Abstract:Corpus callosal agenesis (CCA) is a rare congenital disorder characterized by the partial or complete absence of the corpus callosum, a structure crucial for interhemispheric communication. CCA can occur in isolation or be associated with other anomalies such as heterotopia, holoprosencephaly, cerebellar hypoplasia, coloboma, and hydrocephalus. The prevalence of CCA ranges from 0.020% to 0.025%, though some reports suggest higher rates. This case report describes a 1-year-old male with developmental delays and no significant antenatal or family history. MRI revealed a complete absence of the corpus callosum, asymmetrically dilated lateral ventricles, subependymal gray matter nodules suggestive of gray matter heterotopia, and bilateral posterior globe defects with vitreous herniation, indicating severe ocular anomalies. The child received supportive care including physical therapy and special education services, with regular follow-ups for developmental and ophthalmologic evaluation. This case report details the rare occurrence of CCA, accompanied by gray matter heterotopia and bilateral posterior eye coloboma in a pediatric patient. The combination of these congenital anomalies presents unique diagnostic and management challenges requiring multidisciplinary care. We discuss the clinical presentation, radiological findings, and implications for supportive care and improving the prognosis.

Abstract:To evaluate the diagnostic value of fetal neurosonography combined with fast intrauterine magnetic resonance imaging (MRI) in fetal agenesis of corpus callosum (ACC). Malformations of the central nervous system (CNS) together with fetal heart defects are the most common congenital anomalies. The introduction of three dimensional (3D) ultrasound for evaluating fetal brain structures has made it possible to diagnose pathological findings of the CNS with increasing accuracy. Between January 2014 and December 2017, 117 fetuses, mean gestational age of 29.3 weeks (range 22-40 weeks), with mild ventriculomegaly and diaphanous diaphragm reduction or disappearance diagnosed at prenatal sonography were included in this study. All fetuses underwent intrauterine MRI according to the following protocol: images along the three orthogonal plane according to the longitudinal axis of the mother, and subsequently three orthogonal planes were acquired according to the fetal brain. Postnatal physical examination and diagnostic imaging were the standard of diagnosis. The sensitivity of each modality for detecting anomalies was calculated and compared by the McNemar test. In 117 fetuses, mean axial diameter of the lateral ventricle was 11.7 mm (range 10-17 mm), 69/117 fetuses (58.97%) showed normal morphology of the lateral ventricles, and the entire corpus callosum was visualised. In 48/117 fetuses (41.03%) were confirmed with ACC. 40 cases were diagnosed by detailed fetal neurosonography, 45 cases by intrauterine MRI and their sensitivity were 83.33% and 93.75% respectively. There was a statistically significant difference in the sensitivity for detecting anomalies between US and MRI (P <0.05). The sensitivity of combined diagnosis was 95.83%, with statistically significant difference to US alone (P <0.01). Detailed fetal neurosonography combined with fast MRI can accurately diagnose fetal ACC, especially improve the detection rate of partial corpus callosum agenesis, and provide a reliable basis for clinical diagnosis.

Abstract:Objective To discuss the clinical significance of fetal large cavum septum pellucidum and cavum vergae by prenatal ultrasonography. Methods Prenatal ultrasonography was performed on women at our hospital for prenatal diagnosis during January 2006 to December 2008. The changes of fetal large cavum septum pellucidum and cavum vergae were observed regularly until delivery.The live infants were followed up regularly in one year. Results Totally 58 women with fetal large cavum septum pellucidum and cavum vergae were included in the study,with the incidence of 0.19%.But 7 cases losed in the observating progress. They were diagnosed between 24~37 gestational weeks,with a median of (30±4) weeks. In the pregnancy period,24 cases (24/51,47.0%) of the large cavum septum pellucidum and cavum vergae were decreased,and 19 cases were seen decreased to nomal degree in gestation. There was significant difference between the two groups,which the degree of cavum septum pellucidum and cavum vergae expansion decreased and the other didn’t,also with statistical significance (P<0.05). And between cases which were combined with other abnormalities with large cavum septum pellucidum and cavum vergae and the others not,perinatal outcomes also have significant difference (P<0.05).Conclusion Fetus with large cavum septum pellucidum and cavum vergae occur mainly in the second and third trimesters of pregnancy. When we found that the degree of the expansion degree of fetal large cavum septum pellucidum and cavum vergae decreased,it may be possible good signs of perinatal outcomes. Either it is combined with other intracranial and extracranial structures anomalies can affect the perinatal outcomes. So when the ultrasound examination diagnosis of large cavum septum pellucidum and cavum vergae,we should be close ultrasound follow-up of the size of cavum septum pellucidum and cavum vergae and other intracranial,extracranial structures anomalies.

Abstract:Objective to describe the normal features of the caudo‐thalamic groove at antenatal brain ultrasound in a group of structurally normal fetuses at third trimester and to report a small series of cases with abnormal appearance of the caudothalamic groove at antenatal brain ultrasound. Methods This was an observational study conducted at two referral Fetal Medicine units. A non‐consecutive cohort of pregnant women with a singleton non anomalous pregnancy were prospectively recruited and underwent 3D ultrasound of the fetal brain at 28‐32 weeks. At offline analysis the ultrasound volumes were adjusted in the multiplanar mode according to a standardized methodology, until the caudothalamic groove was visible on the parasagittal plane. To evaluate the inter‐observer agreement, two operators were independently asked to indicate if the caudothalamic groove was visible unilaterally or bilaterally on each volume. The digital archives of the two Centres were also retrospectively searched to retrieve cases with abnormal findings at the level of the caudothalamic groove at antenatal brain ultrasound which were postnatally confirmed. Results 180 non‐consecutive cases fulfilling the inclusion criteria were prospectively included. At offline analysis of the 3D US volumes the caudo‐thalamic groove was identified on the parasagittal plane by both operators at least unilaterally in 176 cases (97.8%) and bilaterally in 174 cases (96.6%). The K‐coefficient for the agreement between the two independent operators in recognizing the caudo‐thalamic groove was 0.89 and 0.83 on one and both hemispheres respectively. At the retrospective search of our archives 5 cases with abnormal appearance of the groove at antenatal brain ultrasound (2 haemorrhage and 3 cyst) were found. Conclusion Our study has demonstrated that the caudo‐thalamic groove is consistently seen among normal fetuses at third trimester submitted to multiplanar neurosonography and that abnormal findings at this level may be antenatally detected.
