Abstract:MRI and fMRI have been used for about three and two decades respectively and much has changed over this time period, both in the quality of the data and in the range of applications for studying the brain. Apart from resolution improvements from around 4mm in the early days to below 0.5mm with modern technology, novel uses of contrast have led to the ability to sensitize images to some of the brain's structural properties at the cellular scale as well as study the localization and organization of brain function at the level of cortical columns. These developments have in part been facilitated by a continuing drive to increase the magnetic field strength. Will the next few decades see similar improvements? Here we will discuss current state of high field MRI, expected further increases in field strength, and improvements expected with these increases.

Abstract:Magnetic resonance imaging (MRI) and spectroscopy (MRS) have contributed considerably to our understanding of the structure and function of the human brain, and are essential in modern radiological practice. The development and applications of MRI systems at fields of 7 Tesla and above provide many new opportunities and technical challenges. The increased signal strength at higher fields may be used to obtain higher resolution images, faster images and/or images with greater contrast to noise. These can be used to improve detection of lesions, for more accurate assessment of the structural anatomy of the brain (including higher resolution tractography of white matter), and improved sensitivity in functional MRI. However, high field MR imaging is also affected by macroscopic field variations caused by inhomogeneities of magnetic susceptibility within the body, and these can degrade spectra and introduce image distortions. Moreover, the performance of radiofrequency (RF) coils is also affected at higher fields, and it is more difficult to create uniform RF fields within large objects. These challenges are being met using various technical innovations such as dynamic shimming, the use of parallel arrays of coils, novel spectral-spatial excitation methods, novel pulse sequences and post-acquisition digital processing. In combination these efforts promise to allow ultra-high field imaging and spectroscopy to contribute significantly to our understanding of brain structure and function in various conditions.

Abstract:Abstract Research in ultrahigh magnetic field strength combined with ultrahigh and ultrafast gradient technology has provided enormous gains in sensitivity, resolution, and contrast for neuroimaging. This article provides an overview of the technical advantages and challenges of performing clinical neuroimaging studies at ultrahigh magnetic field strength combined with ultrahigh and ultrafast gradient technology. Emerging clinical applications of 7-T MRI and state-of-the-art gradient systems equipped with up to 300 mT/m gradient strength are reviewed, and the impact and benefits of such advances to anatomical, structural and functional MRI are discussed in a variety of neurological conditions. Finally, an outlook and future directions for ultrahigh field MRI combined with ultrahigh and ultrafast gradient technology in neuroimaging are examined.

Abstract:Abstract Functional brain imaging technology has developed rapidly in recent years. On the one hand, high-field 7-Tesla magnetic resonance imaging (MRI) has excelled the limited spatial resolution of 3-Tesla MRI, allowing us to enter a new world of mesoscopic imaging from the macroscopic imaging of human brain functions. On the other hand, novel optical pumping magnetometer-magnetoencephalography (OPM-MEG) has broken down the technical barriers of traditional superconducting MEG, which brings imaging of neuronal electromagnetic signals from cortical imaging to whole-brain imaging. This article aims to present a brief introduction regarding the development of conventional MRI and MEG technology, and, more importantly, to delineate that high-field MRI and OPM-MEG complement each other and together will lead us into a new era of functional brain imaging.

Abstract:Human MRI scanners at ultra-high magnetic field strengths of 7 T and higher are increasingly available to the neuroscience community. A key advantage brought by ultra-high field MRI is the possibility to increase the spatial resolution at which data is acquired, with little reduction in image quality. This opens a new set of opportunities for neuroscience, allowing investigators to map the human cortex at an unprecedented level of detail. In this review, we present recent work that capitalizes on the increased signal-to-noise ratio available at ultra-high field and discuss the theoretical advances with a focus on sensory and motor systems neuroscience. Further, we review research performed at sub-millimeter spatial resolution and discuss the limits and the potential of ultra-high field imaging for structural and functional imaging in human cortex. The increased spatial resolution achievable at ultra-high field has the potential to unveil the fundamental computations performed within a given cortical area, ultimately allowing the visualization of the mesoscopic organization of human cortex at the functional and structural level.

Abstract:In the short time since its introduction, magnetic resonance imaging (MRI) has rapidly evolved to become an indispensable tool for clinical diagnosis and biomedical research. Recently, this methodology has been successfully used for the acquisition of functional, physiological, and biochemical information in intact systems, particularly in the human body. The ability to map areas of altered neuronal activity in the brain, often referred to as functional magnetic resonance imaging (fMRI), is probably one of the most significant recent achievements that rely on this methodology. This development has permitted the examination of functional specialization in human and animal brains with unprecedented spatial resolution, as demonstrated by mapping at the level of orientation and ocular dominance columns in the visual cortex. These functional imaging studies are complemented by the ability to study neurochemistry using magnetic resonance spectroscopy, allowing the determination of metabolic processes that support neurotransmission and neurotransmission rates themselves.

Abstract:Magnetic resonance imaging (MRI) is the cornerstone technique for diagnostic medicine, biology, and neuroscience. This imaging method is highly innovative, noninvasive and its impact continues to grow. It can be used for measuring changes in the brain after enhanced neural activity, detecting early cancerous cells in tissue, as well as for imaging nanoscale biological structures, and controlling fluid dynamics, and it can be beneficial for cardiovascular imaging. The MRI performance is characterized by a signal-to-noise ratio, however the spatial resolution and image contrast depend strongly on the scanner design. Here, we reveal how to exploit effectively the unique properties of metasurfaces for the substantial improvement of MRI efficiency. We employ a metasurface created by an array of wires placed inside the MRI scanner under an object, and demonstrate a giant enhancement of the magnetic field by means of subwavelength near-field manipulation with the metasurface, thus strongly increasing the scanner sensitivity, signal-to-noise ratio, and image resolution. We demonstrate experimentally this effect for a commercially available MRI scanner and a biological tissue sample. Our results are corroborated by measured and simulated characteristics of the metasurface resonator, and our approach can enhance dramatically functionalities of widely available low-field MRI devices.

Abstract:Magnetic resonance imaging (MRI) as one of the most important imaging methods is widely used in clinical diagnosis and basic life science research, with advantages including noninvasiveness, high/multiple soft tissue contrast, parametric imaging and the richness of information. During recent years, there have been many advances in the instrument technology and biomedical application of MRI. Advanced techniques, such as ultra-high field, ultra-fast and ultra-sensitive imaging, are now available for routine clinical usage and research settings. Also, the MRI technology is rapidly developing via incorporating with multiple cutting-edge disciplines including big data technology, artificial intelligence and theranostics. Currently, MRI is playing an indispensable role in tumor molecular imaging, functional brain imaging and brain connectome studies. This paper introduces the history and basic principles of MRI, and summarizes the important problems in the research of MRI instrument, methodology and application, providing a perspective on the directions of future MRI research in China.

Abstract:The ability to measure functional brain responses non-invasively with ultra high field MRI (7 T and above) represents a unique opportunity in advancing our understanding of the human brain. Compared to lower fields (3 T and below), ultra high field MRI has an increased sensitivity, which can be used to acquire functional images with greater spatial resolution, and greater specificity of the blood oxygen level dependent (BOLD) signal to the underlying neuronal responses. Together, increased resolution and specificity enable investigating brain functions at a submillimeter scale, which so far could only be done with invasive techniques. At this mesoscopic spatial scale, perception, cognition and behavior can be probed at the level of fundamental units of neural computations, such as cortical columns, cortical layers, and subcortical nuclei. This represents a unique and distinctive advantage that differentiates ultra high from lower field imaging and that can foster a tighter link between fMRI and computational modeling of neural networks. So far, functional brain mapping at submillimeter scale has focused on the processing of sensory information and on well-known systems for which extensive information is available from invasive recordings in animals. It remains an open challenge to extend this methodology to uniquely human functions and, more generally, to systems for which animal models may be problematic. To succeed, the possibility to acquire high-resolution functional data with large spatial coverage, the availability of computational models of neural processing as well as accurate biophysical modeling of neurovascular coupling at mesoscopic scale all appear necessary.

Deep learning enabled fast 3D brain MRI at 0.055 tesla

Abstract:In recent years, there has been an intensive development of portable ultralow-field magnetic resonance imaging (MRI) for low-cost, shielding-free, and point-of-care applications. However, its quality is poor and scan time is long. We propose a fast acquisition and deep learning reconstruction framework to accelerate brain MRI at 0.055 tesla. The acquisition consists of a single average three-dimensional (3D) encoding with 2D partial Fourier sampling, reducing the scan time of T1- and T2-weighted imaging protocols to 2.5 and 3.2 minutes, respectively. The 3D deep learning leverages the homogeneous brain anatomy available in high-field human brain data to enhance image quality, reduce artifacts and noise, and improve spatial resolution to synthetic 1.5-mm isotropic resolution. Our method successfully overcomes low-signal barrier, reconstructing fine anatomical structures that are reproducible within subjects and consistent across two protocols. It enables fast and quality whole-brain MRI at 0.055 tesla, with potential for widespread biomedical applications.

Abstract:Neuroimaging with ultra-high field magnets (≥7T) provides superior signal-to-noise, spatial resolution and tissue contrast; but also greater safety concerns, longer scanning times, and increased distortion and field inhomogeneity. Brain and spinal cord anatomic microstructure and function imaged in greater detail offers improved lesion detection, delineation, and characterization. The ongoing development of novel imaging contrasts and translation of cutting-edge sequences will aid more accurate, sensitive, and precise diagnosis, interventional planning, and follow-up for a variety of pathologic conditions.

A half-century of innovation in technology-preparing MRI for the 21st century

Abstract:MRI developed during the last half-century from a very basic concept to an indispensable non-ionising medical imaging technique that has found broad application in diagnostics, therapy control and far beyond. Due to its excellent soft-tissue contrast and the huge variety of accessible tissue- and physiological-parameters, MRI is often preferred to other existing modalities. In the course of its development, MRI underwent many substantial transformations. From the beginning, starting as a proof of concept, much effort was expended to develop the appropriate basic scanning technology and methodology, and to establish the many clinical contrasts (e.g., T1, T2, flow, diffusion, water/fat, etc.) that MRI is famous for today. Beyond that, additional prominent innovations to the field have been parallel imaging and compressed sensing, leading to significant scanning time reductions, and the move towards higher static magnetic field strengths, which led to increased sensitivity and improved image quality. Improvements in workflow and the use of artificial intelligence are among many current trends seen in this field, paving the way for a broad use of MRI. The 125th anniversary of the BJR is a good point to reflect on all these changes and developments and to offer some slightly speculative ideas as to what the future may bring.

Abstract:Magnetic resonance elastography (MRE) is a non-invasive method for determining the mechanical response of tissues using applied harmonic deformation and motion-sensitive MRI. MRE studies of the human brain are typically performed at conventional field strengths, with a few attempts at the ultra-high field strength, 7T, reporting increased spatial resolution with partial brain coverage. Achieving high-resolution human brain scans using 7T MRE presents unique challenges of decreased octahedral shear strain-based signal-to-noise ratio (OSS-SNR) and lower shear wave motion sensitivity. In this study, we establish high resolution MRE at 7T with a custom 2D multi-slice single-shot spin-echo EPI sequence, using the Gadgetron advanced image reconstruction framework, applying Marchenko-Pastur Principal Component Analysis denoising, and using Nonlinear Viscoelastic Inversion. These techniques allowed us to calculate the viscoelastic properties of the whole human brain at 1.1mm isotropic imaging resolution with high OSS-SNR and repeatibility. Using phantom models and 7T MRE data of eighteen healthy volunteers, we demonstrate the robustness of our method at high resolution while quantifying the feasible tradeoff between resolution, OSS-SNR, and scan time. Using these post-processing techniques, we significantly increased OSS-SNR at 1.1mm resolution with whole-brain coverage by approximately 4-fold and generated elastograms with high anatomical detail. Performing high-resolution MRE at 7T on the human brain can provide information on different substructures within brain tissue based on their mechanical properties, which can then be used to diagnose pathologies (e.g., Alzheimer's disease), indicate disease progression, or better investigate neurodegeneration effects or other relevant brain disorders, in vivo.

Abstract:The advent of portable, low-field MRI (LF-MRI) heralds new opportunities in neuroimaging. Low power requirements and transportability have enabled scanning outside the controlled environment of a conventional MRI suite, enhancing access to neuroimaging for indications that are not well suited to existing technologies. Maximizing the information extracted from the reduced signal-to-noise ratio of LF-MRI is crucial to developing clinically useful diagnostic images. Progress in electromagnetic noise cancellation and machine learning reconstruction algorithms from sparse k-space data as well as new approaches to image enhancement have now enabled these advancements. Coupling technological innovation with bedside imaging creates new prospects in visualizing the healthy brain and detecting acute and chronic pathological changes. Ongoing development of hardware, improvements in pulse sequences and image reconstruction, and validation of clinical utility will continue to accelerate this field. As further innovation occurs, portable LF-MRI will facilitate the democratization of MRI and create new applications not previously feasible with conventional systems.

Abstract:Advances in modern medicine have benefited greatly from technological advances. In brain disease, early diagnosis and individualized treatment are partly limited by the precision of detection methods. For example, traditional magnetic resonance imaging (MRI) diagnostics are based primarily on > 1-mm scale resolution, whereas basic units of brain function are at the submillimeter scale. The importance of mesoscale organization in the brain is becoming increasingly evident from research in non-human primates (NHPs) and is now impacting our understanding of brain disease. In this review, we highlight advances in clinical diagnosis using ultrahigh-field (UHF) MRI, with a particular focus on contributions from NHP research. Impact on neurology, neurosurgery, and psychiatry are illustrated, including UHF MRI contributions to increased spatiotemporal resolutions, improved tissue contrast, and higher sensitivity to neurochemical signatures. We present a future where the early diagnosis and individualized treatment of human brain disease are aided by UHF MRI technology and NHP research.

Abstract:Molecular magnetic resonance imaging (MRI) is an emerging field that is set to revolutionize our perspective of disease diagnosis, treatment efficacy monitoring, and precision medicine in full concordance with personalized medicine. A wide range of hyperpolarized (HP) 129Xe biosensors have been recently developed, demonstrating their potential applications in molecular settings, and achieving notable success within in vitro studies. The favorable nuclear magnetic resonance properties of 129Xe, coupled with its non-toxic nature, high solubility in biological tissues, and capacity to dissolve in blood and diffuse across membranes, highlight its superior role for applications in molecular MRI settings. The incorporation of reporters that combine signal enhancement from both hyperpolarized 129Xe and chemical exchange saturation transfer holds the potential to address the primary limitation of low sensitivity observed in conventional MRI. This review provides a summary of the various applications of HP 129Xe biosensors developed over the last decade, specifically highlighting their use in MRI. Moreover, this paper addresses the evolution of in vivo applications of HP 129Xe, discussing its potential transition into clinical settings.

Abstract:To increase granularity in human neuroimaging science, we designed and built a next-generation 7 Tesla magnetic resonance imaging scanner to reach ultra-high resolution by implementing several advances in hardware. To improve spatial encoding and increase the image signal-to-noise ratio, we developed a head-only asymmetric gradient coil (200 mT m −1 , 900 T m −1 s −1 ) with an additional third layer of windings. We integrated a 128-channel receiver system with 64- and 96-channel receiver coil arrays to boost signal in the cerebral cortex while reducing g-factor noise to enable higher accelerations. A 16-channel transmit system reduced power deposition and improved image uniformity. The scanner routinely performs functional imaging studies at 0.35–0.45 mm isotropic spatial resolution to reveal cortical layer functional activity, achieves high angular resolution in diffusion imaging and reduces acquisition time for both functional and structural imaging.

Abstract:Recent technological advancements have revolutionized routine brain magnetic resonance imaging (MRI) sequences, offering enhanced diagnostic capabilities in intracranial disease evaluation. This review explores 2 pivotal breakthrough areas: deep learning reconstruction (DLR) and quantitative MRI techniques beyond conventional structural imaging. DLR using deep neural networks facilitates accelerated imaging with improved signal-to-noise ratio and spatial resolution, enhancing image quality with short scan times. DLR focuses on supervised learning applied to clinical implementation and applications. Quantitative MRI techniques, exemplified by 2D multidynamic multiecho, 3D quantification using interleaved Look-Locker acquisition sequences with T2 preparation pulses, and magnetic resonance fingerprinting, enable precise calculation of brain-tissue parameters and further advance diagnostic accuracy and efficiency. Potential DLR instabilities and quantification and bias limitations will be discussed. This review underscores the synergistic potential of DLR and quantitative MRI, offering prospects for improved brain imaging beyond conventional methods.
Unveiling the future: Advancements in MRI imaging for neurodegenerative disorders

Abstract:Neurodegenerative disorders represent a significant and growing global health challenge, necessitating continuous advancements in diagnostic tools for accurate and early detection. This work explores the recent progress in Magnetic Resonance Imaging (MRI) techniques and their application in the realm of neurodegenerative disorders. The introductory section provides a comprehensive overview of the study's background, significance, and objectives. Recognizing the current challenges associated with conventional MRI, the manuscript delves into advanced imaging techniques such as high-resolution structural imaging (HR-MRI), functional MRI (fMRI), diffusion tensor imaging (DTI), and positron emission tomography-MRI (PET-MRI) fusion. Each technique is critically examined regarding its potential to address theranostic limitations and contribute to a more nuanced understanding of the underlying pathology. A substantial portion of the work is dedicated to exploring the applications of advanced MRI in specific neurodegenerative disorders, including Parkinson's disease, Alzheimer's disease, Huntington's disease, and Amyotrophic Lateral Sclerosis (ALS). Quantitative Imaging in Medicine and Surgery

Abstract:Background: We aimed to demonstrate the feasibility of generating high-resolution human brain magnetic resonance imaging (MRI) at 5 Tesla (T) using a quadrature birdcage transmit/48-channel receiver coil assembly. Methods: A quadrature birdcage transmit/48-channel receiver coil assembly was designed for human brain imaging at 5T. The radio frequency (RF) coil assembly was validated by electromagnetic (EM) simulations and phantom imaging experimental studies. The simulated B1+ field inside a human head phantom and inside a human head model generated by the birdcage coils driven in circularly polarized (CP) mode at 3T, 5T and 7T was compared. Signal-to-noise ratio (SNR) maps, the inverse g-factor maps for evaluation of parallel imaging performance, anatomic images, angiography images, vessel wall images and susceptibility weighted images (SWI) were acquired using the RF coil assembly at 5T and compared to those acquired using a 32-channel head coil on a 3T MRI scanner. Results: For the EM simulations, 5T MRI provided less RF inhomogeneity compared to that of 7T. In the phantom imaging study, the distributions of the measured B1+ field were consistent with the distributions of the simulated B1+ field. In the human brain imaging study, the average SNR value of the brain in the transversal plane at 5T was 1.6 times of that at 3T. The 48-channel head coil at 5T had higher parallel acceleration capability than the 32-channel head coil at 3T. The anatomic images at 5T also showed higher SNR than those at 3T. Improved delineation of the hippocampus, lenticulostriate arteries, and basilar arteries was observed at 5T compared to 3T. SWI with a higher resolution of 0.3 mm x0.3 mm x1.2 mm could be acquired at 5T, which enabled better visualization of small blood vessels compared to that at 3T. Conclusions: 5T MRI can provide significant SNR improvement compared to that of 3T with less RF inhomogeneity than that of 7T. The ability to obtain high quality in vivo human brain images at 5T using the quadrature birdcage transmit/48-channel receiver coil assembly has significant in clinical and scientific research applications.
