Abstract:The present study is aimed at evaluating the efficacy and safety of long-acting risperidone on early onset schizophrenia in adolescent patients. A total of 31 adolescent patients (13 - 18 years) with schizophrenia met the DSM-IV-TR criteria for schizophrenia and their symptoms were stable when orally taking risperidone or olanzapine. They were admitted into a 24 week, open-label study on the long-acting risperidone. Risperidone was administered every 2 weeks at a dose of 25, 37.5 and up to a maximum dose of 50 mg. The Positive and Negative Syndrome Scale (PANSS), the Clinical Global Impressions (CGI) and the Extrapyramidal symptom rating scale (ESRS) were used to assess the improvement in the symptoms. Improvements in the symptoms of schizophrenia occurred in patients treated with long-acting risperidone at week 6 and continued throughout the study with significant reduction in total PANSS score at week 24 (-4.2 ± 0.2, P < 0.01). At the same time, 51.6% of patients were rated as clinical improvement at the end of study. Among these 31 cases, the most frequently reported adverse events were depression (12.9%), anxiety (9.7%), headache (9.7%) and insomnia (6.4%). ESRS scores were reduced during the treatment with long-acting risperidone. The mean decrease in serum prolactin and body weight was 13.1 ng/ml and 4.5 kg, respectively. Intramuscular administration of long-acting risperidone was safe and well tolerated in adolescent patients. Long-acting risperidone also could improve the symptoms of schizophrenia in adolescent patients.

Abstract:Paliperidone, a benzisoxazole derivative, is the major active metabolite of risperidone. Like risperidone and other atypical antipsychotics, paliperidone possesses a higher affinity for blocking serotonin receptor type 2A(5-HT 2A) and dopamine receptor type 2(DA2). Paliperidone extended-release tablet (paliperidone ER) employs the osmotic-controlled release oral delivery system (OROS); it results in relatively smooth diurnal plasma paliperidone concentrations with low peak-to-trough fluctuations at steady state, allows for the avoidance of dosage adjustment when initiating therapy, and may obtain steady therapeutic effect and less side effects. Paliperidone differs from risperidone and most other antipsychotics as the extent of its enzymatic metabolism is relatively low. Renal excretion is the major route of its elimination. Clinical trials have demonstrated that paliperidone ER (3-15 mg·d-1) is effective in reducing the positive and negative symptoms of schizophrenia, improving cognitive impairment, and increasing the tolerability and compliance as compared with placebo. Compared with placebo, paliperidone ER can significantly improve clinical symptoms in patients previously treated with risperidone. Moreover, paliperidone appears to have a few adverse effects, except for an increased rate of hyperprolactinemia. Therefore, paliperidone ER may provide a valuable new treatment option for patients with schizophrenia.
A Randomized, 13-Week Study Assessing the Efficacy and Metabolic Effects of Paliperidone Palmitate Injection and Olanzapine in First-Episode Schizophrenia Patients

Abstract:Background: This study was conducted to evaluate the efficacy and metabolic effects of paliperidone palmitate (PP) injections against oral olanzapine in first-episode schizophrenia (FES) patients. Methods: Eligible patients were randomized to receive PP or olanzapine. Efficacy assessments and weight-related parameters were assessed at baseline, weeks 1, 5, 9, and endpoint or at early withdrawal. Lipid, glucose, insulin and prolactin were evaluated at baseline and endpoint or at early withdrawal. Results: The Positive And Negative Syndrome Scale (PANSS) scores declined significantly after treatment in both groups. Significant increases in weight-related parameters from baseline to endpoint were shown in both groups. Although there was no significant difference in PANSS scores and weight-related parameters between the two groups through the whole 13-week study. The increased level of triglyceride and HOMA-IR at endpoint from baseline in the olanzapine group was higher than the PP group. There was a stronger elevation of prolactin level in the PP group. Conclusions: In summary, PP and olanzapine showed similar improvement in the treatment of FES patients. This study also reinforced the necessity for regular monitoring of metabolic parameters in schizophrenia patients prescribed atypical antipsychotics.
Clinical study on the efficacy of paliperidone extended-release tablets in the treatment of schizophrenia outpatients and its effect on social function

Abstract:Objective: To investigate the efficacy and safety of paliperidone extended-release tablets(paliperidone ER) in treating schizophrenia outpatients.Methods: Schizophrenia patients were randomly divided into paliperidone ER group(n=76) and risperidone group(n=79) and treated with paliperidone ER or risperidone respectively for 4 weeks.The efficacy,side effects and social function were assessed with PANSS,TESS and PSP at the baseline and week 4 respectively.Results: At the end of 4 week,the scores of PANSS in paliperidone ER group was lower than baseline,and the total scores of PANSS and negative symptoms score of PANSS in were lower than that in risperidone group(P<0.05).At the end of 4 weeks,paliperidone ER group showed more significant improvement of PSP than risperidone group(P<0.05).There had no severe side-effects in both groups.Conclusion: Paliperidone ER shows significant improvement in positive and negative symptoms and social function in schizophrenia patients.It also has good safety.

Abstract:Objective Paliperidone is the active metabolite of risperidone. This single-center, double-blind, randomized, single-dose study characterized the pharmacokinetics of 3mg and 9mg of paliperidone ER OROS (R) in healthy Chinese subjects.Methods 24 subjects (13 male, 11 female), aged 19-35 years, with a BMI of 19.0-24.6 kg/m(2) participated. Blood samples were collected immediately before and over 96 h following single oral doses of 3mg and 9mg paliperidone. Plasma paliperidone concentrations were determined, and pharmacokinetic parameters were analyzed.Results Paliperidone's disposition after oral administration was characterized by a one-compartment pharmacokinetic model. Paliperidone was well absorbed (median t(max): 24 h after a 3-mg dose, and 26 h after a 9-mg dose). Apparent clearance and apparent volume of distribution were not significantly different between the two doses. C-max, AUC(0-t), and AUC(0-infinity) were dose-dependent. Pharmacokinetics was linear with respect to time; Geometric mean t(1/2) was 22.8 h and 21.4 h in 3-mg and 9-mg groups, respectively. No clinically significant safety issues were identified.Conclusions The pharmacokinetic results obtained in Chinese subjects were similar to those obtained in Japanese and Caucasian subjects. Abstract:Objective The study was designed to evaluate the efficacy and safety of flexible doses of paliperidone extended-release tablets (paliperidone ER) (3 -12) mg/d comparing with olanzapine (5 -15)mg/d in acute hospitalized patients with schizophrenia. Methods All 288 hospitalized patients with DSM-Ⅳ schizophrenia were randomized into paliperidone ER ( n = 143 ) or olanzapine ( n=145 ) treatment in a 6-week, multicenter, double-blind, parallel-group study. The primary efficacy measure was the total score changes of the Positive and Negative Syndrome Scale (PANSS). Clinical Global Impression (CGI),response rate and Visual Analogue Scale (VAS) were adopted as secondary efficacy measures. Results Both paliperidone ER and olanzapine groups demonstrated a significant improvement in total PANSS score (P<0.001). The PANSS total score in paliperidone ER group was reduced (32.3 ± 17.1) at end point,and olanzapine group (34.1 ± 17.4). There was no statistically significant difference between the two groups (P =0.369) after 6-week treatment. There were no statistical differences between two groups in CGI,response rate and VAS sleep quality assessments by the end of the treatment. The common adverse events were extrapyramidal symptoms, insomnia, constipation and prolactin increasing in paliperidone ER group,and somnolenee, EPS, abnormal liver function and abnormal lipid metabolism in olanzapine group.Conclusion Paliperidone ER and olanzapine are similarly effective in significantly improving the symptoms of inpatient with acute schizophrenia. Paliperidone ER demonstrates a favorable safety profile with fewer somnolence, abnormal liver function and abnormal lipid metabolism comparing with olanzapine.

Abstract:Background: Risperidone ISM® is a newly developed long-acting injectable (LAI) treatment for schizophrenia in adults. In the absence of head-to-head comparisons with other similar antipsychotics, the objective of this study was to generate indirect evidence of some aspects of the safety and tolerability of Risperidone ISM compared to other LAI antipsychotics for treatment of patients with schizophrenia in the maintenance treatment setting. Methods: A literature review was conducted systematically to identify maintenance treatment studies reporting safety and tolerability outcomes for LAI antipsychotic therapies. Following an assessment of between-trial heterogeneity, a matching-adjusted indirect comparison (MAIC) was performed to account for between-trial imbalances in patient characteristics and to generate comparative evidence for safety and tolerability endpoints. Results: The analysis showed that incidence of extrapyramidal symptoms (EPS) was found to be numerically, but not statistically significantly, lower in patients receiving Risperidone ISM than in those receiving Paliperidone palmitate (PP) (OR [95% CI] 0.63 [0.29, 1.38], p = 0.253) and statistically significantly lower than with Aripiprazole monohydrate once-monthly (AOM) (OR [95% CI] 0.25 [0.12, 0.53], p < 0.001). Use of anticholinergic agents for the alleviation of EPS was also shown to be significantly lower in Risperidone ISM patients than in those receiving PP (OR [95% CI] 0.29 [0.10, 0.83], p = 0.021) or AOM (OR [95% CI] 0.01 [0.003, 0.06], p < 0.001), suggesting a superior tolerability profile for clinically relevant EPS. Results from the sensitivity analyses comparing stabilized and stable patients receiving Risperidone ISM to those receiving AOM yielded similarly favorable conclusions in line with the base case analyses. Conclusions: This MAIC is consistent with the safety and tolerability results obtained during the PRISMA-3 clinical trial in the long-term treatment of schizophrenia and suggests a favorable safety and tolerability profile in terms of EPS incidence and anticholinergic agent use, relative to other antipsychotic therapies used for treatment of patients with schizophrenia in the maintenance setting.

Abstract:BACKGROUND:Paliperidone (9-hydroxyrisperidone), the major active metabolite of risperidone, has been introduced as a novel atypical antipsychotic agent in many countries. It is available both as an oral extended-release (ER) formulation and as a long-acting injection (paliperidone palmitate, PP), which have been approved for treating schizophrenia in the People's Republic of China since 2009 and 2012, respectively. This systematic review summarizes the efficacy, effectiveness, and safety of paliperidone in the treatment of schizophrenia in the Chinese population.METHODS:A systematic literature search was conducted on the databases covering international and Chinese core journals, published from January 1, 2008, to May 22, 2015.RESULTS:A total of 122 publications were retrieved, of which 63 studies were identified for inclusion; most studies were related to paliperidone ER (n=53), nine were related to PP, and one study was related to both agents. Paliperidone ER demonstrated at least comparable efficacy with active comparators, including risperidone, olanzapine, ziprasidone, or aripiprazole, and was found to be superior with respect to the onset of action and improvement in the Personal and Social Performance Scale score. Paliperidone ER appeared to be associated with a lower risk of metabolic syndromes; the most common treatment-emergent adverse events were extrapyramidal symptoms, akathisia, insomnia, and somnolence. Results from interventional and observational studies showed that PP was also an effective and well-tolerated treatment for Chinese patients with schizophrenia. The findings were generally consistent with those observed in non-Chinese populations.CONCLUSION:Both paliperidone ER and PP were effective and well-tolerated agents for the treatment of schizophrenia in the Chinese population according to the data we reviewed. No new safety signals specific for the Chinese population were raised for paliperidone. Further studies may be needed to collect more data on long-term treatment of schizophrenia in the People's Republic of China.

Abstract:Purpose of Review: This is a comprehensive review of the literature regarding the use of paliperidone in the treatment of schizophrenia and schizoaffective disorder. It covers the background and presentation of schizophrenia and schizoaffective disorder, as well as the mechanism of action and drug information for paliperidone. It covers the existing evidence of the use of paliperidone for the treatment of schizophrenia and schizoaffective disorder. Recent Findings: Schizophrenia and schizoaffective disorder lead to significant cognitive impairment. It is thought that dopamine dysregulation is the culprit for the positive symptoms of schizophrenia and schizoaffective disorder. Similar to other second-generation antipsychotics, paliperidone has affinity for dopamine D2 and serotonin 5-HT2A receptors. Paliperidone was granted approval in the United States in 2006 to be used in the treatment of schizophrenia and in 2009 for schizoaffective disorder. Summary: Schizophrenia and schizoaffective disorder have a large impact on cognitive impairment, positive symptoms and negative symptoms. Patients with either of these mental illnesses suffer from impairments in everyday life. Paliperidone has been shown to reduce symptoms of schizophrenia and schizoaffective disorder.
Efficacy and safety of paliperidone ER in the treatment of child and adolescent inpatients with schizophrenia in a natural state and the clinical recommendation

Abstract:Objective: To evaluate the efficacy and safety of paliperidone ER in the treatment of child and adolescent inpatients with schizophrenia for providing a clinical recommendation how to increase the dosage of paliperidone ER and transfer paliperidone from other antipsychotics.Methods: Patients who were admitted in Jun 2009 to July 2011(n=55) were enrolled and treated with paliperidone ER for 12 weeks.We used dosage titration to increase the dose of paliperidone ER.Positive and Negative Syndrome Scale(PANSS) was adopted as the efficacy measure before treatment and at 2,4,6,8,10 and 12 week after treatment.Treatment Emergent Symptom Scale(TESS) and laboratory examinations were adopted as safety measures.Results: All the patients were successfully to increase the dosage of paliperidone ER.None of them have the problem of tolerance.The total PANSS score,positive scale,negative scale,and general psychopathology scale scores demonstrated a significant improvement from the second week in paliperidone ER alone,combination and exchange groups(P<0.05).The adverse events were less severe.The influence on the weight was smaller in paliperidone ER alone group than other two groups.The influence on the prolactin had no statistically significant difference.Conclusion: Paliperidone ER can improve the positive and negative symptoms in child and adolescent patients with schizophrenia.It has demonstrated a favorable safety.When used in the young patients,a way of increasing the dosage of paliperidone ER thought the titration of risperidone can be recommended.

Lower rates of relapse could be expected for patients discontinuing treatment from longer-acting formulations vs their shorter-acting equivalents. Objective: To compare relapse rates and time-to-relapse between the active (analogous to adherent patients) and placebo (analogous to non-adherent patients in the real-world) arms of three different formulations of paliperidone (oral paliperidone extended release [paliperidone ER], paliperidone palmitate once monthly [PP1M], and paliperidone palmitate three monthly [PP3M] long-acting injectables). Methods: Data from three similarly designed, randomized relapse prevention studies in adult patients with schizophrenia were analyzed. Results: In total, 922 patients were included (active treatment: 473, placebo: 449). Lowest percentage of patients experienced relapse with PP3M PP1M (172 days [134– 222 days])> paliperidone ER (58 days [42– 114 days]) and was "not-estimable" in the active treatment group due to low relapse rates. Hazard ratios (HR) of the three paliperidone formulations relative to their respective placebos were PP3M ([HR: 3.81; 95% CI: 2.08, 6.99; P PP1M [HR: 3.60; 95% CI: 2.45, 5.28; P paliperidone ER [HR: 2.83; 95% CI: 1.73, 4.63; P< 0.001]). Conclusion: The lower percentage of relapse during active treatment and longer time to relapse after discontinuing active treatment with longer-duration antipsychotic formulations suggests the benefit of longer-acting over shorter-acting formulations, especially in patients susceptible to poor adherence. Clinical trial registration: paliperidone ER (NCT00086320), PP1M (NCT00111189), and PP3M (NCT01529515). Abstract:Risperidone extended‐release injectable suspension (R‐ERIS; marketed as RYKINDO) is a novel immediate‐release version of risperidone formulated as extended‐release microspheres for biweekly intramuscular injection to treat schizophrenia in adults. The pharmacokinetics (PK) and safety of R‐ERIS were evaluated in a multicenter, randomized, open‐label, multiple‐dose study in patients with stable schizophrenia or schizoaffective disorder. Eligible patients (N = 108) 18 to 65 years old were randomized (1:1) to receive IM injections of R‐ERIS 25 mg or the comparator, a biweekly risperidone long‐acting injectable (BW‐RLAI; marketed as RISPERDAL CONSTA) 25 mg for a total of 5 injections. The primary objective was to evaluate the relative bioavailability of active moiety (risperidone plus 9‐hydroxyrisperidone) at steady state. Blood samples were analyzed for risperidone and 9‐hydroxyrisperidone using a validated, specific, and sensitive liquid chromatography with tandem mass spectrometry method. Plasma concentration–time data were analyzed using non‐compartmental methods. Pharmacokinetic parameters were calculated based on individual patient PK profiles. Safety was assessed using standard measures. At steady state, mean plasma concentrations of the active moiety were similar for R‐ERIS and BW‐RLAI. R‐ERIS rapidly released risperidone after the injection without apparent lag time. Plasma active moiety levels reached steady state after the second injection of R‐ERIS. The elimination of the drug was completed approximately 2 weeks earlier for R‐ERIS as compared to that for BW‐RLAI. R‐ERIS was safe and well tolerated. Overall, R‐ERIS exhibited a faster onset and offset than BW‐RLAI and statistical analysis of exposure parameters demonstrated bioequivalence at steady state.

Abstract:This open-label, rater-blinded, parallel-group study was designed to evaluate noninferiority of paliperidone palmitate (PP), a once-monthly injectable atypical antipsychotic, to once-biweekly risperidone long-acting injectable (RIS-LAI) in adult Chinese patients with acute schizophrenia. Eligible Chinese adults (N=452) with schizophrenia were randomized (1:1) to either PP (N=229; deltoid injections on day 1 [150mg eq.] and day 8 [100mg eq.]; then once-monthly deltoid or gluteal injections, flexibly dosed [50, 100, or 150mg eq.]), or RIS-LAI (N=223; once-biweekly gluteal injections, flexibly dosed [25, 37.5 or 50mg]). RIS-LAI-treated patients received oral risperidone supplementation (1-6mg/day) at initiation and with RIS-LAI dose increases. Mean (SD) Positive and Negative Syndrome Scale (PANSS) total score at baseline was 83.2 (12.44). Mean (SD) change from baseline to endpoint in PANSS total scores (primary efficacy measure) was: −23.6 (16.28) for PP group and −26.9 (15.43) for RIS-LAI group. PP was noninferior to RIS-LAI (least squares mean difference [95% CI]: −2.3 [−5.20; 0.63]; predetermined non-inferiority margin: −5.5). Mean (SD) change from baseline to endpoint in Clinical Global Impression-Severity scale score was: −1.5 (1.24; PP group), −1.7 (1.16; RIS-LAI group) and in Personal and Social Performance Scale scores was: 16.8 (14.76; PP group), 18.6 (13.92; RIS-LAI group). The incidence of treatment-emergent adverse events (TEAEs) was similar between the two groups (73% [PP]; 75% [RIS-LAI]). The most common TEAEs were akathisia, tremor, and insomnia. The study demonstrated the noninferiority of PP (50–150mg eq., flexibly dosed, without oral paliperidone supplementation) to risperidone-LAI (25–50mg, flexibly dosed, with oral risperidone supplementation) for the treatment of acute schizophrenia in adult Chinese patients. PP injections were generally tolerable, and no new safety signals were detected in this population.

Abstract:Objectives: The long-term safety, tolerability, and efficacy of paliperidone extended-release (ER) were evaluated in Chinese patients with schizophrenia. Methods: Patients (aged. 18 years) with schizophrenia (Diagnostic and Statistical Manual of Mental Disorders, 4th edition criteria) who had completed run-in (8-week), stabilization (6-week), and double-blind (DB) phases (variable) of a phase-3, placebo-controlled study entered this 24-week, open-label extension (OLE) study. These patients, who had either experienced a relapse or remained relapse-free through DB phase of the study, were treated with flexible-dose paliperidone-ER (3-12 mg/day) during the OLE phase. Major safety evaluations included treatment-emergent adverse events (TEAEs) and extrapyramidal symptoms. Efficacy endpoints included changes in Positive and Negative Syndrome Scale total score, Clinical Global Impression-Severity scale, and Personal and Social Performance scale from OLE baseline to OLE endpoint. Results: Out of 106 patients who entered the OLE phase (placebo: 59, paliperidone-ER: 47), a total of 85 (80%) completed it. Thirty-five (33%) patients experienced at least one TEAE; most common were akathisia, somnolence, nasopharyngitis, and constipation (3.8% each). Serious TEAEs were noted in two patients (completed suicide; schizophrenia worsening). No TEAEs with an onset during the OLE phase led to discontinuation. Extrapyramidal symptoms related-TEAEs were reported in eight (7.5%) patients. Mean (standard deviation) changes in Positive and Negative Syndrome Scale total scores (-10.4[13.2]), Clinical Global Impression-Severity scores (-0.6[0.96]) and Personal and Social Performance scores (7.4[13.2]) from OLE baseline to OLE endpoint showed patients who had been treated with placebo during the DB phase experienced more pronounced improvements. Conclusion: In this OLE study, flexibly dosed paliperidone-ER (3-12 mg/day) was tolerable and efficacious in Chinese patients with schizophrenia.

Abstract:Introduction Relapse prevention is critical because psychopathology and functionality can worsen in patients with schizophrenia because the repeated episodes and we have strong evidence of antipsychotics efficacy for relapse prevention, but nonadherence rates in patients with schizophrenia are very high, even in comparison with other illness. There is extensive clinical trial evidence for the use of paliperidone palmitate 1-month (PP1M) and paliperidone palmitate 3-month (PP3M) formulations for maintaining treatment continuity and preventing relapses and risk of hospitalizations in patients with schizophrenia. (Najarian et al. Int J Neuropsychopharmacol 2022; 25(3) 238-251). Paliperidone palmitate 6-month (PP6M) formulation is a presentation that provides a dosing interval of once every six months. Objectives The principal aim of this study was to evaluate the effectiveness, safety, and tolerability of the PP6M in patients with non-acute schizophrenia on an outpatient basis Methods Methods: Sample: 22 patients diagnosed with schizophrenia (DSM 5 criteria) that started treatment with PP6M after being stabilized with PP1M (N:10) or PP3M (N:12) (the treatment dose was not changed in the four months before study inclusion) Bimonthly, the following evaluations were performed during a follow-up period of 14 months: The Clinical Global Impression-Schizophrenia scale (CGI-SCH) Treatment adherence, concomitant medication, adverse events and the number of hospitalizations and emergency visits Efficacy values: Percentage of patients who remained free of admissions at the end of 14months of follow-up. Other evaluation criteria: Percentage of patients who never visited the emergency department at the end of 14 months of follow-up, average change from baseline visit to the final evaluation as assessed by score obtained on the following scale: GSI-SCH, treatment adherence rate and tolerability. Results The percentage of patients who remained free of admission at the end of the 14 months follow-up was 90% in the total sample, 83% in the PP3M pre-treatment group and 100% in the PP1M pre-treatment group. The percentage of patients who never visited the emergency department at the end of 14 months follow-up was: 81% in the total sample, 75% in the PP3M pre-treatment group and 90% in the PP1M pre-treatment group. At the end of the study, a mean change of +0.12 (±0.11) on the ICG-SCH-SI scale in the total sample, +0.25 (±0.21) in the PP3M pre-treatment group and 0 in the PP1M pre-treatment group. The treatment persistence rate at the 14 month of follow-up was 100% in the total sample. Treatment was well tolerated, and no safety-related adverse events were collected. There were no tolerability-related withdrawals from treatment. Conclusions In our study, we found that long-term treatment with paliperidone palmitate 6-month formulation is effective and well tolerated in clinical practice conditions. Abstract:Background: Antipsychotic medications facilitate the improvement of psychotic symptoms in patients with first-episode psychosis. Paliperidone extended-release (pali-ER), an atypical antipsychotic, was assessed for efficacy and safety in Chinese patients with first-episode psychosis. Methods: In this 8-week, open-label, single-arm, multicenter study, patients with first-episode psychosis (Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition criteria) and a Positive and Negative Syndrome Scale (PANSS) total score >= 70 were treated with flexible-dose pali-ER tablets (3-12 mg/day). The primary efficacy endpoint was the percentage of patients with an increase of >= 8 points in Personal and Social Performance (PSP) score from baseline to day 56 (8 weeks). Secondary endpoints included reduction in PANSS total score, improvement in Clinical Global Impression-Severity score, PSP score, Subjective Well-being under Neuroleptics Scale score, and relationship between duration of untreated psychosis and PANSS or PSP. Incidences of treatment-emergent adverse events were used to evaluate safety. Results: Overall, 283 of 294 patients (96%) achieved a >= 8-point increase in PSP (primary endpoint, analysis set). For the secondary efficacy endpoints, 284/306 patients (93%) had a >= 30% reduction in PANSS total score; 266/306 patients (87%) achieved a <= 3 Clinical Global Impression-Severity scale score, and 218/294 patients (74%) had a PSP score >= 71. The Subjective Well-being under Neuroleptics Scale score was improved from a baseline mean of 72.7 to 94.7 at endpoint. There was a negative correlation between duration of untreated psychosis and posttreatment PSP score and a positive correlation with posttreatment PANSS total score. The most common treatment-emergent adverse events were extrapyramidal symptoms (12%), and agitation, somnolence, and xerostomia (4% each). Conclusion: An 8-week, flexible-dose (3-12 mg/day) treatment with pali-ER resulted in significant improvements in psychotic symptoms and was generally tolerable.

Abstract:This paper presents two different feature activation methods in a Hierarchical Concatenation Network (HCN). In this experiment the similarity measure was conducted by calculating the proportion of the features of tested and reference patterns in all network's layers. The experiment also implemented the circular shifting of input layers. Results show that HCN-2 without circular shifting provides better results compared to HCN-1. HCN-2 also consumes less processing time than HCN-1.

Abstract:Abstract Background This double-blind (DB), randomized, parallel-group study was designed to evaluate efficacy and safety of paliperidone palmitate 6-month (PP6M) formulation relative to paliperidone palmitate 3-month (PP3M) formulation in patients with schizophrenia. Methods Following screening, patients entered an open-label (OL) maintenance phase and received 1 injection cycle of paliperidone palmitate 1-month (PP1M; 100 or 150 mg eq.) or PP3M (350 or 525 mg eq.). Clinically stable patients were randomized (2:1) to receive PP6M (700 or 1000 mg eq., gluteal injections) or PP3M (350 or 525 mg eq.) in a 12-month DB phase; 2 doses of PP6M (corresponding to doses of PP1M and PP3M) were chosen. Results Overall, 1036 patients were screened, 838 entered the OL phase, and 702 (mean age: 40.8 years) were randomized (PP6M: 478; PP3M: 224); 618 (88.0%) patients completed the DB phase (PP6M: 416 [87.0%]; PP3M: 202 [90.2%]). Relapse rates were PP6M, 7.5% (n = 36) and PP3M, 4.9% (n = 11). The Kaplan-Meier estimate of the difference (95% CI) between treatment groups (PP6M − PP3M) in the percentages of patients who remained relapse free was −2.9% (−6.8%, 1.1%), thus meeting noninferiority criteria (95% CI lower bound is larger than the pre-specified noninferiority margin of −10%). Secondary efficacy endpoints corroborated the primary analysis. Incidences of treatment-emergent adverse events were similar between PP6M (62.1%) and PP3M (58.5%). No new safety concerns emerged. Conclusions The efficacy of a twice-yearly dosing regimen of PP6M was noninferior to that of PP3M in preventing relapse in patients with schizophrenia adequately treated with PP1M or PP3M.
