Abstract:Pediatric idiopathic nephrotic syndrome (INS) is characterized by massive albuminuria, hypoproteinemia, edema and hyperlipidemia, with a long course and high probability of relapse and prolongation. Long-term complications caused by long-term usage of hormones and immunosuppressants in children with INS seriously affect their physical and mental health and quality of life. Most children with steroid-sensitive nephrotic syndrome can be cured before adulthood, while some of them relapse in adulthood. Long-term prognosis of children with steroid-resistant nephrotic syndrome is poor. There have been few studies in China followed the long-term outcomes and its related factors of children with INS over 10 years. The paper reviewed the literatures on the long-term outcomes of children with INS, including renal survival, growth, mental health, learning and work, marriage and fertility, disease recurrence and long-term related complications, to explore the factors related to the poor long-term outcomes of children with INS and to assist in clinical decision-making and follow-up management.
Clinical and Pathological Features of Young Patients with Idiopathic Membranous Nephropathy in 20 Cases

Abstract:Objective To investigate the clinical and pathological features of idiopathic membranous nephropathy (IMN) in young patients.Methods Clinical data of 20 young patients, 16 to 44 years, who were diagnosed as IMN admitted to the Zhejiang Provincial People's Hospital from January 2013 to December 2014 were retrospectively analyzed, comparing to 55 mid-aged patients who were diagnosed as IMN during the same period in the hospital.Clinical and pathological features of above mentioned patients were analyzed.Results Young patients with IMN accounted for 26.7％ of IMN patients.Compared to mid-aged patients, young patients with IMN had lower proportion of hypertension (P=0.003), lower blood glucose level (P=0.010), higher estimated glomerular filtration rate (eGFR) and low-density lipoprotein (LDL) (P=0.012, P=0.038), and lower levels of T3 and T4 (P=0.030, P=0.034).Furthermore, there were less sclerosis glomeruli (P＜ 0.001), hyaline change of arteriole (P=0.040) and arteriolar wall thickening (P ＜ 0.0001), lower positive ratios of IgA (P=0.008),and more without renal tubulointerstitial lesions (P=0.018) in young patients.There were no statistically significant differences between these two groups in other index.Conclusions Compared to mid-aged patients, young patients with IMN have better blood pressure and blood glucose level, higher glomerular filtration rate and LDL.Moreover, thyroid function is significantly affected, meanwhile the lesions of glomerular, interstitial and vascular are mild in young patients.

Abstract:Background: Infection is the most common complication of Idiopathic Nephrotic Syndrome (INS) and the main cause of INS recurrence, severe infection and even leading to mortality. The purpose of this study was to investigate the risk factors of severe infection in INS children and the clinical parameters influencing prognosis. Methods: Totally 147 children with INS and concomitant infections were enrolled and classified into the severe infection group (SIG) and Non-severe infection group (Non-SIG). The clinical characteristics and auxiliary examination results were compared between the two groups, and the early-warning parameters for severe infection and risk factors for poor prognosis were evaluated. Results: There were 49 patients in the SIG, 98 patients in the Non-SIG. In the SIG, the most common severe infections disease included severe pneumonia (63.6%), severe sepsis (30.6%), septic shock (4.1%). In SIG, Gram-positive bacteria (GPB) were more common, as was respiratory syncytial virus (RSV), and the three most common strains were Pseudomonas aeruginosa, Staphylococcus aureus (SA) and Staphylococcus epidermidis. There were more steroid-resistant nephrotic syndrome and combination of steroids and immunosuppressants in SIG, compared with the Non-SIG ( P = 0.000). Patients in the SIG has lower complement 3 (C3, ≤ 0.55 g/L,) and absolute lymphocyte count (ALC, ≤ 1.5 × 10 9 /L) ( P = 0.004). Logistic regression analysis revealed that the independent risk factors for severe infections were the combined use of immunosuppressants [95% confidence interval (CI):1.569–463.541, P = 0.023], steroid resistance (95% CI: 4.845–2,071.880, P = 0.003), C-reactive protein (CRP) ≥8 mg/L (95% CI: 43.581–959, 935.668, P = 0.001), and infections caused by GPB (95% CI: 27.126–2,118, 452.938, P = 0.002), influenza (95% CI: 2.494–1, 932.221, P = 0.012) and RSV (95% CI: 5.011–24 963.819, P = 0.007). The patients in the SIG were classified into the survival group ( N = 39) and the mortality group ( N = 5). Logistic regression analysis showed that white blood cell count (WBC) >15 × 10 9 /L (95% CI: 1.046–2.844, P = 0.033) was an independent risk factor of poor prognosis for these patients. Conclusions: Resistance to steroids, combined with steroids and IS agents, and GPB infections (especially SA) are high-risk factors for severe infection in children with INS. We should monitor CRP ≥ 8 mg/L, C3 ≤ 0.55 g/L and ALC ≤ 1.5 × 10 9 /L to avoid developing severe infection. Accompanied by an increase in ANC, WBC significantly increased, suggesting a fatal infection.

Clinical Characteristics and Prognosis Analysis of Children with Idiopathic Nephrotic Syndrome with Different Responses to Steroid Therapy from a Single Center in 20 Years

Abstract:Objective To analyze the clinical characteristics and prognosis of children diagnosed with idiopathic nephrotic syndrome that showed different responses to steroid treatment.Methods In this study,idiopathic nephrotic syndrome was defined as a 24-hour urinary protein of more than 50 mg/kg body weight as well as a serum albumin concentration of less than 25 g/L.A retrospective study was undertaken to analyze the children who were patients at Peking University First Hospital during Jan.1993 and Dec.2012 and conformed to the above criteria.According to their response to steroid therapy,the children were divided into 3 groups:group that demonstrated remission within 4 weeks after therapy,group that demonstrated remission within 4 to 8 weeks and group that demonstrated resistance to therapy after 8 weeks.Among the children who had more than 1 year of follow up,type of nephrotic syndrome(simple vs nephritic),complications,steroid side effects and serious prognosis (chronic renal failure,renal transplantation or death) were further analyzed.Results Out of total 1 059 children with idiopathic nephrotic syndrome,841 cases had a documented steroid response and were analyzed.Among them,71.7％ (603/841 cases) showed remission within 4 weeks following steroid treatment,while 4.0％ (34/841 cases) showed remission within 4 to 8 weeks and 24.3％ (204/841 cases) showed no remission even after 8 weeks.The average remission time after steroid therapy was(13.2 ± 8.9) d with a median of 10.0 d.Of these children,369 cases had at least 1 year of follow up.In this subset,74.8％ (276/369 cases) went into remission within 4 weeks of steroid therapy,4.9％ (18/369 cases) showed remission within 4 to 8 weeks,and 20.3％ (75/369 cases) were resistant to steroid therapy even after 8 weeks.Onset ages were significantly different(P ＜ 0.01) among the 3 groups [remission within 4 weeks:(5.2 ± 3.3) years old; remission within 4 to 8 weeks:(5.7±3.3) years old ; resistant to therapy after 8 weeks:(6.6 ± 3.8) years old].Simple type nephrotic syndrome was more common in children that responded to steroid therapy within 4 weeks (95.3 ％,263/276 cases) and within 4 to 8 weeks (83.3％,15/18 cases) than in children who were resistant to therapy after 8 weeks(45.3％,34/75 cases).The occurrence rates of acute adrenocortical insufficiency showed a significant difference(P =0.04) among the 3 groups(remission within 4 weeks:4.3％,12/276 cases ; remission within 4 to 8 weeks,2/18 cases:11.1％ ; resistant to therapy after 8 weeks:0).The occurrence rates of poor prognosis were also significantly different(P ＜0.0l) among the 3 groups(remission within 4 weeks:1.4％,4/276 cases ; remission within 4 to 8 weeks:0 ; resistant to therapy after 8 weeks:16.0％,12/75 cases).Conclusions Seventy-one point seven percent of children with idiopathic nephrotic syndrome responded to steroid therapy within 4 weeks.Among children that did not respond within 4 weeks following steroid therapy,14.3％ did respond within 8 weeks,while 85.7％ showed no remission 8 weeks after therapy.The children who responded to steroid therapy within 8 weeks mainly presented as a simple type of nephrotic syndrome and had better prognosis,whereas the children that did not respond within 8 weeks mainly presented with nephritic type nephrotic syndrome and had a poorer prognosis.It should be noted that acute adrenocortical insufficiency was more likely to occur in children that responded to steroid therapy within 4 to 8 weeks.
[Clinical and pathological manifestations of Chinese childhood patients with primary IgA nephropathy: a national collaborative study of 33 hospitals]

Abstract:Objective: Primary IgA nephropathy (IgAN) is characterized by a highly variable course ranging from a totally benign condition to rapidly progressive renal failure. About 30% of children with IgAN will eventually have end-stage renal failure after 20 years from onset. A nation-wide survey was conducted and data of hospitalized children (younger than 14 years old) with IgAN during the period of 1995 to 2004 were analyzed. The aim was to investigate the clinical and pathological characteristics, treatment and outcome of the hospitalized children with IgAN. Methods: Questionnaires concerning children with IgAN were designed by the Subspecialty Group of Nephrology, Chinese Society of Pediatrics and distributed to the doctors of 33 hospitals in China. The criterion of IgAN was prominent and diffuse IgA deposition and to a lesser extent, other immunoglobulins in the glomerular mesangium and/or capillary loops, and purpura nephritis, lupus nephritis and hepatic disease were excluded. The data were collected and analyzed. Results: From January 1, 1995 to December 31, 2004, 1349 hospitalized children were diagnosed as IgAN. The cases of childhood IgAN accounted for 1.37% of the hospitalized cases with urologic-kidney diseases and 11.18% of those who underwent renal biopsies. Complete records were available for 1203 patients. The male to female ratio was 2.07:1. Both the median ages at the disease onset and diagnosis were 9.0 years. The median duration from onset to diagnosis of IgAN was 4 months; 55.94% of patients had predisposing causes, especially infection. Recurrent macroscopic hematuria was the most common clinical manifestation (41.15%), followed by nephritic syndrome (23.77%) and hematuria and proteinuria (20.78%). Subclass III (41.40%) and II (28.51%) were the most common histologic types. The main type of immunofluorescence examination was IgA deposition (34.50%). The intensity of IgA deposition in patients with hematuria and proteinuria and in acute rapidly progressive nephritis was the strongest (+++). There was no unified treatment scheme. Some patients were treated with corticosteroids and immunosuppressants, and 69.24% of the patients with IgAN showed clinical improvement, 10.39% remained unchanged, and 2 cases presented deterioration. The rate of follow-up was 23.35%, the mean duration of follow-up was 24.4 months. Conclusion: The mean age of onset of the primary childhood IgAN was after 6 years. Hematuria was the most common clinical manifestation. Subclass III and II were the most common histologic type. There was no unified treatment scheme. The rate of follow-up was lower and the rate of lost follow-up was high. It is necessary to establish a normalized management, treatment and follow-up system for childhood IgAN in China.
Correlation between Toll-like Receptor Gene Polymorphisms and Idiopathic Nephrotic Syndrome in Chinese Children

Abstract:Objective Idiopathic nephrotic syndrome (INS) is the most common glomerular disease in children. Toll-like receptors (TLRs) have been reported to be associated with response to steroid treatment in children with INS. Nevertheless, the correlation between TLR genes and the progression of INS has not yet been clarified. The present study aimed to investigate the association of single-nucleotide polymorphisms (SNPs) in TLR2, TLR4 , and TLR9 with susceptibility to INS as well as the clinical phenotyping of steroid responsiveness in Chinese children with INS. Methods A total of 183 pediatric inpatients with INS were included and given standard steroid therapy. Based on their clinical response to steroids, the patients were classified into three groups: steroid-sensitive nephrotic syndrome (SSNS), steroid-dependent nephrotic syndrome (SDNS), and steroid-resistant nephrotic syndrome (SRNS). A total of 100 healthy children were employed as controls. The blood genome DNA was extracted from each participant. Six SNPs (rs11536889, rs1927914, rs7869402, rs11536891, rs352140, and rs3804099) in TLR2, TLR4 , and TLR9 were selected and detected by multiplex polymerase chain reaction with next-generation sequencing to assess TLR gene polymorphisms. Results Among the 183 patients with INS, 89 (48.6%) had SSNS, 73 (39.9%) had SDNS, and 21 (11.5%) had SRNS. No significant difference was found in the genotype distribution between healthy children and patients with INS. However, the genotype and allele frequencies of TLR4 rs7869402 were significantly different between SRNS and SSNS. Compared with patients with the C allele and CC genotype, patients with the T allele and CT genotype had an increased risk of SRNS. Conclusion TLR4 rs7869402 affected the steroid response in Chinese children with INS. It might be a predictor for the early detection of SRNS in this population.
Effects of flanker type and position on foveal letter recognition

Abstract:Our ability to identify a foveally viewed letter at resolution threshold reduces when flankers are placed at a certain distance from it, compared to when it is presented on its own. In this study, we investigated how type and position of flankers influences foveal letter recognition. We measured participants’ performance to identify unflanked Sheridan-Gardiner letters (A, H, O, U, T, V or X), using a seven-alternative-forced-choice paradigm with the method of constant stimuli to obtain 80-90% correct response. Performance was measured again to identify a target letter in the presence of different flanker types. Flankers were bars and letters (placed either to the left and right of the target, above and below the target or on four sides of the target), and a surrounding box. Separation between the target letter and flankers varied between zero (abutting) and ten stroke widths (two letter sizes). For all flanker types, separation between the target ad the flankers significantly influenced performance (all p < 0.0001). Flankers abutting the target and at separation of one stroke width caused the largest reduction in performance, regardless of flanker type. For bar flankers, the largest drop in performance (up to 50 percent) occurred with bars placed on all four sides of the target. For letter flankers, flankers placed above and below the target reduced performance as much as four surrounding flankers. At separation of one stroke width, flanking letters and bars caused a further 10% reduction in performance than a surrounding box. Our results would have significant implications on the design of crowded visual acuity charts, especially those that are available on the market, which have different crowding features between them.

Abstract:Abstract. Against the background of the increasing incidence of many immune mediated childhood conditions, this study aimed to identify recent time trends and ethnic patterns of childhood nephrotic syndrome. A population-based cohort of children (0–15 years) diagnosed according to strict criteria with nephrotic syndrome (NS) was ascertained within the northern UK region of Yorkshire between 1987 and 1998. South Asian ethnicity was assigned based on the child’s full name using a dedicated computer algorithm and expert individual checks. NS was diagnosed in 194 children, 170 (88%) of whom were steroid sensitive. The incidence of steroid sensitive NS was 2.0/100,000 pyrs (95% CI 1.7–2.3), peaking in 1–4 year olds (4.1/100,000 pyrs). Over the 12-year study period incidence rates of steroid sensitive NS were fairly stable although south Asian children displayed significantly higher rates than non-south Asians (P<0.01). The size of our population-based series reflects the relative rarity of paediatric nephrotic syndrome but is nonetheless recent and includes larger numbers than previous reports. The absence of any increase in incidence over the last decade contrasts with other paediatric immune mediated conditions such as asthma and diabetes.

Abstract:Introduction: Few studies have addressed the genetic spectrum of NPHS1 variants in Chinese children with nephrotic syndrome. In this multicenter study, the clinical manifestations and features of NPHS1 variants in Chinese children with nephrotic syndrome were researched. Method: Genotypical and phenotypical data from 30 children affected by NPHS1 variants were collected from a multicenter registration system in China and analyzed retrospectively. Results: The patients were divided into two groups: congenital nephrotic syndrome (CNS [ n = 24]) and non-CNS (early onset nephrotic syndrome [ n = 6]). Renal biopsy was performed on four patients in the non-CNS group, revealing minimal change disease in three and focal segmental glomerulosclerosis in one. A total of 61 NPHS 1 variants were detected, involving 25 novel variants. The “recurrent variants” included c.928G>A(p.Asp310Asn) in eight patients with CNS, followed by c.616C>A(p.Pro206Thr) in four, and c.2207T>C (p.Val736Ala) in three. Steroid treatment was applied in 29.2% (7/24)of the patients in the CNS group and 50% (3/6) of the patients in the non-CNS group. One patient in each group experienced complete remission but relapsed subsequently. Immunosuppressants were administered to three patients in the non-CNS group, eliciting an effective response. In the CNS group, three patients underwent renal transplantation and six died mainly from infection. Conclusion: Variants of NPHS1 cause CNS and early childhood-onset nephrotic syndrome. NPHS1 variants in Chinese individuals with nephrotic syndrome (NS) were mainly compound heterozygous variants, and c.928G>A(p.Asp310Asn) in exon 8 may act as a recurrent variant in the Chinese population, followed by c.616C>A(p.Pro206Thr) in exon 6. Steroids and immunosuppressants may be effective in selected patients.

Abstract:Objective To study the relationship between clinical pathologic significance and the appraisal of prognoses in school aged children suffering from primary nephrotic syndrome.Methods The clinical and pathologic data of 32 school aged cases suffering from primary nephrotic syndrome during 1995～1999 year in our hospital were retrospectively analyzed.Renal tubulointerstitial changes were divided into IV grades.Results Mesangial proliferative glomerulonephritis is the main pathologic change in all the specimen studied , accounted for 65 %(21/32). No significant difference was found in renal tubulointerstitial changes among different clinical phenotypes. It was suggested that tubulointerstitial changes varied according to the seriousness of glomerular lesions, however that cases were not always the same.Conclusions Mesangial proliferative glomerulonephritis still ranked the top in pathologic patterns of all the specimen studied. In our brief observation, tubulointerstitial changes may act as a major determinant in the progression of renal damage. β 2 microglobulin examination may be a sensitive indicator of tubulointerstitial function.
Spectrum of mutations in Chinese children with steroid-resistant nephrotic syndrome

Abstract:Background The aim of this study was to elucidate whether genetic screening test results of pediatric patients with steroid-resistant nephrotic syndrome (SRNS) vary with ethnicity. Methods Using high-throughput DNA sequencing, 28 nephrotic syndrome-related genes were analyzed in 110 chil-dren affected by SRNS and 10 children with isolated proteinuria enrolled by 5 centers in China (67 boys, 53 girls). Their age at disease onset ranged from 1 day to 208 months (median, 48.8 months). Patients were excluded if their age at onset of disease was over 18 years or if they were diagnosed as having Alport syndrome. Results A genetic etiology was identified in 28.3% of our cohort and the likelihood of establishing a genetic diagnosis decreased as the age at onset of nephrotic syndrome increased. The most common mutated genes were ADCK4 (6.67%), NPHS1 (5.83%), WT1 (5.83%), and NPHS2 (3.33%), and the difference in the frequencies of ADCK4 and NPHS2 mutations between this study and a study on monogenic causes of SRNS in the largest international cohort of 1,783 different families was significant. A case of congenital nephrotic syndrome was attributed to a homozygous missense mutation in ADCK4 , and a de novo missense mutation in TRPC6 was detected in a case of infantile nephrotic syndrome. Conclusions Our results showed that, in the first and the largest multicenter cohort of Chinese pediatric SRNS reported to date, ADCK4 is the most common causative gene, whereas there is a low prevalence of NPHS2 mutations. Our data indicated that the genetic testing results for pediatric SRNS patients vary with different ethnicities, and this information will help to improve management of the disease in clinical practice.

Abstract:Primary nephrotic syndrome (PNS) is the most common glomerular disease seen in children. The pathogenic basis of PNS, in context of witnessed immunological dysfunction and its relationship with slit diaphragm and the podocyte cytoskeleton, has been explored for years. The outcome and management of PNS in children is determined by the response to corticosteroids and the frequency of relapses. The first episode of nephrotic syndrome should be treated adequately, both in terms of dose and duration of steroids, aiming at reducing the risk for subsequent relapses. While patients with steroid-sensitive nephrotic syndrome have a favorable long-term outcome, almost half of them relapse frequently and are at risk of adverse effects of steroids, Immunosuppressive agents are suggested for patients with frequently relapsing nephrotic syndrome or steroid-dependent nephrotic syndrome. Calcineurin inhibitors are strongly recommended for patients who show resistance to steroids. Rituximab is a new therapy that could be a potential area but still requires further trials. Abstract:Objective To explore the pathology and prognosis of children with idiopathic membranous nephropathy(IMN).Methods Clinical manifestations,pathology,treatment and prognosis of 16 children with IMN were reviewed to explore the prognosis of children's idiopathic membranous nephropathy.Results From 1979 to July 2010,consecutive 16 patients diagnosed as IMN were analyzed,accounting for 0.94% of all renal biopsy patients,with the ratio of male:female:10:6,the median age:(5.2±2.6) years.Eleven patients were with nephritic syndrome(68.8%),5 with asymptomatic proteinuria(31.2%),2 children with hypertension(12.5%),2 children with renal insufficiency(12.5%).Fourteen of 16 patients received phathological examination under electronic microscopy,showing that 6 were graded as stage Ⅰ(42.9%),6 as stage Ⅰ-Ⅱ(42.9%),1 as stage Ⅱ(7.1%),1 as stage Ⅱ-Ⅲ(7.1%).No tubular atrophy or interstitial fibrosis were observed.For the patients having proteinuria50 mg·kg-1,no steroids or immunosuppressants were provided for treatment.The patients having proteinuria50 mg·kg-1 were given steroids(2 mg·kg-1·d-1).Immunosuppressants were given to non-responders or reliers to steroids.After(34.0±18.7) months,14 patients were all remitted,and had normal renal function.Conclusions IMN mainly presents as nephritic syndrome.Better prognosis can be expected among patients with younger age,lower pathologic grade,and not combined with interstitial fibrosis.Diagnosis and treatment early can improve the prognosis of children's idiopathic membranous nephropathy.

Abstract:Objective To study the clinicopathological characteristics and prognosis of asymptomatie IgA nephropathy (IgAN) children with pmteinuria and/or microscope haematuria .Methods Clinical and pathological characteristics of 54 children with IgA nephropathy confirmed by renal biopsy were analyzed . These children with IgAN were divided into two groups according to their clinical characteristics at the first onset: asymptomatic IgAN group (AsIgAN) and symptomatic IgAN group (SIgAN) . Histologic changes were classified by Lee SM and Katafuchi semiquantitative scoring system . Results Eighteen children were in AslgAN group and 36 children were in SIgAN group . The degree of proteinuria in SIgAN group [(2 .3±2 .2) g/d] was higher than that in AslgAN group [(0 .4±0 .3) g/d] at the time of biopsy (P＜0 .05) . Although asymptomatic IgAN children were mainly of Lee's type Ⅰ ～Ⅱ, 2 children (11%) were of Lee's type Ⅳ～Ⅴ and 5 cases (27%) presented interstitial injury . Symptomatic IgAN children were mainly of Lee's type Ⅱ ～Ⅲ, there was no significant difference between two groups (P＞0 .05) . Urine micrealbumin was increased in 87% children presented with microscope haematuria . After an average of (26 .9±8 .8)months follow-up, only one case of Lee's Ⅴ progressed into renal failure and the others maintained normal renal function . Conclusions Although children with asymptomatic IgAN have minor clinical symptoms, severe renal pathological lesion and poor prognosis also occur . Urine screening helps to detect renal diseases and renal biopsy should be performed in the patients with increased urine microalbumin .

Abstract:Steroid-resistant nephrotic syndrome (SRNS) is one of the major causes of end-stage kidney disease (ESKD) in children and young adults. For approximately 30% of children with SRNS results from a genetic cause. In this study, genotype-phenotype correlations in a cohort of 283 pediatric patients with SRNS or early-onset NS (nephrotic syndrome presenting within the first year of life) from 23 major pediatric nephrology centers in China were analyzed. All patients were performed with next-generation sequencing and Sanger sequencing. The overall mutation detection rate was 37.5% (106 of 283 patients). WT1 was the most frequently detected mutation, followed by NPHS1, NPHS2, and ADCK4, and these four major causative genes (WT1, NPHS1, NPHS2, and ADCK4) account for 73.6% of patients with monogenic SRNS. Thirteen of 106 individuals (12.3%) carried mutations in ADCK4 that function within the coenzyme Q10 biosynthesis pathway. In the higher frequently ADCK4-related SRNS, two mutations, c.737G>A (p.S246N) and c.748G>C (p.D250H), were the most prevalent. Our study provides not only definitive diagnosis but also facilitate available targeted treatment for SRNS, and prediction of prognosis and renal outcome. Our indications for genetic testing are patients with FSGS, initial SRNS, cases of positive family history or those with extra-renal manifestations.

Gut Microbiota Dysbiosis in Children with Relapsing Idiopathic Nephrotic Syndrome

Abstract:Background: While the etiology of idiopathic nephrotic syndrome (idiopathic nephrotic syndrome [INS]; characterized by repeated relapses and comorbid allergic conditions) remains unknown, recent evidence suggests that dysfunction in regulatory T cells (Tregs) plays an important role in the development of INS as well as allergic diseases. We hypothesized that dysbiosis involving decreased butyric acid-producing gut microbiota leads to defective induction and differentiation of peripherally induced Tregs, resulting in INS relapse. Methods: Study subjects were 12 children with INS, 8 classified as relapsing (R group; median age: 3.0 years) and 4 as non-relapsing (NR group; median age: 4.3 years), and 11 healthy children (HC group; median age: 5.1 years) serving as normal controls. Measurement of microbiota was performed using 16S ribosomal RNA metagenomic analysis, and fecal butyric acid was measured using high performance liquid chromatography. Flow-cytometric analysis of Tregs and CD4-positive (CD4+) cells in peripheral blood was also performed. Results: Metagenomic analysis of gut microbiota using feces showed that the proportion of butyric acid-producing bacteria was significantly lower in R (median 6.36%) than HC (median 18.84%; p = 0.0013), but no different between NR (median 16.71%) and HC (p = 0.29). Fecal organic acid analysis revealed significantly lower butyric acid quantities in R than HC (medians: 0.48 vs. 0.99 mg/g, p = 0.042). Circulating Tregs as a proportion of CD4+ cells were decreased in 75% of R and NR. Conclusion: Pediatric relapsing INS patients show gut microbiota dysbiosis, characterized by a decreased proportion of butyric acid-producing bacteria and lower fecal butyric acid quantities, concomitant with reduced circulatory Tregs.

Autoimmune Podocytopathies: A Novel Sub-Group of Diseases from Childhood Idiopathic Nephrotic Syndrome

Abstract:A recent study reported that autoantibodies targeting Nephrin were detected in the sera of adults and children with minimal change disease.1 Researchers believe that this autoantibody is related to the outcome of the disease, and the research results have provided strong support for the autoimmune etiology of nephrotic syndrome. We think this is significant and interesting research. At present, the etiology and pathogenesis of idiopathic nephrotic syndrome (INS) are still unclear, and podocyte dysfunction/decrease in number is its main pathologic feature. According to the classic view, the pathogenesis of INS is mainly autoimmune imbalance, which is mainly manifested by abnormal T lymphocyte function and the production of some glomerular permissibility factors. Nevertheless, these mechanisms are not enough to explain the pathogenesis of impaired glomerular filtration barrier and the nephrotic range proteinuria. In recent years, evidence of abnormal B lymphocyte function causing INS has been accumulating. The most robust evidence came from the fact that Rituximab and Ofatumumab targeting B cells would be effective in patients with steroid-sensitive nephrotic syndrome or steroid-resistant nephrotic syndrome with minimal change disease or focal segmental glomerular sclerosis in pathology. We recently added evidence for this theory, finding mononuclear cells in peripheral blood of patients with nephrotic syndrome and found apparent B cell disorder in these patients.2 B cells mainly function through the antibodies secreted after they are transformed into plasma cells. Therefore, since 2017, our team began screening and identifying antipodocyte autoantibodies in the serum of children with INS. At first, we detected an autoantibody against podocyte Annexin A2 protein in the serum, and the detection rate was 17.8%.3 Further mouse models proved that it could cause proteinuria in vivo. Mechanism research shows that Annexin A2 antibody can promote the phosphorylation of Annexin A2 Tyr24 and act on the Rho/ROCK signaling pathway to cause the rearrangement of podocyte skeleton, thus causing podocyte damage. After that, we comprehensively studied the podocyte autoantibody spectrum in the serum of patients with INS by two-dimensional electrophoresis and mass spectrometry. Fortunately, we finally screened out 14 target antigens of autoantibodies, including eight target antigens of podocyte autoantibodies specific to children with nephrotic syndrome and six target antigens of highly expressed podocyte autoantibodies in children with nephrotic syndrome.4 Clinical research finally determined that there are seven kinds of podocyte autoantibodies that are most related to the onset of nephrotic syndrome, and approximately 66% of children with nephrotic syndrome have these podocytes autoantibodies.4 Further analysis shows a positive correlation between the urine protein content of children with nephrotic syndrome and the titer of these podocyte autoantibodies in the same period. When urine protein turns negative, podocyte autoantibodies also decrease in patients effectively treated with steroid and/or immunosuppressants. Our study and the opinion of Watts et al.1 both suggest the existence of autoimmune podocytopathies. Based on the above series of findings, we put forward the concept of Autoimmune podocytopathies.4 We do believe it is a novel subgroup of diseases of childhood nephrotic syndrome. These patients show a high level of podocyte autoantibodies in serum. However, unlike idiopathic membranous nephropathy in adults, no apparent immune complex deposits can be observed in the glomerulus. Disclosures A. Abstract:OBJECTIVE:To investigate the clinicopathological feature and treatment of idiopathic membranous nephropathy (IMN) in children. METHOD:A retrospective analysis of 25 cases of biopsy-proven IMN seen between January 2004 and December 2009. RESULT:The incidence of IMN was 3.81% in all the children patients who underwent renal biopsy. Of 25 patients with IMN, nine were boys and sixteen were girls. The mean age at onset was (9.4 ± 3.4) years with a range of 2 - 14 years. Renal biopsies were performed at a median 2.5 months (range 0.4 - 11 months) after onset. The clinical manifestations included nephrotic syndrome (NS) nephritic type in 21 cases (84%) and glomerulonephritis in 4 cases. All patients presented with hematuria, and 7 had macroscopic hematuria. Hypertension was noted in 4 patients. Two patients were complicated with thrombosis. One patient was in a chronic renal insufficiency(CRI)state. According to the MN staging criteria, 21 cases were in stage II IMN (84%). Six patients showed moderate or severe tubulointerstitial lesion. Focal segmental glomerulosclerosis (FSGS) was found in two patients. Of the 22 patients with NS and nephrotic proteinuria, 21 cases were treated with prednisone initially and in 20 of them the efficacy of corticosteroid therapy was evaluated:one of them was steroid sensitive (became steroid-resistant after relapse) and all the others were steroid-resistant (95%). The subsequent treatment: eight of them were treated with prednisone followed by a taper to alternate-day therapy. Five of them had complete remission and three partial remission. Twelve cases were treated with combined therapy of prednisone and immunosuppressive agents. Of these 12 cases together with one case who received initially combined treatment with prednisone and immunosuppressive agent and one case treated with prednisone initially for five weeks then with combined therapy contained another immunosuppressive agent, totally 14 cases, 5 had complete remission, 2 partial remission, 3 did not achieve remission, and 3 had unknown response. CONCLUSION:Of the patient cohort, the predominant presenting feature was nephrotic syndrome, and with different degree hematuria. Almost all of them were steroid resistant, but followed by a taper to alternate-day therapy, some could achieve remission. The effect of a combination of prednisone and immunosuppressive agent is needed to be further proven in children.

Abstract:Introduction. Eighty percent of children with primitive nephrotic syndrome (NS) will have at least one relapse in their life. Specific risk factors could be associated with a higher incidence of relapses and a worse prognosis. This study aims to deepen the demographic and onset-related risk factors in children with known diagnosis of primitive NS attending the Pediatric Nephrology Unit of the University Hospital of Padua. Methods. Observational, descriptive study of all children (1-11 years old) with a known diagnosis of Primitive NS who attended our Pediatric Nephrology Unit between 1 January 2002 and 31 March 2023. Results. 49 patients were involved. 79.5% had at least one episode of NS relapse during their lifetime. 69.4% were classified as frequently relapsing or steroid-dependent NS. The relapse risk factor "non-Western ethnicity" was related to a worse prognosis and steroid-dependent NS classification (p = 0.041). The onset-related risk factor "thrombocytosis" appears to be related to a better prognosis (p = 0.03). Conclusion. The relapse risk factors "non-Western ethnicity" and "thrombocytosis" are characterized by worse and better prognosis, respectively. This evidence could support the follow-up of primitive NS in pediatric age.
