Abstract:OBJECTIVE:To evaluate the effectiveness of percutaneous monoplanar screw internal fixation via injured vertebrae for treatment of thoracolumbar fracture. METHODS:Between May 2015 and August 2017, 38 cases of thoracolumbar fractures without neurological symptom were treated with percutaneous monoplanar screw internal fixation via injured vertebrae. There were 22 males and 16 females, aged 25-52 years (mean, 32.5 years). There were 23 cases of AO type A3 and 15 cases of AO type A4. The injured vertebrae located at T 11 in 4 cases, T 12 in 9 cases, L 1 in 11 cases, L 2 in 10 cases, L 3 in 3 cases, and L 4 in 1 case. The mean interval between injury and operation was 4.5 days (range, 3-7 days). The pre- and post-operative degrees of lumbodorsal pain were estimated by the visual analogue scale (VAS) score. The X-ray film, CT three-dimensional reconstruction, and MRI were performed, and the ratio of anterior vertebral body height and sagittal Cobb angle were measured to assess the kyphosis of the fractured area. RESULTS:All operations in 38 patients successfully completed without complications such as dural sac, nerve root, or vascular injury. The operation time was (56.2±3.7) minutes and the intraoperative blood loss was (42.3±3.5) mL. All incisions healed by first intention without redness, swelling, or exudation. All patients were followed up 17-33 months, with an average of 21.5 months. The VAS score at each time point after operation significantly improved when compared with that before operation ( P<0.05), and significantly improved at 3 months and last follow-up when compared with that at 1 week ( P<0.05); there was no significant difference between 3 months and last follow-up ( P>0.05). There was no internal fixator loosening, breakage, or delayed kyphosis in all patients. The ratio of anterior vertebral body height and sagittal Cobb angle significantly improved postoperatively ( P<0.05), and no significant difference was found between the different time points after operation ( P>0.05). CONCLUSION:Percutaneous monoplanar screw internal fixation via injured vertebrae is an easy approach to treat thoracolumbar fracture without neurological symptom, which can effectively restore vertebral body height and correct kyphosis, and avoid long-term segmental kyphosis.
Reduce the Fractured Central Endplate in Thoracolumbar Fractures Using Percutaneous Pedicle Screws and Instrumentational Maneuvers: Technical Strategy and Radiological Outcomes.

Abstract:INTRODUCTION:Traumatic thoracolumbar burst fracture is a common condition without a clear consensus on the best treatment approach. Percutaneous pedicle screw fixation (PPSF) techniques are widely used in practice, while its ability to correct fracture deformity is relatively weak, especially for the central area of the endplate. In this study, we reported a novel technique to reduce the fractured central endplate in thoracolumbar burst fractures.METHODS:The new reduction technique uses six percutaneous pedicle screws for the fractured vertebra and its adjacent vertebrae. Pedicle screws implanted in the two adjacent vertebrae were parallel to the superior vertebral endplate, as routinely required. Two monoaxial pedicle screws implanted in the fractured vertebra were placed toward the anteroinferior portion of the fractured vertebral body. After routine instrumentation and ligamentotaxis reduction, the bolt heads of the four screws implanted in the adjacent vertebrae were first tightened, and then the bolt heads of the screws implanted in the fractured vertebra were gradually tighten to elevate the collapsed endplate. A fundamental principle of this technique is to implant the pedicle screw in the fractured vertebra towards the anteroinferior portion of the vertebra in such a way that the angle between the pedicle screw and the rod is oblique on lateral fluoroscopy. As such, when the bolt heads were tightened, the pedicle screws can be swung up to reduce the endplate fragments.RESULTS:The novel technique was performed in 24 patients with neurologically intact thoracolumbar AO type A3 fractures. The middle vertebral height ratio was significantly improved from 69.7%±7.6% after routine reduction to 85.1%±4.5% postoperatively (p<0.01). No complication was noticed for this new reduction technique. At 6-month follow-up, no significant correction loss of the middle and posterior vertebral height ratios, Cobb angle, and vertebral wedge angle was observed, while 5.8% of correction loss was observed for the anterior vertebral height ratio.CONCLUSION:The described reduction technique is simple, safe, and effective in reducing the collapsed central endplate in thoracolumbar burst fractures. Such a practical reduction strategy does not need additional medical costs.
[Surgical Treatment of Type-C Thoracolumbar Fractures].

Abstract:OBJECTIVE:To explore the operative approach and method of internal fixation for the treatment of type-C thoracolumbar fractures. METHODS:T wenty-eight patients (male 20, female 8, ranging in age from 20 to 54 years, with an average of 38.5 years) with type-C thoracolumbar fractures were invovled in the study. Distributed segments involved T11 in 2 patients, T12 in 3 patients, L1 in 11 patients, L2 in 8 patient, L3 in 3 patients and L4 in 1 patient. Twenty-six patients were followed up (range from 12 to 20 months). According to the analysis of X-ray and CT image,height of vertebral body, Cobb angle on sagittal and coronal plane and the percentage of occupancy of vertebral canal were measured. The recovery of nerve, happening of back pain and the failure of internal fixation were observed. RESULTS:The preoperative averaging height-loss decreased from 37.4% to 6.8% and the deformation of coronal plane was completely rectified. The preoperative averaging Cobb angle on sagittal plane recovered from 22.3 degrees to 5.6 degrees and the preoperative occupancy of vertebral canal averaging recovered from 33.7% to 5.9%. The difference was statistically significant (P < 0.05). Moreover, after 1 year follow-up, the changes of the above-mentioned index was no statistically significant (P > 0.05). Except for 8 patients with complete nerve damage losing the possibility of recovery, the others with incomplete nerve damage obtained 1 to 3 degree's improvement. The ratio of back pain occurrence was 19.2%. There was no failure of internal fixation. CONCLUSION:The treatment of thoracolumbar type-C fractures with simple posterior long-segment internal fixation or posterior long-segmental fixation added by anterior autograft fusion is a reliable and effective method. The short-term therapeutic effect is satisfactory and the long-term therapeutic effect is to be further observed.
Clinical Comprehension of Thoracolumbar Spine Fracture Treatmented with Posterior Monosegmental Vertebral Pedicle Screw Fixation at the Injured Vetebrae

Abstract:[Objective] To evaluate the clinical outcome of thoracolumbar spine fracture treatmented with posterior monosegmental transpedicular fixation at the injured vertevrae.[Method] From January 2009 to December 2010,14 cases of thoracolumbar spine fracture were treated by the surgical procedure of posterior monosegmental injured vertebral pedicle internal fixation and injured vertebra bone grafting fusion.The status of internal fixation material was followed up and the vertebral height and kyphosis angle were compared before and after the surgery.[Result] The mean operation time,average bleeding volume during operation,was 83 min(52~125min),120ml(50~350m1),respectively.There was no iatrogenic nerve injure in all cases.All patients obtained the follow up and the average time was 16.8 months(12~24 moths).No internal fixation material breaking,loose and dislocation were found after operation.Bony union was observed in all cases.Postoperation vertebral height and segmental kyphosis angle different chips were statistically significant.[Conclusion] The posterior monosegmental injured vertebral internal fixation is one of effective method for treating thoracolumbar spine fracture.Strict indication and surgical skill is the key to this procedure.

Abstract:Object To explore the feasibility and efficiency of treatment of minimally invasive percutaneous pedicle screws osteosynthesis(MIPPSO) and vertebroplasty by filling the injured vertebrae with injectable artificial bones on thoracolumbar fractures.Methods From Feb 2008 to Sep 2011,10 patients were treated with MIPPSO and vertebroplasty by filling the injured vertebrae with injectable artificial bones on thoracolumbar fractures.Results The average operative time was 80 minutes ﹙range,70~90min﹚,with a mean blood loss of 72ml﹙range,50~100ml﹚ and mean postoperative hospital stay of 8 days ﹙range,7~10 days﹚.23 cases were followed up 3~5 months ﹙average 3.5 months﹚,During the following-up,no case showed internalfixation device loosening or breakage;No case had anterior height loss of body of spine or collapse.CT results showed all injected artificial bones were absorbed within 3~4 months postoperatively.The anterior height and Cobb angle of the injured vertebrae body were significantly less than before treatment[﹙10.78�.53﹚%、﹙53.41�.35﹚%,P<0.0l;8.6�.9��22.72�.6� P<0.0l].Conclusion Fixation with vertebral pedicle screw system and vertebroplasty using injectable artificial bones is a safe and effective minimally invasivesurgical method.
[Minimal Invasive Internal Fixation with U-shaped Break-off Pedicle Screws for Treatent of Thoracolumbar Fractures].

Abstract:OBJECTIVE:To investigate the clinical outcome of minimal invasive internal fixation with U-shaped break-off pedicle screws through paraspinal muscle sparing approach to treat thoracolumbar fractures, and to discuss its advantages.METHODS:From August 2010 to June 2012, we had 40 thoracolumber fractures patients (27 males and 13 females). Their ages ranged from 22 to 60 years. Of the 40 cases, 4 were T11 fractures, 13 T12 fractures ,17 L1 fractures,6 L2 fractures. According to Denis classification,all of them were burst fractures, with vertebral canal compromise less than 1/3. According to AO classification they were type A or type B1 injuries. All the cases had no nerve injury. The patients were randomly divided into two groups. With Group A (20 cases) we took the method of minimal invasive internal fixation with U-shaped break-off pedicle screws to fix one level above and below the injured vertebra through the parespinal muscle sparing approach. With Group B (20 cases), we took the traditional posterior midline approach and open procedure. Then we compared the two groups by operation time, blood loss, drainage, Visual Analogue Scales and X-ray exposure.RESULTS:Minimal invasive group had obvious advatages in operation time, bleeding control and early pain relief of post-operation. The X-ray exposure and long-term follow-up outcome were almost the same.CONCLUSION:Through paraspinal muscle sparing approach minimal invasive internal fixtation with U-shaped break-off pedicle screws to treat thoracolumber has more advantages than traditional procedure in blood control and quick recovery. The method needs no special instruments and don't increase X-ray exposure.
Efficacy of One-Stage Posterior-Anterior Approach for Treatment of Severe Thoracolumbar Fractures

Abstract:Objective To observe the outcomes of one-stage posterior short-level pedicle screw fixation combined with anterior fixation of severe thoracolumbar fractures.Methods A retrospective case series study was performed on 21 patients with severe thoracolumbar fractures stabilized by posterior short-level pedicle fixation combined with anterior internal fixation at one stage from January 2012 to December 2014.There were 16 males and 5 females,at age of 17 and 64 years [(38.7 ± 11.4) years].The involved segments included T11 in 2 patients,T12 in 5,Lt in 6 and L2 in 8.For AO fracture classification,type A fractures were seen in 4 patients,type B in 7 and type C in 10.Thoracolumbar injury classification and severity score (TLICS) was (8.12 ± 0.87) points (range,7-10 points).Frankel neurological performance scale was Grade B in 8 patients,Grade C in 11 and Grade D in 2.Operation time,blood loss,nerve function,kyphosis correction and complications were reported.Results Operation time was (234.5 ±57.3)min (range,180-360 min),and blood loss was (387.4 ± 124.4) ml (range,260-950 ml).Time of follow-up was (19.8 ± 3.5)months (range,14-25 months).Nerve function of 18 patients was improved by at least one Frankel scale.Cobb angle was (4.1 ±5.3)° at postoperative 3 days and (4.0 ± 4.9)°at the final follow-up,showing significant differences from that before operation [(-9.3 ± 4.2) °] (P ＜ 0.05).While the difference of Cobb angle did not differ significantly at postoperative 3 days and at final follow-up.No cerebrospinal fluid leakage,vascular injury,incision infection or nerve function deterioration occurred.Conclusion One-stage posterior short-level pediele screw fixation combined with anterior decompression and bone graft fixation is characterized by short operation time,few blood loss,good correction of traumatic kyphosis and good neurological recovery,indicating a good surgical choice for severe thoracolumbar fractures.
Treatment of Thoracolumbar Fractures by Percutaneous Pedicle Screw Fixation Technique Combined with Three-Step Reduction

Abstract:Abstract Purpose To evaluate the efficacy of percutaneous pedicle screw fixation technique combined with three-step reduction in the treatment of thoracolumbar fracture. Methods This is a retrospective study, based on medical records from the archives of the Fifth People's Hospital of Fudan University, from January 2012 to January 2015. The 49 patients with thoracolumbar fracture were treated with percutaneous pedicle screw fixation technique combined with three-step reduction. The treatment plan was determined by the AO classification and the Thoracolumbar Injury Classification and Severity classification. Baseline data included patient demographics, mechanism of injuries, fracture level, and neurologic status. Kyphotic angle correction and vertebral body height (VBH) restoration were used to assess radiologic outcome. Blood loss, operative complications, hospital stay, and pain score on the visual analog scale (VAS) were used to assess safety and clinical outcomes. Result A total of 49 patients with 51 thoracolumbar burst fractures were treated with percutaneous pedicle screw fixation technique combined with three-step reduction. There was significant improvement in kyphotic angle correction (p < 0.05) and restoration of anterior vertebral height (p < 0.05) after each reduction procedure. These improvements remained statistically significant at the latest follow-up for restoration of anterior VBH (p < 0.05) and kyphotic angle correction (p < 0.05). The preoperative VAS score was 8.2 ± 1.3, and the postoperative VAS score (3 days after the operation) was 3.5 ± 0.7. Conclusion Percutaneous pedicle screw internal fixation technique combined with three-step reduction is a relatively safe and effective technique for treating selected thoracolumbar burst fractures, and it yields satisfactory results.
Perioperative and short-term advantages of minimally invasive approach for the treatment of thoracolumbar fracture

Abstract:Objective To investigate the perioperative and short-term clinical outcomes of minimally invasive approach for the treatment of thoracolumbar fracture ( minimally invasive paraspinal muscle approach +pedicle screw fixation ). Methods From January 2013 to July 2016, 46 patients with thoracolumbar fracture were retrospectively analyzed. Twenty-four patients underwent surgeries by minimally invasive paraspinal muscle approach ( minimally invasive group ), and 22 patients by conventional median approach ( conventional group ). Operation time, intraoperative blood loss, postoperative hospitalization stay were compared between 2 groups. Clinical results were assessed by Oswestry disability index ( ODI ) in the final follow-up. Results There were no significant differences in age, gender, height, weight, body mass index, preoperative visual analogue scale, and preoperative Oswestry disability index ( ODI ) between the 2 groups ( P > 0.05 ). There were no significant differences in the mean operation time between the 2 groups: minimally invasive group ( 114.5 ± 26.3 ) min, conventional group ( 105.7 ± 23.4 ) min ( P >0.05 ). There were significant differences in the intraoperative blood loss: minimally invasive group ( 118.6 ± 32.1 ) ml, conventional group ( 217.4 ± 41.5 ) ml ( P < 0.05 ). There were significant differences in the hospitalization stay:minimally invasive group ( 3.8 ± 1.1 ) days, conventional group ( 5.4 ± 1.4 ) days ( P < 0.05 ). There were significant differences in the ODI in the final follow-up: minimally invasive group ( 15.9 ± 11.4 ), conventional group ( 30.1 ± 15.2 ) ( P < 0.05 ). Conclusions Minimally invasive paraspinal muscle approach has the advantages of being less invasive and quick recovery, but further investigations are still needed to confirm the long term effects.
Outcome Measures of Open versus Minimally Invasive Surgery for Thoracolumbar Spinal Traumatic Fractures: A Systematic Review and Meta-Analysis

Abstract:Objective: To evaluate the efficacy of open and percutaneous pedicle screw fixation in the treatment of thoracolumbar fractures. Methods: Online databases MEDLINE (PubMed), SCOPUS, and Cochrane were searched for English language articles published between January 2001 and December 2023, limited to articles that included the clinical and radiological outcomes of adult patients. The main outcome measures of the study were the Oswestry Disability Index (ODI), the Numeric Rating Scale (NRS) score, and the Cobb angle. Results: A total of 7 studies involving 909 patients were included; 374 (41.1%) procedures were performed with open surgery (OS), while 535 (58.9%) procedures were conducted with minimally invasive surgery (MIS). The mean value of ODI in the MIS group was 8.29% [CI 95% 4.82-11.76], compared to the other group, which was 14.22% (p-value 0.87). Patients receiving a MIS had an average NRS of 1.54 [CI95% 0.98-2.10] whilst OS had 2.31 [CI95% 1.50-3.12] (p-value 0.12). Conclusions: The percutaneous technique is equally safe and effective in resolving the deformity, but the clear advantages are represented by the reduction in blood loss, shorter operative times, a lower incidence of infection, shorter hospitalization, shorter postoperative rehabilitation, and therefore good results in terms of quality of life.
[Percutaneous Pedicle Screw Fixation in Thoracic-Lumbar Fracture Using Mini-Invasive Pedicle Screw System Guided by Navigation].

Abstract:OBJECTIVETo assess the therapeutic effects of thoracic-lumbar fracture fixation with percutaneous mini-invasive pedicle screw system guided by computer navigation.METHODFifty-one thoracic-lumbar fracture patients were divided into two groups: Sextant group (n = 21) undergoing percutaneous screw fixation with a minimally invasive pedicle screw system, and conventional pedicle screw fixation group (n = 30). The differences in the incision size, average blood loss during operation, surgical time, time of resuming walking after operation, numerical analogous pain score, etc. were analyzed.RESULTSThe incision length of the sextant group was 2.4 +/- 0.41 cm, significantly shorter than that of the conventional pedicle screw fixation group (17.4 +/- 3.4 cm, P < 0.001). The average blood loss during operation of the sextant group was 114 +/- 67.4 ml, significantly less than that of the conventional pedicle screw fixation group (317 +/- 113.2 ml, P < 0.001). The surgical time of the sextant group was 2.1 +/- 0.4 hr, significantly shorter than that of the conventional pedicle screw fixation group (2.7 +/- 0.7, P < 0.05). The time of resuming walking after operation of the sextant group was 2.0 +/- 0.7 d, significantly shorter than that of the conventional pedicle screw fixation group (7.2 +/- 2.5 d, P < 0.001). The postoperative NAPS was significantly lower than that before operation in both groups, however, there was no significant difference in the improvement of NAPS between the two groups.CONCLUSIONPercutaneous pedicle screw fixation using mini-invasive pedicle screw system guided by computer navigation is a good surgical therapeutic choice in thoracic-lumbar fracture.

Abstract:PURPOSE The main intension of this paper is to challenge the dominance of emotional labour in professional nursing. DESIGN/METHODOLOGY/APPROACH The article begins by evaluating the central conceptual and definitional aspects of emotional labour, emotion work and emotional work. The purpose of this discussion is to argue against the false public and private dichotomy that has plagued emotional labour and emotion work. Second, it is proposed that the central and helpful defining aspects of emotional labour and emotion work are Marx's concepts of exchange-value and use-value. These defining attributes are used in conjunction with other re-conceptualisations, which unite these terms in order to create more encompassing constructs that are useful for focusing on the waged and unwaged aspects of professional nurses' emotional work response behaviours. Finally, the use of emotional labour in professional nursing is contested on the grounds that the construct has limited theoretical and empirical utility for researching the complex nature of professional nurses' emotional work response behaviours. FINDINGS It is recommended that a more robust encompassing concept needs to be developed, which accurately reflects the nature and complexity of professional nurses' waged and unwaged emotional work response behaviours, as they are important overlooked facets of behaviour that can be theoretically related to professional nurses' contextual performance. ORIGINALITY/VALUE The paper provides a better understanding of professional nurses' emotional work response behaviours, which benefit nursing research and practice by drawing on other areas of theory and research.
Short-term effectiveness of pedicle fixation at the fractured vertebra via paravertebral intermuscular ap-proach for thoracolumbar fractures

Abstract:Objective Pedode foxatoon at the level of fracture vetebral technology is widely used in rebuilding spnal fracture recently.The purpose of minimally invasive surgery for thoracolumbar fractures is to reduce the injury , fix segments, and rebuild the stability of the spine .This study aimed to investigate the short-term clinical effect of pedicle fixation at the fractured vertebra via the pa-ravertebral intermuscular approach for the treatment of thoracolumbar fractures . Methods We treated 24 cases of thoracolumbar fracture by pedicle fixation at the fractured vertebra via the paravertebral intermuscular approach , rebuilt spinal stability , and corrected kyphotic deformity .We evaluated the improvement of pain symptoms and life quality using The Visual Analogue Scale ( VAS) and The MOS 36-Item Short-Form Health Survey (SF-36), and assessed the corrected status of vertebral height loss and kyphotic deformity by radiographic measurement .All the patients were followed up for over 12 months. Results The VAS score of the patients was signifi-cantly higher while the SF-36 score remarkably lower before surgery than the scores in 12 months which were ([7.65 ±0.13] -[1.54 ±0.07], P<0.05) and ([90.21 ±2.02]-[117.21 ±1.02], P<0.05).Imaging evaluation showed desirable correction of the reduced height of the injured vertebrae and kyphotic deformity . Conclusion Pedicle fixation at the fractured vertebra via the pa-ravertebral intermuscular approach , with its advantages of minimal invasiveness and fewer segments of fixation , can effectively correct vertebral height reduction and kyphotic deformity .
Clinical Outcome of Minimally Invasive Short-Segment Posterior Fixation at the Level of Fracture in the Treatment of Thoracolumbar Burst Fracture

Abstract:Background: Traditional short nonsegmental posterior fixation for thoracolumbar burst fractures often suffers from high rates of failure. Constructive stability may be improved by inserting additional screws at the injured level. But there are more intraoperative blood loss, more postoperative pain and longer hospital stay during open surgery. Objective:To evaluate the feasibility and safety of minimally invasive short-segment posterior fixation at the level of the fracture for treating acute thoracolumbar burst fracture. Methods:Twenty patients who sustained thoracic or lumbar fresh A3 burst fracture without neurologic deficits underwent minimally invasive pedicle screw fixation at the level of fracture and adjacent vertebrae. There were 12 males and 8 females with an average age of (50.3 ± 5.5) years (ranging 41-60 years). The injured vertebrae ranged from T11 to L2. Operation time, intraoperative blood loss and hospital stay were recorded. Visual analog scale (VAS) scores, Cobb angles and anterior vertebral body height were measured before surgery and immediately, 1, 2, 3, 6, 12 and 24 months after surgery. Results:The average operation time was (90±9) min (ranging 80-110 min). Blood loss was (68±17) ml during the procedure (ranging 40-95 ml). The average hospital stay was (6.7 ± 1.2) d (ranging 5-8 d). The duration of follow-up was (26 ± 2) months on average in all the patients (ranging 24-29 months). Compared with the preoperative ones, the VAS scores were significantly decreased immediately and 2 years after surgery (8.5±1.1, 1.9±0.6, 0.4±0.5, P<0.001), the Cobb angles were significantly decreased immediately and 2 years after surgery (22.0° ± 1.9° , 3.1° ± 1.8° , 6.1° ± 1.4° , P<0.001), and the com-pression rate of vertebral body height were also significantly decreased immediately and 2 years after surgery (49.3%±7.4%, 5.2%±1.6%, 7.0%±1.5%, P<0.001). No significant changes in both kyphosis correction and vertebral body height restora-tion were observed during follow-up. There was no patient with the failure of instruments during the follow-up. Conclusions:Minimally invasive short-segment posterior fixation at the level of fracture is a good choice for the treatment of acute thoracolumbar burst fracture.
Percutaneous Pedicle Screw Fixation Combined with Transpedicular Interbody Bone Grafting for the Treatment of Thoracolumbar Fractures

Abstract:Objective To investigate the efficacy of percutaneous pedicle screw fixation combined with transpedicular interbody bone grafting for the treatment of thoracolumbar fractures.Methods A total of 20 patients with thoracolumbar fractures (type A,load sharing ≥7,without neurological deficit) underwent percutaneous pedicle screw fixation of Sextant systems combined with transpedicular interbody bone grafting were retrospectively studied.The perioperative parameters,radiographic and clinical outcomes were compared with another 20 patients underwent traditional short-segment pedicle screw fixation combined with transpedicular interbody bone grafting.All patients were followed up for at least 2 years after surgery.Results There were no significant differences between the two groups in sex,age,injury to operation interval,and load sharing scores.However,there were significant differences between the two groups in operating time and blood loss (P＜0.05).The vertebral body height and kyphosis angle were corrected significantly after surgery (P＜O.05),however,the recovery of the vertebral body height and kypbosis angle were reduced significantly one year after surgery in both groups (P＜0.05).At the final follow-up,there were no significant progressive losses of the recovery of the vertebral body height and kyphosis angle,as compared with that of one year after surgery in two groups.There were no significant differences in radiographic parameters and the clinical outcomes between the two groups,even though less radiographic correction and better clinical outcomes were found in percutaneous techniques.Conclusion Short-segment pedicle screw fixation with transpedicular grafting remains a reliable surgical method for the treatment of severe thoracolumbar fractures without neurological deficit.Percutaneous pedicle screw fixation proved to be an effective method with the advantages of minimal invasiveness and less blood loss.
Safety and efficacy of short percutaneous fixation in AO3 and AO4 lumbar fractures: a single-centre experience of 35 cases

Abstract:Introduction. Spinal fractures with subsequent bone fragment dislocation are among the injuries most feared by patients and physicians. The surgical strategy is tailored to the individual patient's characteristics and often consists of pedicle instrumentation with rod-screw systems. Short instrumentation has been associated with worse spinal correction and increased complications. However, recent studies have suggested similar results in terms of kyphosis correction and the maintenance of sagittal alignment compared to longer instrumentation. Material and methods. This single-center retrospective study was conducted between January 2018 and April 2021. We included 35 single lumbar burst fractures AO Spine grade A3 or A4 with evidence of intra-canal fragments. Patients underwent minimally invasive percutaneous posterior lumbar instrumentation with pedicle screws. Patients received short segmental fixation involving only one level above and below the fractured vertebra. Results. An immediate postoperative computed tomography (CT) scan demonstrated a significant reduction in vertebral kyphotic deformation (11.7° ± 1.6 vs 16.7° ± 5, p<0.001) and sagittal Cobb angle (9.8° ± 1.3 vs 11.7° ± 1.5, p < 0.001). The correction was slightly reduced but remained significant at 12 months for both kyphotic (12.3° ± 1.4, p = 0.03) and sagittal Cobb (10.3° ± 0.9, p = 0.04). Upper lumbar vertebrae showed even larger correction indices compared to lower lumbar segments. No implant failure or screws pullout was seen at the last follow-up. Conclusions. Short spinal fixation is a safe and effective treatment of complete and incomplete burst fractures with posterior bone fragment dislocation. All included patients fared well and achieved good kyphotic correction with no perioperative or long-term complications.

Management of hangman’s fracture with percutaneous transpedicular screw fixation

Abstract:Purpose This study describes a percutaneous technique for C2 transpedicular screw fixation and evaluates its safety and efficacy in the treatment of patients with hangman’s fracture. Methods Ten patients with hangman’s fracture were treated by percutaneous C2 transpedicular screw fixation. There are six males and four females, who were, based on the classification of Levine and Edwards, sorted as follows: type I fracture, three cases; type II, five cases; type IIa, two cases. The causes of injury were road traffic accident in six patients and falling injury in four patients. Other associated lesions included rib fractures (7 patients), head injuries (4 patients), and fractures of extremities (6 patients). Results The new technique was performed successfully in all cases. The average operation time was 98 min (range 60–130 min) and the estimated blood loss was 25 ml (range 15–40 ml). No complications such as vascular or neural structures injuries were found intraoperatively. Postoperative CT scans demonstrated that 17 (85 %) of 20 screws were placed satisfactorily, and 3 (15 %) screws showed perforations of the pedicle wall (<2 mm). These patients were asymptomatic and no further intervention was required postoperatively. After 8–25 months follow-up (mean 15.3 months), solid fusion was demonstrated by computed tomography. All cases got well-sagittal alignment and no angulation or dislocation was found at the segment of C2–C3. There was no loss of fixation. Clinical examination showed a full range of motion in the neck in all patients. Conclusions The fluoroscopically assisted percutaneous C2 transpedicular screw fixation method is a technically feasible and minimally invasive technique for hangman’s fracture.

Abstract:Study Design: A prospective cohort Study on selected consecutivc patients. Objective: To evaluate the efficacy of an innovative operative technique called monosegmental transpedicular fixation for the treatment of some thoracolumbar burst fractures. Summary of Background Data: Short-segment pedicle screw instrumentation is accepted by many spinal surgeons as an acceptable technique for the treatment of thoracolumbar burst fractures. Preoperative evaluation using the spinal load-sharing makes this technique more reliable. To preserve more motion segments, some authors have advocated using monosegmental pedicle screw instrumentation (MSPI) to treat thoracolumbar fractures. However. up until now this kind of maneuver is only performed in cases of flexion distraction injuries. Methods: A cohort of 20 patients with thoracolumbar burst fractures fulfilling the inclusion criteria were prospectively submitted to surgical treatment of monosegmental transpedicular fixation plus posterior fusion. All instrumentations were performed with pedicle screws inserted bilaterally into the fractured level and I adjacent level, either superior or inferior depending on the locating side of the intact endplate. All patients were followed up. The preoperative radiographs, the postoperative radiographs within I week of operation, and the radiographs of the most recent follow-up were evaluated for kyphosis correction recorded in the Sagittal Index and Load-Sharing Classification (LSC) index. The postoperative functional outcomes were evaluated using the Frankel Performance Scale together with the Denis Pain Scale. Results: Eighteen patients were followed up successfully with an average final follow-up of 24.7 +/- 8.0 months. The focal kyphotic angulations were corrected satisfactorily with the mean Sagittal Index of preoperative 16.5 +/- 6.6 degrees, initial postoperative 4.0 +/- 2.4 degrees, and latest follow-up 4.8 +/- 4.0 degrees. No obvious loss of correction occurred except for 2 patients who both scored 8 points on the LSC Score. Post-operatively, most patients attained both functional neurologic improvement and pain relief. and only a few complications were noted. Conclusions: For selected thoracolumbar burst fractures, MSPI can provide the same or better fixation and preserve more motion segments than other methods of posterior pedicle instrumentation. With preoperative evaluation using the spinal LSC system, MSPI is effective and reliable for the treatment of thoracolumbar burst fractures when properly indicated.
Minimally invasive corpectomy and percutaneous transpedicular stabilization in the treatment of patients with unstable injures of the thoracolumbar spine: Results of retrospective case series

Abstract:Objective: The objective of this study was to analyze the results of surgical treatment of patients with unstable injuries of the thoracolumbar spine using simultaneous minimally invasive corpectomy and percutaneous transpedicular stabilization. Materials and methods: The retrospective study included 34 patients with isolated single-level unstable injuries of the thoracolumbar spine (5 or more points according to the Thoracolumbar Injury Classification and Severity Score (TLICS), operated on from the moment of injury from 8 to 24 h using the technique of minimally invasive corpectomy and percutaneous transpedicular stabilization simultaneously. The technical features of surgery, clinical data (pain level according to the Visual Analog Scale, quality of life according to the SF-36 questionnaire, subjective satisfaction with the operation according to the MacNab scale, and the presence of complications), and instrumental data (angle of segmental kyphotic deformity and sagittal index to and after surgery). The assessment of clinical data was carried out before surgery, at discharge, after 6 months, and in the long-term period, on average, 30 months after surgery. Results: When evaluating the clinical data, a significant decrease in the severity of pain syndrome was found on average from 90 mm to 5.5 mm in the late follow-up (P < 0.001), as well as a significant improvement in the physical and psychological components of health according to the SF-36 questionnaire on average from 28.78 to 39.26 (P < 0.001), from 36.93 to 41.43 (P = 0.006), respectively. In the long-term period, according to the MacNab scale, the patients noted the result of the operation: excellent - 18 (52.9%), good - 13 (38.3%), and satisfactory - 3 (8.8%); no unsatisfactory results were registered. Four (11.8%) perioperative surgical complications were registered, which were successfully treated conservatively. A significant restoration of the sagittal profile with an insignificant change in blood pressure was recorded in the long-term postoperative period. An average follow-up assessment of 30 months according to the American Spinal Injury Association scale showed the presence of E and D degrees in 85.4% of patients. Conclusion: Minimally invasive corpectomy with percutaneous transpedicular stabilization in the treatment of patients with unstable injuries of the thoracolumbar spine can effectively eliminate kyphotic deformity and prevent the loss of its reduction with a low number of postoperative surgical complications. The technique has minimal surgical trauma with the possibility of early postoperative rehabilitation and provides a significant stable reduction in vertebrogenic pain syndrome, improvement of neurological deficits, and restoration of the quality of life of patients and in the follow-up.
Treatment of Thoracolumbar Fracture by Combined Stabilization with Pedicle-screw and Vertebroplasty through Approach of Para-vertebral Muscle Sparing

Abstract:Objective To investigate the surgical technique and clinical outcome of treatment of acute traumatic thoracolumbar fractures through the approach of para-vertebral muscle sparing by transpedicular vertebroplasty with artificial cement and posterior instrumentation.Methods Thirty-one patients,22 males and 9 females,averaging 46 years of age with single level traumatic thoracolumbar fracture with light or without neurologic deficit were prospectively studied.These patients had 22 compression fractures and 9 burst fractures,located 3 at T11,9 at T12,14 at L1 and 5 at L2.They were treated by short-segment pedicle-screw reduction and fixation and augmentation of the injured vertebra with quick hardening artificial cement through the approach of para-vertebral muscle sparing.Vertebral central and anterior height,kyphotic angle of the vertebral fractures by radiographs and visual analog scale were calculated pre-operatively and post-operatively.Results All 31 patients were operated successfully and were followed for 12-36 months.None neurologic deterioration was observed.The operating time was averaged 95 min and blood loss was 150 mL.All patients reported significant pain alleviation shortly after operation.VAS scores was reduced from 8.97±0.66 to 3.23±0.98 and anterior vertebral body height compressional ratio improved from 44.58±6.71 before surgery to 5.43±1.63 after surgery,whereas the central vertebral body height was improved from 36.42±5.56 to 4.01±0.90; and sustained kypotic abnormality was corrected.Cement was ossified within 3-8 months after operation.There was no instrumentation failure or measurable loss of sagittal curve and vertebral body height correction in any group of patients.Conclusion Posterior instrumentation combing with transpedicular vertebroplasty with artificial cement through the approach of para-vertebral muscle sparing seems to b a feasible option in the management of some thoracolumbar fracture and is less invasive restoring vertebral anterior-middle column strength and decreasing the failure of instrumentation.
