Abstract:Background: The metabolic changes of malignant cells are earlier than these of morphology. F-18-fluorodeoxyglucose (FDG) single-photon emission computed tomography (SPECT)-computed tomography (CT) systems provide functional and anatomic images that could significantly improve its diagnostic capability. The molecular mechanisms of increased FDG uptake are still not fully understood. The correlation between FDG uptake and the expression of Glut in various tumor cells is still under debate. Further study is necessary to correlate increased FDG uptake by tumors with their Glut-1 and Glut-3 expression, which will lead to a better understanding and interpretation of SPECT-CT imaging. In this study, we, therefore, investigated in patients with head and neck carcinoma (HNC) the relationship between tumor FDG accumulation and the mRNA expression and protein expression of Glut-1 and Glut-3. Materials and Methods: Overall, 25 patients with HNC who underwent SPECT-CT imaging and CT or magnetic resonance imaging (MR1) studies were performed between April 2002 and March 2004. Then, the mRNA and protein expression of Glut-1 and Glut-3 in these 25 surgical or biopsied samples were studied with HNC to determine the correlate increased FDG uptake by tumors with their Glut-1 and Glut-3 expression. Results: Visual analysis of F-18-FDG SPECT-CT gave sensitivity, specificity, and accuracy levels of 100%, 62.5%, and 88%, respectively. The tumor-to-background (T/B) ratios were 94.1%, 87.5%, and 92.0% for F-18-FDG SPECT/CT, and 64.7%, 50.0%, and 60.0% for CT and MRI. This indicates that F-18-FDG SPECT-CT is superior to CT and MRI. Significant correlation was found between FDG and Glut-1 mRNA or Glut-1 protein (p < 0.001). There was no correlation between T/B ratio and Glut-3 mRNA (r = 0.14, p > 0.01). Conclusions: F-18-FDG SPECT-CT can be as a prospective toot that can judge the malignancy or benignity of head and neck tumor, stage and class h; the tumor, distinguish recurrence or necrosis or fibrosis of the tumor after treatment by surgery or radiotherapy, and detect unknown primary tumor. Glut-1 may largely mediate basal glucose transport in HNC cells.
Relationships Between Expression of Glucose Transporter Protein-1 and Hypoxia Inducible Factor-1Α, Prognosis and 18F-FDG Uptake in Laryngeal and Hypopharyngeal Carcinomas

Abstract:BACKGROUND:We investigated the relationships between glucose transporter protein-1 (GLUT-1) and hypoxia inducible factor-1α (HIF-1α) expression, 2-deoxy-2-[18F]-fluoro-D-glucose (18F-FDG) uptake and prognosis in laryngeal and hypopharyngeal carcinomas patients.METHODS:Levels of GLUT-1 and HIF-1α proteins were examined by immunohistochemistry in 55 patients with laryngeal and hypopharyngeal carcinomas. All patients underwent 18F-FDG positron emission tomography-computed tomography (PET/CT) before surgery.RESULTS:Levels of GLUT-1 and HIF-1α in laryngeal and hypopharyngeal carcinomas were significantly higher than in vocal cord polyps. GLUT-1 expression was correlated with HIF-1α expression (r=0.571, P<0.001) and SUVmax (r=0.495, P<0.001). HIF-1α expression was not correlated with SUVmax (P=0.199). HIF-1α expression was correlated with tumor size (r=0.351, P=0.009). Multivariate analysis demonstrated that treatment method (P=0.025), tumor size (P=0.008), GLUT-1 expression (P=0.032), and HIF-1α expression (P=0.032) were correlated with overall survival (OS).CONCLUSIONS:Treatment method, tumor size, GLUT-1 and HIF-1α levels were correlated with prognosis in laryngeal and hypopharyngeal carcinomas patients.

Abstract:This report presents a case of nasopharyngeal diffuse large B-cell lymphoma and a literature review concerning the use of [F-18]fluorodeoxyglucose (FDG) positron emission tomography/computed tomography (PET/CT). A 37-year old man was admitted to hospital complaining of nasal secretions with minor epistaxis and a 20-year history of snoring. Nasal endoscopy found diffuse swelling in the nasopharynx and a biopsy was performed. Prior to chemotherapy, FDG-PET/CT showed soft-tissue diffuse thickening and FDG accumulation in the nasopharynx and bilateral cervical lymph nodes; FDG did not accumulate elsewhere. After four cycles of chemotherapy (rituximab, cyclophosphamide, doxorubicin, vincristine) and prednisone treatment, FDG-PET/CT showed that FDG still accumulated in the nasopharynx and bilateral cervical lymph nodes, therefore radiotherapy was initiated. At 1 year, FDG-PET/CT showed no FDG accumulation. Immunohistochemical analysis revealed that the tumour was positive for phosphorylated protein kinase B (Akt), suggesting that FDG uptake may be associated with factors activated by the phosphatidylinositol 3-kinase/Akt signalling pathway.
[Value of 18F-FDG Metabolic Imaging in Diagnosis and Treatment of Head and Neck Tumors and Its Mechanism Study].

Abstract:OBJECTIVE:To study the value of (18)F-FDG dual-head tomography with coincidence (DHTC) and single photon emission computerized tomography (SPECT) coincidence imaging in diagnosis and treatment of head and neck tumors and mechanism thereof and analyze the value of glucose transporter proteins in the mechanism of increased uptake glucose of head and neck malignant tumor.METHODS:Twenty-five patients with head and neck tumors were examined by CT or MRI and underwent (18)F-FDG DHTC and coincidence imaging. The results of these 2 different methods were compared. Fresh tissues of 38 patients with malignant tumors of the head-and-neck underwent RT-PCR and immunohistochemical examination.RESULTS:The sensitivity, specificity, and accuracy of (18)F-FDG coincidence imaging and registration with integrated CT (SPECT/CT) in diagnosis of the head and neck tumors was 100.0%, 87.5%, and 96.0% respectively, all significantly higher than those of the anatomical imaging (64.7%, 50.0%, and 60.0% respectively, all P < 0.05). For the lesions on the same site, SPECT/CT could diagnose exactly the primary tumor site of neck metastasis in four cases and diagnose the malignancy or benignancy of other four cases that anatomical imaging (CT/MRI) could not diagnose exactly. (18)F-FDG coincidence imaging and registration with integrated CT could find extra lesions of tumors. The results of RT-PCR and immunohistochemistry showed that the mRNA expression and protein expression of Glut-1 and Glut-3 were higher in the head and neck cancer than that in the normal tissue of head and neck or in the adjacent tissue (all P < 0.05).CONCLUSION:(18)F-FDG coincidence imaging and registration with integrated CT can be as a prospective tool that can judge the malignancy or benignancy of head and neck tumor, and stage and classify the tumor, and distinguish recurrence or necrosis of tumor after treatment by surgery or radiotherapy, and detect unknown primary tumor. The abnormal expressions of Glut-1 and Glut-3 may be correlated with the increased uptake of glucose of head and head cancer.

Abstract:Background: Positron emission tomography (PET) with 18F-2-fluoro-2-deoxy-D-glucose (FDG) is widely applied to the study of gliomas. The histology of most gliomas is regionally heterogeneous. The relationship between histologic features and glucose metabolism evaluated by PET with FDG may therefore vary within the limits of the tumor. PET with FDG integrated in the planning of stereotactic brain biopsy allows precise comparison between local FDG uptake and histology. Using this approach, the authors investigated whether glucose metabolism of gliomas is related to anaplasia, and whether PET with FDG detects metabolic heterogeneity that parallels histologic heterogeneity of gliomas. Methods: A total of 161 biopsy samples collected from 20 PET-guided procedures performed in patients with gliomas (8 low grade astrocytomas, 8 anaplastic astrocytomas, 1 anaplastic oligoastrocytoma, and 3 glioblastomas) were analyzed for the presence or absence of 8 histologic features. Stereotactic coordinates were used to calculate the metabolic rate of glucose (MRGlu) in the region of each biopsy sample. Gray and white matter MRGlu were used to define four metabolic grades that were compared with local histology. Results: The difference in MRGlu expressed as micromoles per 100 g per minute was highly significant between anaplastic and nonanaplastic samples; the median +/- quartile deviation was 23 +/- 16 in anaplastic samples and 18 +/- 5 in nonanaplastic samples (P < 0.005). Even more significant differences were found when MRGlu was normalized to the cortex or to the white matter. Metabolic grades were different in anaplastic and nonanaplastic samples (P < 0.0001). Approximately 75% of samples metabolically graded 3 or 4 demonstrated signs of anaplasia, compared with 10% of samples graded 0 or 1. Conclusions: FDG uptake in gliomas is anatomically heterogeneous and is regionally related to the presence of anaplasia.
Correlation Between FDG Uptake and Hexokinase-Ii Expression in Untreated Head and Neck Carcinoma

Abstract:1736 Objectives: One of the key factors of glucose catabolism in the cell is thought to be the level of hexokinase expression. This study was to investigate the relationship between HK-II expression and 18F-fluoro-2-deoxy-D-glucose (FDG) uptake in human untreated head and neck carcinoma. Methods: Pre-operatively FDG positron emission tomography (PET) was performed 60 min after FDG injection in all the 36 patients. The tumor FDG uptake was evaluated by using maximum standardized uptake value (SUV). Tumor sections were stained immunohistochemically for HK-II. Results: All the tumor sections stained positive for HK-II. Thirty-four (94%) tumors in HK-II showed immunostained positive area >50%. HK-II findings revealed twenty-one (58%) tumors with strong intensity, twelve (33%) with moderate intensity and four with weak intensity (10%). There was no statistically significant correlation between SUV and the expression of HK-II (p = 0.50). Conclusions: In conclusion, head and neck carcinoma showed increased FDG accumulation and overexpression of HK-II. No significant relationship was found between high FDG uptake and overexpression of HK-II in this patient population, and thus other properties need to be evaluated in order to elucidate key factors responsible for FDG activity in head and neck carcinoma.
Correlation Between 18F-FDG Uptake and Immune Cell Infiltration in Metastatic Brain Lesions

Abstract:Background: The purpose of this study was to investigate the correlation between 18F-fluorodeoxyglucose (FDG) uptake and infiltrating immune cells in metastatic brain lesions. Methods: This retrospective study included 34 patients with metastatic brain lesions who underwent brain 18F-FDG positron emission tomography (PET)/computed tomography (CT) followed by surgery. 18F-FDG uptake ratio was calculated by dividing the standardized uptake value (SUV) of the metastatic brain lesion by the contralateral normal white matter uptake value. We investigated the clinicopathological characteristics of the patients and analyzed the correlation between 18F-FDG uptake and infiltration of various immune cells. In addition, we evaluated immune-expression levels of glucose transporter 1 (GLUT1), hexokinase 2 (HK2), and Ki-67 in metastatic brain lesions. Results: The degree of 18F-FDG uptake of metastatic brain lesions was not significantly correlated with clinical parameters. There was no significant relationship between the 18F-FDG uptake and degree of immune cell infiltration in brain metastasis. Furthermore, other markers, such as GLUT1, HK2, and Ki-67, were not correlated with degree of 18F-FDG uptake. In metastatic brain lesions that originated from breast cancer, a higher degree of 18F-FDG uptake was observed in those with high expression of CD68. Conclusions: In metastatic brain lesions, the degree of 18F-FDG uptake was not significantly associated with infiltration of immune cells. The 18F-FDG uptake of metastatic brain lesions from breast cancer, however, might be associated with macrophage activity.
Expression of Glut-1 and HK-II in Pancreatic Cancer and Their Impact on Prognosis and FDG Accumulation

Abstract:OBJECTIVE: The purpose of this article is to analyze the expression of Glut-1 and HK-II, the association between their expression and 18F-FDG accumulation in pancreatic cancer. METHODS: Fifty patients with histologically proven pancreatic cancer were included in this preliminary study, all of whom received 18F-FDG PET/CT performance before surgery. Immunohistochemical staining of tumor tissue and adjacent normal tissue was performed for Glut-1 and HK-II. By combining proportions and intensity of immunochemical staining, we obtained the modified immunohistological scores for Glut-1 and HK-II respectively. The relationship between expression of Glut-1, HK-II and series of parameters was analyzed, i.e. clinicopathological characteristics, prognosis of patients and SUVmax of PET-CT. RESULTS: Compared with normal tissue, the Glut-1 and HK-II expression in pancreatic cancer tissue was significantly increased (P<.001). There was no correlation between expression of Glut-1, HK-II and age, gender, tumor size, tumor location, tumor histological type, tumor differentiation, the nerve infiltration, vascular invasion, local infiltration, lymph node metastasis or tumor staging in pancreatic cancer (P>.05). During the follow-up period, the survival curves of low Glut-1 group and high Glut-1 group were statistically different (P=.049). Multivariate analysis (Cox regression) revealed that Glut-1 expression was not associated with mortality (P>.05). No statistical difference was found in the survival curves of negative HK-II group and positive HK-II group (P=.545). There was no correlation between 18F-FDG uptake and expression of Glut-1 and HK-II(P>.05). CONCLUSION: The Glut-1 and HK-II expression in pancreatic cancer tissue was significantly increased. There was no correlation between expression of Glut-1, HK-II and clinicopathological characteristics, prognosis and 18F-FDG uptake.
Correlation of metabolism/hypoxia markers and fluorodeoxyglucose uptake in oral squamous cell carcinomas

Abstract:Objective: The objective of this study was to analyze the relationship between the uptake of (18)F-2-fluoro-2-deoxy-d-glucose (FDG) by positron emission tomography-computerized tomography (PET-CT) and glucose metabolism/hypoxia markers in oral squamous cell carcinoma (OSCC). Study design: Thirty-six patients with OSCC (tongue [n = 23], buccal mucosa [n = 7], and floor of the mouth [n = 6]) were assessed and underwent incisional biopsy and subsequently received FDG-PET-CT. Expressions of hypoxia-inducible factor 1α (HIF-1α), glucose transporter protein 1 (GLUT-1), hexokinase-II (HK-II), and glucose-6-phosphatase (G6Pase) were immunohistochemically quantified, and FDG uptake was evaluated by the maximum standardized uptake values (SUV(max)) at the primary tumor site. Results: FDG uptake was found to be significantly correlated with the T classification of OSCC but not with other clinicopathologic characteristics, such as the N classification, clinical type, and histologic grade of malignancy. In the early-stage (T1 and T2) tumor, FDG uptake was significantly associated with the expression levels of GLUT-1, HK II, and HIF-1α, and the expression levels of GLUT-1 and HK-II significantly correlated with HIF-1α expression levels. However, there were no correlations between the expression levels of these molecules and SUV(max) in the late-stage (T3 and T4) tumor. Conclusions: FDG uptake was significantly associated with the expression levels of glucose metabolism-related molecules, such as GLUT-1, HK II, and HIF-1α, especially in early-stage tumors.
Comparison of Cell Proliferation, Protein, and Glucose Metabolism in Musculoskeletal Tumors in a PET Study.

Abstract:11C-choline and18F-FAMT are known to correlate with tumor cell proliferation and amino acid metabolism. We investigated the ability of11C-Choline and18F-FAMT PET in diagnosis of musculoskeletal tumors in thirty-six patients in comparison of18F-FDG PET.11C-Choline and18F-FDG PET were positive in all the malignant tumors (n=13), whereas18F-FAMT was positive in 11 tumors. The mean SUVs for malignant tumors were significantly higher than those for benign lesions in all three tracers imaging. A moderate correlation was found between11C-Choline and18F-FDG (r=0.540,P<.05), or18F-FAMT and FDG (r=0.596,P<.05). The diagnostic sensitivity and specificity for malignancy were 91.7% and 71.4%, respectively, using11C-choline with a SUV cut-off of 2.69. The sensitivity and specificity of18F-FAMT for malignancy were 66.7% and 85.7%, respectively, using a SUV cut-off of 1.26. For18F-FDG, using a SUV cut-off of 2.77, the sensitivity and specificity were 83.3% and 71.4%, respectively. According to ROC analysis, the ROC curves for11C-Choline,18F-FAMT, and18F-FDG were 0.855, 0.734, and 0.847, respectively.11C-Choline PET is superior in the visualization of musculoskeletal tumors with high contrast imaging, whereas the combination of18F-FAMT and18F-FDG PET provides valuable information for the preoperative planning in patients with musculoskeletal tumors.

Abstract:Objectives 18 F-fluorodeoxyglucose positron emission tomography-computed tomography ( 18 F-FDG-PET/CT) is a diagnostic imaging method that is based on the Warburg effect, which is the increased uptake of glucose through aerobic glycolysis in cancer cells. The diagnostic value of 18 F-FDG-PET/CT for thyroid cancer is controversial. However, uptake of 18 F-FDG and the corresponding maximum standardized uptake value (SUVmax) is expected to reflect the metabolic status of cancer cells. In the present study, we sought to determine the relationship between 18 F-FDG uptake and tumor metabolism- associated factors. Methods This was a single-center retrospective study. In the present study, SUVmax was compared with the expression of hexokinase 2 (HK2), glucose transporter 1 (GLUT1), vascular endothelial growth factor (VEGF), and glutaminase 1 (GLS1) in 41 patients with thyroid cancer. Results GLS1 expression was found to be moderately correlated with SUVmax ( p < 0.001, r = 0.51), whereas HK2 and VEGF expression were weakly correlated ( p = 0.011, r = 0.28, p = 0.008, r = 0.29, respectively) and GLUT1 did not correlate with SUVmax ( p = 0.62, r = 0.06). Conclusion Our findings suggest 18 F-FDG PET/CT reflects GLS1 expression in thyroid cancer and could be used to select suitable candidates for GLS1 inhibitor treatment.

Identifying G6PC3 as a Potential Key Molecule in Hypoxic Glucose Metabolism of Glioblastoma Derived from the Depiction of 18F-Fluoromisonidazole and 18F-Fluorodeoxyglucose Positron Emission Tomography

Abstract:Purpose. Glioblastoma is the most aggressive primary brain tumor, characterized by its distinctive intratumoral hypoxia. Sequential preoperative examinations using fluorine-18-fluoromisonidazole (18F-FMISO) and fluorine-18-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) could depict the degree of glucose metabolism with hypoxic condition. However, molecular mechanism of glucose metabolism under hypoxia in glioblastoma has been unclear. The aim of this study was to identify the key molecules of hypoxic glucose metabolism. Methods. Using surgically obtained specimens, gene expressions associated with glucose metabolism were analyzed in patients with glioblastoma ( n=33 ) who underwent preoperative 18F-FMISO and 18F-FDG PET to identify affected molecules according to hypoxic condition. Tumor in vivo metabolic activities were semiquantitatively evaluated by lesion-normal tissue ratio (LNR). Protein expression was confirmed by immunofluorescence staining. To evaluate prognostic value, relationship between gene expression and overall survival was explored in another independent nonoverlapping clinical cohort ( n=17 ) and validated by The Cancer Genome Atlas (TCGA) database ( n=167 ). Results. Among the genes involving glucose metabolic pathway, mRNA expression of glucose-6-phosphatase 3 (G6PC3) correlated with 18F-FDG LNR ( P=0.03 ). In addition, G6PC3 mRNA expression in 18F-FMISO high-accumulated glioblastomas was significantly higher than that in 18F-FMISO low-accumulated glioblastomas ( P<0.01 ). Protein expression of G6PC3 was consistent with mRNA expression, which was confirmed by immunofluorescence analysis. These findings indicated that the G6PC3 expression might be facilitated by hypoxic condition in glioblastomas. Next, we investigated the clinical relevance of G6PC3 in terms of prognosis. Among the glioblastoma patients who received gross total resection, mRNA expressions of G6PC3 in the patients with poor prognosis (less than 1-year survival) were significantly higher than that in the patients who survive more than 3 years. Moreover, high mRNA expression of G6PC3 was associated with poor overall survival in glioblastoma, as validated by TCGA database. Conclusion. G6PC3 was affluently expressed in glioblastoma tissues with coincidentally high 18F-FDG and 18F-FMISO accumulation. Further, it might work as a prognostic biomarker of glioblastoma. Therefore, G6PC3 is a potential key molecule of glucose metabolism under hypoxia in glioblastoma.

Abstract:Abstract Background Non-invasive diagnosis of IDH1 mutation for gliomas has great clinical significance, and PET has natural advantage to detect metabolism, as IDH mutated gliomas share lower glucose consumption. Methods Clinical data of patients with gliomas and 18 F-FDG PET were retrospectively reviewed. Receiver operating characteristic curve (ROC) analysis was conducted, and standard uptake value (SUV) was estimated in combination with grades or IDH1 mutation. The glucose consumption was investigated with U251 cells expressing wild-type or mutated IDH1 by glucose assay. Quantification of glucose was determined by HPLC in clinical tissues. Meanwhile, bioinformatics and western blot were applied to analyze the expression level of metabolic enzymes (e.g. HK1, PKM2, PC) in gliomas. Results Seventy-one glioma cases were enrolled, including 30 carrying IDH1 mutation. The sensitivity and specificity dependent on SUV max (3.85) predicting IDH1 mutation reached 73.2 and 86.7%, respectively. The sensitivity and specificity of differentiating grades by SUVmax (3.1) were 92.3 and 64.4%, respectively. Glucose consumption of U251 IDH1 mutant cells (0.209 ± 0.0472 mg/ml) was obviously lower than IDH1wild-type cells (0.978 ± 0.0773 mg/ml, P = 0.0001 ) and astrocyte controls (0.335 ± 0.0592 mg/ml, P = 0.0451 ). Meanwhile, the glucose quantity in IDH1mutant glioma samples were significantly lower than those in IDH1 wild-type tissues (1.033 ± 1.19608 vs 6.361 ± 4.3909 mg/g, P = 0.0051 ). Silico analysis and western blot confirmed that HK1 and PKM2 in IDH1 wild-type gliomas were significantly higher than in IDH1 mutant group, while PC was significantly higher in IDH1 mutant gliomas. Conclusion SUV max on PET can predict IDH1 mutation with adequate sensitivity and specificity, as is supported by reduced glucose consumption in IDH1 mutant gliomas.

Correlation of Glucose Transporter 1(GLUT1) Increased Expression with FDG Uptaking in Human Lung Cancer

Abstract:Objectve To evaluate the relationship between the increased expression of GLUT1 and FDG uptaking in human lung cancer. Methods The glucose analog FDG uptaking was detected by FDG-SPECT with coincidence in 30 patients with lung cancer preoperatively,whose interpretation was based on visual qualitative analysis of images.Ratio of FDG amount activity of tumor over normal lung(T/N)assessed visually was used as FDG uptaking.The expression of GLUT1 was studied in the corresponding lung cancer tissues by immunohistochemistry.Results were semiquantitively analysed using Imaging Analysis and Treatment System.Results The expression of GLUT1 in all 30 lung cancer tissues was positive.There was one false negative case in FDG-SPECT imagings,compared with postoperative pathologic results.Correlation between GLUT1 expression and FDG uptake(T/N)was significant with r=0.8 (P<0.01).Conclusion These results suggest that GLUT1 plays a singnificant role in the glucose metablism of lung cancer tissues and its overexpression may contribute to the increased uptaking of glucose in pa- tients with lung cancer.

Correlation of 18F-FDG PET/CT metabolic parameters with the expression of immune biomarkers in the tumour microenvironment in lung adenocarcinoma

Abstract:Aim: To explore the association between metabolic parameters evaluated by integrated 2-[18F]-fluoro-2-deoxy-d-glucose (FDG) positron-emission tomography (PET)/computed tomography (CT) and the expression of immune biomarkers in the tumour microenvironment in lung adenocarcinoma. Materials and methods: This study included 134 patients. Metabolic parameters were obtained by PET/CT. Immunohistochemistry analysis was used for FOXP3-TILs (transcription factor forkhead box protein 3 tumour-infiltrating lymphocytes), CD8-TILs, CD4-TILs, CD68-TAMs (tumour-associated macrophages) and galectin-1 (Gal-1) tumour expression. Results: There were significant positive associations between FDG PET metabolic parameters and the median percentage of immune reactive areas (IRA%) covered by FOXP3-TILs and CD68-TAMs. Negative associations with the median IRA% covered by CD4-TILs and CD8-TILs were observed: maximal standardised uptake value (SUVmax), metabolic tumour volume (MTV), total lesion glycolysis (TLG), and IRA% for FOXP3-TILs (rho = 0.437, 0.400, 0.414; p<0.0001 for all parameters); SUVmax, MTV, TLG, and IRA% for CD68-TAMs (rho = 0.356, 0.355, 0.354; p<0.0001 for all parameters); SUVmax, MTV, TLG, and IRA% for CD4-TILs (rho = -0.164, -0.190, -0.191; p=0.059, 0.028, 0.027, respectively); SUVmax, MTV, TLG, and IRA% for CD8-TILs (rho = -0.305, -0.316, -0.322; p<0.0001 for all parameters). There were significant positive associations between tumour Gal-1 expression and the median IRA% covered by FOXP3-TILs and CD68-TAMs (rho = 0.379; p<0.0001; rho = 0.370; p<0.0001, respectively), and a significant negative association with the median IRA% covered by CD8-TILs (rho = -0.347; p<0.0001) was observed. Tumour stage (p=0.008), Gal-1 expression (p=0.008), and median IRA% covered by CD8-TILs (p=0.054) were independent risk factors for overall survival. Conclusion: FDG PET may facilitate a comprehensive evaluation of the tumour microenvironment and predict response to immunotherapy.

Abstract:Background: The present study aimed to explore the efficacy of easily obtained intratumoral heterogeneous parameters, other than regular semi-quantitative parameters, based on static O-(2-[F-18] fluoroethyl)-l-tyrosine (F-18-FET) positron emission tomography (PET) imaging in glioma grade and isocitrate dehydrogenase (IDH) gene 1 mutation prediction. Methods: Fifty-eight adult patients with untreated glioma (grades II-IV) who underwent preoperative F-18-FET PET/computed tomography (CT) imaging were enrolled in the present study. Eight semiautomatically obtained static PET imaging parameters after lesion delineation were chosen for analysis. These were: maximal tumor-to-background ratio (TBRmax), peak tumor-to-background ratio (TBRpeak), mean tumor-to-background ratio (TBRmean), coefficient of variation (COV), heterogeneity index (HI), the standard deviation of lesion standardized uptake value (SUVsd), metabolic tumor volume (MTV), and total lesion tracer standardized uptake (TLU). Pathological and immunohistochemical results were used as a reference. The receiver-operating characteristic analysis was used to investigate the predictive efficacy of these parameters in glioma grade and IDH1 mutation status. Results: TLU [area under the curve (AUC): 0.841, P<0.0001], TBRpeak (AUC: 0.832, P<0.0001), and HI (AUC: 0.826, P<0.0001) had the top 3 single-parameter predictive performance between grade II or III and grade IV glioma patients. Combinations of TBRmax, SUVsd, and TBRmean (AUC: 0.850, P<0.0001); HI, SUVsd, and MTV (AUC: 0.848, P<0.0001); and HI, SUVsd, and TLU (AUC: 0.848, P<0.0001) had the top 3 multiple-parameter predictive performance. SUVsd (AUC: 0.710, P=0.0028), TLU (AUC: 0.698, P=0.0074), and HI (AUC: 0.676, P=0.0159) had the top 3 single-parameter predictive performance in the IDH1 genotype. Combinations of TBRmax, SUVsd, and TBRmean (AUC: 0.821, P<0.0001); SUVsd and TBRmean (AUC: 0.804, P<0.0001); and SUVsd, HI, and TBRmean (AUC: 0.799, P<0.0001) had the top 3 multiple-parameter predictive performance. Conclusions: These easily obtained and highly repetitive heterogeneous parameters based on static F-18-FET PET/CT imaging can non-invasively predict glioma grade and IDH1 mutation, crucial in treatment planning, and prognostic evaluation.

Abstract:ObjectivesTo investigate the effects of O6-methylguanine DNA methyltransferase (MGMT) promoter methylation status of gliomas on O-(2-18F-fluoroethyl)-L-tyrosine ([18F]FET) uptake and cerebral blood flow (CBF) of arterial spin labeling (ASL), evaluated by hybrid PET/MR. Stereotactic biopsy was used to validate the findings.MethodsA set of whole tumor and reference volumes of interest (VOIs) based on PET/FLAIR imaging were delineated and transferred to the corresponding [18F]FET PET and CBF maps in 57 patients with newly diagnosed gliomas. The mean and max tumor-to-brain ratio (TBR) and normalized CBF (nCBF) were calculated. The predictive efficacy of [18F]FET PET and CBF in determining MGMT promoter methylation status of glioma were evaluated by whole tumor analysis and stereotactic biopsy. The correlation between PET/MR parameters and MGMT promoter methylation were analyzed using histological specimens acquired from multiple stereotactic biopsies.ResultsBased on the analysis of whole tumor volume and biopsy site, TBRmean, TBRmax, nCBFmean, and nCBFmax showed no statistically significant differences between gliomas with and without MGMT promoter methylation (all p > 0.05). Furthermore, stereotactic biopsy demonstrated that TBRmean, TBRmax, nCBFmean, and nCBFmax showed no correlation with MGMT promoter methylation (r = −0.117, p = 0.579; r = −0.161, p = 0.443; r = −0.271, p = 0.191; r = −0.300, p = 0.145; respectively).ConclusionsMGMT promoter methylation status shows no effect on [18F]FET uptake and CBF of ASL in gliomas. Stereotactic biopsy validates it and further reveals there is no correlation of [18F]FET PET uptake and CBF with the percentages of MGMT promoter methylation.Key Points• Based on whole tumor VOI assessment, MGMT promoter methylation status shows no effect on[18F]FET uptake and CBF of ASL in gliomas.• For WHO grade IV glioblastomas,[18F]FET PET and ASL parameters based on hybrid PET/MR fail to predict the MGMT promoter methylation status.• Stereotactic image–based histology reveals that there is no correlation of[18F]FET PET uptake and CBF with the status and percentages of MGMT promoter methylation in gliomas.

Abstract:AbstractOur purpose was to evaluate the effect of glycemia on 18F-FDG uptake in normal organs of interest. The influences of other confounding factors, such as body mass index (BMI), diabetes, age, and sex, on the relationships between glycemia and organ-specific standardized uptake values (SUVs) were also investigated. We retrospectively identified 5623 consecutive patients who had undergone clinical PET/CT for oncological indications. Patients were stratified into groups based on glucose levels, measured immediately before 18F-FDG injection. Differences in mean SUVmax values among glycemic ranges were clinically significant only when >10% variation was observed. The brain was the only organ that presented a significant inverse relationship between SUVmax and glycemia (p < 0.001), even after controlling for diabetic status. No such difference was observed for the liver or lung. After adjustment for sex, age, and BMI, the association of glycemia with SUVmax was significant for the brain and liver, but not for the lung. In conclusion, the brain was the only organ analyzed showing a clinically significant relationship to glycemia after adjustment for potentially confounding variables. The lung was least affected by the variables in our model, and may serve as an alternative background tissue to the liver.

Abstract:✓ The aim of this study was to determine whether positron emission tomography (PET) with fluorine-18 ( 18 F)-2-deoxyglucose (FDG) can be used as a prognostic test in patients with high-grade cerebral gliomas, regardless of the treatment given. Forty-five patients with astrocytoma Grade III or IV were included in this analysis. The mean survival time of patients with tumors exhibiting high glucose utilization as determined by PET-FDG was 5 months, whereas patients with gliomas showing lower glucose utilization had a mean survival period of 19 months. It is postulated that PET-FDG scans reflect the biological behavior of high-grade astrocytomas and may be used to predict the survival time of patients harboring such neoplasms.
