A Study of Short-term Breathing Exercises on Different Cardio-Respiratory Parameters

Abstract:Cardiorespiratory health is one of the critical parameters for improving endurance. Breathing exercises in the form of various Pranayama have a significant effect on cardiorespiratory health. To assess the effect of breathing exercises in the form of various pranayamas on cardiorespiratory parameters in people living with sedentary lifestyles. This longitudinal study was done on 30 subjects aged 25 to 35 years. Participants performed Bhastrika, Kapalbhati, Anulomvilom and Bhamri Pranayama with prayers and warm-up for 10 to 40 minutes, progressively increasing over one year. The average daily duration in the initial 3 months was 15 minutes; in the next three months, it was 23 minutes. The average daily duration in the last 6 months was 34 minutes. Cardiorespiratory functions were assessed using Spiro Excel machine spirometer and Mercury sphygmomanometer (Diamond) at the time of enrolment and the end of 1 year of the study. A paired t-test, using statistical software, was used to analyze parameters. In males and females, significant change is observed in pulse rate, systolic blood pressure and diastolic blood pressure. In pulmonary function test parameters, non-significant change was observed in forced expiratory volume in 1 sec (FEV₁) and forced vital capacity (FVC); other parameters, i.e., FEV₁ as percentage of FVC in % [FEV₁ (%)], peak expiratory flow rate in L/s (PEFR) and Minute Ventilation Volume (MVV) L/min showed significant change in males and females. Breathing exercises in the form of various Pranayama positively affect cardiorespiratory health, and further studies are recommended in the diseased population.
Effect of fast and slow pranayama practice on cognitive functions in healthy volunteers

Abstract:Objectives: To compare the cumulative effect of commonly practised slow and fast pranayama on cognitive functions in healthy volunteers. Settings and design: 84 participants who were in self-reported good health, who were in the age group of 18-25 years, who were randomized to fast pranayama, slow pranayama and control group with 28 participants in each group. Material and methods: Fast pranayama included kapalabhati, bhastrika and kukkuriya. Slow pranayama included nadishodhana, Pranav and Savitri. Respective pranayama training was given for 35 minutes, three times per week, for a duration of 12 weeks under the supervision of a certified yoga trainer. Parameters were recorded before and after 12 weeks of intervention: Perceived stress scale (PSS), BMI, waist to hip ratio and cognitive parameters-letter cancellation test, trail making tests A and B, forward and reverse digit spans and auditory and visual reaction times for red light and green light. Statistical analysis: Inter-group comparison was done by one way ANOVA and intra-group comparison was done by paired t-test. Results and conclusion: Executive functions, PSS and reaction time improved significantly in both fast and slow pranayama groups, except reverse digit span, which showed an improvement only in fast pranayama group. In addition, percentage reduction in reaction time was significantly more in the fast pranayama group as compared to that in slow pranayama group. Both types of pranayamas are beneficial for cognitive functions, but fast pranayama has additional effects on executive function of manipulation in auditory working memory, central neural processing and sensory-motor performance.

Abstract:Background: The psycho-physiological changes in brain-body interaction observed in most of meditative and relaxing practices rely on voluntary slowing down of breath frequency. However, the identification of mechanisms linking breath control to its psychophysiological effects is still under debate. This systematic review is aimed at unveiling psychophysiological mechanisms underlying slow breathing techniques (<10 breaths/minute) and their effects on healthy subjects. Methods: A systematic search of MEDLINE and SCOPUS databases, using keywords related to both breathing techniques and to their psychophysiological outcomes, focusing on cardio-respiratory and central nervous system, has been conducted. From a pool of 2,461 abstracts only 15 articles met eligibility criteria and were included in the review. The present systematic review follows the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. Results: The main effects of slow breathing techniques cover autonomic and central nervous systems activities as well as the psychological status. Slow breathing techniques promote autonomic changes increasing Heart Rate Variability and Respiratory Sinus Arrhythmia paralleled by Central Nervous System (CNS) activity modifications. EEG studies show an increase in alpha and a decrease in theta power. Anatomically, the only available fMRI study highlights increased activity in cortical (e.g., prefrontal, motor, and parietal cortices) and subcortical (e.g., pons, thalamus, sub-parabrachial nucleus, periaqueductal gray, and hypothalamus) structures. Psychological/behavioral outputs related to the abovementioned changes are increased comfort, relaxation, pleasantness, vigor and alertness, and reduced symptoms of arousal, anxiety, depression, anger, and confusion. Conclusions: Slow breathing techniques act enhancing autonomic, cerebral and psychological flexibility in a scenario of mutual interactions: we found evidence of links between parasympathetic activity (increased HRV and LF power), CNS activities (increased EEG alpha power and decreased EEG theta power) related to emotional control and psychological well-being in healthy subjects. Our hypothesis considers two different mechanisms for explaining psychophysiological changes induced by voluntary control of slow breathing: one is related to a voluntary regulation of internal bodily states (enteroception), the other is associated to the role of mechanoceptors within the nasal vault in translating slow breathing in a modulation of olfactory bulb activity, which in turn tunes the activity of the entire cortical mantle.

Paced Breathing With a Prolonged Inspiratory Period Increases Sympathetic Activity: A Heart and Brain Analysis

Abstract:Paced breathing exercises with prolonged exhalation have been commonly used to reduce stress and anxiety by stimulating parasympathetic activity. However, increasing sympathetic activity may also provide benefits such as increased alertness and energy levels. In this work, we investigate the physiological impact of an 80-second breathing exercise with a prolonged inspiratory period of 6 seconds followed by 2 seconds of exhalation on the sympathetic system. We collected raw two-channel prefrontal electroencephalography and photoplethysmography signals of 19 subjects using EMOTAI’s headband while performing the proposed exercise every workday for 2 weeks straight. Physiological metrics such as heart rate, heart rate variability, and absolute power of the brain waves were extracted before and during the exercise to measure its effectiveness. A marked increase in beta wave power was observed, along with a significant increase in both heart rate and heart rate variability. The cardiovascular results indicate that the proposed exercise effectively raised sympathetic activity. Simultaneously, the observed neural activity is consistent with that seen during focused attention and heightened mental processing.

Alternate-Nostril Yoga Breathing Reduced Blood Pressure While Increasing Performance in a Vigilance Test

Abstract:BACKGROUND Reports suggest that vigilance or sustained attention increases sympathetic activity. A persistent increase in sympathetic activity can lead to an increase in blood pressure. Alternate-nostril yoga breathing has been shown to be useful to (i) improve attention and (ii) decrease the systolic and diastolic blood pressure. Earlier studies did not report simultaneous recordings of the blood pressure and performance in vigilance tests after alternate-nostril yoga breathing. With this background, the present study was planned to determine if 15 minutes of alternate nostril yoga breathing could improve the performance in a vigilance test without an increase in blood pressure. MATERIAL AND METHODS Fifteen healthy male volunteers participated in the study (group mean age ±SD, 22.4±2.4 years). Participants were assessed on 3 separate days in 3 different sessions. These were (i) alternate nostril yoga breathing, (ii) breath awareness, and (iii) sitting quietly as a control. Blood pressure and the digit vigilance test were simultaneously assessed before and after each session. RESULTS Systolic blood pressure (p<0.01), mean arterial blood pressure (p<0.05), and the time taken to complete the digit vigilance test (p<0.05) significantly decreased following alternate-nostril yoga breathing. The time taken to complete the digit vigilance test differed significantly between sessions (p<0.05). The time taken to complete the digit vigilance test was also significantly decreased after sitting quietly (p<0.01). CONCLUSIONS Alternate-nostril yoga breathing appears to improve performance in the digit vigilance test, along with a reduction in systolic blood pressure. This is suggestive of better vigilance without sympathetic activation.

Abstract:Background and Study Aim. The present study investigated the effects of 6-weeks bhastrika pranayama intervention (BPI) program on health-related components of physical fitness. Material and Methods. We used a quasi-experimental design with fifty-two healthy girls (Mean ± SD; age, 23.6346±1.1551 yrs; body height, 158.0961±3.8616 cm; body weight, 55.6826±3.4002 kg) from Guru Nanak Dev University, Amritsar, Punjab, India. The subjects were divided into two groups: Group-A: Bhastrika Pranayama Intervention (BPI); (n1=26) and Group-B: Control; (n2=26). The sample size (N=52) was calculated using the G*Power 3.1.9.7 software. A power of 0.80 (1-β err prob) and significance level (α) of 0.05. Subjects from both groups (pranayama and control) were assessed at baseline and right after 6-weeks. Group-A: (n1=26) subjected to BPI and Group-B: (n2=26) with no training. Results. As compared BPI with the control group, the BPI group had a positive effect on selected health-related components of physical fitness (maximal oxygen consumption - VO2 max 1.3%; flexibility 1.8%; muscle strength 0.2.%). The 6-weeks Bhastrika Pranayama intervention program had no effect on the BPI group with respect to the parameter muscular endurance. Conclusions. Daily practice of Bhastrika pranayama helps to keep lungs more efficient. Additional research is required to completely comprehend the consequences of this breathing exercise. Bhastrika pranayama is known for oxygen-based breathing exercise which helps to fill up the lungs with more oxygen. Bhastrika pranayama interventions with short as well as long duration in future research studies could be beneficial for healthy individuals with respect to health-related fitness components.

Abstract:The title of this contribution was adopted from one of the many obituaries that were published after the death of Norman Breslow on December 9, 2015, aged 74 years. It is remarkable that not only his home institution, the University of Washington, the International Biometric Society and other statistical societies, but also The Lancet as one of the most prominent, general medical journals published a one-page obituary especially highlighting his contributions to medical research (Watts, 2016). Most biostatisticians will know at least some of his groundbreaking contributions to survival analysis, the most popular of which is his investigation on the statistical properties of the Kaplan–Meier estimator for censored survival data that he published jointly with John Crowley (Breslow and Crowley, 1974). This publication is most likely the major reason, that the Kaplan–Meier estimator was rediscovered for medical statistics and became the accepted standard method of survival analysis since then. There were, however, further contributions of similar importance to this field and closely connected with his name: Sequential analysis of survival data (Breslow, 1969), a K-sample test (“the Breslow test”) for censored survival data (Breslow, 1970), an estimator for the baseline cumulative hazard (“the Breslow estimator”) in a Cox proportional hazard model (Breslow, 1972) and the first modern treatment of survival data in the presence of competing risks (Prentice et al., 1978). Colleagues working in the field of epidemiology will probably know two other publications even better: the IARC monographs on “Statistical Methods in Cancer Research” that he worked out together with Nick Day during his many visits at the International Agency for Research on Cancer (IARC) in Lyon, France. Volume I on “The Analysis of Case-Control Studies” (Breslow and Day, 1980) and Volume II on “The Design and Analysis of Cohort Studies” (Breslow and Day, 1987) served for decades and still serve as the standard textbooks for researchers in the field of cancer epidemiology. I met Norman Breslow for the first time in the early 1980s as I worked at the Institute of Medical Statistics of the Heidelberg University. My project was focused on statistical modeling for clinical trials in breast cancer and was part of the Collaborative Research Center “Stochastic Mathematical Models.” Norman spent a year in Heidelberg at the German Cancer Research Center as a fellow of the Alexandervon-Humboldt Foundation. During this time, I remember one of his seminars on “Estimation of the odds ratio” promoting the use of the Mantel–Haenszel estimator in sparse data situations (Breslow, 1981), a topic that he further developed jointly with Sander Greenland and Jamie Robins (Robins et al., 1986). I was, however, even more impressed by another lecture of Norman on goals and achievements of the National Wilms Tumor Study where he was the responsible statistician. As a member and an integral part of the national study group for more than 30 years, he published about 94 papers (37 of which as first or last author) devoted to the etiology, diagnosis, treatment, and long-term prognosis of a rare pediatric kidney tumor showing his in-depth subject-matter knowledge and dedication to a longlasting cooperation with his clinical colleagues. Effect of paced breathing on ventilatory and cardiovascular variability parameters during short-term investigations of autonomic function

Abstract:Paced breathing (PB) around 0.25 Hz has been advocated as a means to avoid confounding and to standardize measurements in short-term investigations of autonomic cardiovascular regulation. Controversy remains, however, as to whether it causes any alteration in autonomic control. We addressed this issue in 40 supine, middle-aged, healthy volunteers by assessing the changes induced by PB (0.25 Hz for 8 min) on 1) ventilatory parameters, 2) the indexes of autonomic control of cardiovascular function, and 3) the spectral indexes of cardiovascular variability. Subjects were grouped into group 1 ( n = 31), if spontaneous breathing was regular and within the high-frequency (HF) band (0.15–0.45 Hz), or group 2 ( n = 9), if it was irregular or slow (<0.15 Hz). In both groups, PB was accompanied by an increase in minute ventilation (both groups, P < 0.01), whereas tidal volume increased only in group 1 ( P = 0.0003). End-tidal CO 2 decreased by [median (lower quartile, upper quartile)] −0.2 (−0.5, −0.1)% ( group 1, P < 0.0001) and −0.6 (−0.8, −0.5)% ( group 2, P = 0.008). Mean R-R interval and systolic and diastolic pressure remained remarkably stable (all P ≥ 0.13, both groups). No significant changes were observed in spectral indexes of R-R and pressure variability (all P ≥ 0.12, measured only in group 1 to avoid confounding), except in the HF power of pressure signals, which significantly increased (all P < 0.05) in association with increased tidal volume. In conclusion, PB at 0.25 Hz causes a slight hyperventilation and does not affect traditional indexes of autonomic control or, in subjects with spontaneous breathing in the HF band, most relevant spectral indexes of cardiovascular variability. These findings support the notion that PB does not alter cardiovascular autonomic regulation compared with spontaneous breathing.

Exploring the Therapeutic Benefits of Pranayama (Yogic Breathing): A Systematic Review

Abstract:Background: Pranayama (yogic breathing) has demonstrated numerous beneficial health effects. At present, there are no systematic reviews evaluating the beneficial health effects of pranayama alone as a practice. Aim: The aim of this study is to perform a systematic review about the beneficial health effects of pranayama. Methods: Data were obtained using a stepwise search process by searching the online PubMed, Web of Science, and SciVerse Scopus databases using keywords. Controlled clinical trials in humans, using "Pranayama" as an intervention with an appropriate control group and evaluating health-related outcomes were selected for inclusion. Results: Initial database searching indicated 669 potentially eligible articles, of which 18 studies satisfying the inclusion/exclusion criteria were selected. All were controlled trials, of which 13 were randomized and 1 was a crossover study. Number of participants ranged from 16 to 160, and the duration of pranayama practice varied from 4 days to 6 months. Studies demonstrated a significant effect on cardiorespiratory functions, in patients with bronchial asthma, with the improvement of pulse rate, systolic blood pressure, and respiratory function measurements. Furthermore, reduction in the frequency of attacks, severity, and medication requirement was also observed, with improved quality of life (QOL). In patients with chronic obstructive pulmonary disease, symptom, activity, and impact scores were improved. QOL improvement was also noted in cancer patients. Conclusions: Available evidence on pranayama indicates physiological and psychological benefits. Beneficial effects were mostly observed in patients with respiratory diseases such as bronchial asthma. It also helped those with cancer and cardiovascular disease. However, further high-quality randomized trials are required to provide definitive evidence.

Abstract:Background: Accurate blood pressure measurement necessitates meticulous methodology, as even minor variations like speaking or sitting can impact the readings. Additionally, respiration plays a role in blood pressure, exhibiting a dip during inhalation and a subsequent increase during exhalation. However, the influence of breathing patterns, duration, and rate on blood pressure remains largely unexplored. Methods: In this study, we have attempted to quantify the changes in blood pressure and heart rate (HR) of normotensive individuals while performing predefined respiratory patterns – slow, medium, and fast (respiratory rates of 5, 10, and 20/m, respectively). For each respiratory pattern, age-matched and gender-matched (18–40 years) volunteered normotensive individuals were selected as volunteers. Baseline readings were recorded and each volunteer was randomly allotted a respiratory pattern. The volunteers performed the assigned respiratory pattern while the BP and HR were recorded. Results: A fall in both SBP and DBP was observed in all three patterns of respiration. There was no inter-pattern difference in the blood pressure change. A novel finding in our study was a trend of respiratory rate and HR – the change in HR linearly increased with a rise in respiratory rate difference. We also proposed a linear regression equation for this increase in HR with the increase in respiratory rate which was statistically significant. Conclusion: The findings suggest the decline in blood pressure is independent of the respiratory pattern employed. The study also demonstrates that the HR is a linear function of respiratory rate.

Abstract:Deep breathing exercises are commonly used for several health conditions including pain and hypertension. Various techniques are available to practice deep breathing, whereas possible differential psychophysiological effects have not been investigated. We compared four deep breathing techniques and examined outcomes in blood pressure variability, respiratory sinus arrhythmia, baroreflex function, and emotional state. Healthy adult volunteers performed pursed-lips breathing, left and right unilateral nostril breathing, and deep breathing with an inspiratory threshold load (loaded breathing), all at a frequency of 0.1 Hz (i.e., controlled breathing) and for three minutes each. Results showed that blood pressure variability was higher during loaded breathing versus other conditions and higher during pursed-lips breathing versus left and right unilateral nostril breathing. Respiratory sinus arrhythmia was higher during loaded breathing versus other conditions and higher during pursed-lips breathing versus left unilateral nostril breathing. The effect of breathing condition on respiratory sinus arrhythmia was mediated by alterations in blood pressure variability. There was no difference between the breathing conditions in baroreflex sensitivity or effectiveness. Participants rated pursed-lips breathing as more calming and pleasant and with more sense of control (vs. other conditions). Overall, among the four tested deep breathing techniques, loaded breathing was associated with enhanced cardiovascular effects and pursed-lips breathing with better emotional responses, while also enhancing cardiovascular effects (albeit less than loaded breathing). These findings can be informative in applying deep breathing techniques as self-management interventions for health conditions, in which baroreceptors stimulation and autonomic and emotional modulations can be beneficial, such as pain and hypertension.
[Comparison of ovulation induction protocols for in vitro fertilization].

Abstract:In a retrospective study we evaluated 4 controlled ovarian hyperstimulation (COH) protocols for in vitro fertilization (IVF) treatment. Treatment cycles 4 through 8 were analyzed in patients, who in 3 previous attempts reached the stage of embryo transfer (ET) but did not conceive. The 426 recorded cycles included all indications for IVF, but not the male factor. The age range was 25-46 years. Patients underwent from 4 cycles (169 women) to 8 cycles (27 women). The COH protocols used included established combinations of menotropins, clomiphene citrate/hMG, and short and long protocols of GnRH analogs followed by hMG. There were no statistically significant differences in patient age, etiology and duration of infertility, or distribution of the various COH protocols among the groups of patients. No single protocol was significantly superior to any other when the pregnancy rate per cycle was assessed. The results did not justify changing a COH protocol which had lead to ET but not to pregnancy, for another protocol.
The effect of long term combined yoga practice on the basal metabolic rate of healthy adults

Abstract:Background: Different procedures practiced in yoga have stimulatory or inhibitory effects on the basal metabolic rate when studied acutely. In daily life however, these procedures are usually practiced in combination. The purpose of the present study was to investigate the net change in the basal metabolic rate (BMR) of individuals actively engaging in a combination of yoga practices (asana or yogic postures, meditation and pranayama or breathing exercises) for a minimum period of six months, at a residential yoga education and research center at Bangalore. Methods: The measured BMR of individuals practicing yoga through a combination of practices was compared with that of control subjects who did not practice yoga but led similar lifestyles. Results: The BMR of the yoga practitioners was significantly lower than that of the non-yoga group, and was lower by about 13 % when adjusted for body weight (P < 0.001). This difference persisted when the groups were stratified by gender; however, the difference in BMR adjusted for body weight was greater in women than men (about 8 and 18% respectively). In addition, the mean BMR of the yoga group was significantly lower than their predicted values, while the mean BMR of non-yoga group was comparable with their predicted values derived from 1985 WHO/FAO/UNU predictive equations. Conclusion: This study shows that there is a significantly reduced BMR, probably linked to reduced arousal, with the long term practice of yoga using a combination of stimulatory and inhibitory yogic practices.

Abstract:Control of respiration provides a powerful voluntary portal to entrain and modulate central autonomic networks. Slowing and deepening breathing as a relaxation technique has shown promise in a variety of cardiorespiratory and stress-related disorders, but few studies have investigated the physiological mechanisms conferring its benefits. Recent evidence suggests that breathing at a frequency near 0.1 Hz (6 breaths per minute) promotes behavioral relaxation and baroreflex resonance effects that maximize heart rate variability. Breathing around this frequency appears to elicit resonant and coherent features in neuro-mechanical interactions that optimize physiological function. Here we explore the neurophysiology of slow, deep breathing and propose that coincident features of respiratory and baroreceptor afferent activity cycling at 0.1 Hz entrain central autonomic networks. An important role is assigned to the preferential recruitment of slowly-adapting pulmonary afferents (SARs) during prolonged inhalations. These afferents project to discrete areas in the brainstem within the nucleus of the solitary tract (NTS) and initiate inhibitory actions on downstream targets. Conversely, deep exhalations terminate SAR activity and activate arterial baroreceptors via increases in blood pressure to stimulate, through NTS projections, parasympathetic outflow to the heart. Reciprocal SAR and baroreceptor afferent-evoked actions combine to enhance sympathetic activity during inhalation and parasympathetic activity during exhalation, respectively. This leads to pronounced heart rate variability in phase with the respiratory cycle (respiratory sinus arrhythmia) and improved ventilation-perfusion matching. NTS relay neurons project extensively to areas of the central autonomic network to encode important features of the breathing pattern that may modulate anxiety, arousal, and attention. In our model, pronounced respiratory rhythms during slow, deep breathing also support expression of slow cortical rhythms to induce a functional state of alert relaxation, and, via nasal respiration-based actions on olfactory signaling, recruit hippocampal pathways to boost memory consolidation. Collectively, we assert that the neurophysiological processes recruited during slow, deep breathing enhance the cognitive and behavioral therapeutic outcomes obtained through various mind-body practices. Future studies are required to better understand the physio-behavioral processes involved, including in animal models that control for confounding factors such as expectancy biases.

Abstract:Objective The present study investigated how breathing stimuli affect both non-linear and linear metrics of the autonomic nervous system (ANS). Approach The analyzed dataset consisted of 70 young, healthy volunteers, in whom arterial blood pressure (ABP) was measured noninvasively during 5-minute sessions of controlled breathing at three different frequencies: 6, 10, and 15 breaths/min. CO2 concentration and respiratory rate were continuously monitored throughout the controlled breathing sessions. The ANS was characterized using non-linear methods, including Phase-Rectified Signal Averaging (PRSA) for estimating heart acceleration and deceleration capacity (AC, DC), multiscale entropy (MSEn), approximate entropy (ApEn), sample entropy (SampEn), and fuzzy entropy (FuzzyEn), as well as time and frequency domains (low frequency, LF; high-frequency, HF; total power, TP) of heart rate variability (HRV). Main Results Higher breathing rates resulted in a significant decrease in end-tidal CO2 concentration (p < 0.001), accompanied by increases in both ABP (p<0.001) and heart rate (p<0.001). A strong, linear decline in AC and DC (p<0.001 for both) was observed with increasing respiratory rate. All entropy metrics increased with breathing frequency (p<0.001). In the time-domain, HRV metrics significantly decreased with breathing frequency (p<0.01 for all). In the frequency-domain, HRV LF and HRV HF decreased (p = 0.038 and p = 0.040, respectively), although these changes were modest. There was no significant change in HRV TP with breathing frequencies. Significance Alterations in CO2 levels, a potent chemoreceptor trigger, and changes in HR most likely modulate ANS metrics. Non-linear PRSA and entropy appear to be more sensitive to breathing stimuli compared to frequency-dependent HRV metrics. Further research involving a larger cohort of healthy subjects is needed to validate our observations.

Abstract:This study examined the effectiveness of diaphragmatic paced breathing at (0.1Hz) or obtaining biofeedback via a computer-game compared to normal breathing on heart rate variability (HRV). The aim was to determine whether individual differences exist and allow best practice on an individualized basis for relaxation and stress reduction. Six subjects participated in the repeat measures pilot study. HRV data was recorded during every session using HeartMath and analysed using Kubios software. Paced breathing indicated a substantial decrease in heart rate. Maximum change of low frequency power (%) was seen for the balloon condition with lesser change during paced breathing, The most changes between rest and intervention was observed for the very low frequency power (%) with no substantial differences between paced breathing and the balloon intervention. This suggests that the balloon game may more effective when breathing at approximately 0.1Hz and initiates vagal predominance which has been associated with reduced stress.

Abstract:Introduction Smoking is a serious public health problem that leads to chronic obstructive pulmonary disease (COPD). Its harmful effects are more significantly seen in the respiratory system. Approximately 30% of smokers experience compromised lung functions. However, asymptomatic smokers exhibit alteration in lung morphology. This study will give us a better understanding of the respiratory damages that may occur in asymptomatic smokers and if we can delay or prevent the same by the simple practice of Pranayama and Om chanting. Materials and methods An interventional study was conducted in the Department of Physiology at the Faculty of Medicine and Health Sciences, Shree Guru Gobind Singh Tricentenary (SGT) University, Gurugram, Haryana, India. The study duration was eight weeks, and a total of 135 subjects, including both male and female participants aged between 18 and 25 years, were included in this study. Baseline recordings of anthropometric parameters, spirometric parameters like forced vital capacity (FVC), forced expiratory volume (FEV) in 1 second (FEV1), FEV1/FVC ratio, forced expiratory flow (FEF) between 25% and 75% of vital capacity (FEF25-75%), peak expiratory flow rate (PEFR), forced inspiratory vital capacity (FIVC), peak inspiratory flow rate (PIFR) and for respiratory endurance - breath holding time at the end of inspiration (BHTi), breath holding time at the end of expiration (BHTe), 40 mmHg were recorded before the start of the study and again after eight weeks of alternate nostril breathing and om chanting performed for 10 minutes (5 minutes for each protocol). We also recorded nicotine dependence score with the help of Fagerstrom Test for Nicotine Dependence (FTND). A comparison of spirometric parameters, respiratory endurance, and FTND was done using the paired t-test. Result All spirometric measures and respiratory endurance parameters showed significant increases, with the exception of FVC, FEV1, FEV1/FVC, and FTND, which showed no significant improvement. Conclusion Pranayama and Om chanting were found to increase respiratory muscle endurance and support better utilization of the alveolo-bronchial tree, which may help in better oxygenation and delay in onset of the symptoms of COPD.

Abstract:Therefore, in the presence of a very narrow airway, precaution should be taken to avoid intubation with an oversized ETT. Appropriateness of the ETT can be assessed by ensuring air-leak at 20–25 cmH2O (2). During mechanical ventilation, a small ETT can result in high airway pressure; its lumen easily become compromised by secretions that may result in impaired ventilation and complications of auto-PEEP such as impaired venous return, hyperinflation of lungs, and elevated intra-alveolar pressures and increased afterload to the right ventricle (5). A rising phase III on capnogram suggests auto-PEEP; sufficient expiratory time and intermittent disconnection of ventilator circuit can prevent complications of auto-PEEP. Weaning and postextubation period pose special problems. Presumably, because of high airway resistance and low spontaneous TV our patient became distressed during weaning. The situation resolved once spontaneous TV increased to 50–60 ml by increasing PS. Therefore, we decided to extubate the trachea without further weaning. However, because of the possibility of airway edema and difficulty of breathing, possibility of re-intubation was considered and measures to rapidly control the airway were prepared. Airway edema may result in difficulty of clearing airway of secretions, increased work of breathing, and respiratory failure. In such situations, oxygen-enriched helium is useful. In an unfortunate event of inability to clear secretions and respiratory failure, re-intubation or tracheotomy may remain the only option. Re-intubation followed by tracheotomy within 2 h of extubation in similar circumstances is reported (3). Considering extreme problems of airway and ventilation management, in patients presenting with multiple congenital defects including cardiac and bony deformities, the airway should be thoroughly evaluated and if found narrowed, the elective surgery should be postponed. If the airway anomaly is discovered after anesthesia induction and intubation, it may be worthwhile to allow the child to recover from anesthesia and to postpone ⁄ cancel the elective surgery. However, in view of the possibility of airway trauma and edema, preparation for rapid airway control should be available. For general surgery, tracheotomy is an option; however, during open-heart surgery because of the fear of mediastinitis it is not.
