Abstract:The complexity of the carpus explains the difficulty treating carpal injuries. Lesions are dominated by perilunate dislocation, scapholunate dislocation, and scaphoid fractures. The other injuries are trivial. Symptoms include pain and loss of wrist strength, reversible for an acute and well-treated lesion. Too often, these ligament injuries are diagnosed late. For delays longer than 6 weeks, ligament repair is ineffective. These old, complex lesions are potentially highly arthritic in the radiocarpal and mediocarpal joints. Improvements in wrist surgery have mitigated these chronic lesions. Various surgical techniques can preserve a functional wrist; wrist arthrodesis is no longer the only solution for these arthritic wrists. Over the past decade, arthroscopy has contributed to better understanding the injuries of the carpus as well as to better healing them. For acute or chronic ligament injuries without degenerative osteoarthritis, arthroscopy is the treatment of the future. This technique involves a long learning curve and the various arthroscopic techniques must be validated.
Distal radioulnar joint instability: Diagnosis and treatment of acute and chronic lesions

Abstract:Distal radioulnar joint instability is a common clinical condition that can be either acute or chronic. Its management requires extensive knowledge of all the anatomical structures around the wrist area. The clinical examination must identify all damaged structures in order to define a customized reconstruction strategy. The aim of treatment is to prevent the development of chronic instability and/or arthritic lesions, which can only be treated with palliative surgery. Distal radioulnar instability is complex. Its management shares similarities with that of knee instability, with a wide range of conditions ranging from a sprain with isolated ligament tear to complex dislocation in which multiple ligaments are torn. In particular, the triangular fibrocartilage complex, which is a pillar of distal radioulnar joint stability, is not the sole stabilizer of this joint. The treatment strategy is not as simple as just reattaching this ligament to treat all of the acute and chronic clinical signs. The goal of the present study is to describe the various structures and possible lesions in this joint and then how to repair them. The diagnostic and therapeutic difficulties justify treatment at a specialized center.

Abstract:Carpal instability is a set of complex and important wrist pathology, which is usually secondary to different injury severities and patterns to the wrist joint or pathological changes. Scapholunate instability is the most common type of carpal instability, and other prevalent patterns include lunotriquetral instability, midcarpal instability, radiocarpal instability and perilunate injuries. The diagnosis and treatment of carpal instability is relatively difficult, which has drawn much attention in the field of wrist surgery. With deeper understanding of pathological mechanism of wrist instability, there reported new surgical procedures with better suitability to anatomical character and kinematics to repair or reconstruct ligaments. Also, with the rapid development of wrist arthroscopy techniques, many traditional open procedures can now be accomplished in a minimally invasive approach, under assistance of wrist arthroscopy. The authors review the progress in diagnosis and treatment of carpal instability in recent years and discuss the existed confusion so as to provide a reference for clinical work.
Lunotriquetral instability: diagnosis and treatment

Abstract:Isolated injury of the lunotriquetral interosseous ligament complex and associated structures is less common and is poorly understood compared with the other proximal-row ligament injury, scapholunate dissociation. The spectrum of injuries ranges from isolated partial tears to frank dislocation, and from dynamic to static carpal instability. The diagnosis may be difficult to establish because of the many possible causes of ulnar-sided wrist pain and the often normal radiographic appearance. The mechanism of injury is variable and includes attrition by age, positive ulnar variance, and perilunate or reverse perilunate injury. Appropriate treatment requires assessment of the degree of instability and the chronicity of the injury. Options include corticosteroid injection, immobilization, ligament repair, ligament reconstruction with tendon grafts, limited intercarpal arthrodesis, and ulnar shortening.
High-energy injuries of the wrist

Abstract:High-energy injuries to the wrist gather complex fractures of the distal radius, radiocarpal dislocations, perilunate dislocations, and other intracarpal dislocations. Depending on the energy of the injury and the position of the wrist at the time of impact, the patient, often a young male with a high functional demand, presents one of these injuries associating fracture(s) and ligament injury. The trauma is often bilateral, with proximal lesions (elbow) very often associated with contusion or compression of the median nerve. Diagnosis is confirmed by wrist X-rays, which are sufficient to determine treatment for radiocarpal and perilunate dislocations. In cases of distal radius fractures or other intracarpal dislocations, a preoperative CT is necessary. Reduction of the dislocation and relief of neurovascular compression are performed immediately. The final treatment of each lesion (bone fixation, ligament repair) can be undertaken simultaneously or delayed, depending on the patient and the lesions. Cartilage lesions, resulting from the high-energy injury, can be estimated using arthroscopy but cannot be repaired and determine the prognosis. The surgeon's objective is to restore joint congruence, which does not prevent stiffness, the main complication of these rare injuries, which the surgeon must know how to recognize and treat.

Abstract:Wrist arthroscopy is now a popular diagnostic tool as well as an advanced treatment tool for wrist disorders, which includes traumatic and degenerative conditions. We also achieved technological innovations along with advances in anatomical understanding and surgical techniques.[1] However, we still do not know whether wrist arthroscopy is a gold standard for diagnosis on wrist disorders, and the best treatment option on individual pathology of the wrist, such as peripheral tear of the triangular fibrocartilage complex (TFCC) or scapholunate ligament (SLL) injury. There are many experts' opinion papers, in which there should be a bias. Survey of questionnaire to experts worldwide is the best way to answer these questions. This issue includes the "Editor's Pick" of "The Current Role of Arthroscopy in Traumatic Wrist Injuries: An Expert Survey" described by Drs. d'Ailly, Mulders, Coert, and Schep. This survey analyzed answers to the multiple questionnaires about wrist arthroscopic examinations and treatments to the members of the International Wrist Arthroscopic Society that includes famous wrist arthroscopic surgeons worldwide. The survey concludes that the outcomes of wrist arthroscopy strongly depend on surgeons' experience and that wrist arthroscopy is better than magnetic resonance imaging for diagnosing TFCC and SLL injuries. No agreement was reached on the preferred treatment of any type of TFCC or SLL injury. The latter suggests that there is no consensus on many arthroscopic treatments for wrist disorders and that excellent prospective comparative study on this should be necessary. Interesting wrist papers such as the ulnar styloid malunion, carpal tunnel release in distal radius fracture, SLL, capsular shrinkage, four-dimensional computed tomography study on the wrist, lunate fracture morphology in Kienbock's disease, as well as interesting case reports are included in this issue. Wrist arthroscopy for diagnosis and treatment of acute and chronic conditions

Abstract:Wrist arthroscopy is a constantly evolving procedure. Allowing direct visualization and dynamic testing of intra-articular structures led to a novel approach toward traumatic and degenerative lesions based on most of the classifications routinely used in wrist surgery. The development of specific instrumentation, combined with a novel understanding of the local anatomy, progressively allowed wrist surgeons to describe more ambitious and complex surgeries. Wrist arthroscopy has become an increasingly useful tool in hand and wrist surgeons' panoply and seems promised to have further development in the future. This paper discusses the surgical technique and the various pathologies that can be treated by arthroscopy of the wrist.
Open and arthroscopic treatment of perilunate injuries

Abstract:Carpal dislocations are rare and complex injuries. Nearly every combination of radiocarpal and intercarpal dislocation has been described, but few fit neatly into a particular pattern or classification scheme. The injury may be subtle clinically and radiographically, and the diagnosis is frequently delayed. Prompt recognition, accurate reduction, and stable temporary internal fixation all contribute to improved outcomes. Internal fixation techniques depend on the pathology imparted on the carpus. Open reduction, arthroscopic, and fluoroscopically aided percutaneous techniques can be used to successfully treat carpal dislocations. We report our experiences treating perilunate injuries. Traumatic valgus instability of the elbow: pathoanatomy and results of direct repair. Surgical technique

Abstract:Background: The medial collateral ligament provides valgus stability to the elbow. The purpose of the present study was to describe the pathoanatomy of acute traumatic medial collateral ligament ruptures and to report the rationale and results of direct repair. Methods: Between 1996 and 2006, eleven athletes presented with acute rupture of the medial collateral ligament of the elbow and no history of dislocation. Three patients had received steroid injections for the treatment of medial epicondylitis, but none had a history of medial elbow insufficiency. All patients demonstrated gross valgus instability on clinical examination and medial joint space widening on valgus stress radiographs. Complete avulsion of the medial collateral ligament from its humeral origin was documented with magnetic resonance imaging in all patients. Operative findings uniformly demonstrated avulsion of the flexor-pronator muscles with distal retraction. The underlying medial collateral ligament was avulsed in a sleeve-like fashion from the denuded medial epicondyle. The ligament was directly reattached to its footprint. The avulsed flexor-pronator tendon was repaired to the residual tendon with use of interrupted figure-of-eight nonabsorbable sutures. All patients were followed for a minimum of sixteen months with serial clinical examinations, radiographs, and the Disabilities of the Arm, Shoulder and Hand (DASH) questionnaire. Results: Full active range of motion was achieved in ten patients; the remaining patient had a 20 degrees flexion contracture. Three patients had acute ulnar nerve palsies at the time of the injury, and all three recovered complete motor and sensory function by six months after the injury. Nine of the eleven patients returned to competitive college athletics between four and six months. The mean DASH score at the time of the most recent follow-up was 6. Conclusions: Direct repair of an acute traumatic medial collateral ligament avulsion of the elbow reliably restores valgus stability, even in throwing athletes.
Ulnar-Sided Wrist Pain: A Diagnostic Evaluation Guide From 30-Plus Years of Experience

Abstract:INTRODUCTION: While multiple ulnar-sided wrist pain (USWP) diagnostic evaluation guides have been presented, none have included original clinical data or statistical analysis. The purpose of this study is to provide a diagnostic evaluation guide derived from original clinical data and analysis to help clinicians arrive at a differential diagnosis for USWP.METHODS: Using a computer search of patients presenting with sprains, instability, and laxity of the wrist, 385 patient charts were identified. Patient demographics, mechanism of injury, subjective complaints, physical findings, and diagnostic test findings were reviewed. Statistical analysis was performed to determine sensitivity and specificity of diagnostic methods on their ability to identify lunotriquetral ligament tears, triangular fibrocartilage complex (TFCC) tears, and ulnar impaction syndrome. Diagnostic arthroscopy was used as the reference standard.RESULTS: Ninety-three patients, comprising 101 cases of USWP, were included in the study. The onset of injury was traumatic in 83 out of 101 cases with motor vehicle accidents (N=46) being the most common, followed by overuse (N=18), and a fall onto an outstretched hand (N=16). The ulnocarpal tenderness test exhibited sensitivity/specificity of 72%/33%; lunotriquetral ligament laxity test of 42%/62%; bone scan of 80%/33%; radiocarpal arthrogram of 90%/98% for TFCC tears and 50%/91% for lunotriquetral ligament tears; midcarpal arthrogram of 82%/86% for lunotriquetral ligament tears. The mean ulnar variance on standard posteroanterior view radiograph was 0.95 mm, increasing to 2.67 mm on gripping posteroanterior view.CONCLUSION: Physicians should suspect a lunotriquetral ligament and/or TFCC tear with the acute onset of USWP following a loaded dorsiflexed mechanism of injury. Ulnocarpal tenderness tests and pre-operative ulnar variance measures are effective for increasing suspicion of USW pathology. Bone scans are helpful in diagnosing ulnar impaction syndrome in conjunction with radiographic findings. A combination of midcarpal arthrogram for lunotriquetral ligament tears and radiocarpal arthrogram for TFCC tears should be employed.

Abstract:Objective: To analyzed effect of traumatic elbow instability and to increase its level of diagnosis and treatment. Methods: From January 2000 to December 2002,56 cases of traumatic elbow instability were retrospectively analyzed,including distributions of sex and age,traumatic mechanisms,combined injures and therapeutic methods and results. Results: Male and female were 22 and 34 cases respectively in 56 cases of traumatic elbow instability;among them,the directions of displacement of 51 cases were posterolateral;50 cases happened as a result of falls on the out-stretched hand;the simple and complex displacements were 40 and 16 cases respectively;combined peripheral nerve injuries had 4 case.The non-operative and operative therapy were applied respectively in 51 and 5 cases;the follows-up results showed that 3 cases had immobilized elbows in functional position and 22 cases suffered from 10° extension limitation of elbow. Conclusion: The majority of traumatic elbow instability are simple posterolateral rotatory displacements resulting from the axial force on the out-stretched elbow with supination.The non-operative therapies are principal modalities.Some complex displacements require internal fixations,soft tissue repair and reconstruction for combined fractures.
Sports Injuries of the Hand, Wrist, and Elbow

Abstract:Sports injuries of the upper extremity are a common problem seen by those who care for athletes and those who manage upper extremity injuries. The term "high-level athlete" may include adolescents and high school students, collegiate athletes, and of course, the professional athlete. However, the "weekend warrior" can sustain similar injuries and can have a similar desire to return to play as quickly as possible. The challenge in management of these injuries to the elbow, wrist, and hand is identifying those injuries that will benefit from early surgery compared with those that may be safely treated nonsurgically. A rapid return to play may be possible with some injuries with cast or splint immobilization based on the sport and position played. Management of common sports injuries of the hand, wrist, and medial elbow should be reviewed with consideration of return to play.

Floating Wrist: A Rare Case Report and Review of Anatomical, Diagnostic, and Therapeutic Aspects

Abstract:Introduction: Joint injuries in the upper limb are uncommon and have received limited attention in the literature. This case report presents the first described case in the literature of a "floating wrist" and aims to highlight the diagnostic, anatomical, and therapeutic aspects of this injury. Case report: A 27-year-old male was involved in a severe car accident, resulting in a closed deformity of the right wrist. The patient suffered fractures of the forearm bones and dislocation fracture of all five wrist rays. Surgical intervention was performed to repair the fractures and stabilize the wrist. Despite post-operative complications, the patient demonstrated positive functional outcomes after a 12-month follow-up. Discussion: The term "floating wrist" refers to a rare condition characterized by fractures and/or dislocations of all five wrist rays, accompanied by fractures of the forearm bones. Diagnosis can be complicated due to polytrauma and wrist edema. Early surgical management and rehabilitation are essential for achieving favorable outcomes. Further research is necessary to improve our understanding of this uncommon injury. Conclusion: This case report highlights a unique instance of a "floating wrist" and emphasizes the significance of early diagnosis, appropriate surgical intervention, and prompt rehabilitation in a comprehensive management of these complex injuries. Enhanced comprehension of this rare condition is crucial for improving patient outcomes in similar traumatic scenarios.
Scapholunate injuries: challenging existing dogmas in anatomy and surgical techniques

Abstract:The management of scapholunate injuries and dissociation remains debatable. Traditional methods of fixation had centred around open procedures, but advances in wrist arthroscopic techniques has redefined both major anatomical findings and operative approaches. This article challenges two main existing dogmas: first, the anatomy of the scapholunate ligament and second, the management of these injuries. For the first, we propose that scapholunate stability is in fact maintained by a whole anatomic complex, consisting of well-defined capsuloligamentous structures and extrinsic ligaments, and that the scapholunate ligament itself probably has a limited role. For the second, we challenge the notion that scapholunate injuries often require open procedures and propose that mini-invasive arthroscopic dorsal repair of the scapholunate complex is an efficient technique and sufficient for achieving long-term stability.
Emphasis on minimally invasive and precise treatment of wrist bone and ligament injuries

Abstract:Wrist is one of the most frequently used joints in life and work. Wrist injury will seriously affect the ability of self-care, study, work, exercise and fitness. The wrist is small in size but complex in composition, so the bone and ligament structures are relatively fine. Traditional open surgery increases the risk of damage to these structures or their blood supply, and thus the recovery effect is limited. With the development of small arthroscopy-related instruments and techniques, minimally invasive surgery techniques such as ligament injury repair, reduction and fixation of intra-articular fractures, fixation of nonunion bone grafts, and reduction and fixation of complex fractures have emerged, which made the treatment of wrist and ligament injury minimally invasive. The application of the 3D printing guide technology and navigation technology modifies the traditional surgical techniques, as making the treatment of wrist bone and ligament injuries more minimally invasive and precise to reduce the degree and risk of iatrogenic injury and improve the curative effect. Wrist arthroscopy has become an indispensable tool in the surgical diagnosis and treatment of wrist and ligament injuries, and is an essential skill for hand surgeons. The authors introduce the progress of minimally invasive and precise surgical techniques of fracture fixation and ligament repair for wrist bone and ligament injuries, which provides a reference for clinical work.
[Dislocation due to injury with trajectory in the scaphoid-trapezoid-trapezoid periscaphoid-perisemilunar bones. Case report and literature review]

Abstract:Introduction: dislocations of carpal bones without associated fractures are considered a rare injury, the most common mechanism of injury being axial loading with wrist in extension plus ulnar deviation. The literature reports a wide variety of complex carpal injuries, even so, it is possible to identify previously undescribed injuries. Objective: to present an atypical case of a patient with an injury to the midcarpal stabilizing mechanism and the stabilizing mechanism of the proximal row of the wrist following trauma to the hand that required carpectomy as definitive surgical treatment. Presentation of case: a 48 year old male patient is presented who is admitted to our hospital unit after presenting injury to the left hand after being run over by a motor vehicle, with axial load mechanism, presenting deformity in the left thoracic extremity, fracture of the proximal metaphysis of the second phalanx of the third finger as well as dislocation of the proximal interphalangeal joint, with traumatic amputation of the second phalanx of the fourth finger plus extensor injury in zone V of the fifth finger with loss of skin coverage of the fourth and fifth finger, attending our hospital unit 24 hours after the injury. Conclusions: carpal bone dislocations are an orthopedic emergency, with 20% going unnoticed in trauma centers. Early closed reduction is the initial treatment to avoid severe complications, however, surgical treatment is the gold standard for fixation. Carpectomy is considered a mostly adequate sequelae management treatment, however it is well accepted for complex injuries to the wrist stabilization mechanisms, as it can be performed in a short surgical time and early rehabilitation can be initiated and functional ranges of motion can be achieved with low sequelae.

Abstract:Objective To this day, diagnostic standards and uniform definition for acute, isolated syndesmotic injuries are missing. The aim of the current study was to conduct a systematic review of the classification systems and diagnostics currently applied and to propose a best evidence diagnostic approach. Methods Medline (PubMed), Scopus, Cochrane Central Register of Controlled Trials, and Embase were searched from inception to June 5, 2022, for studies reporting the outcome of surgically treated acute, isolated syndesmotic injuries. First, all classifications used in the eligible studies were identified and illustrated according to the individual syndesmotic structures injured. Second, the indication for surgery and stabilization, based on the diagnostics applied and the time point assessed (pre- or intra-operatively), was analyzed, including the applied cutoff criteria. Results Ten out of 4190 studies, comprising 317 acute ligamentous syndesmotic injuries, met the inclusion criteria. Seven studies facilitated one of the three different classification systems (Calder, West Point, or Sikka classification). Eight studies based their indication for surgery on a combination of clinical and radiographic examinations and two on radiographs only. The most applied clinical tests were the external rotation stress test and squeeze test. The most common radiologic diagnostics were plain radiographs and MRI. Intraoperatively, instability was verified most commonly using arthroscopy. Conclusion Current classifications and diagnostics for syndesmotic injuries are heterogeneous, often cannot be attributed to the ligaments injured. An evidence-based diagnostic algorithm based on noninvasive diagnostics and an anatomy-based classification for acute syndesmotic instability is presented.

Abstract:The Mayfield sequence of progressive perilunar instability is commonly encountered. A reverse-perilunate sequence has also been described, where forces are transmitted from the ulnar wrist, resulting in progressive disruption of the ulnocarpal and lunotriquetral intervals. A concomitant dislocation of both the lunate and triquetrum together is a rare injury and has not been reproduced in cadaver models nor widely encountered in clinical cases. We report a rare case of an open volar dislocation of the lunate and triquetrum, and outline the management principles and outcomes in our patient. This case highlights that not all perilunar injuries occur in the conventional sequence. Differing paths of force transmission may result in atypical patterns of carpal disruption. Nonetheless, the broad management principles are similar. Achieving carpal reduction and stabilization, soft-tissue coverage, and early rehabilitation results in reasonable patient outcomes.

Abstract:Ulnar styloid fracture is a common complication of wrist fracture and dislocation, contributing to 40% to 65% of distal fractures of the radius. [ 1 ] There is still a clinical debate regarding the effects of ulnar styloid fractures on the incidence of distal radioulnar joint instability. May et al reported that ulnar styloid base fractures and significant ulnar styloid fractures with displacement increase the incidence of distal radioulnar joint instability. [ 2 ] The anatomical structure of a stable distal radioulnar joint (DRUJ) includes components both inside and outside the articular capsule. The pronator quadratus, Interosseous Membrane, sixth dorsal compartment, and tendinous sheath of the extensor carpi ulnaris; the in-capsule structures comprising the osseous articular surface, triangular fibrocartilage complex (TFCC), and articular capsule are among the extra-capsule structures. [ 3 , 4 ] Additionally, because of their deep anatomical placement, extra-capsule structures are not readily exposed to damage. The displacement of distal fracture fragments in distal fractures of the radius tends to cause TFCC damage of varying degrees for in-capsule structures, while TFCC damage is primarily responsible for DRUJ stability. [ 5 , 6 ]
