Abstract:Primary Sjogren's syndrome (pSS) is a chronic autoimmune disease, characterized by lymphocytic infiltration into exocrine glands, which causes dry eyes, dry mouth, and systemic damage. Although the precise etiology of pSS is not clear yet, highly activated B cells, abundant anti-SSA/Ro, and anti-SSB/La autoantibodies are the hallmarks of this disease. Follicular helper T cells (Tfh), a subset of CD4(+)T cells, with cell surface receptors PD-1 and CXCR5, express ICOS, transcription factor Bcl-6, and a cytokine IL-21. These cells help in the differentiation of B cells into plasma cells and stimulate the formation of germinal center (GC). Previous studies have demonstrated abundant Tfh cells in the peripheral blood and salivary glands (SGs) of the patients with pSS, correlated with extensive lymphocytic infiltration of the SGs and high disease activity scores. Patients with pSS who are treated with abatacept (CTLA-4 Ig) show fewer circulating Tfh cells, reduced expression of ICOS, and lower disease activity scores. Recently identified follicular regulatory T (Tfr) cells, a subset of regulatory T cells, control the function of Tfh cells and the GC reactions. Here, we summarize the observed alterations in Tfh and Tfr cell numbers, activation state, and circulating subset distribution in pSS. Our goal is to improve the understanding of the roles of Tfh and Tfr cells (surface marker expression, cytokine production, and transcription factors) in the pathogenesis of pSS, thus contributing to the identification of candidate therapeutic agents for this disease.

Effect of Chinese Herbal Medicines for Nourishing Yin, Supplementing Qi, and Activating Blood on Reproductive Endocrine Activity and Immune Functions in Patients with Primary Sjogren's Syndrome.

Abstract:OBJECTIVE:To investigate the effect of Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood on the reproductive endocrine-immune network and its mechanisms in patients with primary Sjogren's syndrome (pSS). METHODS:Seventy pSS patients were randomly assigned to two groups using a randomized digital table: the integrative therapy group (36 cases) and the control group (34 cases). Thirty healthy subjects were taken as a normal group. The control group was treated with hydroxychloroquine sulfate tablets alone, and the integrative therapy group was treated by Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with hydroxychloroquine sulfate tablets. The treatment course was 6 months for both groups. Before and after treatment, serum estradiol (E2), testosterone (T), luteinizing hormone (LH), prolactin (PRL) by radioimmunoassay and immunoglobulin (IgG) by immunodiffusion, erythrocyte sedimentation rate (ESR) by Westergren, interferon-γ (IFN-γ) and interleukin-4 (IL-4) by enzyme linked immunosorbent assay were determined. RESULTS:E2 and T levels in all patients were lower than those of normal subjects before treatment (P<0.05) and were increased significantly after 6-month treatment (P<0.05). ESR, FSH, LH, IgG, IFN - γ, IL - 4 and ratios of E2/T, and IFN -γ/IL in the patients were higher than those of normal subjects before the treatments (P<0.05), and were reduced significantly after the treatments (P<0.05). The T and IFN - γ levels and E2/T ratio in the patients treated with integrative therapy were reduced significantly compared with the control group (P<0.05). However, the PRL levels before and after treatment were not significantly changed in the two groups (P>0.05). The ratios of E2/T and IFN -γ/IL-4, and levels of IgG and ESR were positively correlated before and after treatment (P<0.05). CONCLUSIONS:The ratios of E2/T and IFN -γ/IL-4 might be used as indicators of pSS activity. Chinese herbal medicines for nourishing yin, supplementing qi, and activating blood combined with Western medicine could improve the therapeutic effect by regulating the reproductive endocrine-immune network in pSS patients.
Primary Sjogren Syndrome in Han Chinese Clinical and Immunological Characteristics of 483 Patients

Abstract:The epidemiological characteristics of Sjogren syndrome (SS) are significantly varied in different countries. We conducted the present study to survey the epidemiological characteristics of primary SS in China. We recruited 483 primary SS patients from 16 Chinese medical centers nationwide from January 2009 to November 2011 and assessed salivary and lacrimal gland dysfunction, organ involvement, and autoimmunity in these patients. The cohort included 456 women and 27 men (ratio, 17:1; mean age at onset, 42 +/- 11 years; median age at diaposis, 49 years; range, 41-56 years). Male patients showed a lower frequency of xerophthalmia (37.0% vs 60.7%) and a higher frequency of arthritis (40.7% vs 16.4%). Young-onset patients showed a higher frequency of low C3 levels (57.7% vs 36.3%) and pancytopenia (22.2% vs 8.8 A). Patients with systemic involvement had a higher frequency of immunoglobulin A (IgA) (39.4% vs 22.5%) and immunoglobulin M (10,1) (12.4% vs 37.9%). Patients with pulmonary involvement had a higher parotid enlargement (21.4% vs 10.2%), purpura (12.1% vs 5.7%) and higher anti-La/SS-B (61.7% vs 41.8%), immunoglobulin G (IgG) (80.7% vs 64.6%) and IgA (48.9% vs 30.6%) levels. Patients with anti-Ro/SSA antibodies had more frequent exocrine gland symptoms and some extraglandular symptoms and immunological alterations. Compared with previous studies performed in other countries, SS patients in China showed particular clinical manifestation, systemic involvement, and immunological alterations.
Clinical stratification of 1318 Primary Sjögren's Syndrome patients

Abstract:Objective: Primary Sjögren's Syndrome (pSS) is a complex autoimmune disorder characterized by diverse clinical manifestations yet lacking effective therapeutic strategies currently. This study aims to gain a thorough understanding of the clinical landscape of pSS and further delineate its clinical subtypes, thereby enabling the efficient management for pSS. Methods: We conducted a cross-sectional observational study of 1318 pSS patients. The pSS patients were categorized and compared based on gender, anti-SSA antibodies, and labial salivary gland biopsies (LGSB). Unsupervised clustering analysis was employed to identify pSS subtypes using systemic involvement among patients. Furthermore, we assessed clinical and biological variances among these subtypes. Results: Through group comparisons, we observed more pronounced extraglandular manifestations among male patients, SSA-negative group, and those with positive LGSB results. Based on systemic involvement, pSS patients were categorized into four groups. C1 exhibited minimal systemic involvement, lacking hematologic or serologic manifestations, with the lowest ESSDAI scores. C2 presented with serologic changes in all patients, partial joint involvement, and no hematologic systemic manifestations. C3 lacked joint involvement but all members displayed hematologic systemic involvement, with higher rates of renal, cutaneous, and systemic manifestations. C4 encompassed patients with joint and hematologic involvement, displaying the highest ESSDAI scores. The positivity rates of antibodies, immunological parameters, and inflammatory markers exhibited significant differences among the groups. Furthermore, notable variances were observed in the expression of peripheral blood transcriptomic modules among these groups. Conclusion: In this cohort study, we summarized the clinical characteristics of Chinese patients with pSS and identified four distinct subgroups of pSS based on systemic involvement, revealing clinical and molecular disparities that unveil distinct pathobiological endotypes. Our findings hold significant implications for clinical management.
Patients with Early-Onset Primary Sjögren’s Syndrome Have Distinctive Clinical Manifestations and Circulating Lymphocyte Profiles

Abstract:Objectives To further investigate the clinical characteristics and circulating lymphocyte profiles of patients with early-onset primary Sjogren's syndrome (pSS). Method Data of 333 patients with pSS were analysed retrospectively. Early onset was defined as a pSS diagnosis at an age of 35 years or younger. The clinical, laboratory and immunophenotypic profiles of peripheral blood lymphocyte subsets were compared between early- and later-onset pSS. Results Thirty-six (10.81%) patients matched the definition of early-onset pSS, with age at disease onset being 28.97 (5.53) years. Elevated serum IgG level (77.14% vs 31.16%, P <0.001), low C3 (41.67% vs 20.20%, P =0.004) and C4 levels (27.78% vs 6.40%, P <0.001), anti-SSA positivity (91.67% vs 51.85%, P <0.001) and anti-SSB positivity (50% vs 20.54%, P <0.001) were more frequent in early-onset patients. The frequencies of hematological (80.56% vs 52.53%, P =0.001), renal (19.44% vs 5.05%, P =0.005) and mucocutaneous involvement (50% vs 22.56%, P <0.001) were significantly higher in the early-onset pSS group, which showed a higher 2010 EULAR SS Disease Activity Index (ESSDAI) [11(6.25-17) vs 7(3-12); P =0.003], compared with the later-onset group. In addition, profound CD4(+) T-cell lymphopenia was found in patients with early-onset. Conclusions Patients with early-onset pSS have distinctive clinical manifestations and greater activation of the cellular immune system, present with more severe clinical symptoms and immunological features, have increased activation of circulating T cells and have an unfavourable prognosis. Thus, they require more positive treatment with glucocorticoids and/or immunosuppressants and merit closer follow-up and regular monitoring.
Characteristics of Primary Sjögren’s Syndrome Patients with IgG4 Positive Plasma Cells Infiltration in the Labial Salivary Glands

Abstract:The purpose of this study was to investigate the characteristics of primary Sjögren’s syndrome (pSS) patients with IgG4 positive (IgG4+) plasma cell infiltration in labial salivary glands (LSGs). Paraffin sections of LSGs from 336 pSS patients were stained with IgG4 and IgG monoclonal antibodies. According to the infiltration of IgG4+ plasma cells, patients were divided and clinical and serological characteristics were analyzed and compared. Based on the infiltration of IgG4+ plasma cells in the LSGs, patients were divided into three subgroups, low IgG4, moderate IgG4, and high IgG4 groups. A negative association between the number of infiltrated IgG4+ plasma cells and the disease characteristics was observed. We found that the higher the IgG4+ expression in plasma cells, the lower the positive rates of serum anti-SSA antibodies, anti-SSB antibodies, antinuclear antibodies (ANA), and rheumatoid factor (RF). Besides, patients from the high IgG4 group had the highest frequency of interstitial lung disease (ILD, 30.6%) and tubulointerstitial nephritis (TIN, 13.9%), but the lowest frequency of leucopenia (13.9%), thrombocytopenia (11.1%), and abnormal thyroidal function (0%). PSS patients with different IgG4+ plasma cells infiltration in the LSGs had distinctive clinical and laboratory characteristics. It may help us to further understand the role of IgG4+ plasma cells in pSS.
Gender Differences in Clinical and Immunological Characteristics and the Pathogenic Study of Primary Sj(o)gren's Syndrome

Abstract:Objective To investigate the clinical and immunological features of primary Sj(o)gren's syndrome (pSS) in both sexs,and to find out the pathophysiology of pSS.Methods Clinical data of 110 pSS cases were analyzed retrospectively,and cytokine levels of interleukin (IL)-2,IL-4,IL-6,IL-10,IL-17A,Tumour necrosis factor (TNF)-α,interferon (IFN)-α were measured (male=20,female=90) in patients and healthy controls (n=30) using enzyme linked immunosorbent assay (ELISA).Results Compared with female patients,no significant difference was found in male patientsin symptoms of dry mouth and dry eye (90.0％ vs 98.9％,x2=4.874,P＞0.05);the frequency of parotid gland enlargement and lymphadenectasis were higher in male (40.0％ vs 5.6％,x2=18.629,P＜0.01;25.0％ vs 6.7％,x3=6.111,P＜0.05);and the level of immunoglobu lin (Ig)A and C4 [2.2(1.5,3.0) g/L vs 3.3(2.5,5.0) g/L,Z=-3.119,P＜0.01;(0.15±0.05) g/L vs (0.19±0.08) g/L,t=-2.659,P＜0.05] were lower in male,as well as the incidence of positive anti SSA/SSB antibodies (55.0％ vs 78.8％,x2=4.921,P＜0.05).Cytokine levels of IL-2,IL-6,TNF-α (t=-3.783,-6.193,-2.065,P＜0.05) were higher in pSS than HC.Compared with female patients,cytokine levels of IL-10,IFN-γ (t=-1.075,6.286,P＜0.05) were higher in male,however,the levels of IL-2,TNF-o (t=-3.472,-5.867,P＜0.01) were lower in male.Conclusion There are differences in the cytokine levels secreted from Thl and Th2 cell between male and female patients of pSS,which may relate to the clinical and immunological characteristics and can help to reveal the pathophysiology of pSS.
Seronegative primary Sjögren's syndrome, a distinct subtype of primary Sjögren's syndrome in Chinese patients

Abstract:Background: To investigate the clinical and immune characteristics of patients with primary Sjögren's syndrome (pSS) who were negative for anti-Sjögren's-syndrome-related antigen A antibodies (anti-SSA) and anti-Sjögren's-syndrome-related antigen B antibodies (anti-SSB) in Chinese population. Methods: A retrospective study were performed and 232 patients with pSS were analyzed. Patients positive for anti-SSA or/and anti-SSB were termed as seropositive pSS, and these negative for both anti-SSA and anti-SSB (non-antinuclear antibodies) as seronegative pSS. Clinical manifestations and laboratory findings were compared between the two groups. Results: Among the 232 patients with pSS, 192 (82.8%) were seropositive pSS and 40 (17.2%) were seronegative pSS. Compared to seropositive pSS, seronegative pSS were older and with higher percentage of low disease activity (ESSDAI < 5), xerostomia and xerophthalmia, with higher platelet count and level of creatine kinase. This subgroup was with lower levels of gamma globulin, immunoglobulin G, immunoglobulin A and autoantibodies including rheumatoid factor and antinuclear antibody in serum, and less immunoglobulin G deposition in labial gland. Conclusion: Seronegative pSS was a distinct subtype of pSS different from seropositive pSS. Clinical manifestations in seronegative pSS subgroup were restricted to exocrine gland and less B lymphocyte activation, while seropositive pSS were prone to present with systemic involvement and high disease activity. Specific underlying pathogenesis mechanisms and therapeutic strategies in this subgroup needed to be further studied.
The clinical characteristics of 573 cases of primary Sj(o)gren's syndrome

Abstract:Objective To analyze the clinical manifestations, immunological features and prognosis of primary Sj(o)gren's syndrome (pSS). Methods Five hundred and seventy-three patients who fulfilled the 2002 international classification (criteria) for pSS from Peking Union Medical College Hospital between 1985 and 2005 were screened retrospectively and compared with others. T-test, nonparametric test and Chi-square test were used data analysis. Results ① Among the 573 cases of pSS patients, women accounted for 91.4%. Mean age of onset was significantly earlier than foreign patients [(39.0±13.7) yrs vs (52.7+0.9) yrs, P<0.01]. The average duration from disease onset to pSS diagnosis was 48 mo. ② Dry mouth (84.5%) and dry eyes (70.0%) were the most common symptoms, somehow lower than foreign patients (P<0.0l). The positivity of xerophthalmia, xerostomia and salivary gland biopsy were high (91.9%, 94.8% and 90.7%, respectively). ③ 91.4% patients suffered from systemic involvement. Compared with other foreign studies, incidence of fever 41.0%, myositis 4.9%, pericardial effusion 14.8%, pulmonary involvement 42.3%, renal involvement 33.5%, thyroid involvement 32.7% and pancrease involvement 5.6% (P<0.01) was significantly high, while the incidence of fatigue, lymphadenectasis and Raynaud's phenomenon (P<0.01) was significantly lower. ④ Risk factor for poor prognosis was as following: pulmonary artery hypertension, liver damage and interstitial lung disease. Conclusion Chinese pSS isdifferent from those foreign countries in age of onset, systemic involvement, autoantibodies and proportional mortality rate. Lung and liver damage are the high risk factors for poor disease prognosis.

Abstract:Primary Sjögren's syndrome (pSS) is a complex autoimmune disease characterized by lymphocytic infiltration and exocrine dysfunction, particularly affecting the salivary gland (SG). We employed single-cell RNA sequencing to investigate cellular heterogeneity in 11 patients with pSS and 5 non-SS controls. Notably, patients with pSS exhibited downregulated SOX9 in myoepithelial cells, potentially associated with impaired epithelial regeneration. An expanded ACKR1+ endothelial subpopulation in patients with pSS suggested a role in facilitating lymphocyte transendothelial migration. Our analysis of immune cells revealed expanded IGHD+ naive B cells in peripheral blood from patients with pSS. Pseudotime trajectory analysis outlined a bifurcated differentiation pathway for peripheral B cells, enriching three subtypes (VPREB3+ B, BANK1+ B, CD83+ B cells) within SGs in patients with pSS. Fibroblasts emerged as pivotal components in a stromal-immune interaction network, potentially driving extracellular matrix disruption, epithelial regeneration impairment, and inflammation. Our study illuminates immune and stromal cell heterogeneity in patients with pSS, offering insights into therapeutic strategies.

Abstract:Objective: Primary Sjögren's syndrome (pSS) is a systemic autoimmune disorder with an unclear pathogenetic mechanism in the labial gland. This study aims to investigate the cellular and molecular mechanisms contributing to the development of this disease. Methods: Single-cell RNA sequencing (scRNA-seq) was performed on 32,337 cells of labial glands from three pSS patients and three healthy individuals. We analyzed all cell subsets implicated in pSS pathogenesis. Results: Our research revealed diminished differentiation among epithelial cells, concomitant with an enhancement of interferons (IFNs)-mediated signaling pathways. This indicates a cellular functional shift in reaction to inflammatory triggers. Moreover, we observed an augmentation in the population of myofibroblasts and endothelial cells, likely due to the intensified IFNs signaling, suggesting a possible reconfiguration of tissue structure and vascular networks in the impacted regions. Within the immune landscape, there was an apparent increase in immunosuppressive macrophages and dendritic cells (DCs), pointing to an adaptive immune mechanism aimed at modulating inflammation and averting excessive tissue harm. Elevated activation levels of CD4+T cells, along with a rise in regulatory T (Treg) cells, were noted, indicating a nuanced immune interplay designed to manage the inflammatory response. In the CD8+T cell subsests, we detected a notable increase in cells expressing granzyme K (GZMK), signaling an intensified cytotoxic activity. Additionally, the escalated presence of T cells with high levels of heat shock proteins (HSPs) suggests a cellular stress condition, possibly associated with persistent low-grade inflammation, mirroring the chronic aspect of the condition. Conclusions: Our research identified distinct stromal and immune cell populations linked to pSS, revealing new potential targets for its management. The activation of myeloid, B, and T cells could contribute to pSS pathogenesis, providing important guidance for therapeutic approaches.

Abstract:Background Primary Sjögren's syndrome (pSS) is a systemic autoimmune disease affecting predominantly exocrine glands such as salivary and lacrimal glands. Immunoglobulin G4 (IgG4)-related disease (IgG4-RD) is a new clinical concept of multi-organ diseases, which is characterized pathologically by the infiltration of IgG4-bearing plasma cells into involved organs. However, some pSS patients showed infiltration of IgG4+ plasma cells in exocrine glands. Objectives We aim to investigate the characteristics of primary Sjögren's syndrome (pSS) patients with IgG4+ plasma cells in labial salivary glands (LSGs). Methods 336 patients with pSS were enrolled. The LSG tissues from pSS patients were stained with monoclonal antibodies to IgG4 and IgG. Patients were divided into three subgroups according to histopathologic infiltration degree of IgG4+ plasma cells. The clinical and serological characteristics of patients were compared. Results 336 patients were divided into three groups according to infiltration degree of IgG4+ plasma cells: slight-IgG4 group (n=194), moderate-IgG4 group (n=106) and high-IgG4 group (n=36). The results showed that the higher IgG4+ plasma cells expression, the lower positive rates of serum anti-SSA, anti-SSB, anti-nuclear antibodies (ANA) and rheumatoid factor (RF). The comparison of extraglandular manifestations of three groups indicated that patients in high-IgG4 expression group showed a highest frequency of interstitial lung disease (30.6%) and tubulointerstitial nephritis (13.9%), whereas a lowest frequency of leucopaenia (13.9%), thrombocytopaenia (11.1%) and abnormal thyroidal function (0%). Conclusions The results showed that a considerable proportion of pSS patients could manifest abundant IgG4+ plasma cells infiltration in LSGs. These patients have unique laboratory and clinical characteristics. These findings may help further understand pSS, IgG4-RD, and the role of IgG4+ plasma cells in autoimmune diseases. Abstract:Objective To investigate the clinical and serologic features of patients with Sj(?)gren′s syndrome (SS) associated with systemic lupus erythematosus (SLE), and to compare with patients of primary Sjgren′s syndrome (pSS) and systemic lupus erythematosus. Methods Two hundred and eighteen unselected SLE patients were evaluated for the presence of SS. The SLE patients with SS (SLE-SS) were compared with SLE patients without SS on clinical and laboratory parameters. Results SS was identified in 22 SLE patients (10.1%). The SS preceded the development of SLE in 8 (36%), the SS secondary to SLE in 7 (32%), and in 7 (32%) patients, the SS and SLE occurred at the same time. Compared with the SLE patients without SS group, patients with SLE-SS were older, with a higher frequency of Raynaud′s phenomenon, pulmonary involvement, anti-SSA, anti-SSB, and rheumatoid factor, but with a significantly lower frequency of renal involvement. Compared with the pSS patients group, SLE-SS patients were younger, with a higher frequency of Raynaud′s phenomenon, arthritis, mucocutaneous involvement and anti-SSA. Conclusions SLE-SS patients have distinct clinical and laboratory features that differ from SLE or pSS patients.

Abstract:To the Editor : Primary Sjögren's syndrome (pSS) is a chronic autoimmune disease characterized by the infiltration of lymphocytes into dysfunctional exocrine glands. Since the inhibition of lymphotoxin (LT) in the initial stages of pSS can significantly impede lymphocyte infiltration into the glands, early therapeutic intervention becomes more essential. [ 1 ] Therefore, it is critical for physicians to recognize and diagnose pSS at an early stage to ensure prompt and effective treatment. Previous study discovered that excessive follicular helper T (Tfh) cells supported autoantibody production in several autoimmune diseases. [ 2 ] Our previous study showed that activation of Tfh is positively associated with higher titers of autoantibodies and more severe manifestation in systemic lupus erythematosus (SLE) patients. The pathogenesis role of Tfh cells in autoimmune diseases has been verified. [ 3 ] However, the results from pSS studies were inconsistent and controversial. To provide support for further research in pSS, this study focused on describing the profile of the main T helper cells, particularly Tfh cells, in patients with active pSS.

Abstract:Objective To assess the clinical features of newly diagnosed primary Sjogren's syndrome (pSS). Methods Patients were diagnosed according to the international consensus criteria for Sjogren's Syndrome published in 2002. Clinical manifestations and laboratory tests of 86 pSS cases hospitalized in Nanjing Drum Tower Hospital in the past two years were reviewed. Results Among the 86 patients, 95.3% were female and the average disease onset age was 38.6 years. The median time from disease onset to diagnosis was 6 months. Dry mouth, dry eyes and arthralgia were the most common symptoms. Hematologic involvement was found to be prominent in these patients (69.8%). The incidence of abnormal liver function, interstitial lung disease and pulmonary arterial hypertension was 19.8%, 8.2% and 5.8% simultaneously. Younger patients (less than 18 years old) had lower frequency of dry mouth and dry eyes but higher ffrequency of fever and lymph nodes enlargement than the elderly patients (P<0.05). Patients with positive anti-SSA or anti-SSB antibodies had higher incidence of hematological changes as well as ESR than those with negative auto-antibodia. Elevated globulin/IgG and positive antinuclear antibody or rheumatoid factor (P< 0.05). Conclusion pSS is not always a benign disease. Some patients will develop vital organ damages very early and thus need to be identified and treated in time. It should not be overemphasize the importance of dry mouth and dry eyes for the diagnosis of pSS, especially in young patients. Those patients who have fever, high globulin level and positive rheumatoid factor of unknown origin should be screened for pSS.

Abstract:Background/purpose Primary Sjögren's syndrome is a prototypical autoimmune disease, with B cell dysfunction as a dominant feature. Further insights into distribution of B cell subsets in primary Sjögren's syndrome are urgently required to identify the most appropriate target subpopulation. We aimed to evaluate the profiles of B lymphocyte subpopulations in primary Sjögren's syndrome patients and to investigate their clinical significance. Materials and methods Thirty primary Sjögren's syndrome patients and 15 age-and sex-matched healthy controls were recruited. Peripheral B cell subsets were analyzed by flow cytometry. Results Compared to healthy controls, circulating CD19 + B cells, CD19 + CD20 − B cells, CD19 + CD27 − IgD + naïve B cells, CD19 + IgD + CD38 high plasmablasts, CD19 + CD24 high CD38 high transitional B cells and CD19 + CD20 − CD27 + CD38 + plasma cells were elevated in patients with primary Sjögren's syndrome, whereas CD19 + CD27 + memory B cells, CD19 + CD27 − IgD - double negative B cells and CD19 + CD24 hi CD27 + Bregs were decreased. Furthermore, the percentage of circulating CD19 + CD20 − CD27 + CD38 + plasma cells was positively correlated with serum IgG levels and the proportional area of lymphocytic infiltration of labial gland. Conclusion We identified a comprehensive B lymphocyte subset distribution profile in primary Sjögren's syndrome. Moreover, we detected a clinical significance of CD19 + CD20 − CD27 + CD38 + plasma cells, suggesting that these cells might play a key role in disease pathology and represent potential therapeutic targets.

Abstract:OBJECTIVE To comprehend clinical features at onset of primary Sjogren's syndrome (pSS) in order to provide useful data for its clinical management. METHODS In the study, 224 patients diagnosed with pSS in the Department of Rheumatology and Immunology of Peking University People's Hospital from Jun. 1st, 2007 to Aug. 1st, 2008 were investigated, including gender, age of onset, time and site of first hospitalization and definite diagnosis, etc. RESULTS In this 224 pSS cohort (213 females and 11 males), the male/female ratio was 1:19.4, the mean age of onset was (53.5±11.7) years, and median duration was 9.4 years (ranging from 0.2 to 40.0 years).The manifestations showed that up to 33% of the patients (74/224) had leukopenia, 25% (56/224) polyarthralgia, 16.5% (37/224) raynaud phenomenon, 15.6% (35/224) hepatic injury, 12.1% (27/224) pulmonary interstitial fibrosis, 11.6% (26/224) purpuras on lower extremities, 8.0% (18/224) hemogram abnormal, 5.8% (13/224) thrombopenia, and 3.6% (8/224) renal tubule acidosis. When the risk factor of the systemic involvements, was analyzed, two factors were significantly associated with pulmonary interstitial fibrosis: age (OR=1.074, 95% CI=1.031-1.118), and duration (OR=1.075, 95% CI=1.023-1.128). Liver involvement was associated with duration (OR=1.050, 95% CI=1.002-1.100). In addition, 8.0% of the pSS patients(18/224)showed family history of autoimmune diseases and 11.2%(25/224)had family history of tumor. CONCLUSION In this cohort of the pSS patients, female is predominant and the incidence of extro-glandular manifestations, such as leukopenia, lung and liver involvements is high, and pSS has inheritance intention.

Abstract:Primary Sjögren's syndrome (pSS) is an autoimmune condition that causes harm to exocrine glands and also has extra-glandular manifestations (EGM). pSS patients with EGM have a worse prognosis than those with only sicca symptoms. Previous studies have shown that the minor salivary glands (MSG) of pSS patients exhibit a unique profile of cytokines and chemokines compared to healthy controls. However, there is a lack of research comparing pSS with EGM (pSS-EGM) and pSS without EGM (pSS-non-EGM). This study aims to explore potential biomarkers associated with pSS, particularly pSS with EGM.

Abstract:Objective To investigate clinical characteristic of primary Sjogren's syndrome(pSS) with initial manifestations in the extra-gland organs. Methods Clinical data, including symptoms, laboratory parameters and complications of 85 pSS patients with initial manifestations of extra-gland organs hospitalized at the People's Hospital of Peking University during 2003 to 2006 were collected and analyzed retrospectively, as compared to those of pSS with xerophthalmia or xerostomia as initial ones. Results Mean time interval from onset to diagnosis in these patients with initial manifestations involved in the extra-gland organs was 94.5 months, higher than that in those with xerophthalmia or xerostomia as initial ones. Arthritis was manifested initially in 38 of the 85 patients (44.7 percent), peripheral leukopenia in 16 (18.8 percent), thrombocytopenia in nine (10.6 percent) and abnormal liver function in 13 (15.3 percent), respectively. Percentage of serum positive autoantibedies was higher in the patients with initial manifestation of the extra-gland organs than that in those with xerophthalmia or xerostomia as initial ones. Conclusions Initial manifestation of primary Sjogren's syndrome varies, and some patients can be initially involved in their extra-gland organs, with more proportion of arthritis than of other symptoms, and xerophthalmia or xerestomia could appear in them during the course of disease. And, xerophthalmia or xerostomia would not appear even as pSS was diagnosed in some other pSS patients. More attention should be paid to these clinical characteristics mentioned above in practice to avoid misdiagnosis.
