Abstract:AIM: To evaluate results of pre-operative radiochemotherapy followed by surgery for 15 patients with locally advanced un-resectable rectal cancer. METHODS: 15 patients with advanced non-resectable rectal cancer were treated with pre-operative irriadiation of 4046 Gy plus concomitant chemotherapy (5-FU+LV and 5'-DFuR) (RCS group). For comparison, 27 similar patients, treated by preoperative radiotherapy (40-50 Gy) plus surgery were served as control (RS group). RESULTS: No radiochemotherapy or radiotherapy was interrupted and then was delayed because of toxicities in both groups. The radical resectability rate was 73.3% in the RCS group and 37.0% (P = 0.024) in RS group. Sphincter preservation rates were 26.6% and 3.7% respectively (P = 0.028). Sphincter preservation rates of lower rectal cancer were 27.3% and 0.0% respectively (P = 0.014). Response rates of RCS and RS groups were 46.7% and 18.5% (P = 0.053). The tumor downstage rates were 8 (53.3%) and 9 (33.3%) in these groups (P = 0.206). The 3-year overall survival rates were 66.7% and 55.6% (P = 0.485), and the disease free survival rates were 40.1% and 33.2% (P = 0.663). The 3-year local recurrent rates were 26.7% and 48.1% (P = 0.174). No obvious late effects were found in either groups. CONCLUSION: High resectability is possible following preoperative radiochemotherapy and can have more sphincters preserved. It is important to improve the quality of the patients' life even without increasing the survival or local control rates. Preoperative radiotherapy with concomitant full course chemotherapy (5-Fu+LV and 5'-DFuR) is effective and safe.
Tumor pathological response and downstaging of locally advanced rectal cancer after neoadjuvant chemoradiotherapy: comparison of two radiation dose schedules

Abstract:Objective To assess the short-term efficacy and tolerability of different radiotherapy doses schedules as 46 Gy and 50 Gy for locally advanced rectal cancer patients undergoing neoadjuvant chemoradiotherapy.Methods All patients with locally advanced rectal cancer who had received neoadjuvant chemoradiotherapy between Aug 2010 and May 2015 were enrolled.This retrospective analysis was performed according to the radiotherapy dose grouping of 46 Gy and 50 Gy groups.Concurrent chemotherapy regimen was capecitabine-based and oxaliplatin was added only when young patients (＜ 75 years old) were in a good condition.Total mesorector excision was scheduled 6-8 weeks after concurrent chemoradiotherapy.Results Totally 213 patients were enrolled in our analysis,including 61 cases in 46 Gy group and 152 cases in 50 Gy group.There were 145 male and 68 female patients.There were 22 patients diagnosed clinically with T2,180 with T3,and 11 with T4.Tumor distance from the anal verge was ≤5,＞5 and ＜ 10 cm,or ≥10 cm in 82,115 and 16 patients,respectively.T downstaging was observed in 95 (44.6％) patients,pathologic complete response (PCR) was shown in 48 (22.5％) patients.In the 46 Gy and 50 Gy groups,the rate of PCR was 18.0％ vs.24.3％ (P ＞0.05).In addition,good response rate (TRG 3 + 4) was 67.2％ vs.75.0％ (P ＞ 0.05),and T downstaging rate was 39.3％ vs.46.7％ (P ＞0.05).Subgroup analysis for T3N2/T4 patients,the rate of PCR was 6.3％ vs.23.3％ (P＞0.05),good response rate (TRG 3 +4) was 50.0％ vs.72.1％ (P＞0.05),and T downstaging was 31.3％ vs.46.5％ (P ＞0.05).There was no significant difference in treatment-related toxicity between the two groups.Conclusions The two different radiation dose fractionation (50 Gy vs.46 Gy) had no impact on pathologic tumor regression and T downstaging for locally advanced rectal cancer.Nonetheless,a further long-term follow-up is warranted to confirm the preliminary study.
Feasibility Of Relatively Low Neoadjuvantradiation Doses For Locally Advanced Rectal Cancer: A Propensity Score-Matched Analysis

Abstract:BackgroundNeoadjuvant chemoradiation therapy is part of the standard treatment of locally advanced rectal cancer (LARC). Although various options for modifying preoperative radiotherapy protocols have been researched and proposed, there is still no consensus as to the most appropriate dose regimen of neoadjuvant therapy for this disease.AimTo evaluate the effects of relatively low-dose radiation regimens on tumor regression and clinical outcomes in rectal cancer patients treated with neoadjuvant CRT followed by mesorectal excision.Methods and ResultsWe retrospectively analyzed patients with LARC who underwent neoadjuvant concurrent chemoradiation (CCRT) in our hospital from June 2010 to December 2015. A total of 259 consecutive patients were enrolled, receiving 42 to 44 Gy (RLD, n = 31), 46 Gy (SD1, n = 69), or 50 Gy (SD2, n = 159) of CRT, combined with either capecitabine/oxaliplatin or capecitabine only or mFOLFOX6, followed by total mesorectal excision. A 1:4 propensity score matching was employed, and all patients in the RLD group were matched with 124 patients in the SD2 group. Rates of pCR, 3-year local/regional recurrence (LRR), overall survival (OS), and disease-free survival (DFS) in the RLD group were all not significantly different (0.313 for pCR; 0.884 for LRR; and 0.762 for OS; 0.101 for DFS) from those in SD1 and SD2 groups. The RLD group showed a lower incidence of grade 3 to 4 hematologic toxicity than SD2 group (0.019). A propensity score analysis demonstrated no significant differences in the pCR rates and 3-year outcomes between the RLD and SD2 group.ConclusionRelatively low-dose regimen (<= 44 Gy) of neoadjuvant CRT combined with standard concurrent chemotherapy appears to be both safe and effective in Chinese patients with LARC. Further testing by prospective randomized trials is needed.
Comparison of Two Different Neoadjuvant Chemoradiatiotherapy Regimens for Locally Advanced Rectal Cancer: Results of a Phase II, Multicenter, Randomized Trial.

Abstract:3614 Background: The aim of this study is to compare clinical outcomes between two groups of different chemoradiation Regimens. Methods: Eligible patients were randomly assigned to Patients received either IMRT to the pelvis of 50 Gy/25Fxs concurrent with oxaliplatin 50 mg/m2 weekly and capecitabine 625 mg/m2 bid d1–5 weekly (Arm A), or IMRT to the pelvis of 50 Gy/25Fxs and a concomitant boost of 5 Gy to the primary tumor in 25 fractions, followed by a cycle of XELOX two weeks after the completion of chemoradiotherapy (Arm B). Surgery was scheduled eight weeks after the completion of CRT. All patients were recommended to receive adjuvant CT regardless of pathological stages. A total of six cycles of XELOX chemotherapy was recommended including neoadjuvant and adjuvant period. The primary end point was pCR. Results: From February 2010 to December 2011, a total of 120 patients were randomly assigned to Arm A (n = 60) or Arm B (n = 60). One hundred and ten patients (Arm A = 53, Arm B = 57) underwent surgery, the other 10 patients didn’t receive surgery because of unresectable disease. Eight and 14 cases were evaluated as pCR in two groups respectively ( p= .157). Tumor response was further classified into good response and poor response, the former was defined as ypT0N0, ypT1-2N0 and ypT0N1a. The rate of good responses were 30.0% and 55% ( p= .006). No significant differences were found in grade 3 or 4 acute adverse events during the treatment between the two groups. However, number of patients who had delayed incision healing was 13 and 3 patients ( p= .007). There were no statistical differences in OS ( p= .553), DFS ( p= .349) and local-regional control ( p= .856) between the two groups. In the univariate analysis, N stage, MRF involvement and tumor response had significant impacts on OS, DFS and LC. In the multivariate analysis, MRF involvement had significant impacts on OS ( p= .02), DFS ( p= .01) and LC ( p= .01). Conclusions: A dose-intensified radiotherapy in neoadjuvant chemoradiotherapy demonstrated contribution to tumor regression with acceptable toxicity, but led delayed incision healing after surgery. The impact of MRF involvement on survival merits further investigation. Clinical trial information: NCT01064999.
Neoadjuvant Radiotherapy Improves Overall Survival for T3/4N+M0 Rectal Cancer Patients: a Population-Based Study of 20300 Patients

Abstract:BACKGROUND:Neoadjuvant radiotherapy (RT) has been shown to improve local control; however, whether it can improve overall survival (OS) in locally advanced rectal cancer (LARC) patients remains controversial. We therefore aimed to examine the benefits of surgery alone, neoadjuvant radiotherapy (RT), adjuvant RT, and surgery plus chemotherapy in stage II (T3/4N0M0) and III (any T and N + M0) on the OS of rectal cancer patients. METHODS:Date from the Surveillance, Epidemiology, and End Results (SEER) database diagnosed between 2004 and 2016 were used. Kaplan-Meier analyses were used to compare patient prognoses across different treatment modalities. Cox hazard regression analysis were used to identify independent predictors of OS. RESULTS:For stage T3/4N0M0 patients, neoadjuvant RT, adjuvant RT, and surgery plus chemotherapy resulted in similar OS (all p > 0.05; mean survival, 115.89 months (M), 111.97 M, and 117.22 M, respectively), with better OS observed in these patients than in patients who underwent surgery alone (all p < 0.001, mean survival, 88.96 M). For stage T1/2N + M0 patients, neoadjuvant RT, adjuvant RT, and surgery plus chemotherapy resulted in similar OS (all p > 0.05; mean survival, 121.50 M, 124.25 M, and 121.20 M, respectively), with better OS observed in these patients than in patients who underwent surgery alone (all p < 0.001, mean survival 83.81 M). For stage T3/4N + M0 patients, neoadjuvant RT (HR = 0.436; 95% CI, 0.396~0.478; p < 0.001) resulted in significantly longer OS than adjuvant RT and surgery plus chemotherapy (mean survival, 104.47 M, 93.94 M, and 93.62 M, respectively), with better OS observed in these patients than in patients who underwent surgery alone (all p < 0.001, mean survival 54.87 M). Older age (> 60 years), black race, unmarried status, high tumour grade, and tumour size > 5 cm were all associated with a poor prognosis (all p < 0.05). CONCLUSIONS:Neoadjuvant RT, adjuvant RT, and surgery plus chemotherapy results in better OS than surgery alone in LARC patients. Neoadjuvant RT has the potential to be highly recommended over adjuvant RT and surgery plus chemotherapy for T3/4N + M0 patients; however, it showed no OS advantage over adjuvant RT or surgery plus chemotherapy for T3/4N0M0 and T1/2N + M0 patients.
Efficacy of the radiotherapy combined with oxaliplatin and capecitabine in the treatment of local advanced rectal cancer

Abstract:Objective: To evaluate the efficacy and safety of preoperative radiotherapy (RT) combined with concurrent oxaliplatin and capecitabine chemotherapy in the treatment of local advanced rectal cancer at T3 to T4 stage. Methods: Fifty-four patients with T3-T4 local advanced rectal cancer received preoperative pelvic radiation (50 Gy in 25 fractions) and concurrent combined chemotherapy. Oxaliplatin was administered at 100 mg/m2 on d 1 and 22 by intravenous infusion and capecitabine was administered orally at 825 mg/m2 bid on d 1 to 14 and d 22 to 35. Results: Fifty-one patients out of 54 eligible patients received surgery. Among them 10 patients (19.7%) achieved pathologic complete response (pCR). R0 resections were achieved in 100% of patients, and sphincter preservation was accomplished in 51.0% of the patients. The 1-year survival rate was 91.0%, and 2-year survival rate was 83.8%. The most common adverse reaction was hematologic toxicity. The grade 3 adverse reactions mainly included anemia (1.9%), leucopenia (11.2%) and thrombocytopenia (1.9%). The most common non-hematologic toxicity was diarrhea. Conclusion: Preoperative radiotherapy combined with concurrent capecitabine and oxaliplatin chemotherapy was an effective and safe therapeutic regimen for local advanced rectal cancer with low frequency of adverse reactions.
Three-dimensional Conformal Radiotherapy Combined with FOLFOX4 Chemotherapy for Unresectable Recurrent Rectal Cancer

Abstract:AIM:To investigate the effect of three-dimensional conformal radiotherapy (3-DCRT) in combination with FOLFOX4 chemotherapy for unresectable recurrent rectal cancer.METHODS:Forty-eight patients with unresectable recurrent rectal cancer were randomized and treated by 3-DCRT or 3-DCRT combined with FOLFOX4 chemotherapy between September 2001 and October 2003. For the patients without prior radiation history, the initial radiation was given to the whole pelvis by traditional methods with tumor dose of 40 Gy, followed by 3-DCRT for the recurrent lesions to the median total cumulative tumor dose of 60 Gy (range 56-66 Gy); for the post-radiation recurrent patients, 3-DCRT was directly given for the recurrent lesions to the median tumor dose of 40 Gy (36-46 Gy). For patients in the study group, two cycles chemotherapy with FOLFOX4 regimen were given concurrently with radiotherapy, with the first cycle given simultaneously with the initiation of radiation and the second cycle given in the fifth week for patients receiving conventional pelvis radiation or given in the last week of 3-DCRT for patients receiving 3-DCRT directly. Another 2-4 cycles (average 3.6 cycles) sequential FOLFOX4 regimen chemotherapy were given to the patients in the study group, beginning at 2-3 wk after chemoradiation. The outcomes of symptoms relieve, tumor response, survival and toxicity were recorded and compared between the study group and the control group.RESULTS:For the study group and the control group, the pain-alleviation rates were 95.2% and 91.3% (P > 0.05); the overall response rates were 56.5% and 40.0% (P > 0.05); the 1-year and 2-year survival rates were 86.9%, 50.2% and 80.0%, 23.9%, with median survival time of 25 mo and 16 mo (P < 0.05); the 2-year distant metastasis rates were 39.1% and 56.0% (P = 0.054), respectively. The side effects, except peripheral neuropathy which was relatively severer in the study group, were similar in the the two groups and well tolerated.CONCLUSION:Three-dimensional conformal radio-therapy combined with FOLFOX4 chemotherapy for unresectable recurrent rectal cancer is a feasible and effective therapeutic approach, and can reduce distant metastasis rate and improve the survival rate.
Two-week Course of Preoperative Radiotherapy for Locally Advanced Rectal Adenocarcinoma: 8 Years' Experience in a Single Institute.

Abstract:OBJECTIVES:To evaluate local control and survival in locally advanced rectal adenocarcinoma patients who underwent a preoperative 2-week course of radiotherapy (RT) and to identify prognostic factors influencing the survival rate. METHODS:We analyzed 377 consecutively treated patients with locally advanced (T3/T4 or node positive) rectal adenocarcinoma. All patients underwent a preoperative 2-week course of RT (30 Gy in 10 fractions) followed by curative surgery. Regression model was used to examine prognostic factors for the disease-free survival (DFS) and overall survival (OS) rates. The Statistical Analysis System software package, version 9.3, was used for analysis. RESULTS:The median follow-up for all living patients was 63.8 months (range, 5.1 to 131.7). The 5-year DFS and OS rates were 64.5% (95% CI, 59.0-69.4) and 75.6% (95% CI, 70.5-80.0), respectively. The 5-year cumulative incidences of local recurrence and distant metastases were 5.4% (95% CI, 2.9-7.9) and 29.0% (95% CI, 23.9-30.1), respectively. The pathologic complete response rate was achieved in 17 patients (4.5%). The Multivariate Cox Regression model showed that factors affecting DFS were the surgical technique, pre-RT pathologic grade, ypT, ypN, and comorbidity; and factors improving OS were low anterior resection, low pre-RT grade, low ypT, and low ypN. CONCLUSIONS:Patients treated with preoperative RT with 30 Gy in 10 fractions had similar local control, 5-year DFS and OS to reported long course RT regimen. The surgical technique, pre-RT pathologic grade, ypT, and ypN seemed to affect the OS. Further study on combining a 2-week course of preoperative RT with concurrent chemotherapy would be warranted.

Abstract:Background: Preoperative radio-chemotherapy (RCT) may play an important role in decreasing local recurrence and possibly improving survival for patients with advanced rectal carcinoma. Patients and methods: Between 1995 and 1998 we treated 30 patients (pts) (median age 61.5 years) with advanced low rectal cancer. Radiotherapy was administered using a 10-MV linear accelerator delivering a total dose of 45 Gy to the pelvis over 5 weeks. 5-FU (500 mg/m2/d i.v.) was given concomitantly on days 1-3 and 29-31. Surgery was performed 2-4 weeks after RCT and included abdomino-perineal resection (APR) in 15 pts, anterior resection in 11 pts, transanal resection in 1 pt, and explorative laparotomy in 1 pt; 2 pts refused surgery. Results: There were no serious toxicities associated with RCT. Mild to moderate transient radiation dermatitis was noted in 3 pts, and grade II diarrhea in 4 pts. Postoperative pathologic staging was as follows: no tumor found (CR): 4 pts, MAC B1 (T2N0M0): 5 pts, MAC B2 (T3N0M0): 17 pts, B3 (vaginal involvement): 1 pt, MAC C2 (T3N1M0): 2 pts, and MAC D: 1 pt. Necrosis and/or fibrosis was noted as a prominent histopathologic feature. Conclusion: Preoperative RCT in this series of 30 pts with advanced rectal cancer was well tolerated and associated with a high response rate (13% CR, 17% PR, 57% NC). 5-year overall survival was 70%, and 8-year survival was 58%.
Efficacy and safety of neoadjuvant chemoradiotherapy versus neoadjuvant radiotherapy for stages II-III rectal cancer: a Meta-analysis

Abstract:Objective: To evaluate the clinical efficacy and safety of neoadjuvant chemoradiotherapy (nCRT) versus neoadjuvant radiotherapy (nRT) for patients with stages Ⅱ-Ⅲ rectal cancer. Methods: A computer-based online search of PubMed, EMBase, Cochrane Library, China Journal Full-text Database, Wanfang Database and Chinese sci-tech periodical full-text database was performed to include randomized controlled trials (RCTs) about nCRT vs nRT (including neoadjuvant short-course radiotherapy, nSRT) in patients with stages Ⅱ-Ⅲ rectal cancer in accordance with the inclusion and exclusion criteria. The relevant literatures and conference proceedings were also manually searched to include eligible RCTs. After evaluating the quality of included trials and extracting data, the data analysis was conducted using RevMan 5.2 software. Results: A total of six randomized controlled trials (RCTs) involving 2 602 patients with stages Ⅱ-Ⅲ rectal cancer were included. The results of Meta-analysis showed that the nCRT group had higher radical resection rate (odds ratio: 1.62, 95% confidence interval: 1.14-2.30; P = 0.008), pathological complete response rate (odds ratio: 3.17，95% confidence interval: 2.28-4.40; P < 0.000 01), and rate of severe side effects (grades 3-4) (odds ratio: 2.66, 95% confidence interval: 1.29-5.46; P = 0.008), as compared with nRT group. The five-year local recurrence (LR) rate in the nCRT group was obviously lower than that in the nRT group (odds ratio: 0.51，95% confidence interval: 0.39-0.67，P < 0.000 01). Regarding nCRT vs nSRT, the radical resection rate (odds ratio: 2.40, 95% confidence interval: 1.25-4.63; P = 0.009) and the pathological complete response rate (odds ratio: 4.28, 95% confidence interval: 1.72-10.68, P = 0.002) were also higher in the nCRT group than those in the nSRT group. Furthermore, there were no differences in two-, three- and five-year survival rates between nCRT and nRT groups (P < 0.05). Conclusion: As compared with nRT, the nCRT can obviously improve the radical resection rate and complete pathologic response rate, and reduce the local recurrence rate in patients with stages Ⅱ-Ⅲ rectal cancer, but it can cause more severe toxicities. Abstract:BACKGROUND: In China, standard neoadjuvant chemoradiation therapy has not been well accepted, not only because of financial constraints but also because of the poorly-tolerated long duration of the regimen.OBJECTIVE: The current study aimed to evaluate the impact of a modified neoadjuvant radiation regimen on the prognosis of rectal cancer patients in China.DESIGN: This was a nonrandomized cohort study evaluating outcomes of patients who chose to undergo preoperative radiotherapy compared with those who chose not to undergo preoperative radiotherapy (controls).SETTINGS: The study was carried out in Peking University Cancer Hospital, a tertiary care cancer center in China.PATIENTS: Records of patients with locally advanced, mid-to-low rectal cancer who underwent total mesorectal excision at Peking University Cancer Hospital from 2001 through 2005 were analyzed in this study.INTERVENTION: Patients who chose preoperative radiotherapy received a total dose of 30 Gy delivered in 10 once-daily fractions of 3.0 Gy each, with at least a 14-day delay of surgery after delivery of the last fraction.MAIN OUTCOME MEASURES: Tumor downstaging was evaluated. Local recurrence, distant metastases, and disease-free and overall survival were analyzed with the Kaplan-Meier method.RESULTS: A total of 101 patients accepted and 162 patients declined the modified preoperative radiotherapy regimen. Of the 101 patients receiving preoperative radiotherapy, 5 (5%) had a complete response, and 50 (50%) achieved TNM downstaging. The local recurrence rate was 5% with preoperative radiotherapy and 18% in the control groups (p = 0.02). Within the preoperative radiotherapy group, 5-year disease-free survival and overall survival rates were significantly higher in patients with T-, N-, or TNM-downstaging than in patients without downstaging. Evaluation of literature reports indicated that clinical safety and effectiveness of the modified protocol are comparable to results of standard neoadjuvant procedures.LIMITATIONS: The allocation to study groups was not randomized, and patient self-selection may have introduced bias, particularly because patients with greater financial means were more likely to choose to undergo the preoperative radiotherapy regimen.CONCLUSIONS: Compared with surgery alone, this modified preoperative radiotherapy regimen is associated with significantly reduced local recurrence and complication rates, with improved survival in patients who show downstaging. The modified protocol offers a clinical outcome equivalent to standard preoperative radiotherapy regimens while offering an alternative for increasing the flexibility of preoperative radiation regimens in China.
[Efficacy of neoadjuvant radiochemotherapy in treatment of locally advanced low rectal cancer]

Abstract:Objective: To explore efficacy of neoadjuvant radiochemotherapy in locally advanced low rectal cancer. Methods: From May 2001 to August 2005, 105 patients with locally advanced low rectal cancer (T3, T4) were treated by preoperative radiotherapy to pelvis, 2.0 Gy daily up to 40-46 Gy in 4-5 weeks concomitantly with oral capecitabine at 1250 mg x m(-2) x d(-1) for 10 weeks up to surgery. In all patients surgery was carried out under the rule of total mesorectal excision technique. Results: All patients finished the course of neoadjuvant radiochemotherapy. Among them, 36 patients experienced adverse effects. Thirteen patients resulted in complete tumor response and spared the operation. Ninety-two patients were operated on with radical resection, among them 71 patients with low anterior resection, 17 with Parks' colo-anal anastomosis and 4 with abdomino-perineal resection, so sphincter preservation was achieved in 96.2%. In postoperative pathological studies, 11 cases showed complete tumor regression. According to the TNM staging system, 24 cases were ranged T0N0, and 23 cases T2N0, 43 cases T3N0, 2 cases T4N0, 5 cases T2N1, 8 cases T3N1; and according to Dworak's tumor regression grading, 5 cases were ranked TGR0, and 18 cases TGR1, 11 cases TGR2, 47 cases TGR3, 24 cases TGR4. Pathologic downstaging was achieved in 78.1%, including complete response (TGR4) and intermediate response (TGR2 + 3). No operative death occurred. Anastomotic leakage was found in 5 cases, including 3 rectovaginal fistula. All patients have been followed up for 16-67 months, and lung metastasis occurred in 4 cases, liver metastasis in 2 patients and local recurrence in 4 patients. Three patients died of distant metastasis. The 3-year disease-free survival was 82.8% and overall survival was 96.5%. Conclusions: Neoadjuvant radiochemotherapy brings tumor down-staging and increases resectability and sphincter preservation, decreases recurrence and improves survival in locally advanced low rectal cancer.

Abstract:OBJECTIVE:To evaluate the efficacy of preoperative radiochemotherapy and investigate the influencing factors in rectal cancer.METHODS:Fifty-three patients with locally advanced rectal cancer were treated with radiochemotherapy before surgery. Three-field technique of radiation therapy was administered with 46 Gy, 2 Gy per fraction, five times a week. Two cycles of chemotherapy were carried out at day 1, 2 and day 21, 22 during the radiation course. Surgery was performed 4-6 weeks after the radiochemotherapy. Response of preoperative radiochemotherapy was evaluated in all the patients by endoscopic ultrasonography (EUS), spiral computed tomography (SCT) and pathology. Influencing factors of the efficacy of radiochemotherapy were evaluated by univariate and Logistic analysis.RESULTS:Univariate analysis revealed that tumor size and histological grading were associated with the efficacy of preoperative radiochemotherapy. Logistic regression analysis showed that only tumor size was the significant predictive factor for response to preoperative radiochemotherapy. All patients underwent surgical resection after preoperative radiochemotherapy. The tumor was reduced by an average of 32.1%. T-level down-staging was 64.2%. Nodal negativity was 58.1%. Complete pathologic remission occurred in 11 patients.CONCLUSIONS:Preoperative radiochemotherapy can shrink the primary tumor and decrease lymph node metastasis rate. Patient with small tumor may have better response to preoperative radiochemotherapy.

Abstract:Background and Purpose:Rectal cancer is a common malignancy in China.Surgery is the first option for the treatment.Adjuvant radiotherapy can improve local control after surgery for stage Ⅱ/Ⅲ patients.However,whether escalation of radiation dose can further improve the local control and even the survival are unknown.This study analyzed the efficacy of adjuvant radiotherapy and its dose-response relationship.Methods:Seventy-four patients with stage Ⅱ and stage Ⅲ rectal cancer who received postoperative adjuvant radiotherapy were retrospectively analyzed.All the patients received conventional irradiation with 1.8-2.0Gy per fraction daily.The total dose was 40-70Gy(median:54Gy).Sixty-two patients received 1-10 cycles of sequential or concurrent 5-FU based chemotherapy.SPSS 11.5 is used for statistics,the survival rate is calculated by the actuarial method of Kaplan and Meier.Results:The median follow-up was 44.6months(ranges 7.3-119.9 mo).The 5-year overall survival rate and disease-free survival rate are 58% and 60% respectively.Fourteen patients had local recurrence,and 17 patients experienced distant metastasis.All patients were subdivided into three dose groups with the dosage of ≤50 Gy,50-60 Gy and >60 Gy.The 5-year local control rates are 92%,71% and 87% respectively(P=0.9194),and 5-year overall survival rate are 68%,62% and 53% respectively(P=0.4194).There is no significant difference of overall survival and local control rate between these three dose groups.Five patients with dose of more than 50Gy died of late toxicities.Conclusions:Adjuvant radiotherapy for Stage Ⅱ and Ⅲ patients with rectal cancer dose not show dose response.There is no improvement of local control and survival due to the escalation of dose.The dose of conventional radiotherapy is better at less than 50Gy.Overdosage may lead to severe toxicities.

Abstract:OBJECTIVE:To investigate the effect of preoperative radiotherapy as an adjunctive approach on proliferation and apoptosis of cancer cells in rectal cancer.METHODS:Eighty patients with (p)TNM stage II or III rectal cancer were enrolled from April 2000 to December 2002. The patients were divided into two groups, which were group received surgical treatment alone (40 cases) and group received preoperative adjuvant radiotherapy with a radiological dose of 40 G( y ) (40 cases). The expression of Ki- 67 protein in rectal cancer was detected immunohistochemically and apoptosis was detected by TUNEL technique.RESULTS:In radiotherapy group, the tumor volume shrank markedly in 14 cases, partially in 18,slightly in 8 with an overall efficiency rate of 80% .There was significant positive correlation between spontaneous apoptosis and Ki- 69 marker index in cancer tissues before radiotherapy(gamma =0.563P=0.027). The expression of Ki- 67 in rectal cancer tissues was significantly lower after radiotherapy than that before radiotherapy, as well as in the control group(P=0.017 vs. P=0.011).The expression of Ki- 67 in paratumor tissues 2cm from tumor was significantly lower after radiotherapy than that before radiotherapy. Apoptosis index (AI) in rectal cancer tissues was significantly higher after radiotherapy than that before radiotherapy, and that in the control group (P=0.013 vs. P=0.018).CONCLUSION:Radiotherapy can inhibit proliferation and induce apoptosis of rectal cancer cells. The expression level of Ki- 67 can be considered as a screening index for preoperative radiotherapy.

Abstract:For locally advanced rectal cancer, multiple disciplinary team (MDT) has been the standard care. As a part of MDT, radiotherapy (RT) plays an important role in clinical practice. With mounting clinical evidence, RT is swifted from postoperative administration to preoperative. Compared to post-operative RT, pre-operative RT increased local control rate significantly, but not in DFS and OS. Two different preoperative RT models, short-course RT and long-course chemoradiotherapy demonstrated similar local control and long-term survival. However, a better tumor regression was observed in long-course CRT. In recent year, some small sample size studies, optimized the current pre-operative RT model, such as prolonging the interval between RT and surgery and adding consolidation chemotherapy (CT) in the interval, or adding induction CT before pre-operative RT. These optimizations decreased toxicities and increased treatment compliance, then improved the prognosis to a certain extent.
Neoadjuvant Therapy for Advanced Rectal Carcinoma in China: Whether Radiochemotherapy is Superior to Radiotherapy?

Abstract:To verify whether the 30 Gy preoperative radiotherapy regimen is effective to advanced rectal cancer, and whether the preoperative chemoradiation offers an advantage in sphincter preservation and tumor control compared with irradiation alone.A total of 141 patients administered neoadjuvant treatment with resectable lower rectal carcinoma from 2002 to 2006 were collected retrospectively. The patients were divided into two groups: preoperative radiotherapy alone (30Gy by 10 fractions) (PRT group) and preoperative chemoradiotherapy (PCRT group). All patients underwent radical surgery after neoadjuvant treatment.The overall sphincter-preservation rate was 68.8% (97/141), with no significant difference between the two groups. The overall downstaging rate was 48.2% (68/141), including 4 patients completely response (2.8%). The T and N downstaging rate were 30.5% (43/141) and 53.8% (57/106) respectively, showing no statistically difference between the two groups. The 2-year overall survival rate was 93.6%; no survival benefit were observed in PCRT group. The 2-year cumulative local recurrence rates were similar as well (4.2% vs 6.7%, P=0.63). Two patients with severe marrow suppression higher than grade 3 and 1 patient with severe perineum ulcer was observed in PCRT group, which did not occur in PRT group.The preoperative adjuvant treatment of 30Gy radiotherapy alone may be an optional treatment for Chinese lower rectal carcinoma. Preoperative chemoradiotherapy does not show actual superiority compared with radiotherapy alone.

Abstract:Abstract Background We conducted a phase II study to investigate the feasibility and safety of preoperative radiochemo-therapy experimental fractionation, using intensity-modulated radiation therapy with simultaneous integrated boost (IMRT SIB) to shorten the overall treatment time without dose escalation in intermediate/locally advanced rectal cancer with the aim to improving treatment outcome. Patients and methods A total of 51 patients with operable stage II–III rectal carcinoma were included between January 2014 and January 2015. Fifty patients completed preoperative IMRT treatment with an elective dose of 41.8 Gy and simultaneously delivered 46.2 Gy to T2/T3 and 48.4 Gy to T4 tumour in 22 fractions, with concomitant capecitabine (825 mg/m2/12 h, including at weekends). Median follow-up was 70 months (range 11–80 m). Results Forty-seven patients completed treatment per protocol. Acute toxicity occurred in 2 (4%) patients. R0 resection was achieved in all but 1 and pathologic complete response (pCR) in 12 (25.5%) patients who had 5-year overall survival (OS), disease-free survival (DFS) and local control (LC) of 91.7%, 100% and 100%, respectively. The intention-to-treat analysis showed that the type of surgery significantly moderated OS and DFS, while total downstaging and pN were predictive for DFS only. For treatment per protocol 5-year OS, DFS and LC were 80.9% (95% confidence interval [CI] 69.7–92.1), 77.1% (95% CI 65.1–89.1) and 95.2% (95% CI 88.7–100), respectively. The proportion of patients with severe late (CTCAE G ≥ 3) gastrointestinal, urinary and sexual toxicity was 15%, 2% and 8% respectively, with one reported secondary carcinoma. Conclusions Preoperative IMRT-SIB without dose escalation was well tolerated, with a low acute toxicity profile, we achieved a high rate of pCR and showed encouraging 5-year OS, DFS and LC.

Abstract:Abstract Background Although neoadjuvant chemoradiation (nCRT) followed by surgery is standard treatment for locally advanced esophageal or gastroesophageal junction (E/GEJ) cancer, the optimal radiation dose is still under debate. Objective The aim of this study was to assess the impact of different preoperative radiation doses (41.4 Gy, 45 Gy or 50.4 Gy) on pathologic response and survival in E/GEJ cancer patients. Methods All consecutive patients with E/GEJ tumors, treated with curative intent between January 2009 and December 2016 in two referral centers were divided into three groups (41.4 Gy, 45 Gy and 50.4 Gy) according to the dose of preoperative radiotherapy. Pathologic complete response (pCR) rates, postoperative morbidity, overall survival (OS) and disease-free survival (DFS) were compared among the three groups, with separate analyses for adenocarcinoma (AC) and squamous cell carcinoma (SCC). Results From the 326 patients analyzed, 48 were included in the 41.4 Gy group (14.7%), 171 in the 45 Gy group (52.5%) and 107 in the 50.4 Gy group (32.8%). Postoperative complication rates were comparable ( p = 0.399). A pCR was observed in 15%, 30%, and 34% of patients in the 41.4 Gy, 45 Gy and 50.4 Gy groups, respectively ( p = 0.047). A 50.4 Gy dose was independently associated with pCR (odds ratio 2.78, 95% confidence interval 1.10–7.99) in multivariate analysis. Within AC patients, pCR was observed in 6.2% of patients in the 41.4 Gy group, 29.2% of patients in the 45 Gy group, and 22.7% of patients in the 50.4 Gy group ( p = 0.035). No OS or DFS differences were observed. Conclusions A pCR was less common after a preoperative radiation dose of 41.4 Gy in AC patients. Radiation dose had no impact on postoperative morbidity, long-term survival, and recurrence.

Abstract:Objective To evaluate the efficacy and tolerance of preoperative concurrent chemoradiotherapy in the treatment of locally advanced middle-low rectal cancer.Methods From June 2007 to June 2013,51 untreated patients with histopathologically proven rectal cancer (T3/T4 or N (+))were included in this study.Three-dimensional radiotherapy was delivered to the whole pelvic cavity at 45.0-50.4 Gy/25-28 fractions.Two cycles of chemotherapy with FOLFOX4 or XELOX were given concurrently at weeks 1 and 4 of radiotherapy.Surgery was performed at 4-8 weeks after chemoradiotherapy.Adjuvant chemotherapy with FOLFOX4 or XELOX was given within one month after surgery.The Kaplan-Meier method was used to calculate survival rates,and the log-rank test was used for univariate analysis;the Cox regression model was used for multivariate prognostic analysis.Results Fortynine patients completed the preoperative chemoradiotherapy and surgery.The median follow-up was 2.9 years.The overall sphincter preservation rate was 65％;the overall downstaging rate was 59％.Ten (20.4％) of all patients achieved a pathologic complete response (pCR).Grade ≥3 toxicities occurred in 25％ of all patients,and the overall postoperative complication rate was 31％.The 3-and 5-year sample sizes were 24,12,respectively.The 3-and 5-year overall survival rates were 81％ and 69％,respectively;the 3-and 5-year disease-free survival (DFS) rates were 76％ and 60％,respectively;the 3-and 5-year local recurrence-free survival (LRFS) rates were 78％ and 70％,respectively;the distant metastasis-free survival rates were 82％ and 74％,respectively.The multivariate analysis showed that tumor downstaging was an independent prognostic factor for 5-year DFS and LRFS.Conclusions For locally advanced middle-low rectal cancer,preoperative radiotherapy with concurrent FOLFOX4/XELOX chemotherapy can increase pathologic downstaging rate,pCR rate,and sphincter preservation rate.Patients with tumor downstaging may have a better survival advantage.
