Abstract:This issue of the <i>Western Journal of Nursing Research</i> includes our analysis of data from 665 acute care hospitals and over 1.3 million adult surgical patients showing that patients cared for in hospitals with better nurse–physician teamwork had significantly lower odds of both 30-day mortality and failure to rescue. This finding is in line with other research and affirms the many calls from the National Academy of Medicine and other organizations to improve nurse–physician teamwork to achieve the highest quality of care and clinician well-being. The question now is: What do we do? In our view, the answer for researchers is to develop and test interventions at scale. To be sure, there have been advances in intervention research on teamwork. For example, there is evidence on interventions that focus on the process of communication (e.g., education and training in communication skills, and simulation exercises), as well as interventions aimed at creating shared time and care processes like interdisciplinary rounds and unit-based physician assignments (<a class="ref showRefEvent2" href="#">O'Leary et al., 2012</a>). These are a much needed and valuable contribution, but there continues to be significant variation in the quality of nurse–physician teamwork across hospitals. Why does this problem remain for so many hospitals if we know what to do to improve nurse–physician collaboration? One potential explanation highlights an opportunity for a different intervention research agenda—a focus on the work environment in which collaboration (and interventions aimed at improving collaboration) occurs. By work environment, we refer to the characteristics of hospitals that promote the highest quality of professional practice and clinical care including professional autonomy, effective leadership, an authentic voice in hospital policy, and adequate staffing and resources.

Abstract:We surveyed 181 nursing research leaders from Magnet® hospitals, using mixed methods with the online Hospital-Based Nursing Research Clinical and Economic Outcomes survey, to describe the clinical and economic outcomes of nursing research conducted in hospital settings. We used descriptive statistics to analyze the quantitative findings and a qualitative descriptive approach to study the open-ended responses. Most respondents reported that findings from their hospital-based studies were implemented on their units (88.2%), improved health care processes (88.2%), and reduced hospital costs (79%). Over 50% reported positive impacts on core quality measures, including improving patient/family satisfaction (76.8%), nurse satisfaction (65%), length of stay (59.1%), and infection rates (56.5%). Four themes were identified: study evaluation, improvements in care delivery/clinical outcomes, economic impact, and intrinsic and extrinsic rewards. Much of the research reported by respondents focused on quality measures with findings that resulted in improved clinical and economic outcomes.

Defining an evidence-based work environment for nursing in the USA

Abstract:Aims: To describe and compare staff nurses' perceptions of their real and ideal work environment in a tertiary paediatric facility; to provide administrators with research evidence for identifying areas for improvement. Background: The workforce shortage of hospital nurses is a global problem having an impact on the financial resources and efficient operations of an organisation and the quality of care delivered to patients. A direct relationship exists between job satisfaction, retention, turnover and elements of the nurses' work environment. Research identifying specific elements of the current work setting that are in conflict with nurses' views of their preferred practice milieu can help define an evidence-based work environment for nursing. Design and methods: Using a descriptive survey design, a convenience sample of 385 nurses on five inpatient units and the float team completed two forms (real and ideal) of the work environment scale. The work environment scale consists of 10 subscales within three dimensions: relationship, personal growth and system maintenance and change. Results: A consistent pattern was seen across all units with reported high levels of involvement, peer cohesion, task orientation and managerial control. Scores for work pressure and autonomy were moderate-high and physical comfort, supervisor support, clarity and innovation were moderate. Overall, highest scores were reported for involvement and lowest for physical comfort. Significant differences were found between real and ideal subscale scores suggesting that staff were able to identify areas for improvement. Conclusions: Despite moderate work pressure, staff affirmed a highly positive work environment on their units. Specific areas in their current work setting that were not congruent with their preferred work environment were identified and targeted for change. Relevance to clinical practice: Understanding dimensions of the nurses' work environment needing improvement and involving staff in making and evaluating change supports an evidence-based environment to attract and retain qualified staff.

Abstract:Objective: Research plays a critical role in molding nursing as a profession. Healthcare organizations are challenged to build an organizational culture that cultivates the development of nursing research. Creating a culture of nursing research is constantly branded a vital component in advancing nursing science. Therefore, this study described nurses' perceptions about their hospital's nursing research culture. Methods: Semi-structured personal interviews were used to collect data. Seventeen staff nurses were selected to participate in the study by purposive sampling method from December 2022 to April 2023 in the eastern Philippines. The researchers used Braun and Clarke's thematic approach to analyze the qualitative data. Results: The analysis revealed three central themes and seven subthemes: influencing behavior and performance of the employee (help in research information and learning, flexibility and adaptability, collaboration and communication among employees), influencing staff nurses' research participation (leadership strategies, training and professional development), and affecting hospitals to adapt to change (organizational performance and success, employee engagement and job satisfaction). Conclusions: The findings support the benefits of a positive nursing research culture in hospitals due to its critical role in shaping nurses' behavior and performance, participation in nursing research, and hospital adaptation to changes. Collectively, these themes contribute to a deeper understanding of the multifaceted impact that a culture of nursing research can have on various levels of the healthcare system.

Abstract:Within a large integrated health care system that includes local, regional, and national medical centers and offices, quality and patient safety teams, and research institutes, more than 58 000 nurses are employed, with the vast majority providing point-of-care service to patients and members. A small but increasingly important number of nurses are involved in quality improvement and research teams to improve patient care. Within this environment, a number of developments point to a growing need for nursing leadership in systems-level and delivery science policies and research. This article describes a partnership between clinical operations leaders and researchers (nurse scientists) to determine and utilize evidence-based practices to meet a system goal. The first objective of this article is to describe the barriers and potential solutions in combining practice, quality improvement, and research efforts across regions within a large integrated health care system. The second objective is to describe the uniqueness and importance of nurse leaders across interdisciplinary teams to simultaneously pursue research, quality improvement, and operational goals for the organization.

Effectiveness and safety of warfarin initiation in older hemodialysis patients with incident atrial fibrillation.

Abstract:BACKGROUND AND OBJECTIVES Although generally recommended in atrial fibrillation (AF) patients, the effectiveness and safety of oral anticoagulation in dialysis patients with AF is unknown. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS We assembled a cohort of older hemodialysis patients who initiated dialysis without prior record of AF and who had prescription drug benefits through three state-administered programs. The index event was a first hospitalization with diagnosed AF; patients with any recorded prior warfarin use were excluded. Eligible patients survived ≥30 days from discharge, and new warfarin use was recorded from prescription records during that 30-day window. Propensity-matched warfarin users and nonusers were compared using Cox regression. Outcomes included ischemic stroke, hemorrhagic stroke, and mortality. RESULTS Among 2313 patients with new AF who survived 30 days from discharge, 249 (10.8%) filled a prescription for warfarin. Comparing 237 warfarin users and 948 propensity-matched nonusers over 2287 person-years of follow-up, the occurrence of ischemic stroke was similar (HR = 0.92; 95% CI, 0.61 to 1.37), whereas warfarin users experienced twice the risk of hemorrhagic stroke (HR = 2.38; 95% CI, 1.15 to 4.96). The risks of stroke, gastrointestinal hemorrhage, and mortality did not differ between groups. As-treated analyses yielded similar findings, as did analyses restricted to patients with CHADS(2) scores ≥2. CONCLUSIONS Although we confirmed association between warfarin use and hemorrhagic stroke in dialysis patients with AF, we found no association between warfarin use and ischemic stroke. Adequately powered randomized trials are required to conclusively determine the risks and benefits of the studied warfarin indication in hemodialysis patients.

Abstract:This study provides a comprehensive evaluation of nurse resources in Chinese hospitals and the link between nurse resources and nurse and patient outcomes.Survey data were used from 9688 nurses and 5786 patients in 181 Chinese hospitals to estimate associations between nurse workforce characteristics and nurse and patient outcomes in China. Nurse and patient assessments in China were compared with a similar study in Europe.Thirty-eight percent of nurses in China had high burnout and 45% were dissatisfied with their jobs. Substantial percentages of nurses described their work environment and the quality of care on their unit as poor or fair (61% and 29%, respectively) and graded their hospital low on patient safety (36%). These outcomes tend to be somewhat poorer in China than in Europe, though fewer nurses in China gave their hospitals poor safety grades. Nurses in Chinese hospitals with better work environments and higher nurse-assessed safety grades had lower odds of high burnout and job dissatisfaction (ORs ranged from 0.56 to 0.75) and of reporting poor or fair quality patient care (ORs ranged from 0.54 to 0.74), and patients in such hospitals were more likely to rate their hospital highly, to be satisfied with nursing communications, and to recommend their hospitals (significant ORs ranged from 1.24 to 1.40). Higher patient-to-nurse ratios were associated with poorer nurse outcomes (each additional patient per nurse increases both burnout and dissatisfaction by a factor of 1.04) and higher likelihoods of nurses reporting poor or fair quality of care (OR=1.05), but were unrelated to patient outcomes. Higher percentages of baccalaureate nurses were strongly related to better patient outcomes, with each 10% increase in the percent of baccalaureate nurses increasing patient satisfaction, high ratings, and willingness to recommend their hospital by factors ranging from 1.11 to 1.13.Nursing is important in quality and safety of hospital care and in patients' perceptions of their care. Improving quality of hospital work environments and expanding the number of baccalaureate-prepared nurses hold promise for improving hospital outcomes in China.

Nursing skill mix in European hospitals: cross-sectional study of the association with mortality, patient ratings, and quality of care

Abstract:OBJECTIVES: To determine the association of hospital nursing skill mix with patient mortality, patient ratings of their care and indicators of quality of care.DESIGN: Cross-sectional patient discharge data, hospital characteristics and nurse and patient survey data were merged and analysed using generalised estimating equations (GEE) and logistic regression models.SETTING: Adult acute care hospitals in Belgium, England, Finland, Ireland, Spain and Switzerland.PARTICIPANTS: Survey data were collected from 13 077 nurses in 243 hospitals, and 18 828 patients in 182 of the same hospitals in the six countries. Discharge data were obtained for 275 519 surgical patients in 188 of these hospitals.MAIN OUTCOME MEASURES: Patient mortality, patient ratings of care, care quality, patient safety, adverse events and nurse burnout and job dissatisfaction.RESULTS: Richer nurse skill mix (eg, every 10-point increase in the percentage of professional nurses among all nursing personnel) was associated with lower odds of mortality (OR=0.89), lower odds of low hospital ratings from patients (OR=0.90) and lower odds of reports of poor quality (OR=0.89), poor safety grades (OR=0.85) and other poor outcomes (0.80<OR<0.93), after adjusting for patient and hospital factors. Each 10 percentage point reduction in the proportion of professional nurses is associated with an 11% increase in the odds of death. In our hospital sample, there were an average of six caregivers for every 25 patients, four of whom were professional nurses. Substituting one nurse assistant for a professional nurse for every 25 patients is associated with a 21% increase in the odds of dying.CONCLUSIONS: A bedside care workforce with a greater proportion of professional nurses is associated with better outcomes for patients and nurses. Reducing nursing skill mix by adding nursing associates and other categories of assistive nursing personnel without professional nurse qualifications may contribute to preventable deaths, erode quality and safety of hospital care and contribute to hospital nurse shortages.

Relationship between nurse burnout, patient and organizational outcomes: Systematic review

Abstract:Background: Burnout, characterized by emotional exhaustion, depersonalization, and decreased personal accomplishments, poses a significant burden on individual nurses' health and mental wellbeing. As growing evidence highlights the adverse consequences of burnout for clinicians, patients, and organizations, it is imperative to examine nurse burnout in the healthcare system. Objective: The purpose of this review is to systematically and critically appraise the current literature to examine the associations between nurse burnout and patient and hospital organizational outcomes. Design and data sources: A systematic review following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses was conducted. PubMed, CINAHL, PsychInfo, Scopus, and Embase were the search engines used. The inclusion criteria were any primary studies examining burnout among nurses working in hospitals as an independent variable, in peer-reviewed journals, and written in English. The search was performed from October 2018 to January 2019 and updated in January and October 2020. Results: A total of 20 studies were included in the review. The organizational-related outcomes associated with nurse burnout were (1) patient safety, (2) quality of care, (3) nurses' organizational commitment, (4) nurse productivity, and (5) patient satisfaction. For these themes, nurse burnout was consistently inversely associated with outcome measures. Conclusions: Nurse burnout is an occupational hazard affecting nurses, patients, organizations, and society at large. Nurse burnout is associated with worsening safety and quality of care, decreased patient satisfaction, and nurses' organizational commitment and productivity. Traditionally, burnout is viewed as an individual issue. However, reframing burnout as an organizational and collective phenomenon affords the broader perspective necessary to address nurse burnout. A research internship scheme for nurses and its wider implications for clinical managers

Abstract:There is growing evidence that clinical research activity is linked to better patient outcomes and that staff involvement in research is linked to enhanced morale and retention. Clinical managers have a pivotal role in supporting staff to engage with research, but they are not always given the means to do so and are not always aware of the benefits. In 2021 a research internship scheme was set up as a collaboration between two London NHS trusts and a university, enabling nurses and midwives to undergo training and undertake a range of research activities. Some participants experienced challenges in fitting internship activities around clinical duties despite the fact that the scheme was planned to give them protected time. This article describes the scheme, reports the findings of its evaluation at one of the two trusts, and discusses its implications for clinical managers in terms of how they can be supported to contribute to the development of a research culture.

Abstract:Background: The health and well-being of the nursing workforce has received recent attention due to nurse attrition and the critical nurse shortages projected across the country. A nurse’s well-being may impact patient outcomes. Purpose: The purpose of this scoping review was to assess the association between nurse well-being factors and specific patient outcomes. Methods: The preferred reporting items for systematic reviews and meta-analyses (PRISMA) PRISMA Scoping Review protocol and 2020 reporting guidelines were utilized in this review. Results: Staffing, environment, physical health, and mental health of nurses were correlated to specific adverse patient outcomes among the 97 articles included in the final review. The majority of the articles reported significant findings. Conclusions: Patient outcomes were reviewed as discrete events in the articles examined. With mixed results found on key patient outcomes, future research requires more in-depth investigation into the role nurse well-being has on patient outcomes.

Abstract:Despite being considered the proverbial backbone of our healthcare systems, nursing still seems to struggle to scientifically demonstrate its contribution to care experiences and patient outcomes. This leads to erosive tendencies that threaten the development of the profession and its progress as an academic discipline. With this paper, we want to contribute to the theoretical discourse concerning the nature of nursing and the research into its effectiveness. We begin by outlining a set of prevailing paradoxes and their consequences relating to nursing and nursing research: the issue of demonstrating its unique contribution despite a clear societal mandate; a discrepancy between subjectively experienced effectiveness and objectively ascertainable effectiveness; and a mismatch between theoretical premises of nursing and task‐oriented cultures in practice environments. Using an example of a seemingly simple nursing intervention, we intend to demonstrate the qualities and complexities of nursing. We further illustrate this by drawing on several of our research projects using theory‐based evaluation methodologies. From these illustrative examples, we distil two insights relating to nursing interventions that we consider fundamental: the nurse, as a person, is central to its unique effectiveness; and there is always an interplay between context, intervention and its intended effect. We summarise our considerations and argue the case for conceiving research designs in alignment with theoretical premises of nursing.

Abstract:Medical Care Research and Review, Ahead of Print. Addressing patient experience is a priority in the health care system. Hospital Consumer Assessment of Providers and Systems (HCAHPS) survey results incentivize hospitals to elevate patient experience, a factor in patient-centered care. Although hospital nursing resources have been positively associated with better HCAHPS ratings, it is unknown how changes in nursing resources are associated with changes in HCAHPS ratings over time. This two-period longitudinal study ranked the associations between changes in nurse staffing, skill mix, nurse education, and work environment on HCAHPS ratings and found that changes in the work environment had the strongest associations (β = 2.29; p < .001) with improved HCAHPS ratings. Our findings provide hospital administrators with empirical evidence that may help make informed decisions on how to best invest limited resources to improve HCAHPS ratings, including the potential utility of improving the work environment through enhancing Nursing Quality of Care and Nurse Participation in Hospital Affairs.

Impact of collaboration behavior among staff nurses on their quality of work life and job satisfaction

Abstract:Background Nursing collaboration is a key component of promoting a good work environment in a health-care organization, as it helps to support acceptable workloads and levels of complexity, which leads to improved nurses' quality of life and job satisfaction. Aim To assess the impact of collaboration behavior among staff nurses on their quality of work life and job satisfaction. Design A descriptive correlational study design was utilized. Setting The study was conducted in the medical critical care units at New Kasr El Aini Teaching Hospital. Subject A convenience sample of staff nurses ( n =90) who are working in the previous mentioned setting. Tools Three questionnaires were used for this study: nurse collaboration behavior scale (20 items), quality of work life (38 items), and job satisfaction questionnaire (28 items). Results Results of the present study regarding collaboration behavior items revealed that around two-thirds (64.4%) of the participants agreed on listening to each other's opinions and suggestions item, while 34.4% of them disagreed about no qualms in sharing their ideas with one another item. Also, participants were highly perceived for all-quality work life dimensions (mean%=72.1%) and most of the study participants had a moderate level of job satisfaction (78.9%). However, this study showed that there was a statistically significant positive correlation between nurse collaboration behavior and quality of work life, but no correlation was found between nurse collaboration behavior and job satisfaction ( P =0.000). Conclusion The current study concluded that the majority of participants agreed about most of collaboration behavior items, participants were highly perceived for all-quality work life dimensions, and most of them had a moderate level of job satisfaction. However, there was statistically significant positive correlation between collaboration behavior and quality of work life, but no statistically significant correlation between collaboration behavior and job satisfaction as well as there was a positive statistically significant correlation between quality of work life and job satisfaction. Recommendations Nurse managers should display collaboration behaviors to construct an engaging climate, collaborative relationship with nurses and health team to preserve quality of patients' care, and hospital administration must regularly monitor nurse job satisfaction and resolve points of dissatisfaction to encourage staff affiliation with the organization and to improve quality of work life.

Abstract:Background A favourable nursing practice environment and adequate nurse staffing have been linked to reduced patient mortality. However, the contribution of nursing care processes such as patient surveillance and escalation of care, on patient mortality are not well understood. Objective The aim of this study was to investigate the effect of the nursing practice environment, nurse staffing, missed care related to patient surveillance and escalation of care on 30-day inpatient mortality. Design A multi-source quantitative study including a cross-sectional survey of nurses, and retrospective data extracted from an audit of medical and admission records. Setting(s) A large tertiary teaching hospital (600 beds) in metropolitan Sydney, Australia. Methods Data on the nursing practice environment, nurse staffing and missed care were obtained from the nursing survey. Patient deterioration data and patient outcomes data were collected from the medical and admission records respectively. Generalised estimating equations were used to examine the association between the nursing practice environment, patient deterioration and 30-day inpatient mortality accounting for clustering of episodes within patients logistic. Results Surveys were completed by 304 nurses (84.5 % female, mean age 34.4 years, 93.4 % Registered Nurses) from 16 wards. Patient deterioration data was collected for 30,011 patient deterioration events and 63,847 admitted patient episodes of care. Each additional patient per nurse (OR = 1.22, 95 % CI = 1.04–1.43) and the presence of increased missed care for patient surveillance (OR = 1.13, 95 % CI = 1.03–1.23) were associated with higher risk of 30-day inpatient mortality. The use of a clinical emergency response system reduced the risk of mortality (OR = 0.82, 95 % CI = 0.76–0.89) A sub-group analysis excluding aged care units, identified a 38 % increase in 30-day inpatient mortality for each additional patient per nurse (OR = 1.38, 95 % CI = 1.15–1.65). The nursing practice environment was also significantly associated with mortality (OR = 0.79, 95 % CI: 0.72–0.88) when aged care wards were excluded Conclusions Patient mortality can be reduced by increasing nurse staffing levels and improving the nursing practice environment. Nurses play a pivotal role in patient safety and improving nursing care processes to minimise missed care related to patient surveillance and ensuring timely clinical review for deteriorating patients reduces inpatient mortality. Abstract:Rising healthcare costs and the nursing shortage have affected the ability of healthcare organizations to provide a collaborative environment for high-quality care. Recent studies show that the nursing shortage has resulted in increased work loads, fewer support resources, and nurse dissatisfaction, resulting in difficulty providing quality care. Henneman cited a lack of collaboration as a contributing factor to the fragmentation of care and poor outcomes which plague our healthcare system. Knaus et al found that hospitals where collaboration was present reported 41% lower mortality than predicted number of deaths. Hospitals where there was a little collaboration exceeded predicted mortality by 58%. Positive collaborative relations have also been tied to a decrease in negative patient outcomes, increased organizational commitment, and nurse satisfaction as well as reduced cost and greater responsiveness for healthcare providers. The aim of this discussion is to introduce the participant to the concept of collaboration and use of the Donabedian structure-process-outcome model to provide a framework for embedding best practice components necessary for multidisciplinary collaboration in an acute care setting. The National Joint Practice Commission recommendations and the work of Schmalenberg et al were utilized to establish structural and process components necessary for a collaborative practice environment. Trinity Regional Health System utilized this information in conjunction with the Center for Case Management to develop a care model and improve patient outcomes. The average length of stay (LOS) decreased from 4.24 to 3.37 days and cost per admission from $6723 to $5919 in just over 1 year.

The findings demonstrate a positive correlation between medication adherence and improvements in health, underscoring the importance of well-structured medication management .Additionally, the study identified variations in satisfaction levels based on different education levels, underscoring the necessity for tailored patient education strategies. Healthcare professionals, providing their perspectives, express positivity regarding wound care interventions and stress the practical value of ongoing training and resources in this field. These results contribute significantly to existing knowledge, offering nuanced insights into the specific impacts of nursing interventions. The study provides evidence-based recommendations for practitioners and policymakers to enhance patient care strategies.
