Abstract:Natural killer (NK) cells are an important component of innate immune responses to infectious diseases. They mediate protection by being able to rapidly lyse infected cells and produce cytokines (primarily interferon-γ) that shape innate and adaptive immune responses. This review summarizes current knowledge on the phenotype and functional abilities of NK cells from healthy newborns/infants and on NK cell responses against viral, bacterial and protozoan infections in early life. Interestingly, NK cell blood counts are higher in newborns than in adults but they do not display striking differences in phenotype, except for an increased frequency of expression of the inhibitory CD94/NKG2A receptor. They display some inherent functional defects, mainly a lower cytolytic capacity that may contribute to the immaturity of the neonatal immune system. Changes in circulating levels of NK cells observed during pediatric infections and the ability of NK cells from newborns and children to produce interferon-γ at the encounter with pathogens indicate that NK cells participate in the immune response to infectious diseases in early life. Unfortunately, information is currently insufficient to assess whether these NK cell responses really contribute to control infections, either vertically transmitted or acquired in infancy.
Impaired NK cell antiviral cytokine response against influenza virus in small-for-gestational-age neonates

Abstract:The neonates, particularly small-for-gestational-age (SGA) ones, are susceptible to various microbial infections. Natural killer (NK) cells are critical components of host innate immunity system and the main source of the inflammatory cytokines, which provide critical protection during the early phase of viral infections before the development of an appropriate adaptive immune response. However, little is known about the antiviral effects of NK cells in neonates especially the SGA population. Herein, a prospective descriptive study was performed to determine the differences of NK cell immunity among adults, appropriate-for gestational-age (AGA) and SGA neonates. Adults have much higher NK cell number in peripheral blood than that in cord blood from neonates. In response to influenza virus stimulation, neonatal NK cells, especially SGA baby cells, expressed significantly lower antiviral cytokines including perforin, interferon (IFN)-γ and tumor-necrosis factor (TNF)-α responses than adult NK cells. In addition, the antiviral cytokine responses of NK cells were positively correlated with neonatal birth weight. Our data suggested that the depressed antiviral activity and less frequency of NK cells are likely to be responsible for the high susceptibility to microbial infection in neonates, at least in part. Improving the function of innate immunity may provide a new way to defend virus infection.

Abstract:To determine the relationship between total serum bilirubin (TSB) during the first 2 days of life and subsequent neonatal morbidity in very low birth weight (VLBW, less than 1500 g) infants. We performed a prospective study of 582 VLBW infants born between July 1, 2005 and December 31, 2009. TSB was measured in umbilical cord blood (UCB), at 24 and 48 h after birth. Demographic and clinical characteristics of infants in hospital were recorded. The interaction between TSB variables during the first 48 h of life and subsequent neonatal morbidity were assessed in logistic regression analyses adjusted for multiple risk factors. It was found that TSB in UCB was in a negative correlation with occurrence of respiratory distress syndrome (RDS) [OR 0.626, 95% confidence interval (95% CI): 0.446–0.879, p = 0.007], and there was also a negative correlation between TSB in UCB and occurrence of intraventricular hemorrhage (IVH) [OR 0.695, 95% CI 0.826–0.981, p = 0.020]. However, TSB in UCB positively correlated with hyperbilirubinemia [OR 2.471, 95% CI 1.326–3.551, p = 0.012], and TSB at 24 h after birth was also in a positive correlation with early onset sepsis (EOS) [OR 1.299, 95% CI 1.067–1.582, p = 0.011]. VLBW infants with low TSB levels in UCB were more likely to develop RDS and IVH, and those with low TSB levels in UCB were less likely to develop hyperbilirubinemia. Infants with high TSB levels at 24 h after birth were more likely to develop EOS. The protective effect of raised TSB in UCB with respect to RDS and IVH warrants further investigation.

Abstract:Natural killer cell activity was found to be depressed from the early period of pregnancy to the third trimester, as compared to that in the nonpregnant state. Natural killer cell activity in the puerperium tended to be depressed slightly more than during pregnancy. There were no differences in activity between toxemic and normal pregnant women in the same gestational week. A retrospective survey showed a negative correlation between natural killer cell activity during pregnancy and the birth weight of the baby. Although the role of natural killer cells during pregnancy needs to be more clearly elucidated, they may be involved in fetal growth.

Abstract:Objective To explore neutrophil functions of neonates at the molecular level.Methods Neutrophil CD11b median fluorescence intensity(MFI) in 33 normal full term neonates was measured by using whole blood method with flow cytometry and dir ect immunofluorescence technique. 10 healthy adults were measured as controls.Results The neutrophil CD11b MFI in cord blood of vaginally delivered (VD) or in adult venous blood was much lower than that in cord blood of elective caesarian section (ECS) or in neonatal venous blood (all P0.05). There was no significant difference in CD11b MFI between VD cord blood and adult venous blood (P0.05).Conclusions 1.The mode of delivery affects neutrophil CD11b expression of neonates.2.CD11b expression on resting neutrophils of full term neonates is mature.

Abstract:Several immune cell populations are transferred to the newborn through breast milk, including natural killer (NK) cells, which are critical for innate defense and regulation of the immune response, especially in preterm infants. The aim of this study was to analyze the presence of NK cell subpopulations in different types of preterm breast milk. The study quantified the presence of NK cell subpopulations by flow cytometry using the relative expression of CD56 and CD16 markers in colostrum, transitional and mature milk samples from preterm mothers. Flow cytometry analysis revealed the presence of five NK cell subpopulations, but unlike those reported in peripheral blood, CD56 dim CD16 + and CD56 - CD16 + populations are predominantly present in preterm milk, only the CD56 bright CD16 dim population is increased in mature milk. Analysis of NK cell subpopulations in preterm milk revealed a pattern of NK cell presence in preterm breast milk with predominantly cytotoxic phenotypes in relation to CD16 marker expression.

Natural killer cell activity and delivery: possible influence of cortisol and anesthetic agents. A study on newborn cord blood

Abstract:The aim of this study was to evaluate the influence of the method of delivery, the level of cord blood lidocaine, and the cortisol concentration on the cord blood natural killer (NK) activity in the full-term healthy newborn. We studied healthy newborns delivered by elective cesarean section without labor under general anesthesia (n = 24), delivered by cesarean section under epidural anesthesia (n = 21), and delivered vaginally with uncomplicated labor (n = 19). The NK cell activity was significantly lower in newborns delivered by cesarean section under epidural anesthesia than it was in the general anesthesia group, while it was similar to the levels found in vaginally delivered newborns. The cortisol concentration was highest in the vaginal delivery group (589.2 +/- 200 mmol/l) and lowest in the general anesthesia group (199.2 +/- 81.9 mmol/l). The mean serum lidocaine concentration was 414.1 +/- 370 microgram/l in the epidural anesthesia group and undetectable in the other groups. In conclusion, our data suggest that the cord blood NK activity was significantly influenced by the method of delivery. This effect could be related to anesthetics given to the mother for general or epidural anesthesia or to the endocrine-metabolic variations observed after different degrees of delivery-related stress. The NK cells being a first-line defense mechanism against viral infections, the results of this study suggest an association with the occurrence of early perinatal infections, especially in preterm infants.

Abstract:Introduction The immune systems of both the mother and the newborn face significant challenges during birth. Proper immune regulation after birth is essential for the survival of neonates. Numerous studies have demonstrated that the neonatal immune system is relatively immature, particularly in its adaptive arm, placing the primary responsibility for immune surveillance on innate immunity. Methods Given the significant role of neutrophils in protecting the neonate after birth, we conducted a study investigating the properties of neutrophils in newborn cord blood using various methodological approaches. Results Our findings demonstrate the presence of immature low-density neutrophils in the cord blood, which are likely responsible for the observed elevated expression of genes coding for proteins essential to antimicrobial response, including myeloperoxidase, neutrophils elastase, and defensins. Discussion We propose that these cells function normally and support the protection of newborns early after birth. Furthermore, our results suggest that the mode of delivery might significantly influence the programming of neutrophil function. The presented findings emphasize the importance of distinct neutrophil subpopulations in neonatal immunity and their potential impact on early postnatal health.

Evaluation of natural killer cells as diagnostic markers of early onset neonatal sepsis: comparison with C-reactive protein and interleukin-8

Abstract:This study was conducted on thirty-seven neonates and healthy neonates (sixteen full term and fourteen preterm). The study aimed at revealing the role played by the NK cells in neonatal sepsis and evaluating the sensitivity of NK cell number and cytotoxicity as diagnostic markers in infants with suspected early neonatal sepsis compared with the circulating cytokine IL-8 and CRP levels. All samples of peripheral blood lymphocytes were subjected to determination of CD16 and CD56 positive cells using flow cytometry and NK cytotoxicity using the standard 4h 51Cr release assay. Sera were separated to measure IL-8 using ELISA. Determination of CRP, using turbdimetric assay, as well as blood cultures were done for patient's group only. Out of the 37 cases of suspected early neonatal sepsis, 16 were given final diagnosis of sepsis. Seven infants (43.8%) in the sepsis group had culture-proven diagnosis, one of which had meningitis. The median CRP value was significantly higher in sepsis group (88 mg/L; range: 17-159 mg/L) compared with that in non-septic group (15.4 mg/L; range: 7.6-23.2 mg/L, p < 0.001) only 12-60 h after admission. On the other hand, newborns in the sepsis group had significantly higher serum levels of IL-8 (median 310 pg/mL; range: 37-583 pg/ml) at study entry than that in the non septic group (median 63 pg/mL; range: 32-94 pg/ml, P < 0.001). On admission, the NK activity, rather than the number of CD16 and CD56 positive cells was much affected where NK cytotoxicity was significantly lower in sepsis group (3.4 +/- 2.1%, range 0.9-7%) than that of the nonseptic group (18.3 +/- 6.7%: range 10.7- 25.3%, p < 0.01) and healthy neonates (23.8 +/- 4.7%: range 12.2-32.3%, p < 0.001). We may conclude that defective NK cell activity rather than NK cell number plays an important role in susceptibility to early onset neonatal sepsis. Evaluation of NK cytotoxicity as a marker in early diagnosis of neonatal sepsis reveals that the sensitivity, specificity and predictive values of reduced NK cytotoxicity (10% killing) was higher than both of CRP and IL-8, either individually or in combination. Additionally, reduced NK cytotoxicity showed high correlation with the severity and outcome of neonatal sepsis. Our data raise the possibility that the addition of NK cell activity to the standard work-up of critically ill patients with suspected sepsis could increase diagnostic certainty and generate an improved patient management.
ThiNet: A Filter Level Pruning Method for Deep Neural Network Compression

Abstract:We propose an efficient and unified framework, namely ThiNet, to simultaneously accelerate and compress CNN models in both training and inference stages. We focus on the filter level pruning, i.e., the whole filter would be discarded if it is less important. Our method does not change the original network structure, thus it can be perfectly supported by any off-the-shelf deep learning libraries. We formally establish filter pruning as an optimization problem, and reveal that we need to prune filters based on statistics information computed from its next layer, not the current layer, which differentiates ThiNet from existing methods. Experimental results demonstrate the effectiveness of this strategy, which has advanced the state-of-the-art. We also show the performance of ThiNet on ILSVRC-12 benchmark. ThiNet achieves 3.31 x FLOPs reduction and 16.63× compression on VGG-16, with only 0.52% top-5 accuracy drop. Similar experiments with ResNet-50 reveal that even for a compact network, ThiNet can also reduce more than half of the parameters and FLOPs, at the cost of roughly 1% top-5 accuracy drop. Moreover, the original VGG-16 model can be further pruned into a very small model with only 5.05MB model size, preserving AlexNet level accuracy but showing much stronger generalization ability.

Abstract:Human cytomegalovirus (CMV) is a common herpesvirus causing lifelong latent infection in most people and is a primary cause of congenital infection worldwide. Given the role of NK cells in the materno-fetal barrier, we investigated peripheral blood NK cell behavior in the context of CMV infection acquired during pregnancy. We analyzed the NK phenotype and CD107a surface mobilization on PBMCs from CMV-transmitting and non-transmitting mothers and newborns with or without congenital infection. NK cells from non-transmitting mothers showed the typical phenotype of CMV-adaptive NK cells, characterized by higher levels of NKG2C, CD57, and KIRs, with reduced NKG2A, compared to transmitting ones. A significantly higher percentage of DNAM-1+, PD-1+, and KIR+NKG2A-CD57+PD-1+ CD56dim cells was found in the non-transmitting group. Accordingly, NK cells from congenital-CMV (cCMV)-infected newborns expressed higher levels of NKG2C and CD57, with reduced NKG2A, compared to non-congenital ones. Furthermore, they showed a significant expansion of CD56dim cells co-expressing NKG2C and CD57 or with a memory-like (KIR+NKG2A-CD57+NKG2C+) phenotype, as well as a significant reduction of the CD57-NKG2C- population. Degranulation assays showed a slightly higher CD107a geomean ratio in NK cells of mothers who were non-transmitting compared to those transmitting the virus. Our findings demonstrate that both CMV-transmitting mothers and cCMV newborns show a specific NK profile. These data can guide studies on predicting virus transmission from mothers and congenital infection in infants.

Abstract:Objective To analyze the nutrition and clinical status in very low birth weight infant (VLBWI) in hospital. Methods Seventy VLBWI were divided into two groups: gestational age less than 30 weeks (31 cases in group 1) and gestational age more than 30 weeks (39 cases in group 2). Their clinical data were retrospectively analyzed. Results Weight loss,days to regain birth weight were much higher in goup 1 than that in group 2. [9.6%±5.0% vs 6.9%±3.76%,(11.3±3.4) d vs (8.6±3.6) d]. Weight gain in the first three weeks were much lower in group 1 than that in group 2 [(-11.3±13.3) g/d vs (-4.3±14.7) g/d,(7.9±12.1) g/d vs (15.6±12.8) g/d,(16.9±12.6) g/d vs (22.7±8.2) g/d,P<0.05]. The length of time on parenteral nutrition and enteral nutrition increments before meeting the recommended daily calories requirement for age were much longer in group 1 [(27.9±13.2) d vs (21.5±12.1) d,(35.1±16.1) d vs (27.6±9.4) d,P<0.05]. Apnea,hospital acquired infection and ventilation were more common in group 1(P<0.05). Incidence of extrauterine growth restriction(EUGR) was 64.3%. Multiple logistic regression analysis showed gestational age and duration of parenteral nutrition were risk factors of EUGR. Conclusion During hospitalization,most of the VLBW infants had problems with nutrition and growth. Birth weight with larger gestational age and the length of time on parenteral nutrition were risk factors of EUGR. Incidence of complications and ventilation were much higher in smaller gestational age infants.
Neonatal Outcomes of Very Low Birth Weight Infants in Korean Neonatal Network from 2013 to 2016

Abstract:BACKGROUND: This study was performed to determine survival and morbidity rates in very low birth weight infants (VLBWIs) in the Korean Neonatal Network (KNN), and to compare neonatal outcomes with those in other countries.METHODS: Data were collected for 8,269 VLBWIs with gestational age (GA) ≥ 22 weeks who were born between January 1, 2013 and December 31, 2016, and admitted to the neonatal intensive care units of the KNN.RESULTS: The survival rate of all VLBWIs and of infants with GA 22-23, 24-25, 26-27, 28-29, 30-32, and > 32 weeks were 86% (total), 33%, 65%, 84%, 94%, 97%, and 98%, respectively. The bronchopulmonary dysplasia (BPD) rates of all VLBWIs and of infants with GA 22-23, 24-25, 26-27, 28-29, 30-32, and > 32 weeks were 30% (total), 88%, 64%, 47%, 26%, 14%, and 5%, respectively. The intraventricular hemorrhage rates (≥ grade III) of all VLBWIs and of infants with GA 22-23, 24-25, 26-27, 28-29, 30-32, and > 32 weeks were 10% (total), 45%, 27%, 12%, 5%, 2%, and 1%, respectively. In an international comparison, the survival rate of VLBWIs with GA 24-27 weeks in KNN was lower, and the BPD rate of VLBWIs in the KNN was higher than that of the neonatal networks of other countries.CONCLUSION: Despite overall improvements in neonatal outcomes, the survival and morbidity rates of more immature infants with GA 22-27 weeks need further improvement. Therefore, it would be necessary to develop more optimal treatment strategies and perform more active quality improvement to further improve neonatal outcomes of VLBWIs in Korea.

Abstract:Cytomegalovirus (CMV) poses risk of significant morbidity, including sepsis-like syndrome, hepatic dysfunction, and cytopenias [1], in very low birth weight (VLBW, <1500 g) infants. CMV positive breastmilk is an important source of neonatal infection [2]. Despite the high prevalence of CMV in breastmilk, rates of acquired postnatal CMV (pCMV) infection in VLBW infants are low [3]. Several breastmilk factors have been associated with higher rates of CMV transmission, including overall viral load [2]; however, infant host factors that explain the development of pCMV are unclear. One possible host factor mediating neonatal infection is cellular immune competency. Since cellular immunity depends upon normal T-cell development, neonatal immune function can be partially measured via T-cell Receptor Excision Circles (TRECs), which are routinely assayed by newborn screening (NBS). We hypothesized that lower copy numbers of TRECs may be associated with an increased risk of pCMV infection.

Abstract:The production of IL-2, IL-6 by fetal splenic mononuclear cells (FSMC) and relationship between them and the ontogenetic development of natural killer cell function were studied in human fetal spleens. As a results, before 20 weeks of gestation, both IL-2 and NK cell activities were not measured, but IL-6 was done. It was found that IL-2, IL-6 and NK cell activities were increased with the gestational age, and shown that there were linear positive correlation between the activities of three ones above (r > 0.86). Before the birth, the induced IL-2 activity was the same as adult levels (p > 0.05), although both IL-6 production and NK activity in fetal spleens were significantly lower than that in adults (p < 0.01). Lastly, the production of IL-2, IL-6 in relation to the functional development of NK cells during the embryonic development was discussed.

Abstract:Abstract Background: Very low birth weight (VLBW) infants are the key populations in neonatology, wherein morbidity and mortality remain major challenges. Methods: A retrospective cohort study conducted aiming to analyze the clinical characteristics of VLBW in our hospital between January 2016 and December 2021. Neonates with a birth weight of <1500 g were included. Mortality, care practices, and major morbidities were analyzed, and compared with that of previous 7 years (2009–2015). Results: Of the total 1750 VLBW, 1386 infants born with birth weight between 1000–1499 g and 364 were below 1000 g, 42.9% (751/1750) required delivery room resuscitation, 53.9% (943/1750) received non-invasive ventilation only, 38.2% (669/1750) received invasive ventilation; 1517 VLBW infants received complete treatment. Among them, 60.1% (912/1517) of neonates had neonatal respiratory distress syndrome (NRDS), 28.7% (436/1517) had bronchopulmonary dysplasia (BPD), 22.0% (334/1517) had apnea, 11.1% (169/1517) had culture-confirmed sepsis, 8.4% (128/1517) had pulmonary hemorrhage, 7.6% (116/1517) had severe intraventricular hemorrhage (IVH)/periventricular leukomalacia (PVL), 5.7% (87/1517) had necrotizing enterocolitis (NEC), 2.0% (31/1517) had severe retinopathy of prematurity. The total and in-hospital mortality rates were 9.7% (169/1750) and 3.0% (45/1517), respectively. The top three diagnoses of death among those who had received complete treatment were sepsis, NRDS, and NEC. In 2009–2015, 1146 VLBW were enrolled and 895 infants received complete treatment. The incidences of apnea, IVH, and IVH stage ≥3/PVL, were higher in 2009–2015 compared with those in 2016–2021, while the incidences of NRDS and BPD were characterized by significant increases in 2016–2021. The total and in-hospital mortality rates were 16.7% (191/1146) and 5.6% (50/895) respectively in 2009–2015. Conclusion: Among VLBW infants born in 2016–2021, the total and in-hospital mortality rates were lower than those of neonates born in 2009–2015. Incidences of NRDS and BPD increased in 2016–2021, which affected the survival rates and long-term prognosis of VLBW.

Abstract:Low birth weight (LBW) is a leading cause of newborn's mortality however the underlying defects in cellular immunity and immune mechanisms leading to severe neonatal infections in term LBW (tLBW) newborns are not well understood. Neutrophil extracellular traps (NETs), or NETosis, is an innate immune defense mechanism of neutrophils involved in trapping and killing of microbes. The efficiency of NET formation in cord blood derived neutrophils of tLBW and normal birth weight (NBW) newborns in the presence of toll like receptor (TLR) agonist inductions was evaluated. The NET formation was observed to be substantially impaired in tLBW newborns along with NET proteins expression, extracellular deoxyribonucleic acid (DNA) release and reactive oxygen species generation. The placental tissues from tLBW newborns delivery also showed minimal NETosis. These findings suggest impaired NET formation to be an important factor underlying the deficient immune status of tLBW newborns making them susceptible to life- threatening infections.
