Abstract:Objective: To determine how patient factors: age, sex, body mass index (BMI), clinical scores and physical exam findings, are associated with gait recovery after total hip arthroplasty (THA). Method: 145 subjects, who were evaluated with standard gait analysis, the Harris Hip Score (HHS), and a physical exam including passive range of motion (ROM), hip abductor strength assessment, before and after primary unilateral THA, were identified from an IRB-approved repository. Sagittal plane dynamic ROM and 3D peak external moments were averaged from operated-side normal-speed trials at each visit. We used linear regression analysis to evaluate the association among preoperative clinical factors and postoperative gait, with and without controlling for the influence of preoperative gait variables. Results: Sagittal and transverse plane moments, and the peak abduction moment seen in early stance, significantly improved after THA (p < 0.001, effect size d = 0.22-1.04). The peak adduction moment did not change significantly (p = 0.646), although the change ranged from -2.7 to + 4.0 %Body weight × height (-80% to +315%). Preoperative gait, clinical factors and patient characteristics predicted up to 33% of the variability in postoperative gait. Notably, greater preoperative abductor strength was associated with higher postoperative adduction and external rotation moments (R = 0.197-0.266, p < 0.05) after adjusting for age, sex, BMI and preoperative gait. Conclusion: Preoperative clinical factors predicted several specific aspects of objectively-characterized postoperative gait function. Physical exam findings can augment the predictive ability of clinical outcome measures, and potentially help guide rehabilitation plans.

Abstract:Aims Conventional patient-reported surveys, used for patients undergoing total hip arthroplasty (THA), are limited by subjectivity and recall bias. Objective functional evaluation, such as gait analysis, to delineate a patient’s functional capacity and customize surgical interventions, may address these shortcomings. This systematic review endeavours to investigate the application of objective functional assessments in appraising individuals undergoing THA. Methods The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines were applied. Eligible studies of THA patients that conducted at least one type of objective functional assessment both pre- and postoperatively were identified through Embase, Medline/PubMed, and Cochrane Central database-searching from inception to 15 September 2023. The assessments included were subgrouped for analysis: gait analysis, motion analysis, wearables, and strength tests. Results A total of 130 studies using 15 distinct objective functional assessment methods (FAMs) were identified. The most frequently used method was instrumented gait/motion analysis, followed by the Timed-Up-and-Go test (TUG), 6 minute walk test, timed stair climbing test, and various strength tests. These assessments were characterized by their diagnostic precision and applicability to daily activities. Wearables were frequently used, offering cost-effectiveness and remote monitoring benefits. However, their accuracy and potential discomfort for patients must be considered. Conclusion The integration of objective functional assessments in THA presents promise as a progress-tracking modality for improving patient outcomes. Gait analysis and the TUG, along with advancing wearable sensor technology, have the potential to enhance patient care, surgical planning, and rehabilitation. Abstract:Objectives: This study determines gender variation, comparing the significance level between men and women related to functional ambulation characteristics after hip arthroplasty. The study focuses on the broader female pelvis and how it affects the rehabilitation regimen following total hip arthroplasty. Materials and methods: In this cross-sectional study, 20 cases of right hip arthroplasty were divided into 10 male and 10 female cases, aged 40-65 years. The functional ambulation parameters (walking cadence, gait speed, stride length, and gait cycle time) were acquired from the GAITRite device, as well as kinematic values for hip frontal plane displacement and kinetic parameters for ground response force in the medial-lateral direction. Results: An independent t-test showed a significant difference in the kinematic parameter variables for the anterior superior iliac spine, more significant trochanter displacement, and hip abduction angle between the operated and non-operated limbs for each group separately. Regarding the functional ambulation parameters, there was a significant difference in the walking cadence between the operated and non-operated limbs of both male and female groups. Moreover, the output variables of ground reaction force measures revealed significant differences between their operated and non-operated limbs. The linear regression model used was consistent with the current results, demonstrating a weak negative correlation between the abduction angle of the operated hip and gait speed for both male and female groups. Conclusion: Based on the findings, we draw the conclusion that improving a rehabilitated physical therapy program for the abductors of both male and female patients' operated and non-operated limbs is essential for normalizing the ground reaction force value, avoiding focus on the operated hip, and reducing the amount of time that the operated hip's abductors must perform. This involves exposing the surgically repaired limb to the risk of post-operative displacement or dislocation, particularly in female patients.

Abstract:Background: Despite experiencing pain relief and improved activities of daily living after total hip arthroplasty (THA) for osteoarthritis of the hip, a Trendelenburg gait may be observed in some patients. The concentric and eccentric contraction patterns of hip muscles in a Trendelenburg gait are not well understood. Methods: This study included 89 patients (28 males and 61 females, mean age 66.5 ± 8.4 years, mean postoperative period 1.3 ± 0.4 years) after unilateral THA without functional impairment on the contralateral side. Gait analysis utilized a three-dimensional motion capture system to assess pelvis and hip angles, hip moment, and hip power. A Trendelenburg gait was defined as positive when nonoperative pelvic descent occurred at 30 % of the gait cycle, equivalent to mid-stance. Patients were classified into Trendelenburg gait-positive and -negative groups for statistical analysis. Unpaired t-test and chi-square test were used to compare the two groups. Multiple regression analysis was conducted to identify factors associated with the presence of a Trendelenburg gait. Results: A Trendelenburg gait was observed in 24 patients (27 %). Multiple regression analysis indicated that abduction (p < 0.01) and extension (p = 0.03) of hip joint power were significant determining of a Trendelenburg gait. Patients with a Trendelenburg gait exhibited reduced eccentric contraction of the hip abductor muscles and decreased concentric contraction of hip extensor muscles during early to mid-stance of their gait. Conclusion: Centrifugal contraction of hip abductor muscles and diminished eccentric contractility of hip extensor muscles appear crucial for hip stabilization mechanisms during gait after THA.

Gait analysis was performed the day before surgery, and at 6 and 12 weeks after surgery. Time-distance and kinematics analyses were performed by a recently proposed anatomically-based gait analysis protocol. A static double-leg stance and five walking trials at self-selected speeds were recorded on a 9-m walkway.FINDINGS: At 6 weeks follow-up, but in the DA group only, a statistically significant improvement with respect to preoperative status was observed for the percentage of single support and for the stride time. Between 6- and 12-week follow-up, the DA group showed a significant improvement in cadence, stride time and length, walking speed, hip flexion at foot contact, maximum hip flexion in swing, and hip total range of motion in the sagittal and the coronal planes. Between 6 and 12 weeks, the AL group showed significant improvements in opposite foot contact and step time, and in flexion at foot contact, maximum flexion in swing, and range of flexion at the hip joint.INTERPRETATION: Minimally invasive DA patients improved in a larger number of gait parameters than patients receiving the traditional AL approach. The majority of improvements occurred between the 6- and 12-week follow-ups.

Abstract:Although knee biomechanics has been examined, hip and ankle biomechanics in incline ramp walking has not been explored for patients with total knee arthroplasty (TKA). The purpose of this study was to investigate the hip and ankle joint kinematic and kinetic biomechanics of different incline slopes for replaced limbs and non-replaced limbs in individuals with TKA compared to healthy controls. Twenty-five patients with TKR and ten healthy controls performed walking trials on four slope conditions of level (0°), 5°, 10° and 15° on a customized instrumented ramp system. A 3x4 (limb x slope) repeated analysis of variance was used to evaluate selected variables. The results showed a greater peak ankle dorsiflexion angle in the replaced limbs compared to healthy limbs. No significant interactions or limb main effect for other ankle and hip variables. The peak dorsiflexion angle, eversion angle and dorsiflexion moment were progressively higher in each comparison from level to 15°. The peak plantarflexion moment was also increased with each increase of slopes. Both the replaced and non-replaced limbs of patients with TKA had lower hip flexion moments than the healthy control limbs. Hip angle at contact and hip extension range of motion increased with each increase of slopes. Peak hip loading-response internal extension moment increased with each increase in slope and peak hip push-off internal flexion moment decreased with each increase of slope. Our results showed increased dorsiflexion in replaced limbs but no other compensations of hip and ankle joints of replaced limbs compared to non-replaced limbs and their healthy controls during incline walking, providing further support of using incline walking in rehabilitation for patients with TKA

Abstract:This study investigates the impact of contralateral cane placement on the decrease in external knee adduction moment (KAM) and explores the underlying mechanisms. Ten healthy adults participated in a study to test three different lateral cane placements. The "Natural" placement corresponded to the participants' comfortable position, while the "Wide" and "Wider" placements were 10 cm and 20 cm further laterally outward, respectively. Results from repeated measures ANOVA indicated a significant decrease in KAM peaks with more lateral cane placements ( p < 0.01). This reduction was primarily due to a decreased moment arm, particularly at the second peak of KAM. The reduction in walking speed associated with more lateral cane placement had a negligible effect on KAM reduction. The decrease in the lever arm was caused by the displacement of knee joint to the medial side, which outweighed the medial rotation of the ground reaction force vector.

Abstract:Individuals with unilateral transtibial amputation (TTA) using socket prostheses demonstrate asymmetric joint biomechanics during walking, which increases the risk of secondary comorbidities (e.g., low back pain (LBP), osteoarthritis (OA)). Bone-anchored limbs are an alternative to socket prostheses, yet it remains unknown how they influence multi-joint loading. Our objective was to determine the influence of bone-anchored limb use on multi-joint biomechanics during walking. Motion capture data (kinematics, ground reaction forces) were collected during overground walking from ten participants with unilateral TTA prior to (using socket prostheses) and 12-months after bone-anchored limb implantation. Within this year, each participant completed a rehabilitation protocol that guided progression of loading based on patient pain response and optimized biomechanics. Musculoskeletal models were developed at each testing timepoint (baseline or 12-months after implantation) and used to calculate joint kinematics, internal joint moments, and joint reaction forces (JRFs). Analyses were performed during three stance periods on each limb. The between-limb normalized symmetry index (NSI) was calculated for joint moments and JRF impulses. Discrete (range of motion (ROM), impulse NSI) dependent variables were compared before and after implantation using paired t -tests with Bonferroni-Holm corrections while continuous (ensemble averages of kinematics, moments, JRFs) were compared using statistical parametric mapping ( p < 0.05). When using a bone-anchored limb, frontal plane pelvic (residual: pre = 9.6 ± 3.3°, post = 6.3 ± 2.5°, p = 0.004; intact: pre = 10.2 ± 3.9°, post = 7.9 ± 2.6°, p = 0.006) and lumbar (residual: pre = 15.9 ± 7.0°, post = 10.6 ± 2.5°, p = 0.024, intact: pre = 17.1 ± 7.0°, post = 11.4 ± 2.8°, p = 0.014) ROM was reduced compared to socket prosthesis use. The intact limb hip extension moment impulse increased (pre = -11.0 ± 3.6 Nm*s/kg, post = -16.5 ± 4.4 Nm*s/kg, p = 0.005) and sagittal plane hip moment impulse symmetry improved (flexion: pre = 23.1 ± 16.0 %, post = -3.9 ± 19.5 %, p = 0.004, extension: pre = 29.2 ± 20.3 %, post = 8.7 ± 22.9 %, p = 0.049). Residual limb knee extension moment impulse decreased compared to baseline (pre = 15.7 ± 10.8 Nm*s/kg, post = 7.8 ± 3.9 Nm*s/kg, p = 0.030). These results indicate that bone-anchored limb implantation alters multi-joint biomechanics, which may impact LBP or OA risk factors in the TTA population longitudinally.

Abstract:In this communication, a printed unidirectional antenna with improved upper band-edge selectivity is proposed to realize bandpass filtering gain performance. This antenna consists of a printed rectangular loop with two gaps: 1) a parasitic strip and 2) a parasitic loop. The rectangular loop provides good lower band-edge selectivity and the parasitic strip decides the location of upper band-edge selectivity. The parasitic loop, which acts as a director, is used to improve the capacity of upper band-edge selectivity. In addition, the passband bandwidth controllability of the antenna has also been explored. To demonstrate the effectiveness of this design, an antenna prototype is fabricated and tested. Experimental results verified the effectiveness of the proposed design. Measured results show that the fabricated antenna provides a 56.6% passband bandwidth, ranging from 2.18 to 3.9 GHz. Good bandpass filtering gain performance with a flat passband gain better than 4.4 dBi is achieved. Stable unidirectional radiation patterns with nearly identical <inline-formula><tex-math notation="LaTeX">$E$</tex-math></inline-formula>- and <inline-formula><tex-math notation="LaTeX">$H$</tex-math></inline-formula>-plane patterns in the operating bandwidth are also achieved. The measured front-to-back ratio is better than 10 dB.
Quaternion Collaborative and Sparse Representation With Application to Color Face Recognition.

Abstract:Collaborative representation-based classification (CRC) and sparse representation-based classification (SRC) have recently achieved great success in face recognition (FR). Previous CRC and SRC are originally designed in the real setting for grayscale image based FR. They represent the color channels of a query color image separately and ignore the structural correlation information among the color channels. To remedy this limitation, in this paper we propose two novel representationbased classification (RC) methods for color face recognition, namely quaternion collaborative representation-based classification (QCRC) and quaternion sparse representation-based classification (QSRC) using quaternion ℓ1 minimization. By modeling each color image as a quaternionic signal, they naturally preserve the color structures of both query and gallery color images while uniformly coding the query channel images in a holistic manner. Despite the empirical success of CRC and SRC on FR, few theoretical results are developed to guarantee their effectiveness. Another purpose of this paper is to establish the theoretical guarantee for QCRC and QSRC under mild conditions. Comparisons with competing methods on benchmark real-world databases consistently show the superiority of the proposed methods for both color face recognition and reconstruction.
Effect of combined hip and knee exercises on rehabilitation after total knee arthroplasty: A prospective randomized trial

Abstract:Abstract Background: To explore the clinical effect of combined hip–knee joint exercise compared with traditional postoperative rehabilitation after total knee arthroplasty (TKA).Methods: In a prospective, single-blind randomized controlled trial, 60 female patients with osteoarthritis who underwent unilateral TKA were randomly divided into three groups: a resistive intervention group, a nonresistive intervention group and a control group. Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) values, Forgotten Joint Score (FJS)-12 values, isokinetic muscle strength, and gait test results were measured and recorded preoperatively, two weeks postoperatively and three months postoperatively.Results: With the preoperative evaluation as baseline, the resistive hip–knee joint exercise group had lower functional scores and stronger quadriceps femoris and hip abductor muscles than the nonresistive group (p<0.05 for both). Both hip–knee exercise groups had stronger quadriceps femoris, hip abductor and hip rotator muscles than the control group (p<0.05). Two weeks postoperatively, both hip–knee strength training groups had lower functional scores than the control group (p<0.05) but stronger quadriceps femoris and external hip rotator muscles (p<0.05). Preoperative and 2-week postoperative gait analysis revealed that combined hip–knee strengthening could increase step length (p<0.05) and the ability to stand on the affected limb (p<0.05).Conclusion: A 12-week regimen of combined hip–knee exercises could improve the function of the affected limb after TKA, the movement patterns of the knee joint during gait, and the therapeutic effect of TKA and accelerate patients’ rehabilitation. The effect of combined hip–knee exercises positively correlated with increases in exercise load.

Abstract:Total Hip Arthroplasty (THA) remains the gold standard of treatment for patients who suffer with a variety of hip-related pathological degeneration or trauma. These patients often exhibit significantly less post-operative pain and an increase in the range of motion of the joint, but there are still relatively common instances of debilitating periprosthetic complications that call into question the method for pre-surgical implant choice. Currently, there are two principal options for THA prostheses: cemented or non cemented. Utilizing the cemented procedure ensures a faster acquisition of adequate implant stability than with the non cemented procedure, but can eventually lead to an increased periprosthetic fracture risk. Non cemented prosthetic stems are more frequently revised within the first few years following THA due to periprosthetic fracture, but non cemented revision surgeries generally result in fewer complications than those of cemented implants. Surgeons typically rely on experience or simple patient metrics such as age and sex to prescribe which implant procedure is optimal, and while this may work for most patients, there is a clear need to analyze more rigoriously patient conditions that correlate to optimal post-THA outcomes. The results from the investigation reported herein indicate that an understanding of how the percent composition and quality of a patient's quadriceps muscle in both healthy and operated legs may be a better indicator for prosthetic choice. Additionally, these data emphasize that the traditional metrics of age and sex inadequately predict changes in quadriceps composition and quality and thereby have comparatively minor utility in determining the patient-appropriate prosthetic type.

Abstract:Even if associating in single entity polyalgic symptoms remains a matter of discussion, fibromyalgia concept has gained a large acceptance among medical communities and international instances. Its physiopathological background is still in discussion. Its presentation mostly in middle aged women is a chronic, widespread painful syndrome, mostly appendicular of long duration and resistant to any treatment, associated with multiple tender points. Frequently associated are a poor sleep, local and general fatigue and mood disorders. Clinical, biological and radiological search for a definite condition remains negative, at least for primitive presentation. Concomitant forms are accepted by the American College of Rheumatology criteria, position which can be challenged. It seems more accurate to study such cases as differential diagnosis. Physiopathology involves anomalies of perception transmission and integration of painful signals at different levels of nervous system, either peripheral or central. Medical intervention requires first a good deal of empathy and a multidisciplinary approach combining some drugs, insufficient if used alone and different physical cognitive and comportemental measures. Fibromyalgia, which raises difficult social problems, is an evolving concept.

Abstract:The purpose of this study is to evaluate the gait characteristics of bilateral limbs after unilateral total knee arthroplasty (TKA) using three-dimensional (3D) dynamic capture technology.Forty-two patients who underwent TKA were selected from the Orthopedic Medical Center of The Second Hospital of Jilin University from November 2018 to May 2019. We used a 3D dynamic capture system to measure the gait characteristics of patients at 3 months after TKA. The data, including relative position and direction of different body parts, the force between feet and ground, spatial and temporal relationship of the lower limb muscles, were measured. Besides, the surface electromyogram signal and the force plate analog signal were also collected. The walking ability, knee 3D kinematic, and kinetic characteristics were analyzed by the Cortex software.Spatial and temporal parameters, including stride frequency, double support phase, single support phase, step length, step time, step width, stride length, gait cycle, velocity, were no significant difference in bilateral lower extremities (P > .05). The reaction force of hip, knee, and ankle joint in the operation side were less than that of the healthy side, but the difference was not statistically significant (P > .05). However, when compared with the healthy side, the hip joint in operation side had a larger maximum extension angle (P < .001), the knee joint in operation side had a larger maximum valgus angle and valgus activity (P < .05), and had a smaller tibial maximum internal rotation angle (P < .05). Besides, the surface electromyogram signals of tibialis anterior muscles were reduced (P < .05).3D gait analysis, as an objective and quantitative evaluation method, is a safe, effective, and reliable method for evaluating postoperative knee function. The data of gait analysis prove that TKA is a vital treatment to improve the function of patients with knee arthritis. Besides, gait analysis also showed that there were various kinematic and biomechanical abnormalities in the knee after TKA, which may be the reason why the surgical knee could not immediately return to normal level.
Hip Exoskeleton Based Indirect Intervention Strategy Towards Gait Training After Total Knee Arthroplasty

Abstract:Total Knee Arthroplasty (TKA) is the most effective approach for function restoration in patients with severe knee osteoarthritis. However, kinematic, kinetic and muscle activation differences between post-TKA patients and healthy people can be observed in many studies. Exoskeletons have been applied to post-TKA rehabilitation for many years, while few studies concentrated on the stance phase abnormality, neither in the aspect of kinematics nor in muscle activation. In this paper, we propose an indirect resistance strategy for post-operative TKA patient gait training. Three healthy subjects were asked to wear the hip exoskeleton and provided with 8 N·m resistance on the hip extension phase of the gait cycle. The intervention leads to an increment in the knee extension muscle activity as well as the augmentation in maximum knee angle in loading response. The results indicated that the application of resistance in the hip extension phase is a potential therapeutic approach for post-TKA rehabilitation, and may increase the gait training efficiency in the near future.

Abstract:Purpose This study aimed to investigate the differences in spatiotemporal gait parameters in patients who underwent surgery for hip fractures when using walking poles and T-canes. Methods This cross-sectional study enrolled eight patients who underwent surgery for a unilateral hip fracture (mean age, 79.0 ± 7.9 years) and 34 healthy individuals who had no symptoms (mean age, 32.1 ± 6.2 years). The outcome measures were the walking speed, trunk acceleration, and lateral lean angles of the trunk, shoulder, and pelvis during walking. The results were compared among the three types of walking aids, namely, a T-cane, double T-canes, and walking poles. Results Acceleration indices step symmetry in the vertical and anteroposterior directions in walking with walking poles were significantly larger than that in walking with a T-cane. These results were common in patients with fractures and healthy individuals. Conclusions Walking with walking poles might be a more symmetrical gait style than walking with a T-cane in patients who underwent surgery for hip fractures.

Abstract:Background: Concomitant high tibial osteotomy (HTO) and anterior cruciate ligament (ACL) reconstruction is a combined surgical procedure intended to improve kinematics and kinetics in the unstable ACL-deficient knee with varus malalignment and medial compartment knee osteoarthritis (OA). Purpose: To investigate 5-year changes in gait biomechanics as well as radiographic and patient-reported outcomes bilaterally after unilateral, concomitant medial opening wedge HTO and ACL reconstruction. Study Design: Controlled laboratory study. Methods: A total of 33 patients (mean ± SD age, 40 ± 9 years) with varus malalignment (mean mechanical axis angle, −5.9° ± 2.9°), medial compartment knee OA, and ACL deficiency completed 3-dimensional gait analysis preoperatively and 2 and 5 years postoperatively. Primary outcomes were the peak external knee adduction (first peak) and flexion moments. Secondary outcomes were the peak external knee extension and transverse plane moments, peak knee angles in all 3 planes, radiographic static knee alignment measures (mechanical axis angle and posterior tibial slope), and the Knee injury and Osteoarthritis Outcome Score (KOOS). Results: There was a substantial decrease in the knee adduction moment in the surgical limb (%BW × H, −1.49; 95% CI, −1.75 to −1.22) and a slight increase in the nonsurgical limb (%BW × H, 0.16; 95% CI, 0.03 to 0.30) from preoperatively to 5 years postoperatively. There was also a decrease in the knee flexion moment for both the surgical (%BW × H, −0.67; 95% CI, −1.19 to −0.15) and nonsurgical limbs (%BW × H, –1.06; 95% CI, –1.49 to −0.64). Secondary outcomes suggested that substantial improvements were maintained at 5 years, although smaller declines were observed in several measures and in both limbs from 2 to 5 years. Conclusion: Changes in the peak external moments about the knee in all 3 planes during walking were observed 5 years after concomitant medial opening wedge HTO and ACL reconstruction. These findings are consistent with an intended, sustained shift in the mediolateral distribution of knee loads. Clinical Relevance: These findings suggest that concomitant HTO and ACL reconstruction results in substantial changes in gait biomechanics. Future clinical research comparing treatment strategies is both warranted and required for this relatively uncommon but seemingly biomechanically efficacious procedure.
