Abstract:Introduction: Nicotine dependence (ND) is a chronic, relapsing mental disorder characterized by compulsive cigarette seeking and smoking. Although the cerebellum plays an increasingly implicated role in ND, the exact cerebellar alterations in ND remain unclear. Identifying the localization of these cerebellar abnormalities in ND may help to further understand the role of the cerebellum in ND. Thus, we investigated the structural and functional alterations in the cerebellum in a large sample of smokers using the spatially unbiased infratentorial template (SUIT). Methods: High-resolution structural magnetic resonance imaging (MRI) data were acquired from 85 smokers and 41 nonsmokers. We applied voxel-based morphometry (VBM) and the SUIT cerebellar atlas to compare the cerebellar gray matter (GM) volume between smokers and nonsmokers. Using resting-state functional MRI data, we also performed seed-based functional connectivity (FC) analysis to examine the functional correlates of the GM volume changes. Results: Both VBM and lobular analyses revealed smaller GM volume in the bilateral Crus I in smokers. The GM volume of the left Crus I was inversely correlated with the severity of nicotine dependence as assessed by Fagerstrom Test for Nicotine Dependence (r = -.268, p = .013). We also found reduced FC between the bilateral Crus I and brain regions involved in the default mode network and motor system, as well as the frontal and temporal cortex in smokers. Conclusions: Our results indicate that decreased cerebellar GM volume and corticocerebellar FC are associated with ND, and these may underlie the core ND phenotypes, including automatized smoking behavior, cognitive, and emotional deficits.
Correlation between Cerebral Regional Gray Matter Volume and Smoking Cessation Treatment Outcome:A Voxel-Based Morphometric Analysis

Abstract:Objective To investigate the relationship between regional gray matter( GM) volume measured by voxelbased morphometry( VBM) and smoking cessation outcomes( point prevalence abstinence at 4 weeks). Methods A total of 23 chronic smokers were enrolled in this study. All patients received 12-week smoking cessation treatment. Before smoking cessation treatment,MRI scanning together with 3D imaging technique of the brain was performed in all participants.By using VBM analysis together with the independent two-sample t test,the gray matter volumes were compared between quitting smoking group and smoking cessation failure group. Results Of the 23 participants,successfully quitting smoking was obtained in 10,while smoking cessation failure was seen in 13. The gray matter volume of the right inferior frontal gyrus and right inferior temporal gyrus in successful smoking cessation participants was significantly larger than that in smoking cessation failures( P 0. 001). No cerebral region which had smaller gray matter volume was observed in successful smoking cessation participants. Conclusion The results suggest that maintaining smoking abstinence is correlated with greater pre-quit brain volume in the prefrontal cortex,which is the area that can regulate the cognitive function,i. e. attention,impulsive control,learning and memory,delaying exciting and emotion,etc. And greater volume of visual-spatial attention cortex,which plays a key role in visual information processing,is also associated with maintaining smoking abstinence. Maybe there are some brain structure variables which can be used as clinically useful predictors for smoking cessation treatment outcomes,and further large-scale studies are still needed.
Altered White Matter Integrity in Smokers is Associated with Smoking Cessation Outcomes

Abstract:Smoking is a significant cause of preventable mortality worldwide. Understanding the neural mechanisms of nicotine addiction and smoking cessation may provide effective targets for developing treatment strategies. In the present study, we explored whether smokers have white matter alterations and whether these alterations are related to cessation outcomes and smoking behaviors. Sixty-six smokers and thirty-seven healthy non-smokers were enrolled. The participants underwent magnetic resonance imaging scans and smoking-related behavioral assessments. After a 12-week treatment with varenicline, 28 smokers succeeded in quitting smoking and 38 failed. Diffusion parameter maps were compared among the non-smokers, future quitters, and relapsers to identify white matter differences. We found that the future relapsers had significantly lower fractional anisotropy (FA) in the orbitofrontal area than non-smokers, and higher FA in the cerebellum than non-smokers and future quitters. The future quitters had significantly lower FA in the postcentral gyrus compared to non-smokers and future relapsers. Compared to non-smokers, pooled smokers had lower FA in bilateral orbitofrontal gyrus and left superior frontal gyrus. In addition, regression analysis showed that the left orbitofrontal FA was correlated with smoking-relevant behaviors. These results suggest that white matter alterations in smokers may contribute to the formation of aberrant brain circuits underlying smoking behaviors and are associated with future smoking cessation outcomes.

Abstract:Magnetic resonance imaging (MRI) studies showed a link between chronic smoking and structural brain abnormalities. To date, little is known about the long-term effects of smoking on gray matter deficiencies. MRI study was carried for 44 smokers and 44 matched non-smokers to assess gray matter volume differences between the two groups. Decreased gray matter volumes were found in left thalamus, medial frontal cortex and anterior cingulate of smokers in comparison to controls. This voxel-based morphometry study is showing reduction of regional gray matter volume in smokers, which might better guide future research into the pathogenesis of chronic smoking.

Abstract:Smoking cessation is critical for reducing the risk of respiratory, cardiovascular diseases and cancers. However, most cessation attempts resulted in failure. In the present study, we aim to explore whether alterations of brain gray matter (GM) volume and functional connectivity (FC) are related to cessation outcomes, in hope of providing evidence for improving smoking cessation outcomes. Seventy-three smokers and 41 non-smokers were enrolled in the present study. All smokers participated in a 12-week smoking cessation treatment during which Varenicline was used to aid cessation. At the end of treatment, the smokers were divided into quitters and relapsers based on their abstinence performance. Structural magnetic resonance imaging (MRI) and voxel-based morphometry were applied to quantify the differences of regional brain volumes among the three groups at baseline. In addition, resting-state FC was used to investigate the related functional changes. In comparison with non-smokers, the smokers showed smaller GM volume in the left dorsal medial thalamus. Among the 73 smokers, 29 subjects successfully quitted smoking. The quitters showed greater GM volume than the relapsers in the right postcentral gyrus, right putamen\caudate nucleus and left orbitofrontal cortex (OFC). The GM volume in the left OFC was found to be negatively correlated with the pack years and daily smoking amount in the quitters. Furthermore, we found significantly reduced FC between left thalamus and left cerebellum in the relapsers. These findings extended our knowledge of the neural mechanism of smoking cessation, and suggested that brain structural and functional changes were related to smoking cessation outcomes.
Bilateral Fronto-Parietal Integrity in Young Chronic Cigarette Smokers: A Diffusion Tensor Imaging Study

Abstract:Background Cigarette smoking continues to be the leading cause of preventable morbidity and mortality in China and other countries. Previous studies have demonstrated gray matter loss in chronic smokers. However, only a few studies assessed the changes of white matter integrity in this group. Based on those previous reports of alterations in white matter integrity in smokers, the aim of this study was to examine the alteration of white matter integrity in a large, well-matched sample of chronic smokers and non-smokers. Methodology/Principal Findings Using in vivo diffusion tensor imaging (DTI) to measure the differences of whole-brain white matter integrity between 44 chronic smoking subjects (mean age, 28.0±5.6 years) and 44 healthy age- and sex-matched comparison non-smoking volunteers (mean age, 26.3±5.8 years). DTI was performed on a 3-Tesla Siemens scanner (Allegra; Siemens Medical System). The data revealed that smokers had higher fractional anisotropy (FA) than healthy non-smokers in almost symmetrically bilateral fronto-parietal tracts consisting of a major white matter pathway, the superior longitudinal fasciculus (SLF). Conclusion/Significance We found the almost symmetrically bilateral fronto-parietal whiter matter changes in a relatively large sample of chronic smokers. These findings support the hypothesis that chronic cigarette smoking involves alterations of bilateral fronto-parietal connectivity.
Changes in Structure and Perfusion of Grey Matter Tissues During Recovery from Ischaemic Subcortical Stroke: a Longitudinal MRI Study

Abstract:Stroke recovery with changes in volume and perfusion of grey matter (GM) tissues remains largely unknown. We hypothesized that GM atrophy co‐existed with GM plasticity presenting with increased volume and perfusion in specific regions in the period of post‐stroke recovery. Twelve well‐recovered stroke patients with pure subcortical lesions in the middle cerebral artery‐perfused zone were included. All of them underwent structural and perfusion magnetic resonance imaging (MRI) examinations at admission and a mean of 6 months after stroke onset. Differences in GM volume (GMV) on structural images and cerebral blood flow (CBF) derived from perfusion images between two examinations were compared using voxel‐based morphometry. The associations between changes in GMV and CBF with clinical scores were analysed. Decreased GMV was found in post‐central gyrus, pre‐central gyrus, precuneus, angular gyrus, insula, thalamus and cerebellum, and increased GMV was found in hippocampus, orbital gyrus and lingual gyrus (all corrected P < 0.05) at the follow‐up examination. Increased CBF was found in subcallosal cingulate gyrus, hippocampus and lingual gyrus (all corrected P < 0.05) at the follow‐up examination. Only decreased GMV in the anterior lobe of cerebellum was negatively associated with improvement of Barthel index (β = −0.683, P = 0.014). Our study provides the imaging evidence of GM atrophy co‐existing with GM plasticity involving in increased volume and perfusion in specific regions (including cognition, vision and emotion) in well‐recovered stroke patients, which advances our understanding of neurobiology of stroke recovery.
Effects of Smoking on Regional Homogeneity in Mild Cognitive Impairment: A Resting-State Functional MRI Study

Abstract:Background Smoking is a modifiable risk factor for Alzheimer’s disease (AD). However, smoking-related effects on intrinsic brain activity in high-risk AD population are still unclear. Objective We aimed to explore differences in smoking effects on brain function between healthy elderly and amnestic mild cognitive impairment (aMCI) patients using ReHo mapping. Methods We identified 64 healthy elderly controls and 116 aMCI patients, including 98 non-smoking and 18 smoking aMCI. Each subject underwent structural and resting-state functional MRI scanning and neuropsychological evaluations. Regional homogeneity (ReHo) mapping was used to assess regional brain synchronization. After correction for age, gender, education, and gray matter volume, we explored the difference of ReHo among groups in a voxel-wise way based on analysis of covariance (ANCOVA), followed by post hoc two-sample analyses (p < 0.05, corrected). Further, we correlated the mean ReHo with neuropsychological scales. Results Three groups were well-matched in age, gender, and education. Significant ReHo differences were found among three groups, located in the left supramarginal gyrus (SMG) and left angular gyrus (AG). Specifically, non-smoking aMCI had lower ReHo in SMG and AG than smoking aMCI and controls. By contrast, smoking aMCI had greater AG ReHo than healthy controls (p < 0.05). Across groups, correlation analyses showed that left AG ReHo correlated with MMSE (r = 0.18, p = 0.015), clock drawing test (r = 0.20, p = 0.007), immediate recall (r = 0.36, p < 0.001), delayed recall (r = 0.34, p < 0.001), and auditory verbal learning test (r = 0.20, p = 0.007). Conclusion Smoking might pose compensatory or protective effects on intrinsic brain activity in aMCI patients.
Distinct Alteration Patterns of Resting-State Functional Connectivity of the Corticostriatal Circuits Effected by Cigarette Smoking in Mild Cognitive Impairment Patients and Cognitively Normal subjects

Abstract:To explore the interaction effects of smoking status (non-smoking vs. smoking) and disease (cognitively normal (CN) vs. MCI) based on resting-state functional connectivity (rsFC) of the corticostriatal circuits. We included 304 CN non-smokers, 44 CN smokers, 130 MCI non-smokers, and 33 MCI smokers. The seed-based rsFC of striatal subregions (caudate, putamen, and nucleus accumbens [NAc]) with the whole-brain voxel was calculated. Furthermore, we performed mixed effect analysis to explore the interaction effects between smoking status and disease. Significant interaction effects were detected between: (1) right caudate and left inferior parietal lobule (IPL); (2) right putamen and bilateral cuneus; (3) bilateral NAc and bilateral anterior cingulate cortex (ACC). The post-hoc analyses revealed that the CN smokers showed increased rsFC between right caudate and left IPL compared to non-smokers; while the MCI smokers demonstrated decreased rsFC between right putamen and cuneus, and increased rsFC between bilateral NAc and ACC compared to non-smokers. In MCI smokers, the rsFC value between left NAc and ACC was positively correlated with Semantic Verbal Fluency (SVF, r = 0.387, p = 0.026), and the rsFC value between right NAc and ACC was positively correlated with SVF (r = 0.390, p = 0.025), Wechsler memory scale-logical memory (WMS-LM) immediate recall (r = 0.378, p = 0.03), and WMS-LM delayed recall (r = 0.367, p = 0.036). Our findings suggest that chronic nicotine exposure may lead to functional connectivity alterations of corticostriatal circuits in MCI patients, and the pattern is different from CN smokers.
Increased Thalamic Volume and Decreased Thalamo-Precuneus Functional Connectivity Are Associated with Smoking Relapse.

Abstract:The thalamus, with the highest density of nicotinic acetylcholine receptor (nAChR) in the brain, plays a central role in thalamo-cortical circuits that are implicated in nicotine addiction. However, little is known about whether the thalamo-cortical circuits are potentially predictive of smoking relapse. In the current study, a total of 125 participants (84 treatment-seeking male smokers and 41 age-matched male nonsmokers) were recruited. Structural and functional magnetic resonance images (MRI) were acquired from all participants. After a 12-week smoking cessation treatment with varenicline, the smokers were then divided into relapsers (n = 54) and nonrelapsers (n = 30). Then, we compared thalamic volume and seed-based thalamo-cortical resting state functional connectivity (rsFC) prior to the cessation treatment among relapsers, nonrelapsers and nonsmokers to investigate the associations between thalamic structure/function and smoking relapse. Increased thalamic volume was detected in smokers relative to nonsmokers, and in relapsers relative to nonrelapsers, especially on the left side. Moreover, decreased left thalamo-precuneus rsFC was detected in relapsers relative to nonrelapsers. Additionally, a logistic regression analysis showed that the thalamic volume and thalamo-precuneus rsFC predicted smoking relapse with an accuracy of 75.7%. These novel findings indicate that increased thalamic volume and decreased thalamo-precuneus rsFC are associated with smoking relapse, and these thalamic measures may be used to predict treatment efficacy of nicotine addiction and serve as a potential biomarker for personalized medicine.
Altered spontaneous brain activity in chronic smokers revealed by fractional ramplitude of low-frequency fluctuation analysis: a preliminary study

Abstract:Although a substantial body of previous functional magnetic resonance imaging (fMRI) studies have revealed different brain responses to external stimuli in chronic cigarette smokers compared with nonsmokers, only a few studies assessed brain spontaneous activity in the resting state in chronic smokers. The aim of this study was to investigate alterations of brain activity during the resting state in chronic smokers using fractional amplitude of low-frequency fluctuation (fALFF). In the present study, 55 smokers and 49 healthy nonsmokers were included. All the subjects underwent resting-state fMRI scans and the data were analyzed by the fALFF approach. The smokers showed significantly decreased fALFF in the left precuneus, right inferior temporal and occipital gyrus(ITG/IOG), while significantly increased fALFF in the right caudate. Subsequent correlation analysis revealed that the fALFF values of the left precuneus and right ITG/IOG were positively correlated with years of smoking across the smokers. This resting-state fMRI study suggests that the changed spontaneous neuronal activity, as reflected by the fALFF, in these regions may be implicated in the underlying the pathophysiology of smoking.
Alterations in white matter microstructure in alcohol and alcohol-polydrug dependence: Associations with lifetime alcohol and nicotine exposure

Abstract:Evidence suggests that alcohol dependence (AD) is associated with microstructural deficits in white matter, but the relationship with lifetime alcohol exposure and the impact of polydrug dependence is not well understood. Using diffusion tensor magnetic resonance (MR) imaging, we examined white matter microstructure in relation to alcohol and polydrug dependence using data from the Imperial College Cambridge Manchester (ICCAM) platform study. Tract-based spatial statistics were used to examine fractional anisotropy (FA) in a cohort of abstinent AD participants, most of whom had a lifetime history of dependence to nicotine. A further subgroup also had a lifetime history of dependence to cocaine and/or opiates. Individuals with AD had lower FA throughout the corpus callosum, and negative associations with alcohol and nicotine exposure were found. A group-by-age interaction effect was found showing greater reductions with age in the alcohol-dependent group within corpus callosum, overlapping with the group difference. We found no evidence of recovery with abstinence. A comparison of alcohol-only- and alcohol-polydrug-dependent groups found no differences in FA. Overall, our findings show that AD is associated with lower FA and suggest that these alterations are primarily driven by lifetime alcohol consumption and cigarette smoking, showing no relationship with exposure to other substances such as cocaine, opiates or cannabis. Reductions in FA across the adult lifespan are more pronounced in AD and offer further support for the notion of accelerated ageing in relation to alcohol dependence. These findings highlight there may be lasting structural differences in white matter in alcohol dependence, despite continued abstinence.

Abstract:Compared to nonsmokers, smokers exhibit a number of potentially important differences in regional brain function. However, little is known about the associations between the local spontaneous brain activity and smoking cessation treatment outcomes. In the present analysis, we aimed to evaluate whether the local features of spontaneous brain activity prior to the target quit date was associated with the smoking cessation outcomes. All the participants underwent magnetic resonance imaging scans and smoking-related behavioral assessments. After a 12-week treatment with varenicline, 23 smokers succeeded in quitting smoking and 32 failed. Smokers underwent functional magnetic resonance imaging (fMRI) scanning prior to an open label smoking cessation treatment trial. Regional homogeneity (ReHo) was used to measure spontaneous brain activity, and whole-brain voxel-wise comparisons of ReHo were performed to detect brain regions with altered spontaneous brain activity between relapser and quitter groups. After controlling for potentially confounding factors including years of education, years smoked, cigarettes smoked per day and FTND score as covariates, compared to quitters, relapsers displayed significantly decreased ReHo in bilateral posterior cingulate cortex (PCC), as well as increased ReHo in left superior temporal gyrus (STG). These preliminary results suggest that regional brain function variables may be promising predictors of smoking relapse. This study provided novel insights into the neurobiological mechanisms underlying smoking relapse. A deeper understanding of the neurobiological mechanisms associated with relapse may result in novel pharmacological and behavioral interventions.

Abstract:Abstract Background Few studies have been previously published about the resting state brain activity in young chronic smokers, although many previous fMRI studies have shown that the task-related activity pattern is altered in chronic smokers. Methods In the present study, forty-five healthy smokers (age: 27.9 ± 5.6 year) and forty-four healthy non-smoking control subjects (age: 26.3 ± 5.8 year) have been imaged with functional magnetic resonance imaging (fMRI) and analyzed with the regional homogeneity (ReHo) approach. Results Compared with healthy controls, decreased ReHo was found in smokers in the right inferior frontal cortex and increased ReHo was found in the left superior parietal lobe (P < 0.01, 35 Voxels,Alphasim corrected). Conclusions Our data suggested that, during resting state, neural function is less synchronized in the right inferior frontal cortex and more synchronized in the left superior parietal lobe in chronic smokers compared to non-smokers. The decreased synchronization in the right inferior frontal cortex may reflect lacking of control over reward-related behavior, and the increased synchronization may reflect smoking urges.

Abstract:The insula, a cortical region that integrates heterogeneous signals about internal states and contributes to executive functions, has been implicated as an important role in the maintenance of nicotine dependence. Previous studies have indicated that insula damage may contribute to quitting smoking successfully, but few studies have examined whether quitting successfully is related to cortical structural integrity of insular subregions before smoking cessation treatment. Moreover, although prior researches have shown group differences in insular cortex structure in chronic cigarette smokers compared to nonsmokers, less is known about how cortical structural integrity of insular subregions relate to smoking behaviors in smokers. This study, therefore, aimed to (1) further explore the association between the cortical structural integrity of insular subregions prior to the target quit date and the treatment outcomes of smoking cessation therapy; and (2) further evaluate how the cortical structural integrity of insular subregions are related to smoking behaviors. In the present study, a total of 83 smokers and 41 nonsmokers were enrolled and high-resolution structural magnetic resonance images were acquired from all participants. After a 12-week smoking cessation treatment, 28 smokers succeeded in quitting smoking, 46 failed, and 9 were unable to be contacted. Our analysis showed that gray-matter volume of bilateral anterior insula were negatively correlated with nicotine dependence scores. However, the smoking cessation outcomes showed no correlations with the gray-matter volume and seed-based structural covariance network of insular subregions prior to smoking cessation. The present study further clarified the more precise roles of the insular cortex in smoking behaviors, which might improve the understanding of the mechanism in the nicotine dependence.
Smoking, tobacco dependence, and neurometabolites in the dorsal anterior cingulate cortex

Abstract:Cigarette smoking has a major impact on global health and morbidity, and positron emission tomographic research has provided evidence for reduced inflammation in the human brain associated with cigarette smoking. Given the consequences of inflammatory dysfunction for health, the question of whether cigarette smoking affects neuroinflammation warrants further investigation. The goal of this project therefore was to validate and extend evidence of hypoinflammation related to smoking, and to examine the potential contribution of inflammation to clinical features of smoking. Using magnetic resonance spectroscopy, we measured levels of neurometabolites that are putative neuroinflammatory markers. N -acetyl compounds ( N -acetylaspartate + N -acetylaspartylglutamate), glutamate, creatine, choline-compounds (phosphocholine + glycerophosphocholine), and myo -inositol, have all been linked to neuroinflammation, but they have not been examined as such with respect to smoking. We tested whether people who smoke cigarettes have brain levels of these metabolites consistent with decreased neuroinflammation, and whether clinical features of smoking are associated with levels of these metabolites. The dorsal anterior cingulate cortex was chosen as the region-of-interest because of previous evidence linking it to smoking and related states. Fifty-four adults who smoked daily maintained overnight smoking abstinence before testing and were compared with 37 nonsmoking participants. Among the smoking participants, we tested for associations of metabolite levels with tobacco dependence, smoking history, craving, and withdrawal. Levels of N -acetyl compounds and glutamate were higher, whereas levels of creatine and choline compounds were lower in the smoking group as compared with the nonsmoking group. In the smoking group, glutamate and creatine levels correlated negatively with tobacco dependence, and creatine correlated negatively with lifetime smoking, but none of the metabolite levels correlated with craving or withdrawal. The findings indicate a link between smoking and a hypoinflammatory state in the brain, specifically in the dorsal anterior cingulate cortex. Smoking may thereby increase vulnerability to infection and brain injury.

Abstract:Growing evidence from animal studies indicates brain-damaging properties of nicotine exposure. Investigations in humans found a wide range of functional cerebral effects of nicotine and cigarette smoking, but studies focusing on brain damage are sparse. In 22 smokers and 23 never-smokers possible differences of the cerebral structures were investigated using magnetic resonance imaging and voxel-based morphometry. Significantly smaller grey matter volume and lower grey matter density (P = 0.05, corrected) were observed in the frontal regions (anterior cingulate, prefrontal and orbitofrontal cortex), the occipital lobe and the temporal lobe including parahippocampal gyrus, in smokers than in never-smokers. Group differences of either grey matter volume or grey matter density were also found in the thalamus, cerebellum and substantia nigra, among other regions. Smokers did not show greater volumes than never-smokers in any cerebral region. Magnitude of lifetime exposure to tobacco smoke (pack-years) was inversely correlated with volume of frontal and temporal lobes and cerebellum (P = 0.001, uncorrected). The data indicate structural deficits of several cortical and subcortical regions in smokers relative to never-smokers. The topographic profile of the group differences show some similarities to brain networks known to mediate drug reinforcement, attention and working memory processing. The present findings may explain in part the frequently reported cognitive dysfunctions in chronic cigarette consumers.

Abstract:Cigarette smoking is associated with elevated risk of disease and mortality and contributes to heavy healthcare-related economic burdens. The nucleus accumbens is implicated in numerous reward-related behaviors, including reinforcement learning and incentive salience. The established functional connectivity of the accumbens includes regions associated with motivation, valuation, and affective processing. Although the high comorbidity of cigarette smoking with drinking behaviors may collectively affect brain activity, there could be independent effects of smoking in alcohol use disorder that impact brain function and behavior. We hypothesized that smoking status, independent of alcohol use, would be associated with aberrations of nucleus accumbens functional connectivity to brain regions that facilitate reward processing, salience attribution, and inhibitory control. Resting state functional magnetic resonance imaging data from thirty-one nonsmokers and nineteen smoking individuals were analyzed using seed-based correlations of the bilateral accumbens with all other brain voxels. Statistical models accounted for drinks consumed per week. The smoking group demonstrated significantly higher functional connectivity between the left accumbens and the bilateral insula and anterior cingulate cortex, as well as hyperconnectivity between the right accumbens and the insula. Confirmatory analyses using the insula and cingulate clusters generated from the original analysis as seed regions reproduced the hyperconnectivity in smokers between the bilateral insular regions and the accumbens. In conclusion, smoking status had distinct effects on neural activity; hyperconnectivity between the accumbens and insula in smokers may reflect enhanced encoding of the reinforcing effects of smoking and greater orientation toward smoking-associated stimuli.

Abstract:Resting-state EEG reflects intrinsic brain activity and its alteration represents changes in cognition that are related to neuropathology. Thereby, it provides a way of revealing the neurocognitive mechanisms underpinning chronic substance use. In addition, it is documented that some neurocognitive functions can recover following sustained abstinence. We present a systematic review to synthesize how chronic substance use is associated with resting-state EEG alterations and whether these spontaneously recover from abstinence. A literature search in Medline, PsycINFO, Embase, CINAHL, Web of Science, and Scopus resulted in 4088 articles, of which 57 were included for evaluation. It covered the substance of alcohol (18), tobacco (14), cannabis (8), cocaine (6), opioids (4), methamphetamine (4), and ecstasy (4). EEG analysis methods included spectral power, functional connectivity, and network analyses. It was found that long-term substance use with or without substance use disorder diagnosis was associated with broad intrinsic neural activity alterations, which were usually expressed as neural hyperactivation and decreased neural communication between brain regions. Some studies found the use of alcohol, tobacco, cocaine, cannabis, and methamphetamine was positively correlated with these changes. These alterations can partly recover from abstinence, which differed between drugs and may reflect their neurotoxic degree. Moderating factors that may explain results inconsistency are discussed. In sum, resting-state EEG may act as a potential biomarker of neurotoxic effects of chronic substance use. Recovery effects awaits replication in larger samples with prolonged abstinence. Balanced sex ratio, enlarged sample size, advanced EEG analysis methods, and transparent reporting are recommended for future studies.
