This study analyzed a total of 8189 cleavage-stage frozen-thawed embryo transfer cycles between January 2013 and December 2017 at Sir Run Run Shaw Hospital. All cycles were sorted into 3 groups based on patient age: <= 35 years (Group A), 36 to 37 years (Group B), and >= 38 years (Group C). The clinical pregnancy rate (CPR), implantation rate (IR), live birth rate (LBR), miscarriage rate, multiple pregnancy rate (MPR), preterm birth rate, and low-birth-weight rate were compared between the 3 groups. Significant differences in CPR, IR, LBR, MPR, and premature birth rate were found among the 3 groups. The CPR, IR, LBR, and MPR in Group A were higher than those in Group C. Transfers of 2 high-quality embryos resulted in significant differences in CPR, IR, LBR, MPR, and neonatal weight among the 3 groups, but no differences in premature birth and abortion rates were observed. Transfers with 1 high-quality and 1 fair-quality embryo resulted in significant differences in CPR, IR, and LBR among the 3 groups. Comparison of transfers of 2 high-quality embryos with 1 high-quality and 1 fair-quality embryo showed that the CPR and LBR were significantly lower for the latter in Groups A and C, but Group B had no salient changes. Higher IR and LBR and lower MPR may be achieved by selection of optimal embryo types for patients of different ages. Two high-quality embryos need to be transferred in women older than 38 years. For women aged 36 to 37 years, 1 high-quality embryo or 1 high-quality plus 1 fair-quality embryo should be singled out for transfer. For women younger than 35 years, a single high-quality embryo should be selected for transfer.

Abstract:BACKGROUND:Elective single embryo transfer (eSET) has been increasingly advocated, but concerns about the lower pregnancy rate after reducing the number of embryos transferred have encouraged transfer of multiple embryos. Extended embryo culture combined with electively freezing all embryos and undertaking a deferred frozen embryo transfer might increase pregnancy rate after eSET. We aimed to establish whether elective frozen single blastocyst transfer improved singleton livebirth rate compared with fresh single blastocyst transfer.METHODS:This multicentre, non-blinded, randomised controlled trial was undertaken in 21 academic fertility centres in China. 1650 women with regular menstrual cycles undergoing their first cycle of in-vitro fertilisation were enrolled from Aug 1, 2016, to June 3, 2017. Eligible women were randomly assigned to either fresh or frozen single blastocyst transfer. The randomisation sequence was computer generated, with block sizes of two, four, or six, stratified by study site. For those assigned to frozen blastocyst transfer, all blastocysts were cryopreserved and a delayed frozen-thawed single blastocyst transfer was done. The primary outcome was singleton livebirth rate. Analysis was by intention to treat. This trial is registered at the Chinese Clinical Trial Registry, number ChiCTR-IOR-14005405.FINDINGS:825 women were assigned to each group and included in analyses. Frozen single blastocyst transfer resulted in higher rates of singleton livebirth than did fresh single blastocyst transfer (416 [50%] vs 329 [40%]; relative risk [RR] 1·26, 95% CI 1·14-1·41, p<0·0001). The risks of moderate or severe ovarian hyperstimulation syndrome (four of 825 [0·5%] in frozen single blastocyst transfer vs nine of 825 [1·1%] in fresh single blastocyst transfer; p=0·16), pregnancy loss (134 of 583 [23·0%] vs 124 of 481 [25·8%]; p=0·29), other obstetric complications, and neonatal morbidity were similar between the two groups. Frozen single blastocyst transfer was associated with a higher risk of pre-eclampsia (16 of 512 [3·1%] vs four of 401 [1·0%]; RR 3·13, 95% CI 1·06-9·30, p=0·029).INTERPRETATION:Frozen single blastocyst transfer resulted in a higher singleton livebirth rate than did fresh single blastocyst transfer in ovulatory women with good prognosis. The increased risk of pre-eclampsia after frozen blastocyst transfer warrants further studies.FUNDING:The National Key Research and Development Program of China.
[Study of the Best Embryo Transfer Strategy in Frozen-Thawed Embryo Transfer Cycle].

Abstract:OBJECTIVES:To study the relationship of the number and quality of embryos transferred with clinical pregnancy rate and multiple pregnancy rate in frozen-thawed embryo transfer (FET) cycle. METHODS:Retrospective analysis of the clinical data of 995 FET cycles. Patients were stratified as age < 35 or > or = 35, Both groups' clinical pregnancy rate and multiple pregnancy rate were compared according to the number of embryos and high-quality embryos transferred. RESULTS:(1) The clinical pregnancy rates were 50.0%, 56.6% and 56.5%, and twin pregnancy rates were 6.3%, 43.8% and 30.6% respectively in < 35 group when one, two or three embryos were transferred. There is not any significantly statistic difference among these subgroups (P > 0.05). However, the twin pregnancy rate in one-embryo transferred subgroup was significantly lower than that of two-embryo transferred subgroup (P < 0.05). Moreover, the clinical pregnancy rate was not different among single high-quality embryo transferred subgroup and other high-quality embryo transferred subgroups, the multiple pregnancy rate was lower than the others. (2) The clinical pregnancy rates were 0, 47.3% and 53.8%, and twin pregnancy rates were 0, 25.7% and 25.7% respectively in > or = 35 group when one, two or three embryos were transferred. The clinical pregnancy rate and twin pregnancy rate between two-embryo and three-embryo transferred subgroups were not significantly different (P > 0.05). Moreover, the clinical pregnancy rate was not different among two-embryo transferred containing one high-quality embryo subgroup and other high-quality embryo transferred subgroups, the multiple pregnancy rate was lower than the others. CONCLUSIONS:There is a close relationship between the number and quality of embryos transferred and clinical pregnancy rate, multiple pregnancy rate in FET cycles. For the patient aged < 35, we recommend performing single high-quality embryo transfer in order to reduce the multiple pregnancy. For the patient aged > or = 35, two-embryo transfer containing one high-quality embryo is enough to obtain a satisfied clinical pregnancy rate and an acceptable multiple pregnancy rate.

Abstract:(Abstracted from N Engl J Med 2018;378:126–136) A previous randomized controlled trial involving women with polycystic ovary syndrome (PCOS) compared fresh-embryo transfer with elective freezing of all embryos followed by frozen-embryo transfer. The data showed that frozen-embryo transfer had a higher live-birth rate in these women compared with fresh-embryo transfer. Whether frozen-embryo transfer also increases live-birth rates among ovulatory women with infertility without PCOS is uncertain.

Abstract:Multiple pregnancies are one of the major safety concerns of in vitro fertilization (IVF) due to the increased risk of maternal and neonatal complications. Single embryo transfer is the most effective way to reduce the risk of multiple pregnancies. Selection of the embryo and optimization of the implantation environment are crucial to retain the success rate when the number of transferred embryos is diminished. Fresh embryo transfer with supra-physiological levels of hormones has been suggested to have an adverse effect on implantation. Elective frozen embryo transfer has been suggested to result in a higher rate of live birth than fresh embryo transfer. However, there is still a lack of evidence from randomized clinical trials comparing the efficacy and safety between frozen and fresh single blastocyst transfers.

Abstract:BACKGROUND:Reduced endometrial receptivity in hyperstimulated cycles may lead to a lower implantation rate and a lower clinical pregnancy rate, but it is unclear if it is also associated with an increase in pregnancy loss rate. The aim of this study was to compare the implantation, miscarriage, and pregnancy rates between fresh and frozen thawed transfer of one or two day-3 embryos, with a view to understanding whether or not reduced endometrial receptivity encountered in hyperstimulated cycles is associated with an increase in miscarriage rate.METHODS:This study involved a consecutive series of 1 551 single day-3 embryo transfer cycles and consecutive 5 919 double day-3 embryo transfer cycles in the Assisted Reproductive Unit of the Sir Run Run Shaw Hospital, Hangzhou, China, between January 2010 and December 2012.RESULTS:The implantation and clinical pregnancy rates (single embryo 30.7% and double embryos 33.4% and 51.4%) using fresh cycle were both significantly lower than that of frozen-thawed cycles (single embryo 35.8% and double embryos 38.1% and 57.8%). There was no difference in biochemical loss or clinical miscarriage rates between the two groups.CONCLUSIONS:Impairment of endometrial receptivity associated with ovarian hyperstimulation leads to implantation failure at a very early stage, resulting in an increased number of non-pregnancy. It does not lead to increase in biochemical or clinical losses. The significantly reduced ongoing pregnancy rates in both fresh single and double embryo transfer are therefore due to failure to achieve a pregnancy, rather than pregnancy loss after conception.
No advantage of single day 6 good-quality blastocyst transfer versus single day 5 poor-quality blastocyst transfer in frozen-thawed cycles stratified by age: a retrospective study

Abstract:Background: Blastocyst developmental speed, morphological grading and patient age are associated with pregnancy outcomes of frozen-thawed cycles. This study aimed to compare the clinical and neonatal outcomes between poor-quality D5 blastocysts and good-quality D6 blastocysts stratified by patient age. Methods: A total of 1,623 cycles were divided into two groups: group A (n = 723) received one D5 poor-quality blastocyst; group B (n = 900) received one D6 good-quality blastocyst. Pregnancy and neonatal outcomes were compared among the four groups stratified by 35 years of age. Results: When patients were in the same age group, there was no significant difference in terms of age, body mass index, infertility duration, infertility type, fertilization method, proportion of endometrial preparation protocols, and endometrial thickness between D5 poor-quality and D6 high-quality blastocysts groups. Live birth rate of D5 poor-quality blastocysts was higher than that of D6 high-quality blastocysts for patients aged < 35 years (35.48% vs. 31.13%, p > 0.05), but there was no statistical difference. The same trend was showed for patients aged ≥ 35 years (29.09% vs. 21.28%, p > 0.05). Moreover, when patients were in the same age category, there was no significant difference in terms of gestational age, birth weight, birth height, and rates of preterm birth, low birth weight, and very low birth weight between groups A and B. Conclusions: The preferential selection of poor-quality D5 blastocysts for transfer compared to high-quality D6 blastocysts is recommended, especially for advanced age patients. Single good-quality D6 blastocyst transfer can be considered for the acceptable live birth rate.

Abstract:Abstract Background The present evidence is deficient for the trade-offs between the pros and cons of single blastocyst transfer (SBT) versus double blastocyst transfer (DBT) in frozen-thawed embryo transfer cycles for women in advanced reproductive age, especially in the second cycle. The current study aimed to investigate the impact of transferred blastocyst numbers on pregnancy outcomes in the first and second embryo transfer for women ≥ 35 years. Methods This was a retrospective cohort study including 1284 frozen-thawed blastocyst transfer (FBT) cycles from two reproductive centers. We analyzed the pregnancy outcomes after SBT and DBT in the first and second FBT cycles. Moreover, stratified analysis was conducted by maternal age. Results In the first FBT cycle, the LBR was higher in the DBT group than that in the SBT group [52.3% vs. 33.9%; adjusted odds ratio (aOR), 1.65; 95% confidence interval (CI), 1.26–2.15, P < 0.001]. However, the LBR of the DBT group showed no remarkable difference compared with that of the SBT group in the second cycle of FBT (44.3% vs. 33.3%; aOR, 1.30; 95% CI, 0.81–2.08; P = 0.271). Furthermore, stratified analysis by age showed a higher LBR for the DBT group than the SBT group in patients aged 38–42 years (43.1% vs. 33.9%; aOR, 2.27; 95% CI, 1.05–4.90; P = 0.036). Conclusions The present study demonstrated that the SBT regimen is a better choice for both, the first and second frozen-thawed embryo transfer cycles, for women aged 35–37 years. Additionally, the DBT regimen is still recommended to achieve a high LBR in women aged 38–42 years in the second FBT cycle. These findings may be beneficial for deciding the embryo transfer regimens in women of advanced reproductive age.

Abstract:(1) Background: Single blastocyst transfers (SBT) and double blastocyst transfers (DBT) are widely used in clinical practice. The objective of this study was to investigate the application of these two strategies in women of different ages. (2) Methods: Analysis was carried out on 5477 frozen embryo transfer cycles of women in different ages. All the cycles were divided into three groups according to the age of the included women: <35, 35–39, and >39. The live birth rate (LBR) and multiple birth rate (MBR) between the SBT and DBT among these groups, respectively, were compared. (3) Results: In the women < 35 group, the LBR was similar in SBT and DBT, but the MBR was higher in DBT than SBT. In women 35–39, with >10 oocytes retrieved, the MBR in DBT was significantly higher than SBT, but there was no significant difference in LBR between the two groups; with ≤10 oocytes retrieved, the LBR in DBT were significantly higher than SBT, but the MBT was not significantly different between the two groups. In women > 39, the LBR and MBR were lower in the SBT than DBT, but the differences were not statistically significant. (4) Conclusions: Selective SET is appropriate for most young women, whereas older women are advised to make individualized choices based on the number of oocytes retrieved and blastocyst quality.

Abstract:Introduction: The utilization of frozen embryo transfer not only enhances reproductive outcomes by elevating the likelihood of live birth and clinical pregnancy but also improves safety by mitigating the risks associated with ovarian hyperstimulation syndrome (OHSS) and multiple pregnancies. There has been an increasing debate in recent years regarding the advisability of making elective frozen embryo transfer the standard practice. Our study aims to determine the optimal choice between fresh and frozen embryo transfer, as well as whether the transfer should occur at the cleavage or blastocyst stage. Method: In this retrospective cohort study conducted in Taiwan, data from the national assisted reproductive technology (ART) database spanning from January 1st, 2013, to December 31st, 2017, were analyzed. The study included 51,762 eligible female participants who underwent ART and embryo transfer. Pregnancy outcomes, maternal complications, and singleton neonatal outcomes were evaluated using the National Health Insurance Database from January 1st, 2013, to December 31st, 2018. Cases were categorized into groups based on whether they underwent fresh or frozen embryo transfers, with further subdivision into cleavage stage and blastocyst stage transfers. Exposure variables encompassed clinical pregnancy rate, live birth rate, OHSS, pregnancy-induced hypertension, gestational diabetes mellitus (DM), placenta previa, placental abruption, preterm premature rupture of membranes (PPROM), gestational age, newborn body weight, and route of delivery. Results: Frozen blastocyst transfers showed higher rates of clinical pregnancy (CPR) and live births (LBR) compared to fresh blastocyst transfers. Conversely, frozen cleavage stage transfers demonstrated lower rates of clinical pregnancy and live birth compared to fresh cleavage stage transfers. Frozen embryo transfers were associated with reduced risks of OHSS but were linked to a higher risk of pregnancy-induced hypertension compared to fresh embryo transfers. Additionally, frozen embryo transfers were associated with a higher incidence of large for gestational age infants and a lower incidence of small for gestational age infants. Conclusion: The freeze-all strategy may not be suitable for universal application. When embryos can develop to the blastocyst stage, FET is a favorable choice, but embryos can only develop to the cleavage stage, fresh embryo transfer becomes a more reasonable option.

[Analysis of Pregnancy Outcomes of D6 Single Blastocyst Transplantation in Fresh and Frozen-Thawed Cycles].

Abstract:Objective: To compare the main clinical outcomes of Day 6 (D6) single blastocyst transplantation in fresh and frozen-thawed cycles. Methods: The data of fresh blastocyst transplantation patients and frozen-thawed blastocyst transplantation patients from January 2013 to December 2017 were retrospectively analysed. Fresh blastocyst transplantation and frozen-thawed blastocyst transplantation were matched in a ratio of 1∶3 by using propensity score matching, the matching factors included age, body mass index, thickness of endometrium and blastocyst grade. Totally 180 cases were included in the fresh cycle group and 540 cases in the frozen-thawed cycle group. Results: There was no significant difference in basal FSH between the two groups [(6.9±2.5) versus (6.4±3.8) U/L, P=0.334]. The positive rate of hCG in D6 blastocyst fresh cycle transplantation group [32.8%(59/180) versus 48.1%(260/540)], clinical pregnancy rate [28.9%(52/180) versus 43.5%(235/540)] and live birth rate [21.1%(38/180) versus 32.2%(174/540)] were lower than those of frozen-thawed cycle group (all P<0.05). The miscarriage rate was higher [26.9%(14/52) versus 24.7%(58/235)], but there was no statistical difference (P>0.05). Conclusions: The clinical pregnancy outcome of D6 single blastocyst frozen-thawed cycle transplantation is better than that of fresh cycle. In order to obtain better clinical outcomes, frozen-thawed cycle transplantation of blastocysts formed on the 6th day is recommended.

Abstract:Objective: Regarding frozen-thawed embryo transfer (FET), there is limited consensus on whether extending embryo culture from the cleavage stage to the blastocyst stage affects perinatal outcomes. This study aimed to compare perinatal outcomes of singletons between blastocyst-stage transfer (BT) and cleavage-stage transfer (CT) in FET. Methods: A total of 9,408 FET cycles that met the inclusion criteria were included in this retrospective cohort study between 2019 and 2022. Blastocyst-stage embryo transfers were performed in the BT group, and cleavage-stage embryo transfers were performed in the CT group. Multivariate logistic regression analyses were performed, as well as propensity score matching (PSM) to adjust for confounders. Results: After PSM, a higher risk of preterm birth (PTB) (OR 1.23, 95% CI 1.00–1.50, P =0.048) and being large for gestational age (LGA) (OR 1.16, 95% CI 1.00–1.35, P =0.050) was observed in the BT group compared to that in the CT group. After stratified PSM, in the subgroup under 35 years of age, only an increased risk of LGA was observed in the BT group compared to the CT group. Perinatal outcomes in the double-embryo transfer subgroup were similar to those in the unstratified group. However, in the subgroup beyond 35 years of age and the single-embryo transfer subgroup, perinatal outcomes were not statistically different between the BT and CT groups ( P >0.05). Conclusions: In FET, prolonged embryo culture to the blastocyst stage increased the risk of PTB and LGA in single fetuses. However, stratified analysis based on age and the number of transferred embryos yielded different results, necessitating further mechanistic studies.

Abstract:OBJECTIVE:To compare the multiple pregnancy rates of frozen-thawed embryos transfer (FET) or fresh embryos transfer, analyze the factors related to multiple pregnancies after IVF and study pregnancy rates with different number of embryos transferred in FET cycle.METHODS:A retrospective analysis was performed upon multiple pregnancies from 1235 and 1561 clinical pregnancies conceived by FET or fresh embryo transfers.RESULTS:No correlation was found between fresh or cryopreserved embryos transfer and multiple pregnancy rates. There were significant effects of woman's age, number of embryos transferred and stage of embryos upon multiple pregnancy rates. When the same number of cleavage-stage embryos was transferred to women with the same age, twin pregnancy rate or triplet pregnancy rate was the same between FET and fresh cycles. Triplet pregnancy rate with three embryos transferred was significantly higher than that of two embryos transferred. In women under 35 years old, the pregnancy rate with two embryos transferred reached 36.1%.CONCLUSION:Frozen-thawed embryos or fresh embryos transfer has no effect upon the multiple pregnancy rate. Women under 35 years old can achieve acceptable pregnancy rates when two cryopreserved embryos are transferred. It is helpful to reduce the triplet pregnancy rate.
Analysis of the Clinical Characteristics and Outcome of IVF Cycles in Advanced Age Women

Abstract:Objective: To analyze the clinical characteristics and outcome of IVF initiated in women in different age groups,expecting to provide suitable suggestion for treatment.Methods: 5592 fresh non-donor cycles performed from January 2003 to January 2010 in the Reproductive medical center of Nanfang hospital were divided into five groups by female age: 35(35 years old group,n=3915),35~37(35-years old group,n=994),38~40(38-years old group,n=437),41~42(41-years old group,n=153),and≥43 years(≥43 years old group,n=93).The clinical characteristics and outcome were compared and analyzed.Results: Of the 5592 cycles,30% were done in women equal or older than 35 years old,and 9.8% were done in patients older than 38 years.The cancellation rate was 6.1%,7.0%,12.1%,17.6%,and 20.4% in five groups respectively.Of the 5182 transfer cycles,the clinical pregnancy rate in five groups was 46.7%,41.1%,23.4%,19.0%,and 8.1% respectively,the implantation rate was 29.2%,22.3%,11.4%,9.2%%,and 4.5% respectively,the early miscarriage rate was 7.3%,11.6%,23.3%,20.8%,and 33.3% respectively,and the ongoing pregnancy rate of these five groups was 39.9%,31.8%,16.7%,13.5%,and 4.1%.The basal FSH and Gn increased with the female age,while AFC,the number of oocytes retrieved reduced.All of above indicators have significant difference(P0.000)among five groups.The transferable embryos of women whose age was 38 or older declined.The early miscarriage rate of 35 years old group of patients were approximately 2 times greater than 35 years old patients group.Furthermore,≥38 years old patients' early miscarriage rate were 2 times or above greater than those patients whose age was≤37,Conclusions: Female age should be considered as one of the indications of IVF/ICSI.The women≥35 should not be confined to undergo IVF/ICSI,and the women aged 38 or older should be treatment actively even if there were no reasons for their infertility.While,women in 43 years or older should be properly counseled and informed about poor clinical pregnancy rate and about high miscarriage rate.

Abstract:Objective: To explore the choice of embryo transfer schemes for patients with low oocyte retrieval (≤ 3 oocytes). Methods: A retrospective analysis was conducted on patients with oocyte yields ≤ 3 undergoing in vitro fertilization and embryo transfer (IVF-ET) and frozen embryo transfer (FET) at the Maternity & Child Care Center of Qinhuangdao Reproductive Medicine Department from January 2018 to December 2022. The data included 202 fresh cycles, with 104 cycles in Group-A and 98 cycles in Group-B. Additionally, 87 cycles involved the transfer of frozen embryos from fresh cycles that could not be transplanted for various reasons, with 31 cycles in Group-C and 56 cycles in Group-D. General patient information, embryo transfer details, and clinical pregnancy outcomes in both fresh and frozen cycles were statistically analyzed. Results: No significant differences were observed between Groups A and C in age, anti-Müllerian hormone (AMH), basal follicle-stimulating hormone (bFSH), body mass index (BMI), duration of infertility, the proportion of patients with diminished ovarian reserve (DOR), oocyte retrieval count, usable embryo count, or the number of transplanted embryos (all p> 0.05). Advanced age was a risk factor for a decreased pregnancy rate, and FET significantly increased the pregnancy rate (p< 0.05, respectively). Conclusion: For patients under 35 years old with low oocyte retrieval, it is recommended to freeze all embryos when available and then proceed with FET. Abstract:Objective: This study aims to provide an evidence-based assessment of the benefits and harms of four transplantation methods so as to improve the superior pregnancy outcome and reduce the pregnancy rate of multiple embryos in the Mongoloid race. Design: Retrospective analysis Setting: Double Reproductive Centers of Medical University Population or Sample: 24,422 frozen-thawed embryo transfer (FET) cycles recorded from January 2015 to May 2018. Methods：A retrospective cohort study Main Outcome Measures: Biochemical pregnancy, clinical pregnancy, monozygotic twins, miscarriage, maternal complications, live birth. Results: Of the four groups, the biochemical pregnancy rate, the clinical pregnancy rate, and the live birth rate were the lowest in the C-1 group while they were the highest in the B-2 group. However, the B-2 group was accompanied with higher risks of miscarriage, maternal complications, twin births, preterm births and low birth weights. The biochemical pregnancy rates, the clinical pregnancy rates and the live birth rates in the C-2 group and B-1 group were statistically different, with different risks for monozygotic twins, miscarriages. Conclusion: Single blastocyst transplantation seems to be the best choice for all maternal ages to greatly reduce adverse neonatal outcomes. Abstract:Are there differences in pregnancy outcomes when comparing frozen embryo transfer versus fresh embryo transfer among aged women? Frozen embryo transfer yields lower live birth rate for women aged 35-39, but higher rate for women ≥40 compared to fresh embryo transfer. Advanced maternal age was defined as over 35 years old, while recent literature lifted the threshold to 40. An increasing number of women are postponing childbirth, but reproductive issues become more common after the age of 35, with a significant decline in fertility, particularly after 40, even with IVF. Advanced age affects pregnancy rates, prompting some clinicians to prefer fresh embryo transfer to achieve pregnancy quickly. However, it may be less effective with potential negative effects of ovarian stimulation on endometrial receptivity. Frozen embryo transfer is seen as a better strategy, but data specific to advanced maternal age is lacking. This cohort study analyzed the Human Fertilization and Embryology Authority anonymized data from 2011 to 2018 in the United Kingdom. This comprised of a total of 232,942 cycles with advanced maternal age, in which 65,066 patients undergoing frozen embryo transfer cycles, and 167,876 patients undergoing fresh embryo transfer cycles. Data on women with advanced maternal age undergoing IVF/ICSI cycles were analyzed to compare live birth rate and miscarriage rate. The cycles with patients’ age <35, donor oocyte / embryo, PGT, or without embryo to transfer were excluded. Log-binominal regression analysis was performed adjusting for patient age, cause of infertility, number of embryos transferred, year of treatment, and IVF or ICSI cycle. We analyzed data from 232,942 cycles with patients age ≥35, and found that FET cycles exhibited a higher live birth rate than that of fresh embryo transfer cycles (25.5% vs. 24.9%, RR, 1.03; 95% CI, 1.01–1.123). Miscarriage rate (4.6% vs. 3.3%, RR, 1.41; 95% CI, 1.35–1.48) was also different. However, the adjusted RR (aRR) of live birth rate was 0.91; 95% CI, 0.88–0.93, after adjusting for potential confounders. For those patients aged 35-39, live birth rate (28.4% vs. 29.7%, aRR, 0.86; 95% CI, 0.83–0.88) following FET cycles was lower than that of fresh embryo transfer cycles. While FET cycles showed a higher rate of miscarriage (4.6% vs. 3.2%, aRR, 1.83; 95% CI, 1.70–1.98). When patients age≥40, FET cycles showed a higher rate of live birth (18.2% vs. 14.0%, aRR, 1.24; 95% CI, 1.16–1.32) and miscarriage rate (4.7% vs. 3.4%, aRR, 1.72; 95% CI, 1.52–1.94). Besides the live birth rate was sharply decreased than patients aged 35-39 both in FET and fresh embryo transfer cycles. The conclusion of the present study might be limited by the anonymous nature of the dataset. Further randomized trials may be required to provide direct evidence for the best embryo transfer strategy for the advanced age women. The study shows that FET exhibited a higher live birth rate in women aged ≥ 40, but lower for those aged 35-39 than fresh embryo transfer. Additionally, FET demonstrated a higher miscarriage rate. Therefore, it is necessary to tailor the embryo transfer strategy for advanced maternal age women. not applicable
