Abstract:AIM:To analyse the alterations of protein expression in the kidney of spontaneously hypertensive rat with losartan.METHODS:The proteins of the kidney were isolated by two-dimensional gel electrophoresis. The protein spots with significant changes were selected for further identification by LC-MS/MS.RESULTS:The number of average protein spots of two groups was 570 +/- 48 and 686 +/- 30 respectively. Compared with the SHR, 13 spots had changed significantly after treated with losartan. There were 5 protein spots detected only in SHR group, while 4 up-regulated and 4 down-regulated protein spots were detected in SHR-L group. These differentially expressed proteins were detected by mass spectrometry. 7 spots were identified. There were Heat shock protein (Hsp), Tubulin alpha-1 chain, Transthyretin precursor, Liver regeneration-related protein LRRG03, Ezrin-radixin-moesin binding phosphoprotein 50, Phosphoglycerate kinase 1 and Anionic trypsin I precursor.CONCLUSION:The different protein spots expression may play important roles in Losartan's effective protection to hypertension rats renal tissue.
Diminished antiproteinuric effect of the angiotensin receptor blocker losartan during high potassium intake in patients with CKD

Abstract:Abstract Background Angiotensin II type 1 receptor blockers (ARBs) lower blood pressure (BP) and proteinuria and reduce renal disease progression in many—but not all—patients. Reduction of dietary sodium intake improves these effects of ARBs. Dietary potassium intake affects BP and proteinuria. We set out to address the effect of potassium intake on BP and proteinuria response to losartan in non-diabetic proteinuric chronic kidney disease (CKD) patients. Methods We performed a post hoc analysis of a placebo-controlled interventional cross-over study in 33 non-diabetic proteinuric patients (baseline mean arterial pressure and proteinuria: 105 mmHg and 3.8 g/day, respectively). Patients were treated for 6 weeks with placebo, losartan and losartan/hydrochlorothiazide (HCT), combined with a habitual (∼200 mmol/day) and low-sodium (LS) diet (<100 mmol/day), in randomized order. To analyse the effects of potassium intake, we categorized patients based on median split of 24-h urinary potassium excretion, reflecting potassium intake. Results Mean potassium intake was stable during all six treatment periods. Losartan and losartan/HCT lowered BP and proteinuria in all treatment groups. Patients with high potassium intake showed no difference in the BP effects compared with patients with low potassium intake. The antiproteinuric response to losartan monotherapy and losartan combined with HCT during the habitual sodium diet was significantly diminished in patients with high potassium intake (20% versus 41%, P = 0.011; and 48% versus 64%, P = 0.036). These differences in antiproteinuric response abolished when shifting to the LS diet. Conclusions In proteinuric CKD patients, the proteinuria, but not BP-lowering response to losartan during a habitual high-sodium diet was hampered during high potassium intake. Differences disappeared after sodium status change by LS diet.

Abstract:Abstract Background Studies have shown that losartan reduces serum uric acid in adults, unlike angiotensin-converting enzyme inhibitors. A previous study demonstrated that losartan and enalapril had comparable effects on proteinuria in children. Methods We conducted a post hoc analysis of results from a prospective trial in which the proteinuria-reducing effects of losartan and enalapril were compared. We have now evaluated (a) the effects of these medications on SUA in 248 children with proteinuria and (b) the correlation between changes in SUA and eGFR. Results SUA levels after 36 months were found to be increased when compared to baseline in both losartan and enalapril groups. The mean change in SUA from baseline was significantly different at 12 months between 23 hypertensive patients randomised to losartan (3.69% decrease [95% CI 11.31%, 3.93%]) and 24 randomised to enalapril (12.57% increase [95% CI 3.72%, 21.41%]), p = 0.007. This significant difference remained after 24, 30 and 36 months but was observed in the entire group of 248 patients only at 12 months. There was a statistically significant negative correlation between changes in SUA and changes in eGFR at each time point over 36 months. Conclusions Losartan may have long-term beneficial effects on SUA and eGFR in children with proteinuria.

Acei/Arb Therapy for Iga Nephropathy: A Meta Analysis of Randomised Controlled Trials

Abstract:BACKGROUND:Published reports examining the efficacy of RAS blockers: angiotensin converting-enzyme inhibitor (ACEI) and angiotensin II receptor blocker (ARB) agents for preserving renal function in IgA nephropathy (IgAN) have yielded conflicting results. To evaluate systematically the effects of ACEI/ARB agents on IgAN, we conducted a meta analysis of published randomised controlled trials (RCTs).METHODS:MEDLINE, EMBASE, the Cochrane Library and article reference lists were searched for RCTs that compared ACEI/ARB with placebo and any other antihypertensive agents or non-immunosuppressive agents for treating IgAN. The quality of the studies was evaluated with the method of intention to treat analysis and allocation concealment, as well as with the Jadad method. Meta analyses were performed on the outcomes of proteinuria and renal function in patients with IgAN.RESULTS:Eleven RCTs involving 585 patients were included in the review. Seven trials used placebo/no treatment as controls. Four trials used other antihypertensive agents as controls. Overall, ACEI/ARB agents had statistically significant effects on protecting renal function(p < 0.00001) and reduction of proteinuria (p < 0.00001) when compared with control group. Tests for heterogeneity showed no difference in effect among the studies. Systolic and diastolic blood pressure, glomerular filtration rate (GFR), age, did not influence treatment response. ACEI/ARB agents were well tolerated.CONCLUSIONS:The current cumulative evidence suggests that ACEI/ARB agents had statistically significant effects on protecting renal function and reduction of proteinuria in patients with IgAN when compared with control groups. ACEI/ ARB agents are a promising medication and should be investigated further.
Add-on angiotensin II receptor blockade lowers urinary transforming growth factor-beta levels

Abstract:Progression of renal failure, despite renoprotection with angiotensin-converting enzyme (ACE) inhibitors in patients with proteinuric nephropathies, may be caused by persistent renal production of transforming growth factor-beta1 (TGF-beta1) through the angiotensin II subtype 1 (AT1) receptors. We tested the hypothesis that AT1-receptor blocker therapy added to a background of chronic maximal ACE inhibitor therapy will result in a reduction in urinary TGF-beta1 levels in such patients. Sixteen patients completed a two-period, crossover, randomized, controlled trial, details of which have been previously reported. All patients were administered lisinopril, 40 mg/d, with either losartan, 50 mg/d, or placebo. Blood pressure (BP) was measured using a 24-hour ambulatory BP monitor. Overnight specimens of urine were analyzed for urine TGF-beta1, protein, and creatinine concentrations. Mean age of the study population was 53 +/- 9 (SD) years; body mass index, 38 +/- 5.7 kg/m2; seated BP, 156 +/- 18/88 +/- 12 mm Hg; and urine protein excretion, 3.6 +/- 0.71 g/g of creatinine. Twelve patients had diabetic nephropathy, and the remainder had chronic glomerulonephritis. At baseline, urinary TGF-beta1 levels were significantly increased in the study population compared with healthy controls (13.2 +/- 1.2 versus 1.7 +/- 1.1 ng/g creatinine; P < 0.001). There was a strong correlation between baseline urine protein excretion and urinary TGF-beta1 level (r2 = 0.53; P = 0.001), as well as systolic BP and urinary TGF-beta1 level (r2 = 0.57; P < 0.001). After 4 weeks of add-on losartan therapy, there was a 38% (95% confidence interval [CI], 16% to 55%) decline in urinary TGF-beta1 levels (13.3 [95% CI, 11.4 to 15.5] to 8.2 pg/mg creatinine [95% CI, 6.2 to 10.7]). The reduction in urinary TGF-beta1 levels occurred independent of changes in mean urinary protein excretion or BP. Thus, proteinuric patients with renal failure, despite maximal ACE inhibition, had increased urinary levels of TGF-beta1 that improved over 1 month of add-on therapy with losartan. We speculate that dual blockade with losartan and an ACE inhibitor may provide additional renoprotection by decreasing renal production of TGF-beta1.
Protocols to eliminate tuning penalties for packet-switched WDM star networks with large tuning latency

Abstract:15 Abstract. The use of low-speed tunable devices may lead to throughput and delay penalties for packet-switched WDM star networks. When the tuning latency is significant, conventional cost-effective solutions result in significant throughput and delay penalties. Our approach: (i) combines and unifies existing approaches to improve performance by pipelining multiple tunable filters per node and making a TDM schedule with maximum time gap for reception; (ii) introduces 'superpackets' (multiple packets with the same source and destination), thereby relaxing the impact of large tuning latency. Since the superpackets increase throughput at the cost of increased delay, we determine the minimum delay, under a constraint of no throughput penalty. A discrete-time queueing model of the protocol shows that a system with two tunable filters per node has low delay and is wavelength-efficient for uniform traffic.
The effect of a complex training and detraining programme on selected strength and power variables in early pubertal boys

Abstract:Abstract Complex training, a combination of resistance training and plyometrics is growing in popularity, despite limited support for its efficacy. In pre- and early pubertal children, the study of complex training has been limited, and to our knowledge an examination of its effect on anaerobic performance characteristics of the upper and lower body has not been undertaken. Furthermore, the effect of detraining after complex training requires clarification. The physical characteristics (mean±s) of the 54 male participants in the present study were as follows: age 12.3 ± 0.3 years, height 1.57 ± 0.07 m, body mass 50.3 ± 11.0 kg. Participants were randomly assigned to an experimental (n = 33) or control group (n = 21). The training, which was performed three times a week for 12 weeks, included a combination of dynamic constant external resistance and plyometrics. After training, participants completed 12 weeks of detraining. At baseline, after training and after detraining, peak and mean anaerobic power, dynamic strength and athletic performance were assessed. Twenty-six participants completed the training and none reported any training-related injury. Complex training was associated with small increases (≤5.5%) in peak and mean power during training, followed by decreases of a similar magnitude (≤ −5.9%) during detraining (P < 0.05). No changes or minor, progressive increases (≤1.5%) were evident in the control group (P > 0.05). In the experimental group, dynamic strength was increased by 24.3 – 71.4% (dependent on muscle group; P < 0.01), whereas growth-related changes in the control group varied from 0 to 4.4% (P > 0.05). For 40-m sprint running, basketball chest pass and vertical jump test performance, the experimental group saw a small improvement (≤4.0%) after training followed by a decline (≤ −4.4%) towards baseline during detraining (P < 0.05), whereas the control group experienced no change (P > 0.05). In conclusion, in pre- and early pubertal boys, upper and lower body complex training is a time-effective and safe training modality that confers small improvements in anaerobic power and jumping, throwing and sprinting performance, and marked improvements in dynamic strength. However, after detraining, the benefits of complex training are lost at similar rates to other training modalities.
Renal protective effects and mechanism of losartan in young rats with glomerulosclerosis

Abstract:Objectives Clinical and experimental studies indicate that angiotensin II (AT II) plays a pathophysiological role in the progression of chronic renal diseases. Angiotensin converting enzyme inhibitor (ACEI) therapy is now widely regarded as a fundamental component of strategies designed to retard the progression of chronic renal diseases. In diabetic patients,ACEI resulted in greater renal protective effects in randomized trials. AT II type-1 (AT1) receptor antagonists (RA),a novel class of renin- angiotensin system inhibitors,inhibit the effects of AT II mediated by AT1 receptors, through which they show protective effects predominantly on glomerulosclerosis. Most of these findings came through the studies in adult animals and humans. However,the renal protective effect of AT1 RA has not,so far,been extensively stu- died in young animals or children and the precise mechanism of this effect remains unclear. The purpose of this study was to observe protective effects of losartan using a model of the focal and segmental glomerulosclerosis in young rats and to find out the mechanism regulating the accumulation of extracellular matrix (ECM). Methods All experiments were performed on 17 male SD rats (one- month- old,weighing 100 g or so) in three groups: control (n = 5),model (n = 6) and losartan-treated (n = 6). Control group were subjected to sham operation and were injected with normal saline after one week via the tail vein. Glomerulosclerosis in young rats was established by means of uninephrectomy and adriamycin (5 mg/kg) administration. After uninephrectomy,losartan- reated group were treated with losartan 5 mg/(kg·d) for 12 weeks. The levels of urinary protein excretion,serum total protein,albumin,serum creatinine,blood urea nitrogen and total cholesterol were measured at regular intervals. The expression of transforming growth factor- β1 (TGF- β1) in the glomeruli was detected by streptavidin/peroxidase staining. The renal pathological changes of the remnant kidney were investigated at the end of experiment. Results Compared to model group losartan showed effect on reducing the proteinuria at 9 and 12 weeks urinary protein excretion (2.41,1.97) mg/24h vs. (17.28,7.39) mg/24h (P < 0.01) and (2.96,3.54) mg/24h vs. (14.20,0.55) mg/24h (P < 0.01),respectively?. A mild mesangial proliferation at the 12th week after treatment [mesangial expansion rate (3.45,3.36) % vs. (35.40,40.28) %,P < 0.05],glomerulosclerosis [sclerosis index (0.25,0.13) vs. (1.86, 2.61),P < 0.05] and less expression of TGF- β1 [optical density value (0.143 ± 0.023) vs. (0.221 ± 0.036),P < 0.01?] in the injured glomeruli were found in losartan- treated rats. Conclusions These results suggest that losartan show renoprotective effects in reducing proteinuria,mesangial proliferation and glomerular sclerosis in a young rat model with glomerulosclerosis. The lowered expression of TGF- β1 after losartan treatment in the injured glomeruli might be partially responsible for this beneficial effect on accumulation of ECM .

Abstract:The renin-angiotensin-aldosterone (RAAS) system plays a significant role in renal and cardiovascular pathophysiology, since its increased activity is involved in arterial hypertension, heart failure, and kidney disease. ACEIs and ARBs are essential drugs for nephroprotection: they reduce blood pressure values and albuminuria, both related to cardiovascular damage and CKD progression. The nephroprotective effects are evident in both diabetes mellitus and non-diabetic renal disease, and the initial eGFR fall, if not more than 30%, should be considered as a marker of long-term success of renal protection. To optimize the RAAS inhibition salt intake should be strictly controlled, moreover the effective antiproteinuric dose can often be higher than that used as an antihypertensive. In selected and closely monitored cases, it is also possible to consider dual RAAS blockade. Finally, it should be noted that in patients with advanced CKD RAAS inhibition should not be discontinued, either because it does not give any benefit on GFR or because it increases cardiovascular risk.
Impact of irbesartan, an angiotensin receptor blocker, on uric acid level and oxidative stress in high-risk hypertension patients

Abstract:Hyperuricemia is a known cardiovascular risk factor. The angiotensin II receptor blocker (ARB) losartan is known to decrease serum uric acid (UA) level. A recent in vitro study demonstrated a strong interaction between irbesartan and UA transporters that exceeded that of losartan. The purpose of the present study was to evaluate the hypouricemic effect of irbesartan in a clinical setting. A total of 40 high-risk hypertensive outpatients with coronary artery disease, cerebrovascular disease and/or diabetes complications who were taking ARBs other than irbesartan and losartan were enrolled in this study. After a 4-week control period, the patients’ prescribed ARBs were exchanged for an equivalent dose of irbesartan. We assessed blood pressure, heart rate, serum UA level, parameters of lipid and glucose metabolism, cardiac and renal function and inflammatory and oxidative stress markers in blood samples taken immediately before the initiation of irbesartan treatment and again after 12 weeks of treatment. All 40 recruited patients were followed (31 men and 9 women, mean age: 68 years) without any dropouts. During the 12 weeks of irbesartan treatment, no significant changes in blood pressure, heart rate, parameters of lipid or glucose metabolism or other biomarkers of cardiac function, renal function, or inflammation were observed. However, UA level (5.9±1.6 to 5.5±1.6 mg ml−1, P=0.028) and the oxidative stress marker derivative reactive oxygen metabolites (dROMs) (354±83 to 310±65 U.CARR, P<0.001) were significantly lower at 12 weeks of treatment compared with before treatment. These results suggest that irbesartan has beneficial effects on hyperuricemia and oxidative stress.

The angiotensin II type 1 receptor blocker olmesartan preferentially improves nocturnal hypertension and proteinuria in chronic kidney disease

Abstract:Accumulated evidence suggests that an altered ambulatory blood pressure (BP) profile, particularly elevated nighttime BP, reflects target organ injury and is a better predictor of further cardiorenal risk than the clinic BP or daytime BP in hypertensive patients complicated by chronic kidney disease (CKD). In this study, we examined the beneficial effects of olmesartan, an angiotensin II type 1 receptor blocker (ARB), on ambulatory BP profiles and renal function in hypertensive CKD patients. Forty-six patients were randomly assigned to the olmesartan add-on group (n=23) or the non-ARB group (n=23). At baseline and after the 16-week treatment period, ambulatory BP monitoring was performed and renal function parameter measurements were collected. Although the baseline clinic BP levels and the after-treatment/baseline (A/B) ratios of clinic BP levels were similar in the olmesartan add-on and non-ARB groups, the A/B ratios of ambulatory 24-h and nighttime BP levels in the olmesartan add-on group were significantly lower. Furthermore, the A/B ratios of urinary protein, albumin and type IV collagen excretion in the olmesartan add-on group were significantly lower than those in the non-ARB group (urinary protein excretion, 0.72±0.41 vs. 1.45±1.48, P=0.030; urinary albumin excretion, 0.73±0.37 vs. 1.50±1.37, P=0.005; urinary type IV collagen excretion, 0.87±0.42 vs. 1.48±0.87, P=0.014) despite comparable A/B ratios for the estimated glomerular filtration rate in the two groups. These results indicate that in hypertensive patients with CKD, olmesartan add-on therapy improves the ambulatory BP profile via a preferential reduction in nighttime BP with concomitant renal injury inhibition.

Abstract:Objective: Angiotensin receptor blockers (ARB) are among the recommended first-line treatment options in patients with hypertension and chronic kidney disease (CKD). This meta-analysis evaluated the effect of ARB on blood pressure (BP) and renal function in patients with concomitant hypertension and CKD with or without diabetes.Methods: Literature search was performed in PubMed/MEDLINE, EMBASE and BIOSIS to identify parallel-group, randomized controlled trials (≥8 weeks) reporting the effects of ARB on office systolic/diastolic BP (SBP/DBP), estimated glomerular filtration rate (eGFR), serum creatinine (SCr), creatinine clearance (CrCl) or proteinuria in adults with hypertension and CKD. Mean difference (MD, generic inverse variance) with 95% confidence intervals (CIs) was used to report an outcome.Results: Among the 24 studies identified, 19 evaluated ARB as monotherapy, 4 evaluated ARB as combination therapy and one evaluated ARB both as monotherapy and combination therapy. Median (range) duration of the studies was 12 (1.84–54.0) months. ARB monotherapy significantly (p < 0.01) reduced BP (treatment ≥1 year: SBP [MD: −14.84 mmHg; 95% CI: −17.82 to −11.85]/DBP [−10.27 mmHg; −12.26 to −8.27]) and proteinuria (≥1 year [−0.90 g/L; −1.22 to −0.59]). Results were consistent for combination therapy. In these studies, non-significant changes were observed for eGFR, CrCl and SCr. The impact of SBP changes on eGFR was not significant; however, studies were of a relatively short duration.Conclusion: ARB had a favorable impact on BP and renal parameters such as proteinuria with monotherapy as well as with combination therapy, highlighting their potential benefits in patients with hypertension and CKD. During the short follow-up of these studies, no significant change in eGFR was observed.

Antiproteinuric effects of angiotensin receptor blockers: telmisartan versus valsartan in hypertensive patients with type 2 diabetes mellitus and overt nephropathy

Abstract:Background: Renin-angiotensin system blockade reduces proteinuria and prevents nephropathy progression in patients with type 2 diabetes mellitus (T2D). Experimental evidence demonstrates that angiotensin receptor blockers (ARBs) possess anti-inflammatory potential, which might contribute to reducing proteinuria and providing renoprotection. Methods: We conducted a multicentre, double-blind, prospective, parallel-group non-inferiority study of 885 hypertensive [systolic blood pressure/diastolic blood pressure (SBP/DBP) >130/80 mmHg] patients with T2D, proteinuria (> or =900 mg/24 h) and serum creatinine (< or =3.0 mg/dl) who were randomized to once-daily telmisartan 80 mg or valsartan 160 mg; additional antihypertensive therapy was permitted. The primary endpoint was the change from baseline in the 24-h proteinuria after 12 months. Secondary endpoints included changes in 24-h albuminuria, estimated glomerular filtration rate (eGFR) and inflammatory parameters asymmetrical dimethylarginine (ADMA), high-sensitivity C-reactive protein (CRP) and urinary 8-iso-prostaglandin F(2alpha) (8-iso-PGF(2alpha)). Results: Telmisartan and valsartan produced comparable reductions in 24-h urinary protein excretion rates: geometric mean reduction (95% confidence interval) [telmisartan, 33% (27-39%); valsartan, 33% (27-38%)]. No significant differences between treatments were seen in changes from baseline in 24-h urinary albumin excretion rate and eGFR at 12 months. With both treatments, greater renoprotection was seen among patients with better blood pressure control. No significant changes in ADMA or CRP were noted in either group after 12 months, but urinary 8-iso-PGF(2alpha) levels decreased by 14% with telmisartan and by 7% with valsartan (P = 0.040). Conclusions: In patients with T2D, hypertension and overt nephropathy, the renoprotection afforded by telmisartan and valsartan appears similar, and the study was unable to show any effect beyond that due to blood pressure control. At doses used to treat hypertension, there is no evidence of inflammatory parameters being modified by ARBs in patients with more advanced kidney disease due to T2D.
Podocyte-Related Mechanisms Underlying Survival Benefit of Long-Term Angiotensin Receptor Blocker

Abstract:We previously found that short-term treatment (week 8 to 12 after injury) with high-dose angiotensin receptor blocker (ARB) induced the regression of existing glomerulosclerosis in 5/6 nephrectomy rats. We therefore assessed the effects of long-term intervention with ARB vs. nonspecific antihypertensives in this study. Adult rats underwent 5/6 nephrectomy and renal biopsy 8 weeks later. The rats were then divided into three groups with equivalent renal function and glomerular sclerosis and treated with high-dose losartan (ARB), nonspecific antihypertensive triple-therapy (TRX), or left untreated (Control) until week 30. We found that blood pressure, serum creatinine levels, and glomerulosclerosis were lower at sacrifice in ARB and TRX vs. Control. Only ARB reduced proteinuria and maintained the density of WT-1-positive podocytes. Glomerular tufts showed more double-positive cells for CD44, a marker of activated parietal epithelial cells, and synaptopodin after ARB vs. TRX or Control. ARB treatment reduced aldosterone levels. ARB-treated rats had significantly improved survival when compared with TRX or Control. We conclude that both long-term ARB and triple-therapy ameliorate progression, but do not sustain the regression of glomerulosclerosis. ARB resulted in the superior preservation of podocyte integrity and decreased proteinuria and aldosterone, linked to increased survival in the uremic environment.

Abstract:Objective: The pathogenesis of IgA nephropathy (IgAN) is still unknown. Combination therapy with angiotensin-converting enzyme inhibitors (ACEIs) plus angiotensin receptor blockers (ARBs) might provide more benefits to IgAN patients. We conducted a systematic review to assess the efficacy of combination therapy for IgAN. Methods: The MEDLINE, EMBASE, the Cochrane Library and article reference lists were searched for randomised clinical trials (RCTs) which involved combination therapy ACEI plus ARB in only one arm. A meta-analysis was performed on the outcomes of proteinuria and renal function in IgAN patients. Results: Six RCTs involving 109 patients were included in the review. Combined treatment with ACEI plus ARB was more effective than with ACEI/ARB alone for reducing daily proteinuria. This did not translate into an improvement in GFR. Patients receiving ACEI plus ARB therapy did not have an increased risk of hyperkalemia. Conclusions: The current cumulative evidence suggests that combination therapy ACEI plus ARB may provide more benefits to IgAN patients for reducing daily proteinuria. Long-term effects of these agents on renal outcomes, and safety need to be established.
Effect of experimental renal failure on the pharmacokinetics of losartan in rats

Abstract:The purpose of this investigation was to determine whether the pharmacokinetics of the angiotensin II receptor antagonist losartan is altered in renal failure. Male Wistar rats were pretreated with uranyl nitrate or subjected to bilateral ureteral ligation to produce acute renal failure (ARF). Saline-injected and sham-operated rats, respectively, served as controls. Uranyl nitrate-treated rats showed significantly higher serum concentrations of losartan after oral administration and the area under the serum concentration-time curve (AUC(0-24)) of losartan increased about 3-fold compared to control rats. The systemic clearance of losartan significantly decreased from 410 +/- 254ml/h/kg in control to 177 +/- 112ml/h/kg in uranyl nitrate-treated rats. In order to investigate the mechanisms of reduced clearance of losartan associated with ARF, a hepatic microsome fraction was prepared from normal and ARF rats. No significant difference was found in the metabolism of losartan by hepatic microsomes prepared from ARF and control rats. In addition, the metabolic activity of microsomes was examined in the presence of uremic rat serum. The unbound clearance of losartan and the unbound clearance associated with the formation of EXP3174 in the presence of uremic serum were significantly lower than those in the presence of control serum. Furthermore, the metabolism of losartan was inhibited by indoxyl sulfate, a uremic toxin, in an uncompetitive manner. These results suggest that ARF is associated with reduced clearance of losartan due to the inhibition of hepatic metabolism by accumulated uremic toxin(s).

Abstract:Objective　To investigate the effects of angiotensin converting enzyme inhibitor (ACEI) and angiotensin II receptor antagonist (ATRA) on atrial natriuretic factor (ANF) and renin-angiotensin system (RAS) in patients with chronic renal failure (CRF).　Methods　The plasma angiotensin Ⅰ (AI), angiotensin Ⅱ (AII) and ANF levels were determined in 30 healthy volunteers and 40 patients with CRF after administration of losartan (Los) 50 mg once daily or fosinopril (Fos) 10 mg once daily in 0、4、8 and 12 weeks. Sit diastolic blood pressure (SiDBP) and blood urea nitrogen (BUN) and serum creatinine (SCr) were also measured at the same time. The data were analyzed by statistical software SPSS 7.0 .　Results　After 12 weeks, the levels of SiDBP were significantly decreased in both Los and Fos groups. In Fos group, aII was significantly inhibited, ANF and Scr were significantly increased (P<0.01). In Los group, aII was significantly increased (P<0.01), ANF reduced to normal level after it was increased at fourth week. SCr didn't increase.　Conclusion　There are significant effects of ACEI on intrarenal perfusion pressure. The change in ANF level may be used as a parameter for the effects. There are less effects of ATRA on intrarenal perfusion pressure than ACEI.

Abstract:The optimal cooling schedule for simulated annealing is formulated to derive a differential equation for the time-dependent temperature T(t). Based on this equation, the long-term behavior of T(t), entropy production, and the Kullback-Leibler entropy are studied. For some simple examples, such as a many-level system and the small scale traveling salesman problem, the explicit time dependence of the temperature is obtained. Some comments are given on simulated annealing based on Tsallis statistics.

Abstract:Objective:To investigate the effect of angiotensin converting enzyme inhibitor(ACEI),angiotensin Ⅱ receptor blocker(ARB) and combined treatment on general and intrarenal renin-angiotensin system(RAS) in pants with chronic kidney disease(CKD). Methodology:Twenty-four CKD patients were randomly assigned to benazepril,valsartan or conmbined treatment group.During 8 weeks of benazepril treatment (20 mg/d),valsartan treatment (160 mg/d) or conmbined treatment (benazepril 10 mg/d and valsartan 80 mg/d),the blood pressure,serum creatinine,and proteinuria were detected, and the plasma renin activity (PRA),plasma and urinary angiotensinogen(AGT),angiotensin Ⅱ(Ang Ⅱ) and aldosterone were measured by RIA or ELISA. Results:After treatment of 8 weeks,the level of proteinuria [(0.61±0.25)g/24h vs (0.35±0.20)g/24h,P<0.05] and urinary AGT [(60.76±28.05)ng/(mg·Cr) vs (23.09±14.74)ng/(mg·Cr),P<0.05] was lower in benazepril group than that in baseline.In valsartan group, mean artery pressure was lower than baseline [(99.17±10.56)mmHg vs (84.63±9.33)mmHg,P<0.05] and PRA was higher than baseline[(1.33±0.76)ng/(ml·h) vs (6.02±2.59)ng/(ml·h),P<0.01].The primary end point was 87.5% in benazepril group,12.5% in valsartan group and 62.5% in combined treatment group.Compared with valsartan,the primary end point was reduced by benazepril (P<0.05). After 8 weeks treatment,PRA[(3.20±1.25)ng/(ml·h) vs (6.02±2.59)ng/(ml·h),P<0.05] and plasma Ang Ⅱ [(53.32±11.13)pg/ml vs (105.61±59.49)pg/ml,P<0.05] of benazepril group were lower than those of valsartan group. Conclusion:Short-term treatment with benazepril can reduce intrarenal agiotensin Ⅱ activity and urinary protein in patients with CKD. Valsartan may elevate plasma renin activity and angiotensin Ⅱ more significantly than benazepril.
