Abstract:Background. Previous studies showed that there are gender disparities in various respects of acute myocardial infarction (AMI), including risk factors, symptoms, and outcomes. However, few of them noticed the gender disparities in patients’ decision about the management of AMI, which might also be associated with the outcome. Aims. To identify gender disparities in patients’ decisions about the management of myocardial infarction. Methods. In this cohort study, the critical time points including the time of symptom onset, visiting hospital, diagnosis of AMI, consent to coronary angiography (CAG), beginning of CAG, and balloon dilation were recorded. Medication and major adverse cardiac event (MACE) within 6 months were also recorded. Results. Female patients took more time from symptom onset to visiting hospital (P=0.001), from diagnosis of AMI to consent to CAG (P<0.05), and from door to needle/balloon than male (P<0.05). Less female patients accepted CAG (P<0.05) and coronary intervention/bypass grafting (P<0.05). Less female patients kept good inherence to antiplatelet therapy (P<0.05) and statins (P<0.05) than male, more female preferred traditional Chinese medicine (TCM) than male patient (P<0.05), and most of them had MACE within 6 months (P<0.05). Patients’ good adherence to antiplatelet therapy and statins and accepting coronary intervention/bypass grafting were associated with a reduced risk of MACE. Conclusion. Female patients were more reluctant to make decisions about emergency management of AMI and tended to choose conservative treatment. More female patients preferred TCM than evidence-based medicine. Their reluctance about the critical management of AMI and poor adherence to evidence-based medicine were associated with an elevated risk of MACE.

Abstract:<h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Study objective</h3><p>We compare clinical management and outcomes of emergency department (ED) encounters by sex after implementation of a clinical care pathway in 15 community EDs that standardized recommendations based on patient risk, using the History, ECG, Age, Risk Factors, and Troponin (HEART) score.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Methods</h3><p>This was a retrospective analysis of adult ED encounters evaluated for suspected acute coronary syndrome with a documented HEART score from May 20, 2016, to December 1, 2017. The primary outcomes were hospitalization or 30-day stress testing. Secondary outcomes included 30-day acute myocardial infarction or all-cause death (major adverse cardiac event). A generalized estimating equation regression model was used to compare the odds of hospitalization or stress testing by sex; we report HEART scores (0 to 10) stratified by sex and describing major adverse cardiac events.</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Results</h3><p>A total of 34,715 adult ED encounters met the inclusion criteria (56.0% women). A higher proportion of women were classified as low risk (60.5% versus 52.4%; odds ratio [OR] 1.39; 95% confidence interval [CI] 1.33 to 1.45). Women were hospitalized or received stress testing less frequently than men for low HEART scores (18.8% versus 22.8%; OR 0.79; 95% CI 0.73 to 0.84) and intermediate ones (46.7% versus 49.7%; OR 0.88; 95% CI 0.83 to 0.95), but similarly for high-risk ones (74.1% versus 74.4%; OR 0.99; 95% CI 0.77 to 1.28). Women had 18% lower odds of hospitalization or noninvasive cardiac testing (OR 0.82; 95% CI 0.78 to 0.86), even after adjusting for HEART score and comorbidities. Men had higher risks of major adverse cardiac events than women for all HEART score categories but the risk for men was significantly higher among low-risk HEART scores (0.4% versus 0.1%).</p><h3 class="u-h4 u-margin-m-top u-margin-xs-bottom">Conclusion</h3><p>Women with low-risk HEART scores are hospitalized or stress tested less than men, which is likely appropriate, and women have better outcomes than men. Use of the HEART score has the potential to reduce sex disparities in acute coronary syndrome care.</p>

Direct luteotrophic effect of oestradiol-17 beta on pig corpora lutea.

Abstract:Five gilts each received intraluteal implants releasing about 4.4 +/- 1.1 (Group 1), 15.0 +/- 1.1 (Group 2) or 22.4 +/- 1.0 (Group 3) micrograms oestradiol/day as determined by in-vitro incubation of implants of similar weight and oestradiol content. On Day 11 of the oestrous cycle (Day 0 = first day of oestrus), 3 CL in one ovary received oestradiol implants, 3 CL in the other ovary received vehicle implants, and all other CL in both ovaries served as uninjected control CL. An additional group of 6 animals served as controls and included 4 animals receiving bilateral vehicle implants (3 CL per ovary) on Day 11 and 2 unoperated gilts. All animals were slaughtered on Day 19 of their oestrous cycle, and the weight, progesterone content and concentration of each CL were determined. In Group 3 gilts, luteal weight, progesterone content and concentration were greater by 68.7 +/- 24.0 mg, 6.54 +/- 1.33 micrograms and 7.54 +/- 2.00 ng/ml respectively (P less than 0.01) in oestradiol-implanted CL than in vehicle-implanted CL, which appeared to be similar to uninjected control CL. No differences were seen between oestradiol-17 beta and vehicle-treated CL in CL from gilts in Groups 1 or 2. All CL of Group 3 gilts were heavier and contained a greater content and concentration of progesterone (P less than 0.01) than gilts in Groups 1 and 2, and in controls for which the values were similar.(ABSTRACT TRUNCATED AT 250 WORDS)

Introduction Women who suffer Acute Coronary Syndrome (ACS) have worse prognosis, with higher mortality and more complications. These results might be partially influenced by an older age and a higher number of comorbidities in women. Purpose The aim of the study is to analyse gender differences among patients with ACS admitted to our Coronary Care Unit (CCU). Methods Retrospective cohort of all patients with ACS admitted to a CCU between July 2011 and August 2022. Univariate analyses to assess gender influence in intrahospital mortality and complications were carried out. We also performed a multivariate analysis to identify predictive factors of intrahospital mortality. Results 3161 patients were included, with a mean age of 63 years old. Women accounted for 26.4% of patients. Women were older than men (68.9 vs. 61.71 years old), had a higher prevalence of hypertension (72.52% vs. 55.92%), diabetes mellitus (37.5% vs. 32.31%), heart failure and chronic kidney disease. On the other hand, smoking and chronic obstructive pulmonary disease were more frequent in men. In-hospital mortality was higher in women (OR 2.18, IC95% 1.63-2.92; p<0.001). In respect of complications, women showed a higher rate of bleeding (OR 1.35, IC95% 1.05-1.73; p<0.017) and cardiogenic shock (OR 1.43, IC95% 1.1-1.84; p<0.006). There were no significant gender-differences regarding ischemic complications (reinfarction, stent thrombosis, ischemic stroke...), atrial fibrillation or ventricular arrhythmias during hospital admission. Treatment differences were observed in the study. Prasugrel, statins, beta-blockers and IECAS were less prescribed in women, while clopidogrel was prescribed more frequently for them. Ticagrelor was prescribed less for women at hospital discharge but no gender differences in in-hospital prescription were identified. Multivariate analysis adjusting for age, comorbidities and complications demonstrated that diabetes mellitus, age, previous chronic kidney disease, severe bleeding and Killip-Kimball category were predictors of mortality. Female sex however did not reach significance in multivariate analysis (OR 1.28, IC95% 0.89-1.84; p=0.169). Conclusion In our sample, women had higher intrahospital mortality and bleeding rates, although mortality differences disappeared after adjusting by age, complications and comorbidities. Pharmacological treatment was lower and less intensive in them. It is crucial to know these differences in order to guarantee a more equitable medical assistance.

Gender Differences in Use of Invasive Diagnostic and Therapeutic Procedures for Acute Ischaemic Heart Disease in Chinese Adults

Abstract:Objective To investigate gender differences in the use of diagnostic and therapeutic procedures for acute ischaemic heart disease (IHD) in Chinese adults and assess whether socioeconomic or health system factors contribute to such differences. Methods In 2004-2008, the China Kadoorie Biobank recruited 512 726 adults from 10 diverse areas in China. Data for 38 928 first hospitalisations with IHD (2911 acute myocardial infarction (AMI), 9817 angina and 26 200 other IHD) were obtained by electronic linkage to health insurance records until 31 December 2016. Multivariate Poisson regression models were used to estimate women-to-men rate ratios (RRs) of having cardiac enzyme tests, coronary angiography and coronary revascularisation. Results Among the 38 928 individuals (61% women) with IHD admissions, women were less likely to have AMI (5% vs 12%), but more likely to have angina (26% vs 24%) or other IHD (69% vs 64%). For admissions with AMI, there were no differences in the use of cardiac enzymes between women and men (RR=1.00; 95% CI, 0.97 to 1.03), but women had lower use of coronary angiography (0.80, 0.68 to 0.93) and coronary revascularisation (0.85, 0.74 to 0.99). For angina, the corresponding RRs were: 0.97 (0.94 to 1.00), 0.66 (0.59 to 0.74) and 0.56 (0.47 to 0.67), respectively; while for other IHD, they were 0.97 (0.94 to 1.00), 0.87 (0.76 to 0.99) and 0.61 (0.51 to 0.73), respectively. Adjusting for socioeconomic and health system factors did not significantly alter the women-to-men RRs. Conclusions Among Chinese adults hospitalised with acute IHD, women were less likely than men to have coronary angiography and revascularisation, but socioeconomic and health system factors did not contribute to these differences.

Abstract:Few studies have assessed sex differences in the management of suspected acute coronary syndrome (ACS). We aimed to compare the evaluation, treatment, and outcomes between males and females with suspected ACS in the emergency department. Data were obtained from a prospective registry of acute chest pain involving 21 emergency departments in Shandong Province, China. The primary endpoint was 30-day major adverse cardiac events (MACEs). Overlap propensity score weighting was used to address potential confounding. A total of 8046 patients were analysed (42.8% female). Overlap-weighted analysis showed no significant association of female sex with 30-day MACEs (odds ratio, 0.91; 95% CI 0.75 to 1.11; P = 0.363). Secondary analyses found that women were less likely to be identified as high risk at first presentation (odds ratio, 0.86; 95% CI 0.78 to 0.94; P < 0.001). In the emergency department, women were less likely to undergo antiplatelet therapy (odds ratio, 0.87; 95% CI 0.79 to 0.96; P = 0.004) or coronary angiography (odds ratio, 0.78; 95% CI, 0.69 to 0.88; P < 0.001). Women had a longer length of stay in the emergency department and were less likely to be admitted to a ward at disposition. These sex differences existed only in the non-ST-elevation subgroup and were independent of risk stratification. Women with non-ST-elevation chest pain in China received suboptimal treatment in the emergency department. However, their clinical outcomes were not significantly different from those of men. Further studies are needed to determine the causes and impacts of these sex differences.

Abstract:Background Whether women have a higher risk of adverse events compared with men following coronary angiography (CAG) and percutaneous coronary intervention (PCI) remains controversial. We aimed to investigate the sex differences in characteristics, treatments and outcomes among patients undergoing CAG and PCI in a large Chinese cohort. Methods We analyzed patients undergoing CAG and/or PCI in this multi-center registry cohort study Cardiorenal ImprovemeNt II (CIN-II) in 5 Chinese tertiary hospitals from 2007 to 2020. Clinical characteristics, treatment (discharge medication and PCI) and in-hospital outcomes (mortality and major bleeding) were compared between women and men. Results Totally 141,459 patients underwent CAG (44,362 [31.4%] women), of which 69,345 patients underwent PCI (15,376 [22.2%] women). Women were older (64.4 vs. 60.8 years), had more chronic comorbidities and lower PCI rate for stable coronary artery disease (CAD) than men (52.8 vs. 64.2%). Women received less CAG and PCI procedures. Among women undergoing PCI they received similar discharge medication treatment. In addition, women undergoing PCI had mildly lower rate of major bleeding (0.2 vs. 0.3%, P = 0.033) but higher in-hospital mortality (1.2 vs. 0.8%, P < 0.001). After adjustment, women had a higher risk in the major bleeding (adjusted odds ratio, 2.04 [95% CI: 1.07 to 3.62]), and the in-hospital mortality (adjusted odds ratio, 1.87 [95% CI: 1.36 to 2.56]). Conclusion Among our Chinese cohort, women are older with more chronic comorbidities, receiving less PCI procedure and similar discharge medication treatment. Women have nearly 90% higher risk of in-hospital mortality and over 1-fold increased risk of major bleeding after PCI compared with men.

Exploring Gender Bias in Cardiovascular Medical Education Through Clinical Simulation

Abstract:While it has been proven that women suffer disproportionately from cardiovascular disease-related deaths, the origins of this differential gender-based outcome remain unidentified. One possible cause is gender bias and associated discrepancies in how physicians assess the need for interventions in male versus female patients. This study aimed to identify early gender biases in cardiovascular care by assessing medical students' management of a ruptured abdominal aortic aneurysm case presentation in male and female simulated patients. Clerkship students (n = 187) were randomly assigned to either a male or female patient with identical case presentations, simulated using high-fidelity mannequins. Minutes passed until point-of-care ultrasound (POCUS) usage served as a surrogate for diagnostic reasoning, while minutes passed until surgery consultation call served as a surrogate for successful intervention. Two-way ANCOVA of time to surgery call and POCUS use showed no significant interaction between student and patient gender (p=0.819). Likewise, neither patient gender (p=0.210) nor student gender (p=0.653) had an impact on ultimate correct diagnosis. However, there appeared to be an association (p=0.010) between patient and student gender in the factorial ANOVA of POCUS use, F(1,183) = 6.862 effect size 0.36. While slight in-group bias was identified within the context of imaging, students predominantly called for the correct intervention regardless of their own or their patient's gender. It is thus unlikely that medical students develop gender biases serious enough to impact clinical outcomes during clerkship- instead, these heuristics may be formed later in training.
Gender Differences Among Very Young Patients With Acute Coronary Syndrome: Long-Term Follow-Up of the STAMINA Study

Abstract:Angiology, Ahead of Print. The rate of hospitalization for acute coronary syndrome (ACS) among young patients is increasing. Healthcare disparities remain unsolved among female patients. We explored gender differences regarding risk factors, clinical presentation, in-hospital treatment, and long-term outcomes among ACS patients. A total of 445 patients with very early ACS (men ≤ 35 years and women ≤ 40 years of age) were followed for a median of 5 years. Primary clinical endpoint was the composite of cardiac death, non-fatal myocardial infarction, stroke, and coronary revascularization. Women accounted for 16% of cases. Smoking was the most prevalent risk factor, 56% and 60% of the females and males, respectively, continued to smoke after ACS. Chest pain was typical in 85% and 83% of the female and male patients, respectively. In-hospital treatment (pharmacological and reperfusion) as well as the composite clinical endpoint during follow-up did not differ between female and male patients. Lipid-lowering therapy was suboptimal in both genders, and persistence of smoking was the sole predictor for the composite clinical endpoint (hazard ratio: 2.30 [95% CI: 1.26–4.20]; P = .007). In conclusion, in-hospital treatment was similar between male and female patients. However, the majority of them continued smoking, and this was an independent predictor for future adverse outcomes.

Sex differences in the prevalence of peripheral artery disease in patients undergoing coronary catheterization

Abstract:To determine whether there are sex differences in the prevalence of peripheral artery disease, we performed an observational study of 1014 men and 547 women, aged ≥ 40 years, referred for elective coronary angiography. Women were slightly older, more obese, had higher low-density lipoprotein cholesterol (LDL-C) levels and systolic blood pressure (BP), and were more likely to be African American. Women had higher high-density lipoprotein cholesterol (HDL-C) levels, lower diastolic BP, and were less likely to smoke or to have a history of cardiovascular disease. Women had less prevalent (62% vs 81%) and less severe coronary artery disease (CAD) (p < 0.001 for both) by coronary angiography, but more prevalent peripheral artery disease (PAD) as determined by the ankle-brachial index (ABI) than men (23.6% versus 17.2%). Independent predictors of lower ABI were female sex, black race, older age, tobacco use, CAD, diabetes, and triglyceride level. In a full multivariable logistic regression model, women had a risk-adjusted odds ratio for PAD of 1.78 (95% CI 1.25-2.54) relative to men. Among patients referred for coronary angiography, women have less prevalent and less severe CAD, but more prevalent PAD, a sex difference that is not explained by traditional cardiovascular disease risk factors or CAD severity. Abstract:BACKGROUND:A recent randomized trial reported fractional flow reserve (FFR)-guided percutaneous coronary intervention (PCI) strategy was noninferior to the intracoronary ultrasound (IVUS)-guided PCI strategy with respect to clinical outcomes with fewer revascularizations. OBJECTIVES:This study sought to investigate the sex differences in treatment and clinical outcomes according to physiology- or imaging-guided PCI strategies. METHODS:In this secondary analysis of the FLAVOUR (Fractional Flow Reserve or Intravascular Ultrasonography to Guide PCI) trial, the impact of sex on procedural characteristics, PCI rate, and outcomes according to different strategies and treatment types (PCI vs deferral of PCI) was analyzed. The primary outcome was target vessel failure (TVF) at 24 months, defined as a composite of cardiac death, target vessel myocardial infarction, and target vessel revascularization. RESULTS:Of 1,619 patients, 30% were women. Compared with men, women had a smaller minimal lumen area, smaller plaque burden, and higher FFR. They had a lower PCI rate (40.8% vs 47.9%; P = 0.008), which was mainly contributed by FFR guidance. Overall, women showed a lower TVF rate (2.4% vs 4.5%). According to the treatment type, the cumulative incidence of TVF was lower in women than in men among those with the deferral of PCI (1.7% vs 5.2%). However, this trend was not observed in patients who underwent PCI. In both women and men, there were no differences in clinical outcomes between the FFR- and IVUS-guided strategies. CONCLUSIONS:In cases of intermediate stenosis, despite receiving fewer interventions, women had more favorable outcomes than men. The use of FFR led to a lower PCI rate but had a similar prognostic value compared with IVUS in both women and men.

Abstract:BACKGROUND: Coronary heart disease is a leading cause of mortality among women. Systematic evaluation of the quality of care and outcomes in women hospitalized for acute coronary syndrome (ACS), an acute manifestation of coronary heart disease, remains lacking in China. METHODS: The CCC-ACS project (Improving Care for Cardiovascular Disease in China-Acute Coronary Syndrome) is an ongoing nationwide registry of the American Heart Association and the Chinese Society of Cardiology. Using data from the CCC-ACS project, we evaluated sex differences in acute management, medical therapies for secondary prevention, and in-hospital mortality in 82 196 patients admitted for ACS at 192 hospitals in China from 2014 to 2018. RESULTS: Women with ACS were older than men (69.0 versus 61.1 years, P<0.001) and had more comorbidities. After multivariable adjustment, eligible women were less likely to receive evidence-based acute treatments for ACS than men, including early dual antiplatelet therapy, heparins during hospitalization, and reperfusion therapy for ST-segment-elevation myocardial infarction. With respect to strategies for secondary prevention, eligible women were less likely to receive dual antiplatelet therapy, angiotensin-converting enzyme inhibitors/angiotensin receptor blockers, statins at discharge, and smoking cessation and cardiac rehabilitation counseling during hospitalization. In-hospital mortality rate was higher in women than in men (2.60% versus 1.50%, P<0.001). The sex difference in in-hospital mortality was no longer observed in patients with ST-segment-elevation myocardial infarction (adjusted odds ratio, 1.18; 95% CI, 1.00 to 1.41; P=0.057) and non-ST-segment elevation ACS (adjusted odds ratio, 0.84; 95% CI, 0.66 to 1.06; P=0.147) after adjustment for clinical characteristics and acute treatments. CONCLUSIONS: Women hospitalized for ACS in China received acute treatments and strategies for secondary prevention less frequently than men. The observed sex differences in in-hospital mortality were mainly attributable to worse clinical profiles and fewer evidence-based acute treatments provided to women with ACS. Specially targeted quality improvement programs may be warranted to narrow sex-related disparities in quality of care and outcomes in patients with ACS.

Abstract:Background Gender differences in clinical presentation, natural history, and prognosis of coronary artery disease (CAD) have been reported. Fractional flow reserve (FFR) has also been shown to yield higher values in women than men. However, the gender differences in CAD patterns (focal vs diffuse) defined by coronary physiology remain to be studied. Methods This prospective study was performed at 23 sites and enrolled patients with at least one epicardial lesion with an FFR ≤ 0.80 intended to be treated by PCI. All patients underwent a standardized physiological protocol that mandated FFR pullbacks with pullback pressure gradient (PPG) calculation. The aim was to compare CAD patterns, defined by PPG, between genders. Results Overall, 1004 patients were included in the analysis, 236 females (250 vessels) and 768 males (794 vessels). Women were older (69.8 ± 10.3 vs. 67.0 ± 10.1, p <0.001), less frequently smokers (9.7% vs. 18.6%, p = 0.002), had less prior PCI in a non-target vessel (19.1% vs. 30.6%, p = 0.001) and had less prior MIs (14.4% vs. 21.5%, p = 0.021). On the baseline seven-item Seattle Angina Questionnaire (SAQ-7), females reported more physical limitation (70.9 ± 29.5 vs. 80.9 ± 25.1, p < 0.001), more frequent angina (76.7 ± 22.9 vs. 81.5 ± 20.3, p = 0.002) and a lower quality of life (51.8 ± 29.4 vs. 57.8 ± 29.1, p = 0.006) than men for the same degree of flow reduction (FFR 0.69 ± 0.12 vs. 0.67 ± 0.11, p=0.098). In women, the pattern of CAD demonstrated a more focal nature compared to men (PPG 0.65 ± 0.16 vs. 0.61 ± 0.16, p = 0.001). Also, women achieved higher post-PCI FFR (0.88 ± 0.07 vs. 0.87 ± 0.07, p = 0.02). The rate of periprocedural MI was not statistically different between genders (5.1% vs. 5.8%, p = 0.79) Conclusion Among patients with hemodynamically significant CAD, women report more symptoms and lower quality of life. Despite sharing a similar degree of flow impairment, women exhibited a more focal CAD phenotype than men, leading to superior physiological outcomes after PCI.Study flowchartPRO and Phisiology by gender

Women who experience myocardial infarction (MI) and undergo invasive angiography experience higher morbidity and mortality compared to their male counterparts. Data suggests that this mortality difference may be predominantly in women >55 years (2). Treatment bias may be an important factor. Prognostic benefit of optimal medical therapy (OMT) following MI is well established, however studies and meta-analyses suggest women may be less likely to receive this compared to men e.g (3, 4). Purpose We aimed to establish whether there were sex differences in OMT received following invasive angiography for obstructive CAD at a large cardiac centre. Methods In this single centre retrospective study, we examined OMT received by females and males undergoing invasive angiography in 2019 with proven obstructive CAD, as defined by an angiographic lesion ≥50% of luminal diameter. Discharge medications were extracted from the electronic patient record and entered into a database. Pooled analysis of sex differences and sub-group analysis according to diagnosis (STEMI, NSTEMI, stable angina) and age (≤ 55 years or >55 years) was performed. Multivariate binomial logistic regression was used to adjust for confounders including BMI, diabetes, hypertension, hypercholesterolaemia, previous MI and previous percutaneous coronary intervention. Results 4,346 patients underwent invasive angiography during the study period. 2,358 with obstructive CAD and full medication details available at the present time were included (STEMI: 649 [mean age: 59.7, 20.6 % female], NSTEMI: 439 [mean age 63.4, 22.6% female], Stable angina: 1,270 [mean age 63.7, 22.2% female]). Pooled analysis showed females were less likely to receive aspirin and beta-blockers (OR=0.64, p=0.009, OR=0.66, p=0.001). They also received fewer angiotensin-converting enzyme inhibitors (ACE-I) but more angiotensin receptor blockers (ARB) (OR=0.58, p 55 with STEMI were less likely to receive beta-blockers and high potency antiplatelets compared to males (OR=0.49, p=0.009, OR=0.44, p=0.011). In the NSTEMI group females ≤55 received fewer beta-blockers and statins (OR=0.16, p=0.020, OR=0.004, p=0.045). In the stable angina group females ≤55 were less likely to receive aspirin (OR=0.07, p<0.001). Fewer ACE-I and more ARB in females was predominantly in the stable angina group (OR=0.56, p<0.001, OR= 1.50, p=0.021). Conclusion Women undergoing invasive angiography for obstructive CAD appeared to receive less OMT compared to men across several drug categories, diagnoses and age groups. Future work will be directed at understanding why these differences occurred and the mechanistic consequences of these differences in OMT pharmacology between females and males.

Abstract:Sarcoidosis may involve the skin primarily with a variety of lesions and in addition affect other tissues and systems. Thus, the lymph glands, bones and various viscera may be involved. The debate about the etiology of this disease has been warm and extensive, with the clearest overtones heard with regard to a tuberculous cause. The majority of subjects react negatively to tuberculin even in high concentration, and most of the minority show weakly positive reactions. It has been held that a special immunologic state in sarcoidosis decreases or entirely prevents the positive reaction to tuberculin which otherwise would be present to indicate the underlying tuberculous infection. When persons with negative reactions to tuberculin, who presumably, therefore, are free from past or present tuberculous infection, are inoculated with BCG vaccine, 90 per cent manifest positive reactions in two weeks and about 100 per cent do so in four weeks.
Sex Disparities in the Management of Acute Coronary Syndromes: Insights From the Veterans Affairs Clinical Assessment, Reporting, and Tracking Program

Abstract:Background: Previous work has demonstrated disparities in the management of cardiovascular disease among men and women. We sought to evaluate these disparities and their associations with clinical outcomes among patients admitted with acute coronary syndromes to the Veterans Affairs Healthcare System. Methods and results: We identified all patients that were discharged with acute coronary syndromes within the Veterans Affairs Healthcare System from October 1, 2015 to September 30, 2022. Medical and procedural management of patients was subsequently assessed, stratified by sex. In doing so, we identified 76 454 unique admissions (2327 women, 3.04%), which after propensity matching created an analytic cohort composed of 6765 men (74.5%) and 2295 women (25.3%). Women admitted with acute coronary syndromes were younger with fewer cardiovascular comorbidities and a lower prevalence of preexisting prescriptions for cardiovascular medications. Women also had less coronary anatomic complexity compared with men (5 versus 8, standardized mean difference [SMD]=0.40), as calculated by the Veterans Affairs SYNTAX score. After discharge, women were significantly less likely to receive cardiology follow-up at 30 days (hazard ratio [HR], 0.858 [95% CI, 0.794-0.928]) or 1 year (HR, 0.891 [95% CI, 0.842-0.943]), or receive prescriptions for guideline-indicated cardiovascular medications. Despite this, 1-year mortality rates were lower for women compared with men (HR, 0.841 [95% CI, 0.747-0.948]). Conclusions: Women are less likely to receive appropriate cardiovascular follow-up and medication prescriptions after hospitalization for acute coronary syndromes. Despite these differences, the clinical outcomes for women remain comparable. These data suggest an opportunity to improve the posthospitalization management of cardiovascular disease regardless of sex.

Abstract:OBJECTIVE: To study gender differences in management and outcome in patients with non-ST-elevation acute coronary syndrome.DESIGN, SETTING AND PATIENTS: Cohort study of 53 781 consecutive patients (37% women) from the Register of Information and Knowledge about Swedish Heart Intensive care Admissions (RIKS-HIA), with a diagnosis of either unstable angina pectoris or non-ST-elevation myocardial infarction. All patients were admitted to intensive coronary care units in Sweden, between 1998 and 2002, and followed for 1 year.MAIN OUTCOME MEASURES: Treatment intensity and in-hospital, 30-day and 1-year mortality.RESULTS: Women were older (73 vs 69 years, p<0.001) and more likely to have a history of hypertension and diabetes, but less likely to have a history of myocardial infarction or revascularisation. After adjustment, there were no major differences in acute pharmacological treatment or prophylactic medication at discharge. Revascularisation was, however, even after adjustment, performed more often in men (OR 1.15; 95% CI, 1.09 to 1.21). After adjustment, there was no significant difference in in-hospital (OR 1.03; 95% CI, 0.94 to 1.13) or 30-days (OR 1.07; 95% CI, 0.99 to 1.15) mortality, but at 1 year being male was associated with higher mortality (OR 1.12; 95% CI, 1.06 to 1.19).CONCLUSION: Although women are somewhat less intensively treated, especially regarding invasive procedures, after adjustment for differences in background characteristics, they have better long-term outcomes than men.

Abstract:Circulation: Cardiovascular Quality and Outcomes, Ahead of Print. BACKGROUND:Sex differences in acute myocardial infarction treatment and outcomes are well documented, but it is unclear whether differences are consistent across countries. The objective of this study was to investigate the epidemiology, use of interventional procedures, and outcomes for older females and males hospitalized with ST-segment–elevation myocardial infarction (STEMI) and non–ST-segment–elevation myocardial infarction (NSTEMI) in 6 diverse countries.METHODS:We conducted a serial cross-sectional cohort study of 1 508 205 adults aged ≥66 years hospitalized with STEMI and NSTEMI between 2011 and 2018 in the United States, Canada, England, the Netherlands, Taiwan, and Israel using administrative data. We compared females and males within each country with respect to age-standardized hospitalization rates, rates of cardiac catheterization, percutaneous coronary intervention, and coronary artery bypass graft surgery within 90 days of hospitalization, and 30-day age- and comorbidity-adjusted mortality.RESULTS:Hospitalization rates for STEMI and NSTEMI decreased between 2011 and 2018 in all countries, although the hospitalization rate ratio (rate in males/rate in females) increased in virtually all countries (eg, US STEMI ratio, 1.58:1 in 2011 and 1.73:1 in 2018; Israel NSTEMI ratio, 1.71:1 in 2011 and 2.11:1 in 2018). Rates of cardiac catheterization, percutaneous coronary intervention, and coronary artery bypass graft surgery were lower for females than males for STEMI in all countries and years (eg, US cardiac catheterization in 2018, 88.6% for females versus 91.5% for males; Israel percutaneous coronary intervention in 2018, 76.7% for females versus 84.8% for males) with similar findings for NSTEMI. Adjusted mortality for STEMI in 2018 was higher for females than males in 5 countries (the United States, Canada, the Netherlands, Israel, and Taiwan) but lower for females than males in 5 countries for NSTEMI.CONCLUSIONS:We observed a larger decline in acute myocardial infarction hospitalizations for females than males between 2011 and 2018. Females were less likely to receive cardiac interventions and had higher mortality after STEMI. Sex disparities seem to transcend borders, raising questions about the underlying causes and remedies.

Abstract:Objective A higher mortality risk for women with acute ST elevation myocardial infarction ( STEMI) has been a common finding in the past. It is unclear if these differences persist among patients undergoing contemporary primary percutaneous coronary intervention ( PCI ) with focus on early reperfusion in China. Methods We assessed 578 patients with STEMI undergoing acute coronary angiography with the intention of primary PCI between January 2008 and December 2013. Data were examined for gender-specific differences regarding patients' characteristics, angiographic findings, procedural details, and in-hospital mortality after PCI. Results A total of 123 women and 455 men were included in the study. Compared with men, women were older than men [ (68 ± 10) years old vs. (61 ± 12) years old, t= -6. 817, P﹤0. 01], and more frequently had a history of hypertension (75. 6﹪ vs. 52. 3﹪,x2 =21. 484, P﹤0. 01) and diabetes mellitus (43. 1﹪ vs. 27. 7﹪,x2 =10. 737, P﹤0. 01). Men were more likely to be smokers. Female sex was associated with a higher unadjusted in-hospital mortality (8. 9﹪ vs. 4. 4﹪,x2 =3. 945, P=0. 047). After multivariable adjustment this sex difference disappeared (OR:1. 036, 95﹪CI: 0. 339 -3. 169, P =0. 95) . Conclusions Female sex is associated with an apparent hazard of increased mortality among patients undergoing primary PCI for STEMI, but this difference is likely explained by older age and worse baseline comorbidities among women.
