Abstract:Background: Low T3-(/T4-) syndrome, also known as non-thyroidal Illness Syndrome (NTIS) describes a decrease in free serum thyroid hormones without a concomitant increase in TSH, frequently observed in critically ill patients. However, whether NTIS is only a metabolic adaption to stress in critically ill or plays a crucial role as an independent risk factor for ICU mortality, remains unknown. Our study aimed to evaluate NTIS as an independent risk factor for increased ICU mortality. Methods: All patients admitted to the interdisciplinary intensive care unit (ICU) at the University Hospital of Leipzig between 2008 and 2014 were retrospectively analyzed for thyroidal function. Baseline data, information on additional thyroid function tests, disease progression, hospital and ICU length of stay (LOS) and patient outcome were retrospectively analyzed from the hospitals digital information system. For statistical evaluation, univariate analysis, matched pairs analysis and multivariate logistic regression were conducted. Results: One thousand, seven hundred ninety patients were enrolled in the study, of which 665 showed NTIS. Univariate analysis revealed a positive association of NTIS with ICU- and hospital-LOS, need for mechanical ventilation, incidence of sepsis, acute respiratory distress syndrome, acute liver failure and increased ICU mortality. Results of matched pair analysis confirmed these findings. In multivariate logistic regression, NTIS was associated with an increased ICU-LOS, increased duration of mechanical ventilation, acute kidney injury and liver failure, but showed no independent association with increased ICU-mortality. Conclusion: Duration of mechanical ventilation as well as incidence of acute kidney injury, sepsis and acute liver failure were detected as independent predictors of mortality in patients with NTIS. NTIS itself was no independent predictor of increased ICU-mortality.

Abstract:Objective To analyze predictive value of plasma baseline thyrotropin levels in nonthyroid illness syndrome (NTIS) patients with mechanical ventilation during hospitalization.Methods From January 2015 to August 2016,188 patients received incubation and had no history of thyroid illness in ICU of Beijing Shijitan Hospital were continuously enrolled.All patients were divided into NTIS group of 156 cases and without NTIS group of 32 cases according to thyroid function.The morbidity and clinical characters of NTIS patients were summarized.156 patients with NTIS were divided into two subgroups of low T3 syndrome and low T4 syndrome according to T3 and T4 levels.156 patients with NTIS were divided into three subgroups of ＜0.27 mIU/L,0.27-＜2.50 mIU/L,2.50-＜4.50 mIU/L according to TSH level.Clinical data and prognosis of each group were compared.Results The morbidity of NTIS was 83％.The levels of PCT and CRP in NTIS group were higher than those in without NTIS group,while the levels of ALB and Ca2+ were lower than those in without NTIS group (P ＜ 0.05).The levels of ALB,Ca2+ and Hb in low T4 syndrome group were lower than those in low T3 syndrome group (P ＜ 0.05).TSH 2.50-＜4.50 mIU/L group had the lowest T3 level,TSH 0.27-＜2.50 mIU/L group had the shortest mechanical ventilation time (P ＜ 0.05).COX survival analysis showed that TSH 2.50-＜4.50 mIU/L was an independent predictor of good prognosis for 14 days (P ＜ 0.05).There was no significant difference in prognosis between different levels of TSH group (P ＞ 0.05).Conclusion For patients with mechanical ventilation with NTIS,TSH 2.50-＜4.50 mIU/L is an independent predictor of good prognosis for 14 days.
Study of non-thyroidal illness syndrome and its recovery in critically ill patients at a tertiary care centre in South India

Abstract:Background and Objectives: Transient thyroid hormone alterations are common during critical illness and are termed non-thyroidal illness syndrome (NTIS). We studied the prevalence of NTIS in the ICU setting and its impact on predicting mortality and other outcomes and compared it to the Acute Physiology and Chronic Health Evaluation II (APACHE II) score.Materials and Methods: The study included 119 consecutive patients admitted with a critical illness. APACHE II score was calculated. Total T3, total T4, TSH, free T3, and free T4 were measured at admission and after six weeks of discharge. NTIS and euthyroid groups were studied for ICU, hospital stays, mortality, readmission, and recovery. Predictors of mortality were compared between survivors and non-survivors.Results: The mean age was 60.15 ± 14.50 years with M:F = 84 (71%):35 (29%). NTIS was observed in 84 (71%), low T3 being the most common abnormality in 53 (63%). The occurrence of NTIS was significantly higher among non-survivors (28/30, 93%) versus survivors (56/89, 63%) (P = 0.002). Non-survivors showed significantly lower T3, TSH, and FT3/FT4 ratios and higher readmissions. NTIS group showed significantly greater ICU stay (P = 0.02) and had higher readmission rates (P = 0.032). Baseline T3 had the greatest power to predict mortality. APACHE II score also correlated significantly with mortality (19.60 ± 10.58 vs 11.99 ± 6.80, P < 0.001). The area under the curve (0.677) for the T3 level was lower than the APACHE II score (0.760). After six weeks, 61% had recovered from NTIS.Conclusions: NTIS was common amongst critically ill patients (71.5%), which reversed in 61% at six weeks. Low T3 was the most common abnormality and independently predicted mortality. Free T3/free T4 also significantly predicted mortality. The correlation between thyroid dysfunction and the severity of primary illness makes it an additional attractive low-cost marker of mortality.

Abstract:Background: We explored the impact of mechanical ventilation (MV) on hospital mortality in septic patients based on the sepsis-3 criteria. Methods: We performed a retrospective cohort study of adult Intensive Care Unit (ICU) admissions from 2008 to 2012 of tertiary teaching hospital in Boston, Massachusetts. Multivariate logistic regression explained the relationship between MV and hospital mortality. Results: In total, 5,783 ICU admissions met for the sepsis-3 criteria with 2,783 (48.1%) receiving invasive ventilation. Compared with sepsis patients without MV, individuals given MV had a higher hospital mortality [odds ratio (OR) 1.6, 95% confidence interval (CI), 1.49–1.75)]. MV was also associated with ICU mortality, 30-day mortality, ICU and hospital length of stay (LOS) for septic patients. Conclusions: Sepsis patients receiving MV independently tended to have higher in-hospital mortality and other outcomes in admissions of ICU.

Abstract:balsam of Peru and the extract, both at 25% pet. 57 patients had clearly allergic reactions to the balsam, but only 9 to the extract preparation. 7 patients showed questionable results when tested with the balsam, 4 with the extract. I Irritant reaction was seen. The test matenal and results are summanzed m Table 1. These data are m contrast to the results obtamed by Hjorth. We could not verify the correspondence between test results with native balsam of Peru and the proposed extract, neither at I 0 nor 25% pet. Further work needs to be done to Identify the allergemc fractiOn of balsam of Peru.

Abstract:During parturition in the rat there is a sustained increase in oxytocin secretion, superimposed on which are pulses of secretion, each associated with the birth of a pup. The sustained increase is the result of an increase in the background electrical activity of magnocellular oxytocin neurones, and the pulses are the result of intermittent high-frequency bursts of activity. The bursts occur approximately synchronously in all the magnocellular oxytocin neurones, in both supraoptic nuclei and both paraventricular nuclei (1). Whereas the increase in ‘background’ firing is typical of the responses of oxytocin neurones to many other stimuli, bursting is elicited in oxytocin neurones only during parturition or in response to suckling. During parturition, bursts appear to accompany the passage of a foetus through the birth canal, but bursts can also be elicited by suckling in latepregnant or parturient rats (2). Thus the ability of oxytocin neurones to fire in bursts is already present before births start. Because bursting is only exhibited in late pregnancy and parturition, it is often suspected that pregnancy induces specific adaptations that favour the expression of bursting behaviour. This is a superficially attractive hypothesis but may be quite wrong: oxytocin cells even during lactation do not exhibit bursts in response to any stimulus other than suckling; in lactating rats oxytocin neurones respond to most stimuli in much the same way as they do in virgin rats. Nevertheless, there are a number of well documented changes that seem to reflect significant changes in the intrinsic properties of oxytocin neurones, or significant changes in the neuronal networks that directly regulate them. What these changes mean is less clear, though the interpretation often offered is that these changes contribute to the ability to generate bursts. For instance, there is an increase in the incidence of GABA synapses on oxytocin cells by the end of pregnancy, but there seems to be a reduction in the efficacy of GABA as the tonic GABA current density in each oxytocin cell is not altered (3). The potentiation by progesterone metabolites at these synapses in pregnancy seems to guard against premature activation of oxytocin neurones rather than contributing to the burst-firing mechanism, because progesterone secretion collapses prior to parturition. There are marked changes in the glial architecture of the supraoptic nucleus at parturition that may favour dendro–dendritic interactions between oxytocin neurones mediated by oxytocin release, and which may favour glutamatergic excitation by reducing extracellular glutamate uptake (4); but then again, oxytocin released from dendrites is believed to be a key element in bursting, and this suppresses glutamate release (5). Not everything that happens to oxytocin neurones in pregnancy will be part of the explanation for bursting, and perhaps none of the observed changes have much to do with it. For all that we really know, the only reason why we don’t see bursting in oxytocin neurones in a virgin rat or a male rat is because they’re not giving birth or being suckled. However, the explanation of the ability of oxytocin neurones to fire in bursts has proved to be a challenging problem, precisely because this phenomenon is only observed in such exceptional circumstances. In oxytocin neurones generally, every spike is followed by a hyperpolarization that results from activation of Ca-dependent K channels, and this hyperpolarization limits the firing rate of oxytocin cells. There are, broadly, two components to the postspike hyperpolarization, a large fast transient hyperpolarization (the hyperpolarizing afterpotential, HAP), and a small long-lasting hyperpolarization (the afterhyperpolarization, or AHP). Under almost all known physiological circumstances, there will be an interval of at least 30 ms between successive spikes, leading to a maximum firing rate of less than 40 Hz, and in practice the maximum sustainable discharge rate of oxytocin cells is closer to 15 Hz. During a milk-ejection burst, interspike intervals as short as 6 ms are observed, and intervals of less than 10 ms are common. Between bursts however, such short intervals are not seen. A paper in this issue of The Journal (6) adds to the catalogue of changes in oxytocin neurones that may reflect adaptations to facilitate bursting, or which may reflect adaptations to facilitate interactions between oxytocin neurones. Compared with virgin rats, oxytocin neurones from late pregnant rats or from lactating rats have broader action potentials reflecting enhanced voltage-gated Ca influx, have larger AHPs, but with faster decay times, and more of them display depolarizing after-potentials (DAPs). A cell exhibiting a HAP, DAP and AHP will show a sequence of hyperpolarization, depolarization and afterhyperpolarization following an individual spike. DAPs are rarely seen in oxytocin neurones normally, but are commonly seen in neighbouring vasopressin neurones. The consequences of an enhanced AHP with a more rapid decay are not easy to predict; because the AHP is a small, long-lasting hyperpolarization, for it to exert a significant effect on firing rate requires summation following repetitive spike activity. The AHP seems to serve as a damping, smoothing influence on firing rate, and the observed changes in AHP decay rate may suggest that greater irregularity in firing rate will result. However the enhanced amplitude of the AHP would certainly not favour the ability to express bursts. On the other hand, the greater incidence of a DAP will tend to make these neurones more excitable, and may enhance the effect of any positive-feedback effects of dendro–dendritic interaction between oxytocin neurones. Abstract:The growing importance of vesicular trafficking and cytoskeleton dynamic reorganization during plant development requires the exploitation of novel experimental approaches. Several genetic and cell biological studies have used diverse pharmaceutical drugs that inhibit vesicular trafficking and secretion to study these phenomena. Here, proteomic and cell biology approaches were applied to study effects of brefeldin A (BFA), an inhibitor of vesicle recycling and secretion, in Arabidopsis roots. The main aim of this study was to obtain an overview of proteins affected by BFA, but especially to identify new proteins involved in the vesicular trafficking and its cross-talk to the actin cytoskeleton. The results showed that BFA altered vesicular trafficking and caused the formation of BFA-compartments which was accompanied by differential expression of several proteins in root cells. Some of the BFA-up-regulated proteins belong to the class of the vesicular trafficking proteins, such as V-ATPase and reversibly glycosylated polypeptide, while others, such as profilin 2 and elongation factor 1 alpha, are rather involved in the remodeling of the actin cytoskeleton. Upregulation of profilin 2 by BFA was verified by immunoblot and live imaging at subcellular level. The latter approach also revealed that profilin 2 accumulated in BFA-compartments which was accompanied by remodeling of the actin cytoskeleton in BFA-treated root cells. Thus, profilin 2 seems to be involved in the cross-talk between vesicular trafficking and the actin cytoskeleton, in a BFA-dependent manner.

Abstract:BACKGROUND:Several respiratory abnormalities can be present in primary hypothyroidism and can be reversed with adequate hormone treatment. However, the role of thyroid hormone replacement therapy on the respiratory system in patients with nonthyroidal illness syndrome is still unclear. This physiologic study evaluated the effect of thyroid hormone treatment on respiratory muscle function in subjects with nonthyroidal illness syndrome and while on mechanical ventilation. The primary end point was neuromechanical efficiency, which provides an estimate of the efficiency of diaphragmatic contraction. Secondary end points were the transdiaphragmatic pressure-time product and the swing of the electrical activity of the diaphragm, which reflect the work of breathing and inspiratory effort, respectively.METHODS:Fifteen subjects on mechanical ventilation for ≥48 h and with a diagnosis of nonthyroidal illness syndrome who had a failed spontaneous breathing trial, received intravenous triiodothyronine. The hormone was administered as an intravenous bolus of 0.4 μg/kg triiodothyronine, followed by continuous perfusion at 0.6 μg/kg for 24 h. Neuromechanical efficiency was calculated as the ratio between the drop in airway pressure during an expiratory occlusion and the corresponding electrical activity of the diaphragm peak. Recordings were taken at baseline and after 3, 6, and 24 h.RESULTS:After study completion, free triiodothyronine serum concentrations increased in all the subjects (mean ± SD increase, 0.84 ± 0.34 pg/mL). Neuromechanical efficiency showed no significant changes throughout the study (mean ± SD baseline, 1.40 ± 0.87 cm H2O/μV; 3 h, 1.28 ± 0.64 cm H2O/μV; 6 h, 1.33 ± 0.87 cm H2O/μV; 24 h, 1.41 ± 0.96 cm H2O/μV). Similarly, no variations in transdiaphragmatic pressure-time product per min (mean ± SD baseline, 238.1 ± 124 cm H2O × s/min; 3 h, 242.5 ± 140.3 cm H2O × s /min; 6 h, 247.5 ± 161.7 cm H2O × s/min; 24 h, 281.2 ± 201.2 cm H2O × s/min) or swing of electrical activity of the diaphragm (mean ± baseline, 20.9 ± 13.1 μV; 3 h, 17.2 ± 8.3 μV; 6 h, 17.4 ± 11.3 μV; 24 h, 20.3 ± 13.7 μV) were observed during hormone administration.CONCLUSIONS:In the subjects on mechanical ventilation who were admitted to the ICU with nonthyroidal illness syndrome, thyroid hormone replacement treatment did not yield any benefit on respiratory muscle function when assessed by neuromechanical efficiency, which indicated that, in these subjects restoring normal levels of serum thyroid hormones is debatable. Abstract:Introduction: Non-thyroidal illness syndrome (NTIS) is considered to be associated with adverse outcomes in critically ill children.The hypothesis that thyroid hormones and inflammatory markers are associated with increased prediction of mortality risk scores is tested in this paper. Methods: A prospective observational study was set up in a pediatric intensive care unit (PICU). One hundred and three patients were included. NTIS was defined as a low free triiodothyronine (FT3) value for the patient's age. Thyroid hormones levels and inflammatory markers were determined at admission: FT3, FT4 (free thyroxine), TSH (thyroid-stimulating hormone), rT3 (reverse triiodothyronine), CRP (C-reactive protein) and PCT (Procalcitonin). They were compared between children with a pediatric risk of mortality score PRISM-III >75th percentile (group A, n = 25) and the rest (group B, n = 78). Results: A FT4 value lower than 16.6 pmol/L showed an area under the curve (AUC) of 0.655 (0.56–0.78, p = 0.02), with 76% sensitivity and 61.5% specificity to detect a high risk of mortality. A multiple regression analysis revealed that a FT4 lower than 16.6 pmol/L [OR: 4.92 (1.60–18.19), p = 0.009] and having NTIS [OR: 6.04 (1.45–27.93), p = 0.016] could predict a high risk of mortality. Conclusions: In unselected critically ill children, FT4 and FT3 values at admission could be used as a good predictor of a high mortality risk. We have not achieved a predictive model that combines hormones with inflammatory markers.

Different subtypes of nonthyroidal illness syndrome on the prognosis of septic patients: a two-centered retrospective cohort study

Abstract:Background: Nonthyroidal illness syndrome (NTIS) is a common endocrine dysfunction predicting unfavorable outcomes in critical illness. The objective of the study is to evaluate the association between different NTIS subtypes with outcomes in septic patients. Methods: Septic patients in two Chinese academic centers from October 2012 and October 2022 are enrolled in analysis. Multivariable regressions are used to assess associations between NTIS and outcomes. Outcomes include in-hospital mortality, length of stay in hospital (LOS), non-invasive ventilation failure and weaning failure. Patients with NTIS are categorized into 4 types according to the different levels of FT4 and TSH. The association between different NTIS subtypes and mortality are further analyzed. Survival curve is plotted using the Kaplan–Meier method. Results: After screening, a total of 1226 septic patients with complete thyroid hormones result are eventually enrolled. Among them, 520 (42.4%) patients are diagnosed as NTIS. In multivariable regression analysis, NTIS is independently associated with increased 30-days mortality (OR=1.759, CI 1.009-3.104, p=0.047), but has no association with 60-days mortality (OR=1.524, CI 0.893-2.618, p=0.123), 90-days mortality (OR=1.411, CI 0.831-2.408, p=0.203), LOS, non-invasive ventilation failure or weaning failure. In NTIS subtypes, NTIS patients with low FT3 and TSH levels, regardless of the FT4 values, have significantly higher mortality than euthyroid patients (30-days mortality, OR= 6.488, CI 1.546-27.808, p=0.01; 60-days mortality, OR=3.973, CI 1.006-15.579, p=0.046; 90-days mortality, OR=3.849, CI 0.977-15.088, p=0.051). This result is consistent in patients with low FT3 and FT4 levels, regardless of the TSH values (30-days mortality, OR=3.349, CI 1.402-7.957, p=0.006; 60-days mortality, OR= 2.594, CI 1.122-5.930, p=0.024; 90-days mortality, OR=2.55, CI 1.110-5.804, p=0.025). There is no survival difference between NTIS patients with low FT3 only and euthyroid patients. Survival plot shows the worst prognosis is in NTIS patients with low FT3, FT4 and TSH level. Conclusions: NTIS is frequent in sepsis. A reduction of FT3 together with FT4 or TSH, but not FT3 only, is associated with an increased risk of mortality.

Abstract:Background This study investigates the impact of nontuberculous mycobacterial lung disease (NTM-LD) on mortality and mechanical ventilation use in critically ill patients. Methods We enrolled patients with NTM-LD or tuberculosis (TB) in intensive care units (ICU) and analysed their association with 30-day mortality and with mechanical ventilator-free survival (VFS) at 30 days after ICU admission. Results A total of 5,996 ICU-admitted patients were included, of which 541 (9.0%) had TB and 173 (2.9%) had NTM-LD. The overall 30-day mortality was 22.2%. The patients with NTM-LD had an adjusted hazard ratio (aHR) of 1.49 (95% CI, 1.06–2.05), and TB patients had an aHR of 2.33 (95% CI, 1.68–3.24), compared to ICU patients with negative sputum mycobacterial culture by multivariable Cox proportional hazard (PH) regression. The aHR of age<65 years, obesity, idiopathic pulmonary fibrosis, end-stage kidney disease, active cancer and autoimmune disease and diagnosis of respiratory failure were also significantly positively associated with ICU 30-day mortality. In multivariable Cox PH regression for VFS at 30 days in patients requiring invasive mechanical ventilation, NTM-LD was negatively associated with VFS (aHR 0.71, 95% CI: 0.56–0.92, p =0.009), while TB showed no significant association. The diagnosis of respiratory failure itself predicted unfavourable outcome for 30-day mortality and a negative impact on VFS at 30 days. Conclusions NTM-LD and TB were not uncommon in ICU and both were correlated with increasing 30-day mortality in ICU patients. NTM-LD was associated with a poorer outcome in terms of VFS at 30 days.

Abstract:Abstract Background Non-thyroidal illness syndrome (NTIS) is considered when patients demonstrate altered thyroid hormones and is frequently seen in patients with sepsis and septic shock. Levels keep affected with disease progression and usually get normalized after the sickness is cured. NTIS is not studied well in pediatric population. Aim of the work Our primary outcome was to assess the frequency of hormonal changes of NTIS in sepsis and septic shock patients. The secondary outcome was to follow-up the severity of NTIS and its effect on the prognosis of the primary illness. Patients and methods This study (1st phase: cross-sectional, 2nd phase: prospective) included 40 critically ill children categorized into two groups: (i) sepsis group: defined according to standard international criteria using pediatric Sequential Organ Failure Assessment (p SOFA) score and sepsis was considered when p SOFA score > 2; (ii) septic shock group: defined by a vasopressor requirement to maintain a mean arterial pressure ≥ 65 mmHg and having a serum lactate level > 2 mmol/L despite adequate fluid resuscitation, with 20 patients in each group. Patients were admitted to the pediatric intensive care unit (PICU). Thyroid hormone levels were assessed and compared in day 1 and day 5 in all patients and subgroups. All patients were followed up until discharge or death. Results NTIS was found in 47.5% of patients. NTIS was higher among septic shock group than sepsis 65.5% versus 30% ( p = 0.027). NTIS was associated with each of ventilation, catecholamines infusion and SOFA score ( p = 0.044, 0.027, and 0.033) respectively. FT3 (free triiodothyronine) levels were lower and rT3 (reverse T3) levels were higher in day 5 of sickness than day 1 ( p = 0.041 and 0.000) respectively. Furthermore, FT3 levels in day 5 were lower, and rT3 levels in day 1 and day 5 were higher in non-survivors than survivors ( p = 0.002, 0.015, and 0.003) respectively. ROC curve was done to assess predictors of mortality and revealed that FT3 levels in day 5 was the best in predicting PICU mortality, followed by SOFA score day 5. Conclusion NTIS is common among critically ill children and higher among septic shock group than sepsis. Also, beside the SOFA score, FT3 measured in day 5 of sickness were the best predictors of PICU mortality.
Non-thyroidal Illness Syndrome in Patients with Cardiovascular Diseases: A Systematic Review and Meta-Analysis

Abstract:Background Non-thyroidal illness syndrome (NTIS) is characterized by decreased serum triiodothyronine level without increased thyroid-stimulating hormone level during critical illness. The summary data on the prevalence of NTIS in cardiovascular patients are lacking, and its prognostic role in cardiovascular patients is also unclear. Methods We performed a systematic review and meta-analysis to comprehensively determine the prevalence and the prognostic role of NTIS in cardiovascular patients. The prevalence of NTIS was pooled using random-effect meta-analysis and the hazard ratios (HRs) for all-cause mortality, cardiac mortality and major adverse cardiovascular events (MACE) were also pooled. Results Forty-one studies were finally included. The pooled prevalence of NTIS in cardiovascular patients was 21.7% (95% CI 18.4%–25.3%). Subgroup by the types of cardiovascular diseases showed the prevalence of NTIS was highest in patients with heart failure (24.5%), followed by acute myocardial infarction (18.9%) and acute coronary syndrome (17.1%). Meta-analysis of studies using strict diagnostic criteria of NITS showed that the pooled prevalence of NTIS in cardiovascular patients was 17.6% (95% CI 14.5%–21.2%). NTIS was independently associated with increased risks of all-cause mortality (HR = 2.52, 95% CI 1.87–3.40, P < 0.001) and cardiac mortality (HR = 2.06, 95% CI 1.58–2.69, P < 0.001) in cardiovascular patients. NTIS was also an independent predictor of MACE in cardiovascular patients (HR = 1.73, 95% CI 1.32–2.26, P < 0.001). Conclusion NTIS is very common in patients with cardiovascular diseases. NTIS is an independent prognostic factor in cardiovascular patients and is associated with increased risks of all-cause mortality, cardiac mortality and MACE.
Lower Free T3 Levels Linked to Poorer Outcomes in Chronic Obstructive Pulmonary Disease Patients with Acute Hypercapnic Respiratory Failure

Abstract:Aim of the study: Non-thyroidal illness syndrome (NTIS) is often observed in critically ill patients. This study aimed to examine thyroid hormone changes in patients with chronic obstructive pulmonary disease (COPD) experiencing acute hypercapnic respiratory failure (AHRF) and to evaluate the impact of these alterations on clinical outcomes. Materials and methods: This retrospective investigation involved 80 COPD patients (age 71.5±9.5 years; 57.5% male) admitted to the intensive care unit (ICU) due to AHRF. NTIS was identified when free triiodothyronine (fT3) levels were below the lower limit, and thyroid-stimulating hormone (TSH) and free thyroxine (fT4) levels were within the normal range or below the lower limits. Results: NTIS was detected in 63.7% of the patients. Decreased fT3 levels were found in 36.3% of the patients, reduced T4 levels in 33.8%, and diminished TSH levels in 15%. Patients with low fT3 levels exhibited elevated C-reactive protein levels, white blood cell counts, and APACHE II scores, necessitated vasopressor infusion more frequently during their ICU stay, and had increased mortality. The in-hospital mortality rate was 28.8%. Logistic regression analysis revealed that fT3 level (odds ratio [OR]., 0.271; 95% confidence interval [CI]., 0.085-0.865; p=0.027), APACHE II score (OR, 1.155; 95% CI, 1.041-1.282; p=0.007), and vasopressor use (OR, 5.426; 95% CI, 1.439-20.468; p=0.013) were crucial predictors of in-hospital mortality. Conclusions: A high prevalence of NTIS is observed in COPD patients with AHRF, with low fT3 levels frequently observed. The presence of lower levels of fT3 is associated with a greater severity of the disease and a significant prognostic indicator.

Abstract:The non-thyroidal illness syndrome (NTIS) was first reported in the 1970s as a remarkable ensemble of changes in serum TH (TH) concentrations occurring in probably any severe illness. Ever since, NTIS has remained an intriguing phenomenon not only because of the robustness of the decrease in serum triiodothyronine (T3), but also by its clear correlation with morbidity and mortality. In recent years, it has become clear that (parenteral) feeding in patients with critical illness should be taken into account as a major determinant not only of NTIS but also of clinical outcome. Moreover, both experimental animal and clinical studies have shown that tissue TH concentrations during NTIS do not necessarily reflect serum low TH concentrations and may decrease, remain unaltered, or even increase according to the organ and type of illness studied. These differential changes now have a solid basis in molecular studies on organ-specific TH transporters, receptors and deiodinases. Finally, the role of inflammatory pathways in these non-systemic changes has begun to be clarified. A fascinating role for TH metabolism in innate immune cells, including neutrophils and monocytes/macrophages, was reported in recent years, but there is no evidence at this early stage that this may be a determinant of susceptibility to infections. Although endocrinologists have been tempted to correct NTIS by TH supplementation, there is at present insufficient evidence that this is beneficial. Thus, there is a clear need for adequately powered randomized clinical trials (RCT) with clinically relevant endpoints to fill this knowledge gap.
Symptoms in Patients Receiving Noninvasive Ventilation in the Intensive Care Unit

Abstract:Background: Although a multitude of studies have demonstrated the effectiveness of noninvasive ventilation (NIV) for treatment of respiratory insufficiency, there have been few investigations of patients' experiences while receiving this common treatment. Identification of the presence, intensity, and distress of symptoms during NIV will inform the development and testing of interventions to best manage them and improve patients' intensive care unit (ICU) experiences. Objective: The objectives of this study were (a) to identify the presence, intensity, and distress of symptoms in patients receiving NIV in the ICU using a modified version of the Edmonton Symptom Assessment Scale (MESAS) and (b) to describe the most common and distressing symptoms experienced by patients. Methods: A cross-sectional descriptive design was used with a convenience sample of 114 participants enrolled from three ICUs at one Midwestern medical center. Participants were approached if they were English-speaking, were 18 years old or older, and had an active order for NIV; exclusions included use of personal NIV equipment, severe cognitive impairment, or problems communicating. Demographic and clinical data were obtained from the electronic health record. Presence, intensity, and distress of patient-reported symptoms were obtained once using a modified, 11-item version of the MESAS. Results: The mean age of participants was 68 years old, and 54.4% were male. The primary type of NIV was bi-level positive airway pressure; a nasal/oral mask was most frequently used. The symptoms experienced by most of the participants were thirst, anxiety, tiredness, and restlessness; these symptoms were rated as moderate or severe in both intensity and distress by most participants experiencing the symptoms. Discussion: Patients in the ICU experience both intense and distressful symptoms that can be severe while undergoing treatment with NIV. Future research is warranted to determine these symptoms' interrelatedness and develop interventions to effectively manage patient-reported symptoms.

Abstract:This article reviews the pathophysiology of non-thyroidal illness syndrome (NTIS) and euthyroid sick syndrome (ESS), a multifactorial phenomenon characterized by suppression of thyroid hormone levels that has been described in several disease states, probably due to different causes in different patients.
