Abstract:Media representations of suffering bodies from medical humanitarian organisations raise ethical questions, which deserve critical attention for at least three reasons. Firstly, there is a normative vacuum at the intersection of medical ethics, humanitarian ethics and the ethics of photojournalism. Secondly, the perpetuation of stereotypes of illness, famine or disasters, and their political derivations are a source of moral criticism, to which humanitarian medicine is not immune. Thirdly, accidental encounters between members of the health professions and members of the press in the humanitarian arena can result in misunderstandings and moral tension. From an ethics perspective the problem can be specified and better understood through two successive stages of reasoning. Firstly, by applying criteria of medical ethics to the concrete example of an advertising poster from a medical humanitarian organisation, I observe that media representations of suffering bodies would generally not meet ethical standards commonly applied in medical practice. Secondly, I try to identify what overriding humanitarian imperatives could outweigh such reservations. The possibility of action and the expression of moral outrage are two relevant humanitarian values which can further be spelt out through a semantic analysis of 'témoignage' (testimony). While the exact balance between the opposing sets of considerations (medical ethics and humanitarian perspectives) is difficult to appraise, awareness of all values at stake is an important initial standpoint for ethical deliberations of media representations of suffering bodies. Future pragmatic approaches to the issue should include: exploring ethical values endorsed by photojournalism, questioning current social norms about the display of suffering, collecting empirical data from past or potential victims of disasters in diverse cultural settings, and developing new canons with more creative or less problematic representations of suffering bodies than the currently accepted stereotypes.
Media Contribution to Violence Against Health Workers in China: a Content Analysis Study of 124 Online Media Reports

Abstract:BackgroundViolence against health workers has been widely reported by the Chinese media. We aimed to identify the characteristics of the violent events as reported by the media and explore the possible role of media in such violence.MethodsWe used content analysis to study all available online newspaper reports of violence against health workers in China from Jan 1, 2013, to March 31, 2015. Duplicates of the same event were analysed in combination. Two researchers independently coded the data. Ethical approval was obtained from Zhejiang University Research Ethics Committee. Informed consent was not necessary because no patients were involved.FindingsWe found 124 incidents from 26 provinces. 73 occurred in tertiary-care facilities, 35 in secondary care, three in community facilities, and 13 in facilities such as private hospitals. 41 (33%) were reported from the emergency department, 22 (18%) in general medicine, 17 (14%) in general surgery, nine (7%) in obstetrics and gynaecology, eight (6%) in paediatrics, two (2%) in psychiatry, and 25 (20%) in other departments (eg, intensive-care unit, operating rooms, and radiology). 77 victims were doctors and 39 were nurses. In 116 (93·5%) cases the perpetrator was male, and 21 (17%) were reported to be drunk. In 31 (25%) cases the episode followed the death of a patient. The incidents included six (5%) murders and 12 (10%) serious injuries, mostly stabbing, including a doctor who had his throat cut. Two doctors were forced to kneel down before the body. 92 (74%) incidents involved relatives or friends of the patient, and 14 (11%) involved "Yi Nao gangs" who threatened and assaulted hospital personnel and damaged facilities to extort compensation. Two perpetrators were imprisoned, two sentenced to death, and one committed suicide, but the overwhelming majority were unpunished. Most reports attributed the violence to spontaneous justified anger resulting from poor care, unmet demands, or high costs.InterpretationThese reports represent extreme examples and are the tip of the iceberg. But the way in which they are reported and widely disseminated has bred fear in health professionals. Concerns that such reports of compensation, obtained with impunity, might contribute to copycat behaviours have also been expressed on social media.FundingNone.
[Communication advice and demands of the hospital: how to deal with them?]

Abstract:The hospital should not actively involve with publications by physicians. It is undesirable and even unnecessary, especially in case of scientific publications: chances of a negative impact on the hospital are odd. A physician is bound by Hippocratic oath as well as by the guidelines and code of conduct of the own professional group. Nevertheless, this professional group should be more self-correcting when these guidelines and codes are not followed by physicians - like in (potential) media exposure of COVID-19 related issues that can increase the communal unrest and polarisation. Sometimes active involvement of the hospital can be recommended, for example in giving professional guidance in case of interviews. On many other occasions, the freedom of speech can be jeopardized or can be followed by censorship.

Abstract:Palliative care professionals are regularly asked to give interviews and be part of documentaries for television, radio and online outlets. This can be important to give our view on (sometimes) controversial topics and also reduce stigma around topics like death and dying. But what are the pitfalls when agreeing to talk on camera or microphone in your healthcare setting? How do we safely bring patients into this space, without compromising their care? This article aims to address some of the common areas of miscommunication with the media, including consent, vetting, contractual arrangements and safety.In the last few years, I have been asked to take part in several television, online and audio documentaries, for different topics; palliative care, communication at the end of life and opioid guidelines 1–3 Sometimes, I have declined requests. I have had both positive and negative experiences with journalistic media, and what follows may help individuals and/or organisations considering such requests.

Abstract:Historically, natural and manmade disasters create many victims and impose pressures on health-care infrastructure and staff; potentially hampering the provision of patient care and overloading clinician capacity. Throughout the course of history, clinicians have performed heroics to work well above their required duty, despite limitations, even putting their own health and safety at risk. In times when clinicians needed to either physically abandon patients or consider abandoning active treatment, we have seen extreme hesitancy to do so, fearing that they may be giving up too soon, that undue harm may come to patients, or even feeling unsure of legal or moral burdens that may ensue. In times when clinicians are placed in this unimaginable position, feeling isolated and overwhelmed, it is essential that they be supported and provided with resources to standardize decision-making.

A critical analysis of newspaper accounts of violence against doctors in India

Abstract:This paper presents a critical analysis of newspaper articles (N = 60) published in a leading vernacular newspaper about violence against doctors in India. Adopting a theoretical perspective that considers 'news as a cultural practice,' a qualitative content analysis was conducted to examine how the phenomenon is framed and presented in the news, i.e., what is problematized, what causal links are drawn or hinted at, what moral stance is taken or alluded to, and what solutions are proffered and why, to arrive at a nuanced understanding of various aspects of this social phenomenon. Three overlapping key themes emerged from the analysis, namely the narrative of victimization, the changing doctor-patient relationship, and the crisis facing the 'noble profession' of medicine. It reveals how the media shapes public opinion and attitudes towards the state of the medical profession while in turn, reflecting existing opinions, attitudes, and cultural values; the analysis also reveals missing perspectives such as the voices of the patients and the public. We highlight how the findings are not merely the dominant ways in which the rise in incidents of violence against doctors is reported and is understood in society, but how media might have shaped the popular discourse around the issue and why. We reflect on what the reportage says about the state of the medical profession and its standing in society in India.

Abstract:While COVID-19 brings unprecedented challenges to the US healthcare system, understanding narratives of historical disasters illuminates ethical complexities shared with COVID-19. In 2005, Hurricane Katrina revealed a lack of disaster preparation and protocol, not dissimilar to the challenges faced by COVID-19 healthcare workers. A case study of Memorial Hospital during Hurricane Katrina reported by journalist-MD Sheri Fink reveals unique ethical challenges at the forefront of health crises. These challenges include disproportionate suffering in structurally vulnerable populations, as seen in COVID-19 where marginalised groups across the USA experience higher rates of disease and COVID-19-related death. Journalistic accounts of Katrina and COVID-19 offer unique perspectives on the ethical challenges present within medicine and society, and analysis of such stories reveals narrative trajectories anticipated in the aftermath of COVID-19. Through lenses of social suffering and structural violence, these narratives reinforce the need for systemic change, including legal action, ethical preparedness and physician protection to ensure high-quality care during times of crises. Narrative Medicine-as a practice of interrogating stories in medicine and re-centering the patient-offers a means to contextualise individual accounts of suffering during health crises in larger social matrices.

Abstract:Media depictions of mental health difficulties and suicide are an important element in shaping how the public understands the issues and how relevant agencies devise strategies to advocate and reduce stigma. The role of the media in the production and reproduction of stereotypical portrayals of mental health difficulties and suicide has been of concern for a number of decades. However, relatively little research attention has been paid to date to the process through which stories on mental illness and suicide are produced by journalists. This article aims to contribute to that analysis by focusing on Irish journalists’ experiences of and insights into the challenges underpinning reporting on mental health difficulties and suicide. Despite some generic and platform differences, journalists noted that their output was shaped by a number of factors, which included, sensitivity to the topic of suicide; care for families; balancing public and private interests; challenges to reporting facts, finding appropriate sources and meeting the needs of media platforms; interpreting guidelines on reporting and the discovery of the relative absence of mental illness stories. Understanding how journalists deal with these challenges can help them and advocacy agencies to address these challenges and thereby improve the nature of coverage of mental health difficulties and suicide in media output.

Abstract:Background: It is important for health organizations to communicate with the public through newspapers during health crises. Although hospitals were a main source of information for the public during the COVID-19 pandemic, little is known about how this information was presented to the public through (web-based) newspaper articles. Objective: This study aims to examine newspaper reporting on the situation in hospitals during the first year of the COVID-19 pandemic in the Netherlands and to assess the degree to which the reporting in newspapers aligned with what occurred in practice. Methods: We used a mixed methods longitudinal design to compare internal data from all hospitals (n=5) located in one of the most heavily affected regions of the Netherlands with the information reported by a newspaper covering the same region. The internal data comprised 763 pages of crisis meeting documents and 635 minutes of video communications. A total of 14,401 newspaper articles were retrieved from the LexisNexis Academic (RELX Group) database, of which 194 (1.3%) articles were included for data analysis. For qualitative analysis, we used content and thematic analyses. For quantitative analysis, we used chi-square tests. Results: The content of the internal data was categorized into 12 themes: COVID-19 capacity; regular care capacity; regional, national, and international collaboration; human resources; well-being; public support; material resources; innovation; policies and protocols; finance; preparedness; and ethics. Compared with the internal documents, the newspaper articles focused significantly more on the themes COVID-19 capacity ( P <.001), regular care capacity ( P <.001), and public support ( P <.001) during the first year of the pandemic, whereas they focused significantly less on the themes material resources ( P =.004) and policies and protocols ( P <.001). Differences in attention toward themes were mainly observed between the first and second waves of the pandemic and at the end of the third wave. For some themes, the attention in the newspaper articles preceded the attention given to these themes in the internal documents. Reporting was done through various forms, including diary articles written from the perspective of the hospital staff. No indication of the presence of misinformation was found in the newspaper articles. Conclusions: Throughout the first year of the pandemic, newspaper articles provided coverage on the situation of hospitals and experiences of staff. The focus on themes within newspaper articles compared with internal hospital data differed significantly for 5 (42%) of the 12 identified themes. The discrepancies between newspapers and hospitals in their focus on themes could be attributed to their gatekeeping roles. Both parties should be aware of their gatekeeping role and how this may affect information distribution. During health crises, newspapers can be a credible source of information for the public. The information can also be valuable for hospitals themselves, as it allows them to anticipate internal and external developments.

Abstract:In public health emergencies, local media are important sources of information for the public. Not much is known about the professional and personal roles and experiences of Sierra Leonean journalists during the Ebola virus disease (EVD) outbreak between 2014–2015.This qualitative study is based on semi-structured interviews with 13 Sierra Leonean journalists based in urban Freetown and rural Waterloo in February and March 2016. The majority of the journalists worked for radio stations. The mostly male journalists represented national, regional and local radio stations. Transcripts of the interviews were analysed using thematic analysis. The analysis was inspired by previously reported journalist roles and agenda-setting theory, which state that the media filter what is happening, making a few core issues more salient. Sierra Leonean journalists shifted from being sceptical monitors in the beginning of the outbreak, to collaborative instructors towards the end. While they adapted to different roles, journalists struggled with their own fears for the virus, which hampered their work. They indicated that the training they received about Ebola helped them overcome their fear. Being trained gave a sense of security that helped them carry out their jobs. By turning into instructors, journalists stepped away from their journalistic professional detachment–potentially exacerbated by their personal experiences.The first months of the outbreak were marked by passive agenda-setting roles of Sierra Leonean journalists. It took several months before the outbreak became a core issue in local media. In health emergencies, efforts should be made to partner with local media to use their platforms for local, trusted journalists and leaders to disseminate public health messages. Whereas this might hamper journalists' credibility and can be challenging in areas with problematic press freedom, Sierra Leonean journalists experienced the outbreak as a driver of necessary change in their profession.Local media are important sources of information for the public during a health emergency. However, not much is known about the roles and experiences of local journalists who have to report under extraordinary circumstances. In this qualitative study, we interviewed Sierra Leonean journalists who reported during the 2014–2015 Ebola outbreak. The thematic analysis shows that the journalists adapted different roles over the course of the outbreak. They went from being sceptical about the existence of an outbreak at the start to changing into instructors at the end of the outbreak. Journalists struggled with their own fear for the virus, which they could overcome through targeted training, that helped them identify safe practices of reporting in the field. Our results show that it is possible to effectively partner with local media to disseminate public health messages. This might be challenging in settings with problematic press freedom, but Sierra Leonean journalists actually felt that the outbreak helped improve their practices.

Hidden in plain sight: the covering of patients' eyes and a microethics of medical photography

Abstract:This article uses the author's experience of researching historical photographs of facial injury and surgical reconstruction to think through the ethics of writing about and publishing images of patients anonymised by excising or covering their eyes. This article specifically highlights tensions between the British Medical Journal's guidelines for patient anonymity in imagery and those of the archives of the British Association of Plastic, Reconstructive and Aesthetic Surgeons. The rules for reproducing these sensitive images are not standardised across disciplines nor across journals and medical archives. But by using lived academic experience, visual analysis and philosophical enquiry, a flexible personal directive (or microethics) for working with these images can be reached.In order to more fully understand where the present-day suggestion of and debates around blocking out patients' eyes for anonymity come from, this ethical analysis is tied back to the historical precedent of Harold Gillies' 1920 publication Plastic Surgery of the Face, in which civilians' eyes are covered. Theories of looking and of photography unpick some of the complex ideas that these images raise regarding patient agency in medical imagery. This article will have direct application for any researcher grappling with similarly difficult material wondering how to frame their own microethics or ethics in practice for discussing, showing or publishing these types of images.
Exploring perceptions of reporting violence against healthcare workers in the emergency department: A qualitative study

Abstract:Background: Violence against healthcare workers is a pervasive, yet in many cases, under-reported problem. This is due to various factors, including lack of time, support and a universal understanding of what constitutes a reportable event. This study explored facilitators and barriers to reporting workplace violence among emergency department nurses. Methods: In this descriptive, qualitative study, researchers conducted open-ended interviews with emergency nurses considered to be "high-" and "non-reporters" of violent events and analyzed for themes. Results: Participants cited consistent factors associated with less reporting, factors associated with more reporting and effectiveness of existing safety measures. Conclusions: To encourage the reporting of violent events, frequently cited barriers and facilitators should be addressed. Strategies such as integrating reporting mechanisms into the health record, creating nuanced definitions of reportable events, and consistent education with positive feedback can promote reporting by staff. These efforts should be combined with prevention strategies to ensure we are collecting correct data about the success or failure of these programs.

Abstract:The magnitude of suffering on both the Israeli and Palestinian sides of the current war is beyond comprehension. Political agendas, misinformation and bias related to the conflict are being seen far too frequently in healthcare and medical academia. We believe it is time for healthcare professionals to redirect our attention away from politics and use our medical training to advocate for peace, care, and the welfare of all people, regardless of which side of the conflict they fall into. Politics in the workplace, particularly when disseminated information is divisive and, at times, based on opinion rather than fact, risks significant harm to patients, their families, and healthcare staff, as well as to institutional reputation. If we genuinely care for the well-being of patients and staff, we must lead by example and prevent healthcare systems and medical journals from being hijacked by politics.

Assembling a global health image: Ethical and pragmatic tensions through the lenses of photographers

Abstract:Background: Recently, global health has been confronting its visual culture, historically modulated by colonialism, racism and abusive representation. There have been international calls to promote ethicality of visual practices. However, despite this focus on the history and the institutional use of global health images, little is known about how in practice contemporary images are created in communities, and how consent to be in photographs is obtained. Methods: We conducted semi-structured interviews with 29 global health photographers about the ethical and practical challenges they experience in creating global health images, and thematically analysed the findings. Findings: The following themes were identified: (1) global health photography is undergoing a marketing transformation and images are being increasingly moderated; (2) photographers routinely negotiate stereotypical and abusive tropes purposefully sought by organisations; (3) local scenes are modified, enhanced and staged to achieve a desired marketing effect; (4) 'empowerment' is becoming an increasingly prominent dehumanising visual trope; (5) consent to be photographed can be jeopardised by power imbalances, illiteracy, fears and trust; (6) organisations sometimes problematically recycle images. Interpretation/discussion: This research has identified practical and ethical issues experienced by global health photographers, suggesting that the production cycle of global health images can be easily abused. The detected themes raise questions of responsibility and accountability, and require further transdisciplinary discussion, especially if promoting ethical photojournalism is the goal for 21st century global health.

Abstract:A recent publication in the Internal Medicine Journal touches on some sensitive points. It emphasizes deficiencies in communication between doctors and patients and between doctors and families of the patients, particularly in extreme medical circumstances that are likely to end in death. It stresses as well the trauma of seeing a loved one reduced from a human being with a distinctive biography to an impersonal clinical problem. Such articles have a long history but seldom produce any change in the attitudes or practices of the medical profession. There seem to be a number of reasons for this, including clinicians' ambivalence towards the real importance of ethics in medicine and the lack of a forum in which consumers and doctors might air and discuss their experiences of suffering and disappointment. A forum for mutual discussion is suggested as the most constructive step to take. Medical education has not proven to be an answer to the problems of sensitive communication.
