Abstract:A Convolutional Neural Network was used to predict kidney function in patients with chronic kidney disease from high-resolution digital pathology scans of their kidney biopsies. Kidney biopsies were taken from participants of the NEPTUNE study, a longitudinal cohort study whose goal is to set up infrastructure for observing the evolution of 3 forms of idiopathic nephrotic syndrome, including developing predictors for progression of kidney disease. The knowledge of future kidney function is desirable as it can identify high-risk patients and influence treatment decisions, reducing the likelihood of irreversible kidney decline.

Texture Branch Network for Chronic Kidney Disease Screening Based on Ultrasound Images

Abstract:Chronic kidney disease (CKD) is a widespread renal disease throughout the world.Once it develops to the advanced stage,serious complications and high risk of death will follow.Hence,early screening is crucial for the treatment of CKD.Since ultrasonography has no side effects and enables radiologists to dynamically observe the morphology and pathological features of the kidney,it is commonly used for kidney examination.In this study,we propose a novel convolutional neural network (CNN) framework named the texture branch network to screen CKD based on ultrasound images.This introduces a texture branch into a typical CNN to extract and optimize texture features.The model can automatically generate texture features and deep features from input images,and use the fused information as the basis of classification.Furthermore,we train the base part of the network by means of transfer learning,and conduct experiments on a dataset with 226 ultrasound images.Experimental results demonstrate the effectiveness of the proposed approach,achieving an accuracy of 96.01％ and a sensitivity of 99.44％.

Abstract:PurposeIn robotic-assisted kidney surgery, computational methods make it possible to augment the surgical scene and potentially improve patient outcome. Most often, soft-tissue registration is a prerequisite for the visualization of tumors and vascular structures hidden beneath the surface. State-of-the-art volume-to-surface registration methods, however, are computationally demanding and require a sufficiently large target surface. To overcome this limitation, the first step toward registration is the extraction of the outer edge of the kidney.MethodsTo tackle this task, we propose a deep learning-based solution. Rather than working only on the raw laparoscopic images, the network is given depth information and distance fields to predict whether a pixel of the image belongs to an edge. We evaluate our method on expert-labeled in vivo data from the EndoVis sub-challenge 2017 Kidney Boundary Detection and define the current state of the art.ResultsBy using a leave-one-out cross-validation, we report results for the most suitable network with a median precision-like, recall-like, and intersection over union (IOU) of 39.5 px, 143.3 px, and 0.3, respectively.ConclusionWe conclude that our approach succeeds in predicting the edges of the kidney, except in instances where high occlusion occurs, which explains the average decrease in the IOU score. Abstract:Kidney ultrasound (US) images are primarily employed for diagnosing different renal diseases. Among them, one is renal localization and detection, which can be carried out by segmenting the kidney US images. However, kidney segmentation from US images is challenging due to low contrast, speckle noise, fluid, variations in kidney shape, and modality artifacts. Moreover, well-annotated US datasets for renal segmentation and detection are scarce. This study aims to build a novel, well-annotated dataset containing 44,880 US images. In addition, we propose a novel training scheme that utilizes the encoder and decoder parts of a state-of-the-art segmentation algorithm. In the pre-processing step, pixel intensity normalization improves contrast and facilitates model convergence. The modified encoder–decoder architecture improves pyramid-shaped hole pooling, cascaded multiple-hole convolutions, and batch normalization. The pre-processing step gradually reconstructs spatial information, including the capture of complete object boundaries, and the post-processing module with a concave curvature reduces the false positive rate of the results. We present benchmark findings to validate the quality of the proposed training scheme and dataset. We applied six evaluation metrics and several baseline segmentation approaches to our novel kidney US dataset. Among the evaluated models, DeepLabv3+ performed well and achieved the highest dice, Hausdorff distance 95, accuracy, specificity, average symmetric surface distance, and recall scores of 89.76%, 9.91, 98.14%, 98.83%, 3.03, and 90.68%, respectively. The proposed training strategy aids state-of-the-art segmentation models, resulting in better-segmented predictions. Furthermore, the large, well-annotated kidney US public dataset will serve as a valuable baseline source for future medical image analysis research.

Abstract:The segmentation of kidney layer structures, including cortex, outer stripe, inner stripe, and inner medulla within human kidney whole slide images (WSI) plays an essential role in automated image analysis in renal pathology. However, the current manual segmentation process proves labor-intensive and infeasible for handling the extensive digital pathology images encountered at a large scale. In response, the realm of digital renal pathology has seen the emergence of deep learning-based methodologies. However, very few, if any, deep learning based approaches have been applied to kidney layer structure segmentation. Addressing this gap, this paper assesses the feasibility of performing deep learning based approaches on kidney layer structure segmetnation. This study employs the representative convolutional neural network (CNN) and Transformer segmentation approaches, including Swin-Unet, Medical-Transformer, TransUNet, U-Net, PSPNet, and DeepLabv3+. We quantitatively evaluated six prevalent deep learning models on renal cortex layer segmentation using mice kidney WSIs. The empirical results stemming from our approach exhibit compelling advancements, as evidenced by a decent Mean Intersection over Union (mIoU) index. The results demonstrate that Transformer models generally outperform CNN-based models. By enabling a quantitative evaluation of renal cortical structures, deep learning approaches are promising to empower these medical professionals to make more informed kidney layer segmentation.

Abstract:Kidney stones are currently considered a very common disease and recent studies have shown a tendency for the incidence of this disease to increase in recent years. The disease is recognized as a serious threat to the population's health because it is associated with other serious illnesses that can greatly compromise people's quality of life. The development of technologies and strategies aimed at aiding the diagnosis and treatment of this disease has the potential to improve the quality and effectiveness of services provided by health professionals. Diagnosis based on medical images has been one of the main tools for diagnosing kidney stones and Deep Learning techniques have been widely proposed to perform this task. This study proposes a Deep Learning model for detecting kidney stones in computed tomography images. The model was trained with a dataset composed of images obtained from individuals who underwent examinations to analyze diseases in the urinary system. The model achieved an accuracy rate of 96.20% in its predictions and proved to be a suitable tool for treating the problem in question. The results obtained in this study demonstrate the potential of Deep Learning techniques as tools to help improve healthcare procedures related to imaging diagnosis.

Abstract:Kidney cancer is a public health problem that affects thousands of people worldwide. Accurate kidney tumor segmentation is an important task that helps doctors to reduce the time to interpret images, diagnose, and plan the treatment of the diseases. However, because of the heterogeneity of tumors, manually segmenting is a time-consuming process and subject to variability among specialists. In the last years, convolutional neural networks (CNNs) have gained popularity for automatic medical image segmentation. Three-dimensional (3D) networks have a high segmentation capacity, but they are complex and have high computational costs. Thus, two-dimensional networks are the most used owing to the relatively low memory consumption, but they do not exploit 3D features. Therefore, in the present study, a 2.5D network that balances memory consumption and model complexity is proposed to assist doctors specialized in kidney tumor diagnosis in computed tomography (CT). There are three major steps in the proposed method: (1) preprocessing of the KiTS19 dataset, (2) initial segmentation using the DeepLabv3+ 2.5D model with DPN-131 encoder, and (3) reduction of false positives using image processing. The performance of the proposed method was assessed in 210 CTs from the available KiTS19 dataset. In the best scenario, the proposed method presented accuracy of 99.71%, sensitivity of 84.24%, specificity of 99.82%, Dice coefficient of 85.17%, Jaccard index of 75.62%, Hausdorff distance of 18.39 mm, and average surface distance of 3.36 mm. Thus, the 2.5D neural network approach is capable of efficiently solving the problem of segmentation of kidney tumors in CT images, presenting results comparable to those of high-performance 3D neural networks.

Abstract:Abstract Motivation Pediatric kidney disease is a widespread, progressive condition that severely impacts growth and development of children. Chronic Kidney Disease (CKD) is often more insidious in children than in adults, usually requiring a renal biopsy for diagnosis. Biopsy evaluation requires copious examination by trained pathologists, which can be tedious and prone to human error. In this study, we propose an Artificial Intelligence (AI) method to assist pathologists in accurate segmentation and classification of pediatric kidney structures, named as AI-based Pediatric Kidney Diagnosis (APKD). Results We collected 2,935 pediatric patients diagnosed with kidney disease for the development of APKD. The dataset comprised 93,932 histological structures annotated manually by three skilled nephropathologists. APKD scored an average accuracy of 94% for each kidney structure category, including 99% in the glomerulus. We found strong correlation between the model and manual detection in detected glomeruli (Spearman correlation coefficient r = 0.98, p-value < 0.001; intraclass correlation coefficient ICC = 0.98, 95% CI = 0.96-0.98). Compared to manual detection, APKD was approximately 5.5 times faster in segmenting glomeruli. Finally, we show how the pathological features extracted by APKD can identify focal abnormalities of the glomerular capillary wall to aid in the early diagnosis of pediatric kidney disease. Abstract:It remains challenging to automatically segment kidneys in clinical ultrasound (US) images due to the kidneys' varied shapes and image intensity distributions, although semi-automatic methods have achieved promising performance. In this study, we propose subsequent boundary distance regression and pixel classification networks to segment the kidneys, informed by the fact that the kidney boundaries have relatively homogenous texture patterns across images. Particularly, we first use deep neural networks pre-trained for classification of natural images to extract high-level image features from US images, then these features are used as input to learn kidney boundary distance maps using a boundary distance regression network, and finally the predicted boundary distance maps are classified as kidney pixels or non-kidney pixels using a pixel classification network in an end-to-end learning fashion. We also adopted a data-augmentation method based on kidney shape registration to generate enriched training data from a small number of US images with manually segmented kidney labels. Experimental results have demonstrated that our method could effectively improve the performance of automatic kidney segmentation, significantly better than deep learning-based pixel classification networks.

Abstract:Kidney disease affects millions worldwide which emphasizes the need for early detection. Recent advancements in deep learning have transformed medical diagnostics and provide promising solutions to detect various kidney diseases. This paper aims to develop a reliable AI based learning system for effective prediction and classification of kidney diseases. The research involves a dataset of 12,446 kidney images which include cysts, tumor, stones, and healthy samples. The data undergoes thorough preprocessing to eliminate noise and enhance the quality of image. Segmentation techniques like Otsu's binarization, Distance transform, and watershed transformation are applied to accurately delineate and identify distinct regions of interest followed by contour feature extraction which includes parameters like area, intensity, width, height, etc. Subsequently, different deep learning models such as DenseNet201, EfficientNetB0, InceptionResNetV2, MobileNetv2, ResNet50V2, and Xception are trained on incorporating with three optimizers—RMSprop, SGD, as well as Adam and are examined for the metrics such as accuracy, loss, precision, recall, RMSE, and F1 score. Notably, the Xception model outperformed others by achieving an accuracy of 99.89% with RMSprop. Similarly, ResNet50V2 and DenseNet201 demonstrated impressive accuracy of 99.68% with SGD and Adam optimizers respectively. These findings highlight the effectiveness of AI and deep transfer learning in accurate and effective kidney disease detection as well as classification.

Abstract:Kidney diseases result from various causes, which can generally be divided into neoplastic and non-neoplastic diseases. Deep learning based on medical imaging is an established methodology for further data mining and an evolving field of expertise, which provides the possibility for precise management of kidney diseases. Recently, imaging-based deep learning has been widely applied to many clinical scenarios of kidney diseases including organ segmentation, lesion detection, differential diagnosis, surgical planning, and prognosis prediction, which can provide support for disease diagnosis and management. In this review, we will introduce the basic methodology of imaging-based deep learning and its recent clinical applications in neoplastic and non-neoplastic kidney diseases. Additionally, we further discuss its current challenges and future prospects and conclude that achieving data balance, addressing heterogeneity, and managing data size remain challenges for imaging-based deep learning. Meanwhile, the interpretability of algorithms, ethical risks, and barriers of bias assessment are also issues that require consideration in future development. We hope to provide urologists, nephrologists, and radiologists with clear ideas about imaging-based deep learning and reveal its great potential in clinical practice.

Abstract:Background: Artificial intelligence (AI) systems have showed promising results in digital pathology, including digital nephropathology and specifically also kidney transplant pathology. Aim: Summarize the current state of research and limitations in the field of AI in kidney transplant pathology diagnostics and provide a future outlook. Materials and methods: Literature search in PubMed and Web of Science using the search terms "deep learning", "transplant", and "kidney". Based on these results and studies cited in the identified literature, a selection was made of studies that have a histopathological focus and use AI to improve kidney transplant diagnostics. Results and conclusion: Many studies have already made important contributions, particularly to the automation of the quantification of some histopathological lesions in nephropathology. This likely can be extended to automatically quantify all relevant lesions for a kidney transplant, such as Banff lesions. Important limitations and challenges exist in the collection of representative data sets and the updates of Banff classification, making large-scale studies challenging. The already positive study results make future AI support in kidney transplant pathology appear likely.

Abstract:Challenges in accurate tumor detection paves the way to haste the improvement of solid kidney tumor semantic segmentation methodologies. Accurate segmentation of kidney tumor in computer tomography (CT) images is a challenging task due to the non-uniform motion, similar appearance and various shape. Inspired by this fact, in this manuscript, we present a novel kidney tumor segmentation method using deep learning network termed as hyper vision Net model. All the existing U-net models are using a modified version of U-net to segment the kidney tumor region. In the proposed architecture, we introduced supervision layers in the decoder part, and it refines even minimal regions in the output. A dataset consists of real arterial phase abdominal CT scans of 300 patients, including 45964 images has been provided from KiTs19 for training and validation of the proposed model. Compared with the state-of-the-art segmentation methods, the results demonstrate the superiority of our approach on training dice value score of 0.9552 and 0.9633 in tumor region and kidney region, respectively.
A web-based tool for real-time adequacy assessment of kidney biopsies

Abstract:The escalating incidence of kidney biopsies providing insufficient tissue for diagnosis poses a dual challenge, straining the healthcare system and jeopardizing patients who may require re-biopsy or face the prospect of an inaccurate diagnosis due to an unsampled disease. Here, we introduce a web-based tool that can provide real-time, quantitative assessment of kidney biopsy adequacy directly from photographs taken with a smartphone camera. The software tool was developed using a deep learning-driven automated segmentation technique, trained on a dataset comprising nephropathologist-confirmed annotations of the kidney cortex on digital biopsy images. Our framework demonstrated favorable performance in segmenting the cortex via 5-fold cross-validation (Dice coefficient: 0.788±0.130) (n=100). Offering a bedside tool for kidney biopsy adequacy assessment has the potential to provide real-time guidance to the physicians performing medical kidney biopsies, reducing the necessity for re-biopsies. Prediction of Vascular Access Stenosis by Lightweight Convolutional Neural Network Using Blood Flow Sound Signals

Abstract:This research examines the application of non-invasive acoustic analysis for detecting obstructions in vascular access (fistulas) used by kidney dialysis patients. Obstructions in these fistulas can interrupt essential dialysis treatment. In this study, we utilized a condenser microphone to capture the blood flow sounds before and after angioplasty surgery, analyzing 3819 sound samples from 119 dialysis patients. These sound signals were transformed into spectrogram images to classify obstructed and unobstructed vascular accesses, that is fistula conditions before and after the angioplasty procedure. A novel lightweight two-dimension convolutional neural network (CNN) was developed and benchmarked against pretrained CNN models such as ResNet50 and VGG16. The proposed model achieved a prediction accuracy of 100%, surpassing the ResNet50 and VGG16 models, which recorded 99% and 95% accuracy, respectively. Additionally, the study highlighted the significantly smaller memory size of the proposed model (2.37 MB) compared to ResNet50 (91.3 MB) and VGG16 (57.9 MB), suggesting its suitability for edge computing environments. This study underscores the efficacy of diverse deep-learning approaches in the obstructed detection of dialysis fistulas, presenting a scalable solution that combines high accuracy with reduced computational demands.

Deep learning-based classification of kidney transplant pathology: a retrospective, multicentre, proof-of-concept study

Abstract:Background: Histopathological assessment of transplant biopsies is currently the standard method to diagnose allograft rejection and can help guide patient management, but it is one of the most challenging areas of pathology, requiring considerable expertise, time, and effort. We aimed to analyse the utility of deep learning to preclassify histology of kidney allograft biopsies into three main broad categories (ie, normal, rejection, and other diseases) as a potential biopsy triage system focusing on transplant rejection. Methods: We performed a retrospective, multicentre, proof-of-concept study using 5844 digital whole slide images of kidney allograft biopsies from 1948 patients. Kidney allograft biopsy samples were identified by a database search in the Departments of Pathology of the Amsterdam UMC, Amsterdam, Netherlands (1130 patients) and the University Medical Center Utrecht, Utrecht, Netherlands (717 patients). 101 consecutive kidney transplant biopsies were identified in the archive of the Institute of Pathology, RWTH Aachen University Hospital, Aachen, Germany. Convolutional neural networks (CNNs) were trained to classify allograft biopsies as normal, rejection, or other diseases. Three times cross-validation (1847 patients) and deployment on an external real-world cohort (101 patients) were used for validation. Area under the receiver operating characteristic curve (AUROC) was used as the main performance metric (the primary endpoint to assess CNN performance). Findings: Serial CNNs, first classifying kidney allograft biopsies as normal (AUROC 0·87 [ten times bootstrapped CI 0·85-0·88]) and disease (0·87 [0·86-0·88]), followed by a second CNN classifying biopsies classified as disease into rejection (0·75 [0·73-0·76]) and other diseases (0·75 [0·72-0·77]), showed similar AUROC in cross-validation and deployment on independent real-world data (first CNN normal AUROC 0·83 [0·80-0·85], disease 0·83 [0·73-0·91]; second CNN rejection 0·61 [0·51-0·70], other diseases 0·61 [0·50-0·74]). A single CNN classifying biopsies as normal, rejection, or other diseases showed similar performance in cross-validation (normal AUROC 0·80 [0·73-0·84], rejection 0·76 [0·66-0·80], other diseases 0·50 [0·36-0·57]) and generalised well for normal and rejection classes in the real-world data. Visualisation techniques highlighted rejection-relevant areas of biopsies in the tubulointerstitium. Interpretation: This study showed that deep learning-based classification of transplant biopsies could support pathological diagnostics of kidney allograft rejection. Abstract:Assessment of renal function and structure accurately remains essential in the diagnosis and prognosis of Chronic Kidney Disease (CKD). Advanced imaging, including Magnetic Resonance Imaging (MRI), Ultrasound Elastography (UE), Computed Tomography (CT) and scintigraphy (PET, SPECT) offers the opportunity to non-invasively retrieve structural, functional and molecular information that could detect changes in renal tissue properties and functionality. Currently, the ability of artificial intelligence to turn conventional medical imaging into a full-automated diagnostic tool is widely investigated. In addition to the qualitative analysis performed on renal medical imaging, texture analysis was integrated with machine learning techniques as a quantification of renal tissue heterogeneity, providing a promising complementary tool in renal function decline prediction. Interestingly, deep learning holds the ability to be a novel approach of renal function diagnosis. This paper proposes a survey that covers both qualitative and quantitative analysis applied to novel medical imaging techniques to monitor the decline of renal function. First, we summarize the use of different medical imaging modalities to monitor CKD and then, we show the ability of Artificial Intelligence (AI) to guide renal function evaluation from segmentation to disease prediction, discussing how texture analysis and machine learning techniques have emerged in recent clinical researches in order to improve renal dysfunction monitoring and prediction. The paper gives a summary about the role of AI in renal segmentation.

Integrating neural networks with advanced optimization techniques for accurate kidney disease diagnosis

Abstract:Kidney diseases pose a significant global health challenge, requiring precise diagnostic tools to improve patient outcomes. This study addresses this need by investigating three main categories of renal diseases: kidney stones, cysts, and tumors. Utilizing a comprehensive dataset of 12,446 CT whole abdomen and urogram images, this study developed an advanced AI-driven diagnostic system specifically tailored for kidney disease classification. The innovative approach of this study combines the strengths of traditional convolutional neural network architecture (AlexNet) with modern advancements in ConvNeXt architectures. By integrating AlexNet's robust feature extraction capabilities with ConvNeXt's advanced attention mechanisms, the paper achieved an exceptional classification accuracy of 99.85%. A key advancement in this study's methodology lies in the strategic amalgamation of features from both networks. This paper concatenated hierarchical spatial information and incorporated self-attention mechanisms to enhance classification performance. Furthermore, the study introduced a custom optimization technique inspired by the Adam optimizer, which dynamically adjusts the step size based on gradient norms. This tailored optimizer facilitated faster convergence and more effective weight updates, imporving model performance. The model of this study demonstrated outstanding performance across various metrics, with an average precision of 99.89%, recall of 99.95%, and specificity of 99.83%. These results highlight the efficacy of the hybrid architecture and optimization strategy in accurately diagnosing kidney diseases. Additionally, the methodology of this paper emphasizes interpretability and explainability, which are crucial for the clinical deployment of deep learning models.

Abstract:Background: Kidney biopsy is the gold-standard for diagnosing medical renal diseases, but the accuracy of the diagnosis greatly depends on the quality of the biopsy specimen, particularly the amount of renal cortex obtained. Inadequate biopsies, characterized by insufficient cortex or predominant medulla, can lead to inconclusive or incorrect diagnoses, and repeat biopsy. Unfortunately, there has been a concerning increase in the rate of inadequate kidney biopsies, and not all medical centers have access to trained professionals who can assess biopsy adequacy in real time. In response to this challenge, we aimed to develop a machine learning model capable of assessing the percentage cortex of each biopsy pass using smartphone images of the kidney biopsy tissue at the time of biopsy. Methods: 747 kidney biopsy cores and corresponding smartphone macro images were collected from five unused deceased donor kidneys. Each core was imaged, formalin-fixed, sectioned, and stained with Periodic acid-Schiff (PAS) to determine cortex percentage. The fresh unfixed core images were captured using the macro camera on an iPhone 13 Pro. Two experienced renal pathologists independently reviewed the PAS-stained sections to determine the cortex percentage. For the purpose of this study, the biopsies with less than 30% cortex were labeled as inadequate, while those with 30% or more cortex were classified as adequate. The dataset was divided into training (n=643), validation (n=30), and test (n=74) sets. Preprocessing steps involved converting High-Efficiency Image Container iPhone format images to JPEG, normalization, and renal tissue segmentation using a U-Net deep learning model. Subsequently, a classification deep learning model was trained on the renal tissue region of interest and corresponding class label. Results: The deep learning model achieved an accuracy of 85% on the training data. On the independent test dataset, the model exhibited an accuracy of 81%. For inadequate samples in the test dataset, the model showed a sensitivity of 71%, suggesting its capability to identify cases with inadequate cortical representation. The area under the receiver-operating curve (AUC-ROC) on the test dataset was 0.80. Conclusion: We successfully developed and tested a machine learning model for classifying smartphone images of kidney biopsies as either adequate or inadequate, based on the amount of cortex determined by expert renal pathologists. The model's promising results suggest its potential as a smartphone application to assist real-time assessment of kidney biopsy tissue, particularly in settings with limited access to trained personnel. Further refinements and validations are warranted to optimize the model's performance.

Abstract:Kidney volume is greatly affected in several renal diseases. Precise and automatic segmentation of the kidney can help determine kidney size and evaluate renal function. Fully convolutional neural networks have been used to segment organs from large biomedical 3D images. While these networks demonstrate state-of-the-art segmentation performances, they do not immediately translate to small foreground objects, small sample sizes, and anisotropic resolution in MRI datasets. In this paper we propose a new framework to address some of the challenges for segmenting 3D MRI. These methods were implemented on preclinical MRI for segmenting kidneys in an animal model of lupus nephritis. Our implementation strategy is twofold: 1) to utilize additional MRI diffusion images to detect the general kidney area, and 2) to reduce the 3D U-Net kernels to handle small sample sizes. Using this approach, a Dice similarity coefficient of 0.88 was achieved with a limited dataset of n=196. This segmentation strategy with careful optimization can be applied to various renal injuries or other organ systems.
