Abstract:Systemic sclerosis (SSc) is an autoimmune disease characterized by abnormalities in microcirculation, extracellular matrix accumulation, and immune activation. Autoantibodies are markers of immune abnormalities and provide diagnostic and predictive value in SSc. Anti-topoisomerase antibodies (ATAs), anticentromere antibodies (ACAs), and anti-RNA polymerase antibodies (ARAs) are the three classical specific antibodies with the highest availability and stability. In this review, we provide an overview of the recent progress in SSc research with respect to ATAs, ACAs, and ARAs, focusing on their application in distinguishing clinical phenotypes, such as malignancy and organ involvement, identifying genetic background in human leukocyte antigen (HLA) or non-HLA alleles, and their potential roles in disease pathogenesis based on the effects of antigen-antibody binding. We finally summarized the novel analysis using ATAs, ACAs, and ARAs on more detailed disease clusters. Considering these advantages, this review emphasizes that classical SSc-specific autoantibodies are still practical and have the potential for patient and risk stratification with applications in precise medicine for SSc.

Abstract:The pathology of adult patients with USD is complicated, and no specific clinical feature was found for distinguishing between benign and malignant lesions. The tumor took a quite proportion in adult patients with USD. Therefore, careful consideration should be taken before diagnosing patients with USD in order to reduce misdiagnosis rate.

Abstract:Although the original purpose of the systemic lupus erythematosus (SLE) classification criteria was to distinguish SLE from other mimic diseases, and to facilitate sample selection in scientific research, they have become widely used as diagnostic criteria in clinical situations. It is not known yet if regarding classification criteria as diagnostic criteria, what problems might be encountered? This is the first study comparing the three sets of classification criteria for SLE, the 1997 American College of Rheumatology (ACR'97), 2012 Systemic Lupus International Collaborating Clinics (SLICC'12) and 2019 European League Against Rheumatism/American College of Rheumatology (EULAR/ACR'19), for their ability to distinguish patients with SLE from patients with pure mucocutaneous manifestations (isolated cutaneous lupus erythematosus without internal disease, i-CLE) in the lupus disease spectrum. 1,865 patients with SLE and 232 patients with i-CLE were recruited from a multicenter study. We found that, due to low specificity, none of the three criteria are adept at distinguishing patients with SLE from patients with i-CLE. SLICC'12 performed best among the original three criteria, but if a positive ANA was removed as an entry criterion, EULAR/ACR'19 would performed better. A review of previous studies that compared the three sets of criteria was presented in this work.

Abstract:This article first addresses the issue of defining ancient masks and briefly explores masking cultures in the Iron Age Mediterranean, then emphasizes the role that Carthage appears to have played in both maintaining and renewing the Levantine disguised traditions. Among the newly invented masks of Carthage, it focuses on and reviews a small group of masks and amulets portraying black African twisted faces and dating to between the seventh and mid-sixth century BC. These masks support the existence of wooden prototypes and currently provide the earliest evidence of black African imagery used for ritual purposes. Based on their features and compelling parallels, it is argued that their iconography was inspired by facial paralysis and they should be interpreted as images of a demon used in curing rituals.

Abstract:Background: Rhesus and ABO blood group systems exhibit significant differences due to genetics, geography, ethnic diversity. The purpose of this study was to determine the prevalence of the ABO and Rh blood groups among Purana (old) population in Sigiriya, Sri Lanka, and to predict the risk of diseases with reference to their blood group.Methods: Total of 384 adult Purana individuals were randomly chosen for a cross-sectional descriptive study in Purana villages of Talkote, Diyakepilla, Nagalaweva, and Pidurangala. Employing slide method and commercially available antisera, ABO and Rh phenotypes were determined. Using Hardy-Weinberg equilibrium, the frequencies of alleles A, B, O, D, and d were estimated. SPSS software version 23 was used for analysis. The disease risk in this community was predicted according to the prevailing literature.Results: Blood type "O" (46%) was the predominant phenotype. The "AB" blood group exhibited the lowest phenotypic frequency (4%). There were 39% and 11% of "B" and "A" blood types, respectively. 99% of subjects exhibited Rh phenotype, while 1% were Rh negative. AA, AO, BB, BO, and AB genotype distributions were 7.3%, 28%, 0.2%, 6.4%, and 3.5%, respectively. Majority (48%) belonged to the OO genotype. Distribution of DD, Dd, and dd genotypes was 76%, 22%, and 2%, respectively. According to the results, risk of diseases for the majority of Purana population such as coronary artery disease, infectious diseases such as cholera, mumps and cancers could be predicted by previous evidence.Conclusion: This study shows that pattern of phenotype and genotype distribution of Purana population is different from the distribution pattern among modern Sri Lankans. Majority in this population has risk of developing diseases such as coronary artery disease, peptic ulcers, infectious diseases such as cholera, mumps and tuberculosis infection.

As living standards advance, there is an escalating emphasis on the breathability of masks. Furthermore, this paper delves into the prospective significance and potential challenges associated with the development of comfortable, biocompatible, self-cleaning masks in the future. Different technologies, including mask detection systems, and advanced materials have been proposed in the framework of a multidisciplinary context. Abstract:SUMMARYThis narrative review and meta-analysis summarizes a broad evidence base on the benefits-and also the practicalities, disbenefits, harms and personal, sociocultural and environmental impacts-of masks and masking. Our synthesis of evidence from over 100 published reviews and selected primary studies, including re-analyzing contested meta-analyses of key clinical trials, produced seven key findings. First, there is strong and consistent evidence for airborne transmission of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) and other respiratory pathogens. Second, masks are, if correctly and consistently worn, effective in reducing transmission of respiratory diseases and show a dose-response effect. Third, respirators are significantly more effective than medical or cloth masks. Fourth, mask mandates are, overall, effective in reducing community transmission of respiratory pathogens. Fifth, masks are important sociocultural symbols; non-adherence to masking is sometimes linked to political and ideological beliefs and to widely circulated mis- or disinformation. Sixth, while there is much evidence that masks are not generally harmful to the general population, masking may be relatively contraindicated in individuals with certain medical conditions, who may require exemption. Furthermore, certain groups (notably D/deaf people) are disadvantaged when others are masked. Finally, there are risks to the environment from single-use masks and respirators. We propose an agenda for future research, including improved characterization of the situations in which masking should be recommended or mandated; attention to comfort and acceptability; generalized and disability-focused communication support in settings where masks are worn; and development and testing of novel materials and designs for improved filtration, breathability, and environmental impact.

Abstract:The pervasive use of electronic health databases has increased the accessibility of free-text clinical reports for supplementary use. Several text classification approaches, such as supervised machine learning (SML) or rule-based approaches, have been utilized to obtain beneficial information from free-text clinical reports. In recent years, many researchers have worked in the clinical text classification field and published their results in academic journals. However, to the best of our knowledge, no comprehensive systematic literature review (SLR) has recapitulated the existing primary studies on clinical text classification in the last five years. Thus, the current study aims to present SLR of academic articles on clinical text classification published from January 2013 to January 2018. Accordingly, we intend to maximize the procedural decision analysis in six aspects, namely, types of clinical reports, data sets and their characteristics, pre-processing and sampling techniques, feature engineering, machine learning algorithms, and performance metrics. To achieve our objective, 72 primary studies from 8 bibliographic databases were systematically selected and rigorously reviewed from the perspective of the six aspects. This review identified nine types of clinical reports, four types of data sets (i.e., homogeneous–homogenous, homogenous–heterogeneous, heterogeneous–homogenous, and heterogeneous–heterogeneous), two sampling techniques (i.e., over-sampling and under-sampling), and nine pre-processing techniques. Moreover, this review determined bag of words, bag of phrases, and bag of concepts features when represented by either term frequency or term frequency with inverse document frequency, thereby showing improved classification results. SML-based or rule-based approaches were generally employed to classify the clinical reports. To measure the performance of these classification approaches, we used precision, recall, F-measure, accuracy, AUC, and specificity in binary class problems. In multi-class problems, we primarily used micro or macro-averaging precision, recall, or F-measure. Lastly, open research issues and challenges are presented for future scholars who are interested in clinical text classification. This SLR will definitely be a beneficial resource for researchers engaged in clinical text classification.

Abstract:A mask serves as a simple external barrier that protects humans from infectious particles from poor air conditions in the surrounding environment. As an important personal protective equipment (PPE) to protect our respiratory system, masks are able not only to filter pathogens and dust particles but also to sense, reflect or even respond to environmental conditions. This smartness is of particular interest among academia and industries due to its potential in disease detection, health monitoring and caring aspects. In this review, we provide an overlook of the current air filtration strategies used in masks, from structural designs to integrated functional modules that empower the mask's ability to sense and transfer physiological or environmental information to become smart. Specifically, we discussed recent developments in masks designed to detect macroscopic physiological signals from the wearer and mask-based disease diagnoses, such as COVID-19. Further, we propose the concept of next-generation smart masks and the requirements from material selection and function design perspectives that enable masks to interact and play crucial roles in health-caring wearables.

Filarial diseases in Ceylon: a geographic and historical analysis

Abstract:Medical records concerning filarial diseases in Ceylon date from the account of Davy[1], though there are hints as to the more obvious manifestations in the old chronicles of the country, too. A first survey was conducted in 1912/1913 concentrating on urban areas, followed by a second survey in the 1930s with emphasis on the rural parts. The results displayed a remarkable distribution pattern: Wuchereria bancrofti, the so-called "urban type", concentrated in Galle and Matara towns, whereas Brugia malayi, the "rural type", widespread along the southwest coast from Matara to Negombo, plus isolated pockets in the northwest, central north, east and south. The survey of the 1930s lead to the supposition that the occurrence of B. malayi must have something to do with the distribution of certain water plants, a suspicion later on confirmed in that Pistia stratiotes in particular--but other water plants as well--are essential for the survival of the vector (Taeniorhynchus (Mansonia) uniformis) during its early (submersed) stages of development. A determined effort to remove the water plants from tanks etc. reduced the rural type with encouraging results. At the same time, a combination of factors, in particular the war-time sojourn of masses of troops from Africa, already infected by filarial diseases, in the southwestern coastal areas triggered off an unexpected spread of the urban type out of its early "bridge-heads" in Galle and Matara towns to invade the southwest coastal areas, and, later on, supported by increased population mobility, to advance further inland too. At present, there is no remedy within sight to give some hope to come to grips with this problem as the vector, Culex pipiens fatigans, is ubiquitous and finds suitable breeding grounds practically everywhere. Research into the history of filarial diseases in Ceylon points as far as B. malayi is concerned, to an invasion by a Malayan army under the Kalinga kings during the days of close relations between Ceylon and southeast Asia, i.e. during the 12th and 13th centuries, and as far as W. bancrofti is concerned, a Chinese army, invading the southern coast in the early 15th century, is made responsible. Filarial diseases in Ceylon present a particular interesting case of geomedical research; but inspite of encouraging results in fighting the rural type, i.e. B. malayi, the urban type, W. bancrofti, seems to remain a problem of public health in the island for the forseeable future.

Abstract:Introduction Sri Lanka has a predominantly rural population. However, there is a dearth of research on health and socioeconomic issues in this group. ObjectiveTo describe basic socioeconomic characteristics and health profile in a rural population.Methods Adescriptive cross-sectional household survey was conducted in 1950 households in three rural districts, selected by a three-stage stratified cluster sampling method.Results The population pyramid showed an ageing population (dependency ratio of 50%). Only 39% had completed GCE (ordinary level). Unemployment rates were high (25% males, 76% females). Agriculture and related work were main occupations. Most lacked amenities (e.g. 61% households lacked a refrigerator) and practiced inappropriate methods of waste disposal (e.g. open burning by 72%). Household illnesses were frequent: episodes of acute illness within two weeks, injuries within past year and chronic illness were reported from 35.9%, 14.9% and 48.3% households. The prevalence of chronic diseases in adults >20 years were high: diabetes 13.5%, hypertension 16.7% and overweight/obesity 28.2%. Of the males, 22.1% smoked and 12.3% took alcohol. Almost 25% adults chewed betel. Reports of snake bite, dog bites and suicide/attempted suicide were seen in 15.5%, 9.7% and 3.0% households respectively. ConclusionsThis study shows a unique clustering of health-related problems in rural Sri Lanka. This was characterized by demographic transition, burden from snake bites, chronic diseases and acute illnesses. There were resource limitations and low levels of education. Cohort studies and comparisons with urban areas will enable further elucidation of determinants of health and other issues in rural Sri Lanka. Published on 2019-09-30 00:00:00

Abstract:Surgical face masks (SFM) are pivotal in preventing surgical site infections (SSI) in the operating room (OR). However, there are currently no specific recommendations for their most effective use. SFM effectiveness is influenced by factors such as material, fit, and duration of use, sparking ongoing debates about their benefits and risks in surgery. SFMs act as a protective barrier, but their ability to filter out harmful compounds is questioned. They can also impact communication and create a false sense of security. Nevertheless, SFMs aid in infection prevention and provide psychological comfort. Clear guidelines are needed to ensure their appropriate use in the OR. This paper offers a historical overview of surgical masks, emphasizing their role in infection prevention. It explores SFM effectiveness for both the surgical team and patients during surgery and considers their future in surgical settings. As we navigate the evolving landscape of SFMs, clear and concise guidelines are imperative to ensure their judicious and effective use in the OR. This paper serves as an essential resource for understanding the historical significance, contemporary efficacy, and prospective trajectory of SFMs in surgical practice.

Abstract:This essay synthesizes key analytical perspectives from the vast social science literature on disease classification spanning various disciplines. The aim is to develop a framework for analyzing changes in Western medicine framed under umbrella terms such as 'precision medicine' and 'personalized medicine.' I argue that understanding these contemporary changes in the conception and enactment of disease requires attention to historical developments in disease classifications. Spinal muscular atrophy is used as an example case to illustrate this argument. To guide further inquiry into the current move in biomedicine towards precision medicine, I outline two analytical dimensions—material and social interactionist aspects of disease classification—and show how these can be fruitfully combined. Possible starting points and questions for analysis are presented and discussed.

Abstract:The Middle Eastern prevalence of sickle cell anemia, a genetic disorder that affects red blood cells, necessitates additional research. On a molecular level, we sought to identify and sort the oral microbiota of healthy individuals and those with sickle cell anemia. Furthermore, it is crucial to comprehend how changes in the genetic makeup of the oral microbiota impact the state of sickle cell anemia. Using next-generation sequencing, the 16S rRNA amplicon was examined using saliva samples from 36 individuals with sickle cell anemia and healthy individuals. These samples were obtained from sickle cell anemia patients (18 samples) and healthy control participants (controls, 18 samples). Various analyses are conducted using bioinformatic techniques to identify distinct species and their relative abundance. Streptococcus, followed by Fusobacterium nucleatum, Prevotella, and Veillonella were the most prevalent genera of bacteria in the saliva of the SCA and non-SCA individuals according to our findings. Rothia mucilaginosa, Prevotella scoposa, and Veillonella dispar species were the dominant species in both sickle cell anemia and non-sickle cell anemia subjects. Streptococcus salivarius, Actinomyces graevenitzii, Actinomyces odontolyticus, and Actinomyces georgiae spp. were the most prevalent bacterial spp. in the studied SCA cases. The sequencing of the 16S rRNA gene yielded relative abundance values that were visualized through a heatmap analysis. Alterations in the oral microflora's constitution can significantly affect the susceptibility of sickle cell anemia patients to develop more severe health complications. Salivary diagnosis is a potential tool for predicting and preventing oral microbiome-related diseases in the future.

Abstract:Sickle cell disease (SCD) and β-thalassemia are the most common monogenic diseases, disproportionately affecting low- and middle-income countries, where low-cost and accurate diagnostic tools are needed to reduce the global disease burden. Although the sickling test is commonly used to screen for the sickle mutation, it cannot distinguish between the asymptomatic sickle cell trait (SCT) and SCD, or identify β-thalassemia. Here, we enhanced the inexpensive sickling test using automated microscopy and morphology-based machine learning classification to detect SCD, trait conditions (SCT and β-thalassemia trait) and normal individuals with an overall area under receiver operating curve, sensitivity and specificity of 0.940 (95% confidence intervals: 0.938-0.942), 84.6% (84.2%-84.9%), and 92.3% (92.1%-92.4%), respectively. Notably, the sensitivity and specificity to detect severe disease (SCD) was over 97% and 98%, respectively, thus establishing a low-cost automated screening option for disease detection in low-resource settings. Furthermore, leveraging high-throughput microscopy, we generated an open-access dataset comprising over 300,000 images with 1.5 trillion segmented cells from 138 individuals in Canada and Nepal including individuals with sickle and/or β-thalassemia mutations, to accelerate further research.

Abstract:The efficacy of traditional Chinese medicine (TCM) treatments for Western medicine (WM) diseases relies heavily on the proper classification of patients into TCM syndrome types. The authors developed a data-driven method for solving the classification problem, where syndrome types were identified and quantified based on statistical patterns detected in unlabeled symptom survey data. The new method is a generalization of latent class analysis (LCA), which has been widely applied in WM research to solve a similar problem, i.e., to identify subtypes of a patient population in the absence of a gold standard. A well-known weakness of LCA is that it makes an unrealistically strong independence assumption. The authors relaxed the assumption by first detecting symptom co-occurrence patterns from survey data and used those statistical patterns instead of the symptoms as features for LCA. This new method consists of six steps: data collection, symptom co-occurrence pattern discovery, statistical pattern interpretation, syndrome identification, syndrome type identification and syndrome type classification. A software package called Lantern has been developed to support the application of the method. The method was illustrated using a data set on vascular mild cognitive impairment.

New Classification of Maxillary Sinus Contours and Its Relation to Sinus Floor Elevation Surgery

Abstract:BackgroundIt is complicated to select an appropriate sinus floor elevation and the procedure for sinus floor elevation lacks of consensus. Sinus contour plays an important role in choosing a surgery approach. But there are still no published articles revealing the influence of sinus contours to sinus floor elevation surgery. PurposeWe propose a new classification depending on sinus contours from cone-beam computed tomography (CBCT), analyze clinical characters of different types, and investigate the relationship between sinus contours and sinus floor elevation. Materials and MethodsWe divide sinus into five categories: narrow tapered, tapering, ovoid, square, and irregular. For the first four types, subtypes are classified into three categories: without recess, with buccal-sinus-recess (BSR), and with palate-nasal-recess (PNR). For irregular type, subtypes are classified into three categories: tooth protruding into sinus floor, irregular floor, and septa/exostosis on sinus floor. Then the distribution features of sinuses of 698 patients are described. Sinus widths are measured at second premolar, first and second molar on both sides, and are compared among different types and subtypes. ResultsNarrow tapered sinus occupies 88% at second premolar sites, while tapered sinus occupies almost 50% at first and second molar sites. At second premolar and first molar sites, 62% are without recess types. While 92% are without recess types at second molar. Sinuses with BSR present in only three of 3765 sites. There is an increasing trend of sinus width from narrow tapered to irregular type. Sinus width of the group with recesses is significantly higher than the one without recess. At the end, we provide corresponding treatment recommendations for each sinus types and subtypes. ConclusionThis is the first classification system that gives treatment recommendations for sinus floor elevation surgery based on sinus contours. The classification system is consistent, easy to visualize, and practicable.

Abstract:The classification of seizures and epilepsies by the International League Against Epilepsy (ILAE), 2017 is the most recent classification model which aimed to simplify terminologies that patients and their caregivers can easily understand, identify seizures that have both focal and generalized onset and incorporate missing seizures. We have exhaustively reviewed the studies, discussed its scope, outlined its limitations and gave recommendations that could help in forming subsequent reviews. We have also described the terminologies that have been replaced, redefined or removed to have a clear view of the previous and the current classification models. We have recommended the use of multidimensional classification model which incorporated the clinical semiology, disease location, etiology and associated comorbidities. The benefits of this model is for prompt diagnosis which will results into early management and then better patient outcomes. It would also have a profound effects on the kind of treatment patients might receive especially in developing countries where there are scarcity of the diagnostic techniques. Overall, in this study we have reviewed the current study on seizures and epilepsy classification model by ILAE, 2017 to clarify the descriptions and coverage, outlined some limitations and suggested recommendations.
A classification system for identifying persons with an unknown cardiovascular disease (CVD) status for a multiracial/ ethnic Caribbean population

Abstract:Background: The need for classification systems for cardiovascular disease (CVD) that is population-specific is important towards understanding the clinical disease and diagnostics associated with the disease. This paper presents the form and validation results of this classification system. Method: The survey data used was captured from 778 participants, 526 persons with no prior CVD, and 252 who reported prior CVD. Binomial logistic regression and Discriminant analysis were utilised to develop classification models. This classification system provided a general measure of severity of disease by utilising scores estimated from two algorithms developed from 13 routine physiologic measurements, along with demographic information of age and ethnicity, inter alia, and previous health status. Results: For each model, specific score ranges were identified, which gave the best classification for those with a prior CVD incident (higher scores) and for others labelled as non-CVD (lower scores). The two classification models (Logistic Regression Model and Discriminant Analysis Model) developed had high area under the receiver-operating characteristic (AUROC) values (98% & 99%) and sensitivity (86 and 90%), which improved discrimination between Non-CVD and CVD participants and, more importantly, correctly classified a greater proportion of CVD participants. New to this type of research was the estimation and detailed evaluation of a range of scores, labelled non-differentiating, which fell in the middle of the spectrum and which contained the higher-end scores for the non-CVD individuals and the lower-end scores for CVD patients, all of whom were incorrectly classified, based on their prior history. Conclusion: The classification system of scores is able to differentiate the CVD status of individuals, with good predictability, and could assist physicians with recommending different treatment plans. The two models in this classification system each individually outperformed the three established models in terms of the strength of their correct classifications of individuals with or without prior reported CVD incidents. More importantly, they have smaller non-differentiating ranges than the three known models and, in that range, the two new models have lower CVD/non-CVD ratios suggesting they are more likely to misclassify non-CVD individuals compared to CVD patients, which is a more benign misclassification. Further, when used in combination, the two models increased the sensitivity, in classifying individuals of different ethnicities, beyond that of either one used independently or of any of the three standard European/North American models. These efforts will be instrumental in advancing personalised CVD management strategies and improving health outcomes across diverse populations.

Abstract:Among the medical bamboo slips unearthed from Han tomb in Laoguanshan of Chengdu, the Zhu bing (All Diseases) is a monograph to discuss the characteristics of signs and symptoms. Based on the differences of writing styles, diseases involved, expounding methods and writing rules, the book is divided, by the research team, into 2 parts: All Diseases(1) and All Diseases(2). All Diseases(1) includes over 130 slips, 2 000 characters with totally more than 100 disease names, containing multiple clinical disciplines. The elaborated classification, varied naming methods, grasping the symptom characteristics guided by the four diagnostic approaches, paying attention to the comparison of similar diseases, and dealing with the prognosis and healthcare in this part reflect the holism of correspondence between human body and natural environment, and syndrome differentiation thought of combining disease with symptoms and signs, revealing its academic significance.
