Abstract:Tai Chi: Moving for Better Balance (TCMBB) is an evidence-based fall prevention exercise program being disseminated in selected communities through state injury prevention programs. This study: (1) describes the personal characteristics of TCMBB participants; (2) quantifies participants’ functional and self-reported health status at enrollment; and (3) measures changes in participants’ functional and self-reported health status post-intervention. There were 421 participants enrolled in 36 TCMBB programs delivered in Colorado, New York, and Oregon. Of the 209 participants who completed both baseline enrollment and post-intervention surveys, the average age of participants was 75.3 (SD ± 8.2) years. Most participants were female (81.3%), non-Hispanic (96.1%), White (94.1%), and described themselves as in excellent or very good health (52.2%). Paired t-test and general estimating equation models assessed changes over the 3-month program period. Pre- and post-assessment self-reported surveys and objective functional data [Timed Up and Go (TUG) test] were collected. On average, TUG test scores decreased (p < 0.001) for all participants; however, the decrease was most noticeable among high-risk participants (mean decreased from 18.5 to 15.7 s). The adjusted odds ratio of reporting feeling confident that a participant could keep themselves from falling was five times greater after completing the program. TCMBB, which addresses gait and balance problems, can be an effective way to reduce falls among the older adult population. By helping older adults maintain their functional abilities, TCMBB can help community-dwelling older adults continue to live independently.
[A Case-Crossover Study on Air Pollutants and the Mortality of Stroke].

Abstract:OBJECTIVE:To investigate the short-term effect of particulate matter in air on the mortality of stroke.METHODS:Using time-stratified case-crossover study design, an association was examined between stroke mortality and particulate matter with aerodynamic diameter of < 10 microm (PM10) of 2002 - 2004 in Hangzhou city. Meanwhile, the acute health effect of other gaseous pollutants (sulfur dioxide, SO2 and nitrogen dioxide, NO2) was also analyzed.RESULTS:A total of 9906 deaths of stroke were included. The crude stroke mortality was 83.54 per 100 000. After being adjusted for meteorological factors, when an increase of 10 microg/m3 in PM10, SO2 and NO2 in three days was noticed, it appeared that the increases of mortality of stroke were 0.56% (95% CI: 0.14%-0.99%), 1.62% (95% CI: 0.26% - 3.01%) and 2.07% (95% CI: 0.54% - 3.62%) respectively. There was no distinct association in multi-pollutant models. In sensitivity analysis, the associations were found in all single-pollutant models but not statistically significant in multi-pollutant models after replacing the missing values.CONCLUSION:It is suggested that the short-term elevation in PM10 as well as SO2 and NO2 daily concentrations were related to the increase of stroke mortality in Hangzhou city.
Air Pollution Associated With Incident Stroke, Poststroke Cardiovascular Events, and Death

Abstract:Ambient air pollution has been widely linked with morbidity and mortality of stroke. However, its effects on dynamic progression trajectory of stroke remain unknown.We investigated the effects of ambient air pollution on progression trajectory from healthy status to incident stroke, poststroke cardiovascular diseases, and subsequent death.We retrieved 318,752 participants from the UK Biobank. The annual concentrations of air pollution [particulate matter (PM2.5, PMcoarse, PM10 and PM2.5 absorbance), nitrogen dioxide (NO2), and nitrogen oxides (NOx)] were estimated through land use regressions. A multistate regression model was used to investigate the effects of air pollution on each stage of the progression of stroke.During 3,765,630 person-years of follow-up, we identified 5,967 incident stroke patients, 2,985 poststroke cardiovascular patients, and 1,020 deaths afterward. Each 5 μg/m3 increase in PM2.5, NO2, and NOx was associated with the transition from healthy status to incident stroke [hazard ratios (HRs) (95% confidence intervals (CI)) = 1.24 (1.10, 1.40); 1.02 (1.00, 1.03); 1.01 (1.00, 1.02)] and transitions from healthy status to death directly [1.30 (1.21, 1.40); 1.03 (1.02, 1.04); and 1.02 (1.01, 1.02)]. We also observed positive associations for poststroke CVDs, especially for NO2 [1.04 (95% CI: 1.02, 1.06)], but these effects gradually declined for mortality outcome.This study provides the first evidence that ambient air pollution is one important factor associated with the progression of stroke, and the effects differed across different clinical stages. A better understanding of the differential effects of air pollutants on different stroke transition stages could provide valuable insight toward targets for health management and clinical prevention.
Ambient Air Pollution and the Dynamic Transitions of Stroke and Dementia: a Population-Based Cohort Study

Abstract:Background Stroke and dementia are the leading causes of neurological disease burden. Detrimental effects of air pollution on both conditions are increasingly recognised, while the impacts on the dynamic transitions have not yet been explored, and whether critical time intervals exist is unknown. Methods This prospective study was conducted based on the UK Biobank. Annual average air pollution concentrations at baseline year 2010 estimated by land-use regression models were used as a proxy for long-term air pollution exposure. Associations between multiple air pollutants (PM2.5, PM2.5-10, and NO2) indicated by air pollution score and the dynamic transitions of stroke and dementia were estimated, and the impacts during critical time intervals were explored. The date cutoff of this study was February 29, 2020. Findings During a median follow-up of 10.9 years in 413,372 participants, 6484, 3813, and 376 participants developed incident stroke, dementia, and comorbidity of stroke and dementia. For the overall transition from stroke to comorbid dementia, the hazard ratio (HR) for each interquartile range (IQR) increase in air pollution score was 1.38 (95% CI, 1.15, 1.65), and the risks were limited to two time intervals (within 1 year and over 5 years after stroke). As for the transition from dementia to comorbid stroke, increased risk was only observed during 2-3 years after dementia. Interpretation Our findings suggested that air pollution played an important role in the dynamic transition of stroke and dementia even at concentrations below the current criteria. The findings provided new evidence for alleviating the disease burden of neurological disorders related to air pollution during critical time intervals.Copyright (c) 2023 The Author(s). Short-term effects of outdoor air pollution on acute ischaemic stroke occurrence: a case-crossover study in Tianjin, China

Abstract:Objective Ambient air pollution is associated with ischaemic stroke incidence. However, most of the previous studies used stroke-related hospital admission rather than stroke onset itself. This study aimed to evaluate the relationship between ambient air pollutant exposures and acute ischaemic stroke based on the timing of symptom onset. Methods A time-stratified, case-crossover analysis was performed among 520 patients who had ischaemic stroke admitted to the Second Hospital of Tianjin Medical University (Tianjin, China) between 1 April 2018 and 31 March 2019 (365 days). Daily air pollutant concentrations of particulate matter with aerodynamic diameter 2.5 μm, particulate matter with aerodynamic diameter 10 μm (PM10), sulfur dioxide, nitrogen dioxide, carbon monoxide and ozone were obtained from fixed-site monitoring stations. We used conditional logistic regression to estimate OR and 95% CI corresponding to an increase in IQR of each air pollutant after adjusting for the effects of temperature and relative humidity. Results Overall, a higher risk of ischaemic stroke was found between April and September. During this period PM10 was associated with an increased risk of ischaemic stroke (1-day lag: OR=1.49, 95% CI 1.09 to 2.02; 3-day mean: OR=1.58, 95% CI 1.09 to 2.29) among patients between 34 and 70 years old. Positive associations were also observed between PM10 (1-day lag: OR=1.51, 95% CI 1.10 to 2.07; 3-day mean: OR=1.57, 95% CI 1.08 to 2.29), ozone (1-day lag: OR=1.83, 95% CI 1.16 to 2.87; 3-day mean: OR=1.90, 95% CI 1.06 to 3.42) and ischaemic stroke occurrence among those with hyperlipidaemia. Conclusion Our results suggest that air pollution is associated with a higher risk of ischaemic stroke in younger people or people with hyperlipidemia. These findings still need to be further investigated.

Long-term Air Pollution Exposure and Incident Stroke in American Older Adults: A National Cohort Study

Abstract:Aims:Stroke is a leading cause of death and disability for Americans, and growing evidence suggests that air pollution may play an important role. To facilitate pollution control efforts, the National Academy of Sciences and the World Health Organization have prioritized determining which air pollutants are most toxic. However, evidence is limited for the simultaneous effects of multiple air pollutants on stroke.Methods and results:We constructed a nationwide population-based cohort study, using the Medicare Chronic Conditions Warehouse (2000-2017) and high-resolution air pollution data, to investigate the impact of long-term exposure to ambient PM2.5, NO2, and ground-level O3 on incident stroke. Hazard ratios (HR) for stroke incidence were estimated using single-, bi-, and tri-pollutant Cox proportional hazards models. We identified ~2.2 million incident stroke cases among 17,443,900 fee-for-service Medicare beneficiaries. Per interquartile range (IQR) increase in the annual average PM2.5 (3.7 μg/m3), NO2 (12.4 ppb), and warm-season O3 (6.5 ppb) one-year prior to diagnosis, the HRs were 1.022 (95% CI: 1.017-1.028), 1.060 (95% CI: 1.054-1.065), and 1.021 (95% CI: 1.017-1.024), respectively, from the tri-pollutant model. There was strong evidence of linearity in concentration-response relationships for all three air pollutants in single-pollutant models. This linear relationship remained robust for NO2 and O3 in tri-pollutant models while the effect of PM2.5 attenuated at the lower end of concentrations.Conclusion:Using a large nationwide cohort, our study suggests that long-term exposure to PM2.5, NO2, and O3 may independently increase the risk of stroke among the US elderly, among which traffic-related air pollution plays a particularly crucial role.

Abstract:Background and Purpose-There have been no multicity studies on the acute effects of air pollution on stroke mortality in China. This study was undertaken to examine the associations between daily stroke mortality and outdoor air pollution (particulate matter <10 mu m in aerodynamic diameter, sulfur dioxide, and nitrogen dioxide) in 8 Chinese cities.Methods-We used Poisson regression models with natural spline-smoothing functions to adjust for long-term and seasonal trends, as well as other time-varying covariates. We applied 2-stage Bayesian hierarchical statistical models to estimate city-specific and national average associations of air pollution with daily stroke mortality.Results-Air pollution was associated with daily stroke mortality in 8 Chinese cities. In the combined analysis, an increase of 10 mu g/m(3) of 2-day moving average concentrations of particulate matter <10 mu m in aerodynamic diameter, sulfur dioxide, and nitrogen dioxide corresponded to 0.54% (95% posterior intervals, 0.28-0.81), 0.88% (95% posterior intervals, 0.54-1.22), and 1.47% (95% posterior intervals, 0.88-2.06) increase of stroke mortality, respectively. The concentration-response curves indicated linear nonthreshold associations between air pollution and risk of stroke mortality.Conclusions-To our knowledge, this is the first multicity study in China, or even in other developing countries, to report the acute effect of air pollution on stroke mortality. Our results contribute to very limited data on the effect of air pollution on stroke for high-exposure settings typical in developing countries. (Stroke. 2013;44:954-960.)
Mixtures of long-term exposure to ambient air pollution, built environment and temperature and stroke incidence across Europe

Abstract:Introduction: The complex interplay of multiple environmental factors and cardiovascular has scarcely been studied. Within the EXPANSE project, we evaluated the association between long-term exposure to multiple environmental indices and stroke incidence across Europe. Methods: Participants from three traditional adult cohorts (Germany, Netherlands and Sweden) and four administrative cohorts (Catalonia [region Spain], Rome [city-wide], Greece and Sweden [nationwide]) were followed until incident stroke, death, migration, loss of follow-up or study end. We estimated exposures at residential addresses from different exposure domains: air pollution (nitrogen dioxide (NO2), particulate matter < 2.5 μm (PM2.5), black carbon (BC), ozone), built environment (green/blue spaces, impervious surfaces) and meteorology (seasonal mean and standard deviation of temperatures). Associations between environmental exposures and stroke were estimated in single and multiple-exposure Cox proportional hazard models, and Principal Component (PC) Analyses derived prototypes for specific exposures domains. We carried out random effects meta-analyses by cohort type. Results: In over 15 million participants, increased levels of NO2 and BC were associated with increased higher stroke incidence in both cohort types. Increased Normalized Difference Vegetation Index (NDVI) was associated with a lower stroke incidence in both cohort types, whereas an increase in impervious surface was associated with an increase in stroke incidence. The first PC of the air pollution domain (PM2.5, NO2 and BC) was associated with an increase in stroke incidence. For the built environment, higher levels of NDVI and lower levels of impervious surfaces were associated with a protective effect [%change in HR per 1 unit = -2.0 (95 %CI, -5.9;2.0) and -1.1(95 %CI, -2.0; -0.3) for traditional adult and administrative cohorts, respectively]. No clear patterns were observed for distance to blue spaces or temperature parameters. Conclusions: We observed increased HRs for stroke with exposure to PM2.5, NO2 and BC, lower levels of greenness and higher impervious surface in single and combined exposure models.
Outdoor air pollution, mortality, and hospital admissions from coronary heart disease in Sheffield, UK: a small-area level ecological study

Abstract:Aims: To examine the hypothesis that coronary heart disease mortality and emergency hospital admission rates are higher in areas with higher outdoor air pollution levels. Methods and results: Modelled nitrogen oxides (NO(x)), particulate matter (PM(10)), and carbon monoxide (CO) levels were interpolated to 1030 census enumeration districts using an ecological study design. Results, based on 6857 deaths and 11,407 admissions from 1994-98 and a population of 199,682 aged >or=45 years, were adjusted for age, sex, deprivation, and smoking prevalence. Mortality rate ratios were 1.17 (95% CI 1.06-1.29), 1.08 (95% CI 0.96-1.20), and 1.05 (95% CI 0.95-1.16) in the highest relative to the lowest NO(x), PM(10), and CO quintile categories, respectively. Corresponding admission rate ratios were 1.00 (95% CI 0.90-1.10), 1.01 (95% CI 0.90-1.14), and 0.88 (95% CI 0.79-0.98). Conclusion: The results are consistent with an excess risk of coronary heart disease mortality in areas with high outdoor NO(x), a proxy for traffic-related pollution, but residual confounding cannot be ruled out. If causality were assumed, 6% of coronary heart disease deaths would have been attributable to outdoor NO(x,) and targeting pollution reduction measures at high pollution areas would be an option for coronary mortality prevention.

Abstract:Ten years ago implantable cardioverter-defibrillators (ICDs) were cumbersome, required thoracotomy for placement of the epicardial patches, used only high energy shocks for treatment, and had short battery lives. Current devices are smaller and have lead systems that do not require thoracotomy. They also have facilities for antitachycardia pacing, low energy cardioversion, high energy defibrillation, backup bradycardia pacing, and documentation of the treated arrhythmias-and the batteries last longer. These improvements have added to the appeal of this treatment, and in the United States it is forecast that about 30 000 ICDs will be implanted in 1995.

Abstract:Background: Ambient particulate matter of ≤2.5 μm in diameter (PM2.5) is named as a risk factor for cerebrovascular diseases. This investigation aimed to evaluate the impact of ambient air pollution with PM2.5 on stroke occurrence. Methods: The study was performed in Kaunas, Lithuania, from 2010 to 2022. The daily numbers of ISs, subarachnoid hemorrhages (SAHs), and intracerebral hemorrhages (ICHs) were obtained from the Kaunas Stroke Register. The association between stroke occurrence and PM2.5 exposure was assessed by time- and seasonally stratified Poisson regression. Results: Among middle-aged persons, 3377 had a stroke, of which 2686 (79.5%) had an IS, 469 (13.9%) had an ICH, and 222 (6.6%) had SAH. The relative risk (RR) of SAH was increased by 1.7% with an increase in daily PM2.5 by 1 μg/m3 on the same day and at a lag of 1 day, and by 2.2% with an increase in mean PM2.5 concentration at a lag 0–1 days by 1 μg/m3. The RR of having a SAH was increased by 0.7% with an increase in daily PM2.5 by 1 μg/m3 on the same day. Conclusions: Significant associations between stroke occurrence and air pollution with PM2.5 were found in the SAH and HS patients, and only in middle-aged subjects.

Abstract:BackgroundInconsistent results have been found on the association between air pollution and stroke mortality. Additionally, evidence on people who are potentially sensitive to air pollution-associated stroke mortality is limited.MethodsDaily stroke mortality of adults aged over 65 between 2003 and 2008 in Shanghai, China were collected. The time-stratified case-crossover approach was used to assess the association between daily concentrations of air pollutants including particles with size <10 mu m, sulfur dioxide (SO2) and nitrogen dioxide (NO2) and stroke mortality.ResultsBoth total-stroke and ischaemic-stroke mortalities were found to be significantly associated with all three air pollutants. Haemorrhagic stroke was significantly associated with SO2 and NO2 only. Substantial differences were observed for effect estimates of ischaemic-stroke mortality in relation to NO2 among people with cardiac diseases compared with those without; for an increase of 10g/m(3) in NO2, the increase in ischaemic-stroke mortality was 7.05% (95% CI 1.92% to 12.17%) for people with comorbid cardiac diseases versus 0.60% (95% CI -0.49% to 1.68%) for those without. We did not find evidence of effect modification by hypertension and diabetes.ConclusionsThis study provides new evidence for the association between exposure to ambient air pollution and stroke mortality. Our results also suggest that underlying cardiac disorder may increase the risk for ischaemic-stroke mortality in relation to air pollution exposure, especially NO2. .

Abstract:ISEE-11 Background: Health Effects of Air Pollution on Susceptible Subpopulations (HEAPSS) is an EU-funded study of the effects of air pollution on cardiac health in five European cities characterized by different air quality and climates. It aimed to determine acute effects of air pollution on: 1) fatal non-hospitalized coronary events; 2) hospitalized acute myocardial infarctions (AMI), as well as; 3) re-hospitalizations for a new AMI or any cardiac cause; and 4) late mortality, in AMI survivors (a hypothesized susceptible subpopulation). Methods: Hospital discharge records and AMI registries identified 26,888 AMI patients, 1992-2000. In one city, data on 5,144 out-of-hospital coronary deaths were collected. Mortality and hospital readmissions (for acute myocardial infarction and other cardiac causes) were assessed in follow-ups of 28-day survivors. PM10, CO, NO2, NO, SO2, and O3 were collected for each city from existing networks of fixed monitors. Particle Number Concentration (PNC) was measured for one year (using condensation particle counters) and retrospectively estimated for the study period. City specific time series analyses with Poisson regression were conducted. Results were pooled using fixed or random effects models. Results: Estimated PNC (unit: 10,000 particles/cm3) and CO (unit: 0.2 mg/m3) at immediate lag showed the most consistent results. PNC (2.8% increase, 95% CI = 1.1-4.6%) and CO (1.0% CI = 0.3-1.8%) were strongly associated with out-of-hospital coronary events. Hospitalizations for first AMI were weakly associated with PNC and CO. The associations were stronger in the three cities using hospital discharge records and no upper age limit: PNC was associated with 1.3% (CI = 0.0-2.6%) and CO with 0.7% (CI = 0.1-1.2%) increase in first MI hospitalizations, and the strongest associations were observed among persons aged 65 and over. There was variation between cities with respect to effect modification by age. PNC (2.6%, CI = 0.5-4.8) and CO (1.4%, CI = 0.1-2.6%) were associated with cardiac readmissions among myocardial infarction survivors in the five cities. Air pollution was weakly associated with all-cause mortality in AMI survivors, with considerable heterogeneity between cities. In the two centers using AMI register data, an association was found for both PNC (7.0%, CI = 2.0-12%) and CO (6.0%, CI = 0-12) at lag 0-1. Conclusions: Air pollution from traffic is associated with out-of-hospital coronary deaths, hospitalizations for myocardial infarction, and subsequent cardiac readmissions among AMI survivors. Weak and inconsistent results were found for total mortality. Differences in air pollution levels, age and sex structure, and treatment practices among centers might explain the heterogeneity of the results and require further analysis.

Abstract:Previous studies showed that ambient air pollution is a risk factor for stroke. But its effects on patients with different comorbidity accumulation patterns and the causal relationship remain unclear. Hospitalization data were collected from the Beijing Municipal Health Commission Information Center. Pollutant data, including particulate matter (PM 2.5 , PM 10 , PM 1 , PM 1-2.5 ), sulfur dioxide (SO 2 ), nitrogen dioxide (NO 2 ), carbon monoxide (CO), and ozone (O 3 ) from 2014 to 2017, were obtained from the widely-used ChinaHighAirPollutants dataset. An individual-level case-crossover design was employed to identify case days and control days. The exposure-response association was estimated by fitting conditional logistic regression models. The newly proposed marginal 'between-within' models were used to estimate counterfactual hospitalization probability. In a total of 237 487 stroke patients, 211 741 diagnosed with ischemic stroke (IS) and 25 641 with hemorrhagic stroke (HS). Except for SO 2 , the seven pollutants were associated with higher risk of overall stroke and IS. Particulate matter showed larger estimated effects in patients with hypertension and diabetes but without hyperlipidemia. NO 2 and CO showed larger estimated effects in patients with hypertension but without diabetes. O 3 showed larger estimated effects in patients with hypertension, diabetes, and hyperlipidemia. PM 1 , PM 10 , NO 2 , and CO had statistically significant and persistent causal relationships with stroke risk during the lag periods. This study highlighted the need for targeted interventions of air pollution. Prioritizing control measures for PM 1 , PM 10 , NO 2 , and CO is particularly crucial in stroke prevention efforts.

Association Between Ambient Air Pollution and Daily Hospital Admissions for Ischemic Stroke: A Nationwide Time-Series Analysis

Abstract:BACKGROUND:Evidence of the short-term effects of ambient air pollution on the risk of ischemic stroke in low- and middle-income countries is limited and inconsistent. We aimed to examine the associations between air pollution and daily hospital admissions for ischemic stroke in China. METHODS AND FINDINGS:We identified hospital admissions for ischemic stroke in 2014-2016 from the national database covering up to 0.28 billion people who received Urban Employee Basic Medical Insurance (UEBMI) in China. We examined the associations between air pollution and daily ischemic stroke admission using a two-stage method. Poisson time-series regression models were firstly fitted to estimate the effects of air pollution in each city. Random-effects meta-analyses were then conducted to combine the estimates. Meta-regression models were applied to explore potential effect modifiers. More than 2 million hospital admissions for ischemic stroke were identified in 172 cities in China. In single-pollutant models, increases of 10 μg/m3 in particulate matter with aerodynamic diameter <2.5 μm (PM2.5), sulfur dioxide (SO2), nitrogen dioxide (NO2), and ozone (O3) and 1 mg/m3 in carbon monoxide (CO) concentrations were associated with 0.34% (95% confidence interval [CI], 0.20%-0.48%), 1.37% (1.05%-1.70%), 1.82% (1.45%-2.19%), 0.01% (-0.14%-0.16%), and 3.24% (2.05%-4.43%) increases in hospital admissions for ischemic stroke on the same day, respectively. SO2 and NO2 associations remained significant in two-pollutant models, but not PM2.5 and CO associations. The effect estimates were greater in cities with lower air pollutant levels and higher air temperatures, as well as in elderly subgroups. The main limitation of the present study was the unavailability of data on individual exposure to ambient air pollution. CONCLUSIONS:As the first national study in China to systematically examine the associations between short-term exposure to ambient air pollution and ischemic stroke, our findings indicate that transient increase in air pollution levels may increase the risk of ischemic stroke, which may have significant public health implications for the reduction of ischemic stroke burden in China.
Acute Effects of Air Pollution on Ischaemic Stroke Onset and Deaths: a Time-Series Study in Changzhou, China.

Abstract:OBJECTIVE:To investigate the acute effect of air pollutants on ischaemic stroke (IS) and IS-related death.SETTING:Five urban districts in Changzhou, China, between 9 January 2015 and 31 December 2016.PARTICIPANTS:A total of 32 840 IS cases and 4028 IS deaths were enrolled.MAIN OUTCOME MEASURES:A time-series design, generalised additive model and multivariable regression model were used to examine the percentage change (95% CI) in daily IS counts and deaths with an IQR increase in air pollutant levels for different single or multiple lag days in single-pollutant and two-pollutant models.RESULTS:Daily IS counts increased 0.208% (95% CI 0.036% to 0.381%) with an IQR increment in the levels of nitrogen dioxide (NO2). The estimated risk of NO2 was more robust in males and in the cold season. For daily IS counts, the estimated effects of NO2 and sulfur dioxide (SO2) were more significant when adjusted for particulate matter with aerodynamic diameters <2.5 µm (PM2.5) and PM10. An IQR increment in the concentration of PM10, SO2 and NO2 significantly increased IS deaths with 6 days of cumulative effects (0.268%, 95% CI 0.007% to 1.528%; 0.34%, 0.088% to 0.592%; and 0.263%, 0.004% to 0.522%, respectively). Young individuals (<65 years old) had a higher IS mortality risk for PM2.5, PM10, NO2 and CO. For IS death, the effect estimates of SO2 in the elderly, females and the cold season were more pronounced; statistical significance was also identified for SO2 when adjusted for carbon monoxide (CO).CONCLUSIONS:This study suggested that short-term exposure to ambient NO2 was associated with increased IS risk. In addition, SO2 was associated with increased IS onset and death.

Abstract:Air pollution is associated with stroke, but few previous studies have explored the impact of exposure to a sustained high air pollution level (which we define as an air pollution wave) on stroke. We decomposed the effect estimates of air pollution waves into a main effect estimate and an added effect estimate. The main effect estimate was due to the absolute daily level of air pollution, and the added effect estimate was due to the duration of high-level air pollution. PM10, SO2, NO2 and the Air Pollution Index (API) were chosen as indictors to explore the comprehensive evaluation indictor. The generalized additive model was used to evaluate the impact of air pollution wave on years of life lost (YLLs) due to stroke in Tianjin, China, from 2006 to 2011. Finally, we adopted different air pollution wave definitions to explore features of the association. When using the API as an indicator of air pollution, the association between the air pollution wave and stroke YLLs were significantly increased. When the duration of persistent pollution became longer, the main effect estimate did not change much, but the added effect estimate continued to increase. When the duration was 3 days, the added effect estimate was a maximum of 36.47 years (95% CI: 6.46, 66.48), while the main effect estimate was only 27.87 years (95% CI: 7.62, 48.11). For the other air pollution indicators, the added effect estimate of PM10 were not statistically significant, whereas that of SO2 was significant. The mortality risk associated with the different indicators showed a similar pattern. The comparison results between different indicators suggest that API can better reflect impacts of air pollution waves. Sustained exposure to air pollution had main and added impacts on stroke. The government and public should attach importance to air pollution wave and take individual protection to reduce exposure to pollutants.
