Abstract:Objective: to define an index that summarizes the main dimensions of the socioeconomic context for research purposes, evaluation and monitoring health inequalities. Methods: the index was created from the 2010 Brazilian Demographic Census, whose variables selection was guided by theoretical references for health studies, including seven socioeconomic dimensions: education, mobility, poverty, wealth, income, segregation and deprivation of resources and services. The index was developed using principal component analysis, and was evaluated for its construct, content and applicability components. Results: GeoSES-BR dimensions showed good association with HDI-M (above 0.85). The model with the poverty dimension best explained the relative risk of avoidable cause mortality in Brazil. In the intraurban scale, the model with GeoSES-IM was the one that best explained the relative risk of mortality from circulatory system diseases. Conclusion: GeoSES showed significant explanatory potential in the studied scales.

Abstract:Brazil has the world's fifth-largest population and seventh-largest economy. However, it also has many inequalities, especially in health education, which impacts health sector services. Thus, this article aims to describe the situation of planetary health and health education in Brazil, identifying how current policies support the cause of planetary health. This study had a qualitative approach characterised as exploratory research based on an integrative review and documentary research. The results show that, in recent decades, there have been positive improvements to achieve collective and planetary health, which advocates empathy and pro-environmental and humanitarian attitudes. However, the pursuit of planetary health in Brazil is being influenced by various challenges, ranging from the need for a sound policy framework to provisions of education and training on planetary health. Based on the need to address these deficiencies, the paper suggests some measures which should be considered as part of efforts to realise the potential of planetary health in the fifth largest country in the world.
Political struggles for a universal health system in Brazil: successes and limits in the reduction of inequalities

Abstract:Background: Brazil is a populous high/middle-income country, characterized by deep economic and social inequalities. Like most other Latin American nations, Brazil constructed a health system that included, on the one hand, public health programs and, on the other, social insurance healthcare for those working in the formal sector. This study analyzes the political struggles surrounding the implementation of a universal health system from the mid-1980s to the present, and their effects on selected health indicators, focusing on the relevant international and national contexts, political agendas, government orientations and actors. Main text: In the 1980s, against the backdrop of economic crisis and democratization, Brazil's health reform movement proposed a Unified Health System (SUS), which was incorporated into the 1988 Constitution. The combination of a democratic system with opportunities for interaction between various developmental and social agendas and actors has played a key role in shaping health policy since then. However, the expansion of public services has been hampered by insufficient public funding and by the strengthening of the private sector, subsidized by the state. Private enterprises have expanded their markets and political influence, in a process that has accelerated in recent years. Despite these obstacles, SUS has produced significant health-status improvements and some (although incomplete) reductions in Brazil's vast health inequalities. Conclusions: We find that a combination of long-term structural and contingent factors, international agendas and interests, as well as domestic political struggles, explains the advances and obstacles to building a universal system in an economically important yet unequal peripheral country. Further consolidation of SUS and reduction of health inequalities hinge on the uncertain prospects for democracy and national development, on enlarging the political coalition to support a public and universal health system, and on strengthening the state's ability to regulate the private sector.

Abstract:Abstract How does interpersonal trust relate to trust in public institutions, people’s willingness to contribute to redistributive actions, and the strong bonds of social cohesion in society? This article presents how Brazilians’ trust in others and in various state institutions has varied since the 1990s and how the interaction between interpersonal and institutional trust affects social cohesion. Also, we examine how widespread social perceptions of institutional corruption and different levels of inequality affect the levels of interpersonal social. Using the analytical concept of social responsibility, we argue strategies centered on the design and performance of public institutions contribute to breaking the vicious circle of lack of trust and its social costs. We used descriptive statistics and regression analysis based on data extracted from the Latinobarómetro World Values Survey as well as interview data with different social classes in Brazil.
Brazil's unified health system: the first 30 years and prospects for the future

Abstract:In 1988, the Brazilian Constitution defined health as a universal right and a state responsibility. Progress towards universal health coverage in Brazil has been achieved through a unified health system (Sistema Único de Saúde [SUS]), created in 1990. With successes and setbacks in the implementation of health programmes and the organisation of its health system, Brazil has achieved nearly universal access to health-care services for the population. The trajectory of the development and expansion of the SUS offers valuable lessons on how to scale universal health coverage in a highly unequal country with relatively low resources allocated to health-care services by the government compared with that in middle-income and high-income countries. Analysis of the past 30 years since the inception of the SUS shows that innovations extend beyond the development of new models of care and highlights the importance of establishing political, legal, organisational, and management-related structures, with clearly defined roles for both the federal and local governments in the governance, planning, financing, and provision of health-care services. The expansion of the SUS has allowed Brazil to rapidly address the changing health needs of the population, with dramatic upscaling of health service coverage in just three decades. However, despite its successes, analysis of future scenarios suggests the urgent need to address lingering geographical inequalities, insufficient funding, and suboptimal private sector-public sector collaboration. Fiscal policies implemented in 2016 ushered in austerity measures that, alongside the new environmental, educational, and health policies of the Brazilian government, could reverse the hard-earned achievements of the SUS and threaten its sustainability and ability to fulfil its constitutional mandate of providing health care for all.

Abstract:Background: Health care is a complex economic and social system, which combines market elements and public and social interest. This combination in Brazil, like systems in China and United States of America, is operationalized through the public and private system. The sector represents approximately 9% of the country's GDP, of which 56% is privately sourced and 44% is of public origin. In the private sector includes a structure with 711 private health institutions, 47 million beneficiaries and revenues of US$30 billion a year. Methods: Therefore, this research describes and analyzes the complementarity of Private Health before the Brazilian Unified Health System, highlighting its main characteristics, scenarios, and trends in the face of the health system and the Brazilian market. This descriptive and exploratory research uses secondary data from various sources, submitted to quantitative data analysis methods. The object of the research is the history of private health in Brazil and its main actors. Results: The data are organized into three groups, each with its approach of collection and analysis. Thus, it is perceived as the notorious growth of large operators, to the detriment of operators with a lower concentration of beneficiaries; the increasing concentration of the market through mergers and acquisitions promoted by large publicly traded corporations, especially in regions with a lower rate of private health coverage; and the growth of the sector through business plans, whose central characteristic is the dependence on the country's employability rate. Conclusions: It is possible to perceive an intense trend of concentration of Brazilian private health in large institutions that have capitalized and have a great appetite for growth through mergers and acquisitions, whether from smaller operators or health institutions that integrate their health networks, following complementary health models already consolidated in countries such as China, and the United States of America, among others. This concentration projects a market with fewer options and competitiveness, reduction in transaction costs and increase the operational effectiveness of health care.

Abstract:Introduction: The issue of violence in Brazil is characterized by high rates of homicide and sexual violence, particularly targeting black people, young individuals, and women. While technology has been employed to prevent crimes, the absence of reliable data complicates the formulation of effective public policies to address these forms of violence. Objectives: To analyze the status of the websites belonging to Brazil's state public security departments, with a specific focus on the accessibility of crime data and statistics. Methodology: In the first stage of the research, we delineated the aspects to be examined within the portals of public security secretariats, focusing on the availability of criminal data, communication of the secretariats' actions, and domestic violence, including violence against women, children, or adolescents. In the second stage, we visited the 27 portals to collect data. Results: The analysis indicates that 22.2% of secretariats make data available for deeper analyses. Moreover, 77.8% of secretariats present statistical reports, although these reports lack format, data, metadata, or layout standardization. On the other hand, it shows that 96.3% of the sites analyzed have news systems to disseminate security information and that 74.1% have at least one profile on social media, indicating concern about keeping the population informed about security-related issues. Conclusion: The issue of public security data in Brazil still needs to evolve significantly. While over 77% of state secretariats practice disseminating statistical reports, only six of them publicly provide data that can be analyzed more in-depth, yielding more meaningful results.

Abstract:This study is part of an extensive research project entitled “The Brazilian’s Leisure”. The aim of this paper is to identify in a broad manner how Brazilians’ use their leisure time based on their education, income and social class. The work was accomplished through empirical research and, whilst using broad reasoning procedures, comparisons were drawn between the representation of leisure and its actual activities. This paper specifically discusses the variables relating to education, income and social classes present from the data collected. It was identified that such variables have a distinct influence on the manner in which Brazilians spend their leisure time, and that it contributes to the rise of inequality when it comes to leisure.
Multicultural Brazil in the BRICS Countries: Potentials for the Social Sciences and Humanities

Abstract:This study argues a case for multiculturalism as a possible approach for addressing the complexities of societies such as the BRICS, taking Brazil as a case study. It contends that knowledge derived from such a study can benefit Social Studies and Humanities worldwide, particularly considering that cultural diversity has been increasingly present due to globalisation, internationalisation and growing mobility of groups of people, including refugees all over the world, requiring new epistemologies and narratives in research. The paper firstly analyses the concept of multiculturalism, discussing its perspectives from more liberal approaches up to more critical, postcolonial and decolonial perspectives that focus on the challenge of prejudices that operate in terms of control and denial of pluralism. It discusses the multicultural composition of the population of Brazil as a BRICS country, analysing its main geographical and population characteristics. The main focus of the paper is to delve in the ways that Brazilian educational policies have been trying to address both international standards and the valuing of cultural identities and equity-oriented approaches that are inclusive and multicultural. It particularly focuses on the National Plan for Education and of the recent project for higher education institutions (called “Future-se”). Finally, it concludes by pointing out tensions and possibilities of such Brazilian endeavours in the context of the BRICS countries. Such a study may be relevant comparatively, hopefully providing reflections for new epistemologies and the potential value of these for the Social Sciences and Humanities.

A public micro pension programme in Brazil: Heterogeneity among states and setting up of benefit age adjustment

Abstract:Brazil is the 5th largest country in the world, despite of having a ``High Human Development'' it is the 9th most unequal country. The existing Brazilian micro pension programme is one of the safety nets for poor people. To become eligible for this benefit, each person must have an income that is less than a quarter of the Brazilian minimum monthly wage and be either over 65 or considered disabled. That minimum income corresponds to approximately $2$ dollars per day. This paper analyses quantitatively some aspects of this programme in the Public Pension System of Brazil. We look for the impact of some particular economic variables on the number of people receiving the benefit, and seek if that impact significantly differs among the 27 Brazilian Federal Units. We search for heterogeneity. We perform regression and spatial cluster analysis for detection of geographical grouping. We use a database that includes the entire population that receives the benefit. Afterwards, we calculate the amount that the system spends with the beneficiaries, estimate values \textit{per capita} and the weight of each UF, searching for heterogeneity reflected on the amount spent \textit{per capita}. In this latter calculation we use a more comprehensive database, by individual, that includes all people that started receiving a benefit under the programme in the period from 2nd of January 2018 to 6th of April 2018. We compute the expected discounted benefit and confirm a high heterogeneity among UF's as well as gender. We propose achieving a more equitable system by introducing `age adjusting factors' to change the benefit age.

Abstract:In addressing the complexity inherent in comparing socioeconomic indicators across diverse countries, a substantial barrier arises from the wide array of distinct calculation approaches. Consequently, a highly pertinent question emerges: how does one compare values when the underlying calculation methodologies are diverse? In this context, we propose a multidisciplinary and heterogeneous approach, employing numerical and visual comparisons to analyze socioeconomic indicators of municipalities in the US, Brazil and other Latin American countries. We identify independent-scale patterns in three stages: initially, by compiling a database sourced from respected institutions in each country and identifying correlations within this dataset; next, by creating graphical representations using a binning methodology for scatter plots of the identified patterns; finally, by transforming the dispersions from the initial stage into graphs using a variant of the Gravitational Clustering Algorithm technique from astrophysics. We find significant relationships, specifically in the logarithmic relationship between Municipal HDI and population size. In some countries, global minima were identified, suggesting the existence of a critical population density at which municipalities above this threshold exhibit a positive and significant correlation with an increase in their HDI.

Abstract:The health sector is one of the major fields of economic, social, scientific, and technological development in Brazil, and has been the arena of dispute between capitalist interests that regard health as a commodity and advocates of universal access who regard health as an essential good. The Brazilian Federal Constitution of 1988 determined that health is a fundamental right and the State's responsibility, and since then the country has made progress with public policy, but has suffered setbacks and blockages due to its status as a peripheric country, historically subordinated to the interests of core countries, which see Brazil as a broad consumer market. These external interests associated with the internal bourgeoisie have been taking advantage of the Brazilian State since the 1960s, when the foundation of business groups expanded, dominating various health segments, especially since the neoliberal policies of the 1990s. These aspects are much explored in publications in Public Health, but this text seeks a new approach, using the Marxist Theory of Dependency as a reference to analyze, albeit in a preliminary way, the situation of political, economic, and technological dependence that has distanced health policy from the ideals of a public and universal system, defended in the Brazilian Health Reform.

Abstract:This is a documentary, exploratory, descriptive study, which is part of a multicenter international study assessing the national health systems with a care model based on primary health care of Brazil, Spain, Italy, and Portugal, funded by the Brazilian National Research Council (CNPq, acronym in Portuguese). It aims to identify the basic health legislation, the right to health, and the doctrinal and organizational principles of each country with a focus on the impact of social determinants of health on the national health systems. The results showed these countries have similar legislation and doctrinal principles, with a constitutional right to health, based on primary health care, and with a care model of the family health type. The challenges identified were low birth rate and high life expectancy at birth in European countries and criteria for access to medication and care financing. Based on our findings, the countries with higher investment in a structural basis, ensuring more dignified, solid, and vigilant socioeconomic and sanitary conditions, provide an important differentiation in responsiveness and sustainability of the national health system and direct impact on the quality of life.

Abstract:This study examines Brazil's evolution from a primarily agricultural economy to a pivotal player within global supply chains, contesting the traditional view of emerging markets as mere suppliers of raw materials and labor. It closely examines Brazil's assertive efforts and the obstacles it encounters, focusing on its role within the context of Latin America. Utilizing macroeconomic data, the research reveals the barriers to Brazil's deeper integration into global supply chains and outlines novel strategies to enhance its global position. Export complexity, industrial complexity and regional economic growth in Brazil

Abstract:Research on productive structures has shown that economic complexity conditions economic growth. However, little is known about which type of complexity, e.g., export or industrial complexity, matters more for regional economic growth in a large emerging country like Brazil. Brazil exports natural resources and agricultural goods, but a large share of the employment derives from services, non-tradables, and within-country manufacturing trade. Here, we use a large dataset on Brazil's formal labor market, including approximately 100 million workers and 581 industries, to reveal the patterns of export complexity, industrial complexity, and economic growth of 558 micro-regions between 2003 and 2019. Our results show that export complexity is more evenly spread than industrial complexity. Only a few -- mainly developed urban places -- have comparative advantages in sophisticated services. Regressions show that a region's industrial complexity is a significant predictor for 3-year growth prospects, but export complexity is not. Moreover, economic complexity in neighboring regions is significantly associated with economic growth. The results show export complexity does not appropriately depict Brazil's knowledge base and growth opportunities. Instead, promoting the sophistication of the heterogeneous regional industrial structures and development spillovers is a key to growth.

Confidence in the Judicial System: Evidence from Brazil

Abstract:One issue that affects the economic and social development of a country is the ability of the judiciary to present itself as a legitimate instance in resolving conflicts that arise in the social, business and economic development. One way to measure this is through legitimacy of the motivations that lead citizens to trust or not in the Judiciary. We created the Brazilian Confidence in Justice Index (BCJI) as a validation argument for our confidence measure. The BCJI is a measure of perception, which shows the opinion of the population about Brazil’s judiciary. Our results indicate that race and gender are important predictors once controlled for other characteristics of respondents. Blacks have a slightly lower level of confidence in the judiciary than whites. Women also present less confidence than men. We also show that people with low income have lower levels of trust. Our findings also have other important implications for confidence in the judicial system. We show that there is a positive and strong relationship between confidence in the judicial system and propensity to seek the judiciary. This result indicates that although judiciary has lower levels of trust for a pool of demographic and economic variables, most Brazilians perceive it as a legitimate way to seek solution to their problems and would not hesitate to go to court to resolve conflicts of their daily lives.
Private health care sector investment in Brazil: opportunities and obstacles

Abstract:The Brazilian health system is based upon the constitutional right formulated in 1988, according to which health is the peoples' right and duty of the State. So being, it is essentially the government's responsibility, expressed in the so-called Sistema Unico de Saúde--SUS (single health system) Since its creation, however, it admits the existence of a supplementary health system, left to the private sector. In general terms, the public system is considered unsatisfactory in the services it renders. Its resources are distributed heterogeneously, favoring centers of advanced medical practice, to the detriment of basic health care. The supplementary system is considered of better quality, however with great variations and frequent accusations of being essentially profit driven, instead of being driven to the needs of the assisted population. The growing search for health plans is a direct consequence of the image perceived by the population regarding the quality and accessibility of the public services, as well as of the peoples' growing consciousness of their needs, rights and duties as citizens. The need for continuous quality improvement and cost reduction offers numberless opportunities for actions and investments. Initiatives to identify and implement the best medical practices, medical guidelines and actions are essential regarding those illnesses which are most frequent, of higher cost and of greater risk. Health plans and healthcare providers will necessarily have to focus on their common client. Therefore, organizations must be created in order to develop initiatives aimed to the quality of patient care, as well as to the collection and dissemination of data regarding the production and results of the main service providers. Consequently, immense opportunities are being opened for investments in the area of Information Technology, collection, analysis, and data dissemination. This paper analyses the main trends in the Brazilian health sector and from the data and tendencies, draws various conclusions on the opportunities and barriers for private investment.

Abstract:Abstract: The article aims to analyze which factors contributed to the inequality across income distribution of Brazilian workers in rural areas, occupied in agricultural and non-agricultural activities. Quantile regression with sample selection bias correction and counterfactual decomposition of income by quantiles were applied, using the microdata from the National Continuous Household Survey (PNAD-C) for the years 2012 and 2019. The results showed that there is income inequality favorable to workers occupied in non-agricultural activities concerning agricultural activities, which is intensive for those with lower incomes, as well as for those with high incomes. The presence of sectorial segmentation was also confirmed, of which the largest portion in 2012 corresponds to the labor market duality. However in 2019, in lower quantiles the segmentation obtained greater explanatory power for the difference in income between the groups, while in higher quantiles the theory of human capital prevailed.
