Abstract:Backgrounds: Although worldwide evidence tends to prove that diabetes adversely affects periodontal health, there are insufficient clues indicating whether periodontitis may aggravate metabolic control and systemic inflammation. This study, as a preliminary part of an ongoing research project, aims to clarify the relationship of periodontal parameters with metabolic levels and systemic inflammatory markers in patients with diabetes.Methods: A total of 140 qualified, adult patients with type 2 diabetes and periodontitis were recruited into this study. Periodontal examinations, including a full-mouth assessment of probing depths (PDs), bleeding on probing, gingival recession, and clinical attachment level, were determined. Blood analyses were carried out for glycated hemoglobin (HbA1c), fasting glucose, high-sensitivity C-reactive protein (hsCRP), tumor necrosis factor-alpha (TNF-alpha), and lipid profiles. Subjects were divided into three groups according to tertiles of the mean PD and compared.Results: Upon an analysis of covariance, subjects with an increased mean PD had significantly higher levels of HbA1c and hsCRP (P <0.05). No significant difference was found among different groups in the levels of serum TNF-alpha, fasting glucose, and lipid profiles (P >0.05). After controlling for age, gender, body mass index, duration of diabetes mellitus, smoking, regular physical exercise, and alcohol consumption, positive correlations were found between mean PD and HbA1c (r = 0.2272; P= 0.009) and between mean PD and hsCRP (r = 0.2336; P= 0.007). After adjustment for possible confounders, the mean PD emerged as a significant predictor variable for elevated levels of HbA1c and hsCRP (P<0.05).Conclusion: Chronic periodontitis was associated with glycemic metabolic and serum hsCRP levels in patients with type 2 diabetes. Abstract:Background: Scientific evidence on the effects of chronic periodontitis on diabetes mellitus remains inadequate and inconclusive. This intervention study is designed to evaluate the effects of periodontal treatment on clinical response, systemic inflammatory parameters, and metabolic control in patients with Type 2 diabetes. Methods: A total of 134 patients were randomly allocated into two treatment groups and one control group. Treatment group 1 underwent non-surgical periodontal treatment at baseline and additional subgingival debridement at the 3-month follow-up. Patients in treatment group 2 received non-surgical periodontal treatment and supragingival prophylaxis at the 3-month follow-up, and those in the control group received no intervention throughout the study. All participants were reexamined at 1.5, 3, and 6 months after initial treatment. At each visit, clinical periodontal examinations were conducted and blood samples were taken to evaluate high-sensitivity C-reactive protein (hsCRP), tumor necrosis factor-a (TNF-alpha), glycated hemoglobin (HbA1c), fasting plasma glucose (FPG), and lipid profiles. Results: Both treatment groups had a significantly lower hsCRP level after periodontal therapy (P < 0.05). Although HbA1c declined significantly in treatment group 2 (P <0.05), the intergroup difference for HbA1c, FPG, TNF-alpha, and lipid profiles was not statistically significant after therapy (P > 0.05). Conclusions: Non-surgical periodontal treatment can effectively improve periodontal and circulating inflammatory status. Despite a lack of strong evidence, trends in some results support improved glycemic control after periodontal treatment in patients with diabetes. J Periodontol 2012;83:435-443.

Abstract:Studies have shown that there is a possible correlation between the amount of glycated hemoglobin and the periodontal status. The goal of this study was to investigate the relationship between glycated hemoglobin (HbA1c) and the prevalence of gingival pathogens and circulating interleukin levels in type II diabetic Tunisian subjects.The research included four groups; 30 healthy subjects (H group), 30 non-diabetic subjects suffering from chronic periodontitis (CP group). Type-II diabetic patients were divided according to HbA1c level into 30 adequately-controlled type-II diabetes subjects (HbA1c ≤ 7 percent (ATIID&CP group)) and 30 inadequately-controlled type-II diabetes subjects and HbA1c > 7 percent (ITIID&CP group). Clinical periodontal condition parameters and assessment of salivary interleukin IL-1beta, IL-6 and IL-10 were assessed. Quantitative Polymerase Chain Reaction used for detection of Subgingival biofilm of periodontal pathogens.Clinical parameters analyzed were positively associated with HbA1c levels (p < 0.05). A. Actinomycetemcomitans were found in 80 percent of ITIID&CP, 65 percent of CP and almost absent in H group. Porphyromonas gingivalis was present in 100 percent of CP, 85 percent of ITIID&CP, 50 percent of ATIID&CP and 3 percent of H group. T. Denticola had an equivalent occurrence. While Tannerella forsythia was scarce in ITIID&CP groups, but abundant in the H group. ITIID&CP had the highest IL-6 and IL-1beta/IL-10 ratios.HBA1c levels affect periodontal status, pathogens and salivary interleukins in Type-II diabetic Tunisians with chronic periodontitis, compared with stable and chronic periodontitis groups and can interact with periodontal infections and increase the inflammatory state.

Abstract:Elderly patients with type 2 diabetes mellitus (T2DM) exhibit considerable periodontitis frequency, which causes tooth loss and poor quality of life. To investigate the impact of periodontitis on gut microbiota, we used 16S rRNA amplicon sequencing to characterize the composition and structure of gut microbiota among elderly patients with T2DM and periodontitis (T2DM_P), elderly patients with T2DM alone (T2DM_NP), and healthy volunteers. We identified 34 key gut microbiota markers that distinguished participants with different periodontal conditions and investigated their connections to other gut bacteria, as well as their clinical correlates. The most striking differences in co-occurrence networks between the T2DM_P and T2DM_NP groups comprised interactions involving dominant genera in the oral cavity (i.e., Streptococcus and Veillonella). Of the 34 identified key gut microbiota markers that distinguished participants with different periodontal conditions, 25 taxa were correlated with duration of diabetes, dry mouth or the peripheral levels of pro-inflammatory cytokines (e.g., tumor necrosis factor-α, interferon-γ, prostaglandin E2, interleukin-17, and interleukin-6) and metabolic parameters (e.g., hemoglobin A1c), respectively. Our findings suggest that gut microbial shifts driven by periodontitis may contribute to systemic inflammation and metabolic dysfunction during the progression of T2DM.

Abstract:Dans cette these nous nous interessons a des problemes d’apprentissage automatique dans le cadre de sorties structurees avec une structure sequentielle. D’une part, nous considerons le probleme de l’apprentissage de mesure de similarite pour deux tâches : (i) la detection de rupture dans des signaux multivaries et (ii) le probleme de deformation temporelle entre paires de signaux. Les methodes generalement utilisees pour resoudre ces deux problemes dependent fortement d’une mesure de similarite. Nous apprenons une mesure de similarite a partir de donnees totalement etiquetees. Nous presentons des algorithmes usuels de prediction structure, efficaces pour effectuer l’apprentissage. Nous validons notre approche sur des donnees reelles venant de divers domaines. D’autre part, nous nous interessons au probleme de la faible supervision pour la tâche d’alignement d’un enregistrement audio sur la partition jouee. Nous considerons la partition comme une representation symbolique donnant (i) une information complete sur l’ordre des symboles et (ii) une information approximative sur la forme de l’alignement attendu. Nous apprenons un classifieur pour chaque symbole avec ces informations. Nous developpons une methode d’apprentissage fondee sur l’optimisation d’une fonction convexe. Nous demontrons la validite de l’approche sur des donnees musicales.
Association between periodontal and systemic inflammation : a study of pro- and anti-inflammatory mediators

Abstract:The principal aim of this study was to explore associations between systemic inflammatory status and periodontal inflammation and tissue destruction. The study population consisted of 61 patients with chronic periodontitis, 30 periodontally healthy control subjects, and 80 subjects with type 1 diabetes mellitus (T1DM). The T1DM subjects were periodontally treated and re-examined eight weeks after completion of the treatment. The periodontal measures included plaque, probing depth (PD), bleeding on probing (BOP) and attachment level (AL). The serum levels of interleukin (IL)6, tumor necrosis factor (TNF)-α, and interleukin (IL)-10, as well as the gingival crevicular fluid (GCF) level of matrix metalloproteinase (MMP)-8 were analyzed using commercially available ELISA assays, and serum high density lipoprotein (HDL) level using direct enzymatic methods. Serum IL-6 level associated significantly with the extent of inflamed periodontal pockets in T1DM subjects. Moreover, serum IL-6 modulated local periodontal inflammation in T1DM patients; periodontal healing turned out to be poorer in subjects with a high level of serum IL-6 than in those with a low level. Serum TNF-α/IL-10 ratio was three times higher in chronic periodontitis patients than in periodontally healthy control subjects. In T1DM subjects a significant inverse association between serum HDL level and the extent of inflamed periodontal pockets was found; subjects with a low serum HDL level presented 50% more inflamed periodontal sites than subjects with a high serum HDL level. A significant association between GCF MMP-8 level in shallow crevices and the extent of periodontal attachment loss in chronic periodontitis patients was observed. In conclusion, we focused on analyzing associations between systemic inflammatory status and periodontal conditions. According to our results, periodontal inflammation/infection is associated with systemic inflammatory status using serum IL-6, TNF-α/IL-10 ratio and HDL as indicators. Also, a high level of MMP-8 in GCF in shallow crevices could be indicative of higher susceptibility to periodontal infection and tissue destruction.

Abstract:Introduction: Periodontitis, a chronic inflammatory disease associated with dental biofilm, poses a significant threat to oral health. This study explores the roles of interleukin 1β (IL-1β) and nesfatin-1 in periodontal diseases, aiming to contribute to the molecular understanding of their pathogenesis. Material and methods: A diverse cohort of 62 participants was recruited, spanning ages 20 to 60, and categorized into healthy, gingivitis, and periodontitis groups. Clinical measurements, including plaque index, gingival index, probing pocket depth, and bleeding on probing, were conducted. Gingival crevicular fluid (GCF) samples were collected for IL-1β and nesfatin-1 analysis using enzyme-linked immunosorbent assay (ELISA). Statistical analysis employed Kruskal-Wallis and Spearman correlation tests. Results: Significant differences in oral hygiene habits were observed among groups, particularly in the 40-60 age range. Clinical indices showed variations, with the highest IL-1β levels in the periodontitis group and the lowest nesfatin-1 levels. Correlation analysis revealed positive associations between IL-1β, nesfatin-1, and oral indices. Conclusions: While providing valuable insights, we acknowledge this study's limitations, including a cross-sectional design and a specific age range. Future research should employ longitudinal designs and larger cohorts, and explore broader inflammatory markers, genetic influences, and confounding variables for a more comprehensive understanding of periodontal diseases. The findings underscore the complex interplay between inflammatory markers and periodontal health.

Abstract:Background: The two-way relationship between periodontitis and type 2 diabetes mellitus (T2D) is well established. An inflammatory link exists between diabetic retinopathy (DR) and periodontitis. This study was undertaken to compare the prevalence, extent and severity of periodontitis, to correlate clinical attachment level (CAL) and periodontal inflamed surface area (PISA) with glycemic status, serum IL-6 and lipoprotein a [Lp (a)] in T2D subjects with and without DR. Method: This cross-sectional study comprised 40 T2D subjects with DR and 40 T2D subjects without DR. All were assessed for periodontal parameters (BOP, PPD, CAL, OHI-S Index, PI Index and PISA) and systemic parameters (HbA1c, FBG, PPBG, FLP, serum IL-6 and serum Lp (a)). Results: The Proportion, Extent and severity of periodontitis and PISA were higher in T2D with DR compared to T2D without DR. T2D with DR subjects had a mean of 69% sites with gingival bleeding and 40% in T2D without DR. HbA1c was positively correlated with CAL (r = 0.123, p=0.001) and PISA (r = 0.154, p=< 0.001) in T2D with DR. A positive correlation was found between PISA and IL6 (Pearson correlation coefficient 0.651, p < 0.0001); PISA and Lp (a) (Pearson correlation coefficient 0.59, p < 0.001); CAL and IL6 (Pearson correlation coefficient 0.527, p < 0.0001) and CAL and Lp (a) (Pearson correlation coefficient 0.631, p < 0.001). Serum IL-6 and Lp (a) are higher in retinopathy patients with periodontitis than those without periodontitis. Conclusion: Despite both groups having poor glycemic control and comparable plaque scores, the periodontal and inflammatory parameters were higher in the DR group compared to T2D without the DR. The presence of diabetic retinopathy may have contributed to the severity of periodontal destruction and periodontitis may have influenced the progression of diabetic retinopathy. Abstract:Susceptibility to periodontal disease depends on individual factors within the host response to the bacterial challenge. The study of these factors requires longitudinal studies of an undisturbed development of the disease process. On the basis of the original longitudinal studies on the natural histology of periodontal disease staged in Sri Lanka and Oslo/Norway, several analyses of periodontal parameters and tooth status have been performed. The main findings were that in the first 20 years of complete absence of oral hygiene practices or preventive services attachment was lost at various rates. Three groups of subjects could be identified: rapidly progressing (RP) (8%), moderately progressing (MP) (81%), and subjects with no disease progression (NP) (11%). In the second two decades, the RP subjects have lost most of their teeth and no NP patients were identified anymore. The progression rate in these two decades was much slower, and the tooth mortality decreased. It could be predicted that subjects who had lost more than 2 mm at age 30 would not maintain a functional dentition at age 60. The corresponding control population in Oslo was used to study the influence of gingival inflammation on the initiation and progression of periodontal disease. The pattern and rates of attachment loss were identified in a population that was exposed to optimal and regular preventive services from age 3 onward. In the observation period of 26 years, it could be demonstrated that gingival inflammation varied little throughout adult life and always bleeding sites occurred consistently in 10% to 20%. The role of ongoing gingivitis in the pathogenesis of attachment loss was identified and also reflected stability whenever it was absent. Tooth mortality was only found in subjects with ongoing gingivitis. After 50 years of tooth age, 63% of the teeth were still maintained, while 99.8% were maintained after 50 years when gingival inflammation had been absent. Consequently, continuous gingivitis represented a risk factor for tooth loss.

Abstract:Chronic periodontitis is a common complication in diabetes. The aim of this study was to evaluate some clinical and microbiological parameters in controlled and uncontrolled type 2 diabetes mellitus (type 2 DM) patients compared to non-diabetic (NDM) individuals, as well as to assess the effect of non-surgical periodontal therapy on these parameters. The study was performed in 61 type 2 DM patients with periodontitis (group 1A: 29 patients having achieved good metabolic control, HbA1c <7%; group 1B: 32 patients with poor metabolic control, HbA1c ≥7%), and 31 NDM individuals suffering from periodontitis. Periodontal indices (plaque index, PI; gingival index, GI; probing pocket depth, PPD; and clinical attachment level, CAL) were measured and subgingival plaque samples were analyzed using polymerase chain reaction prior to treatment initiation and 3 months post-treatment. The results recorded on the majority of measured parameters indicated that differences in treatment success achieved in the three treatment groups were not statistically significant (∆PI p=0.646; ∆GI p=0.303; and ∆CAL p=0.233). Likewise, comparison of the effectiveness in microorganism reduction revealed no significant differences between DM groups and NDM patients. Therefore, study results supported the hypothesis that periodontal therapy outcome was unaffected by the level of glycemic control in patients with diabetes.

Abstract:The salivary microbiome in Type 2 diabetes mellitus patients with/without periodontitis was examined. Knowledge of key oral biological features could help address the observed poorer clinical presentation in T2DM. Specific members of the oral microbiome related directly to the clinical disease features and/or salivary biomolecules in T2DM individuals with periodontitis. Objective To examine the characteristics of the salivary microbiome in Type 2 diabetes mellitus (T2DM) patients with or without periodontitis. Background Periodontitis has been identified as clear sequelae of T2DM. This chronic oral disease also impacts the management of the clinical features of diabetes. The oral microbiome characteristics in T2DM with and without periodontitis, as well as the response of this oral microbiome to nonsurgical therapy have not been well described. Knowledge of key oral biological features could help address the observed poorer clinical presentation of T2DM patients. Methods The oral microbiome in saliva of adult cohorts periodontally healthy/non‐diabetic (non‐periodontitis; NP; n = 31), T2DM without periodontitis (DWoP; n = 32), and T2DM with periodontitis (DWP; n = 29) were characterized by microbial molecular analysis using V3–V4 sequencing and Luminex or ELISA techniques for salivary host analytes. Results Phyla distribution showed DWP with significantly lower levels of Firmicutes and Actinobacteria and higher levels of Fusobacteria and Spirochetes compared to the healthier groups. Approximately 10% of the detected microbial species showed significant differences in frequency and level of colonization among the DWP, DWoP, and NP samples. A subset of bacteria were significantly correlated with clinical disease features, as well as a specific repertoire of salivary analytes, in particular matrix metalloproteinase (MMP)8/MMP9, interleukin‐1ß, B‐cell activating factor, and resistin differed between the groups and were related to specific taxa. Principal component analysis that identified a majority of the DWP subjects microbiome was unique based upon an array of 27 taxa out of up to 255 detected in the saliva samples. Conclusion T2DM patients with periodontitis show unique oral microbiome and salivary analyte composition compared to diabetics or non‐diabetic persons without periodontitis. Specific members of the oral microbiome relate directly to the clinical disease features and/or salivary biomolecules in T2DM individuals.

Abstract:Objective: This study aims to analyse the association between the baseline microbial load of selected periodontopathogenic bacteria collected from gingival crevicular fluid (GCF) and the primary outcome of steps I and II therapy. Materials and methods: 222 patients with stage III periodontitis were included into this retrospective analysis that received steps 1 and 2 periodontal therapy without adjunctive systemic antibiotics. Baseline GCF samples were quantitatively analysed using ELISA-based kits for levels of periodontopathogens (Porphyromonas gingivalis (Pg), Aggregatibacter actinomycetemcomitans (Aa), Prevotella intermedia (Pi), Fusobacterium nucleatum (Fn), Treponema denticola (Td), and Tannerella forsythia (Tf)) and associated with the primary therapy outcome using a "treat-to-target" therapy endpoint (TE) defined as ≤ 4 sites with PD ≥ 5 mm six months after therapy. Results: 38.2% of the patients achieved TE. Patients failing to achieve TE revealed significantly increased levels of Pg, Fn, and Tf at baseline (Pg: p = 0.010, Fn: p = 0.008 Tf: p = 0.004). Multivariate binary logistic regression adjusted for sex, mean probing depth, diabetes, and current smoking status showed an independent relationship between Tf and the TE (aOR 2.570, p = 0.023). Conclusion: Increased microbial load is associated with decreased responsiveness to therapy. The findings suggest that specifically baseline Tf levels are associated with poorer treatment outcomes and might improve the accuracy of periodontal diagnosis. Clinical relevance: The findings of this study support the concept of a critical biomass that is sufficient to induce and maintain an immune response within the periodontal pocket, which ultimately leads to irreversible tissue destruction. Abstract:We propose an action describing chiral fermions with an arbitrary gauge group and with manifest $(8,0)$ worldsheet supersymmetry. The form of the action is inspired by and adapted for completing the twistor-like formulation of the $D=10$ heterotic superstring.

Abstract:Biofilm of the gingival sulcus from 22 patients with type 2 diabetes mellitus and periodontitis, 30 patients with periodontitis not complicated by diabetes mellitus (reference group), and 22 healthy volunteers without signs of gingival disease (control group) was studied by quantitative PCR. Quantitative analysis for the content of P. gingivalis, T. forsythia, A. ctinomycetemcomitans, T. denticola, P. intermedia, F. nucleatum/periodonticum, and P. endodontalis in the dental plaque was performed with a Dentoscreen kit. The presence of other bacterial groups was verified by metagenomic sequencing of the 16S rRNA gene to evaluate some specific features of the etiological factor for periodontitis in type 2 diabetes mellitus. Specimens of the Porphiromonadaceae and Fusobacteriaceae families were characterized by an extremely high incidence in combined pathology. The amount of Sphingobacteriaceae bacteria in the biofilm was shown to decrease significantly during periodontitis. Metagenomic analysis confirmed the pathogenic role of microbiota in combined pathology, as well as the hypothesis on a possible influence of periodontitis on the course and development of type 2 diabetes mellitus.
