Abstract:Purpose: To examine prescription trends and patterns in five antihypertensive drug classes during 2007 - 2012 in China. Methods: A retrospective time series data was investigated from 2007 to 2012. Prescription data of five classes of antihypertensive drugs (calcium channel blockers (CCBs), angiotensin receptor blockers (ARBs), angiotensin-converting enzyme inhibitors (ACEIs), beta-blockers, and diuretics) were accessed from the Hospital Prescription Analysis Program Database, including 59 hospitals in four cities in China. The drugs were coded using the Anatomic Chemical Therapeutic (ATC) classification. Quantities were standardized using the defined daily dose (DDD) measurement methodology. Results: The total number of DDDs of the five classes of antihypertensive drugs prescribed in the sample hospitals increased by 83.3% and the total cost of antihypertensive drugs increased by 92.4% from 2007 to 2012. During the whole study period, CCBs, ARBs, ACEIs, beta-blockers, and diuretics represented in DDDs 42.8%, 28.3%, 13.3%, 10.8%, and 4.8%, respectively, of the total five classes of anti-hypertensive drugs. The average annual increase rate of ARBs, beta-blockers, CCBs, ACEIs, and diuretics, in DDDs, was 23.4%, 13.3%, 13.2%, 1.4%, and -4.2%, respectively. Conclusions: The consumption of the five classes of antihypertensive drugs in China nearly doubled from 2007 to 2012. The top-prescribed antihypertensive drug classes were CCBs and ARBs, and the latter increased most rapidly.

Abstract:Objective:To measure sales of antibiotics without a prescription in pharmacies in China in 2017 and 2021, before and during the coronavirus disease 2019 (COVID-19) pandemic, and determine the factors associated with such sales.Methods:We conducted cross-sectional surveys using the simulated patient method in retail pharmacies in 13 provinces in eastern, central and western China in 2017 and 2021. At the pharmacies, the simulated patients (trained medical students) reported that they had mild respiratory tract symptoms and asked for treatment, using a three-stage process: (i) request some treatment; (ii) request antibiotics; (iii) request specific antibiotics. We used multivariable logistic regression analysis to determine factors associated with sale of antibiotics without a prescription.Findings:Of the pharmacies visited in 2017, 83.6% (925/1106) sold antibiotics without a prescription; this figure was 78.3% (853/1090) in 2021 (P-value: 0.002). After excluding pharmacies prohibited from selling antibiotics because of COVID-19, this difference was not significant (83.6% versus 80.9%; 853/1054; P-value: 0.11). Factors significantly associated with selling antibiotics without a prescription in both 2017 and 2019 were: location in central and western China compared with eastern China; being in a township or village compared with in a city; and presence of a counter where antibiotics were dispensed.Conclusion:Although laws became stricter between 2017 and 2021, antibiotic sales without a prescription were still common in pharmacies across China. Existing regulations need to be more strictly enforced, and pharmacy staff and the public should be made more aware of the risks of antibiotic misuse and dangers of antimicrobial resistance.

Abstract:Background Access to antibiotics without a prescription from retail pharmacies has been described as a major contributor to anti-microbial resistance (AMR) globally. In the context of high rates of AMR, the Chinese government has recently introduced strict policies regarding hospital antibiotic use, but the existing ban on antibiotic sales without prescription in retail pharmacies has not been strongly enforced. In 2016, a goal of prescription-only antibiotics by 2020 was announced. The objective of the study was to determine progress towards the 2020 goal, through estimating the proportion of retail pharmacies selling antibiotics without prescription across the three regions of mainland China. Methods Using the Simulated Patient method, we conducted a cross-sectional survey across purposively-sampled retail pharmacies in urban and rural areas of 13 provinces in eastern, central and western China. Medical students presented a scenario of a mild upper respiratory tract infection, following a strict three-step protocol. They recorded the pharmacy characteristics, and details of their experience, including at which step antibiotics were offered. Results Complete data were obtained from 1106 pharmacies. Antibiotics were obtained in 925 (83.6, 95% CI: 81.5, 85.8%) pharmacies without a prescription, 279 (25.2%) at Stage 1 (symptoms only described), 576 (52.1%) at stage 2 (asked for antibiotics), and 70 (6.3%) at Stage 3 (asked for penicillin or cephalosporins). There were significant differences between provinces, with antibiotic access (at any stage) ranging from 57.0% (57/100) in Zhejiang (81/82) to 98.8% in Guizhou. However, there were no significant differences in access to antibiotics by level of city, county, township or village (P = 0.25), whether the pharmacy was part of a chain or independent (P = 0.23), whether a licensed pharmacist was attending (P = 0.82) or whether there was a sign saying that prescriptions were required for antibiotics (P = 0.19). Conclusions It is easy to obtain antibiotics without a prescription in retail pharmacies in China, despite the fact it is against the law. This must be addressed as part of the wider anti-microbial stewardship effort which could include intense enforcement of the existing law, supported by a public education campaign.

This included whether they were given an antibiotic and at what stage in the protocol, whether the pharmacy was part of a chain or independent, whether the attending pharmacist attending them had a licence, and whether there was a sign stating prescriptions were required for antibiotic. We used χ2 tests to compare categorical variables. Ethical approval was obtained from Zhejiang University Research Ethics Committee. Findings Complete data were obtained from 1106 pharmacies. Antibiotics were obtained in 925 pharmacies (83·6%, 95% CI 81·5–85·8). There were differences between provinces, ranging from 57% in Zhejiang to 98·8% in Guizhou There were no significant differences in access to antibiotics by level of city, county, or township (p=0·25), whether the pharmacy was part of a chain or independent (p=0·23), whether a licensed pharmacist was attending (p=0·82), or whether there was a standard sign saying that prescriptions were required for antibiotics (p=0·19). Pharmacists overall provided a poor standard of service, with very little questioning about the students' symptoms or no explanation about correct use of antibiotics. Interpretation It is very easy to obtain antibiotics without a prescription in retail pharmacies in China. Given that effective anti-microbial stewardship programmes have been introduced in many hospital settings in China, especially at tertiary level, the failure of the government to enforce the existing law on prohibition of antibiotic sales without prescription is all the more concerning. Such enforcement should be addressed as a matter of urgency, alongside a targeted public education campaign. Clinical Outcome and Medical Cost of Originator and Generic Antihypertensive Drugs: A Population-Based Study in Yinzhou, China

Abstract:Background: The substitution of generic drugs can effectively alleviate the rapid growth of drug costs; however, the clinical effectiveness and medical costs of originator products and generics were barely studied in China.Objectives: To compare the effectiveness of antihypertensive drugs and hypertension-related medical costs between originator and generic initiators in Yinzhou, China.Methods: We conducted a population-based retrospective cohort study using the Chinese Electronic Health Records Research in Yinzhou (CHERRY), from July 1, 2011, to December 31, 2018. Hypertension patients initiating with originator products were compared with patients initiating with generic counterparts. We used 1:1 propensity score matching to pair the two groups based on sociodemographic, clinical, and health service utilization variables. Cox proportional regression was adopted to compare the rate of hospitalization for hypertension-related cardiovascular disease between matched originator and generic initiators. Wilcoxon matched-pairs signed-rank test was used to compare annual hypertension-related medical costs.Results: Matched pairs (10,535) of patients were included in the comparative study of originator products and generics, corresponding to seven antihypertensive drugs including amlodipine, felodipine, nifedipine, irbesartan, losartan, valsartan, and metoprolol. The average age of patients included in the analysis was around 60 years (originator vs. generics initiators: from 59.0 vs. 59.1 years in losartan to 62.9 vs. 63.6 years in nifedipine). Higher hospitalization rates among originator initiators were observed for three calcium channel blockers (hazard ratio[95% CI]: amlodipine, 3.18[1.43, 7.11]; felodipine, 3.60[1.63, 7.98]; and nifedipine, 3.86[1.26, 11.81]; respectively). The remaining four out of seven drugs of the clinical endpoint estimates showed comparable outcomes between originator products and generics (hazard ratio[95% CI]: irbesartan, 1.19[0.50, 2.84]; losartan, 1.84[0.84, 4.07]; valsartan, 2.04[0.72, 5.78]; and metoprolol, 1.25[0.56, 2.80]; respectively). Higher median annual hypertension-related medical costs were observed in originator initiators (all p < 0.001), except for metoprolol (p = 0.646).Conclusion: We observed comparable or even better clinical outcomes and less medical cost associated with the use of antihypertensive generics compared to originator counterparts. This could help increase patient and provider confidence in the efficacy of generic medicines to manage hypertension diseases.

Abstract:OBJECTIVE: To determine the prevalence of and factors associated with dispensing antibiotics without a prescription in online and community pharmacies in China.METHODS: We conducted a nationwide cross-sectional study of online and community pharmacies in 27 cities and counties in nine provinces in China (selected by multistage sampling) from July 2017 to December 2018. We assessed sale of antibiotics without a prescription and quality of pharmacy services through simulated clients who asked to buy specific antibiotics. We compared the prevalence of sales between online and community pharmacies, and between location and features of community pharmacies.FINDINGS: Of 220 online and 675 community pharmacies, 174 (79.1%) and 586 (86.8%) sold antibiotics without a valid prescription, respectively. About half of the online pharmacies had a notice on their website about the illegality of selling prescription-only medicines without a prescription while none of the community pharmacists had. More online pharmacies without this notice dispensed antibiotics without a valid prescription (P < 0.001). Antibiotics' sale without a prescription was significantly less prevalent in provincial capital cities (71.6%; 161/225) than prefectural-level cities (95.1%; 214/225) and counties (93.8%; 211/225; P < 0.001). Most pharmacy staff did not ask for important information from clients before dispensing the antibiotic or provide them with necessary information about the antibiotic.CONCLUSION: Given the high proportion of sales of prescription-only medicines without a prescription, there is a need to strengthen enforcement of regulations, improve public education on antibiotics, train pharmacy staff and consolidate public involvement in antibiotic stewardship in retail pharmacies in China.

Abstract:In the present study,we aimed to survey whether Shanghai residents were willing to buy prescription drugs online.This study randomly surveyed the influence factors of Shanghai residents of different age,educational background,income,and occupation in the form of online questionnaires about whether they would buy prescription drugs online.Age had a significant effect on the awareness rate of online prescription drug purchases.There was no significant difference between the awareness rate of online prescription drug purchase and education,"whether the respondents are practitioners in pharmaceutical industry or not" and monthly income.Age and monthly income were related to the willingness of online prescription drug purchases."Whether the respondents are practitioners in pharmaceutical industry or not" was not correlated with the willingness of online prescription drug purchases.It is necessary to strengthen the popularity of online drug stores and online prescription drug purchases and reduce consumers' expenses on the above-mentioned drugs.
Medication-taking behavior in outpatients with primary hypertension in China:an epidemiological study of 14 capital cities

Abstract:Objective To investigate the medication-taking behavior of outpatients with primary hypertension in China,so as to lay a foundation for further control of hypertension.Methods Questionnaire investigations as well as physical examination were conducted among 11 861 outpatients with primary hypertension in 14 cities in China.The investigation items included general characteristics,history of diseases,history of drugs and drug-taking behavior.Physical examination included blood pressure,heart rate,height,weight,biochemical test and ECG.Results Of all the participants 69.9% were under treatment.There were 4.8% patients with free medical care,63.2% having the medical insurance,and 30.3% having to pay the medicine at their own expenses.When taking anti-hypertensive agents,only 81.2% patients follow doctor's advice.Efficacy,price,adverse effect and convenience were the main influential factors for choosing drugs,while doctor and nurses,family and friends and health education were the main personal factors.Seeking more medical advice,economic and convenience mainly influenced patients to choose the place for drug purchase.58 yuan per month for anti-hypertensive drugs was the highest charge they can accept,and the number of those with high income was higher than that of the patients with low income.Conclusions Price is an important factor for outpatients to choose drugs,so economic reasonable drugs are appropriate for Chinese hypertension patients.It is necessary to strengthen community health education,correct the arbitrariness for drug-taking in part of the patients,and improve the control rate of hypertension in China.

Abstract:Introduction: Online sales of antibiotics have increased public access to these medicines. This study aimed to analyze the online antibiotic purchase behavior of the Chinese residents and identify its associated factors. Methods: We conducted a nationwide cross-sectional online survey among Chinese community residents from January 20 to February 28, 2019. A structured questionnaire was used to collect data on their sociodemographic characteristics, health-related variables, and the online antibiotic purchase behavior in the past 3 months. Descriptive statistics and logistic regression analyses were used. The statistical analyses were performed using SAS version 9.4 (SAS Institute Inc.). Results: A total of 101,120 respondents were included in the analysis. The weighted prevalence of antibiotic purchase online was 3.71% (95% CI, 3.53–3.88%). Residents who purchased antibiotics online were more likely to be older (age≥65 years), be a male, live in rural areas, have a higher education level, report an excellent economic status, suffer from chronic diseases, and search for health information on the internet. Conclusion: Numerous residents had purchased antibiotics online in the past 3 months throughout China. We should pay more attention to this behavior. There is a need to strengthen regulation of antibiotic sales online and improve public education on antibiotic purchase online. More comprehensive information on antibiotic purchase online as well as the advantages and disadvantages of online sales of antibiotics should be investigated in the future studies.

Abstract:Objective To investigate use of oral antihypertensive drugs among community hypertensive patients in Shanghai and find out factors related to their unreasonable use to direct their clinical use. Methods Seven hundred and three hypertensive patients were surveyed with questionnaire by stratified cluster sampling at three neighborhoods and one village of Dahua community, Baoshan district, Shanghai during April to June 2009 to understand their use of antihypertensive agents, including kinds and forms of drugs, rationale of drug use. Results Five hundred and eighty-two (82. 8% ) of 703 hypertensive patients interviewed were using antihypertensive drugs, 271 (38. 5% ) of them used only one kind of non-compound antihypertensive drug, 182 (25.9%) used one kind of compound agent, 311 (53.4%) used two or more kinds of drugs in combination, including 117 patients ( 16. 8% ) used two kinds of agents combined and 12 patients ( 1.6% ) used three kinds of agents combined. Two hundred and sixty-six (47.5%) patients took orally calcium-channel blockers and 205 ( 35.2% ) used compound agents. Conclusions Frequency of combined use of two or more kinds of antihypertensive agents is reasonable and significantly higher than that of use of one kind of drug at Dahua community in Shanghai. Calcium channel blocker plays a predominant role in treatment for hypertension, non-long-acting compound agents are used in a higher proportion. But,guidelines for hypertension prevention and treatment are not so well complied with in local hypertensive patients. So, it is suggested that training for community physicians and management for standard use of antihypertensive agents at community should be strengthened further.
Investigation of drug use in newly-diagnosed hypertensive outpatients in secondary and tertiary hospitals in China

Abstract:Objective To investigate the drug use of newly-diagnosed hypertensive outpatients in secondary and tertiary hospitals in China. Methods The cross-sectional study was conducted from March to May 2011in 4departments(cardiology,general internal medicine,hypertension and geriatrics)of 129secondary and tertiary hospitals in29cities of China. Data of 4120hypertensive outpatients were provided by doctors,each of which registered 5consecutive outpatients undergoing treatments. Demographic characters,blood pressures and drug use data were collected from 4117patients,excluding 3cases of logical errors. Results Of the 4117subjects,515were initially diagnosed hypertensive patients and 98.3%received treatment. Of the 515newly-diagnosed hypertensive,109were classified as mild hypertension and 99.1%received treatment. The 4177prescriptions with at least one antihypertensive prescription frequencies are:angiotensin receptor blockers(ARB 71.5%),calcium channel blockers(CCB,32.6%),beta-blockers(18.1%),angiotensin-converting enzyme inhibitors(ACEI 9.3%)and diuretics(7.8%).Monotherapy accounted for 62.7% with ARB was most commonly applied,two-drug combination accounted for24.7% with CCB/ARB was the most prescribed and three-drug combinations accounted for 10.9% with beta-blockers/CCB/ARB was most prescribed. Conclusions Compared with the requirement of "2010Chinese guideline for the management of hypertension",there is a phenomenon of irrational drug use in newly-diagnosed hypertensive outpatients who mainly receive monotherapy or dual-combination treatment.

Abstract:Objective: To identify the effects of a digital healthcare solution for hypertension management at retail pharmacies by pharmacists. Methods: We developed a digital healthcare solution that includes BP telemonitor, an App, and a cloud database with algorithms to assist: 1) CVD risk screening, 2) personalized meal planning, 3) personalized health coaching, and 4) medication guidance and compliance during the pharmacy consultation service. This solution has been implemented in 5,067 retail pharmacies in 141 cities in China as a decision support system to help pharmacists manage their hypertensive members. During Jan 1 st 2015 to Dec 31 st 2016, a total of 1,817,594 patients received in-store hypertension care and 23,024 of them were selected in this analysis following 4 inclusion criteria: 1) baseline BP was hypertensive (≥ 140/90 mm Hg), 2) the frequency of in-store hypertensive care was ≥ 1 time per month, 3) the intervention duration between the first- and last-time hypertensive care was ≥ 1 month, and 4) baseline characteristics were complete. Dynamic changes of SBP, DBP, disease staging, and population distribution over time were the major end points in this real-world, retrospective analysis. Results: SBP and DBP of the study population (n=23,024) were reduced on average by 11.2 mm Hg (baseline: 151.7 ± 14.8 mm Hg, last time: 140.5 ± 18.8 mm Hg, P< 0.001) and 6.3 mm Hg (baseline: 91.2 ± 11.9 mm Hg, last time: 84.9 ± 12.7 mm Hg, P<0.001), respectively. A total of 13,001 (56.5%) patients demonstrated improved hypertension stages by the last time BP measures with 9,625 (41.8%) of them returning to the normal range. The principal BP control effect took place after the first-time intervention, followed by gradual BP reduction over time. Simulation analysis revealed linear descending relationship of both SBP and DBP to time, and the concentration distributions were moving towards the normal BP level. The linear fitting functions were: SBP, Y = -0.103x + 140.278; DBP, Y = -0.060x + 84.799. Conclusion: Digital healthcare solution is effective in assisting retail pharmacists to deliver community hypertension care in real world.

Abstract:In 2020, the National Center for Cardiovascular Diseases with National Committee on Hypertension Management in Primary Health Care in China issued revised national clinical practice guidelines on the management of hypertension in primary health care based on the 2018 Chinese guidelines for the management of hypertension. To evaluate adherence to the guidelines, this retrospective study assessed the real-world status of antihypertensive drug prescribing for Chinese patients with hypertension, classified by comorbidity: coronary heart disease, diabetes mellitus, heart failure, stroke, and renal disease. About 1088212 hypertensive patients who received their first prescription for antihypertensive therapy between January 2021 to June 2021, were obtained from a database of Hangzhou Kang Sheng Health Consulting CO., Ltd. Calcium channel blockers (CCBs) and angiotensin II receptor blockers (ARBs) were the most common drugs prescribed for each comorbidity subgroup. Whereas diabetes mellitus or renal disease is a compelling indication for use of renin-angiotensin system inhibitors, CCBs were often administered in these subgroups. The treatment pattern for patients with coronary heart disease was closely similar to that for the overall patient population. Beta-blockers (BBs) were prescribed more frequently for patients with heart failure than for those with other comorbidities. Although antihypertensive drug prescription varied by comorbidity, pharmacological decisions were largely made under Chinese recommendations while physicians could select antihypertensive drugs based on the patients' comorbidities. However, educational initiatives are still necessary to inspire clinicians to better familiarize themselves with the guidelines and manage hypertension comorbid diseases.

Abstract:BACKGROUND: Generic drugs are bioequivalent to their brand-name counterparts; however, concerns still exist regarding the effectiveness and safety of generic drugs because of small sample sizes and short follow-up time in most studies. The purpose of this study was to evaluate the long-term antihypertensive efficacy, cost-effectiveness and cardiovascular outcomes of generic drugs compared with brand-name drugs.METHODS: In a multicenter, community-based study including 7955 hypertensive patients who were prospectively followed up for an average of 2.5 years, we used the propensity-score-matching method to match the patients using brand-name drugs to those using generic drugs in a ratio of 1:2, 2176 patients using brand-name drugs and 4352 patients using generic drugs.RESULTS: There were no significant differences between generic drugs and brand-name drugs in blood pressure (BP)-lowering efficacy, BP control rate, and cardiovascular outcomes including coronary heart disease and stroke. The adjusted mean (95% confidence interval [CI]) of systolic BP (SBP)-lowering was -7.9 mmHg (95% CI, -9.9 to -5.9) in the brand-name drug group and -7.1 mmHg (95% CI, -9.1 to -5.1) in the generic drug group after adjusting for age, sex, body mass index, number of antihypertensive drugs and traditionally cardiovascular risk factors. Among patients aged <60 years, brand-name drugs had a higher BP control rate (47% vs. 41%; P = 0.02) and a greater effect in lowering SBP compared with generic drugs, with the between-group difference of 1.5 mmHg (95% CI, 0.2-2.8; P = 0.03). BP control rate was higher in male patients using brand-name drugs compared with those using generic drugs (46% vs. 40%; P = 0.01). Generic drugs treatment yielded an average annual incremental cost-effectiveness ratio of $315.4 per patient per mmHg decrease in SBP compared with brand-name drugs treatment.CONCLUSIONS: Our data suggested that generic drugs are suitable and cost-effective in improving hypertension management and facilitating public health benefits, especially in low- and middle-income areas.

How willing are the public to pay for anti-hypertensive drugs for primary prevention of cardiovascular disease: a survey in a Chinese city.

Abstract:BACKGROUND:Current recommendations on drug treatment of hypertension for primary prevention of cardiovascular disease are primarily determined by the evidence of effectiveness, disregard the resources available and values of people, and recommend a universally fixed risk cutoff for initiating drug treatment. The guidelines may have over-estimated the willingness of the public to accept and pay for these drugs and a fixed cutoff may not fit all populations. Moreover, the public may have been misinformed and are unable to make the right decision even if they are consulted. We conducted this study to address these issues and to describe the gap between current policy and what the public truly want. METHODS:A cross-sectional survey with face-to-face interviews of rural and urban residents in northern China. Before providing any information, we asked the residents whether they would accept drug treatment if they had hypertension and also asked them to estimate the 5-year cardiovascular risk in untreated hypertension and the benefits from anti-hypertensive drugs. We then informed the participants of necessary information and asked them above what benefit they would be willing to pay the current cost, and how much they would be willing to pay for the actual benefit, for anti-hypertensive drugs out of pocket. RESULTS:Eight hundred and eighty-seven rural residents and 921 urban residents were interviewed with a response rate of 97%. Ninety-five percent [95% confidence interval (CI) 94-96%] of the residents said they would take anti-hypertensive drugs if they had hypertension, although 91% (95% CI 89-92%) said they did not have sufficient knowledge to make a decision. Seventy-eight percent (95% CI 76-80%) believed that anti-hypertensive drugs were primarily to lower blood pressure or relieve symptoms. They over-estimated the cardiovascular risk of untreated hypertension by approximately 12 times and the absolute benefit of drug treatment by 20 times. Given the actual absolute benefit of the drugs, only 23% (95% CI 21-25%) were willing to pay the current annual cost of $500 Ren Min Bi (US$73.3, euro 54.8 as of 8 May 2009) for these drugs. Given the current cost, they were, on average, willing to pay for the drugs only when the 5-year cardiovascular disease risk was as high as 35% (95% CI 31-38%) or even higher. CONCLUSION:The public in China are significantly misinformed and considerably over estimate the risk of hypertension and the benefit of treatment. The public's willingness to pay for anti-hypertensive drugs is much lower than the current guidelines implicitly assume. The willingness to pay should be considered, along with other factors, when prescribing anti-hypertensive drugs to an individual patient or making hypertension guidelines for a population.
Gap Between Evidence and Physicians' Knowledge and Practice Regarding Hypertension and Its Drug Treatment: A Survey in A Chinese City

Abstract:Background Evidence-based medicine has come into its second decade. How prepared clinicians are in practicing it in particular in developing countries remains unclear. Thus we conducted this survey of physicians in urban hospitals in China to determine the size of the gap between research evidence and physicians' knowledge and practice regarding antihypertensive drugs for primary prevention of cardiovascular diseases in China.Methods A cross sectional survey by a face-to-face interview was conducted in 20 tertiary general hospitals in China in 2005. A total of 444 physicians (mostly cardiologists) in internal medicine who had treated at least one hypertensive patient in the past 12 months were invited for the interview on their perception of the cardiovascular risk of hypertension, the magnitude of the benefit of antihypertensive drugs, knowledge on the overall risk approach, first-line drugs used, the risk above which drug treatment is recommended, and knowledge on evidence-based medicine.Results A total of 444 of the 468 eligible physicians were successfully interviewed with a response rate of 94.9%. They estimated that a hypertensive man with an actual 5-year cardiovascular risk of 8.4% would have a 5-year cardiovascular risk of 40% (95% Cl: 38% to 42%) if not treated, and have an absolute risk reduction and relative risk reduction from drug treatment by 20% (95% Cl: 18% to 22%) and 39% (95% Cl: 37% to 42%) respectively, as compared to 3.3% and 33% respectively shown in research evidence. On average, the physicians would recommend drug treatment at a number needed to treat (NNT) of 368 or smaller, as compared to the actual NNT of 50 for drug treatment in an average hypertensive Chinese. Fifty-five percent (95% Cl: 50% to 59%) of them had never intently used the national hypertension guidelines. The majority still prescribed drugs primarily based on blood pressure alone by ignoring other risk factors or the overall risk and 78% (95 % Cl: 76% to 83%) used new expensive drugs such as calcium channel blockers and angiotensin converting enzyme (ACE) inhibitors as first-line treatment. Only 13% (95% Cl: 9% to 18%) could correctly interpret the NNT. Forty-three percent (95% Cl: 39% to 48%) did not know the randomized controlled trial was scientifically the most rigorous among other study designs for evaluating the effectiveness of anti-hypertensive drugs. Ninety-two percent (95% Cl: 90% to 94%) did not know they could start by searching systematic reviews when looking for evidence on the effectiveness of anti-hypertensive drugs as opposed to trials. Ninety-six percent (95% Cl: 94% to 98%) did not know the Cochrane Library was an important source of systematic reviews.Conclusions The surveyed physicians significantly over-estimated the cardiovascular risk of hypertension and the benefit of drug treatment, and had insufficient knowledge on the overall risk approach. They recommended drug treatment at a cardiovascular risk which was even much lower than the cutoff suggested for western populations, which would make many more people eligible for drug treatment. They also tended to prescribe new expensive drugs although the older cheaper ones may be more appropriate in many patients. Abstract:Objectives: The study aimed to estimate the effects of National Volume-based Drug Procurement (NVBP) policy on drug utilization and medical expenditures of hypertension patients in public medical institutions in mainland China. Methods: This study used patient-level data based on electronic health records retrieved from the hospital information system of Nanjing Hospital of Chinese Medicine. Data on patients with hypertension who received care at this institution between 2016 and 2021 was used for analysis. Segmented linear regression models incorporating Interrupted Time Series (ITS) analysis were adopted to examine the effects of NVBP policy on drug utilization and health expenditures of eligible patients. Drug utilization volume and health expenditures were the primary outcomes used to assess the policy effects, and were measured using the prescription proportion of each drug class and the overall per-encounter treatment costs. Results: After the implementation of NVBP policy, the volume of non-winning drugs decreased from 54.42% to 36.25% for outpatient care and from 35.62% to 15.65% for inpatient care. The ITS analysis showed that the volume of bid-winning drugs in outpatient and inpatient settings increased by 9.55% ( p < 0.001) and 6.31% ( p < 0.001), respectively. The volume changes in non-volume based purchased (non-VBP) drugs differed between outpatients and inpatients. The proportion of non-VBP drugs immediately increased by 5.34% ( p = 0.002) overall, and showed an upward trend in the outpatient setting specially ( p < 0.001) during the post-intervention period. However, no significant differences were observed in the proportion of non-VBP drugs in inpatient setting ( p > 0.05) in term of level change ( p > 0.05) or trend change ( p > 0.05). The average per-visit expenditures of outpatients across all drug groups exhibited an upward trend ( p < 0.05) post policy intervention. In addition, a similar increase in the overall costs for chemical drugs were observed in inpatient settings (coefficient = 2,599.54, p = 0.036), with no statistically significant differences in the regression slope and level ( p = 0.814). Conclusion: The usage proportion of bid-winning drugs increased significantly post policy intervention, indicating greater use of bid-winning drugs and the corresponding substitution of non-winning hypertensive drugs. Drug expenditures for outpatients and health expenditures per visit for inpatients also exhibited an upward trend, suggesting the importance of enhanced drug use management in Traditional Chinese Medicine hospital settings.

Abstract:OBJECTIVE: To assess the prevalence, awareness, treatment, and control of hypertension and their associated factors in an urban Chinese population.METHODS: A cross-sectional study was conducted in three cities in northeast China from 2009 to 2010, using a multistage cluster sampling method to select a representative sample. A total of 25 196 adults, aged 18-74 years, were examined in 33 communities. Hypertension was defined as a mean SBP of at least 140 mmHg, DBP at least 90 mmHg, and/or use of antihypertensive medications.RESULTS: Overall, the prevalence of hypertension was 28.7% for urban residents, and 39.1% for middle-aged and elderly residents (aged ≥35 years). Among all the hypertensive patients examined in the study (n = 7237), 42.9% were aware of their condition, 28.2% were receiving treatment, and only 3.7% had their blood pressure adequately controlled. Female hypertensive patients had more effectively controlled blood pressure than their male counterparts. Among the study participants, 37.9% did not think that high blood pressure would endanger their lives. Among hypertensive patients aware of their conditions, the primary reason for not taking antihypertensive medication was a lack of money (34.8%). Age, sex, education, occupation, income, body mass, waist circumference, and family hypertension history significantly correlated with the prevalence of hypertension.CONCLUSION: Hypertension is highly prevalent in the urban population of China, and the effects of being overweight/obesity on hypertension were much larger than any other examined factors. The percentage of hypertensive patients aware of their condition, receiving proper treatment, and keeping their hypertension under control is unacceptably low.

Abstract:PURPOSE: Essential hypertension, as a complex disorder with unknown etiology cause, is a major public health problem worldwide. Patients need constant drug therapy to maintain their blood pressure in a normal range. However, the current facts suggest that the treatment is not optimized in a large number of patients, and as a result they are at risk for compliance resulting in uncontrolled blood pressure. Genetic and environmental factors associated with individual variation in response to anti-hypertensive drug remain largely unknown.METHODS: In order to illustrate the existence and to attempt to identify the factors modifying drug effect, we conducted a large-scale follow-up study in two Chinese rural counties differing in both genetic background and residential environment. Hypertensive patients were treated with benazepril, a commonly used angiotensin converting enzyme inhibitor, for 15 days, and the end-point effect was evaluated.RESULTS: We found that there were large and significant differences in drug response between subjects from two counties, even after adjustment for known factors. The responses to benazepril, measured in diastolic blood pressure drop, in male patients from Yuexi was twice as effective as their counterparts from Huoqiu.CONCLUSIONS: These results suggest that adjustment of treatment regimen is necessary to improve efficacy, and it could be done at the population level to make it more feasible and affordable.
