Abstract:This paper presents the results of a retrospective cohort study of cancer incidence and mortality among emergency workers of the Chernobyl accident, for the follow-up period 1992–2009. The cohort selected for analysis consists of 67,568 emergency workers who worked in the Chernobyl exclusion zone in 1986–1987. External radiation whole-body absorbed dose varied from 0.0001 gray (Gy) to 1.24 Gy, with a median of 0.102 Gy. Over the follow-up period 1992–2009, a total of 4,002 solid cancers of different sites were identified as the result of annual compulsory health examination, and a total of 2,442 deaths from all solid cancers in the study cohort were reported. Poisson regression was applied for the analysis of cancer incidence and mortality. The analysis of the standardized incidence ratio (SIR) has shown a statistically significant increase in cancer incidence in the cohort as compared with baseline cancer incidence among males of Russia. The average excess over the entire follow-up period is 18 % [SIR = 1.18, 95 % confidence interval (CI) 1.15; 1.22]. In contrast, however, no increase in the mortality from all cancers among the emergency workers as compared to the baseline mortality in Russian men was found. Values of excess relative risk of cancer incidence and mortality per 1 Gy (ERR Gy−1) are 0.47 (95 % CI 0.03; 0.96, p value = 0.034) and 0.58 (95 % CI 0.002; 1.25, p value = 0.049), respectively. These values are statistically significant.

Assessment of radiation risks of malignant neoplasms among the population of Russian regions contaminated with radionuclides as a result of the Chernobyl accident

Abstract:Some areas of the Russian Federation were contaminated with radionuclides released as a re-sult of the Chernobyl accident. The current paper clarifies some items of the previously published study based on NRER (National Radiation-Epidemiological Registry, the Registry) data. The pre-vious issue presented estimates of radiation risks of cancer incidence among the population of four the most contaminated regions of the Russian Federation: Bryansk, Kaluga, Oryol and Tula oblasts. Risks of thyroid cancer are estimated with account of screening effect. The cohort con-sisted of more than 327 thousand people resided in the contaminated areas. The average dose to the thyroid of children and adolescents was 0.174 Gy, the average dose to the thyroid of adults was 0.035 Gy. Thyroid cancer was diagnosed in 1549 members of the cohort during the follow-up period from 1991 over 2019, among them 423 cases were children and adolescents at the accident and 1126 cases were adults. Significant increase in thyroid cancer incidence was ob-served in adults exposed internally through the intake of iodine-131 at a young age. The statisti-cally significant radiation risk of thyroid cancer persists over 30 years after exposure. The risk de-creases with increasing the attained age. In people exposed to radiation before the age of 1 year and reaching the age of 20 years, the coefficient of excess relative risk of thyroid cancer (ERR/Gy) was 4.61. Radiation associated thyroid cancer is attributed to 20.6% of the cohort mem-bers exposed internally through the intake of radioiodine isotopes in their childhood and adoles-cence, and thyroid cancer from intake of radioiodine is attributed to 58% of cases exposed to ra-diation under 4 years of age. Marked screening effect (ES) was found during the disease diag-nosing, screening coefficient was 5.6 for children and 2.1 for adults. There were not attributed to radiation statistically significant risks of solid cancers, including radiation induced breast cancer in females, in the cohort of people resided in the areas contaminated with radionuclides. Radia-tion risk of leukemia was also not identified, although the average value of excess relative risk in men exceeded zero. Further observations of the cohort of persons resided in areas contaminated with radionuclides will increase the statistical power of radiation-epidemiological studies and clarify the assessment of radiation risks of human for chronic exposure at low doses.
Method of estimation of radiation risks of mortality among Chernobyl clean-up workers with consideration to probable errors in cause-of-death reporting on death certificate

Abstract:In the past decade the lawful use of radiation risks of non-cancer diseases estimates to solve ra-diation safety problems has become more significant. The highest statistical power of low dose (less than 0.2 Gy) radiation risks research is possible in the cohort of the Chernobyl clean-up workers (liquidators) registered and been under medical monitoring in the system of the National Radiation Epidemiological Register (NRER) since 1986. The liquidators total number exceeds 130 thousand people. Because of the large-scale annual collection of health data, carried out in the NRER system, verification of the diseases diagnoses and medically certified causes of death, a natural question of radiation risk estimates compatibility with consideration to probable errors in a disease diagnosis and cause-of-death reporting on death certificates arises. The work is aimed at estimating the radiation risk of mortality from diseases of the circulatory system and diseases of the digestive organs in the Russian cohort of liquidators, with account of possible errors in the disease diagnosis and certified causes of death. The size of the cohort considered for mortality analysis was 91,013 people, the average age at the Chernobyl zone entry was 33.6 years, average radiation dose was 0.133 Gy. In this study, the method of maximizing the partial likelihood function was used to assess radiation risks, with consideration of individual radiation doses to liquidators and the assessment of background mortality using the Breslow method. The specificity of the diagnosis of the primary causes of death in the NRER system significantly exceeds the level of 0.99. Considering the specificity index of 0.99, the estimates of radiation risk coefficients ERR/Gy for mortality from diseases of the circulatory system and diseases of the digestive system in the cohort of Russian liquidators do not differ from the estimates obtained in the assumption that there are no first kind errors (false positive diagnosis) in the diagnoses of the primary causes of death in the NRER database. The results obtained confirm the high stability and validity of the assessments of radiation risks of mortality from diseases of the circulatory system and diseases of the digestive system, obtained earlier from the data on liquidators registered in the NRER.

Abstract:Purpose: Investigation of the influence of the possible uncertainty of exposure doses of Russian participants in the liquidation of the consequences of the Chernobyl accident on the assessment of radiation risks of the incidence of solid cancer in this cohort. Material and methods: Epidemiological and dosimetric data on a cohort of Russian participants in the liquidation of the consequences of the accident at the Chernobyl NPP, registered in the National Radiation and Epidemiological Register (NRER), are used as initial data for assessing radiation risks. The assessment of radiation risks is carried out by the statistical method of maximum likelihood in the framework of a linear non-threshold model of excess relative risk. Uncertainties in the liquidator’s exposure dose in the adopted risk assessment method are considered in the form of two error models. Dose estimates based on data from individual dosimeters are characterized by a classical model of measurement errors. In the case of estimates of unknown individual doses from group dosimetry data or group route doses, the Berkson assignment error model is used. Results: A method for assessing radiation risks has been developed, accounting for the uncertainty in dose estimates, based on the observed likelihood function. When taking into account the uncertainty of estimates of individual doses in the cohort of Russian liquidators, the estimate of the coefficient of the excess relative rate per dose unit (ERR/Gy) for the incidence of solid malignancies decreases by 7%, compared with the estimate obtained directly from the doses registered in the NRER database. The ERR/Gy estimate derived from the doses recorded in the NRER database was 0.69 with a 95% confidence interval (CI) of 0.37–1.04. The estimate of ERR/Gy, obtained accounting for the uncertainty in estimates of individual doses of liquidators, was 0.64 at 95% CI (0.33–0.98). This estimate bias is not significant, since it is within 95% CI for both ERR/Gy estimates, the statistical range of which is of the order of magnitude of the estimates themselves. Conclusions: Considering the uncertainty of individual dose estimates in the cohort of Russian liquidators, the estimate of the excess relative rate per dose unit (ERR/Gy) for the incidence of solid cancer does not statistically significantly differ from the estimate obtained directly from the doses registered in the NRER database. The bias in the estimate of the radiation risk coefficient observed, due to the dose uncertainty introduced into the calculation, is due to the statistical properties of the traditional radiation risk models used for radiation epidemiology. The results obtained confirm the high stability and validity of the radiation risk assessments obtained earlier from the doses registered in the NRER for the Russian cohort of Chernobyl liquidators. Further research will allow generalization of the developed method for assessing radiation risks, accounting for the uncertainty of dose estimates, based on the observed likelihood function, to other types of radiation epidemiological risk studies, including case-control and case-cohort studies.
Prospective lifetime radiation risk of cancer among Russian cleanup workers in Chernobyl

Abstract:As stated in the Basic sanitary rules of radiation safety, radiation risk is one of the main character-istics of radiation safety. Before the work persons involved in cleanup operations after radiation accidents should be informed about subsequent risks to their health. However, mathematical models for radiation risk prediction and methods for its calculation are currently at the stage of scientific research and have not yet been standardized for solving practical problems of risk pre-diction. At the international level, UNSCEAR, ICRP and WHO developed radiation risk models on the basis of radiation-epidemiological data on survivors of Hiroshima and Nagasaki atomic bombings in August 1945. In recent decades, data on radiation-epidemiological follow-up of co-horts contained people exposed to radiation after the Chernobyl accident in 1986, in particular NRER data, have been used to identify and assess radiation risks. The radiation risk models iden-tified from the Chernobyl cohorts of exposed individuals differ from the models identified from the Japanese cohort, which leads, respectively, to different projections of lifetime radiation risk. The purpose of this work is to compare quantitatively the possible lifetime radiation risks of can-cer for a cohort of Russian liquidators of the Chernobyl accident, evaluated with radiation risk models developed by UNSCEAR, WHO and NRER. It is shown that after 2020, 1297 cases of cancer are expected in the cohort of Russian liquidators, this figure is 2.4 times higher than esti-mates obtained with the international models. The radiation risk model of leukemia, built on the basis of NRER follow-up data, predicts 145 cases of leukemia in the observed cohort of liquida-tors, which is 8 times higher than the values estimated with the use of international models. Since the liquidators of the considered cohort are generally over 50 years old, the results obtained may indicate the need to adjust the dose limits established by the current radiation safety standards based on ICRP risk models for people over 50 years old.
Impact of radiation on the atherosclerosis incidence in men participated in mitigation of the consequences of the accident at the Chernobyl NPP

Abstract:The statistical relationship between radiation dose to the cohort of workers (liquidators), participated in mitigation of effects of the Chernobyl accident are considered in the paper. The follow-up data collected from 1992 to 2021 and accumulated in the National Radiation and Epidemiological Register (NRER) were used for analysis. The cohort of liquidators consisted of 67,304 males. The average radiation dose in the cohort was 0.13 Gy, and the average age at radiation exposure was 34 years. During the follow-up period 10,790 atherosclerosis cases (ICD-10, code 170) were detected for the first time. To identify statistical relationship between radiation dose and atherosclerosis incidence, general “data mining” approaches were used, that do not use hypotheses about the probabilistic laws of random variable distributions, that determine the relationship between radiation dose and atherosclerosis incidence. A group of liquidators with accumulated during the period of work individual radiation doses below 0.15 Gy was selected as the reference group. The maximum radiation dose in the studied cohort was 1.4 Gy. Statistically significant relationship between radiation doses and atherosclerosis incidence (ICD-10, code 170) were found: relative radiation risk RR=1.05 at 95% lower confidence bound 1.027. The relative risks (RR) for individual diagnoses are: RR=1.22 (95% CI lower bound – 1.20) for ICD-10 – I70.0 (aortic atherosclerosis); RR=1.03 (95% CI lower bound – 1.01); for ICD-10 – I70.2 (limb artery atherosclerosis); RR=1.06 (95% CI lower bound – 1.04) for ICD-10 – I70.9 (generalized and unspecified atherosclerosis). The RR increases with the age at diagnosis. The results obtained can be the basis for the design of radiation-epidemiological studies for the detailed quantitative study of the detected dose-effect relationships.

Abstract:The paper provides research on statistical relationship between endocrine diseases registered in the Chernobyl liquidators and radiation dose they received. Data of clinical and epidemiological observations accumulated at the National Radiation Epidemiological Register (NRER) for the period from 1986 to 2020 were used for analysis. The cohort contained 41,979 liquidators with 107,713 established diagnoses included in the ICD-10 Е00-E90 blocks. Statistical relationship between radiation doses and endocrine diseases was investigated with the use of statistical methods. Statistical Data Mining methods free of apriori guesses for probable doses and diagnoses distribution, models of dose-effect (disease) relationship were used for analysis of statistical relationship between radiation dose and the diagnoses structure. Statistically significant relationship between radiation doses and diagnoses were found in the blocks E01 (iodine deficiency), E04 (goiter), E06 (thyroiditis), E11 (type II diabetes mellitus). Relationships between radiation dose and diagnosis were also found in the E01.2 block (endemic goiter, caused due to iodine deficiency) and in E04.0 block (non-toxic diffuse goiter). Existence of identified statistical relationship is supported with the estimates of excess relative risk (ERR) and the relative risk (RR). The threshold relationship between RR and radiation dose with threshold of 0.3 Gy shows that radiation effect on human endocrine system is a deterministic effect (tissue reactions). Quantitative risks estimates for endocrine diseases in the liquidators are stable in relation to the diagnostic specificity index up to 90%. The general statistic Data Mining method to search relationship between a disease diagnosis and radiation dose is useful for the diseases identification in order to estimate radiation risk with conventual radiation epidemiology approaches.

Abstract:Currently, the system of standards and rules of radiation protection considers the need to con-straint the absorbed radiation doses in order to prevent the development of deterministic effects of ionizing radiation on humans. Deterministic effects in terms of mortality include bone marrow syndrome with a dose threshold of 1 Gy, gastrointestinal syndrome and pneumonitis with dose thresholds of 6 Gy. For inclusion in international radiation safety standards, a threshold of 0.5 Gy for cardiovascular and cerebrovascular diseases is being discussed. The wide experience of ra-diation-epidemiological studies shows that the deterministic effects of mortality are not limited to those effects that are currently considered in the system of standards and rules of radiation pro-tection. This paper presents estimates of radiation risks of mortality from diseases of the circula-tory system (CSD) and the digestive system, as well as dose thresholds corresponding to these effects, in the Russian cohort of liquidators of the Chernobyl accident registered in the National Radiation and Epidemiological Register (NRER), who have registered doses from external whole-body exposure to gamma radiation within 1 Gy. The risks were estimated over the follow-up peri-od 1986-2020. The stability of estimated radiation risks of mortality from CSD and from the di-gestive system diseases in relation to the uncertainty of personal doses received by liquidators has been studied. The absorbed doses received by 11% of the liquidators were estimated from the data of personal dosimeters, the maximum error was about 50%. For the remaining 89% of liquidators, group and route doses were registered, and the maximum uncertainty of individual doses was about 500%. For CSD mortality, the excess relative rates per 1 Gy (ERR/Gy) were 0.361 and 0.349, excluding and considering dose uncertainties, respectively. For mortality from diseases of the digestive system, ERR/Gy=0,791 excluding dose uncertainty and ERR/Gy=0,726 with considering dose uncertainty. Uncertainties in individual dose estimates result in 5% and 8% reductions in ERR/Gy estimates (for CSD mortality and digestive disease, respectively). This re-duction in risk estimates is not related to a bias in the dose estimates. It is caused by the statistical properties of radiation risk models in conventional radiation epidemiology. If the excessive mor-tality from the studied causes is attributed to deterministic effects, then the estimates of the corre-sponding dose thresholds, considering the uncertainty of individual doses, also increase quite slightly: for CSD – from 0,028 Gy to 0,029 Gy, and for diseases of the digestive system – from 0,013 Gy to 0,014 Gy. The obtained results confirm the high stability and validity of radiation risk assessments of non-oncological diseases obtained earlier from the doses of Russian partici-pants in the liquidation of the consequences of the Chernobyl accident registered in the unified federal database of the NRER.
Radiation Risk Analysis of Neuropsychiatric Disorders in Ukrainian Chornobyl Catastrophe Liquidators

Abstract:Goal: To explore the possible impact of ionizing radiation in the pathophysiology of neuropsychiatric disorders amongst clean-up workers of the Chornobyl catastrophe (liquidators). Design, object, and methods: Retrospective-prospective study (1987-2015) of liquidators from the State Register of Ukraine (SRU) with radiation doses records and Clinical-Epidemiological Register (CER) of the State Institution ≪National Research Center for Radiation Medicine of the National Academy of Medical Sciences of Ukraine≫ (NRCRM). Moreover, cohort and cross-sectional studies of the randomized sample of liquidators from the CER (exposed group, 198 subjects) were examined. Internal control group included the liquidators irradiated in doses <50.0 mSv (42 persons). All subjects were assessed by a detailed clinical examination and a battery of standardized neuropsychiatric scales, psychometric, and neuropsychological tests. Descriptive and variation statistics, non-parametric criteria, regression-correlation analysis, survival analysis by Kaplan & Meier, and risk analysis were used. Results: Exposed group vs. control group showed cognitive disorders in 99 (50.0%) vs. 20 (18.1%), (P = 0.04); affective disorders in 96 (48.3%) vs. 36 (32.7%) (P = 0.007), and stress-related disorders in 115 (58.4%) vs. 8 (7.3%) (P < 0.001). In the main group exposed to ≥50 mSv vs. internal control group (exposed to <50 mSv), affective disorders were present, respectively, in 89 (56.4%) vs. 7 (19.1%) (P < 0.001), and stress-related disorders in 98 (62.8%) vs. 17 (40.4%) (P = 0.009). Relative risks (RR) and 95% confidential intervals (95%CI) of Incidence of some neuropsychiatric disorders in liquidators of 1986-1987 related to internal control (doses <50 mSv) were as follows: organic psychosis (RR = 3.15; 95% CI: 2.6; 3.7); non-psychotic organic brain damage (RR = 1.99; 95% CI: 1.6; 2.5); acute (RR = 1.40, 95% CI: 1.3; 1.5), and chronic cerebrovascular disorders (RR = 1.23; 95% CI 1.0;1.5). Neuropsychiatric diseases show a strong, increasing, and approximately quadratic statistically significant (Pv < 0.001) relationship with individual dose, yielding an estimated excess relative risk ERR = 2.76 Sv-2 (95% CI 1.06-7.15). Conclusions: Liquidators have an excess of cognitive, affective, and stress-related disorders. The risk of diseases rises with radiation dose. Radiation risks are revealed for organic psychoses, non-psychotic organic brain damage, acute and chronic cerebrovascular pathology.
Dose Reconstruction for Epidemiological Studies among Ukrainian Chernobyl Cleanup Workers

Abstract:The present paper provides an overview of the methods and summarizes the results of estimating radiation doses and their uncertainties for Ukrainian-American epidemiological studies among the Chernobyl (Chornobyl) cleanup workers. After the Chernobyl accident occurred on April 26, 1986, more than 300,000 Ukrainian cleanup workers took part between 1986 and 1990 in decontamination and recovery activities at the site of the Chernobyl Nuclear Power Plant. The U.S. National Cancer Institute in collaboration with the Ukrainian National Research Center for Radiation Medicine conducted several epidemiological studies in this population. An important part of these studies was the reconstruction of the study participants' radiation doses and the assessment of uncertainties in doses. A method called realistic analytical dose reconstruction with uncertainty estimation (RADRUE) was used to calculate the doses from external irradiation during cleanup missions, which was the main exposure pathway for most study participants. At the initial phase of the accident during the atmospheric releases of radioactivity from the destroyed reactor, the cleanup workers also received doses from inhalation of radionuclides. In addition, study participants received doses at their places of residence, especially those who lived in highly contaminated areas. The radiation doses estimated for 2,048 male cleanup workers included in the Ukrainian-American epidemiological studies varied widely: (i) bone-marrow doses from external irradiation in the case-control study of leukemia of 1,000 cleanup workers ranged from 3.7 × 10-5 mGy to 3.3 Gy (mean = 92 mGy); (ii) thyroid doses in the case-control study of thyroid cancer in 607 persons from all exposure pathways combined were from 0.15 mGy to 9.0 Gy (mean = 199 mGy); (iii) gonadal doses in 183 cleanup workers from all exposure pathways combined in the study of germline mutations in the offspring after parental irradiation (trio study) ranged from 0.58 mGy to 4.1 Gy (mean = 392 mGy); (iv) thyroid doses in the human factor uncertainties study among 47 persons were from 20 mGy to 2.1 Gy (mean = 295 mGy); and (v) lung doses in the study of germline genetic variants associated with host susceptibility to COVID-19 estimated for 211 cleanup workers were from 0.024 mGy to 2.5 Gy (mean = 249 mGy). Doses of female cleanup workers were much lower than those of male cleanup workers: the mean doses for female cleanup workers were 27 mGy for 34 women included in the trio study and 56 mGy for 48 women participated in the study of germline genetic variants associated with host susceptibility to COVID-19. Uncertainties in dose estimates included two components: (i) inherent uncertainties arising from the stochastic random variability of the parameters used in exposure assessment and from a lack of knowledge about the true values of the parameters; and (ii) human factor uncertainties due to poor memory recall resulting in incomplete, inaccurate, or missing responses during personal interviews with cleanup workers conducted long after exposure. This paper also discusses possible developments and improvements in the methods to assess the radiation doses and associated uncertainties for cleanup workers.
The Russian radiation legacy: its integrated impact and lessons

Abstract:Information about the consequences of human exposure to radiation in the former Soviet Union has recently become available. These data add new insights and provide possible answers to several important questions regarding radiation and its impact on occupational and public health. The 1986 Chernobyl accident initiated a major and early increase in childhood thyroid cancer that resulted from ingestion of iodine-131 (131I) by young children living in the most heavily contaminated areas of Belarus, Ukraine, and Russia. No significant additional cancer or other adverse medical effects have yet been reported in the affected populations and among clean-up workers. Major psychological stress independent of radiation dose has been observed in those people thought to be exposed. During the early days of the atomic energy program in the former Soviet Union, some unfortunate events occurred. The country's first atomic test in Semipalatinsk in 1949 exposed over 25,000 people downwind from the blast to significant doses of fission products, especially 131I. During the late 1940s and the early 1950s nuclear material production facilities were developed near Chelyabinsk in the South Ural Mountains, which resulted in major releases into the environment and significant overexposures for thousands of workers and nearby populations. Chronic radiation sickness was observed early in exposed workers, and increases in leukemia and other cancers were also reported. The series of plutonium inhalation-related lung cancers and fatalities among workers exposed in that first decade appears to be unique. Long-term consequences of chronic radiation sickness and four decades of follow-up are being described for the first time. Villagers downstream from the plant consumed high levels of 137Cs and 90Sr and, it is reported, manifested increases in leukemia from internal and external exposures. Although the 40-year databases for retrospective dosimetry epidemiology studies are just beginning to be integrated and evaluated, preliminary evaluations suggest that there may be graded, significant dose-rate amelioration factors for cancer and leukemia risks in workers and the general population relative to the risk data on the Japanese atomic bomb survivors. Even for plutonium-induced lung cancers in workers, such a dose-rate effect may be evident. These experiences give us insight into the consequences of protracted radiation at high and low doses and rates. If these findings are validated and confirmed, they can provide information that reduces some of the uncertainties in retrospective radiation dosimetry and radiation risk estimates (especially for low-level, chronic exposures) for activities related to medicine as well as the handling of nuclear materials and nuclear facility decommissioning, decontamination, and demilitarization.
Chernobyl cleanup workers from Estonia: cohort description and related epidemiological research

Abstract:The Estonian study of Chernobyl cleanup workers was one of the first investigations to evaluate the possible health consequences of working in the Chernobyl area (the 30 km exclusion zone and/or adjacent territories) after the 1986 reactor accident. The cohort consists of 4831 men who were dispatched in 1986-1991 for tasks involving decontamination, construction of buildings, transport, radiation measurement, guard duty or other activities. By 31 December 2012, the follow-up of the cohort yielded 102 158 person-years of observation. Exposure and health data were collected by postal questionnaires, biodosimetry evaluations, thyroid screenings, and record-linkages with cancer, causes of death and health insurance reimbursement registers and databases. These data cover socio-demographic factors, employment history, aspects of health behaviour, medical history, work and living conditions in the Chernobyl area, biomarkers of exposure, cancer and non-cancer disease occurrence and causes of death. Cancer incidence data were obtained for 1986-2008, mortality data for 1986-2011 and non-cancer morbidity data for 2004-2012. Although the cohort is relatively small, it has been extensively examined and benefited from comprehensive nationwide population and health registers. The major finding was an increased risk of suicide. Thyroid examinations did not reveal an association with thyroid nodular disease and radiation dose, but did indicate the importance of accounting for screening when making comparisons with unscreened populations. No risk of leukaemia was observed and risks higher than 2.5-fold could be excluded with 95% confidence. Biodosimetry included GPA analyses and chromosomal translocation analyses and indicated that the Estonian cleanup workers experienced a relatively low mean exposure of the order of 0.1 Gy. One value of the Estonian study is in the methodologic processes brought to bear in addressing possible health effects from the Chernobyl accident. Twenty-five years of research are summarised and opportunities for the future listed.

Abstract:Purpose: To give a general idea of the most common combinations of diseases in the male population of the Russian Federation and to identify the combinations of diseases, the development of which could be influenced by ionising radiation. Material and methods: Cohort study of the influence of external gamma irradiation on multimorbidity of the Chernobyl accident liquidation participants according to the data of the National Radiation Epidemiological Registry. Multimorbidity was defined as the presence in one participant of the cohort of two or more diseases from the list consisting of ten groups of diagnoses: diseases of lower respiratory tract, diseases of musculoskeletal system, diseases of endocrine system, mental disorders, oncology, neurology, diseases of digestive organs, diseases of cardiovascular system, diseases of genitourinary system, diseases of sense organs. The cohort consisted of men born between 1919 and 1969 who worked in the accident zone from 1986 to 1987 and had a documented whole-body external gamma dose. Cohort follow-up period: 1992–2021. The cohort size at the beginning of the follow-up was 59 290 people. The study participants were divided into two groups according to external gamma dose: up to 150 mGy – 34602 individuals, 150 mGy and more – 24 688 individuals. For all possible combinations of diagnoses, relative radiation risk was calculated as a measure of the association of exposure with diseases. The relative radiation risk was considered statistically significant if the left border of the one-sided 95 % confidence interval was greater than 1. Statistical analyses were performed using the R programming language for statistical computing and the R arules package. Results: During 30 years of follow-up, multimorbidity was noted in 62 % of individuals. The most common combinations of chronic diseases were combinations of cardiovascular diseases with digestive diseases (23 % of the original cohort), with lower respiratory diseases (22 %) and with musculoskeletal diseases (18 %). A combination of all four diagnosis groups was identified in 5 % of individuals. Nineteen multimorbid combinations with statistically significant relative radiation risk in the range (1.07–1.23) were identified. Conclusions: No effect of radiation exposure on the number of individuals with multimorbidity was found, but in the studied cohort there are individuals with cardiovascular diseases, endocrine diseases, oncology and combinations of these groups of diagnoses with diseases of other body systems may be caused by radiation exposure. The statistically significant relative radiation risk for combinations of diagnoses is, in general, greater than for the individual diseases that make up these combinations.

Abstract:Purpose: To evaluate the evidence for the correlation between radiation exposure of the population following the Chernobyl accident and cancer mortality using statistical links. Materials and methods: Data on deaths and causes of death for the period between 1993 and 2017 used in the study were taken from the National Radiation Epidemiological Registry (NRER). The number of deaths in males is 30771 persons including 5407 cancer deaths, the number of deaths in females – 29033 persons including 3472 cancer deaths. For analysis of statistical links between radiation exposure and causes of death data mining algorithms free of a priori statements on probabilistic distributions of doses and diagnoses were used. Tables of contingency of deaths in two dose groups: the group 0 – <0.014 Sv and the group 1 – ≥0.014 Sv; and in three age groups: group 0 – <17 years old, group 1 – 18–60 years old, group 2 – 60+ years old were used for analysis. About 90 % of individual effective doses were accumulated from 1986 till 1993. Mean effective dose is 0.024 Sv. Results: For the population of four areas of the Russian Federation which are most contaminate after to accident on the CNPP significant association of all causes of death from cancer and for three-digit headings ICD-10 with an exposure dose was not found. Statistically significant association between dose and pancreatic cancer (C25.9 ICD-10) and stomach cancer (С16.9) in males, stomach cancer (С16.9) and malignant neoplasms of bronchus and lungs (C34.9) in females was found. The association between dose and cancer death was demonstrated in 150–230 cases. These diagnoses of causes of death first of all have to be a subject of more sensing and specific radiation and epidemiological analysis considering possible the confounding factors Conclusion: The presented method is exploratory in nature and can be used to find a direction of research necessary for making more accurate evaluation of epidemiological evidence for the correlation between dose and effect. The method may be useful for evaluation of the association between radiation exposure and pathological response (death).

Abstract:The aim of the present study was to assess the risk of primary central nervous system (CNS) tumour incidence in a cohort of 22,377 Mayak Production Association workers chronically exposed to ionising radiation. There were 96 primary CNS tumours, including 42 cases of glioma and 44 cases of meningioma, registered during the whole follow-up period (1948–2018). The study demonstrated that the risk of primary CNS tumour incidence was associated with sex, attained age, calendar period, tall body height, age at the beginning of exposure, and facility type. There was no association found between risk of CNS tumour incidence and body mass index, smoking (males) and alcohol consumption status. The study did not find an effect of the total external gamma radiation dose absorbed in the brain on risk of CNS tumour incidence irrespective of whether an adjustment for the total external neutron dose absorbed in the brain was included or not. Excess relative risk per 1 Gy of external gamma brain dose was 0.05 (95% confidence interval (CI) −0.30; 0.70) for all CNS tumours, −0.18 (95% CI –; 0.44) for gliomas, and 0.38 (95% CI −0.32; 2.08) for meningiomas without adjustment for total neutron brain dose. There was no effect modification by sex, attained age, age at hire or facility.

Abstract:This study considers the exposure of the population of the most contaminated Gomel and Mogilev Oblasts in Belarus to prolonged sources of irradiation resulting from the Chernobyl accident. Dose reconstruction methods were developed and applied in this study to estimate the red bone-marrow doses (RBMs) from (i) external irradiation from gamma-emitting radionuclides deposited on the ground and (ii) 134 Cs, 137 Cs and 90 Sr ingestion with locally produced foodstuffs. The mean population-weighted RBM doses accumulated during 35 years after the Chernobyl accident were 12 and 5.7 mGy for adult residents in Gomel and Mogilev Oblasts, respectively, while doses for youngest age groups were 20–40% lower. The highest mean area-specific RBM doses for adults accumulated in 1986–2021 were 63, 56 and 46 mGy in Narovlya, Vetka and Korma raions in Gomel Oblast, respectively. For most areas, external irradiation was the predominant pathway of exposure (60–70% from the total dose), except for areas with an extremely high aggregated 137 Cs soil to cow's milk transfer coefficient (≥ 5.0 Bq L −1 per kBq m −2 ), where the contribution of 134 Cs and 137 Cs ingestion to the total RBM dose was more than 70%. The contribution of 90 Sr intake to the total RBM dose did not exceed 4% for adults and 10% for newborns in most raion in Gomel and Mogilev Oblasts. The validity of the doses estimated in this study was assessed by comparison with doses obtained from measurements by thermoluminescence dosimeters and whole-body counters done in 1987–2015. The methodology developed in this study can be used to calculate doses to target organs other than RBM such as thyroid and breast doses. The age-dependent and population-weighted doses estimated in this study are useful for ecological epidemiological studies, for projection of radiation risk, and for justification of analytical epidemiological studies in populations exposed to Chernobyl fallout.

Abstract:The increased risk of liver malignancies was found in workers of the first Russian nuclear production facility, Mayak Production Association, who had been chronically exposed to gamma rays externally and to alpha particles internally due to plutonium inhalation. In the present study, we updated the radiogenic risk estimates of the hepatobiliary malignancies using the extended follow-up period (1948-2018) of the Mayak worker cohort and the improved «Mayak worker dosimetry system-2013». The cohort comprised 22,377 workers hired at the Mayak PA between 1948 and 1982. The analysis considered 62 liver malignancies (32 hepatocellular carcinomas, 13 intrahepatic cholangiocarcinomas, 16 angiosarcomas, and 1 anaplastic cancer) and 33 gallbladder adenocarcinomas. The analysis proved the positive significant association of the liver malignancy risk (the total of histological types, hepatocellular carcinoma) with the liver absorbed alpha dose from internal exposure. The excess relative risk per Gy (95% confidence interval) of alpha dose (the linear model) was 7.56 (3.44; 17.63) for the total of histological types and 3.85 (0.95; 13.30) for hepatocellular carcinoma. Indications of non-linearity were observed in the dose-response for internal exposure to alpha radiation. No impact of external gamma-ray exposure on the liver malignancy incidence was found. In the study cohort, the number of angiosarcomas among various types of liver malignancies was very high (25.8%), and most of these tumors (73.3%) were registered in individuals internally exposed to alpha radiation at doses ranging between 6.0 and 21.0 Gy. No association with chronic occupational radiation exposure was observed for the incidence of gallbladder malignancies.

Abstract:To address points arising from the recent study of nuclear workers in the USA and the International Nuclear Workers Study (INWORKS), concerning the difference in solid cancer risk estimates between those first hired in earlier and later calendar years, subsidiary analyses were conducted on a cohort of 172,452 workers in the National Registry for Radiation Workers (NRRW) from the UK. A total of 18,310 incident first primary solid cancer cases were registered in the period from 1955 until 2011 in the NRRW cohort and workers accrued 5.25 million person-years of follow-up. Incidences rates of all solid cancers combined, lung cancer and solid cancer excluding lung cancer were examined in terms of external radiation doses in the full cohort and in a sub-cohort of workers who had no record of internal exposure monitoring and were defined by the periods of first hire before and after the beginning of the years 1960, 1965 and 1970. All analyses were carried out using Poisson Regression. These analyses demonstrated that only for lung cancer between the pre-1965 and post-1964 periods is there strong evidence for a difference in the risks using the NRRW full cohort. In the other calendar period breakdowns and for the other cancer groups, there is no clear evidence of differences in the risks. The NRRW estimation of risks between recent and early workers is not generally consistent with the US workers cohort or the INWORKS evaluations that later hired workers are at much higher solid cancer risk than earlier hired workers, although INWORKS contains a significant part of the latest updated NRRW cohort as well as the US data. The conclusion that the INWORKS and US study data demonstrate a real difference in excess solid cancer risk from external radiation exposure between earlier and later workers is premature. The results presented here should also be treated with caution because of the limited corroborating evidence from other published studies. Information on internal doses, neutron doses as well as non-radiation factors such as smoking and asbestos exposure would be needed to make definitive inferences.

Abstract:This study evaluates the reliability of information obtained by standardized questionnaires used in by personal interviews for estimation of radiation thyroid doses of 1065 individuals in the Belarusian cohort of individuals who were exposed in utero and early life following the Chernobyl accident in April 1986. Data from two interviews conducted in 2012-2017 and in 2018-2022 with mothers, who were pregnant or gave birth shortly after the Chernobyl accident, were analysed. The most reliable answers dealt with various attributes related to residential history. In contrast, the reliability of answers regarding consumption rates of milk from privately owned cows or trade network was moderate, while the agreement in responses for consumption of milk products and leafy vegetables was fair. Information from the two interviews was used to calculate thyroid doses received by the cohort members. Specifically, 'model-based' thyroid doses due to 131I were estimated using input data on individual residential history and food consumption reported during the personal interviews and ecological data (131I ground deposition in the corresponding settlements). In addition, for a subset of cohort subjects (n = 205) whose mothers were measured for 131I thyroid activity, 'measurement-based' thyroid doses were calculated by adjusting the model-based dose using a scaling factor that is defined as the ratio of measured 131I thyroid activity to model-based 131I thyroid activity calculated for the date of measurement. A moderate agreement was observed for total (prenatal and postnatal) model-based thyroid doses due to 131I intake, the arithmetic mean ± standard deviation for the Jaccard similarity coefficient ([Formula: see text]) was 0.45 ± 0.34 (median = 0.39), while measurement-based doses showed a much better agreement with a [Formula: see text] of 0.78 ± 0.29 (median = 0.93). For model-based thyroid doses from external irradiation and from ingestion of 134Cs and 137Cs, [Formula: see text] was 0.82 ± 0.23 (median = 0.90) and 0.84 ± 0.24 (median = 0.96), respectively. Measurement-based doses due to ingestion of radiocaesium isotopes resulted in an almost perfect agreement, [Formula: see text] was 0.91 ± 0.19 (median = 1.0). The present findings suggest that long-term memory recall can be reliable, if a person is asked about unique or important life events, such as pregnancy and childbirth occurring around the time of a nuclear reactor accident. However, the substantial difference (more than 10 times) observed for model-bases doses calculated using the two questionnaires represents an important source of human factor uncertainties that needs to be considered in any dose response analyses. Other lessons learned from this study are that (i) individual measurements of radionuclides in the human body are the most valuable source of information for estimating radiation doses, and (ii) whenever a radiation accident occurs, a sample of affected people should be asked to keep a diary, if at all possible.
