Abstract:Background: Narrow-band ultraviolet B (NB-UVB) is the standard therapy for vitiligo. Objective: The objective of this study is to compare the safety and clinical efficacy of a handheld NB-UVB comb device with the standard whole-body NB-UVB therapy in localized stable vitiligo. Materials and methods: Thirty-one vitiligo patients were allocated to either daily therapy with a home-based handheld comb device (group A, n = 17) or thrice-weekly hospital-based whole-body NB-UVB therapy (group B, n = 14) for 4 months, based on their preference. The primary and secondary outcomes were assessed at each follow-up, and appropriate statistical tools were used for analysis. Results: Of the 31 patients enrolled, 26 patients (study groups A/B: 15/11) completed the study. Primary outcome: Median percentage repigmentation of the representative patch in groups A and B were 51.35% and 63.85%, respectively (P = 0.64). The median size reduction of the representative patch in both groups was statistically significant (P < 0.05). The mean difference between "per protocol analysis" and "intention to treat" showed noninferiority. Secondary outcomes: Both groups were comparable on Lund and Browder score, patient global assessment and investigator global assessment scores, adverse events, color match, and change in the quality of life. The comparison group had a significantly greater number of missed sessions (P = 0.02). The majority of patients had a "good" response in both groups. Conclusion: Handheld NB-UVB comb device daily with a fixed dose of fluence was found to be noninferior with better compliance to standard whole-body NB-UVB therapy.
Effect of NB-UVB in the treatment of Mycosis fungoides

Abstract:Objective To assess the effect of Narrowband UVB(NB-UVB) on Mycosis fungoides(MF).Methods Retrospectively reviewed the response of 31 patients who were clinically and pathologically diagnosed as MF in our departmet and treated with NB-UVB from Janunary 2008 to Decamber 2010.Results In all 31 patients,17 in stage ⅠA-ⅠB and 14 in stage ⅡB-ⅠV.Seventeen reached CR(54.84%),6 reached PR(19.35%) and 8(25.81%) failed.The total effective rate was 74.19%.Separately,in early stage(ⅠA-ⅠB),15 reached CR(88.24%) and 2 reached PR(11.76%) and the response rate was 100%.In late stage(ⅡB-Ⅳ),2 reached CR(14.29%),4 reached PR(28.57%),8 failed(57.14%) and the response rate was 42.86%.Mean(26.92±11.47) times of treatment were needed to reach remission and the mean period of relapse-free remission was(14.83±7.99) months.The No.of treatments(23.29±7.27) in early stage(ⅠA-ⅠB) were less than that in late stage(35.71±15.37) and the remission period was longer in early stage(18.41±7.27) months than in late stage(7.86±6.01) months,and the differences were significantly(P<0.01).Conclusion NB-UVB was a safe and effective treatment with higher efficacy and longer remission time for MF of early stage compared to late stage.Maintenance treatment prolonged the remission period.NB-UVB could be the choice for relapse.Patients of late stage could reach CR or PR after treatment of NB-UVB,as a part of combined therapy.

Abstract:Importance: Office-based phototherapy is cost-effective for psoriasis but difficult to access. Home-based phototherapy is patient preferred but has limited clinical data, particularly in patients with darker skin. Objective: To compare the effectiveness of home- vs office-based narrowband UV-B phototherapy for psoriasis. Design, setting, and participants: The Light Treatment Effectiveness study was an investigator-initiated, pragmatic, open-label, parallel-group, multicenter, noninferiority randomized clinical trial embedded in routine care at 42 academic and private clinical dermatology practices in the US. Enrollment occurred from March 1, 2019, to December 4, 2023, with follow-up through June 2024. Participants were 12 years and older with plaque or guttate psoriasis who were candidates for home- and office-based phototherapy. Interventions: Participants were randomized to receive a home narrowband UV-B machine with guided mode dosimetry or routine care with office-based narrowband UV-B for 12 weeks, followed by an additional 12-week observation period. Main outcomes and measures: The coprimary effectiveness outcomes were Physician Global Assessment (PGA) dichotomized as clear/almost clear skin (score of ≤1) at the end of the intervention period and Dermatology Life Quality Index (DLQI) score of 5 or lower (no to small effect on quality of life) at week 12. Results: Of 783 patients enrolled (mean [SD] age, 48.0 [15.5] years; 376 [48.0%] female), 393 received home-based phototherapy and 390 received office-based phototherapy, with 350 (44.7%) having skin phototype (SPT) I/II, 350 (44.7%) having SPT III/IV, and 83 (10.6%) having SPT V/VI. A total of 93 patients (11.9%) were receiving systemic treatment. At baseline, mean (SD) PGA was 2.7 (0.8) and DLQI was 12.2 (7.2). At week 12, 129 patients (32.8%) receiving home-based phototherapy and 100 patients (25.6%) receiving office-based phototherapy achieved clear/almost clear skin, and 206 (52.4%) and 131 (33.6%) achieved DLQI of 5 or lower, respectively. Home-based phototherapy was noninferior to office-based phototherapy for PGA and DLQI in the overall population and across all SPTs. Home-based phototherapy, compared to office-based phototherapy, was associated with better treatment adherence (202 patients [51.4%] vs 62 patients [15.9%]; P < .001), lower burden of indirect costs to patients, and more episodes of persistent erythema (466 of 7957 treatments [5.9%] vs 46 of 3934 treatments [1.2%]; P < .001). Both treatments were well tolerated with no discontinuations due to adverse events. Conclusions and relevance: In this randomized clinical trial, home-based phototherapy was as effective as office-based phototherapy for plaque or guttate psoriasis in everyday clinical practice and had less burden to patients. Abstract:Background. Mycosis fungoides (MF) is the most common subtype of cutaneous T-cell lymphoma. The aim of the present study was to produce up-to-date information on different phototherapy approaches on skin cytokines in patients with MF. Methods. A total of 27 patients with mycosis fungoides were treated with phototherapy: NB-UVB (narrow‐band ultraviolet B therapy) (10 patients) and PUVA (long-wavelength ultraviolet radiation of spectrum A with the use of skin-photosensitizing furocoumarins) therapy (17 patients). Evaluation of the effectiveness of treatment was carried out using BSA (body surface area) and the modified assessment of the severity of the skin lesions scale (mSWAT) used to quantify tumor mass in cutaneous T-cell lymphomas. Average numbers of procedures were 30.2 and 27.8 in the NB-UVB and PUVA groups, respectively. The median total dose of NB-UVB irradiation was 19.9 J/cm2 and PUVA therapy was 104.0 J/cm2. The overall response to therapy including complete and partial remission was 74.9% in the total group; 70% in the NB-UVB group, and 77.7% in the PUVA therapy group. In the obtained biopsies from lesions, surrounding tissue before treatment and skin samples of four healthy volunteers, the concentration of the IL-1β, IL-4, IL-6, IL-10, IL-17A, IL-17F, IL-21, IL-22, IL-23, IL-25, IL-31, IL-33, IFN-γ, sCD40L, and TNF-α cytokines was studied. An increase in IL-4 and TNF-α levels was shown in the lesional skin of patients compared to the skin of healthy controls. After the treatment, positive correlations of mSWAT with the levels of IL22, IL33, and TNF-α in the tumor tissue were found. The levels of IL10 and IFN-γ after PUVA treatment were increased in comparison to baseline. There was no difference in cytokine levels before/after NB-UVB therapy.

Abstract:Narrowband UVB (NB-UVB) phototherapy and psoralen-UVA (PUVA) photochemotherapy are widely used phototherapeutic modalities for a range of skin diseases. The main indication for NB-UVB and PUVA therapies is psoriasis, and other key diagnoses include atopic eczema, vitiligo, cutaneous T-cell lymphoma (CTCL), and the photodermatoses. The decision on choice of phototherapy is important and NB-UVB is usually the primary choice. NB-UVB phototherapy is a safe and effective therapy which is usually considered when topical agents have failed. PUVA requires prior psoralen sensitization but remains a highly effective mainstay therapy, often used when NB-UVB fails, there is rapid relapse following NB-UVB or in specific indications, such as pustular or erythrodermic psoriasis. This review will provide a perspective on the main indications for use of NB-UVB and PUVA therapies and provide comparative information on these important dermatological treatments.

Evaluation of Biological Effects and Transcriptome Changes Induced by LED-Based Narrow-Band UVB Phototherapy

Abstract:Ultraviolet (UV), particularly UVB, is widely used in the treatment of skin diseases including psoriasis, atopic dermatitis, vitiligo, mycosis fungoides and pruritus. Recently, there has been a trend of replacing broad-band UVB (BB-UVB) units with narrow-band UVB (NB-UVB), as studies have demonstrated that NB-UVB is more efficacious in the treatment of psoriasis. The purpose of this study is to evaluate the biological effects and transcriptome changes induced by light-emitting diode-based NB-UVB (NB-UVB LED) phototherapy. Cell viability and the cell migration ability were significantly decreased posttreatment, as well as apoptosis and ROS levels were remarkably increased. NB-UVB-induced S phase arrest was observed 12 h postirradiation. Bioinformatics analysis of transcriptome sequencing data revealed that NB-UVB LED irradiation induced dose-depended changes in multiple key signaling pathways, such as PI3K and cytoskeletal-related pathways. The depolymerization of cytoskeleton induced by NB-UVB was observed 24 h posttreatment. In addition, the expression levels of cytoskeleton-related proteins FN1, ITGB4, ITGA1, RAC2 and DOCK1 decreased significantly 12 h after irradiation. Our results indicated that NB-UVB LED may serve as a novel option for the development of NB-UVB phototherapy devices.

Abstract:Abstract Vitiligo is the most common pigmentary disorder, and affects around 0.4% of the population in the UK. Recently conducted, two large retrospective observational studies using the UK Clinical Practice Research Datalink and Hospital Episode Statistics databases demonstrated that adults with vitiligo have an increased risk of subsequently being diagnosed with new-onset depression (25%) and anxiety (23%). The studies also found that people with both vitiligo and a mental health comorbidity have increased use of primary care services and are twice as likely to have recorded time off work and unemployment. Interestingly, the majority of patients (85%) were not prescribed any vitiligo-related treatments within a year of diagnosis for vitiligo. Second-line treatment for vitiligo includes either whole-body or localized phototherapy. Traditionally, narrowband ultraviolet B (NB-UVB) is reserved for people with extensive vitiligo and is delivered in secondary care using full-body units, requiring regular hospital attendance. In the current National Health Service (NHS) crisis, many NHS dermatology departments offer phototherapy either to a very limited number of patients with vitiligo or not at all, due to constraints on phototherapy services. The British Association of Dermatologists guidelines recommend that limited vitiligo can be treated with handheld NB-UVB devices. This recommendation was based on the results of a large, pragmatic, randomized controlled trial of home handheld phototherapy combined with topical corticosteroid for people with active, limited vitiligo (HI-Light trial). The trial was completed almost 5 years ago; however, only one centre in Wales chose to continue with the home handheld phototherapy service on the NHS after the completion of the trial. A new, unique, national, tertiary, research and treatment centre for children and adults with vitiligo has recently been established in England. In our centre, we offer home handheld phototherapy to eligible patients, who are often referred from out of area, therefore making a combination of topicals and NB-UVB more accessible. We designed patient-facing literature, a special pro forma for vitiligo clinic, and several other documents to assist in safety assurance and quality improvement processes. We describe the pathway that we followed when establishing this new service in the NHS. We have trained 20 patients so far. We describe the safety and efficacy results of home handheld phototherapy combined with topical treatments such as calcineurin inhibitors and corticosteroids, as well as oral antioxidants. We hope that our roadmap will pave the way for a home handheld phototherapy service in other centres, which would make phototherapy more accessible for patients with a variety of skin diseases such as vitiligo, hand eczema and scalp psoriasis, among others.

Abstract:Background: Laser therapies can cause hyper- and hypopigmentation of the skin. There is little evidence in the literature of effective treatments for these types of problems in Fitzpatrick skin phototypes IV–VI. The main aim of this retrospective study is to evaluate the effects of a new therapy that combines the application of electromagnetic fields and vacuum on a subject with Fitzpatrick skin phototype VI, who presented extensive, laser-induced facial dyschromia. The secondary aim is to test the effectiveness of a free imaging software for assessing skin pigmentation. Methods: The level of improvement after therapy was evaluated, with a 5-point Likert scale, one month after the end of the treatment by the subject and by the doctor who performed the treatment, and by two blinded dermatologists. With the free software, a three-dimensional reconstruction of the treated area and the evaluation of the color distribution were performed. Results: Both the subject and the doctors involved in the study positively evaluated the effects of the treatment. The image analysis highlighted the homogenization of the skin color in the treated area. Conclusions: The combination of electromagnetic fields and vacuum for dyschromia treatments appears promising. The new method of assessing melanin levels resulted particularly efficient.

Abstract:Background: The risk of developing non-melanoma skin cancers (NMSCs) in patients with psoriasis is highly debated, and, to date, there is no unambiguous consensus opinion. Psoriasis is known to be related to an increased likelihood of other comorbidities such as psoriatic arthritis, obesity, metabolic syndrome, depression, and cardiovascular disease. Regarding cancer risk, previous studies have reported a greater tendency for the development of cutaneous T-lymphomas and colon, breast, kidney, and lung cancers. Furthermore, data from network meta-analyses have shown that patients with psoriasis have a higher risk of developing squamous cell carcinomas (SCCs) and/or basal cell carcinomas (BCCs). Multiple factors may contribute to the development of NMSCs in psoriatic patients, ranging from immunosuppression induced by biologic agents to previous phototherapy. However, the extent to which each factor may impact this risk has not been entirely assessed. The aim of this study was to evaluate the risk of developing NMSCs in patients with psoriasis observed for at least 5 years, by directly comparing patients only treated with phototherapy and patients treated with anti-tumor necrosis factor α (TNFα) agents, naive to other systemic treatments or phototherapy. Methods: We conducted a single-center retrospective study at Siena University Hospital, Italy, on 200 adult patients with psoriasis divided into two groups: (i) group 1, including 100 patients treated with narrow-band UVB phototherapy (nb-UVB), and (ii) group 2, including 100 patients treated with anti-TNFα. The patients included in group 2 had to be naive to cDMARDs and biologics and treated with anti-TNFα continuously for 5 years without loss of efficacy. All patients were observed for 5 years and underwent annual dermatologic examinations to assess for the occurrence of BCC or SCC. Results: A total of 34 out of 100 patients treated with phototherapy had one BCC or one SCC and 10 out of 34 developed two skin cancers. In particular, five had both types (one BCC and one SCC), and five had two BCCs. Conclusions: The results of our study highlight how the risk of developing NMSCs is greater in patients undergoing phototherapy compared to those treated with anti-TNFα. It also draws attention to the consideration that patients with scalp psoriasis might need closer follow-up as they could be more at risk of developing NMSCs.

Beneficial UVB phototherapy preceding a fractional CO2 laser-UVB protocol in refractory non-segmental vitiligo

Abstract:Objectives: To explore the short- and long-term effects of UVB phototherapy preceding a fractional CO2 laser-UVB protocol in patients with resistant nonsegmental vitiligo. Materials and methods: This single-center, prospective, split-face/body, evaluator-blinded study included adult patients with stable vitiligo refractory to conventional treatments. Two symmetrical lesions were selected. Phototherapy was delivered with one side covered, until minimal erythema. Within 3 days, 31-month-apart sessions of laser were performed on both sides. After each laser session, phototherapy was resumed three times weekly, with all lesions uncovered, until 8 weeks after the last laser session. At baseline, at the end of treatment, and 5 years later, a Mean Improvement Score by Physician (MISP) and a patient satisfaction 10-point visual analog score (VAS) were recorded. Results: Ten patients (8 women and 2 men) were included; their mean age was 32.9 years; phototype III was predominant; the mean duration of vitiligo was 3 years. At the end of treatment and 5 years later, scores of lesions treated with UVB-CO2 -UVB (mean MISP 3.0 and 2.9, mean ΔVAS 4.5 and 3.9, respectively), were higher than those of lesions treated only with CO2 -UVB (mean MISP 2.5 and 2.4, mean ΔVAS 4.1 and 3.6, respectively). After 5 years, one patient lost his partial response and two patients developed light hyperpigmentation on both sides. Conclusion: Exposure to UVB before CO2 -UVB explains the higher scores as it was the only variable between the two sides. It may improve the response of resistant lesions with a constantly sustained result over 5 years.
Comparative study between excimer light and topical antioxidant versus excimer light alone for treatment of vitiligo

Abstract:BACKGROUND: Vitiligo is an acquired idiopathic cutaneous disease characterized by pearly white patches of variable shapes and sizes. Various medical and surgical therapeutic options have been proposed to achieve repigmentation; phototherapy is one of the most efficient options. Topical therapies have been a mainstay of vitiligo treatment, with or without phototherapy.AIM OF THE WORK: To compare the efficacy of combined topical antioxidant hydrogel and excimer light versus excimer light alone in treating vitiligo.PATIENTS AND METHODS: Thirty patients were included in this comparative, prospective, randomized study. For each patient, at least 2-4 vitiliginous macules were randomly selected and treated while an untreated vitiliginous macule served as control. Lesions were divided into two groups: Group A received combination therapy of daily topical antioxidant plus excimer light, while Group B received only excimer light. Lesions were treated twice a week for a maximum of 24 sessions. Initial fluencies were adjusted individually according to the minimal erythema dose in vitiliginous skin. Efficacy based on repigmentation percentages were blindly evaluated by two independent physicians.RESULTS: Group A lesions showed significant efficacy than group B (p < 0.001), specially on treating UV-sensitive lesions with no side effects.CONCLUSION: Topical antioxidant and excimer light represents a valuable, effective therapy for localized vitiligo.

Abstract:Abstract We evaluated response to thrice weekly nb-UVB for AD (maximum 40 exposures), to assess outcomes immediately post-treatment and 12 months later. Post-treatment follow up was 3 monthly for 12 months. We sub-analysed response in different ethnicities. 58 patients were recruited: 37 (64%) male, 21 (36%) female, age-range 4.1-16.9 (mean 10.6) years. Ethnicities were: White Caucasian 22 (38%), Southern Asian (Pakistani/Indian/Bangladeshi) 17 (29%), Black African/Caribbean 9 (16%), Other 10 (17%). Immediately post-treatment, EASI 75 was achieved in: Overall 49%, Caucasian 48%, Southern Asian 50%, African-Caribbean 44%, other 56%. Mean EASI reduction was: Whole group 10.0, Caucasian 12.4, Southern Asian 8.6, African-Caribbean 2.5, Other 7.5. 12 months post-treatment (compared to pre-treatment), EASI 75 was achieved in: Overall 28%, Caucasian 33%, Southern Asian 40%, African-Caribbean14%, other 13%. Mean EASI reduction was: Whole group 7.6, Caucasian 9.0, Southern Asian 8.6, African-Caribbean -2.1 (worsening), Other 8.0. Erythema was the most common side effect noted in 33% of patients, the incidence in Caucasians 50% (n=11), other 30% (n=3), African-Caribbean 22% (n=2) and Southern Asian 12% (n=2). A similar proportion achieved EASI 75 across the different skin types, however, more variation in EASI score reduction was noted in skin of colour. A lower incidence of post-treatment erythema noted in darker skin types may be due to erythema underscoring or treatment underdosing. Our study showed sustained AD improvement up to a year after stopping treatment in 28% overall, highest in Caucasian and Southern Asian patients, with greater risk of relapse noted in African-Caribbean patients.

Abstract:Abstract Narrowband ultraviolet B (NB-UVB) is recommended by NICE as a treatment for chronic plaque and guttate psoriasis that cannot be controlled with topical therapies. However, ultraviolet radiation is a carcinogen that causes DNA mutations in the skin, and in some cases results in skin cancer development. To date, there have been no studies determining how many mutations develop in human skin as a result of a therapeutic course of NB-UVB, and therefore we investigated the mutation burden of NB-UVB in the current study. Although whole-exome or whole-genome sequencing and target-enriched sequencing can detect mutations in large clones of mutated skin cells, these approaches do not detect mutations occurring only in single skin cells, hence we used a novel type of sequencing called Nanorate sequencing (which can identify mutations occurring in just a single molecule of DNA) in this study. We obtained 6-mm punch biopsies of normal skin (buttock and/or forearm) before and after a course of UV from patients (n = 16) undergoing NB-UVB treatment for psoriasis. Epidermis was microdissected from the dermis and the epidermal DNA was sequenced using nanorate sequencing. DNA from venous blood underwent whole-genome sequencing as germline control for accurate identification of mutations in the epidermal samples. The numbers of DNA mutations were compared between the skin samples taken before and after NB-UVB in order to quantify the mutation burden attributable to the course of NB-UVB. Minimal erythema doses ranged from 0.3 to 0.9 J cm−2 and the total dose received during the course of NB-UVB ranged from 14.8 to 133 J cm−2. There was a significant rise in the number of mutations in buttock and forearm skin due to the NB-UVB treatment, with median increases in single-base substitutions per megabase of 1.09 SBS/Mb (P = 0.001) and 3.77 SBS/Mb (P = 0.0098) in buttock and forearm epidermis, respectively. The change in number of mutations secondary to NB-UVB treatment was predominately due to UV signatures SBS7a and SBS7b, with the DBS1 (CC>TT mutations) double-base substitution signature also identified in buttock and forearm skin samples. Comparison of the increase in mutation burden from NB-UVB with the mutation burden in keratinocyte cancers indicated that, for more susceptible patients, a course of NB-UVB therapy contributed as much as one-sixteenth of the total mutational burden of a cutaneous squamous cell cancer.

Abstract:The narrow-band (NB-UVB) was developed for use in phototherapy, as an alternative to a broad-band UVB source and to photochemotherapy, both of which have significant side effects and carry a risk of carcinogenesis. NB-UVB is a new phototherapy option that has proved to be particularly effective at clearing psoriasis vulgaris, with a reduced capacity to produce erythema. This study was designed to explore the effects of NB-UVB on psoriasis vulgaris in comparison with the conventional UVB. 73 psoriasis patients were divided into treatment group (43 patients) and control group (30 patients). The treatment group was radiated with NB-UVB while the control group radiated with conventional UVB for 6 weeks, 3 times a week; in addition, 30 healthy volunteers were selected as healthy control group. Among the 43 patients of treatment group, 11 were clinically cured after radiated with NB-UVB, and the total effective rate was 83.7 %; while only 3 were clinically cured after radiated by conventional UVB among the 30 patients of control group, with a total effective rate of 37.8%.
