Abstract:Abstract Dengue fever (DF), caused by the dengue virus (DENV), is a major public health problem worldwide, with approximately 390 million reported annual infections. The lack of specific antiviral therapies and limited means of vector control has led to the development of a dengue vaccine as a promising strategy to combat this disease. The purpose of this review is to provide a comprehensive analysis of the current status and prospects of dengue vaccines. This review describes different approaches used in dengue vaccine development, including live-attenuated, inactivated, subunit, and viral vector vaccines. Each approach is evaluated for immunogenicity, safety, and efficacy based on preclinical and clinical studies, highlighting the strengths and limitations of each vaccine candidate. Future directions and research priorities in dengue vaccine development are highlighted. In conclusion, dengue vaccines have great potential to reduce the burden of DF worldwide. However, some challenges remain regarding vaccine safety, efficacy, delivery, and availability. By addressing these challenges and leveraging advances in vaccine technology, there can be more effective control and prevention of DF and protection of public health, leading to improvement in the quality of life.

Abstract:According to recent statistics, 96 million apparent dengue infections were estimated worldwide in 2010. This figure is by far greater than the WHO prediction which indicates the rapid spread of this disease posing a growing threat to the economy and a major challenge to clinicians and health care services across the globe particularly in the affected areas.This article aims at bringing to light the current epidemiological and clinical status of the dengue fever. The relationship between genetic mutations, single nucleotide polymorphism (SNP) and the pathophysiology of disease progression will be put into perspective. It will also highlight the recent advances in dengue vaccine development.Thus far, a significant progress has been made in unraveling the risk factors and understanding the molecular pathogenesis associated with the disease. However, further insights in molecular features of the disease and the development of animal models will enormously help improving the therapeutic interventions and potentially contribute to finding new preventive measures for population at risk.

Abstract:PURPOSE For external beam in vivo measurements, the dosimeter is normally placed on the patient's skin, and the dose to a point of interest inside the patient is derived from surface measurements. In order to obtain accurate and reliable measurements, which correlate with the dose values predicted by a treatment planning system, a dosimeter needs to be at a point of electronic equilibrium. This equilibrium is accomplished by adding material (buildup) above the detector. This paper examines the use of buildup caps in a clinical setting for two common detector types: OSLDs and diodes. Clinically built buildup-caps and commercially available hemispherical caps are investigated. The effects of buildup cap thickness and fabrication material on field-size correction factors, C(FS), are reported, and differences between the effects of thickness and fabrication material are explained based on physical parameters. METHODS Measurements are made on solid water phantoms for 6 and 15 MV x-ray beams. Two types of dosimeters are used: OSLDs, InLight∕OSL Nanodot dosimeters (Landauer, Inc., Glenwood, IL) and a P-type surface diode (Standard Imaging, Madison, WI). Buildup caps for these detectors were fabricated out of M3, a water-equivalent material, and sheet-metal stock of Al, Cu, and Pb. Also, commercially available hemispherical buildup caps made of plastic water and brass (Landauer, Inc., Glenwood, IL) were used with Nanodots. OSLDs were read with an InLight microStar reader (Landauer, Inc., Glenwood, IL). Dose calculations were carried out with the XiO treatment planning system (CMS∕Elekta, Stockholm) with tissue heterogeneity corrections. RESULTS For OSLDs and diodes, when measurements are made with no buildup cap a change in C(FS) of 200% occurs for a field-size change from 3 cm × 3 cm to 30 cm × 30 cm. The change in C(FS) is reduced to about 4% when a buildup cap with wall thickness equal to the depth of maximum dose is used. Buildup caps with larger wall thickness do not cause further reduction in C(FS). The buildup cap fabrication material has little or no effect on C(FS). The perturbation to the delivered dose caused by placing a detector with a buildup cap on the surface of a patient is measured to be 4%-7%. A comparison between calculated dose and dose measured with a Nanodot and a diode for 6 and 15 MV x-rays is made. When C(FS) factors are carefully determined and applied to measurements made on a phantom, the differences between measured and calculated doses were found to be between ±1.3%. CONCLUSIONS OSLDs and diodes with appropriate buildup caps can be used to measure dose on the surface of a patient and predict the delivered dose to depth dmax in a range of ±1.3% for 100 cGy. The buildup cap: can be fabricated from any material examined in this work, is best with wall thickness dmax, and causes a perturbation to the delivered dose of 4%-7% when the wall thickness is dmax. OSLDs and diodes with buildup caps can both give accurate measurements of delivered dose.

Abstract:Dengue is currently the highest and rapidly spreading vector-borne viral disease, which can lead to mortality in its severe form. The globally endemic dengue poses as a public health and economic challenge that has been attempted to suppress though application of various prevention and control techniques. Therefore, broad spectrum techniques, that are efficient, cost-effective, and environmentally sustainable, are proposed and practiced in dengue-endemic regions. The development of vaccines and immunotherapies have introduced a new dimension for effective dengue control and prevention. Thus, the present study focuses on the preventive and control strategies that are currently employed to counter dengue. While traditional control strategies bring temporary sustainability alone, implementation of novel biotechnological interventions, such as sterile insect technique, paratransgenesis, and production of genetically modified vectors, has improved the efficacy of the traditional strategies. Although a large-scale vector control strategy can be limited, innovative vaccine candidates have provided evidence for promising dengue prevention measures. The use of tetravalent dengue vaccine (CYD-TDV) has been the most effective so far in treating dengue infections. Nonetheless, challenges and limitation hinder the progress of developing integrated intervention methods and vaccines; while the improvement in the latest techniques and vaccine formulation continues, one can hope for a future without the threat of dengue virus.

Therapeutic Advances in Vaccines

Abstract:Dengue is a mosquito-borne disease which is currently an expanding global health problem. The disease is caused by four closely related viruses, the dengue virus. There are no specific dengue therapeutics and prevention is currently limited to vector control measures. Development of an effective tetravalent dengue vaccine would therefore represent a major advance in the control of the disease and is considered a high public health priority. While a licensed dengue vaccine is not yet available, the scope and intensity of dengue vaccine development has increased dramatically in the last decade. The uniqueness of the dengue viruses and the spectrum of disease resulting from infection have made dengue vaccine development difficult. Several vaccine candidates are currently being evaluated in clinical studies. The candidate currently at the most advanced clinical development stage, a live-attenuated tetravalent vaccine based on chimeric yellow fever dengue virus, has progressed to phase III efficacy studies. Several other live-attenuated vaccines, as well as subunit, DNA and purified inactivated vaccine candidates, are at earlier stages of clinical development. Additional technological approaches, such as virus-vectored and virus-like particle-based vaccines, are under evaluation in preclinical studies.

Consequently, drugs directed at the viral targets or critical host mechanisms that can be used safely as prophylaxis or treatment to effectively ameliorate disease or reduce disease severity and fatalities are still needed to reduce the burden of dengue. This review will provide a brief account of the status of therapeutics research and development for dengue.

Abstract:Objectives: As per the national survey data, the prevalence of dengue has been still rising consistently, and the burden of the disease is predominantly affecting the adults with progression toward the DF, dengue hemorrhagic fever, and dengue shock syndrome. The higher hospitalization rate result in massive mortality. Methods: Vector control alone cannot reduce the transmission of viral infection, so effective vaccine development is made high priority. Results: Several vaccine candidates including live attenuated, live chimeric, inactivated and many other vector vaccines are key elements needed to achieve the dengue public health targets. Global strategies like diagnosis, care management, outbreak preparedness and integrated surveillance, sustainable vector control, and future vaccine implementation are needed to be addressed. Conclusion: This review mainly describes the dengue treatment challenges, in addition to a brief discussion of dengue vaccine challenges and future outlook on dengue that might help in making efficacious vaccine in near future.

Abstract:Background: Dengue virus (DENV) is the most widespread arbovirus. The World Health Organization (WHO) declared dengue one of the top 10 global health threats in 2019. However, it has been underrepresented in bibliometric analyses. This study employs bibliometric analysis to identify research hotspots and trends, offering a comprehensive overview of the current research dynamics in this field. Results: We present a report spanning from 1995 to 2023 that provides a unique longitudinal analysis of Dengue virus (DENV) research, revealing significant trends and shifts not extensively covered in previous literature. A total of 10,767 DENV-related documents were considered, with a notable increase in publications, peaking at 747 articles in 2021. Plos Neglected Tropical Diseases has become the leading journal in Dengue virus research, publishing 791 articles in this field-the highest number recorded. Our bibliometric analysis provides a comprehensive mapping of DENV research across multiple dimensions, including vector ecology, virology, and emerging therapies. The study delineates a complex network of immune response genes, including IFNA1, DDX58, IFNB1, STAT1, IRF3, and NFKB1, highlighting significant trends and emerging themes, particularly the impacts of climate change and new outbreaks on disease transmission. Our findings detail the progress and current status of key vaccine candidates, including the licensed Dengvaxia, newer vaccines such as Qdenga and TV003, and updated clinical trials. The study underscores significant advancements in antiviral therapies and vector control strategies for dengue, highlighting innovative drug candidates such as AT-752 and JNJ-1802, and the potential of drug repurposing with agents like Ribavirin, Remdesivir, and Lopinavir. Additionally, it discusses biological control methods, including the introduction of Wolbachia-infected mosquitoes and gene-editing technologies. Conclusion: This bibliometric study underscores the critical role of interdisciplinary collaboration in advancing DENV research, identifying key trends and areas needing further exploration, including host-virus dynamics, the development and application of antiviral drugs and vaccines, and the use of artificial intelligence. It advocates for strengthened partnerships across various disciplines to effectively tackle the challenges posed by DENV.

Abstract:Dengue, caused by the dengue virus (DENV), affects millions of people worldwide every year. This virus has two distinct life cycles, one in the human and another in the mosquito, and both cycles are crucial to be controlled. To control the vector of DENV, the mosquito Aedes aegypti, scientists employed many techniques, which were later proved ineffective and harmful in many ways. Consequently, the attention shifted to the development of a vaccine; researchers have targeted the E protein, a surface protein of the virus and the NS1 protein, an extracellular protein. There are several types of vaccines developed so far, such as live attenuated vaccines, recombinant subunit vaccines, inactivated virus vaccines, viral vectored vaccines, DNA vaccines, and mRNA vaccines. Along with these, scientists are exploring new strategies of developing improved version of the vaccine by employing recombinant DNA plasmid against NS1 and also aiming to prevent the infection by blocking the DENV life cycle inside the mosquitoes. Here, we discussed the aspects of research in the field of vaccines until now and identified some prospects for future vaccine developments.

Abstract:Dengue is the most common viral disease in the tropical and subtropical regions of the world. At present no vaccine or antiviral drugs are available to treat a dengue infection, which leaves supportive care in hospitals as the only available treatment option. Since dengue epidemics often put great strain on healthcare systems, there is an urgent need for novel strategies to combat this disease. The development of vaccines for dengue is complicated by the presence of four different serotypes, which has increased the importance of antiviral drug discovery. In the past 10 years, dengue has changed from a disease that was basically unknown to drug discovery scientists into a vibrant field of research for both biologists and chemists. In this chapter, we will review the different drug targets that have been identified for dengue and critically assess the progress that has been made in antiviral drug discovery. While these efforts have not yet resulted in a clinical development candidate, the progress has been impressive, considering the limited resources. Nevertheless, a concerted effort is required to identify drug candidates for the most promising targets, dengue NS3 protease and dengue NS5 polymerase.

Abstract:The dengue virus (DENV), which causes dengue, infects 100 to 300 million people globally each year [1]. Aedes aegypti mosquitoes carry the Dengue virus, which causes dengue fever, which is still a serious global health risk, particularly in tropical and subtropical areas. Since there are now no targeted antiviral therapies and due to difficulties with traditional vector control techniques, the focus has shifted to developing vaccines as the main preventive measure. In order to create a variety of vaccination forms, including live attenuated, recombinant subunit, inactivated virus, viral vectored, DNA, and mRNA vaccines, scientists have targeted E protein and NS1. An important advancement in the fight against Dengue fever has been the development of vaccinations that are able to neutralize all four serotypes. But there are obstacles such as lack of vaccine effectiveness against specific serotypes, side effects produced by vaccine in progeny and vaccine-induced immunological enhancement (ADE).Immunization can lessen the burden on healthcare systems, lower the number of hospitalizations caused by dengue, and help stop outbreaks. The development and implementation of dengue vaccine research are significantly accelerated by international cooperation, public-private partnerships, and regulatory cooperation. Effective programs show how crucial it is to work together to combat dengue fever on a worldwide scale. The difficulties involved in developing a dengue vaccine, ongoing funding, campaigning, and research are crucial. There are several viable approaches to increase the efficacy and accessibility of the Dengue vaccine, including cross-disciplinary cooperation, novel vaccination platforms, and emerging technology. In conclusion, overcoming obstacles unique to each serotype, embracing creative approaches to vaccine design, and negotiating scientific intricacies are all necessary steps on the path to developing successful Dengue virus vaccines. The convergence of state-of-the-art technologies, computational modeling, and cooperative research endeavors can potentially realize the objective of vaccination-based comprehensive dengue prevention.

Abstract:Background: Dengue, the world's fastest-growing vector-borne disease, has skyrocketed in the 21st century. Dengue has harmed human health since its first known cases among Spanish soldiers in the Philippines to its 21st-century outbreaks in Southeast Asia, the Pacific, and the Americas. In light of the current circumstances, it is imperative to investigate its origin and prevalence, enabling the implementation of effective interventions to curb the upsurge. Methods: Our study examines the history of dengue outbreaks, and evolving impact on public health, aiming to offer valuable insights for a more resilient public health response worldwide. In this comprehensive review, we incorporated data from renowned databases such as PubMed, Google Scholar, and Scopus to provide a thorough analysis of dengue outbreaks. Results: Recent dengue outbreaks are associated with rapid urbanization, international travel, climatic change, and socioeconomic factors. Rapid urbanization and poor urban design and sanitation have created mosquito breeding places for dengue vectors. Also, international travel and trade have spread the pathogen. Climate change in the past two decades has favored mosquito habitats and outbreaks. Socioeconomic differences have also amplified the impact of dengue outbreaks on vulnerable communities. Dengue mitigation requires vector control, community engagement, healthcare strengthening, and international cooperation. Conclusion: Climate change adaptation and urban planning are crucial. Although problems remain, a comprehensive vector control and community involvement plan may reduce dengue epidemics and improve public health in our interconnected world.

Abstract:Dengue is the fastest growing mosquito-borne viral infection and is prevalent in the tropical regions of the world. It causes a wide spectrum of illness from mild asymptomatic illness to severe fatal dengue hemorrhagic fever/dengue shock syndrome. Its impact today is thirty times >50 years ago. Global incidence of dengue has drastically upped in the last few years. According to the World Health Organization, there are about 390 million cases of dengue fever worldwide, and of the total number of cases, 96 million require medical treatment. Worldwide, it has been seen a doubling up of cases on dengue from 2015 to 2016 and it can cause infection in all age groups. As vaccines or antiviral drugs are not available for dengue viruses, the only effective way to prevent dengue is to control the mosquito vector, Aedes aegypti and prevent its bite.

Abstract:Zoonoses have rapidly spread globally, necessitating the implementation of vaccination strategies as a control measure. Emerging and re-emerging vector-borne diseases are among the major global public health concerns. Dengue, a zoonotic viral infection transmitted to humans by a vector, the Aedes mosquito, is a severe global health problem. Dengue is a serious tropical infectious disease, second only to malaria, causing around 25,000 deaths each year. The resurgence of Dengue is mainly due to climate change, demographic transitions and evolving social dynamics. The development of an effective vaccine against Dengue has proven to be a complex undertaking due to four different viral serotypes with distinct antigenic profiles. This review highlights the urgent need to address the dengue threat by exploring the application of biotechnological and -OMICS sciences.

Dengue virus: pathogenesis and potential for small molecule inhibitors

Abstract:Dengue, caused by dengue virus (DENV), is now endemic in nearly 100 countries and infection incidence is reported in another 30 countries. Yearly an estimated 400 million cases and 2200 deaths are reported. Effective vaccines against DENV are limited and there has been significant focus on the development of effective antiviral against the disease. The World Health Organization has initiated research programs to prioritize the development and optimization of antiviral agents against several viruses including Flaviviridae. A significant effort has been taken by the researchers to develop effective antivirals against DENV. Several potential small-molecule inhibitors like efavirenz, tipranavir and dasabuvir have been tested against envelope and non-structural proteins of DENV, and are in clinical trials around the world. We recently developed one small molecule, namely 7D, targeting the host PF4-CXCR3 axis. 7D inhibited all 4 serotypes of DENV in vitro and specifically DENV2 infection in two different mice models. Although the development of dengue vaccines remains a high priority, antibody cross reactivity among the serotypes and resulting antibody-dependent enhancement (ADE) of infection are major concerns that have limited the development of effective vaccine against DENV. Therefore, there has been a significant emphasis on the development of antiviral drugs against dengue. This review article describes the rescue effects of some of the small molecule inhibitors to viral/host factors associated with DENV pathogenesis.

Abstract:ABSTRACT The elevated rise in dengue infection rate has been a health burden worldwide and it will continue to impact on global health for years to come. Accumulated literature holds accountable to the geographical expansion of the mosquito species transmitting the DENV. The frequency of this viral disease outbreaks have increased rapidly in the recent years, owing to various geo-climatic and anthropological activities. In situation of scarcity of any effective control measures, there has been a continuous traceable rise in mortality and morbidity rates. However, it has been reported that the spate of incidences is directly related to density of the virus infected vector (mosquito) population in a given region. In such a scenario, systems capable of detecting virus infected vector population would aid in estimating prediction of outbreak, as well as shall provide time to deploy suitable management strategies for vector control, and to break the vector-human transmission chain. This would also help in identifying areas, where much improvement is needed for vector management. To this context, we illustrate an exhaustive overview of both gold standards and as well as emerging advents for sensitive and specific mosquito population strategized viral detection technologies. We summarize the cutting-edge technologies and the challenges faced in pioneering to field application. Regardless the proven popularity of the gold standards for detection purpose, they offer certain limitations. Thus with the surge in the infection rate globally, approaches for development of newer advancements and technique upgradation to arrest the infection escalation and for early detection as a part of vector management should be prioritized.

Abstract:Dengue, a mosquito-borne viral infection, is rapidly increasing worldwide and affects over half of the world’s population in at-risk areas. Factors such as globalization, urbanization, and climate change have fueled its rapid geographical expansion. Although no indigenous dengue cases have been identified in Korea, the number of imported dengue cases has increased from travel to endemic regions. In Korea, dengue diagnosis relies mainly on detecting antidengue antibodies or viral nucleic acids using real-time polymerase chain reaction. Although specific antiviral treatments for dengue are currently unavailable, promising progress has been made in developing antiviral agents that target viral replication. Single-dose tetravalent live-attenuated dengue vaccine candidates are currently being evaluated for their safety and efficacy. Innovative vector control methods, including Wolbachia-infected and genetically modified species of Aedes mosquitos, have demonstrated promising results. Owing to the limited therapeutic options, vector control strategies remain a primary focus for preventing transmission, alongside ongoing research on antiviral drugs and vaccine development. This review provides insight into dengue fever transmission, clinical manifestations, and diagnosis. Additionally, it covers current global control measures, emerging treatment options, and the status of vaccines in development.

Abstract:Recombinant proteins are gaining enormous importance these days due to their wide application as biopharmaceutical products and proven safety record. Various recombinant proteins of therapeutic and prophylactic importance have been successfully produced in microbial and higher expression host systems. Since there is no specific antiviral therapy available against dengue, the prevention by vaccination is the mainstay in reducing the disease burden. Therefore, efficacious vaccines are needed to control the spread of dengue worldwide. Dengue is an emerging viral disease caused by any of dengue virus 1-4 serotypes that affects the human population around the globe. Dengue virus is a single stranded RNA virus encoding three structural proteins (capsid protein, pre-membrane protein, and envelope protein) and seven non-structural proteins (NS1, NS2a, NS2b, NS3, NS4a, NS4b, NS5). As the only licensed dengue vaccine (Dengvaxia) is unable to confer balanced protection against all the serotypes, therefore various approaches for development of dengue vaccines including tetravalent live attenuated, inactivated, plasmid DNA, virus-vectored, virus-like particles, and recombinant subunit vaccines are being explored. These candidates are at different stages of vaccine development and have their own merits and demerits. The promising subunit vaccines are mainly based on envelope or its domain and non-structural proteins of dengue virus. These proteins have been produced in different hosts and are being investigated for development of a successful dengue vaccine. Novel immunogens have been designed employing various strategies like protein engineering and fusion of antigen with various immunostimulatory motif to work as self-adjuvant. Moreover, recombinant proteins can be formulated with novel adjuvants to enhance the immunogenicity and thus conferring better protection to the vaccinees. With the advent of newer and safer host systems, these recombinant proteins can be produced in a cost effective manner at large scale for vaccine studies. In this review, we summarize recent developments in recombinant protein based dengue vaccines that could lead to a good number of efficacious vaccine candidates for future human use and ultimately alternative dengue vaccine candidates.

Abstract:The severity of the dengue epidemic is on the rise, with its geographic range had expanded to southern Europe by 2024. In this August, the WHO updated the pathogens that could spark the next pandemic, dengue virus was on the list. Vaccines and drugs serve as powerful tools for both preventing dengue infections and treating patients. Animal models play a pivotal role in vaccine development and drug screening. Available potential susceptible animals, including non-human primates, rodents, pigs, and tree shrews, have been extensively explored to establish animal models of dengue disease. Despite significant advancements, there are still notable limitations. Different animal models exhibit distinct constraining factors such as viraemia, host susceptibility, immune function of the host, clinical symptoms, ADE (antibody-dependent enhancement) phenomena, cytokine storm response to various serotypes and strain variations. Furthermore, despite extensive research on the dengue virus receptor in recent years, genetically modified animal models immunocompetent harbouring dengue virus susceptibility receptors have not yet been available. This work reviewed the research progress of dengue virus receptors and dengue animal models, suggesting that the development of genetically modified murine models expressing dengue virus functional receptors may hold a promise for future dengue disease research, especially for its vaccine development.
