Abstract:A 61-year-old man who underwent total gastrectomy and splenectomy for gastric cancer was placed a Port-A-Cath through the anterior abdominal wall just lateral to the rectus muscle in the right upper quadrant for intraperitoneal chemotherapy 14 years ago. He recovered well after the surgery and chemotherapy. The Port-A-Cath was left in situ for 14 years. The patient’s course was uncomplicated until approximately one year ago, when he presented with recurrent redness, swelling and pain around the subcutaneous port. The symptoms were alleviated after antibiotic treatment and the Port-A-Cath was removed in the local hospital. Three days later, rigor and fever with recordings of a 40 degrees Celsius body temperature occurred. The patient was thus admitted to the local hospital. Two days later, he developed abdominal distension and a cankerous sore at the site of the operative incision. The wound healed after local application of gentamycin and frequent wound dressing changes, yet 7 days later, the patient experienced the same symptoms of rigor and fever with a temperature of 40 degrees Celsius. After the administration of several antibiotics and glucocorticoids, his body temperature hovered between 37.5 and 38.0 degrees Celsius prompting admittance to our university hospital. Laboratory tests revealed leukocytosis (11.0×10/L), with a neutrophilic percentage of 78.7% and an elevated C-reactive protein (CRP) of 69 mg/L. The thoracic computed tomography (CT) showed slight bilateral pleural effusion, pericardial effusion and mild mediasternal pneumatosis while ultrasound tests confirmed the pleural effusion and revealed a small quantity of ascites. With time, antibiotics ceased to have an effect on the patient’s symptoms and he gradually developed dyspnea, palpitation and orthopnea with a body temperature of 39 degrees Celsius. Abstract:A vessel port, implanted into the central venous system, is used for long-term intravenous drug administration in oncology patients. Although essential for frequent chemotherapy and other treatments, ports can lead to complications such as infection and thrombosis. This article discusses a rare but serious complication: the displacement of a catheter fragment. A 67-year-old gastric cancer patient, experienced malignant recurrence with jaundice and bile duct infiltration post Roux-Y subtotal gastrectomy and D2 lymphadenectomy. After nine cycles of chemotherapy, a catheter fragment from the venous port detached and lodged in a branch of the pulmonary artery in segment VIII of the right lung. Thoracotomy was performed to remove the foreign body. Our aim is to report on the surgical treatment of a displaced detached catheter and to raise awareness about the potential rare complications associated with the use of vascular ports in patients undergoing chronic oncological treatment. Additionally, we screened the PubMed database for similar surgical treatment reports and compared the collected data. Venous port malfunction or non-specific patient symptoms may indicate rare complications, such as port component detachment, necessitating a multidisciplinary approach for prompt diagnosis and management in oncological patients.
Pseudoaneurysms formation with hemoptysis after thoracic aortic dissection stent implantation: a case report

Abstract:Abstract Background: Aortic dissection is a surgical emergency. It refers to a pathological change in which the blood in the aortic lumen enters the media of the aorta from the tear of the intima of the aorta, which separates the media and expands along the long axis of the aorta. This results in a pathological change in the separation of the true and false lumens of the aorta. Blood pressure and aortic middle layer disease are the main causes of aortic dissection. Thoracic endovascular aortic repair (TEVAR) can successfully treat this disease. However, pseudoaneurysm formation is a common and important complication after TEVAR, and its rupture is life-threatening. Pseudoaneurysm formation can be found in patients after TEVAR, which requires reoperation.Case presentation: A 43-year-old Chinese man was hospitalized with "sudden hemoptysis with chest and back pain and fever for 4 hours. Examination in the emergency room revealed a thoracic aortic pseudoaneurysm with hematoma formation after aortic dissection stenting, which compressed the left lower lung. The patient's symptoms disappeared after a second thoracic aortogram stent implantation.Conclusions: Patients with acute aortic dissection should be treated with caution. Especially for patients with aortic pseudoaneurysm after aortic dissection stent implantation, the local arterial wall is relatively weak after stent implantation, and continuous bulging may form pseudoaneurysm. Continuous progression of pseudoaneurysm and hematomas compression of lung tissue leading to hemoptysis are relatively rare in clinical practice. If not handled in time, large-scale lung tissue necrosis and para-stent infection or asphyxia are likely to occur. In case of symptomatic pseudoaneurysm formation, aortic angiogram stent implantation should be performed in time.

Abstract:BACKGROUND:Central venous catheter placement is an important aspect of patient care for the administration of fluids and medications and for monitoring purposes. However, it is still associated with significant morbidity and mortality.CASE PRESENTATION:We report a case of iatrogenic inferior thyroid artery pseudoaneurysm during the central line placement due to internal jugular vein puncture. This is a rare complication of central venous cannulation. Fortunately the pseudoaneurysm was monitored closely, diagnosed promptly and obliterated by using radiological intervention. We discuss the risk factors and management of the unintended artery puncture.CONCLUSION:The pathway of the management post arterial puncture depends on the size of the needle or catheter, which is direct related to the consequence of arterial injuries. Identifying risk factors is very important to avoid the complications. However, the use of ultrasound guided venipuncture is the most important method to avoid mechanical complications.
Massive Hemothorax Caused by Intercostal Artery Pseudoaneurysm:A Case Report

Abstract:Intercostal artery pseudoaneurysm is rare and at the risk of rupture. The aetiology is always reported to be iatrogenic and traumatic injury. Embolisation is the most common therapeutic method. Here, we report a case of spontaneous intercostal artery pseudoaneurysm and cured by combining covered stent grafting and surgical management. A 60-year-old man complained of acute right back pain for 5 h. Computed tomography showed right massive hemothorax and a giant mass with distinct feeding vessel originated from the thoracic aorta within the right hemithorax. Thoracocentesis was performed, and then a covered stent was positioned across the origin of the feeding vessel. The patient was diagnosed with intercostal artery pseudoaneurysm. Finally, we successfully resected the pseudoaneurysm and ligated the proximal part of the artery. Histologic examination have proved the diagnosis. The postoperative course was uneventful, and the patient was discharged on postoperative day 10. There is no recurrence reported during follow-up. Spontaneous intercostal artery pseudoaneurysm is extremly rare. Delayed hemothorax due to rupture of the pseudoaneurysm may occur years after the formation. Early diagnosis is important and a combined treatment of endovascular intervention and surgical management is feasible, especially for the case of ruptured large tumour-like mass presentation of the pseudoaneurysm.
Surgical Treatment of Post-Traumatic Pseudoaneurysms and Arteriovenous Fistulas

Abstract:OBJECTIVE: To report the experience in the diagn osis and treatment of post-traumatic pseudoaneurysms and arteriovenous fistulas. METHODS: A series of 30 patients (11 women and 19 men) with pos ttraumatic pseudoaneurysms were reviewed retrospectively. Among them 7 patients (5 women and 2 men) were associated with arteriovenous fistula. RESULTS: The causes included sharp penetrating trauma (18 cases ), blunt trauma (6 cases) and iatrogenic arterial injury (6 cases). The main cli nical manifestations consisted of local pulsatile mass (26 cases), vascular brui ts (19 cases), thrill (13 cases), ischemia of distal limb (9 cases), neuropathy (5 cases) and pseudoaneurysm rupture (2 cases). All patients underwent surgery. The operations included: ligation of the vessels (12 cases), surgical resection and primary suture repair of the vascular defect or anastomosis (11 cases), vasc ular reconstruction with autogenous saphenous vein (3 cases) and synthetic vascu lar graft (4 cases). CONCLUSIONS: Because of the imminent clinical course, early op eration is usually indicated. The operative treatment is effective and safe for most of the patients with post-traumatic pseudoaneurysms and arteriovenous fist ulas.
Pseudoaneurysm of the Aortic Arch A Rare Case Report of Pulmonary Cancer Complication

Abstract:Background: Pseudoaneurysm of the aortic arch is uncommonly associated with cancer, and is extremely rare in pulmonary cancer. Here, we report an unusual and successfully treated case of aortic arch pseudoaneurysm in a male patient with lung squamous cell carcinoma.Methods: A 64-year-old male patient was admitted to the Emergency Department, presenting with massive hemoptysis (>500mL blood during the 12 hours prior to treatment). The diagnosis of aortic arch pseudoaneurysm was confirmed after inspection of computed tomographic angiography and three-dimensional reconstruction. We processed the immediate endovascular stent-grafting for this patient.Results: This patient recovered with no filling or enlargement of the pseudoaneurysm, no episodes of hemoptysis, and no neurological complications during the 4-week follow-up period.Conclusion: Herein, we compare our case with other cancer-related pseudoaneurysms in the medical literature and summarize the clinical features and treatment of this unusual case.
Partial anomalous pulmonary venous connection associated with hemoptysis: A case report.

Abstract:Rationale: Partial anomalous pulmonary venous connection (PAPVC) is a rare congenital anomaly characterized by the failure of fusion of embryologic pulmonary venous system with left atrium. Patient Concerns: A 45-year-old male patient with PAPVC who was hospitalized because of mild hemoptysis. Images showed the anomalous vein originated from the left upper pulmonary vein and flowed into the left brachiocephalic vein. No other underlying causes for hemoptysis were detected. Diagnosis: After multi-disciplinary discussion, the patient was diagnosed as PAPVC of left upper pulmonary vein draining into the left brachiocephalic vein with intact atrial septum. Interventions: Although surgical correction of PAPVC was feasible, left upper lobectomy was performed as the definitive treatment for both hemoptysis and PAPVC. Outcomes: The patient had an uneventful postoperative hospital course and was followed up for nearly 2 years without recurrence of hemoptysis. Lessons: PAPVC is associated with atrial septal defect in 80% to 90% of cases while isolated PAPVC with intact atrial septum is an extremely rare entity. We present a rare isolated PAPVC patient with hemoptysis. To our best knowledge, PAPVC associated with hemoptysis has never been reported before.

Abstract:A coronary pseudoaneurysm is a rare complication of chest trauma. In this report, we describe the case of a 65-year-old man with a mediastinal lesion. On admission, he complained of chest tightness and dry cough, and a pseudoaneurysm was confirmed in the left anterior descending branch of the coronary artery on chest computerized tomography, angiography, and coronary angiography. The patient had experienced chest trauma 5 years previously. With the help of extracorporeal bypass surgery, the pseudoaneurysm was resected under direct observation. The patient recovered well after surgery. Traumatic coronary artery pseudoaneurysms usually are asymptomatic and often misdiagnosed. Preoperative coronary angiography is a crucial diagnostic used for deciding appropriate surgical management.
Inferior Epigastric Artery Pseudoaneurysm Following Laparoscopic Appendectomy

Abstract:An inferior epigastric artery pseudoaneurysm is an exceptionally rare occurrence. Formation of an inferior epigastric artery pseudoaneurysm can be seen following surgical intervention and is more common after laparoscopy. A sixty-eight-year-old male presented with a right upper quadrant bulge at his incision site two months following laparoscopic appendectomy. The patient reported sudden onset of a non-reducible bulge at a 5 mm trocar incision site with minimal pain and without obstructive symptoms. Computed tomography of his abdomen and pelvis with intravenous contrast revealed a 4.2 cm pseudoaneurysm with peripheral thrombosis within the right inferior epigastric artery. The patient subsequently underwent open exploration with the evacuation of pseudoaneurysm thrombus and ligation of arteriovenous fistula. The patient recovered well without complication from pseudoaneurysm. Inferior epigastric artery pseudoaneurysm following any laparoscopic procedure is rare. This case highlights the importance of understanding the abdominal wall anatomy and its vascular supply to avoid such injury. We present this case to bring light to this rare occurrence and to highlight the importance of proper trocar placement during any laparoscopic procedure.

Abstract:Gastrointestinal bleeding due to aortoenteric fistula is extremely rare. Aortoenteric fistula is difficult to be diagnosed timely and entails a significant morbidity and mortality. Herein, we present an uncommon case of gastrointestinal bleeding caused by aortoduodenal fistula, which was a complication of a successful aortic reconstruction 4 months ago for an aortic pseudoaneurysm resulted from a stab wound 12 years ago. An urgent laparotomy confirmed an aortoduodenal fistula and repaired the defects in aorta and duodenum, but a prolonged shock led to the patient’s death. In summary, early diagnosis and surgical intervention for aortoenteric fistula are vital for survival.

Abstract:Post-traumatic ventricular pseudoaneurysms are a rare complication of chest trauma that necessitate surgical correction. In this case report, we describe a 22-year-old male patient presenting with a left ventricular pseudoaneurysm 45 days following primary surgical repair of a penetrating left ventricular injury with a background of stabbing chest trauma. The pseudoaneurysm was successfully surgically treated at our hospital after a thorough evaluation despite the vague clinical presentation at the time of referral. The patient fully recovered afterward and his case enriched our understanding of this life-threatening yet rare complication.
Late development of aortic pseudoaneurysm after coarctation repair with fistulization to the bronchial tree. A case report

Abstract:Background: Fistulous communication between the aorta and the tracheobronchial tree is an uncommon and serious cause of hemoptysis secondary to complications of a previous operation performed on the aorta. In cases in which an appropriate surgical intervention is carried out, the survival rate approaches 76%. This surgery is considered one of the most risky operations on the aorta, challenging the surgeon's ability to resolve the problem. Methods: We present the case report of a 43-year-old female with massive hemoptysis. Her medical history disclosed repair of coarctation of the aorta (15 years before). She underwent emergency left thoracotomy; surgical exploration revealed a false aneurysm from the previous aortic patch repair which communicated to a subsegmental bronchus of the left upper lobe. Results: The thoracic aorta was isolated and clamped, and the previous patch was removed. The bronchial side of the fistula was managed with left superior lobectomy and the aorta was repaired with the placement of a coated woven dacron graft onto healthy aortic tissue. Conclusions: The patient had an uneventful recovery and remains asymptomatic six months after discharge.
A case report of an aorto-pulmonary-venacaval fistula after penetrating cardiac injury

Abstract:Abstract Background Penetrating cardiac injury (PCI) is an accepted burden in high violent crime areas. Traumatic intracardiac fistulae are however not that commonly detected on initial presentation, with most of these injuries being detected post-operatively or at routine follow-up. The literature is devoid of general principles around the pre-operative planning and intra-operative management in these cases, and thus warrant documented case reports by clinical units experienced in the management of these challenging clinical scenarios. Case summary We describe a case report of a 29-year-old male patient presenting to our Cardiothoracic Unit with an aorto-pulmonary-venacaval fistula after a traumatic PCI. We describe the clinical presentation, diagnostic challenges, and institutional experience in the operative management of this case. Discussion The patient was treated successfully with repair via sternotomy and femoral cardiopulmonary bypass with no neurological, pulmonary, or cardiac sequelae at early follow-up. The importance of selective pre-operative imaging in PCI, appropriate pre-operative surgical planning, and surgical experience in the management of these injuries is highlighted in this case presentation.

Pseudoaneurysm Leading to Necrotizing Fasciitis at the Femoral Arterial Puncture Site

Abstract:In this article, we present two cases of femoral pseudoaneurysm (PA) at the femoral arterial puncture site followed by necrotizing fasciitis, which is rare but can be fatal when not managed appropriately. PA was revealed by lower-extremity angiography and color-flow Doppler ultrasonography. Hematoma removal, thrombolysis, and bleeder ligation with Gelfoam were repeatedly performed by a vascular surgeon. When necrotizing fasciitis developed, aggressive surgical drainage and creation of a viable wound bed for reconstruction were mandatory. We adopted a vacuum-assisted closure device (Kinetics Concepts International) as the standard treatment for complicated, serious, infected PA of the puncture site. Excellent clinical outcomes were obtained.

Abstract:A 41-year-old man presented with a 2-month history of dyspnea, chest pain, progressive facial and upper arm edema, and head fullness. His medical history was significant for poorly controlled Behçet’s syndrome. He underwent a Bentall procedure with a mechanical valved conduit at our center 7 years ago because of dilated aortic root and ascending aorta, severe aortic regurgitation, and bicuspid aortic valve. However, this patient was lost to follow-up after discharge. Physical examination revealed facial edema and plethora, bilateral upper arm swelling, and jugular venous distention. A painless and throbbing mass was palpated at the right sternal border. Computed tomography angiography confirmed a pseudoaneurysm located in the distal ascending aorta (Fig 1, white arrow: pseudoaneurysm). The superior vena cava was severely compressed by the pseudoaneurysm (Figs 2 and 3, white arrow: pseudoaneurysm; yellow arrow: superior vena cava). We performed an emergency reoperative aortic pseudoaneurysm resection and repair. Abstract:Arterioportal fistula (APF) combined with a visceral artery pseudoaneurysm is an exceptionally rare and critical vascular disorder of the abdominal viscera, with pseudoaneurysm rupture being potentially fatal and severe APF leading to portal hypertension, both of which necessitate immediate intervention. An 87-year-old woman with a history of pancreatitis presented with upper abdomen and back pain. Laboratory tests revealed elevated amylase levels and severe anemia. A computed tomography (CT) scan showed a large dorsal pancreatic artery (DPA) pseudoaneurysm with a fistula to the main portal vein. Given her advanced age, surgery was deemed high-risk, and endovascular treatment was selected. Transcatheter arterial embolization was successfully performed using coils to embolize the DPA pseudoaneurysm. A follow-up CT 1 week postprocedure confirmed the absence of a pseudoaneurysm and no further progression of anemia.
Iatrogenic popliteal artery pseudoaneurysm: A case report

Abstract:Background: Popliteal artery pseudoaneurysm is a state of vascular wall rupture in the popliteal artery. It is generally a rare situation and the most common etiologic factor is iatrogenic causes. Case presentation: This case report presents a successful diagnosis and management of a 31-year-old patient who was presented with a mass behind the knee, three months after femoropopliteal bypass for the treatment of a gunshot to the knee. An endovascular approach using coils was utilized for the patient, which led to complete remission for the patient. Discussion: The current patient had two primary therapeutic challenges: bleeding and hematoma growth, clot development causing blood flow restriction, and limb ischemia. Pseudoaneurysm also caused internal inflammation, which increased the risk of thrombosis and bypass graft damage during open surgery. Due to the risk of recurrence and graft damage, ultrasound-guided compression was not possible. Thus, endovascular therapy was preferred. Conclusion: The endovascular approach using coils is an option for the management of PAP. However, the endovascular approach should be considered carefully according to the patient's status.

Abstract:Bleeding and infection may complicate LVAD implantation. We present the case of a patient with recurrent fevers after LVAD implantation who was found to have an infected pseudoaneurysm of the outflow graft at the graft-aortic anastomosis, with successful surgical intervention. Patients with infected pseudoaneurysms post LVAD insertion require urgent surgical intervention that may differ from that for regular pseudoaneurysms.
