A Qualitative Exploration of the Perceptions of Women Living with Pelvic Floor Disorders and Factors Related to Quality of Life

Abstract:Background: Pelvic floor dysfunction encompasses conditions like urinary and fecal incontinence, pelvic organ prolapse, and pelvic pain, significantly affecting women's quality of life. Despite its prevalence, few studies have adopted a qualitative approach to understanding women's perceptions and emotions regarding these issues. This study aims to delve into how women with pelvic floor disorders perceive their condition and its impact on their daily lives. Methods: We analyzed qualitative data from interviews with 160 women suffering from pelvic floor dysfunctions. Using inductive qualitative content analysis, we systematically examined the data to identify variations, differences, and similarities. Results: The analysis revealed four primary themes in the women's narratives: "Physical Impacts", "Emotional and Psychological Impacts", "Social and Relational Impacts", and "Sexual Health Impacts", along with 12 subthemes. The findings predominantly highlight how pelvic floor dysfunctions detrimentally affect women's quality of life and emotional well-being, instilling fear and insecurity in daily activities, compounded by sleep disturbances and sexual dysfunction. Conclusions: Women living with pelvic floor dysfunction face multifaceted challenges that adversely affect various aspects of their lives, diminishing their overall quality of life. This includes notable impacts on sleep, physical, and sexual activities. However, not all affected women report these issues, often due to fear of stigma, choosing instead to conceal their struggles in an effort to maintain an appearance of normalcy.

Therefore, the goal of this study is to comprehensively explore the perceptions and emotions of women with Pelvic Floor Disorders and how these perceptions affect their daily lives. ### Methods The researchers employed a qualitative descriptive research design, using inductive qualitative content analysis methods, and recruited participants through social media. The research team designed a series of questions to encourage participants to describe in their own words how pelvic floor issues affect their daily lives. A total of 160 women participated in the survey, with an average age of 36.2 years. ### Results Through rigorous analysis of the narratives from the 160 participants, the researchers identified four main themes and 12 sub-themes: 1. **Physical Impact**: - Impact on daily life: Urinary incontinence prevents many women from engaging in satisfactory physical activities, even causing leakage during running or jumping. - Emotional impact: The discomfort of continuously using pads makes them feel helpless and fearful of urinary incontinence in public. - Self-management: Many women have to frequently use pads and must use them even during non-aerobic and impact activities. 2. **Emotional and Psychological Impact**: - Discomfort and insecurity: The constant need to wear pads and the fear of urinary incontinence in public lead to low mood and social withdrawal. - Anxiety and sadness: Worrying about urinary incontinence and anal intercourse leads to anxiety and depression. - Embarrassment: The fear that the smell of urine leakage will be noticed by others causes embarrassment and emotional distress. 3. **Social and Relationship Impact**: - Culture of silence and shame: Frequent nighttime trips to the bathroom affect sleep quality. - Impact on partner relationships and social interactions: The fear and insecurity caused by urinary incontinence affect intimate relationships and social activities. 4. **Sexual Health Impact**: - Penetrative pain: Pain during insertion in certain positions. - Decreased libido: Lack of sexual desire, dryness, burning sensation, pain, lack of lubrication, and pleasure. - Fear of urinary incontinence during intercourse: Fear of leakage during intercourse leads to fear and tension. ### Conclusion Women with Pelvic Floor Disorders face multifaceted challenges that not only affect their physical health but also severely impair their emotional and social lives. The study's findings highlight the importance of comprehensive care strategies that not only alleviate physical symptoms but also provide psychological support, reduce social stigmatization, and restore women's confidence and quality of life.
