Abstract:The paper aims at investigating health information technologies impacts on electronic business in quality and revenue hospitals. Specifically, first, a hospital electronic customer chain (HECC) model is built to analyse details on how health information technologies are integrated in the hospital business to improve quality and revenue. Second, the HECC model was further decomposed to derive standardised electronic business solutions. Data was collected from two groups of hospitals with succeeding on quality and revenue, respectively. Third, a comparison was conducted to investigate the electronic business implementation pattern in both quality and revenue groups. The results from two-sample t-test show that the majority of the e-business solutions of the revenue group have the average score less than or equal to that of the quality group. The findings will help hospitals to accelerate the health information technologies adoption in the hospital electronic business. Theoretical and practical implications are also presented.

Abstract:The chemisorption of a two-level (at E1 and E2) adsorbate on the (001) surface of a tightbinding metal is investigated using the Green's function formalism and the phase shift technique. The adorbital density of states (DOS)ϱa(E) as well as the change in the electronic DOS Δϱ(E; E1, E2) due to chemisorption are calculated for the ordered overlayers with c(2 x 2), p(2 x 1), p(2 X 2), p(4 X 1) and c(4 X 2) structures. It is assumed that the chemisorbed species sit over the twofold bridge site of the (001) surface of the model transition metal and have a π-bonding interaction with the two substrate atoms. It is shown that the electronic states of the overlayers are very sensitive to the adsorbate coverage (0), adsorbate structure and adsorbate species (one level or two level adsorbates). Furthermore, it is shown that there are marked differences in the Δϱ(E) curves between the chemisorption of two level adsorbates Δϱ(E; E1, E2) and that of single level adsorbates Δϱ(E; E1) + Δϱ(E; E2) (simulating the changes in the electronic DOS during the dissociation of diatomic molecules).
Leveraging health information technology to achieve the "triple aim" of healthcare reform

Abstract:Objective: To investigate experiences with leveraging health information technology (HIT) to improve patient care and population health, and reduce healthcare expenditures. Materials and methods: In-depth qualitative interviews with federal government employees, health policy, HIT and medico-legal experts, health providers, physicians, purchasers, payers, patient advocates, and vendors from across the United States. Results: The authors undertook 47 interviews. There was a widely shared belief that Health Information Technology for Economic and Clinical Health (HITECH) had catalyzed the creation of a digital infrastructure, which was being used in innovative ways to improve quality of care and curtail costs. There were however major concerns about the poor usability of electronic health records (EHRs), their limited ability to support multi-disciplinary care, and major difficulties with health information exchange, which undermined efforts to deliver integrated patient-centered care. Proposed strategies for enhancing the benefits of HIT included federal stimulation of competition by mandating vendors to open-up their application program interfaces, incenting development of low-cost consumer informatics tools, and promoting Congressional review of the The Health Insurance Portability and Accountability Act (HIPPA) to optimize the balance between data privacy and reuse. Many underscored the need to "kick the legs from underneath the fee-for-service model" and replace it with a data-driven reimbursement system that rewards high quality care. Conclusions: The HITECH Act has stimulated unprecedented, multi-stakeholder interest in HIT. Early experiences indicate that the resulting digital infrastructure is being used to improve quality of care and curtail costs. Reform efforts are however severely limited by problems with usability, limited interoperability and the persistence of the fee-for-service paradigm-addressing these issues therefore needs to be the federal government's main policy target.

Abstract:Digital technology has encompassed all aspects of healthcare. There are many international and national organizations, guidelines, and formats available in health information systems (HIS), but many are presently still not being used in India. The aim is to give a flawless, secure, and user-friendly health information technology (IT) system for Indian healthcare. We discuss the timeline of digital technology in hospital administration, administrative applications, and the importance of clinical quality in health. Clinical perspectives of clinical information systems (CIS), both in acute as well as chronic clinical care models. Cross-integration of healthcare in IT (HIT) in electronic health records (EHR) or electronic medical records (EMRs), in chronic disease management (CDM) systems, and in clinical decision support systems (CDSS) are elaborated. Also, practical strategic application methods are discussed. The limitations of the current HIS software in India are mostly used for transaction reporting, prescription, and administrative tools. They lack CIS and strategic business applications as compared to mature multinational company (MNC) HIS software. Along with this, various features and levels of HIS Software, challenges of HIT adoption, Indian health IT standards, and the future framework of IT in health in India are systematically analyzed. We aim at all physicians in India and at all levels of practice, from individuals, group practices, health institutes, or corporate hospitals, and to encourage them to make strategic use of CIS and strategic IT applications in their individual practice and hospital management. Process Model of Governance for Agility: A Healthcare Sysmtem Implementation in China.

Abstract:For an organization to sustain in the dynamic environment today, agility has become a key solution in its survival struggle. In attaining agility capability the standard formula for an organization is to achieve high flexibility in responding to stimulus and change. From this perspective, we have found a unique case study in which a hospital has successfully implemented three healthcare IT systems -EMR, HIS and PACS within two months in responding to internal and external challenges. In this case study,21 informants were interviewed to find how institutional governance for agility influences the progress of Hospital Information Systems (HIS) implementation in ZCH hospital.Data analysis was performed in tandem with collected data, recursively iterating between the empirical data, theoretical lens, and literature, in the mould of building the process model. This process model of governance for agility suggests that an agile HIS implementation is achievable via strategic use of different IT governance models to promote greater use of organizational capabilities in preparing hospitals for change. It has value-added the literature by introducing different IT governance models at different phases of HIS implementation. Practitioners could plan an agile HIS implementation by referring to the model as a systematic roadmap in governing and strategizing hospital resources and capabilities.

Abstract:BACKGROUND: Experts consider health information technology key to improving efficiency and quality of health care.PURPOSE: To systematically review evidence on the effect of health information technology on quality, efficiency, and costs of health care.DATA SOURCES: The authors systematically searched the English-language literature indexed in MEDLINE (1995 to January 2004), the Cochrane Central Register of Controlled Trials, the Cochrane Database of Abstracts of Reviews of Effects, and the Periodical Abstracts Database. We also added studies identified by experts up to April 2005.STUDY SELECTION: Descriptive and comparative studies and systematic reviews of health information technology.DATA EXTRACTION: Two reviewers independently extracted information on system capabilities, design, effects on quality, system acquisition, implementation context, and costs.DATA SYNTHESIS: 257 studies met the inclusion criteria. Most studies addressed decision support systems or electronic health records. Approximately 25% of the studies were from 4 academic institutions that implemented internally developed systems; only 9 studies evaluated multifunctional, commercially developed systems. Three major benefits on quality were demonstrated: increased adherence to guideline-based care, enhanced surveillance and monitoring, and decreased medication errors. The primary domain of improvement was preventive health. The major efficiency benefit shown was decreased utilization of care. Data on another efficiency measure, time utilization, were mixed. Empirical cost data were limited.LIMITATIONS: Available quantitative research was limited and was done by a small number of institutions. Systems were heterogeneous and sometimes incompletely described. Available financial and contextual data were limited.CONCLUSIONS: Four benchmark institutions have demonstrated the efficacy of health information technologies in improving quality and efficiency. Whether and how other institutions can achieve similar benefits, and at what costs, are unclear.

Value of Information Analysis for a New Technology: Computer-Assisted Total Knee Replacement.

Abstract:Objectives: The aim of this study was to demonstrate how value of information analysis can measure the upper limit on returns to future research and identify the research priorities for computer-assisted total knee replacement (CAS-TKR).Methods: Using a previous economic analysis of CAS-TKR compared with conventional TKR, the population expected value of perfect information (EVPI) was calculated using Monte Carlo simulation to provide an estimate of the upper limit on returns to future research. The population expected value of partial perfect information (EVPPI) for both individual parameters and groups of parameters was estimated to inform specific future research priorities.Results: The UK individual EVPI would be £21.4 if the willingness to pay for one QALY (quality-adjusted life-year) were £30,000. The population EVPPI would be £8.3 million, assuming a 10-year time horizon for CAS-TKR. In this instance, the expected value of information is positively related to willingness to pay for one QALY for the range of £0 to £50,000. Although each individual parameter had an EVPPI of £0, groups of utility parameters had positive EVPPI. Population EVPPI was £5.6 million for utility parameters, £20,000 for transition probabilities relating to CAS-TKR, and £5,000 for transition probabilities related to conventional TKR.Conclusions: The study provides evidence on which parameters further information may be of most value. Focusing research on the utility values associated with health states relating to TKR would be of greatest value.

Abstract:Healthcare knowledge management systems can mitigate hospitals' operational inefficiency. As a healthcare information technology, the electronic health record (EHR) receives much attention from medical institutions due to its considerable impact on operational cost performance. This paper focuses on EHR systems to address operational inefficiency by which patients pay more for health care services, and many U.S. hospitals are filing for bankruptcy. From the theoretical perspective of the practice-based view, this paper introduces a path to implement EHR systems for improving cost performance. The empirical investigation is archival data of 200 hospitals collected from the U.S. healthcare agencies. Findings contribute to prior work by hypothesizing moderating and mediating roles in EHR systems implementation. This paper introduces absorptive capacity and monitoring mechanisms as enablers of implementing EHR systems. The results showed that hospital monitoring strengthens the relationship between absorptive capacity and electronic health record systems implementation, which results in better operational cost performance. Theoretically, this study supports the long-term potential benefits of EHR adoption, and its findings are consistent with optimizing efficiency through data standardization and interoperability. From a practical perspective, this study supports hospitals' investments in evolving healthcare information technology systems through the development of a knowledge-based system employing EHR, particularly when hospitals are merging or need a financial strategic plan to control expenses.

Abstract:There have been thousands of books and articles published as to the utility and value of information technology (IT) in support of the business functions in today's society. There have probably been as many studies conducted to justify this “supposition.” Most pharmaceutical industry professionals would concede that information technology has utility, and through that utility, it has value; but can that value be defined? And does that value extend to innovation in the use of information or the conduct of business? This paper summarizes how the information technologists in pharmaceutical research define, measure, justify, disseminate, and communicate the value of IT to clients and management. The results of an industry-wide, multi-disciplinary survey are provided and discussed. Finally, some recommendations are given to improve the process of valuing, justifying, expanding, and simply understanding the use and role of IT in the pharmaceutical industry.

Abstract:Purpose This paper advances a research agenda for service researchers at the intersection of healthcare and information technologies to improve access to quality healthcare at affordable prices. The article reviews key trends to provide an agenda for research focusing on strategies, governance and management of key service processes. Design/methodology/approach This paper synthesizes literature in information systems, service management, marketing and healthcare operations to suggest a research agenda. The authors draw on frameworks such as the interpretive model of technology, technology acceptance model, assemblage theories and Baumol's cost disease to develop their arguments. Findings The paper situates strategy-related service management questions that service providers and consumers face in the context of emerging healthcare and technology trends. It also derives implications for governance choices and questions related to that. Research limitations/implications The paper discusses service management challenges and concludes with an agenda for future research that touches on governance and service management issues. Practical implications This paper provides implications for healthcare service providers and policymakers to understand new trends in healthcare delivery, technologies and facilities management to meet evolving customer needs. Social implications This paper provides implications for managing healthcare services that touch on many social and societal concerns. Originality/value This conceptual paper provides background and review of the work at the intersections of information systems, marketing and healthcare operations to draw implications for future research.

Employing health information technology in the real world to transform delivery

Abstract:Objectives: Strong leadership and a supportive culture are critical to effective organizational transformation, but organizations pursuing change also need the infrastructure and tools to do so effectively. As policy makers seek to transform healthcare systems-specifically the delivery of care-we explore the real-world connection between health information technology (HIT) and the transformation of care delivery. Study design and methods: This study is based on interviews with diverse federal and health system leaders and federal officials. The work was funded by the Office of the National Coordinator for Health Information Technology as part of a global assessment of the Health Information Technology for Economic and Clinical Health Act. Results: The functionalities supported by HIT are integral to creating the information flow required for innovations such as medical homes, accountable care organizations, and bundled payment. However, such functionalities require much more than the presence of electronic health records; the data must also be liquid, integrated into the work flow, and used for analysis. Even in advanced systems, it takes years to create HIT infrastructure. Building this infrastructure and transforming delivery simultaneously is difficult, although probably unavoidable, for most providers. Progress will likely be slow and will require creative strategies that take into account the real-world environment of organizations and communities. Conclusions: While the rapid transformation of delivery and infrastructure is appealing, both types of change will take time and will progress unevenly across the nation. Policy makers serious about transforming the delivery of healthcare can benefit by recognizing these realities and developing practical strategies to deal with them over a relatively long period of time.
Assessing the strategic value of Information Technology: An analysis on the insurance sector

Abstract:Even though companies today are well aware of the pitfalls in making IT investments and that many IT applications have become commodities, IT projects still fail. Obviously IT has a strategic value to firms but they have not determined how IT management capabilities and IT governance schemes explain commonalities and differences in their use of IT.We analyzed the insurance industry in the US and Europe through case studies and an analysis of 30 Italian firms. Twenty case studies carried out between 1998 and 2003 led us to conclude that technological and business path dependencies, along with time compression diseconomies, resulted in diversities in IT adoption dynamics due to their differences in IT governance and management practice. Our analysis showed that most of the firms in the Italian insurance sector increased their productivity through IT regardless of their IT management capabilities. It also showed that competitive advantages were not correlated with IT spending levels nor with the kind of IT investments that made general productivity growth in the industry possible.

Abstract:Background and aims: Although many health strategic plans have been developed by scholars and organizations, they still suffer from a limited view. Since most health-related strategies in the future will depend on information technology (IT), as the main driver of today's industry, technology, and society, IT merits attention in health strategic plans. While the majority of the health strategic plan developed based on the interviews and questioner and these plans didn't consider the role of IT in their actions, this research will develop a framework to integrate risk and maturity analysis with the strategic planning process in health information technology strategic plan. Methods: The present research introduces an integrated framework based on a balanced scorecard (BSC) and control objectives for information and related technologies (COBIT). Also, The American Productivity & Quality Centre framework and COBIT were employed in this model to define the processes and activities of health IT (HIT) organizations. The organization's maturity and risk are analyzed in terms of information and management criteria using BSC, COBIT, and the analytical hierarchy process. Results: Later this model was implemented in a Remote Health care System to improve the strategic management process for this technology. Using this framework, 17 business goals have been developed and presented for the case. Also, the total related risk was 48% and the maturity level was at 1/18. The results are presented as decision-making parameters and strategies for successful IT implementation. Conclusion: The presented framework provides deeper insight for the decision-maker through integrating risk and maturity analysis in the HIT strategic planning process. These strategies help investors decide about resource allocation based on the risk-taking capability, certainty, and uncertainty results of the plan.

Abstract:Objective: This study evaluated the influence of technology on accurately measuring costs using time-driven activity-based costing (TDABC) in healthcare provider organizations by identifying the most recent scientific evidence of how it contributed to increasing the value of surgical care. Methods: This is a literature-based analysis that mainly used two data sources: first, the most recent systematic reviews that specifically evaluated TDABC studies in the surgical field and, second, all articles that mentioned the use of CareMeasurement (CM) software to implement TDABC, which started to be published after the publication of the systematic review. The articles from the systematic review were grouped as manually performed TDABC, while those using CM were grouped as technology-based studies of TDABC implementations. The analyses focused on evaluating the impact of using technology to apply TDABC. A general description was followed by three levels of information extraction: the number of cases included, the number of articles published per year, and the contributions of TDABC to achieve cost savings and other improvements. Results: Fourteen studies using real-world patient-level data to evaluate costs comprised the manual group of studies. Thirteen studies that reported the use of CM comprised the technology-based group of articles. In the manual studies, the average number of cases included per study was 160, while in the technology-based studies, the average number of cases included was 4,767. Technology-based studies, on average, have a more comprehensive impact than manual ones in providing accurate cost information from larger samples. Conclusion: TDABC studies supported by technologies such as CM register more cases, identify cost-saving opportunities, and are frequently used to support reimbursement strategies based on value. The findings suggest that using TDABC with the support of technology can increase healthcare value.

Abstract:The importance of the impact of IT for organizations around the world, especially in light of a very slow recovery from the global financial crisis, has amplified the need to provide a better understanding of the specific geographic similarities and differences of IT managerial and technical trends. Going beyond identifying these influential factors is also the need to understand the considerations for addressing them in light of recognizing the respective local characteristics, especially when operating in a globally linked environment, although somehow heavily influenced locally. By comparing and contrasting IT trends from different geographies, this paper presents important local and international factors (e.g., management concerns, influential technologies, budgets/spending, organizational considerations) necessary to prepare IT leaders for the challenges that await them. It can also serve as an indicator as the respective geographies evolve from the economic conundrum. The research is based on data from four geographic regions (United States (US), Europe, Asia, and Latin America). The same questionnaire (although translated for the respective respondents), based on the lead authors well-respected and long-running Society for Information Management survey, was applied across the geographies. This paper presents the major findings based on survey responses from 620 respondents (275 US, 100 European, 59 Asian, and 186 Latin) in mid-2011. The top five management concerns were: (1) IT and business alignment; (2) business agility and speed to market; (3) business process management and business process reengineering; (4) business productivity and cost reduction; (5) IT reliability and efficiency. The five most influential technologies were: (1) business intelligence; (2) enterprise resource planning systems; (3) cloud computing; (4) mobile and wireless applications; (5) customer relationship management.

Abstract:The environmental impact of the healthcare sector has become an important factor globally and is continuing to draw the attention of regulators. The energy use of the healthcare sector (whose largest sub-segment is 'hospitals') has been growing due to many factors. These factors include the rapid growth and adoption of Information and Communication Technology (ICT) in healthcare. The new IT technologies and applications used in healthcare include 'cloud computing', `mMedicine', i.e. `mobility in Health', eHealth, and tele(health) care for `remote delivery of healthcare services'. The healthcare industry needs to reap the benefits of emerging technologies such as Data Science, Big Data, Analytics, Mobile computing and Cloud computing, along with the use of Health Information Technology (HIT) to help solve the ever growing operating cost problems. Because of the complexity of the healthcare industry, there is a need for an extensive use of data science and Big Data analytics as an ideal way to help determine how the green healthcare outcomes can be improved. Hospitals and healthcare can make significant steps in the green direction by combining big data analytics with cloud computing. Information Technology (IT) is continually being refreshed at hospitals and most organizations as part of the need for the hospitals to keep vital technology up to date. Any electronic waste (eWaste) from this refresh cycle must be recycled as part of a sustainability program. Replacing the IT equipment and upgrading the application architecture with energy efficient systems such as virtual servers, virtual data storage, and efficient application and database structures, can easily reduce IT power consumption for the replaced equipment by 50 percent. A second compelling reason to move to green IT is that virtual server and virtual data storage technology are methods that allow hospitals to reduce equipment and system management costs. Include private cloud computing for both your production and test / development systems, and the savings are even greater. So the latest green technology is based around a very solid business case without even considering the savings due to reduction in energy costs. This paper gives examples of how Data Science, Big Data, and Analytics can be used in healthcare to significantly improve a hospital's IT Energy Efficiency along with information on the best ways to improve energy efficiency for healthcare in a cost effective manner. This paper builds on the work done in other sectors (mainly data centers) in effectively measuring and improving IT energy efficiency.

Abstract:Current research on digital transformation in the healthcare context is limited, and doesn't consider the patient's definition of value, the need to support traditional healthcare process improvement in parallel with new value creation activities, nor does it clearly examine changes that occur in the structure of healthcare organizations due to technologically driven change. This research seeks to address these issues and determine the capabilities that influence the creation of new and existing value propositions and the subsequent structural changes of digital transformation in healthcare by analyzing interviews with 19 American healthcare executives. Results highlight the need for patient-focused modes of value creation necessary to improve care coordination, which is affected by the ability to integrate data and reconfigure processes. End user focused modes compel healthcare organizations to digitize their services, however, this can be hampered by difficulties with change management and enforcing IT governance processes.

Abstract:This chapter discussed the importance of enterprise resource planning (ERP) in different organization and how it will enhance productivity for the workers and for the patients by providing best services using innovative technologies. Also, this research will define the impact of innovative technologies in organization productivity with ERP system. Moreover, this chapter determine dependent which is organization productivity and independent variables such as RFID, telemedicine, mobility, artificial intelligence and innovative technologies which can be integrated with ERP system and it will help in improving organization productivity to describe the best use of ERP system to the healthcare organization when adopting innovative technology. As a result, implementing innovative technologies within healthcare organizations it will benefit the patients and physicians working in organization. Hence, the critical role of IT department is to determine the overall success in organizations and provide flexible, economical services to physicians, patients and end users involved in the organization.
