of volume status, hemodynamics and solute clearance on left ventricular (LV) parameters in a randomized trial of extended hours dialysis. Methods and Results: A Clinical Trial of IntensiVE (ACTIVE) Dialysis randomized 200 patients undergoing hemodialysis to extended dialysis hours (>= 24 hours/week) or standard hours (12-18 hours/week) for 12 months. In a prespecified substudy, 95 participants underwent cardiac magnetic resonance imaging (CMR) at baseline and at the study's end. Generalized linear regression was used to model the relationship between changes in LV parameters and markers of volume status (normalized ultrafiltration rate and total weekly interdialytic weight gain), hemodynamic changes (systolic and diastolic blood pressure) and solute control (urea clearance, dialysis hours and phosphate). Randomization to extended hours dialysis was not associated with change in any CMR parameter. Reduction in ultrafiltration rate was associated with reduction in LV mass index (P = 0.049) and improved ejection fraction (P = 0.024); reduction in systolic blood pressure was also associated with improvement in ejection fraction (P = 0.045); reduction in interdialytic weight gain was associated with reduced stroke volume (P = 0.038). There were no associations between change in urea clearance, phosphate or total hours per week and CMR parameters. Conclusions: Reduction in ultrafiltration rate and blood pressure are associated with improved myocardial parameters in hemodialysis recipients independently of solute clearance or dialysis time. These findings underscore the importance of fluid status and related parameters as potential treatment targets in this population.
The impacts of the initiation of hemodialysis for patients with newly diagnosed end-stage renal disease and normal left ventricular systolic function

Abstract:Abstract Background Patients with end-stage renal disease (ESRD) are at increased risk of cardiovascular disease, left ventricular dysfunction, and subsequent morbidity and mortality. How the renal replacement therapy would affect the myocardial function and the clinical outcomes remains elucidated. Methods A total of 60 patients with newly diagnosed ESRD and then received hemodialysis were enrolled. The participants would undergo echocardiographic studies before and at 1 year after the initiation of hemodialysis. Left ventricular global longitudinal strain (GLS) was measured. Subjects with a reduced left ventricular ejection fraction (LVEF<50%) were excluded. The study population were stratified according to their post-hemodialytic GLS values. Clinical outcomes, including mortality and heart failure hospitalization were evaluated. Result A total of 47 patients (61.7% men, age 66.6±14.5 years) were included in this study. After hemodialysis, 27 (57.4%) patients had normal GLS, while 20 (42.6%) patients had reduced GLS. The baseline characteristics were similar in each group, except that coronary artery disease (CAD) was significantly more presented in subjects with reduced GLS (p=0.047). Moreover, LV end-systolic volume (LVESV) and right ventricular systolic pressure (RVSP), were significantly higher in those with reduced GLS than the counterpart. During a mean follow-up duration, the Kaplan-Meier survival analysis showed subjects with reduced GLS were associated with higher mortality (p=0.032). The logistic regression analysis, both LV end-systolic volume (LVESV) and right ventricular systolic pressure (RVSP) were identified as independent predictors of reduced LV GLS. After adjusting for age, sex and CAD, RVSP remained a significant predictor of reduced GLS (p=0.037). Conclusion In patients with ESRD, the study has demonstrated the baseline RVSP was associated with post-hemodialytic GLS, which was further correlated with long-term survival. The results may suggest RVSP could potentially reflect the impaired left ventricular systolic function. These findings may contribute to better risk stratification and clinical management in this population.

Evaluation of Left Ventricular Strains in Patients with Maintenance Hemodialysis Using Layer-Specific Two-Dimensional Speckle Tracking Imaging

Abstract:Objective To evaluate the impact of maintenance hemodialysis(MHD) on left ventricular(LV) strains via layer-specific speckle-tracking imaging (LSTI).Methods Twenty-eight MHD patients and 28 matched controls were enrolled in the study.Conventional echocardiography and LSTI were performed on patients before and shortly after one hemodialysis session,as well as on controls.Results LV end-diastolic volume (LVEDV) were markedly reduced after one session of HD [(52.3±22.4)ml/m2 vs (55.0±15.3)ml/m2,P=0.033],but still larger than the control group (47.4±6.4) ml/m2.LV ejection fraction (LVEF) decreased in MHD patients after HD [(62.5±8.1)% vs (64.6±7.3)%,P=0.06],which was similar to those in the control group (66.0±4.2)% and in MHD patient before HD (P=0.391).The three layer circumferential and longitudinal strains decreased in the order of the control group,patients before HD and after HD [CS-endo:(-26.3±3.6)% vs (-22.2±4.7)% vs (-19.2±5.4)%;CS-mid:(-19.0±2.7)% vs (-15.3±3.5)% vs (-13.0±3.8)%;CS-epi:(-11.1±2.3)% vs (-8.0±2.6)% vs (-6.5±2.3)%;LS-endo:(-23.7±3.0)% vs (-18.6±3.6)% vs (-16.4±4.5)%;LS-mid:(-20.9±2.7)% vs (-16.3±3.1)% vs (-14.0±4.1)%;LS-epi:(-18.4±2.4)% vs (-14.1±2.8)% vs (-12.0±3.6)%;all P<0.01],while the synchrony index,standard deviation of times to peak longitudinal strain,increased in these three groups [(3.6±1.0)% vs (6.3±1.5)% vs (7.5±2.2)%,all P<0.01].Linear correlation analysis showed ultrafiltration volume was negatively related with the difference of longitudinal strains before and after HD (difference of LS-endo:r=-0.428,P=0.023;difference of LS-mid:r=-0.423,P=0.025;difference of LS-epi:r=-0.422,P=0.025).Conclusions One session of HD may have injurious effects on LV strains and systolic synchrony.The reduction of ultrafiltration volumes may be helpful to protect cardiomyocytes.
Effect of dialysis modality on left ventricular hypertrophy in patients with end-stage renal disease

Abstract:Objective To investigate the effect of hemodialysis(HD) and peritoneal dialysis(PD) on left ventricular hypertrophy(LVH) in patients with end-stage renal disease,and analyze the associated factors.Methods 48 HD and 62 PD patients receiving treatment for at least 3 months in dialysis center were enrolled.In addition,43 patients with the stage from 3 to 5 chronic kidney diseases(CKD) were also included.All patients underwent blood pressure measurements and echocardiographic examinations.The left ventricular mass index(LVMI) was calculated.Volume status was assessed by bioelectrical impedance analysis.At the same time,biochemistry parameters were also measured.For HD patients,all the following measurements were performed within one hour post-dialysis.In PD and CKD patients,the measurements were performed at a monthly scheduled visit,during the time the abdominal cavity was empty.Results There was no baseline difference in demographics among the three groups.As expected,systolic blood pressure(SBP) in HD patients(156.7±28.4) mmHg was significantly higher than that in PD(147.7±20.6) mmHg(P<0.05) and CKD patients(143.4±21.9) mmHg(P<0.01).Extracellular water(ECW) was lower in HD(14.51±2.51) L than PD(15.67±3.26) L(P<0.05) and CKD patients(15.84±3.27) L(P<0.05).The LVMI was markedly higher in HD patients 137.6±38.5 as compared to PD 116.2±34.5(P<0.01) and CKD patients 104.5±30.0(P<0.01),while there was no significant difference in LVMI between PD and CKD patients.Thus,the prevalence of LVH according to the Framingham criteria was 68.8% in HD patients,45.2% in PD patients,and 32.6% in CKD patients.Finally,in a linear stepwise regression model(r2=0.364,P<0.01),SBP,treatment modality(to be in HD),and ECW were all independent predictors of LVMI.Conclusion In a cross-sectional analysis of patients in dialysis center,HD patients showed a higher LVMI and a higher prevalence of LVH than PD patients.The LVMI was associated with high blood pressure,volume overload and others.

Abstract:We observe that recruitment efforts aimed at alleviating the shortage of skilled workforce in computer engineering must be augmented with strategies for retaining and motivating the students after they have enrolled in our educational programmes. At the University of California, Santa Barbara, we have taken a first step in this direction by offering a required freshman seminar entitled “Ten Puzzling Problems in Computer Engineering”. This one-unit pass/not-pass gateway course, which is graded based solely on attendance, introduces our students to some of the most challenging problems faced by computer engineers in their daily professional endeavors and at the frontiers of research. To accomplish this feat in a manner that is both understandable and appealing to freshmen, the problems are related to popular mathematical and logical puzzles. Each 1-hour class session begins by introducing the students to puzzles of a particular kind and letting them participate in formulating solutions. Historical context, background, and general solution methods for the puzzles are then discussed by the instructor, who finally proceeds to demonstrate how the puzzles and their solution strategies are related to real technical challenges in computer engineering. The new course, which has been offered twice already, is supported by a website containing complete lecture slides, class handouts, and reference information.
