Abstract:Prepubertal primary testicular tumors account for ∼1% of all pediatric solid tumors. We report a new case of prenatal diagnosis of juvenile-type granulosa cell tumor (JGCT). A fetal ultrasound performed at the 38th week of gestation for suspected nonvertex presentation identified a left multilocular septated cystic testicular mass, suggestive for JGCT. At birth, a painless left scrotal mass was detected. Ultrasound re-evaluation excluded torsion of the testis. Tumor markers and abdominal ultrasound were normal for age. Inguinal exploration revealed a cystic mass beneath the tunica albuginea that had replaced all the normal parenchyma. Since organ-sparing surgery was thus not feasible, an orchiectomy was performed and diagnosis of JGCT was confirmed. At 7-year follow-up, the child presented an uneventful outcome. Our case shows that neonatal JGCT, which has an intrauterine genesis, can be diagnosed prenatally by ultrasound in the last weeks of pregnancy.

Abstract:We report a rare subtype of adult cystic granulosa cell tumor (AGCT) characterized by elevated anti-Mullerian hormone and hyperandrogenism. A 35-year-old woman with primary infertility, hyperandrogenism, and irregular menses who was previously diagnosed with polycystic ovarian syndrome was diagnosed with AGCT based on histopathological examination and FOXL2 genetic test after laparoscopy. Due to fertility aspirations, she underwent controlled ovarian stimulation followed by embryo cryopreservation before salpingo-oophorectomy, and two embryos were frozen-thawed and transferred after surgery. A healthy female infant was delivered at 40 weeks' gestation. Cystic granulosa cell tumors should be considered a differential diagnosis in patients with persistent ovarian cysts and hyperandrogenism. Younger patients with AGCT with fertility goals should consider active assisted reproduction measures to preserve fertility before treatment for AGCT.
Clinicopathological Characteristics and Treatment Outcomes of Pregnancy Complicated by Malignant Ovarian Germ Cell Tumors.

Abstract:PURPOSE:This study aimed to analyze the clinicopathological features, treatment, and feto-maternal outcomes of pregnancy complicated by malignant ovarian germ cell tumors (MOGCTs), to increase the awareness on this condition.PATIENTS AND METHODS:We retrospectively reviewed the medical records of patients diagnosed with MOGCTs during pregnancy, who were treated and followed-up at Peking Union Medical College Hospital from January 2000 to December 2017. The demographic characteristics, pathological features, treatment and prognosis were analyzed.RESULTS:The histological subtypes varied in 14 patients (dysgerminoma, n=1; immature teratoma, n=4; yolk sac tumor, n=6; and mixed germ cell tumors, n=3). Ten (71.4%) patients, including three who opted for conservative therapy until childbirth, one who only received salvage chemotherapy during pregnancy, and six who underwent cystectomy or unilateral salpingo-oophorectomy during pregnancy, desired fetal preservation. After undergoing surgery, four patients chose surveillance instead of timely adjuvant chemotherapy. Eight patients delivered their babies, and the preterm delivery rate was 50.0%. One newborn died of premature birth. The median follow-up period was 44 (range: 13 to 86) months. During the current study period, 12 patients had survived and did not report any diseases. However, two died due to disease progression.CONCLUSION:Pregnant women with MOGCTs had favorable outcomes. However, when a malignant tumor is suspected, surgery cannot be avoided. Thus, instead of timely postoperative adjuvant chemotherapy, close surveillance may be an acceptable alternative for pregnant women with low-risk MOGCTs.

Abstract:We report on the first direct observation of magnetic fluctuations in magnetically ordering heavy-fermion U systems above T/sub N/. They were measured for UCu/sub 5/ and U/sub 2/Zn/sub 17/ via the quasielastic lines by inelastic neutron scattering. Both compounds behave very similarly. We observe a common broad Lorentzian quasielastic line with width of order GAMMA/sub QE/approx. =10 meV, virtually unaffected by the magnetic phase transition, but also a narrow Gaussian quasielastic line, which we attribute to critical spin fluctuations. We discuss the occurrence of magnetic order in spite of the fast valence-fluctuation rate as in contrast to similar magnetically ordering 4f systems.

Abstract:We report a case of rare bilateral massive ovarian luteinized follicular cysts in a twin pregnancy. A 28-year-old, gravida 1, para 0, abortus 0 woman was incidentally found to have bilateral massive ovarian cysts (left side, 12 × 10 × 8 cm; right side, 15 × 10 × 6 cm) during the cesarean delivery of a twin pregnancy at 36 weeks of gestation. After 1,950-g and 2,350-g female babies were delivered in good condition, the patient received bilateral oophorectomy. Subsequent histopathology led to the diagnosis of giant bilateral ovarian luteinized follicular cysts.

Abstract:BACKGROUND:Intrahepatic cholestasis of pregnancy (ICP) is a common gestational complication characterized by pruritus and elevated bile acids, usually occurring in the third trimester when the serum estrogen and progesterone levels are highest. Hyperandrogenism during pregnancy is a pathological state that is mostly induced by polycystic ovary syndrome (PCOS) but rarely by concomitant androgen-secreting ovarian tumours. To date, no correlation has been drawn between ICP and hyperandrogenism. CASE PRESENTATION:Here, we present a rare case of early-onset severe ICP in a PCOS patient conceived via in vitro fertilization-embryo transfer, with worsening hirsutism and acne due to high levels of testosterone and dehydroepiandrosterone sulphate, both of which were produced by a fast-growing ovarian Sertoli-Leydig cell tumour. Her serum estradiol was also very high, which was speculated to be converted from the circulating androgens by the placenta. She had preterm premature rupture of membranes and delivered at 30 weeks, followed by a rapid remission of ICP as her serum estradiol dropped. However, the excessive androgens did not retreat until the large ovarian tumour was surgically removed. CONCLUSION:This unusual case highlights the concurrence of original hyperandrogenism and subsequent hyperestrogenism during pregnancy and the resultant confounding manifestations. Obstetricians should be aware of the potential association between androgen excess and ICP via placental aromatization.
Male hermaphroditism with rare malignant transformation of gonadal organs: a case report

Abstract:A 27-year-old man was admitted to the hospital after a year of marriage due to infertility. During laparoscopic exploratory surgery, the presence of a retrovesical uterus was clearly observed, and the gonadal organs were visible on both sides. However, the testicles or ovaries were not identifiable, nor were the spermatic vessels and fallopian tubes at the joint. Intraoperative bilateral gonad biopsy was performed. Cryopreservation of the right gonadal gland revealed gonadoblastoma and malignant germinoma (asexual tumor/seminoma) with sclerosis and atrophy of testicular tissue. No proliferation of germ cells and sertoli cells was observed in spermatic tubule. The left gonad was diagnosed as a gonadoblastoma. Finally, total hysterectomy and bilateral gonadal tumor organectomy were performed to seal the vaginal stump. Local radiotherapy was administered after surgery. In general, tumors were found on both sides of the gonads, especially gonadoblastoma and malignant germinoma on the right side and gonadoblastoma on the left side.

Abstract:Unclassified mixed germ cell-sex cord-stromal tumor (UMGC-SCST) is a rare ovarian neoplasm composed of germ cells and sex cord elements, which occurs in genetically and phenotypically normal women without the usual histological features seen in gonadoblastoma. Few cases have been reported in the literature so far. The age of presentation is more frequent in girls younger than 10 years of age, although it can also occur in adult women. It can be associated with isosexual pseudoprecocity. The preferred management is the resection of the gonad that contains the tumor and the conservation of the opposite ovary and tube. This is a case of a 14-year-old patient, with precocious puberty and normal phenotype, diagnosed with this kind of ovarian tumor. A fertility preserving surgery with the resection of the right ovarian tumor and tube was performed. The patient was classified as stage IA according to the 2014 International Federation of Gynecology and Obstetrics (FIGO). She received adjuvant chemotherapy with bleomycin-etoposide-cisplatin for three cycles. After a follow-up of 24 months, she was found to be asymptomatic and free of relapse.
Unusual radiologic imaging in juvenile granulosa cell tumor with precocious puberty: A unilocular cyst

Abstract:Juvenile Granulosa Cell Tumor (JGCT) represents 5% of all granulosa cell cancers. Precocious puberty is a frequent feature of this tumor. A 2-year and 2-month-old girl was referred with a diagnosis of suspected ovarian cancer, dysfunctional uterine bleeding, and precocious puberty. Radiologic examination revealed the following: Abdominal ultrasonography showed a solitary anechoic cystic lesion in the pelvic cavity. MRI confirmed the existence of solid components on its walls. JGCT was then confirmed using immunohistochemistry (IHC) markers. JGCT, along with adult granulosa cell tumors (AGCT) are subgroups of granulosa cell tumors (GCTs), which are part of pure sex cord tumors. The 2 forms share imaging findings due to their comparable gross appearance. GCTs require diagnostic imaging tests to distinguish them from other ovarian tumors. Two ultrasound patterns can be identified GCTs, and MRI showed that GCTs are more heterogeneous than other sex-cord stromal tumors (OSCs). In our case, the imaging characteristics for juvenile granulosa cell tumors were nonspecific and these tumors cannot be reliably distinguished from other ovarian neoplasms based on imaging alone. Although GCTs have imaging characteristics that can help to distinguish them from other tumors, confirmation by histopathology and IHC is still mandatory, especially in cases with nonspecific radiological features.

Abstract:Ovarian causes of precocious pseudo-puberty (PPP) include McCune-Albright syndrome (MAS) and juvenile granulosa cell tumour (JGCT). We describe a case of PPP in which bilateral ovarian enlargement with multiple cysts progressed to unilateral JGCT. A girl aged 2.17 years presented with three months of breast development, and rapid growth. Examination showed tall stature, height +2.6 standard deviations, Tanner stage B3P2A1. A single café au lait patch was noted. Bone age was advanced at 5 years. Pelvic ultrasound showed bilaterally enlarged ovaries (estimated volumes 76 mL on the left, 139 mL on the right), each containing multiple cysts. Luteinizing hormone (LH) and follicle stimulating hormone (FSH) values before/after gonadotrophin administration were 0.43/0.18 and <0.1/<0.1 mUI/mL, serum estradiol 130 pg/mL, (prepubertal range <20 pg/mL). PPP of ovarian origin was diagnosed, and tamoxifen 20 mg daily started. However, after only seven weeks height velocity escalated and breast development increased to B3-4 with menorrhagia. Basal/stimulated LH and FSH were still suppressed at 0.13/0.25 and <0.1/<0.1 mUI/mL and, serum estradiol 184 pg/mL. Repeat imaging now showed normal right ovary (volume 1.8 mL) and a large left-sided vascular solid/cystic ovarian tumour which was excised (weight 850 g). Histology showed JGCT, International Federation of Gynecology and Obstetrics stage IA. DNA from tumour tissue showed no mutation in GNAS, exon 3 of AKT1 (which contains a mutational hotspot) or FOXL2. The observation that bilateral ovarian activity progressed to unilateral development of JGCT in this patient is novel. This case highlights current uncertainties in the ontology of JGCT, and its possible relationship with MAS.

Abstract:Lipophilic fluorescent dye merocyanine 540 is believed to stain cell membranes with increasing affinity as the lipid components become more disordered and has been associated with changes in membrane fluidity. The aim of this study was to determine whether membrane lipid disorder is associated with capacitation of macaque spermatozoa. To induce capacitation, spermatozoa from 5 rhesus macaques were incubated at 37 degrees C (5% CO(2) in air) for 2 hours in a modified Biggers-Whitten-Whittingham medium containing 30 mg/mL bovine serum albumin and 36 mmol/L NaHCO(3). Caffeine (1 mmol/L) and dbcAMP (1.2 mmol/L) were added to the medium, and incubation was performed for an additional 30 minutes. Sperm motility was determined by computer-assisted sperm analysis, and membrane lipid order and sperm viability was determined by flow cytometry with merocyanine (2.7 micromol/L) and Yo-Pro-1 (25 nmol/L), respectively. Tyrosine phosphorylation of proteins in sperm tail was immunohistochemically examined by means of anti-phosphotyrosine (alpha-PY; clone 4G10). Capacitation resulted in a significant increase in the amplitude of lateral head displacement and beat cross frequency (P < .005) and a significant decrease in linearity and straightness in capacitated spermatozoa (P < .005), compared with control spermatozoa, which suggests the expression of hyperactivated motility. Animals in which capacitation was induced had a significant increase in the number of spermatozoa showing tyrosine phosphorylation of tail proteins (P < .0001) and a significant increase in the intensity of merocyanine fluorescence (P < .0001), compared with control animals. The observed decrease in membrane lipid order after capacitation was induced was not associated with surface exposure of phosphatidylserine, as determined by flow cytometry with annexin V-Alexa Fluor 488. Merocyanine may be a useful tool for investigating the role of the plasma membrane on capacitation and other cytotoxic events in macaque spermatozoa.

Abstract:46, XY pure gonadal dysgenesis, which is also known as Swyer syndrome, is a medical condition of gonadal abnormalities. The authors report a case of a 15-year-old phenotypic female who presented with bulky abdominal mass and underwent laparotomy with right cystectomy, as well as left ovary biopsy. Post-operational pathology demonstrated that malignant mixed germ cell tumors in the right ovary. Taking consideration of no menstruation, abnormal hormone levels, and malignant mixed germ cell tumors in the ovary, the patient took the karyotype test and the result was confirmed: 46, XY. Taking into consideration pure gonadal dysgenesis with mixed malignant germ cell tumors, surgical resection of bilateral gonads is primarily recommended. The patient and her families were well consented on sex selection, cancer treatment strategy, reproductive function, and resection of the pelvic mass, together with uterus, it was decided to maintain female phenotype without reproductive function. Post-operational pathology showed ovarian gonadal blastoma and dysgerminoma, with bilateral calcification. The nodule from the excavatio rectouterina was identified as mature teratoma, without immature components. The patient received four cycles of postoperative chemotherapy using bleomycin, etoposide, and cisplatin (BEP) regime. Long-term hormone replacement therapy was recommended. There has been no sign of recurrence since then.

Abstract:Granulosa cell tumors (GCTs) account for less than 5% of all ovarian malignancies, occur in younger ages, are usually diagnosed in their early stages, and have a good prognosis. GCTs usually present with features of hyperestrogenism. This paper reports the unusual case of an adult with a GCT with manifestations including amenorrhea, mild hirsutism, infertility, clomiphene citrate (CC) resistance (CC), mildly elevated testosterone, high anti-Müllerian hormone (AMH) and normal estrogen levels. The patient was initially diagnosed with polycystic ovarian syndrome (PCOS), and after four attempts at ovarian stimulation she was diagnosed with CC resistance. The patient later underwent laparoscopic evaluation on account of a solid mass on her left ovary. The pathology report described it as a borderline adult type GCT and four weeks after surgery she had a positive pregnancy test. Twelve months after delivery, the patient had no obvious symptoms of disease and her menstrual cycle was normal. Serial measurements of serum inhibin B, AMH, estrogen, and testosterone levels were within normal range. In conclusion, the resistance to clomiphene manifested by the patient might be explained by a potential mechanism implicating inhibin B and AMH due to the presence of a GCT. Further studies are required to evaluate the role of AMH and Inhibin B in the mechanism of CC resistance in women with PCOS.
A Rare Case Report of Ovarian Juvenile Granulosa Cell Tumor with Massive Ascites As the First Sign, and Review of Literature: Case Report and Review of Literature.

Abstract:RATIONALE:Massive ascites as the first sign of ovarian juvenile granulosa cell tumor (JGCT) in an adolescent is an extremely rare, and its clinical features and treatment methods have not been well described.PATIENT CONCERNS:The clinical characteristics, diagnosis, and treatment methods in a 19-year-old girl who presented with massive abdominal distention and ascites was retrospectively reviewed. Abdominopelvic ultrasonography showed a large amount of ascites. The nature of ascites was exudate. All tumor markers were normal, but ascites and serum tumor CA125 levels were significantly increased. Abdominal CT showed left attachment area teratoma and right attachment area capsule solid change.DIAGNOSES:Histological and immunohistochemical results were compatible with JGCT. Based on the FIGO classification, the patient with only malignant ascites was categorized into stage IC.INTERVENTIONS:The patient underwent mass resection with salpingoophorectomy. Following the operation, she received 6 courses of adjuvant chemotherapy with Nedaplatin and Paclitaxel liposome.OUTCOMES:The patient was followed up postoperatively for 6 months to date without recurrence.LESSONS:We should be highly vigilant the JGCT with massive ascites as the first clinical manifestation.
The benefits of clinical pathways (CP) for radiation oncology in a large cancer care network.

Abstract:148 Background: UPMC CancerCenter and the University of Pittsburgh Cancer Institute (UPMC) created a radiation oncology CP program (now incorporated separately as Via Pathways) in 2003 to standardize care at 19 radiation sites. The program exists to ensure consistency and rapid adoption of best evidence-based care in a large, integrated NCI-designated comprehensive cancer center. METHODS The web-based CP portal integrates with electronic medical records to provide point-of-care patient-specific decision support. CP content is developed and maintained by committees of oncologists culled from the network. Meetings occur semi-annually to review and update treatment recommendations for the nearly 95% of cancers covered. Evidence is evaluated hierarchically on efficacy, toxicity, then cost. When the committee modifies CP content, the software is updated within 35 days and new recommendations are available to the 38 network providers. Through analysis of the utilization data, leaders can monitor adherence to the CP recommendations and research practice patterns at an unprecedented level of granularity. RESULTS Since data collection in the portal began in 2009, UPMC has captured over 48,000 visits and adhered to CP recommendations 96% of the time. Off-pathway decisions were approved prior to treatment by a designated peer review radiation oncologist at UPMC. Reasons for going off pathway and the alternative approach to care were recorded. Efforts to increase efficiencies and trial accruals have been successful due in large part to access to such a vast and comprehensive database. CONCLUSIONS CP are an integral part of standardizing to high-value care across large networks and geographic regions. The committee process, rapid electronic deployment, and easy access to evidence reviews have garnered high physician buy-in and consistent adherence to CP recommendations. Other benefits of the program include: a transition from volume to value care models; facilitation of onboarding new physicians, residency trainees, and fellows; providing transparency through peer review, a committee process, and evidence reviews. CP are now used by more than 1000 physicians across the US in a variety of academic and community settings.
The anatomy and physiology of the posterior stomach nerve (p.s.n.) in some decapod crustacea

Abstract:One of the major paired nerves in the decapod stomatogastric nervous system innervates the posterior part of the gastric mill. The morphology has been completely described previously only in Pugettia producta. This nerve is called the posterior stomach nerve (p.s.n.) in this paper. It is redescribed for Homarus vulgaris, and less detailed information is given for Palinurus vulgaris and Cancer pagurus. In these three species the p.s.n. contains numerous cell bodies. The majority of the cells are in one group and the long distal processes of these cells form a large proportion of the fibres in the nerve which runs to the gastric mill. Many of these fibres terminate by ramifying in the connective tissue which invests the ossicles of the gastric mill. The Brachyuran C. pagurus was used for most of the physiological experiments reported here. Only a small percentage of the animals exhibited spontaneous movements of the gastric mill when the carapace was removed and this activity ceased rapidly. However, a cycle of movements was usually observed in the active gastric mills and the p.s.n. contains many elements which respond to these normal movements. The p.s.n. does not appear to contain motor fibres. In tracellular recordings show that cells in the p.s.n. are proprioceptors. The distal processes of the major group of cells are morphologically dendrites, but they probably support action potentials. Repetitive electrical stimulation of a p.s.n. when it is isolated from the gastric mill evokes changes in the outut from the stomatogastric ganglion. The p.s.n. sensory system could therefore function in the normal reflex control of the activity of the gastric mill. This investigation may assist in the analysis of the functions of the stomato gastric ganglion.
Rapid growth of an ovarian clear cell carcinoma expressing LH/hCG receptor arising from endometriosis during early pregnancy

Abstract:The complete clinical course of a case of ovarian clear cell carcinoma expressing luteinizing hormone (LH)/human chorionic gonadotropin (hCG) receptor arising from endometriosis in a pregnant woman is presented. A 31-year-old woman visited a private clinic in May 1993 for screening tests for infertility. Transvaginal ultrasonography revealed no abnormal findings in the uterus or ovaries. Her menstrual cycle was regular; however, a slight luteal insufficiency was noted. She had been treated with clomiphene, and soon became pregnant. She was diagnosed to be at 5 weeks gestation in June, and at the same time, an ovarian tumor with the diameter of 5 cm was identified. Since the tumor had grown rapidly and was 9 cm in diameter 1 week later, she was referred to our hospital. When she was admitted to our hospital at 9 weeks gestation, the tumor diameter was 14 cm and we found the solid portion within the ovarian tumor. The levels of the tumor markers CA125 and CA19-9 were 106 and 51 U/ml, respectively. The crown-rump length of the fetus (24 mm) was compatible with the gestational age, and fetal heartbeat was confirmed. Under the diagnosis of ovarian carcinoma, right salpingo-oophorectomy was performed at 10 weeks of gestation. Postoperative histological examination revealed a clear cell carcinoma and endometriosis of the right ovary. Immunohistochemically, the clear cell carcinoma stained positively for LH/hCG receptors and estrogen receptors, but not progesterone receptor. No malignant cells were detected by ascitic cytology. Exploratory specimens obtained at the time of operation from the left ovary and pelvic lymph nodes exhibited no malignant cells. Based on these findings, the pregnancy was allowed to proceed, and she delivered a 3010-g male baby at 39 weeks of gestation. She had no signs of recurrence for 2 years after the operation.

Abstract:The majority of abdominal cystic formations in newborns are benign and usually originate from the urinary tract, genital system or gastrointestinal system. Fetal ovaries are predisposed to development of cysts due to the maternal hormonal environment of advancing gestational age. Prenatal diagnosis became possible with improvement of ultrasound techniques. Complex pattern of the cysts are at higher risk for complications, the most concerning being ovarian torsion and consequent compromised fertility. Management is controversial, involution of simple cysts few months after birth is possible considering the cease of ovarian stimulation by maternal and fetal hormones but surgical approach is more indicated in symptomatic or complex cysts. We report the case of a prenatally diagnosed cystic abdominal mass at 33 weeks of gestation with antenatal ovarian torsion and need for right oophorectomy in the neonatal period. At two months of age appearance of ovarian cyst was observed on the contralateral ovary by ultrasound, being managed conservatively.

Abstract:<p>Ovarian granulosa cell tumor (GCT) is a rare, low-grade malignant tumor that accounts for 70% of the sex cord-stromal tumors, it has two histopathological types with different clinical and biological features: adult GCT and juvenile GCT. Most women diagnosed with the adult GCT have a favorable prognosis, with a 5-year survival rate of 97-98%, but adult GCT has a feature of late relapse, the recurrence time could be more than 20 years after diagnosis. Juvenile GCT had a survival rate of 97% in stage I and a 5-year survival rate of 0%-22% in advanced stage with earlier relapse than adult GCT. Consequently, the scenario emphasizes the need for early diagnosis, standardized treatment protocols and long-term follow-up. However, there is a lack of consensus regarding accurate diagnosis of GCT and adjuvant treatment. Furthermore, GCT tends to occur in young women, which emphasizes the viability of fertility-sparing surgery. The current review performed a systematic literature review of 60 articles in order to summarize the latest advancements of GCT, with an emphasis on the molecular pathogenesis and survival after fertility-sparing surgery. We found that young women with fertility-sparing surgery had a desirable reproductive and survival outcome compared to radical surgery.</p>
