Abstract:Objective To learn the physiology of esophageal motility and to examine the esophageal motility basis in the occurrence of gastroesophageal reflux (GER) in children.Methods Esophageal manometry was measured by low compliance capillary infusion system (PC Polygraf HR) in 60 healthy children and 62 patients with pathological GER diagnosed by 24-hr esophageal pH detection.The esophageal motility function was investigated among different age groups in healthy children and between the patients and the healthy at the same ages.Results Lower esophageal sphincter length (LESL),the intro-abdominal length and intro-thoracal length of LES increased with age in healthy children(P<0.01).Lower esophageal sphincter pressure(LESP)was higher in 1-year group compared to the other age groups(P<0.05).Upper esophageal sphincter length(UESL)increased with the age.Upper esophageal sphincter pressures(UESP)were lower in 1 and 3-year groups than that in other age groups(P<0.01).Except for the lower velocity of peristalsis in 7-year group,there were no significant differences in esophageal motility between the GER patients and the age matched control groups.Conclusion\ There is no correlation between the antireflux ability of LESP and the age in children.The LEST,UESL and UESP function will mature gradually after 3 years old.It suggests that the LESP and LESL are not the major factors that determine the occurrence of pathological GER in children.
The Evaluation of Acid Reflux Index in the Diagnosis of Pathological Gastroesophageal Reflux in Children: the Analysis of Data of Esophageal Ph Monitoring of 4109 Cases in 12 Years

Abstract:Objective The aim of the present study was to evaluate the diagnostic value of acid reflux index(RI) in the pathological gastroesophageal reflux(GER) in children and to discuss the diagnostic criteria of GER.Methods The patients performed by esophageal pH monitoring in Children's Hospital of Zhejiang University School of Medicine were all enrolled in this study from 1994 to 2006,except for the cases repeated or unfinished the measurement.According to the Boix-Ochoa criteria,in which pathological GER was confirmed if Boix-Ochoa score larger than 11.99,receiver operating characteristic(ROC) of RI was analyzed and the best diagnostic cutoff value for pathological GER was obtained.The difference and goodness of fitness of different results from other diagnostic criteria of pathological GER based on all kinds of RI were compared by statistical method such as McNemar test and kappa test.The sensitivity and specificity among different diagnostic criteria were evaluated.Results A total of 4109 cases(male 2 692 and female 1 417) performed esophageal pH monitoring were enrolled in this study.Among them,476 cases were neonate,1 553 younger than 1 year,658 younger than 3 years,837 younger than 7 years,513 younger than 12 years,and 72 younger than 16 years.There were thirteen kinds of diseases,including asthma(366 cases),bronchitis(468),pneumonia(385),persist pneumonia(143),chronic cough(184),pharyngitis(29),neonate apnea(102),neonate vomiting(283),neonate pneumonia(105),vomiting and/or upper abdominal pain(1817),hiatal hernia(140),non-cardiac chest pain(55),and malnutrition(32).ROC analysis indicated that the diagnostic cutoff value of RI was 2.9,and the sensitivity for pathological GER was 90.4%,while the specificity was 95.6%(false positive rate lower than 5.0%).The area under curve was 0.981,standard error was 0.002,and 95% confidence interval was from 0.977 to 0.984.It showed statistical significance for diagnoses of pathological GER(P=0.000).According to the new diagnostic criteria of pathological GER for both RI 4.0 criteria(RI more than 4.0%) and RI 2.9 criteria(RI more than 2.9%),the difference and goodness of fitness of results from Boix-Ochoa criteria were compared in 140 cases with hiatal hernia confirmed by surgery.The goodness of fitness from RI 2.9 criteria was significantly higher than that from RI 4.0 criteria(κ coefficient was 0.892、0.715,respectively,P all equal to 0.000).The sensitivity,accuracy,and negative predictive value for diagnostic pathological GER were significantly higher from RI 2.9 criteria than that from RI 4.0 criteria except for the specificity,while positive predictive value was almost equal between the two criteria.Conclusions RI 2.9 criteria had of significance value in the diagnosis of pathological GER.It was helpful to improve the sensitivity and accuracy of GER diagnoses in children compared with RI 4.0 criteria.
Bile and Acid Reflux in the Pathogenesis of Reflux Oesophagitis in Children

Abstract:Aim: The aim of this study was to investigate the role of bile and acid reflux in the pathogenesis of reflux oesophagitis (RE) in children.Methods: A total of 44 patients aged 5-17 years with gastro-oesophageal reflux symptoms were enrolled. Simultaneous 24-h oesophageal Bilitec 2000 (Medtronic Instruments, Minneapolis, MN, USA) bilirubin monitoring and pH monitoring, in biopsy of oesophageal mucosa by gastro-endoscopy, were performed in all patients.Results: According to the diagnostic criteria of pathological acid reflux and pathological bile reflux, 10 of 44 cases (22.7%) had acid reflux, 10 (22.7%) had isolated bile reflux, 16 (36.4%) had mixed acid and bile reflux, and the other eight (18.2%) had no reflux. Significant difference was observed in the ratio of different patterns of reflux between the RE group (26 cases) and the non-erosive reflux disease (NERD) group (18 cases) (chi(2) = 9.096, P < 0.01). All the parameters of acid reflux in the RE group were higher significantly than that in the NERD group (P < 0.05 or P < 0.01). A total of 20 out of 26 cases (76.9%) with RE had oesophageal acid reflux as against six out of 18 cases (33.3%) in patients with NERD (P < 0.01). The difference of each parameter of bile reflux had not reached significance between the two groups.Conclusions: Mixed reflux is the predominant form of reflux in the causation of oesophageal mucosal injury in children. Isolated bile reflux also plays a role in the development of RE, although only in patients without acid reflux.
[Noncardiac Chest Pain and Gastroesophageal Reflux in Children].

Abstract:OBJECTIVE To investigate the relationship between noncardiac chest pain (NCCP) and gastroesophageal reflux (GER) in children and to assess the diagnostic value of esophageal pH monitoring in patients with NCCP. METHODS Ambulatory 24 h esophageal pH monitoring was performed in 36 NCCP cases (patient group) with recurrent chest pain and without respiratory diseases and musculoskeletal abnormalities in chest, and 20 of them underwent gastric endoscopy. Eleven of the 20 cases were found to have esophagitis (esophagitis group), while the other 9 cases showed no signs of esophagitis (non-esophagitis group) according to the diagnostic criteria of esophagitis. RESULTS The episode of esophageal pH < 4, the number of reflux longer than 5 min, the longest reflux time, the percent time of pH < 4 and Boix-Ochoa score in the patient group were 60 +/- 7, 2.44 +/- 0.74, (12.4 +/- 2.8) min, 6.72 +/- 1.39, 24.6 +/- 3.9, respectively, as compared to 33 +/- 4, 0.35 +/- 0.11, (4.3 +/- 0.9) min, 1.25 +/- 0.19, 7.7 +/- 0.9 in the control group. The difference between the two groups had reached significance (t = 3.44, 2.79, 2.73, 3.89, 4.24, respectively, all P < 0.01). The positive rate of acid reflux in the patient group was 58.3% (21/36) whose Boix-Ochoa score higher than 11.99. Eleven of the 20 cases (55.0%) receiving endoscopy were found to have esophagitis, among them the positive rate of GER occurred 81.8% (9/11). Whereas, the other 9 cases (45.0%, 9/20) showed non-esophagitis and the positive rate of GER showed 33.3% (3/9). The number of reflux longer than 5 min and the percent time of pH < 4 in the esophagitis group were 5.8 +/- 2.0, 12.5 +/- 3.5, respectively, as compared to 0.9 +/- 0.5, 3.4 +/- 1.4 in the non-esophagitis group. Significant difference were observed between the two groups (Z = -2.400, -2.545, respectively, all P < 0.05). The episode of esophageal pH < 4, the longest reflux time, and Boix-Ochoa score in the esophagitis group were 73 +/- 11, (26.4 +/- 7.3) min, 41 +/- 10, respectively, as compared to 34 +/- 11, (4 +/- 3) min, 14 +/- 5 in the non-esophagitis group. The difference between the two groups showed significance (Z = -2.926, -2.675, -2.584, respectively, all P < 0.01). CONCLUSIONS GER is very common in children with NCCP. Esophageal pH monitoring has a therapeutic predictive value in addition to its diagnostic merit for NCCP in children.
Role of proximal gastric acid reflux in the development of respiratory symptoms in children with gastro-esophageal reflux

Abstract:Objective To understand the impact of proximal gastric acid reflux in the development of respiratory symptoms in children with gastroesophageal reflux(GER). Methods 24- hour esophageal pH monitoring with a dual pH probe were investigated in 31 children with GER and without respiratory symptoms,23 children with both GER and respiratory symptoms,and 35 healthy children as controls,respectively. Results All acid reflux indexes of distal esophagus and main acid reflux index of proximal esophagus were significantly higher in all GER children with or without respiratory symptoms than that in control group.No significant difference in each parameter of pH monitoring in both of proximal esophagus and distal esophagus was observed between GER children with respiratory symptoms and GER children without respiratory symptoms.However,each parameter of distal esophagus was significantly higher than that of proximal esophagus in all GER children with or without respiratory symptoms. Conclusions Proximal gastric acid reflux do not play a role in the development of respiratory symptoms in children with GER.Distal gastric acid reflux is a major form of reflux in children with GER regardless of the occurrence of respiratory symptoms.
A Study on Acid Reflux in Children with Hiatal Hernia

Abstract:Objective To investigate the acid reflux in children with hiatal hernia (HH) and the characteristic of reflux among different types of HH. Methods Ambulatory 24 h lower esophageal pH monitoring was studied in 35 patients with HH confirmed by barium swallow, of whom 18 cases had sliding hernia, 8 had paraoesophageal hernia, 9 had mixed hernia, and 16 healthy infants formed the control groups. Results All parameters in HH patients were significantly higher than those in controls ( P 0 01). Thirty three of 35 patients (accounting for 94%) had pathological acid reflux according to Boix Ochoa criterion, while the other 2 cases were considered to have physiological reflux, diagnosed as paraoesophageal hernia and mixed hernia, respectively. In HH patients, the number of reflux episodes lasting for 5 min, the duration of the longest episode, mean duration of reflux episodes and the percentage of time of pH4 for total periods were significantly higher at lying than at standing position ( P 0 05). No significant differences in all parameters except for the duration of the longest episode were found in the patients among three kinds of hernia. Conclusions These results showed that pathological acid reflux almost always existed in children with HH, especially at lying position. The association between the severe degree of acid reflux and the type of HH was not obvious. The study revealed that it is important to treat the HH patients with antireflux therapy early and effectively regardless its types.
Relationship Between Chronic Cough and Gastroesophageal Reflux in Children

Abstract:Objectives To investigate the relationship between chronic cough and gastroesophageal reflux(GER)in children.Methods Twenty-four-hour esophageal pH monitoring was performed in 170 cases(patient group)with chronic cough( 4 weeks),and 32 asymptomatic children served as control group.Results All the parameters of esophageal acid reflux including episodes of acid reflux,number of the reflux longer than 5 min,the longest reflux time(min),percentage time of pH 4 and Boix-Ochoa score were significantly higher in patient group than that in control group(P 0.01=.According to the diagnostic criteria of GER(Boix-Ochoa score 11.99 and reflux index ≥ 4.0%),pathological GER was detected in 63 of 170(37.1%)in patient group,while it was only 3.1%(1/32)in controls,with the statistical difference(χ2 = 14.327,P 0.01).The significant differences were also observed in the positive rate of GER among the three different age group,they were 47.4%(27/57),41.5%(17/41)and 26.4%(19/72)in patients aged from two months to one years,less than 3 years and more than 3 years,respectively(χ2 = 6.453,P 0.05).Conclusions It was suggested that GER could be the underlying cause of chronic cough in children,especially in those aged less than 3 years.24-hour ambulatory esophageal pH monitoring is a useful test for identifying the etiology of chronic cough in children.
Role Of Proximal Gastric Acid Reflux In Causation Of Respiratory Symptoms In Children With Gastroesophageal Reflux

Abstract:Objectives: The association of gastroesophageal reflux (GER) and respiratory disorders is well known but the mechanism is still unclear. This study aims to evaluate the presence and severity of proximal gastric acid reflux in children having GER with or without respiratory symptoms. Methods: 24-hour esophageal pH monitoring with a dual pH probe placed in the proximal and distal esophagus it,as performed in 23 and 31 children having GER with or without respiratory symptoms, respectively. Results: No significant difference in the parameters of pH monitoring in either proximal or distal esophagus was observed between GER patients with or without respiratory symptoms. The proportion of patients having proximal GER among those with respiratory symptoms was not significantly different from those without respiratory symptoms (P > 0.05). Conclusion: Proximal esophageal acid reflux does not seem to play. a role in the development of persistent respiratory symptoms in children with GER. Distal esophageal acid reflux is the predominant form of reflux in children wth GER regardless of the occurrence of respiratory symptoms.
Chronic Laryngopharyngeal Inflammation Associated with Gastroesophageal Reflux in Children

Abstract:Objectives To investigate the relationship between chronic laryngopharyngeal inflammation and gastroesophageal reflux(GER)in children.Methods Twenty-four-hour esophageal pH monitoring was performed in 37 cases(patient group)with chronic refractory laryngopharyngeal inflammation( 4 weeks),and 30 asymptomatic children served as control group.Results Each parameter of esophageal acid reflux in patient group such as the episodes of acid reflux,the frequency of reflux longer than 5 min,the longest reflux time(min),the percentage time of pH 4 and Boix-Ochoa score were significantly higher than that in control group(all P 0.01).According to the diagnostic criteria of pathological GER(Boix-Ochoa score 11.99 and reflux index ≥ 4.0%),pathological GER was detected in 40.5%(15/37)of patients with chronic laryngopharyngeal inflammation,while it was 3.3%(1/30)in control.There is statistical significance between the two groups(χ2 = 12.617,P 0.01).Conclusions It suggests that GER is closely related to chronic refractory laryngopharyngeal inflammation in children.When routine treatment to childhood chronic laryngopharyngeal inflammation fails,GER may be considered for differential diagnosis.

Abstract:Objective To explore the mechanism of gastroesophageal reflux diseases (GERD) and the application of esophageal manometry in the diagnoses of GERD in children. Methods Esophageal manometry was applied in 30 children with pathological gastric acid reflux confirmed by 24 hr esophageal pH monitoring. Results Lower esophageal sphincter pressure (LESP) and barrier pressure (BP) between gaster and esophagus in patients were significantly lower than those in controls, whereas the difference of lower esophageal sphincter length (LESL) between the two groups was not significant. The frequency of transitory lower esophageal sphincter relaxation (TLESR) was significantly higher in patients compared to controls. Conclusion It suggests that LESP, BP and TLESR may be the major pathogenetic factors of GERD in children, while LESL may not be an important factor.
Evaluation of Gastroesophageal Reflux in Symptomatic Young Infants Using Multichannel Intraluminal pH-Impedance Testing: A large Cohort Study from a Single Center

Abstract:Objectives This study aimed to assess the use of combined multichannel intraluminal impedance and pH studies (MII–pH) in a large group of symptomatic young infants, to characterize the occurrence of gastroesophageal reflux disease (GERD), and to establish temporal association of the reflux behaviors with gastroesophageal reflux using symptom indices. Study Design This is a retrospective cohort study on 181 infants who underwent MII–pH studies for clinical behaviors that were suggestive of GERD. Symptom index (SI) and symptom association probability (SAP) were used to establish symptom association with reflux. More than 100 GER episodes in 24 hours or acid reflux index > 10% was considered pathological reflux. Results A total of 181 infants (median age: 60 days, interquartile range [IQR]: 34–108) underwent MII–pH studies with median study duration of 22.41 hours (IQR: 21.5–23.32). A total of 4,070 hours of data were analyzed, with 8,480 reflux events (2,996 [35%] acidic, 5,484 [65%] nonacidic). A total of 2,541 symptoms were noted, 894 (35%) were temporally related to reflux events. A total of 113 infants (62.4%) had positive symptom association with SI > 50% and/or SAP > 95% for at least one symptom. There was modest symptom association for choking and gagging, but apnea, bradycardia, and desaturations had poor symptom association. Only 29 infants (16%) had pathological reflux, and only 18 infants (10%) had both pathological reflux and positive symptom association. Conclusion MII–pH can be used to characterize GERD in young infants, along with establishing temporal association with symptoms. Pathological reflux in symptomatic young infants is not common, but symptom association may occur without frequent or acidic reflux. Key Points Gastroesophageal reflux (GER) disease can be studied in young infants using MII–pH, to characterize the frequency and nature of GER events. The probability of GER events being associated temporally with GER symptoms can also be determined using MII–pH in this population. Using frequency of GER events, reflux indices, and symptom association indices with MII–pH, infants having true GER disease can be identified, thereby reducing unnecessary therapy. Abstract:(1) Background: Multichannel intraluminal impedance-pH (MII-pH) monitoring is commonly used to diagnose gastroesophageal reflux disease (GERD). The mean nocturnal baseline impedance (MNBI) is an important parameter, reflecting the esophageal mucosal integrity and improvement in GERD. This study aims to evaluate the correlation between conventionally measured MNBI and a recently described simple MNBI measurement method in diagnosing pediatric GERD. (2) Methods: This prospective observational study enrolled 64 children aged one month to 18 years who underwent 24 h MII-pH monitoring. Conventional MNBI was measured during stable 10 min intervals at night, while the simple MNBI method averaged impedance throughout the nocturnal supine period. (3) Results: Strong correlations were found between conventional and simple MNBI values across all impedance channels in both infants (r > 0.85) and older children (r > 0.9). Conventional and simple MNBIs in the most distal channel (Z6) effectively differentiated non-erosive reflux disease (NERD) from other phenotypes, with AUCs of 0.864 and 0.860, respectively. The simple MNBI demonstrated good diagnostic performance with similar sensitivity and specificity to the conventional MNBI. (4) Conclusions: Including MNBI measurements into routine MII-pH monitoring may enhance GERD diagnosis and reduce the need for more invasive procedures.
In vivo Raman spectroscopy reveals biochemical composition of the esophageal tissue in pediatric eosinophilic esophagitis.

Abstract:Background and Aims: Biochemical alterations in the esophagus of patients with eosinophilic esophagitis (EoE) is poorly understood. We used Raman spectroscopy via a pediatric endoscope to identify key Raman features reflective of the esophageal biochemical composition to differentiate between children with EoE from non-EoE controls, and between children with active (aEoE) and inactive EoE (iEoE). Methods: Spectral measurements were obtained using a customized pediatric endoscope-compatible fiberoptic Raman probe in real-time during an esophagogastroduodenoscopy. Chemometric analysis was performed to identify key Raman features associated with EoE. Pearson's correlation analysis was used to assess relationship between the key Raman features and EoE activity indices. Their diagnostic utility was ascertained using the receiver operator characteristic curve analysis. Results: Forty-three children were included in the study [EoE=32 (74%), non-EoE control=11 (26%); aEoE=20 (63%) and iEoE=12 (37%)]. Raman intensities assigned to lipids, proteins, and glycogen:protein ratio accurately distinguished children with EoE from those without EoE, and aEoE from iEoE. They significantly correlated with EoE activity indices. The Raman peak ratio for lipids had 90.6% sensitivity, 100% specificity, and an AUC of 0.95 to differentiate children with EoE from non-EoE controls. The glycogen:protein ratio had 70% sensitivity, 91.7% specificity, and an AUC of 0.75 to distinguish children with aEoE from iEoE. Conclusion: Real-time intra-endoscopy Raman spectroscopy is an effective method for identifying spectral markers reflective of the esophageal biochemical composition in children with EoE. This technique may aid in the diagnosis and monitoring of EoE and help to elucidate EoE pathogenesis.

Abstract:Background/Objectives: Researchers have proposed two novel impedance-pH parameters, mean nocturnal baseline impedance (MNBI) and the post-reflux swallow-induced peristaltic wave (PSPW) index, to enhance the diagnosis of gastroesophageal reflux disease (GERD) and enable better predictions of the effectiveness of anti-reflux therapies. This systematic review aims to synthesize the available evidence on the utility of the PSPW index and MNBI as diagnostic tools for pediatric GERD. Methods: A systematic search of studies reporting PSPW index and MNBI values in patients with GERD was performed in PubMed, Embase, Clarivate, Scopus, Cochrane and Google Scholar databases from their beginning until April 2024. The following terms were used: GERD, children, pediatric, PSPW and MNBI. Results: Eight studies were included, describing 479 patients ranging from 2 months to 17 years old over an 8-year period in 12 pediatric centers. Four studies demonstrated that children with pathological acid exposure have a significantly lower MNBI, with a good discriminatory ability to diagnose GERD. The PSPW index showed lower values in patients with reflux hypersensitivity (RH) compared to those with functional heartburn (FH). Conclusions: Patients with pathological acid exposure tend to exhibit lower MNBI and PSPW index values compared to those with normal acid exposure. MNBI and the PSPW index show promise as diagnostic tools in distinguishing between different GERD phenotypes. Further research is needed to establish standardized diagnostic criteria and optimize the clinical applicability in GERD diagnosis and management.

Abstract:Two hundred and twenty patients with symptoms suggestive of pathologic gastroesophageal reflux were investigated to elucidate the ability of symptoms and endoscopic findings in establishing a diagnosis of reflux disease as measured by ambulatory 24-h pH-monitoring. Daily occurrence of heartburn or acid regurgitation had positive predictive values of 59% and 66%, respectively. pH-monitoring showed pathologic reflux in 75% of patients with esophageal mucosal erosions. Endoscopic erythema of the distal esophagus predicted reflux disease in only 53%. Symptom registration during ambulatory 24-h pH-monitoring showed that about half of the symptomatic events reported by patients with pathologic reflux occurred within 5 min of a reflux episode. The corresponding figure for patients with normal pH-monitoring was less than 20%. We conclude that it is difficult to establish a diagnosis of gastroesophageal reflux disease by patient history alone, that erythema at endoscopy correlates poorly with pathologic reflux, and that reflux disease may be present even with normal endoscopy findings.

[Monitoring of the permeability of the inferior esophageal sphincter using sound signals]

Abstract:An original method is described for graphic display of sounds arising from the cardia during the swallowing of saliva or water. Two dynamic microphones were placed on the skin of epigastric xyphoid area and simultaneously on the latero-cervical position. The following conditions must be filled: position of the subject (upright or lying), correct application of microphone with pressure (2 kPa), specially for the xyphoid situation. For graphic restitutions from magnetic signal, a time-recording paper was used, on line or off line, with an analogical device including Butterworth band active filter (optimal frequency range 0.8-1.2 kHz), amplifier and integrator (tau = 2 s). Occurrence of typical cardia sounds were 86-97 % for saliva and 95-100 % for water in two normal subjects for the evaluation of reproducibility (one man and one woman 35 and 47 years old respectively, n = 234 sequences, interval 20 s). Using the distance between the upper and lower microphones, the esophageal time transit can be measured. The signal was more unconstantly recorded for saliva 40 +/- 44 % in adults (n = 10) and 76 +/- 17 % in children (n = 7), than for 80 et 87 % for water respectively. The optimal interval between two swallows appeared to be 15-25 s. In patients with presumption of achalasia (n = 8), the two phonic signals were recorded during esophageal pressure measurement. The presence of esophageal sounds seems to be the sign of absence of achalasia.
Low back pain and sciatica prevalence and intensity reported in a Mediterranean country: ordinal logistic regression analysis.

Abstract:The objective of this retrospective cross-sectional study was to estimate the 6-month prevalence and severity of low back pain and sciatica in a representative sample of an adult Mediterranean population. The study group comprised a sample of 674 adults aged 20 years or older from a mainly (74.8%) urban population. Information regarding low back pain and sciatica prevalence and severity and its related aspects, as well as socioeconomic and demographic characteristics, was collected by personal interviews with a validated questionnaire. The association between the intensity of low back pain and sciatica with several sociodemographic parameters was tested using ordered univariate and multivariate logistic regression analysis.A total of 266 (39.5%) patients reported low back pain and 166 (24.6%) reported sciatica during the previous 6-month period. A woman living in a Mediterranean country reported low back pain of increased severity if she was a married housewife aged older than 65 years who was a smoker and suffered from depression. More severe sciatic pain was reported by working married women older than 65 years who were smokers.
Measurement of Oesophageal Hiatus Surface Area by Multiplanar Reconstruction of MDCT: Relationship with Lower Oesophageal Sphincter Pressure and Acid Reflux.

Abstract:AIM: To evaluate the relationship between oesophageal hiatus surface area (OHSA) and gastro-oesophageal reflux disease (GERD).MATERIALS AND METHODS: Patients who underwent 24-h pH monitoring, oesophageal high-resolution manometry, and upper abdominal contrast-enhanced multidetector computed tomography (MDCT) during 2014-2021 were enrolled. Patients with a hiatus hernia (HH) on MDCT or who had a history of gastro-oesophageal surgery were excluded. Multiplanar reconstruction (MPR) of the MDCT image was used for the measurement of OHSA. Correlations of OHSA with acid exposure time (AET) and lower oesophageal sphincter (LOS) pressure of all patients were analysed.RESULTS: Seventy-eight patients were included in the study. OHSA was much less in the AET <4% group than in the AET >6% group (1.61 +/- 0.42 versus 2.09 +/- 0.55 cm(2), p<0.001). Correlation analysis reveals that OHSA correlated positively with AET (correlation coefficient = 0.47, p<0.001). Receiver operating characteristic (ROC) curve analysis reveals that OHSA can significantly distinguish patients in different groups divided by AET (area under the ROC curve [AUC] = 0.76, 95% confidence interval [CI]: 0.63-0.90). OHSA was not related to LOS pressure (correlation coefficient =-0.268, p=0.051). There was no difference in OHSA between the low LOS pressure group and the normal LOS pressure group (1.84 +/- 0.61 versus 1.74 +/- 0.50 cm(2), p=0.52). CONCLUSIONS: OHSA significantly correlated with AET but has no relationship with LOS pressure.
