Abstract:Objective Clinial analysis of tumor induced osteomalacia (TIO) complicated with deep vein thrombosis (DVT) and pulmonary embolism (PE) cases in China to improve the clinicians' recognition of this rare disease and its severe complications.Methods Two cases of TIO complicated with DVT and PE were reviewed and summarized.Results The two patients (one male and one female),middle-aged onset,suffered from TIO more than 2 years.Clinical manifestations included diffuse bone pain,muscle weakness,difficulty in walking.They demonstrated hypophosphatemia,elevated serum alkaline phosphatase and reduced tubular maximum for phosphorus/glomerular filtration rate.Low bone density and the responsible tumors were discovered by imaging examinations.The diagnosis of TIO was clear.Both patients had elevated D-dimer levels and the possibility of PE revealed by the Wells score.They were confirmed with DVT and PE by ultrasonography and CT pulmonary artery angiography (CTPA).They received anticoagulant therapy,preoperative placement of inferior vena cava filter and resection of the responsible tumor.TIO was cured and there was no recurrent thrombosis during the follow up.One patient with vitamin D deficiency osteomalacia complicated with DVT and fatal PE had been reported.Analysis of the three patients indicated that the risk factors for venous thromboembolism were immobilization,fractures and age over 40 yeats.The severity of PE varied widely.Some might be asymptomatic or just have slight chest tightness and shortness of breath,while others could present as acute respiratory and circulatory arrest.Conclusion TIO concomitant with DVT and PE constitutes a complicated,severe and potentially fatal clinical situation.The combination of clinical probability scores and D-dimer testing are helpful to in the diagnosis of VTE screening,while ultrasonography and CTPA can confirm the diagnosis VTE.Early recognition,appropriate treatment and multidisciplinary collaboration are essential to improve the patients' outcome.

Abstract:Trousseau syndrome (TS) is a malignant tumor-mediated complication of the hypercoagulable state with an unknown etiology. Laboratory testing results in patients with TS have indicated elevated D-dimer levels. The imaging analysis in patients who had undergone stroke has shown the presence of several cerebral infarction lesions in multiple regions. Since patients have had malignant tumors for a long time when they suffer from a secondary stroke, the optimum time for radical tumor treatment is usually missed. This study reports a case to improve the early screening and detection of TS and reduce the risk of recurrence of cerebral infarction.

Unusual Presentation of Thoracic Chordoma with Spinal Epidural Hematoma: A Rare Case Report and PRISMA-Driven Systematic Review

Abstract:A chordoma is a slow growing, locally invasive, low-grade tumor belonging to the sarcoma family. It mainly affects the sacrum and skull base. We present a case of thoracic chordoma initially presented with epidural hematoma (EDH), which is a rare clinical entity. We reported this case, and also performed a PRISMA-driven systematic review to summary the similar cases in the literature. This review includes the clinical characteristics and outcome of thoracic chordoma. Our case involves a 60-year-old male who, despite no history of trauma, presented with acute paraparesis. An epidural hematoma was identified at T6 level, leading to a surgical intervention involving T4-6 laminectomy and fixation. Six months subsequent to surgery, the patient experienced progressive lower limb weakness and spasticity. Computed tomography (CT) exhibited erosion of T6 and an associated aggressive mass. Magnetic resonance imaging (MRI) revealed a large heterogenous soft tissue mass arising from the vertebral body and right pedicle of D6, protruding in the epidural space and compressing the spinal cord focally at this level. The mass measured approximately 5 × 4 × 3.5 cm. Magnetic resonance myelography indicated a filling defect at T5-6 level, confirming the intraspinal location of the soft tissue lesion. Complete excision of the mass confirmed the diagnosis of thoracic chordoma. Postoperative follow-up demonstrated notable improvement in the lower limb spasticity and paraparesis, and the patient started adjuvant radiotherapy. This case underscores the importance of maintaining a high index of suspicion when evaluating presentations resembling EDH.

Abstract:Trousseau Syndrome (TS) is defined as the occurrence of thromboembolic events prior to or simultaneously with the diagnosis of visceral neoplasia. In cases of multiple thromboembolisms, considering the possibility of TS, a screening for neoplasms may be warranted. We present a case study of a 61-year-old female who presented a neurological deficit. Brain magnetic resonance imaging (MRI) showed multiple hyperintense bihemispheric foci in subcortical and cortical regions involving three different vascular territories in the FLAIR sequence, associated with restricted diffusion inferring cytotoxic edema and indicating that they were all recent ischemic lesions, raising the hypothesis of TS. The patient underwent neoplastic screening with a subsequent diagnosis of colon cancer. TS should be considered when the patient presents thromboembolic events without an established cause. The three-territories-sign (TTS) is an essential radiographic biomarker related to cancer-associated ischemic stroke (CAIS). We propose that our findings be considered for the inclusion of guidelines that determine the investigation of an occult tumor (particularly gastric, pancreatic, lung, and colorectal) in patients who present thrombotic events, especially TTS.

Abstract:Trousseau syndrome (TS) is a type of paraneoplastic syndrome, which is characterized by abnormal activation of the blood clotting system in the body, resulting in thromboembolism. Mucin-producing adenomas (e.g., gastric, lung, pancreatic, and ovarian cancers) are more likely to cause TS. [ 1 , 2 ] The diagnosis of TS is based on the presence of multifocal diffusion-restricted lesions on Magnetic Resonance Imaging (MRI) and a hypercoagulable state suggested by elevated D-dimer levels during the acute phase. and hypercoagulability suggested by elevated D-dimer levels in the acute phase. In the case of TS, malignancy activates the patient's coagulation system, leading to systemic thrombosis and cerebral infarction. [ 3 , 4 ] The incidence of TS is low, the clinical manifestations are diverse, and the current therapeutic regimen continues to follow the principles of anticoagulation with low molecular heparin and symptomatic treatment, [ 5 ] but the prognosis is mostly poor. In this article, we report a case of a patient with small bowel malignancy combined with acute cerebral infarction, which may help to improve the accurate understanding of the disease and increase the possibility of early intervention for treatment, thus prolonging patient survival.

Abstract:Chordomas are rare but locally aggressive cancer, which originate from primitive notochord remnants. Guidelines have recently been proposed to include the option of choosing chordomas in different locations. However, there is still a great challenge in the modern management of chordomas, primarily due to the high recurrence rate and poor prognosis. On this basis, there is a high demand for new therapeutic approaches and reliable prognostic factors. Recent progress in studying the molecular basis of this specific type of cancer has deepened the understanding of this mechanism, which overall facilitates the discovery of specific biomarkers or indicators of the disease. It also gives rise to potential targeted therapies against chordomas as evidenced by the fact that some RTK inhibitors in a clinical context have been evaluated in relation to chordomas. This article summarizes these achievements including the studies relative to pathogenesis, prognostic factors, and targeted therapies for chordomas. The theme of existing problems is also mentioned, which would facilitate general future efforts in this field.

Abstract:Purpose of review Chordoma is an exceedingly rare subtype of bone sarcoma. This review aims to provide a comprehensive insight into chordoma epidemiology, and an update on the recent advances in disease, biology and medical therapies. Recent findings The incidence of chordoma is approximately 0.08/100 000 and the 5-year overall age-adjusted relative survival is 72% in the United States and 61% in Europe. Over the last years, significant steps forwards have been done in the comprehension of chordoma complexity, with insights gained into the biology and morphology of this disease. New entities have been described and potentially druggable molecular targets identified. This is becoming all the more relevant today, as new potentially active agents are under development. Summary Chordoma is a complex disease because of its rarity, biological heterogeneity and peculiar clinical behaviour. Despite the progress done, the outcome in this disease remains unsatisfactory and the identification of active systemic treatments remains an urgent, unmet medical need.

Ovarian clear cell cancer associated with Trousseau syndrome: A case report and literature review

Abstract:Rationale: Trousseau syndrome is known as patients with tumors have a combination of multiple thrombi with systemic organ infarction. The incidence of ovarian clear cell cancer associated with Trousseau syndrome is relatively low. We report the case of a 41-year-old woman diagnosed with ovarian clear cell cancer with deep vein thrombosis and cerebral infarction who had a poor prognosis. Patient concerns: A 41-year-old woman was brought to our hospital with abdominal pain. Abdominal computerized tomography scan suggested large mass of ovarian origin which was considered an ovarian tumor with pelvic metastasis and peritoneal metastasis. Laboratory analyses indicated an elevated levels of serum tumor marker carbohydrate antigen 125 was 321.9 U/mL and the level of D-dimer was 16.71 mg/L. Diagnosis: The patient underwent pelvic mass aspiration was diagnosed with ovarian clear cell cancer. B-ultrasound revealed thrombosis of the lower limbs. Interventions: She underwent 2 neoadjuvant chemotherapies, along with anticoagulation therapy. However, it had a poor therapeutic effect, and the patient suffered from acute cerebral infarction that worsened. Outcomes: Chemotherapy and anticoagulation failed to stop the tumor and blood clot progression. The patient died 2 months after cerebral infarction without surgical treatment. Lessons: Gynecologists should be aware of the need for clinical suspicion of the risk of thrombosis during the treatment period of ovarian cancer and make careful decisions.
Clinical Features and Prognostic Factors of Patients with Chordoma in the Spine: a Retrospective Analysis of 153 Patients in a Single Center.

Abstract:Background. Chordoma in the spine is relatively rare, and minimal information has been published in the literature regarding this subject. Moreover, there are controversies over prognostic factors of this disease.Methods. A retrospective analysis of chordoma in the spine was performed by survival analysis. Local relapse-free survival (LRFS) and overall survival (OS) were analyzed from the date of surgery to the date of local recurrence and death. The LRFS and OS rates were estimated using the Kaplan-Meier method to identify potential prognostic factors. Factors with P values <= .1 were subjected to multivariate analysis by Cox regression analysis. P values <= .05 were considered statistically significant.Results. A total of 153 patients with spinal chordoma were included in the study. The mean follow-up period was 72.0 months (range, 1-279 months). Local recurrence was detected in 51 cases after initial surgery in our center, while death occurred in 42 cases. The statistical analysis suggested that tumor location of C3-L5, dedifferentiated chordoma, preoperative Frankel scores A-C, and total spondylectomy were independent prognostic factors for LRFS. In addition, total en bloc spondylectomy and Karnofsky' performance status (KPS) >= 80% were favorable factors for OS.Conclusions. Total spondylectomy, by either en bloc or piecemeal method, could significantly reduce LRFS for spinal chordoma. Location of C3-L5 is a favorable factor for LRFS, while dedifferentiated subtype and preoperative Frankel scores A-C are adverse prognostic factors. In addition, total en bloc spondylectomy and KPS >= 80% significantly improve overall survival of patients with spinal chordoma.
Intrasellar chordoma masquerading as a pituitary neuroendocrine tumor: Illustrative case

Abstract:Background: Chordomas are rare, locally aggressive neoplasms recognized as derivatives of the notochord vestiges. These tumors typically involve the midline axial skeleton, and intracranial chordomas exhibit proclivity for the spheno-occipital region. However, purely intrasellar occurrences are extremely rare. We report a case of intrasellar chordoma, which masqueraded as a pituitary neuroendocrine tumor. Case description: An 87-year-old female presented with an acutely altered mental state after a few-week course of headaches and decreased left vision. Adrenal insufficiency was evident, and magnetic resonance imaging revealed an intrasellar lesion with heterogeneous contrast enhancement and marked T2 hyperintensity. Central adrenal insufficiency due to an intrasellar lesion was suspected. Cortisol replacement was initiated, and transsphenoidal surgery was performed. Anterosuperior displacement of the normal pituitary gland and the absence of the bony dorsum sellae were notable during the procedure. Histological examination led to a diagnosis of conventional chordoma, and upfront adjuvant stereotactic radiosurgery was executed. She has been free from tumor progression for 12 months. Conclusion: This case and literature review suggested that the pathognomonic features of intrasellar chordoma were heterogeneous contrast enhancement, marked T2 hyperintensity, osteolytic destruction of the dorsum sellae, and anterosuperior displacement of the pituitary gland. Clinical outcomes seemed slightly worse than those of all skull base chordomas, which were the rationale for upfront radiosurgery in our case. Neurosurgeons should include intrasellar chordomas in the differential diagnosis of intrasellar lesions, carefully dissect them from the adjacent critical anatomical structures, and consider upfront radiosurgery to achieve optimal patient outcomes.

Abstract:Background Chordomas of the upper cervical spine are rare and present unique surgical challenge. This study aimed to describe the clinical characteristics and surgical management of patients with chordomas of the upper cervical spine. Methods Twenty-one patients with chordornas of the upper cervical spine who were treated in Peking University Third Hospital from January 1999 to October 2012 were retrospectively analyzed. Survival was calculated by the Kaplan-Meier method and was compared between groups using the log-rank test. Results The postoperative diagnosis was classical chordoma in 20 cases and chondroid chordoma in one case. The mean operative time was 9.5 hours (range 6-17 hours), and the mean blood loss was 2 812 ml (range 700-4 800 ml). There were two postoperative deaths. Unilateral vertebral artery ligation was performed in six patients, cervical nerve roots were cut in six patients, and the external branch of the superior laryngeal nerve was repaired after being cut in one case. Two patients developed postoperative velopharyngeal incompetence, and loosening of the occipitocervical screws was observed in one patient. The recurrence rate was 66.7% (10/15) after a mean follow-up period of 46.8 months (range 14-150 months). The 5- and 10- year overall survival rates were (39.8 +/- 13.1)% and (31.9 +/- 12.7)%, respectively. There was a significant difference in survival rate between patients who underwent surgery and those who did not. Conclusion In spite of the high rates of recurrence and complications after surgical treatment of chordomas of the upper cervical spine, intralesional resection combined with adjuvant radiotherapy remains the optimal treatment to prolong survival.
Trousseau's Syndrome: A Paraneoplastic Complication

Abstract:Trousseau syndrome, also known as thrombophlebitis migrans or migratory superficial thrombophlebitis, is a rare but significant paraneoplastic manifestation associated with various cancers. This syndrome is characterized by the occurrence of recurrent deep or superficial venous thrombosis in patients with malignancies. Patients with cancer have a greatly increased risk of venous thrombosis, especially in the first few months after diagnosis and in the presence of distant metastases. This article describes the case of a 72-year-old female patient who suffered a deep vein thrombosis in the right lower limb, which led to Trousseau syndrome secondary to non-Hodgkin's lymphoma.

Abstract:OBJECTIVE: The combination of cancer and hypercoagulable states is often called Trousseau syndrome. In particular, cerebral infarction caused by Trousseau syndrome is reported to have a poor prognosis. In gynecology, there are many reports of ovarian cancer and a few of uterine cancer. Since there has been no comprehensive report of Trousseau syndrome in cervical cancer, we aimed to summarize Trousseau syndrome in cervical cancer.METHODS: Cerebral infarction caused by cancer-related arterial thrombosis was defined as Trousseau syndrome. Patients with cervical cancer diagnosed at our hospital between January 2014 and December 2021 were retrospectively reviewed using the hospital's medical records.RESULTS: A total of 1,432 patients were included in the study. Trousseau syndrome occurred in 6 patients (0.4%). The mean age of patients with Trousseau syndrome was 63 years (range: 53-78 years). Of the 6 patients who developed Trousseau's syndrome, 4 patients had it before or during initial treatment, and 2 during recurrent/relapsed disease treatment. The 4 patients who developed the syndrome before or during initial treatment had advanced disease: 1 in stage IIIC and 3 in stage IVB. In all cases, the disease was associated with progressive distant metastasis. The median survival time from the onset of Trousseau syndrome was 1 month (range: 0-6 months).CONCLUSION: Cervical cancer causes Trousseau syndrome in cases of advanced disease with a short time between the onset of the syndrome and mortality.

Abstract:Chordoma is a rare malignant bone tumor that mainly occurs in the sacrum and the clivus/skull base. Surgical resection is the treatment of choice for chordoma, but the local recurrence rate is high with unsatisfactory prognosis. Compared with other common tumors, there is not much research and individualized treatment for chordoma, partly due to the rarity of the disease and the lack of appropriate disease models, which delay the discovery of therapeutic strategies. Recent advances in modern techniques have enabled gaining a better understanding of a number of rare diseases, including chordoma. Since the beginning of the 21st century, various chordoma cell lines and animal models have been reported, which have partially revealed the intrinsic mechanisms of tumor initiation and progression with the use of next-generation sequencing (NGS) techniques. In this study, we performed a systematic overview of the chordoma models and related sequencing studies in a chronological manner, from the first patient-derived chordoma cell line (U-CH1) to diverse preclinical models such as the patient-derived organoid-based xenograft (PDX) and patient-derived organoid (PDO) models. The use of modern sequencing techniques has discovered mutations and expression signatures that are considered potential treatment targets, such as the expression of Brachyury and overactivated receptor tyrosine kinases (RTKs). Moreover, computational and bioinformatics techniques have made drug repositioning/repurposing and individualized high-throughput drug screening available. These advantages facilitate the research and development of comprehensive and personalized treatment strategies for indicated patients and will dramatically improve their prognoses in the near feature.

Bibliometric analysis of the global research trends and hotspots in chordoma from 2000 to 2020

Abstract:Chordoma is formed from embryonic residues or ectopic chordae and locally aggressive or malignant tumors. We visually analyzed the research tendency and hotspot of chordoma. The bibliometric analysis was conducted from the Web of Science Core Collection database over the past two decades. The term and strategies were as follows: "TS = (chordoma) OR TS = (chordoblastoma) OR TS = (chordocarcinoma) OR TS = (chordoepithelioma) OR TS = (chordosarcoma) OR TS = (notochordoma). AND Language: English. AND Reference Type: Article OR Review". A total of 2,118 references were retrieved and used to make a visual analysis by VOSviewer 1.6.15. The chordoma was on a steady rise and chordoma but remained the focus of scholars and organizations over the last two decades. The Chinese institutions and scholars lacked cooperation with their counterparts in other countries. The citations of documents and co-citation analysis of cited references suggested that M.L. McMaster, B.P. Walcott, P. Bergh, and S. Stacchiotti were leading researchers in this field of chordoma and their papers had been widely accepted and inspired recent researches. Keywords associated with recent chemotherapy, PD-1-related immunotherapy, and SMARCB1/integrase interactor 1 (INI1) in chordoma were a shortage of research and there may be more research ideas in the future by scholars. The research of chordoma will continue to be the hotspot. Thus, explaining the molecular mechanism and potential role of transcriptional inhibition and immunologic responses to SMARCB1/INI1-negative poorly differentiated chordoma will be available for preclinical experiments and clinical trials and lead to new therapeutic opportunities for chordoma patients.

Total resection was achieved in 5 cases,and subtotal resection in 13 cases.17 cases were typical chordomas and 1 case was chondroid chordomas.14 patients were followed up,4 died,10 survived.Conclusion: Chordomas in the skull base is an rare tumor and difficult to perform total resection.Microsurgery combined with radiotherapy or(γ-knife) therapy is recommended.The prognosis is very poor.

Abstract:PurposeChordoma is a rare, locally aggressive neoplasm of the bone that arises from embryonic notochordal remnants. In less than 5 % of cases, chordomas contain a highly malignant sarcomatous component. Because of the rarity of such tumors, little is known about their clinical features and optimal treatment options. Herein, we report two chordoma cases with malignant sarcomatoid areas, consistent with high-grade osteosarcoma in the primary spine lesions, and discuss the presentation and characteristics of this disease.Methods and resultsIn both patients, the diagnosis on the first computed tomography (CT)-guided trocar biopsy of the tumor was a conventional chordoma. The two cases represent dedifferentiated chordomas with a sharp demarcation between the conventional chordoma and the high-grade sarcomatous component, which was identified on T2-weighted magnetic resonance imaging (MRI). One patient experienced a symptomatic tumor recurrence 4 months after carbon-ion radiotherapy, and underwent en bloc wide resection of the tumor following chemotherapy. The patient remained well 36 months after surgery without tumor recurrence. The other patient underwent a gross total excision as the second surgery followed by carbon-ion radiotherapy. At the 39-month follow-up, there was no evidence of active disease.ConclusionsAccurate analyses of MRI and positron emission tomography scans should suggest the most representative section for histological assessment. Unlike the treatment of conventional chordomas, the treatment of this disease should include chemotherapy first, followed by en bloc resection and/or carbon-ion radiation.

Abstract:Trousseau's syndrome is a cancer-associated thromboembolism that significantly impacts patients' prognosis and quality of life (QOL). This study aimed to investigate the frequency, characteristics, and prognosis of Trousseau's syndrome in lung cancer patients at a Japanese community hospital and examine the effects of therapeutic agents on this condition. We retrospectively reviewed the electronic medical records of lung cancer patients diagnosed with thrombotic complications at the time of diagnosis in our department between August 2013 and April 2019. Patients' characteristics, thromboembolism sites, treatments, and prognosis were analyzed. Among 956 lung cancer patients, 19 (2%) had Trousseau's syndrome. The median age was 65 years, and adenocarcinoma was the most common histologic type (78.9%). The most common site of thromboembolism was the brain (84.2%). The median survival time was 84 days, and 52.6% of patients died within 90 days of diagnosis. Patients who survived longer than 90 days tended to have a higher frequency of non-adenocarcinoma histology, EGFR gene mutations, and therapeutic induction with immune checkpoint inhibitors (ICI). Trousseau's syndrome in lung cancer patients is associated with poor prognosis. Histologic type, EGFR mutation status, and treatment with ICI may influence the prognosis. Future larger-scale studies are needed to validate these potential prognostic factors and to develop personalized treatment strategies.
Trousseau's Syndrome With Severe Visual Loss As the Initial Symptom

Abstract:There are limited reports on patients with Trousseau syndrome, a condition characterized by hypercoagulability associated with malignant tumors, initially manifesting with reduced visual function. We present a case of a patient who experienced bilateral vision loss and was subsequently diagnosed with Trousseau's syndrome following examination and investigations. A 70-year-old man, undergoing chemotherapy for advanced pancreatic cancer, reported decreased visual acuity in both eyes. A dilated fundus examination revealed retinal pigment epithelial atrophy in the posterior pole and cotton-wool spots. Optical coherence tomography exhibited partial disruption of the ellipsoid zone in the parafoveal region, and full-field electroretinogram results were subnormal, although the macular retinal structure was preserved. Brain magnetic resonance imaging (MRI) detected occipital lobe infarction. Elevated coagulability markers, including D-dimer (5.5μg/mL), led to the diagnosis of Trousseau's syndrome. In cases where patients with malignant tumors present with profound visual loss, considering the possibility of Trousseau's syndrome and conducting assessments of brain function and coagulability is crucial for accurate diagnosis and appropriate management.

Abstract:OBJECTIVE:To discuss the early diagnosis and the treatment of chordoma in the jugular foramen area.METHODS:Three cases of chordoma in the jugular foramen area were diagnosis and reviewed retrospectively. The initial symptom was facial paralysis. Surgical removal was performed through the infratemporal fossa approach in 1 case, through petro-occipital approach in 2 cases. For repairing facial nerve defect, great auricular nerve was used in 1 case and facial-hypoglossal anastomosis was used in another 2 cases.RESULTS:The tumor were totally removed in all 3 cases. The patients were follow-up for 6 months to 1 year and no recurrence was found. The function of facial nerve was II grade (House-Brackmann) in one patient and IV grade in another 2 patients. No complications occurred in 3 cases.CONCLUSIONS:Although chordoma originating from the jugular foramen area were extremely rare, it was possible to make early diagnosis through finding clinical feature and imaging methods. The final diagnosis depended on pathology. Generally, the different surgical approaches were used according to the size and position of the tumor. The classic infratemporal fossa approach for tumor removal and facial nerve reconstruction should be considered in the patients with chordoma around jugular foramen.
