Abstract:Fatigue occurs in every material under repetitive loading. Because bone is also being loaded under physiological condition, fatigue also occurs in bone and it is observed as microdamages under light microscope. However, in vivo bone is different from that ex-vivo. It is strongly suggests that in vivo microdamage is repaired by bone remodeling. Once generation and repair of microdamage is imbalanced, microdamage accumulates in bone and causes fatigue fracture.

It is suspected that insufficiency fractures of subtrochanteric femur in patients under long term bisphophonate treatment are caused by severely suppressed bone turnover.

Abstract:Fatigue microdamage accumulates in bone as a result of physiological loading. The damage is often manifested as microcracks, which are typically 50-100 mum long. These types of cracks develop in the interstitial bone and frequently abut osteon cement lines. In vitro experimentation has shown that an accumulation of fatigue damage reduces the material properties of bone (e.g., elastic modulus). An accumulation of fatigue damage has been implicated in the etiology of stress fractures and fragility fractures. However, bone has a remarkable ability to detect and repair fatigue microdamage. This article reviews the experimental techniques for identifying and quantifying different types of microdamage in bone, the density of in vivo microcracks at different skeletal locations, the effect of microdamage on bone material properties, the role of microdamage in bone fracture, and the biological mechanisms for the detection and repair of fatigue microdamage.

Abstract:Accumulation of microdamage during fatigue can lead to increased fracture susceptibility in bone. Current techniques for imaging microdamage in bone are inherently destructive and two-dimensional. Therefore, the objective of this study was to image the accumulation of fatigue microdamage in cortical bone using micro-computed tomography (micro-CT) with a barium sulfate (BaSO(4)) contrast agent. Two symmetric notches were machined on the tensile surface of bovine cortical bone beams in order to generate damage ahead of the stress concentrations during four-point bending fatigue. Specimens were loaded to a specified number of cycles or until one notch fractured, such that the other notch exhibited the accumulation of microdamage prior to fracture. Microdamage ahead of the notch was stained in vitro by precipitation of BaSO(4) and imaged using micro-CT. Reconstructed images showed a distinct region of bright voxels around the notch tip or along propagating cracks due to the presence of BaSO(4), which was verified by backscattered electron imaging and energy dispersive spectroscopy. The shape of the stained region ahead of the notch tip was consistent with principal strain contours calculated by finite element analysis. The relative volume of the stained region was correlated with the number of loading cycles by non-linear regression using a power-law. This study demonstrates new methods for the non-destructive and three-dimensional detection of fatigue microdamage accumulation in cortical bone in vitro, which may be useful to gain further understanding into the role of microdamage in bone fragility.

Abstract:Femoral neck fractures occurring in senior osteoporotic patients are closely related to bone fatigue. It is considered that the fatigue strength and durability of bone are decreased by the low metabolic activity in such cases. In this chapter, a theoretical analysis and an experimental study were carried out to estimate the progression of osteoporosis and the fatigue life of bone in living body. According to these studies, it was found that fatigue life is determined by the accumulation rate of mechanical damage and by self-healing activity. From these results, it is clear that remodeling activity varied with mechanical stimulation and that fatigue life is short in a high-frequency-cycle loading condition.
Spatiotemporal Characterization of Microstructure Morphology, Mechanical Properties and Bone Remodeling of Rat Tibia Under Uniaxial Compressive Overload Loading

Abstract:Bone tissue is subjected to increased mechanical stress during high-intensity work. Inadequate bone remodeling reparability can result in the continuous accumulation of microdamage, leading to stress fractures. The aim of this work was to investigate the characteristics and repair mechanisms of tibial microdamage under several degrees of overload. Also, we aimed at better understanding the effects of overload on the multi-scale structure and mechanical properties of bone. Sixty 5-month female rats were divided into three groups with different time points. Micro-CT was used to evaluate the three-dimensional microstructure, and three-point bending, quasi-static fracture toughness and creep mechanical test were carried out to evaluate the mechanical properties. SEM was used to observe the morphological characteristics of fracture surfaces. Section staining was used to count the microdamage parameters and numbers of osteoblasts and osteoclasts. The microarchitectures of cancellous and cortical bones in the three overload groups showed different degrees of damage. Overload led to a messy crystal structure of cortical bone, with slender microcracks mixed in, and a large number of broken fibers of cancellous bone. The properties associated with the elastic plasticity, fracture toughness, and viscoelasticity of cortical bone reduced in three groups, with that corresponding to day 30 presenting the highest damage. The accumulation of microdamage mainly occurred in the first 14 days, that is, the crack density peaked on day 14. Peak-targeted bone remodeling of cortical and cancellous bones occurred mainly between days 14 and 30. The influence of overload mechanical environment on bone quality at different time points was deeply investigated, which is of great significance for the etiology and treatment of stress fractures.
Mechanical impact of regional structural deterioration and tissue-level compensation on proximal femur trabecular bone

Abstract:Introduction: Osteoporosis-induced changes in bone structure and composition significantly reduce bone strength, particularly in the human proximal femur. This study examines how these changes affect the mechanical performance of trabecular bone to enhance diagnosis, prevention, and treatment strategies. Methods: A proximal femur sample was scanned using micro-CT at 40 μm resolution. Five regions of interest were selected within the femoral head, femoral neck, and greater trochanter. Structural models simulating various stages of osteoporosis were created using image processing software. Micro-finite element analysis evaluated the mechanical properties of trabecular bone under different conditions of structural deterioration and tissue-level elastic modulus variations. The combined effects of structural deterioration and tissue-level mechanical properties on trabecular bone mechanical performance were further analyzed. Results: The mechanical performance of trabecular bone generally follows a power-law relationship with its microstructural characteristics. However, in any specific region, the apparent mechanical properties linearly decrease with structural deterioration. The femoral neck and greater trochanter are more sensitive to structural deterioration than the femoral head. A 5% bone mass loss in the femoral head led to a 7% reduction in mechanical performance, while the femoral neck experienced a 12% loss. Increasing tissue-level elastic modulus improved mechanical performance, partially offsetting bone mass reduction effects. Conclusion: Trabecular bone in low bone mass regions is more affected by bone mass loss. Structural deterioration primarily reduces bone strength, but improvements in tissue-level properties can mitigate this effect, especially in early osteoporosis. Targeted assessments and interventions are crucial for effective management. Future research should explore heterogeneous deterioration models to better understand osteoporosis progression.
Finding of Microdamage Morphology Differences in Mouse Femoral Bones with Distinct Mineralization Levels

Abstract:Microdamage progression in bone is dependent on the ultrastructure of the tissue. Thus, any pathological changes in bone ultrastructure may be reflected in the pattern and capacity of microdamage accumulation. A previous numerical study of microdamage progression in bone using a probabilistic failure analysis approach predicts that the microdamage morphology (either linear microcracks or diffuse damage) is very sensitive to the level of mineralization in bone, which is also implicated in some experimental observations. To examine the prediction, femurs from two strains of mice (C57BL/6J, N = 10 and C3H/HeJ, N = 11) that have distinct mineralization levels were fatigued under four-point bending to create damage in the bone tissues. After testing, the microdamage morphology of the bone samples was examined using bulk-staining technique with basic fuchsin. The results demonstrate that more linear microcracks are observed in femurs of C3H/HeJ (higher mineralization), whereas more diffuse-like damage is found in C57BL/6J femurs (less mineralized). Compared with linear microcracks, the formation of diffuse damage tends to dissipate more energy and help bone to avoid catastrophic failures. Therefore, results from this study may help explain why highly mineralized bone tends to be more brittle. Observations from this study are consistent with the numerical prediction from the previous study, suggesting that mineralization has a significant effect on the microdamage morphology of bone.

Abstract:Most bones are anisotropically loaded and seem to be adapted to the anisotropic stress or strain field by changing the anisotropy in their microstructure. Osteocyte (OCY) is believed to play an important role as a mechanosensor and regulator of modeling and/or remodeling orchestrating osteoblast and osteoclast activity to make bone suitable to resist the mechanical environment. In general, osteocytes sense magnitude of stress (strain) applied upon the bone and then work as a trigger to change bone mass to adjust bone’s mechanical function to the stress field. This structural optimization is an important aspect of the bone functional adaptation; another inevitable optimization might be achieved through the change in intrinsic material anisotropy including the preferential c -axis orientation of biological apatite (BAp) crystal. To achieve this adaptation through material anisotropy, osteocyte needs to be a mechanosensor which can detect anisotropic stress field. In the present study, osteocyte lacunae and canaliculi in the mid-diaphysis and the distal part of the rat femur were stained by a fluorescein dye for visualization and analysis. The mid-diaphysis shows greater degree of the preferential c -axis orientation of BAp crystal than the distal part in relation to the magnitude of uni-axial stress field. It was found that the osteocytes in long bone preferentially align along the bone long axis and the degree of alignment is greater in the mid-diaphysis than in the distal region, which seems to be effective for the sensation of the site-dependent specific stress field applied on the long bone.

Abstract:Microdamage formation in bone is dependent on its ultra-structural and material properties. Previous studies of probabilistic failure analysis of mineral-collagen composites have indicated that mineral modulus affects the formation of microdamage in bone [1]. Therefore, the hypothesis of this study is that mineralization influences the morphology of microdamage in bone. To test this hypothesis, bone femurs from two types of mice (C57BL/6J and C3H/HeJ) with various bone mineral density [2] were fatigued to produce microdamage and the differences of microdamage morphology were then examined. In addition, nanoindentation techniques were employed to assess the difference of mineralization between these two mice strains.

Abstract:Quasi-brittle fracture mechanics is used to evaluate fracture of human cortical bone in aging. The approach is demonstrated using cortical bone bars extracted from one 92-year-old human male cadaver. In-situ fracture mechanics experiments in a 3D X-ray microscope are conducted. The evolution of the fracture process zone is documented. Fully developed fracture process zone lengths at peak load are found to span about three osteon diameters. Crack deflection and arrest at cement lines is a key process to build extrinsic toughness. Strength and toughness are found as size-dependent, not only for laboratory-scale experimental specimens but also for the whole femur. A scaling law for the length fracture process zone is used. Then, size-independent, tissue fracture properties are calculated. Linear elastic fracture mechanics applied to laboratory beam specimens underestimates the tissue toughness by 60%. Tissue fracture properties are used to predict the load capacity of the femur in bending within the range of documented data. The quasi-brittle fracture mechanics approach allows for the assessment of the combined effect of bone quantity and bone quality on fracture risk. However, further work is needed considering a larger range of subjects and in the model validation at the organ length scale.

Methods for assessing bone quality: a review

Abstract:Background: Bone mass, geometry, and tissue material properties contribute to bone structural integrity. Thus, bone strength arises from both bone quantity and quality. Bone quality encompasses the geometric and material factors that contribute to fracture resistance. Questions/purposes: This review presents an overview of the methods for assessing bone quality across multiple length scales, their outcomes, and their relative advantages and disadvantages. Methods: A PubMed search was conducted to identify methods related to bone mechanical testing, imaging, and compositional analysis. Using various exclusion criteria, articles were selected for inclusion. Results: Methods for assessing mechanical properties include whole-bone, bulk tissue, microbeam, and micro- and nanoindentation testing techniques. Outcomes include structural strength and material modulus. Advantages include direct assessment of bone strength; disadvantages include specimen destruction during testing. Methods for characterizing bone geometry and microarchitecture include quantitative CT, high-resolution peripheral quantitative CT, high-resolution MRI, and micro-CT. Outcomes include three-dimensional whole-bone geometry, trabecular morphology, and tissue mineral density. The primary advantage is the ability to image noninvasively; disadvantages include the lack of a direct measure of bone strength. Methods for measuring tissue composition include scanning electron microscopy, vibrational spectroscopy, nuclear magnetic resonance imaging, and chemical and physical analytical techniques. Outcomes include mineral density and crystallinity, elemental composition, and collagen crosslink composition. Advantages include the detailed material characterization; disadvantages include the need for a biopsy. Conclusions: Although no single method can completely characterize bone quality, current noninvasive imaging techniques can be combined with ex vivo mechanical and compositional techniques to provide a comprehensive understanding of bone quality.
Non-Invasive Analysis of the Micro-Structural and Biomechanical Properties of Trabecular Bone in Human Femoral Head

Abstract:This study investigates micro-structural and mechanical properties of trabecular bone in human femoral head with and without osteoporosis using a micro-CT and a finite element model. 15 cored trabecular bone specimens with 20 of diameter were obtained from femoral heads with osteoporosis resected for total hip arthroplasty, and 5 specimens were removed from femoral head of cadavers, which has no history of musculoskeletal diseases. A high-resolution micro-CT system was used to scan each specimen to obtain histomorphometry indexes. Based on the micro-images, a FE-model was created to determine mechanical property indexes. While the non-osteoporosis group had increases the trabecular thickness, the bone volume, the bone volume fraction, the degree of anisotropy and the trabecular number compared with those of osteoporotic group, the non-osteoporotic group showed decreases in trabecular separation and structure model index. Regarding the mechanical property indexes, the reaction force and the Young's modulus were lower in the osteoporotic group than in non-osteoporotic group. Our data shows salient deteriorations in trabecular micro-structural and mechanical properties in human femoral head with osteoporosis.

Abstract:Age-related bone fractures are a major health concern to the elderly population. In addition to the loss of bone mass, the deterioration of bone quality is another major reason for such fractures. The decline of bone quality is manifested with the accumulation of microdamage in bone with age [1]. Two major types of microdamage have been observed in bone tissue: linear microcracks and diffuse damage [2]. Linear microcracks are individual cracks at a size of microns or larger and are usually visible under an optical microscope. On the other hand, diffuse damage is detectable only by staining and consists of an extensive network of fine, ultrastructural-level defects. The mechanisms for the formation of these two distinct types of microdamage in bone are still unclear. In this study, probabilistic finite element models of mineral-collagen composites were used to study the progression of microdamage in bone, thereby exploring the conditions under which linear microcracks and diffuse damage are produced in bone.Copyright © 2008 by ASME
Effect of the Material Properties and Knee Position to the Bone Bruise Pattern in Skeletally Mature and Immature Subjects

Abstract:A bone bruise is generated by a bony collision that could occur when the anterior cruciate ligament (ACL) is injured, and its pattern reflects the injury mechanism and skeletal maturity. Thus, the bone bruise pattern is useful to predict a subject-specific injury mechanism, although the sensitivity and/or effect of the material property and the knee position at injury is still unclear. The objective of the present study was to determine the effect of the material property and knee position on the bone bruise pattern in skeletally mature and immature subjects using finite element analysis. Finite element models were created from a magnetic resonance (MR) image in the sagittal plane of a skeletally mature (25 y. o.) and immature (9 y. o.) male subject. The femur and tibia were collided at 2 m/s to simulate the impact trauma and determine the maximum principal stress. The analysis was performed at 15, 30, and 45 deg of knee flexion, and neutral, 10 mm anterior and posterior translated position at each knee flexion angle. Although high stress was distributed toward the metaphysis area in the mature model, the stress did not cross the growth plate in the immature model. The size of the stress area was larger in the mature model than those in the immature model. The location of the stress area changed depending on the joint position. Young's modulus of cartilage and trabecular bone also affected the location of the stress area. The Young's modulus for the cartilage affected peak stress during impact, while the size of the stress area had almost no change. These results indicate that the bone bruise pattern is strongly associated with subject-specific parameters. In addition, the bone bruise pattern was affected not only by knee position but also by tissue qualities. In conclusion, although the bone bruise distribution was generally called footprint of the injury, the combined evaluation of the quality of the structure and the bone bruise distribution is necessary for properly diagnosing tissue injury based on the MR imaging.

Abstract:Strength of bones depends on bone matrix volume (BMV), bone microarchitecture, and also on the degree of mineralization of bone (DMB). We have recently shown in osteoporotic patients treated with alendronate that fracture risk decreased and bone mineral density increased with a parallel increase of the DMB due to prolonged secondary mineralization but without modifications of BMV or bone microarchitecture. DMB and strength were both measured at the tissue level in calcaneus bone samples taken at autopsy from 20 subjects (aged 78 +/- 8 years, 8 women, 12 men) who died suddenly without apparent bone disease. DMB parameters measured on microradiographs (mean DMB, distribution of DMB, most frequent maximum DMB value, and width at half maximum, an index reflecting the homogeneity of DMB) were compared with those reported in iliac cancellous bone samples of 43 human bones. Histomorphometric measurements of microarchitectural parameters (TbTh, TbN, and TbSp) were also measured. Compression tests were performed on contiguous samples of the same calcaneus on a universal screw-driven machine (Schenck RSA 250). A 5000-N load cell (TME, F 501 TC) measured the compressive load. The displacement was measured directly on the sample using a specific displacement transducer developed by the <<Laboratoire de Mécanique des Contacts et des Solides (LaMCoS).>> The apparent Young's modulus (E), the maximal strength (sigma(max)), and the work (W) until failure were measured. In human cancellous bone tissue, mean DMB (+/- SD) was higher in calcaneus (1.135 +/- 0.147 g/cm(3)) than in iliac crest (1.098 +/- 0.077 g/cm(3)). The mean most frequent maximum DMB values (mean DMB freq. max.) were 1.118 +/- 0.175 g/cm(3) in calcaneus and 1.108 +/- 0.095 g/cm(3) in iliac samples, and DMB was more heterogeneous in calcaneus than in iliac samples (mean width at half maximum were 0.270 +/- 0.127 versus 0.227 +/- 0.056 g/cm(3), respectively). Compression tests revealed significant positive linear correlations between DMB and both elastic modulus (r(2) = 0.69) and maximal strength (r(2) = 0.69). Correlations with DMB persisted (P < 0.003) even after adjustment for both calcified bone volume, for the Young's modulus (E), the maximal strength (sigma(max)) (r(2) = 0.44 and 0.41, respectively), and microarchitectural parameters (0.50 < r(2) < 0.56, P < 0.001). The same results were obtained with the work to fracture (W) (0.23 < r(2) < 0.46, P < 0.045). We conclude that the more the cancellous tissue was mineralized, the higher was its stiffness and compressive strength. This may explain the increase in bone strength when DMB is modified in a physiological range without necessary changes of BMV and bone microarchitecture. The impact of such modifications on fracture risk and the therapeutic implications of these data remain to be analyzed.

Abstract:Osteoporosis is a disease defined by decreased mass and alteration of microarchitecture which results in increased fragility and increased risk of fracture.The major complication of osteoporosis,i.e.,fracture,is due to a lower strength.Bone strength depends not only on the quantity of mineral but also on the mineral density(BMD) measured by dual-energy X-ray absorptiometry(DXA) which is used for osteoporosis diagnosis,reflects the quantity of mineral.Several other properties that influence strength have been defined as bone quality.Bone quality includes remodeling(bone turnover),bone geometry,bone microstructure,bone matrix,bone mineralization,bone collagen,bone microdamage and its repair.Studies have shown that quality influences osteoporotic fractures from these above aspects.This review aims to update the concepts about quality,its relationship with biomechanics and how it influences osteoporotic fracture.

Abstract:In vivo quantitative techniques for assessing the microstructure of trabecular bone noninvasively and non-destructively include high-resolution CT and magnetic resonance (MR). Compared with MR imaging, CT-based techniques have the advantage of directly visualizing the bone in the axial skeleton, with high spatial resolution, but the disadvantage of delivering a considerable radiation dose. The evaluation of microstructure is useful to investigate the biomechanical property, and to assess the efficacy of anti-osteoporotic agents. Recently, cortical microstructure has been recognized important in relation to biomechanics. Further progress in bone imaging technology is promising to bring new aspects of bone structure in the every day clinical setting.

Abstract:Finite element models of the thorax are under development to assist vehicle safety researchers with the design of countermeasures such as advanced restrain systems. Computational models have become more refined with increasing geometrical complexity as element size decreases. These finite element models can now capture small geometrical features with an attempt to predict fracture. However, the bone material properties currently available, and in particular the rate sensitivity, have been mainly determined from compression tests or tests on long bones. There is a need for a new set of material properties for the human rib cortical bone. With this objective, a new clamping technique was developed to test small bone coupons under tensile loading. Ten coupons were harvested from the cortical shell of the sixth and seventh left ribs from three cadavers. The coupons were tested to fracture under quasi-static (target strain rate of 0.07 %/s) and dynamic loading (target strain rate of 170 %/s). Prior to testing, each coupon was imaged with a computed micro-tomograph to document the bone microstructure. An optical method was used to determine the strain field in the coupon for the quasi-static tests. The rib bone coupons were found to be elastic, with brittle fracture. No plastic behavior was observed in this test series. The bone coupons were assumed isotropic, homogeneous and elastic linear, and the average Young's modulus for the quasi-static tests (13.5 GPa) and the failure stresses (quasi-static: 112 MPa, dynamic: 124.6 MPa) were in line with published data. Fracture however did not always occur in the gage area where the cross-sectional area was the smallest, which contradicted the assumption of isotropy and homogeneity. The comparison with material properties obtained for long bones suggests that the effective cross-section has an effect on the calculated material properties.
Relationship between the microstructural energy release rate of cortical bone and age under compression condition

Abstract:Most studies evaluated the energy release rate of cortical bone macrostructure under Mode I, Mode II, and mixed Mode I-II loading conditions. However, testing the macrostructural energy release rate requires an initial crack and recording the applied load and the corresponding crack length in real-time, which may introduce measurement errors and differences with the actual fracture scenarios. To further understand how the energy release rate contributed to the cortical bone fracture characteristics, this study predicted the microstructural energy release rate of cortical bone and then investigated its age-related varitions. The microstructural energy release rate of femoral cortical bone in rats from different ages was back-calculated by fitting the experimental and simulated load–displacement curves under compression load. The trends in the microstructural energy release rate were revealed, and the underlying reasons for the age-related changes were investigated by integrating the discussion on the cortical bone mechanical parameters at various levels obtained from the previous experiment. The predicted microstructural energy release rate of femoral cortical bone in the rats from 1, 3, 5, 7, 9, 11, and 15 months of age were in the range of 0.08–0.12, 0.12–0.14, 0.15–0.19, 0.25–0.28, 0.23–0.25, 0.19–0.22, and 0.13–0.16 N/mm, respectively. The statistical analyses showed the significant differences in the microstructural energy release rate at different ages. The results indicated an increasing trend followed by a decrease from 1 to 15 months of age, and the correlations between microstructural energy release rate and age were significant. The age-related variations in the microstructural energy release rate may be linked to the changes in the microarchitecture, and the fracture load is influenced by the micro-level mechanical parameters. Notably, the age-related trends in microarchitecture and energy release rate were similar. These findings were valuable for understanding the mechanism underlying the weakening mechanical properties of cortical bone microstructure with age from an energy perspective.
