Abstract:OBJECTIVE: To investigate the clinical significance of plasma homocysteine Hcy in patients with coronary atherosclerosis. METHODS Plasma Hcy levels of 85 patients with coronary atherosclerosis and 68 normal controls were determined by fluorescence polarization immunoassay. RESULTS The mean levels of plasma Hcy were (9.31+/-3.80)&mgr;mol/L in normal controls and (13.39+/-6.06)&mgr;mol/L in patients with coronary atherosclerosis. That was (11.36+/-3.86)&mgr;mol/L in the patients with micro-pathological changes of coronary artery, (13.32+/-6.09)&mgr;mol/L with single-vessel disease,(13.39+/-4.92)&mgr;mol/L with double-vessel disease, and (18.23+/-8.98)&mgr;mol/L with three-vessel disease by coronary angiography. Statistically, the mean plasma Hcy concentrations in male and female patients was higher than that in the corresponding control subjects(13.77+/-6.68 compared with 10.50+/-4.07, 11.50+/-3.58 compared with 7.80+/-2.85 &mgr;mol/L,P<0.001 respectively). CONCLUSION The patients with coronary atherosclerosis present hyperhomocysteinemia is very important to determine plasma homocysteine for diagnosis and therapy in the patients with coronary heart disease.

Abstract:It has been reported that chronic inflammation of the vessel wall is a hallmark of atherosclerosis. Interleukin-6 (IL-6) is regarded as an important modulator of inflammatory events occurring during all stages of atherogenesis. Although many factors that induce IL-6 expression have been identified, the effect of homocysteine (Hcy) on the expression of IL-6 in atherogenesis and the underlying mechanisms are not entirely clear. The objective of the present study was to investigate the role of Hcy in IL-6 expression in rat aorta vascular smooth muscle cells (VSMCs). After VSMCs were incubated with Hcy for various time periods, enzyme-linked immunosorbent assay (ELISA) and semi-quantitative reverse-transcriptase polymerase chain reaction (RT-PCR) were performed to measure the expression of IL-6. Electrophoretic mobility shift assay (EMSA) was used to examine nuclear factor κB (NF-κB) activity. Hcy (0.01–0.25mmol/l) significantly increased the expression of IL-6 mRNA and protein in rat VSMCs. The increase in IL-6 expression was associated with the activation of NF-κB. Pyrrolidine dithiocarbamate (PDTC) suppressed Hcy-induced IL-6 release, a finding compatible with involvement of reactive oxygen species as second messengers in cytokine production mediated by Hcy. The present study has clearly demonstrated the ability of Hcy to elicit an inflammatory response in rat VSMCs by stimulation of IL-6 production and activation of NF-κB. Inflammation activation on vessel walls by elevation of Hcy may contribute to the pathogenesis of atherosclerosis.
Reduced Number and Activity of Circulating Endothelial Progenitor Cells from Patients with Hyperhomocysteinemia.

Abstract:Background. Hyperhomocysteinemia (HHcy) contributes to atherosclerosis and coronary artery diseases by inducing endothelial cell injury and dysfunction. Recent studies provided increasing evidence that endothelial progenitor cells (EPCs) participated in ongoing endothelial repair. The changes of EPCs in patients with HHcy have not yet been elucidated. Therefore, we investigated the number and functional activity of EPCs in patients with HHcy.Methods. Human EPCs were isolated and cultured from patients with HHcy (n = 30) and matched volunteers (n = 30). Circulating EPCs were enumerated as ACl33(+)KDR(+) cells via fluorescence-activated cell sorter analysis. Additionally, EPC were expanded from human blood in vitro and identified by Dil-acLDL uptake and lectin staining by direct fluorescent stainina under a laser scanning confocal microscope. EPC migration activities were determined by modified Boyden chamber assay. EPC adhesion assay was performed by replating cells on fibronectin-coated dishes and then counting adherent cells.Results. A significant decrease was observed in circulating EPC (AC133(+)KDR(+) cells) numbers in patients with HHcy compared with control subjects (63.9 +/- 11.7 cells/mL vs. 91.5 +/- 14.2 cells/mL blood, p <0.01). In addition, the numbers of EPCs also decreased in patients with HHcy after ex vivo cultivation (36.1 +/- 6.5 vs. 51.5 +/- 8.3 EPCs/X200 field, p <0.01). Both circulating EPCs and differentiated EPCs were inversely correlated with total homocysteine levels. In addition, EPCs from patients with HHcy were significantly impaired in their migratory capacity and ability to adhere to fibronectin compared with controls.Conclusions. Abstract:Aim To explore the effects of homocysteine(Hcy) on the expression of matrix metalloproteinase-2(MMP-2) in rat vascular smooth muscle cells(VSMC). Methods Cultured rat VSMC was incubated with different concentration of Hcy in vitro for 24,48 and 72 h.The expression of MMP-2 was determined by using the methods of gelatin zymography and western blotting. Results 0.05～1.0 mmol/L Hcy increased the expression of MMP-2 significantly.Incubated with the same concentration of Hcy the level of MMP-2 of 72 h was higher than that of 24 h and 48 h. Hcy >5.0 mmol/L reduced the expression of MMP-2.Incubated with the same concentration of Hcy(>5.0 mmol/L) the level of MMP-2 of 72 h was lower than that of 24 h and 48 h. Conclusions These datas suggested that Hcy can affect the expression of MMP-2 in VSMC.It may be one of factors in the pathogenesis of atherosclerosis induced by Hcy.

Abstract:In the Berkooz-Douglas matrix model of M theory in the presence of longitudinal M5-brane, we investigate the effective dynamics of the system by considering the longitudinal M5-brane as the background and the spherical M5-brane related with the other space dimensions as the probe brane. Due to there exists the background field strength provided by the source of the longitudinal M5-brane, an open membrane should be ended on the spherical M5-brane based on the topological reason. The formation of the bound brane configuration for the open membrane ending on the 5branes in the background of longitudinal 5-brane can be used to model the 4-dimensional quantum Hall system proposed recently by Zhang and Hu. The description of the excitations of the quantum Hall soliton brane configuration is established by investigating the fluctuations of D0-branes living on the bound brane around their classical solution derived by the transformations of area preserving diffeomorphisms of the open membrane. We find that this effective field theory for the fluctuations is an SO(4) non-commutative Chern-Simons field theory. The matrix regularized version of this effective field theory is given in order to allow the finite D0-branes to live on the bound brane. Abstract:Homocysteine (Hcy) is an intermediate amino acid formed during the conversion from methionine to cysteine. When the fasting plasma Hcy level is higher than 15 μmol/L, it is considered as hyperhomocysteinemia (HHcy). The vascular endothelium is an important barrier to vascular homeostasis, and its impairment is the initiation of atherosclerosis (AS). HHcy is an important risk factor for AS, which can promote the development of AS and the occurrence of cardiovascular events, and Hcy damage to the endothelium is considered to play a very important role. However, the mechanism by which Hcy damages the endothelium is still not fully understood. This review summarizes the mechanism of Hcy-induced endothelial injury and the treatment methods to alleviate the Hcy induced endothelial dysfunction, in order to provide new thoughts for the diagnosis and treatment of Hcy-induced endothelial injury and subsequent AS-related diseases.

Atherogenic Effect of Homocysteine, a Biomarker of Inflammation and Its Treatment

Abstract:Hyperhomocysteinemia (HHcy) is an independent risk factor for atherosclerosis. Ischemic stroke and heart disease, coronary heart disease, and cardiovascular disease are events resulting from long-lasting and silent atherosclerosis. This paper deals with the synthesis of homocysteine (Hcy), causes of HHcy, mechanism of HHcy-induced atherosclerosis, and treatment of HHcy. Synthesis and metabolism of Hcy involves demethylation, transmethylation, and transsulfuration, and these processes require vitamin B 6 and vitamin B 12 folic acid (vitamin B 9 ). Causes of HHcy include deficiency of vitamins B 6 , B 9 , and B 12 , genetic defects, use of smokeless tobacco, cigarette smoking, alcohol consumption, diabetes, rheumatoid arthritis, low thyroid hormone, consumption of caffeine, folic acid antagonist, cholesterol-lowering drugs (niacin), folic acid antagonist (phenytoin), prolonged use of proton pump inhibitors, metformin, and hypertension. HHcy-induced atherosclerosis may be mediated through oxidative stress, decreased availability of nitric oxide (NO), increased expression of monocyte chemoattractant protein-1, smooth muscle cell proliferation, increased thrombogenicity, and induction of arterial connective tissue. HHcy increases the generation of atherogenic biomolecules such as nuclear factor-kappa B, proinflammatory cytokines (IL-1β, IL-6, and IL-8), cell adhesion molecules (intercellular adhesion molecule-1, vascular cell adhesion molecule-1, and E-selection), growth factors (IGF-1 and TGF-β), and monocyte colony-stimulating factor which lead to the development of atherosclerosis. NO which is protective against the development of atherosclerosis is reduced by HHcy. Therapy with folic acid, vitamin B 6 , and vitamin B 12 lowers the levels of Hcy, with folic acid being the most effective. Dietary sources of folic acid, vitamin B 6 , vitamin B 12 , omega-3 fatty acid, and green coffee extract reduce Hcy. Abstaining from drinking coffee and alcohol, and smoking also reduces blood levels of Hcy. In conclusion, HHcy induces atherosclerosis by generating atherogenic biomolecules, and treatment of atherosclerosis-induced diseases may be by reducing the levels of Hcy. Experience with beta-blockers in long term management of peripartum cardiomyopathy.

Abstract:Peripartum cardiomyopathy (PPCM) is an ominous complication of pregnancy, about which little is known. Although the role of Beta Blockers is well established in heart failure, there is limited data evaluating their use in Peripartum cardiomyopathy. We report the use of Beta-Blockers (metoprolol) in conjunct with standard heart failure therapy in two patients of PPCM with favorable long-term outcome. Our experience, although limited, highlights the significance of use of Beta-Blockers in this rare life threatening condition.

Abstract:Atherosclerosis is associated with multiple genetic and modifiable risk factors. There is an increasing body of evidences to indicate that epigenetic mechanisms also play an essential role in atherogenesis by influencing gene expression. Homocysteine is a sulfur-containing amino acid formed during methionine metabolism. Elevated plasma level of homocysteine is generally termed as hyperhomocysteinemia. As a potential risk factor for cardiovascular diseases, hyperhomocysteinemia may initiate or motivate atherogenesis by modification of DNA methylation. The underlying epigenetic mechanism is still unclear with controversial findings. This review focuses on epigenetic involvement and mechanisms of hyperhomocysteinemia in atherogenesis. Considering the potential beneficial effects of anti-homocysteinemia treatments in preventing atherosclerosis, further studies on the role of hyperhomocysteinemia in atherogenesis are warranted.

Abstract:Homocysteine is a sulphur-containing non-proteinogenic amino acid. Hyperhomocysteinaemia (HHcy), the pathogenic elevation of plasma homocysteine as a result of an imbalance of its metabolism, is an independent risk factor for various vascular diseases, such as atherosclerosis, hypertension, vascular calcification and aneurysm. Treatments aimed at lowering plasma homocysteine via dietary supplementation with folic acids and vitamin B are more effective in preventing vascular disease where the population has a normally low folate consumption than in areas with higher dietary folate. To date, the mechanisms of HHcy-induced vascular injury are not fully understood. HHcy increases oxidative stress and its downstream signalling pathways, resulting in vascular inflammation. HHcy also causes vascular injury via endoplasmic reticulum stress. Moreover, HHcy up-regulates pathogenic genes and down-regulates protective genes via DNA demethylation and methylation respectively. Homocysteinylation of proteins induced by homocysteine also contributes to vascular injury by modulating intracellular redox state and altering protein function. Furthermore, HHcy-induced vascular injury leads to neuronal damage and disease. Also, an HHcy-activated sympathetic system and HHcy-injured adipose tissue also cause vascular injury, thus demonstrating the interactions between the organs injured by HHcy. Here, we have summarized the recent developments in the mechanisms of HHcy-induced vascular injury, which are further considered as potential therapeutic targets in this condition. Diet-Induced Severe Hyperhomocysteinemia Promotes Atherosclerosis Progression and Dysregulates the Plasma Metabolome in Apolipoprotein-E-Deficient Mice

Abstract:Atherosclerosis and resulting cardiovascular disease are the leading causes of death in the US. Hyperhomocysteinemia (HHcy), or the accumulation of the intermediate amino acid homocysteine, is an independent risk factor for atherosclerosis, but the intricate biological processes mediating this effect remain elusive. Several factors regulate homocysteine levels, including the activity of several enzymes and adequate levels of their coenzymes, including pyridoxal phosphate (vitamin B6), folate (vitamin B9), and methylcobalamin (vitamin B12). To better understand the biological influence of HHcy on the development and progression of atherosclerosis, apolipoprotein-E-deficient (apoE−/− mice), a model for human atherosclerosis, were fed a hyperhomocysteinemic diet (low in methyl donors and B vitamins) (HHD) or a control diet (CD). After eight weeks, the plasma, aorta, and liver were collected to quantify methylation metabolites, while plasma was also used for a broad targeted metabolomic analysis. Aortic plaque burden in the brachiocephalic artery (BCA) was quantified via 14T magnetic resonance imaging (MRI). A severe accumulation of plasma and hepatic homocysteine and an increased BCA plaque burden were observed, thus confirming the atherogenic effect of the HHD. Moreover, a decreased methylation capacity in the plasma and aorta, indirectly assessed by the ratio of S-adenosylmethionine to S-adenosylhomocysteine (SAM:SAH) was detected in HHD mice together with a 172-fold increase in aortic cystathionine levels, indicating increased flux through the transsulfuration pathway. Betaine and its metabolic precursor, choline, were significantly decreased in the livers of HHD mice versus CD mice. Widespread changes in the plasma metabolome of HHD mice versus CD animals were detected, including alterations in acylcarnitines, amino acids, bile acids, ceramides, sphingomyelins, triacylglycerol levels, and several indicators of dysfunctional lipid metabolism. This study confirms the relevance of severe HHcy in the progression of vascular plaque and suggests novel metabolic pathways implicated in the pathophysiology of atherosclerosis.

[The Relationship Between Carotid Artery Atherosclerosis and Hyperhomocysteinaemia].

Abstract:OBJECTIVE:To explore the relationship between the carotid artery atherosclerosis (CAA) and the level of homocysteine (Hcy) as well as folate, vitamin B(12) and lipids.METHODS:The diameter of common carotid arteries and internal carotid arteries on both sides were measured by B-mode ultrasound in 126 subjects who were divided into normal and A, B, C, D groups according to the severity of stenosis of CAA. With fasting serum, Hcy as well as folate, vitamin B(12), triglycerides, total cholesterols (TC), low density lipid (LDL), high density lipid (HDL) were detected.RESULTS:When stenosis of CAA became severer, serum Hcy was higher. The levels of the normal and A, B, C, D groups were (13.22 +/- 6.15, 16.29 +/- 9.81, 19.49 +/- 11.16, 27.21 +/- 17.47, 24.14 +/- 8.64) micromol/L, respectively. Rank test showed a significant difference between the normal and other groups (P < 0.01). Folic acid and VitB(12) were negatively correlated with the Hcy concentrations. Spearman correlation coefficient were -0.23 and -0.42 (P = 0.000). According to the severity of stenosis of CAA, Hcy, age, weight, blood pressure, level of glucose, triglyceride, TC, LDL, high density lipid (HDL) and history of hypertension, diabetes, heart disease and stroke were brought into logistic analysis; the result showed that Hcy, TC, HDL had significant negative correlation with the severity of CAA, Spearman correlation coefficient were 0.56, 0.25 and 0.22 (P = 0.003, 0.02, 0.04).CONCLUSION:Hyperhomocysteinaemia is an independent risk factor of CAA and the severity of stenosis of CAA is highly correlated with Hcy concentration. The causes of hyperhomocysteinaemia may be the results of decrease of folate and VitB(12) levels. Clinical trials are now required to evaluate the effect of treatment with these vitamins in the primary and secondary prevention of vascular diseases.
Hyperhomocysteinemia and Hyperlipidemia in Coronary Heart Disease.

Abstract:Objective To examine the relationship between coronary heart disease (CHD) and serum lipid, plasma homocysteine (HCY) as well as the factors related to HCY metabolisms.Methods The mutation of the 677C-->T transition of 5, 10-methylenetetrahydrofolate reductase (MTHFR) was determined by PCR-based assay. Whole-blood and plasma folate and plasma vitamin B-12, as cofactors of MTHFR, were determined by radio-immunologic assay. Plasma HCY was determined by HPLC.Results Patients with CHD had elevated plasma HCY concentrations (17.38 +/- 1.94 mu mol/L vs 10.25 +/- 1.57 mu mol/L, P < 0.01). In patients with myocardial infarction (MI) and family history (FH) of CHD, plasma HCY were elevated even higher (P < 0.05). Plasma HCY concentrations had significant non-linear inverse correlation with plasma folate and B-12 concentrations, i.e. the lower the serum folate or B-12 concentrations, the higher the plasma HCY concentrations (P < 0.01). Patients with homozygous mutants had higher plasma HCY concentrations. Patients with CHD had increased serum Chol and LDL-C and Apo-B levels (P < 0.01, P < 0.05 and P < 0.05 respectively). But plasma HCY concentrations had no correlation with serum lipid levels. 24.1% of the patients had high lipid and high HCY level, 25.9% had high lipid level and normal HCY level, 20.4% had normal lipid and high HCY level, and 29.6% had normal lipid and HCY level.Conclusions HCY may have strong association with the genesis of CHD. Low plasma folate and B-12 concentrations may induce Hyperhomocysteinemia [HH(e)]. Plasma HCY concentrations have no correlation with serum lipid levels, so HCY may be an independent risk factor. CHD may be induced by different mechanisms and can be classified into hyperlipidemia, HH (e) normolipidemia, and normohomocys-teinemia.

Abstract:Hyperhomocysteinemia (HHcy) is associated with increased diabetic cardiovascular diseases. However, the role of HHcy in atherogenesis associated with hyperglycemia (HG) remains unknown. To examine the role and mechanisms by which HHcy accelerates HG-induced atherosclerosis, we established an atherosclerosis-susceptible HHcy and HG mouse model. HHcy was established in mice deficient in cystathionine beta-synthase (Cbs) in which the homocysteine (Hcy) level could be lowered by inducing transgenic human CBS (Tg-hCBS) using Zn supplementation. HG was induced by streptozotocin injection. Atherosclerosis was induced by crossing Tg-hCBS Cbs mice with apolipoprotein E-deficient (ApoE(-/-)) mice and feeding them a high-fat diet for 2 weeks. We demonstrated that HHcy and HG accelerated atherosclerosis and increased lesion monocytes (MCs) and macrophages (MOs) and further increased inflammatory MC and MO levels in peripheral tissues. Furthermore, Hcy-lowering reversed circulating mononuclear cells, MC, and inflammatory MC and MC-derived MO levels. In addition, inflammatory MC correlated positively with plasma Hcy levels and negatively with plasma s-adenosylmethionine-to-s-adenosylhomocysteine ratios. Finally, L-Hcy and D-glucose promoted inflammatory MC differentiation in primary mouse splenocytes, which was reversed by adenoviral DNA methyltransferase-1. HHcy and HG, individually and synergistically, accelerated atherosclerosis and inflammatory MC and MO differentiation, at least in part, via DNA hypomethylation.

Generalized linear regression was used to investigate the relationship between Hcy-related indicators (plasma total Hcy [tHcy], vitamin B12, and folate) and myocardial injury biomarkers (high-sensitivity troponin T [hs-cTnT], high-sensitivity troponin I [hs-cTnI] measured using 3 assays [Abbott, Siemens, and Ortho], and N-terminal pro-B-type natriuretic peptide [NT-proBNP]). Results: Among 10,871 participants, the weighted mean levels for tHcy, folate, and vitamin B12 were 8.58 μmol/L, 32.43 nmol/L, and 447.08 pmol/L, respectively. Plasma tHcy levels were positively associated with elevated hs-cTnT, hs-cTnI, and NT-proBNP, whereas folate and vitamin B12 were not inversely related to myocardial injury biomarkers. Multivariable-adjusted odds ratios for elevated hs-cTnT (19 ng/L) and NT-proBNP (125 pg/mL) per doubling of tHcy were 2.80 (95% CI: 1.17-6.73; P < 0.001) and 1.58 (95% CI: 1.20-2.08; P < 0.001), respectively. The associations of tHcy levels with elevated hs-cTnI (Abbott: 28 ng/L; Siemens: 46.5 ng/L; Ortho: 11 ng/L) were consistent. Indirect effects of tHcy on cardiovascular mortality risk via hs-cTnT and NT-proBNP explained up to 26.6% and 12.3% of the total effect, respectively. Conclusions: Plasma tHcy, not folate or vitamin B12, is significantly associated with elevated hs-cTnT, hs-cTnI, and NT-proBNP in adults without CVD. Subclinical myocardial injury may substantially mediate Hcy-related cardiovascular mortality risk.

Abstract:Hyperhomocysteinemia (HHcy) has been recognized as an independent risk factor for atherosclerosis for more than 30years, but the mechanisms by which HHcy leads to atherosclerosis are not well fully understood. In this review, we will summarize the immunoregulatory effects of homocysteine on cardiovascular diseases from humoral immunity, monocyte/macrophage and T lymphocyte activity. Homocysteine can induce chemokine and cytokine secretion in monocytes and T lymphocytes and also directly stimulate B lymphocyte proliferation and IgG secretion. In addition, the cellular mechanisms that may explain the pro-inflammatory effect of HHcy are included. Homocysteine may directly or indirectly lead to oxidative stress or endoplasmic reticulum (ER) stress. Elevated levels of homocysteine also decrease the bioavailability of nitric oxide and modulate the levels of other metabolites including S-adenosyl methionine and S-adenosyl homocysteine which may result in cardiovascular diseases.

Abstract:Homocysteine (Hcy) is a thiol group containing the amino acid, which naturally occurs in all humans. Hcy is degraded in the body through two metabolic pathways, while a minor part is excreted through kidneys. The chemical reactions that are necessary for degradation of Hcy require the presence of folic acid, vitamins B6 and B12. Consequently, the level of the total Hcy in the serum is influenced by the presence or absence of these vitamins. An elevated level of the Hcy, hyperhomocysteinemia (HHcy) and homocystinuria is connected with occlusive artery disease, especially in the brain, the heart, and the kidney, in addition to venous thrombosis, chronic renal failure, megaloblastic anemia, osteoporosis, depression, Alzheimer’s disease, pregnancy problems, and others. Elevated Hcy levels are connected with various pathologies both in adult and child population. Causes of HHcy include genetic mutations and enzyme deficiencies in 5, 10-methylenetetrahydrofolate reductase (MTHFR) methionine synthase (MS), and cystathionine β-synthase (CβS). HHcy can be caused by deficiencies in the folate, vitamin B12 and to a lesser extent, deficiency in B6 vitamin what influences methionine metabolism. Additionally, HHcy can be caused by the rich diet and renal impairment. This review presents literature data from recent research related to Hcy metabolism and the etiology of the Hcy blood level disorder. In addition, we also described various pathological mechanisms induced by hereditary disturbances or nutritional influences and their association with HHcy induced pathology in adults and children and treatment of these metabolic disorders.

Abstract:Over the last 30 years, a growing body of evidence has documented the role of hyperhomocysteinemia (HHcy) as an independent vascular risk factor. However, the mechanisms through which elevated circulating levels of homocysteine (Hcy) cause vascular injury and promote thrombosis remain elusive. Most findings have been achieved in in vitro studies employing exceedingly high concentrations of Hcy, whereas only a few studies have been carried out in vivo in humans. In homocystinuric patients, homozygotes for mutations of the gene coding for the cystathionine beta-synthase enzyme, abnormalities of coagulation variables reflecting a hypercoagulable state, have been reported. In vitro studies provide a biochemical background for such a state. In homocystinuric patients, an in vivo platelet activation has also been reported. The latter abnormality is not corrected by the bolus infusion of concentrations of hirudin, which determines a long-lasting impairment of the conversion of fibrinogen to fibrin by thrombin; in contrast, it appears at least in part lowered by the administration of the antioxidant drug probucol. During the autooxidation of Hcy in plasma, reactive oxygen species are generated. The latter initiate lipid peroxidation in cell membranes (potentially responsible for endothelial dysfunction) and in circulating lipoproteins. Oxidized low-density lipoproteins (LDL) may trigger platelet activation as well as some of the hemostatic abnormalities reported in such patients. Thus the oxidative stress induced by Hcy may be a key process in the pathogenesis of thrombosis in HHcy. Accumulation of adenosylhomocysteine in cells (a consequence of high circulating levels of homocysteine) inhibits methyltransferase enzymes, in turn preventing repair of aged or damaged cells. This mechanism has been recently documented in patients with renal failure and HHcy and provides an additional direction to be followed to understand the tendency to thrombosis in moderate HHcy.

Abstract:An association between arteriosclerosis and homocysteine (Hcy) was first demonstrated in 1969. Hcy is a sulfur containing amino acid derived from the essential amino acid methionine (Met). Hyperhomocysteinemia (HHcy) was subsequently shown in several age-related pathologies such as osteoporosis, Alzheimer’s disease, Parkinson’s disease, stroke, and cardiovascular disease (CVD). Also, Hcy is associated with (but not limited to) cancer, aortic aneurysm, hypothyroidism and end renal stage disease to mention some. The circulating levels of Hcy can be increased by defects in enzymes of the metabolism of Met, deficiencies of vitamins B6, B12 and folate or by feeding Met enriched diets. Additionally, some of the pharmaceuticals currently in clinical practice such as lipid lowering, and anti-Parkinsonian drugs are known to elevate Hcy levels. Studies on supplementation with folate, vitamins B6 and B12 have shown reduction in Hcy levels but concomitant reduction in certain associated pathologies have not been definitive. The enormous importance of Hcy in health and disease is illustrated by its prevalence in the medical literature (e.g. > 22,000 publications). Although there are compelling data in favor of Hcy as a modifiable risk factor, the debate regarding the significance of Hcy mediated health effects is still ongoing. Despite associations between increased levels of Hcy with several pathologies being well documented, whether it is a causative factor, or an effect remains inconclusive. The present review though not exhaustive, is focused on several important aspects of Hcy metabolism and their relevance to health.

Serum level of homocysteine is correlated to carotid artery atherosclerosis in Chinese with ischemic stroke

Abstract:Objectives: To investigate the relationship between serum level of homocysteine (Hcy) and carotid artery atheroscleosis (CAA). Methods: Both sides of the common carotid artery and internal carotid artery in 126 Chinese patients with ischemic stroke were measured by B-mode ultrasound. The patients were divided into groups: normal, A, B, C and D according to the severity of CAAs. With fasting serum, Hcy as well as folate, vitamin B-12 and lipids were detected. The mean +/- SD age was 64 +/- 13 years ( range 39 - 87 years). Results: In a logistic regression model, the Hcy concentrations were associated with an elevated risk of CAAs independent of all traditional risk factors, and when CAAs became severer, the serum Hcy was higher. The levels of the normal group and A, B, C, D groups were 13.22 +/- 6.15 mu mol/l, 16.29 +/- 9.81 mu mol/l, 19.49 +/- 11.16 mu mol/l, 27.21 +/- 17.47 mu mol/l, and 24.14 +/- 8.64 mu mol/l, respectively. Rank test showed a significant difference between normal and other groups (p < 0.05). The levels of folate and vitamin B-12 were negatively correlated with the Hcy concentrations. Spearman correlation coefficient were -0.23 and -0.42 (p < 0.05). Conclusions: Hyperhomocysteinaemia is an independent risk factor of CAAs and the degree of CAAs is highly correlated with the level of Hcy in serum. The causes of hyperhomocysteinemia may be the result from the decrease in folate and vitamin B-12. Clinical trials are now required to evaluate the effect of treatment with these vitamins on the primary and secondary prevention of cerebral vascular diseases.
