Abstract:Nakitanda and colleagues 1 report their findings of a large population-based cohort study investigating the risk of developing major congenital malformations after exposure to tetracyclines during the first trimester of pregnancy. They conclude that exposure to tetracyclines during the first trimester is not associated with an increased risk of developing major congenital malformations. However, the authors indicate that additional investigations are still necessary to rule out potential risks because of power limitations for several major congenital malformation subgroups and individual malformations. It is important to realize that their study 1 is considerably larger (almost 4-fold) than earlier reports in terms of the number of exposed infants (6340 vs 1694 infants in Cooper et al 2 ) and documented cases, and also covered a broad range of major congenital malformation subgroups, as well as individual major congenital malformations. Despite all these great improvements in their population-based study, it is still impossible to reach a definitive conclusion regarding the safety of tetracyclines during pregnancy. Bevelander et al 3 reported 65 years ago that tetracyclines administered during pregnancy pass through the placenta, are concentrated in the fetal skeleton, and may diminish skeletal growth. On the basis of these and subsequent additional findings, tetracyclines have been labeled as potentially harmful because of the ability to cause developmental malformations in the fetus, lifelong caramel discoloration of the teeth after exposure prenatally or during the first 8 years of life, and, once in a while, lethal drug-induced liver toxicity described in pregnant individuals. 4 However, we need to realize that doxycycline is currently the most frequently used tetracycline during pregnancy. 5 It was developed after tetracyclines as a class of drugs already had been labeled as potentially harmful. Doxycycline is prescribed to cure a myriad of infections, such as malaria, sexually transmitted diseases (eg, chlamydia and syphilis), dermatitis and acne, and rickettsial illnesses, including Lyme disease, which is caused by the bite of ticks, mites, flees or lice. In addition, it is effective against tularemia, cholera, and bubonic plague, which might be used during bacteriological warfare. 4 ,6 A few years ago, Cross and colleagues 4 provided several arguments, based on their systematic review, that doxycycline might not be associated with the same adverse events as the other tetracyclines. In other words, the jury is still out on the safety of the use of doxycycline during pregnancy. Its use might be less or even not harmful at all compared with the use of the older tetracyclines. This lack of necessary knowledge for optimal use of doxycycline or any of the other tetracyclines during pregnancy all boils down to the fact that pregnant persons still need to be viewed as therapeutic orphans. Pregnant individuals (and those of child-bearing potential) have historically been excluded from clinical trials because of the well-intentioned attempt to protect the vulnerable fetus. Unfortunately, an unintended consequence of this protection has been an information gap that makes prescribing drugs for pregnant individuals with preexisting illnesses or new-onset disorders difficult and potentially dangerous for both the pregnant individuals and their fetuses. In the meanwhile, we have the moral obligation to support pregnant individuals and their health care practitioners until there is more certainty about the safety of using the different tetracyclines during the 3 stages of pregnancy. As with the administration of any other medication, the potential benefits of using tetracyclines during pregnancy need to be weighed against the risk to the fetus. To improve the current situation, various countries are establishing registries to document possible adverse effects of use of drugs during the different stages of pregnancy. There is, however, a need to promote active, spontaneous reporting of adverse events during pregnancy to ensure rapid generation of evidence regarding the safety of a drug in pregnant individuals. The most frequently dispensed medications during pregnancy are analgesics, antibiotics, and antiemetics. 5 Mansour and colleagues 5 emphasized in their study that additional research is needed on the safety of specific anti-infectives (antibiotics, antifungals, and antivirals) during pregnancy. In summary, pregnant individuals are the last therapeutic orphans and it is time to put everything in place to change that unacceptable situation. Abstract:BACKGROUND AND AIM: Triclosan (TCS) is widely used as an antibacterial agent in consumer products and is suspected of having endocrine-disrupting properties, limited epidemiological studies examined the developmental effects of TCS exposure before pregnancy. Meanwhile, Vitamin D deficiency is a global health issue and the consequences of vitamin D deficiency during pregnancy cannot be underestimated. We investigated association between maternal TCS exposure and anogenital distance (AGD) and other genital measurements in infants at birth after considering maternal vitamin D status. METHODS: Participating 258 women were recruited from Shanghai Birth Cohort between 2012 and 2013. TCS concentration was quantified from maternal urine samples collected before pregnancy. Vitamin D was measured by maternal plasma during early pregnancy. A standardized measure of AGD was obtained in infants at birth. Anogenital index (AGI) was defined as AGD divided by weight (kg) at examination. Effect modification by Vitamin D was estimated. RESULTS: We found no significant associations between maternal TCS and AGD in male or female infants. However, after stratified by maternal plasma vitamin D status (deficiency (<20ng/ml) versus insufficiency or sufficient (≥20 ng/mL)), per unit increase in maternal lg concentration of TCS, AGI increased on average by 0.23 in male infants (95% confidence interval (CI): 0.02, 0.45) when maternal VD concentration was lower than 20 ng/mL. No significant association was observed between maternal TCS and AGD in male infants when VD concentration is higher than 20 ng/mL. Furthermore, no significant association of maternal TCS with AGD or AGI was found in female infants. CONCLUSIONS: These data support the hypothesis that maternal TCS exposure at environmental levels can adversely affect male reproductive development especially when maternal Vitamin D status is deficiency. Covid-19 infection and vaccination during first trimester and risk of congenital anomalies: Nordic registry based study

Abstract:Abstract Objectives To evaluate the risk of major congenital anomalies according to infection with or vaccination against covid-19 during the first trimester of pregnancy. Design Prospective Nordic registry based study. Setting Sweden, Denmark, and Norway. Participants 343 066 liveborn singleton infants in Sweden, Denmark, and Norway, with an estimated start of pregnancy between 1 March 2020 and 14 February 2022, identified using national health registries. Main outcome measure Major congenital anomalies were categorised using EUROCAT (European Surveillance of Congenital Anomalies) definitions. The risk after covid-19 infection or vaccination during the first trimester was assessed by logistic regression, adjusting for maternal age, parity, education, income, country of origin, smoking, body mass index, chronic conditions, and estimated date of start of pregnancy. Results 17 704 (5.2%) infants had a major congenital anomaly. When evaluating risk associated with covid-19 infection during the first trimester, the adjusted odds ratio ranged from 0.84 (95% confidence interval 0.51 to 1.40) for eye anomalies to 1.12 (0.68 to 1.84) for oro-facial clefts. Similarly, the risk associated with covid-19 vaccination during the first trimester ranged from 0.84 (0.31 to 2.31) for nervous system anomalies to 1.69 (0.76 to 3.78) for abdominal wall defects. Estimates for 10 of 11 subgroups of anomalies were less than 1.04, indicating no notable increased risk. Conclusions Covid-19 infection and vaccination during the first trimester of pregnancy were not associated with risk of congenital anomalies.

Abstract:Objective: To characterize associations of exposure to newer antiretroviral medications in the first trimester with congenital anomalies among infants born to persons with HIV in the United States. Design: Longitudinal cohort of infants born 2012-2022 to pregnant persons with HIV enrolled in the Surveillance Monitoring for ART Toxicities (SMARTT) study. Methods: First-trimester exposures to newer antiretrovirals (ARVs) were abstracted from maternal medical records. Trained site staff conducted physical exams and abstracted congenital anomalies from infant medical records. Investigators classified anomalies using the Metropolitan Atlanta Congenital Defects Program classification system. The prevalence of major congenital anomalies identified by age one year was estimated for infants exposed and unexposed to each ARV. Generalized estimating equation models were used to estimate the odds ratio (OR) of major congenital anomalies for each ARV exposure, adjusting for potential confounders. Results: Of 2034 infants, major congenital anomalies were identified in 135 [6.6%; 95% confidence interval (CI): 5.6-7.8%]. Cardiovascular ( n = 43) and musculoskeletal ( n = 37) anomalies were the most common. Adjusted ORs (95% CI) of congenital anomalies were 1.03 (0.62-1.72) for darunavir, 0.91 (0.46-1.81) for raltegravir, 1.04 (0.58-1.85) for rilpivirine, 1.31 (0.71-2.41) for elvitegravir, 0.76 (0.37-1.57) for dolutegravir, and 0.34 (0.05-2.51) for bictegravir, compared to those unexposed to each specific ARV. Findings were similar after adjustment for nucleoside/nucleotide backbones. Conclusions: The odds of congenital anomalies among infants with first-trimester exposure to newer ARVs did not differ substantially from those unexposed to these specific ARVs, which is reassuring. Continued evaluation of these ARVs with larger studies will be needed to confirm these findings.

Abstract:For the first time, a congenital cataract due to tetracycline hydrochloride in animal experiments and in human foetuses was demonstrated. This cataract was found after intramuscular or peroral administration of tetracycline hydrochloride to pregnant animals or pregnant women. No eye changes were observed in sucklings due to lactation, when the mothers were treated with tetracycline post-partum. The most dangerous period for tetracycline cataract formation seems to be the first trimester of pregnancy.

Risk of Dental Discoloration and Enamel Dysplasia in Children Exposed to Tetracycline and Its Derivatives

Abstract:Purpose: To examine the risk of dental abnormalities after exposure to tetracycline and its derivatives (TCs) in Korean children. Materials and methods: Children aged 0-17 years with a claim for prescriptions of TCs between 2002 and 2015 were identified from the Sample Research Database 2.0 of the National Health Insurance Service. Children not exposed to TCs were selected as the control group by matching sex and age (1:4). Cumulative incidence rate and relative risk of dental abnormalities after TCs exposure were investigated. Results: The 10-year cumulative incidence rate in the 0-12 years group was 3.1% [95% confidence interval (CI), 2.3-3.9]. The 10-year cumulative incidence rates were 7.0%, 1.9%, and 1.6% in the 0-7, 8-12, and 13-17 years age groups (95% CI: 4.7-9.3, 1.2-2.6, and 1.3-1.9, respectively). There was no significant difference in the risk of dental abnormalities according to TC exposure among the age groups of 0-7 years [adjusted hazard ratio (aHR)=1.0], 8-12 years (aHR=1.1), and 13-17 years (aHR=1.2). Conclusion: Short-term exposure to TCs does not appear to increase the risk of dental abnormalities in children aged 0-7 and 0-12 years. Restrictions on the use of TCs in children aged 8-12 years, in some countries, may warrant consideration.
Selective Serotonin Reuptake Inhibitor Use During Early Pregnancy and Congenital Malformations: a Systematic Review and Meta-Analysis of Cohort Studies of More Than 9 Million Births

Abstract:BACKGROUND:In 2005, the FDA cautioned that exposure to paroxetine, a selective serotonin reuptake inhibitor (SSRI), during the first trimester of pregnancy may increase the risk of cardiac malformations. Since then, the association between maternal use of SSRIs during pregnancy and congenital malformations in infants has been the subject of much discussion and controversy. The aim of this study is to systematically review the associations between SSRIs use during early pregnancy and the risk of congenital malformations, with particular attention to the potential confounding by indication.METHODS:The study protocol was registered with PROSPERO (CRD42018088358). Cohort studies on congenital malformations in infants born to mothers with first-trimester exposure to SSRIs were identified via PubMed, Embase, Web of Science, and the Cochrane Library databases through 17 January 2018. Random-effects models were used to calculate summary relative risks (RRs).RESULTS:Twenty-nine cohort studies including 9,085,954 births were identified. Overall, use of SSRIs was associated with an increased risk of overall major congenital anomalies (MCAs, RR 1.11, 95% CI 1.03 to 1.19) and congenital heart defects (CHD, RR 1.24, 95% CI 1.11 to 1.37). No significantly increased risk was observed when restricted to women with a psychiatric diagnosis (MCAs, RR 1.04, 95% CI 0.95 to 1.13; CHD, RR 1.06, 95% CI 0.90 to 1.26). Similar significant associations were observed using maternal citalopram exposure (MCAs, RR 1.20, 95% CI 1.09 to 1.31; CHD, RR 1.24, 95% CI 1.02 to 1.51), fluoxetine (MCAs, RR 1.17, 95% CI 1.07 to 1.28; CHD, 1.30, 95% CI 1.12 to 1.53), and paroxetine (MCAs, RR 1.18, 95% CI 1.05 to 1.32; CHD, RR 1.17, 95% CI 0.97 to 1.41) and analyses restricted to using women with a psychiatric diagnosis were not statistically significant. Sertraline was associated with septal defects (RR 2.69, 95% CI 1.76 to 4.10), atrial septal defects (RR 2.07, 95% CI 1.26 to 3.39), and respiratory system defects (RR 2.65, 95% CI 1.32 to 5.32).CONCLUSIONS:The evidence suggests a generally small risk of congenital malformations and argues against a substantial teratogenic effect of SSRIs. Caution is advisable in making decisions about whether to continue or stop treatment with SSRIs during pregnancy.

Pregnancy outcomes after first-trimester exposure to fluoroquinolones: Findings based on an integrated database from two Japanese institutions

Abstract:Given the paucity of safety data on fluoroquinolone antibiotics in pregnancy, a prospective observational cohort study was conducted in pregnant women who sought help and advice on drug use at two teratology information institutes in Japan. The primary endpoint of the study was the incidence of major congenital anomalies. The study population included pregnant women exposed to (i) fluoroquinolones (fluoroquinolone group), (ii) β-lactams (infectious control group), or (iii) other agents considered to be nonteratogenic in humans (nonteratogenic control group) during the first trimester. The frequency of major congenital anomalies was compared across groups using a logistic regression model that adjusted for maternal age, smoking status, drinking status, facility consulted, and time of consultation. The fluoroquinolone group consisted of 411 women who had 383 children born alive. The infectious control and nonteratogenic control groups consisted of 1416 and 1482 women who had 1322 and 1401 children born alive, respectively. The incidence of major congenital anomalies was 1.5%, 2.0%, and 1.6% in the fluoroquinolone group, infectious control, and nonteratogenic control groups, respectively. Logistic regression showed that fluoroquinolone exposure is not a significant risk factor for major congenital anomalies. In conclusion, first-trimester exposure to fluoroquinolone antibiotics was not associated with increased maternal or fetal risks.

Abstract:Background: Triclosan (TCS) is known to possess endocrine disrupting properties and metabolize rapidly in the human body. Human data concerning repeated measurements of TCS throughout pregnancy in relation to fetal and childhood growth are sparse. Objectives: We investigated the associations between multiple measurements of maternal urinary concentrations of TCS during pregnancy and fetal and early-childhood growth. Methods: The study population included 850 mother-infant pairs who participated in a prenatal cohort established between 2014 and 2015 in Wuhan. Prenatal TCS exposure was measured in a complete series of urine samples collected at the first, second and third trimesters. General linear models and generalized estimating equation models were applied to evaluate the associations of the averaged maternal urinary concentrations of TCS over trimesters and trimester-specific urinary TCS with the z-scores of estimated fetal weight, head circumference, abdominal circumference, and femur length at 16, 24, and 31 gestational weeks, weight and length at birth, and weight and height at 0.5, 1, and 2 years of age. Results: In overall infants, we did not observe any significant association of the averaged maternal urinary concentrations of TCS over trimesters with ultrasound parameters and size at birth. However, a positive association of borderline statistical significance was found between averaged prenatal TCS exposure and the thirdtrimester estimated fetal weight z-score in girls in sex-stratified analyses (beta = 0.054, 95% CI: -0.005, 0.113, p = 0.07). Moreover, averaged prenatal TCS exposure was positively associated with 2 year-old weight z-score among total infants (beta = 0.046, 95% CI: 0.005, 0.087). After stratifying by sex, the same association was observed in girls with more prominent estimation (beta = 0.062, 95% CI: 0.000, 0.124), whereas the association weakened and became not significant in boys (beta = 0.033, 95% CI: -0.024, 0.089). TCS exposure at 1st and 2nd-trimester were positively associated with weight z-score at 2 years, in both overall and female infants. Conclusions: Prenatal exposure to triclosan was associated with elevated third-trimester estimated fetal weight and 2 year-old weight z-score in girls, and the early and middle stage of pregnancy may be the windows of vulnerability. Apart from these findings, we did not find strong evidence for prenatal triclosan exposure in relation to fetal and early-childhood growth.
Hydroxychloroquine in lupus or rheumatoid arthritis pregnancy and risk of major congenital malformations: a population-based cohort study

Abstract:Abstract Objectives To assess the infant risk of major congenital malformations (MCM) associated with first-trimester exposure to hydroxychloroquine (HCQ) among mothers with systemic lupus erythematosus (SLE) or rheumatoid arthritis (RA). Methods This population-based cohort study utilised Swedish nationwide registers and included all singleton births (2006–2021) among individuals with prevalent SLE or RA in Sweden. The exposure was filling ≥1 HCQ prescription during the first trimester. The outcome was infant MCM within one year of birth. Inverse probability of treatment weighting was applied to adjust for potential confounders (e.g. maternal smoking, body mass index, pregestational diabetes, and corticosteroids). Modified Poisson regression models with robust variance estimated risk ratios and 95% confidence intervals (RR 95%CI). Results We included 1,007 births (453 exposed) and 2,500 births (144 exposed) in the SLE and RA cohorts, respectively. The MCM risks in the SLE overall cohort, exposed, and unexposed groups were 3.6%, 3.7%, and 3.4%, respectively. The corresponding figures in the RA cohort were 4.4%, 5.6%, and 4.3%, respectively. The adjusted RRs (95%CI) were 1.29 (0.65–2.56) in the SLE cohort, 1.32 (0.56–3.13) in the RA cohort, and 1.30 (0.76–2.23) in the pooled analysis. The adjusted risk difference (exposed vs unexposed) was small (0.9% in SLE and 1.3% in RA). Sensitivity analyses examining different exposure and outcome windows yielded similar findings. Conclusions First-trimester exposure to HCQ was not associated with a significantly increased risk of MCM. HCQ’s benefits may outweigh the risks in managing SLE or RA during pregnancy.

Abstract:Purpose: Antimicrobial agents are frequently prescribed during pregnancy. This study aims to explore the association between antimicrobial exposure and pregnancy outcomes.Patients and Methods: A multi-center retrospective cohort study of pregnant patients (n=370) was conducted in 22 tertiary hospitals in China. Adverse pregnancy outcomes and admission of neonate to neonatal intensive care unit (NICU) were considered as outcomes. The effect of antimicrobial exposure on pregnancy outcomes was assessed using a multivariate logistic regression model.Results: Use of first-generation cephalosporins during pregnancy was associated with a significantly higher risk of adverse pregnancy outcomes (odds ratio [OR]: 3.64 [95% confidence interval, CI: 1.43-9.24], P = 0.007) and admission of neonate to the NICU (OR: 3.41, 95% CI: 1.37-8.53, P=0.009) compared with use of third-generation cephalosporins, after adjusting for gestational age of exposure to antimicrobial agents, cesarean section, and antimicrobial dose. Similarly, a higher risk of adverse pregnancy outcomes (OR: 14.76, 95% CI: 4.43-49.11) and neonatal NICU admission (OR: 11.74, 95% CI: 3.59-38.35) were observed among women with first-generation cephalosporins use compared with mothers with no antimicrobial use.Conclusion: Both first-and third-generation cephalosporins use was associated with an increased risk of adverse pregnancy and neonatal outcomes. In addition, first-generation cephalosporins were associated with an increased risk of those pregnant and neonatal outcomes, when compared with third-generation cephalosporins. We should require to determine the indications and contraindications for use of cephalosporins during pregnancy.

Abstract:Objective To test the hypothesis that maternal drug use or treatment for infertility is related to the occurrence of infant craniostenosis. Design and Material Maternal drug use and infertility treatment were studied in 398 cases of craniostenosis, identified from various Swedish health registers. Exposure information was ascertained in early pregnancy, and comparisons after adjustment for some confounders were made with all infants born. In order to validate some findings, data from the Central-East France Registry were studied for first trimester drug exposure in 235 infants, and use of ovarian stimulation in 315 infants with craniostenosis. Results A statistically significant association between maternal use of anticonvulsants and infant craniostenosis was found (risk ratio [Swedish data], 6.9; 95% confidence interval, 1.10 to 7.94). With the Swedish data, an association was found with three nitrosatable drugs (risk ratio, 3.4; 95% confidence interval, 1.10 to 7.94), previously associated with the occurrence of craniostenosis, but this was based on only five exposures, and no such exposure occurred in the French data set. No association with subfertility (odds ratio, 0.72; 95% confidence interval, 0.60 to 1.86) or infertility treatment (odds ratio, 1.06; 95% confidence interval, 0.60 to 1.87) was found in the Swedish data and no statistically significant increase in the use of ovulation stimulation in the French data. Conclusions A strong association was found between the maternal use of anticonvulsants and infant craniostenosis, and a tentative association was found with the use of nitrofurantoin and two other nitrosatable drugs. There was no association with maternal subfertility or infertility treatment.

Abstract:BACKGROUND AND AIM: The neurodevelopment effects of triclosan (TCS) have been observed in animals and school-age children. However, epidemiologic studies are limited to confirm the associations for young children. We investigated the potential effect of prenatal TCS exposure on neurodevelopment of 2-year-old children. METHODS: From September 2010 to December 2013, 294 mother-infant pairs were enrolled from a prospective cohort study in Shandong, China. Maternal urine samples were collected for TCS measurement. Gesell Developmental Schedules (GDS) were used to assess children's neurodevelopment at 2 years of age. Multiple linear regression models and generalized linear models were used to analyze the association between prenatal TCS exposure and the children's developmental quotient scores (DQs). RESULTS:The median value of TCS concentration was 0.24ug/L or 0.67ug/g creatinine. When TCS were modeled as quartile variables, maternal TCS levels were adversely associated with children's DQs in gross motor domain (2ndquartile: β=-3.44, 95% CI: -6.39 to -4.89, p=0.02) and adaptive domain (3rdquartile: β=-3.11, 95% CI: -5.95 to -0.28, p=0.03). In gender-stratified analyses, maternal TCS levels were significantly adversely associated with the gross motor domain DQs only among girls (p-trend=0.03) and the adaptive domain DQs only among boys (p-trend=0.01). Additionally, a log-unit increase in TCS was related to decrease in language domain DQs among boys (β=-2.13, 95% CI: -4.06 to -0.19, p=0.03). CONCLUSIONS:The findings suggested the potential impacts of prenatal TCS exposure on neurodevelopment of 2-year-old children in gross motor domain, adaptive domain and language domain. Abstract:Screening for the common autosomal trisomies has been a cornerstone of obstetric care over the past 4 decades. 1,2 Although important, it is essential to recognize that aneuploidies represent only a small portion of the total fetal genetic and structural risks. Congenital malformations occur in 3–5% of births and are associated with genetic abnormalities and death. 3–5 The combination of cell-free DNA screening and early anatomic imaging has been demonstrated to have a higher detection rate for abnormalities in the latter part of the first trimester than either method alone. 6–10 Although 25–50% of structural malformations have an identifiable genetic cause, it is expected that the percentage attributable to a genetic cause will increase with our rapidly expanding use of next-generation sequencing. 11–17

Risk of congenital malformations and miscarriages following maternal use of oral fluconazole during the first trimester of pregnancy: a systematic review and meta-analysis

Abstract:The risks related to fluconazole use during the first trimester of pregnancy (T1) remain controversial. The aims of this systematic review and meta-analysis were to assess the association between oral fluconazole during T1 and major congenital malformations (MCM) overall and by subtype, minor malformations and miscarriages. We searched MEDLINE, EMBASE, Cochrane, ICTRP and ClinicalTrials.gov from inception to 02/12/24. Randomized controlled trials and observational studies were included. ROBINS-I was used for risk of bias assessment. Both fixed- and random-effects models meta-analyses were performed. GRADE was used to assess the certainty of the evidence. Among 1403 references, nine observational studies were included (3,764,897 pregnancies, including 116,425 exposed to fluconazole). The association between any fluconazole use during T1 and overall MCM was significant when combining crude estimates (ORc 1.18, 95%CI (1.08–1.29), I 2 23%, seven studies), but not when combining adjusted estimates (ORa 1.02, 95%CI (0.98–1.07), I 2 0%, six studies). Results were consistent for cumulative dose of fluconazole. In sensitivity analyses considering only studies with a valid definition of MCM, the association between fluconazole > 150 mg and overall MCM remained significant when combining adjusted estimates. For the subtypes of MCM (cardiac, genito-urinary, musculoskeletal) we found no significant association. A significant association was found between fluconazole use and miscarriages (ORa 1.60, 95% CI (1.06–2.42). Fluconazole use during T1 does not significantly increase the risk of MCM overall or by subtype when considering adjusted estimates. However, potential risks, particularly at cumulative doses greater than 150 mg which show a potential association with MCM, deserve much attention.
