Abstract:BACKGROUND: There is uncertainty about the variation in infection prevention practices for central venous catheters (CVC) in neonatal units (NNUs) and how practices relate to national guidance.AIM: To evaluate evidence supporting infection prevention practices for CVCs recommended in national guidelines and to compare with reported practices for peripherally inserted central catheters (PICC), a type of CVC widely used in NNUs.DESIGN: We searched national guidelines for neonates and children to identify infection prevention practices for CVCs and conducted an overview of studies to determine the quality of evidence underpinning recommendations. We surveyed 134 NNUs in England and Wales to ascertain reported practice.RESULTS: We found low quality evidence supporting CVC care bundles and use of 2% alcoholic chlorhexidine to decontaminate catheter ports and skin before insertion. Moderate quality evidence supported recommendations against routinely replacing CVCs and against chlorhexidine-impregnated dressings. 90% (44/49) of NICUs and 40% (34/85) of LNUs responded. 66% (48/73) of NNUs reported using CVC care bundles for insertion; 62% (45/73) used bundles for maintenance. 63% (32/51) of those using bundles reported monitoring adherence. 85% (61/72) of NNUs did not routinely replace PICCs and 89% (63/71) did not use chlorhexidine-impregnated dressings. Antiseptic use varied with alcoholic 2% chlorhexidine used for skin preparation in 33% (23/71) of NNUs and for catheter ports in 52% (37/71).CONCLUSIONS: Lack of consistency across NNUs in antiseptic use and low rates of reported CVC care bundle use may reflect the low quality of evidence of the effectiveness and safety of these interventions in NNUs. Clinical trials are needed to quantify benefits and harms of infection prevention practices in NNUs.

Abstract:ABSTRACT Context World Health Organization (WHO) defines healthcareassociated infection (HCAI) as infection acquired in hospital or a healthcare setting by a patient who was admitted for a reason other than that infection. The healthcare associated infections are one of the leading causes of mortality, morbidity and increase cost. Adherence to standard precautions for infection control and simple techniques like effective hand hygiene is essential for reducing healthcare associated infections. However, compliance of healthcare workers to hand hygiene (HH) guidelines are reportedly poor. It is important, therefore, to instill adequate knowledge and good attitudes and practices at the time of primary training of the healthcare workers. This study is an attempt to identify gaps in knowledge, attitudes and practices (KAP) to improve existing training programs and give recommendation to enhance good practices in the future. Aims The aim of the study is to analyze KAP of nursing professionals of intensive care units (ICUs) in a tertiary care hospital and to find the impact of knowledge and attitude of the ICU nurses on self-reported practices. Settings and design The study design is a survey research which has used a self-administered questionnaire to compare the KAP of nursing professionals of an ICU in a tertiary care hospital. Materials and methods The WHO standard precautions for infection control were used as a guideline for preparing the self-administered questionnaire. The scoring system was based on a study done by Uba et al (2015). Statistical analysis Correlation and analysis of variance (ANOVA) were used to establish associations between the independent and dependent variables. Results Participants had an average level of knowledge (79%), good attitude (89%) toward infection control guidelines and very good self-reported practices (91%). The collective KAP score of all the participants is good (85%) which indicates that average levels of knowledge are balanced by good attitude and very good practices. However, good knowledge is crucial for ensuring expected levels of infection control practices, and hence ensures patient safety. Conclusion To achieve an environment of patient safety, it is essential that the healthcare staff should have sound knowledge and positive attitude. The study shows the need for further improvement of the existing infection control training programs to address the gaps in KAP. Key message Good knowledge and positive attitude are essential for attaining expected levels of infection control practices among critical care nurses. Abstract:Background: Hand hygiene protocols, if followed stringently reduce the possibility of the spread of infection from healthcare workers (HCWs) to patients, to almost 99%. However, in actual practice, compliance of these protocols is poor. Aims: (1) To observe the practice of hand hygiene among HCWs and (2) assess knowledge and awareness regarding hand hygiene protocol and identify the factors hampering its effective practice. Prospective observational study at an multispeciality hospital. Patients and Methods: Consultants, junior doctors, nursing in-charges and nurses and ancillary staff like ward attendants, ward secretaries, housekeeping staff and lift operators were (1) Observed during their hand hygiene practice and (2) 200 of these HCWs were provided a written questionnaire, in order to assess their depth of hand hygiene knowledge. Data were studied and analysed. Results: Female HCWs, right from consultants to ward attendants and ancillary workers showed better knowledge and compliance with hand hygiene practices. The junior nurses, interns and junior residents were more compliant with hand hygiene procedures in comparison to consultants and in-charge nurses. Hand hygiene compliance was poor in outpatient department, particularly on days with heavy workloads. Lack of knowledge regarding hand hygiene was observed amongst ancillary staff. Conclusions: The study emphasised stringent compliance and constant re-enforcement of protocol of correct hand hygiene practice amongst HCWs including ancillary staff.
Barriers to Effective Infection Prevention in the Neonatal Intensive Care Unit: A Qualitative Study

Abstract:Background: Infection prevention (IP) behaviors such as hand hygiene (HH) and mobile device disinfection are important to reduce the risk of infection transmission from both family members and hospital staff to critically ill neonates. Purpose: To inform the design of educational interventions to improve both patient family and staff IP behaviors, we engaged separate groups of nurses and family members to understand perceptions about the spread of infection and barriers to implementing effective IP strategies. Methods: This was a qualitative study using focus groups to gather data from neonatal nurses and patient family members. Data were triangulated with hospital-wide survey data and analyzed using inductive content analysis. Results: Twelve nurses and 4 patient family members participated. Themes related to communication about IP between staff and family members emerged: stakeholders expressed discomfort with the timing and nature of just-in-time HH education. These communication challenges contributed to stress levels within the neonatal intensive care unit. This finding was reflected in the hospital-wide survey. Implications for practice and research: Steps should be taken to improve communication about IP behaviors between patient family members and frontline staff. Reducing nurse burden of providing just-in-time HH reminders to patient family members through increased IP education may decrease stress and facilitate IP behaviors. This has the potential to decrease infection spread and improve patient outcomes. The development of interventions targeting stakeholder communication is therefore warranted, but additional research is needed to understand the timing and process for delivery of the educational material.
The Practice of Arterial Catheters in ICUs and Nurses’ Perceptions of Infection Prevention: A Multicentre Cross-Sectional Study

Abstract:BACKGROUNDPeripheral arterial catheters (AC) are increasingly used in intensive care units (ICUs). Arterial catheter-related bloodstream infection is a serious complication that can increase patients' morbidity and length of stay. Standardised infection prevention practices are important when using AC. However, the current practices regarding AC insertion, use and removal and the perceived infection prevention attitudes of nurses in ICUs are unknown.METHODSThis was a multicentre cross-sectional study; 20 tertiary general hospitals were selected with a stratified random method in Beijing, China, using a self-reported internet survey.RESULTSA total of 981 valid questionnaires were collected. Overall, some infection prevention practices, such as AC insertion and disinfection of the blood sample hub, were generally consistent with clinical guidelines, whereas others were inconsistent: eye protection, skin antiseptic solution, dressing choice, blood sample collection and replacement of AC. More than 60% of participants mentioned occasionally or never having used eye protection. Only 6.0% of them stated using the chlorhexidine dressings. Among the participants, 80.6% reported that they replaced AC routinely rather than based on clinical indications, 64.2% self-rated that they did not routinely culture a catheter specimen after removal and 53.4% of participants positively agreed that AC could cause infection. Nurses with a higher education level were more likely to agree that an infection risk with AC exists (trend χ2 = 5.456, p = 0.019*).CONCLUSIONSSignificant heterogeneity exists across hospitals in China in terms of antiseptic techniques and perception of infection prevention during AC insertion, use and removal. Critical care nurses' practices partially complied with guideline recommendations. Educational level was found to be a risk factor for their perceived infection prevention attitudes. Nurses with a lower education level underestimated the infection risk of AC. Future research may examine optimal preventive strategies for reducing infection.

Abstract:Abstract Background Central line-associated bloodstream infections (CLABSI) are largely preventable when evidence-based guidelines are followed. However, it is not clear how well these guidelines are followed in intensive care units (ICUs) in China. This study aimed to evaluate Chinese ICU nurses’ knowledge and practice of evidence-based guidelines for prevention of CLABSIs issued by the Centers for Disease Control and Prevention, US and the Department of Health UK. Method Nurses completed online questionnaires regarding their knowledge and practice of evidence-based guidelines for the prevention of CLABSIs from June to July 2019. The questionnaire consisted of 11 questions, and a score of 1 was given for a correct answer (total score = 0–11). Results A total of 835 ICU nurses from at least 104 hospitals completed the questionnaires, and 777 were from hospitals in Guangdong Province. The mean score of 11 questions related to evidence-based guidelines for preventing CLABSIs was 4.02. Individual total scores were significantly associated with sex, length of time as an ICU nurse, educational level, professional title, establishment, hospital grade, and incidence of CLABSIs at the participant’s ICU. Importantly, only 43% of nurses reported always using maximum barrier precautions, 14% of nurses reported never using 2% chlorhexidine gluconate for antisepsis at the insertion site, only 40% reported prompt removal of the catheter when it was no longer necessary, and 33% reported frequently and routinely changing catheters even if there was no suspicion of a CLABSI. Conclusion Chinese ICU nurses in Guangdong Province lack of knowledge and practice of evidence-based guidelines for the prevention of CLABSIs. National health administrations should adopt policies to train ICU nurses to prevent CLABSIs.

Abstract:Background: Nosocomial infections are particularly common among hospitalized patients who undergo resuscitation compared with other types of care. Hand hygiene remains the simplest and most effective measure to prevent and control the risk of infection. Objective: The main objectives are to evaluate hand hygiene compliance among nursing staff in the different intensive care units and to identify the factors influencing the adherence of nursing staff to the practice. Methods: It was a quantitative descriptive study using a questionnaire and an observation grid with all the nursing staff working in all the intensive care units of our university hospital. Results: The study showed a hand contamination rate of (80%), a hand hygiene compliance rate of (21.3%), it also showed the different factors explaining non-adherence, these are not necessarily related to training, nor to the availability of material resources but related to hidden reasons, non-apparent factors, which are often more important. (Workload and work environment) Conclusion: This study examined hand hygiene in the ICU setting, obtained data on overall compliance, which remains poor, and the various factors influencing nurses' adherence to the practice.

Abstract:Background: The ongoing COVID-19 pandemic has highlighted the critical importance of Infection prevention practices among healthcare workers. Prioritizing this crucial aspect of healthcare can mitigate the spread of infectious diseases and ensure the well-being of our healthcare heroes and their communities. Objective: The purpose of the research was to investigate the knowledge and practice of Infection prevention and control. Methods: The study was a cross-sectional study that used self-administered paper-based questionnaires. 316 eligible healthcare workers were selected using stratified sampling. Data was entered into EPI Info version 7.2 and exported to SPSS version 27 for analysis. The study was approved by the ethics committees of the University and the Hospital. Results: The majority of participants 116 (36.7%) were nurses. The mean age was 34.79 years ± 8.37, 118(37.30%) were male while 198(62.7%) were female. Only 169(53.9%) knew the recommended duration for hand washing. 132 (41.8%) of healthcare workers believed needles should be recapped following use. Healthcare workers were twice as likely to wash their hands before contact and five times more likely to wash their hands after contact with a patient, their bedding or after a procedure (AOR 1.82, 95%CI 1.04-3.20), (AOR 4.51, 95%CI 1.76-11.54) respectively. Personal protective equipment (PPE) was twice as likely to be unavailable (AOR 2.39, 95%CI 1.31-4.37). The findings revealed suboptimal knowledge and practice of hand hygiene, indicating the need for healthcare workers to be trained on IPC. PPE(s) must be provided for healthcare workers to improve compliance with IPC practices.

Abstract:Aims and objectivesTo evaluate intensive care unit (ICU) nurses' knowledge of the updated guidelines for the prevention of intravascular catheter-related infections; to identify the factors that affect the nurses' knowledge and to explore the barriers to adherence to evidence-based guidelines in clinical practice in China.MethodsCross-sectional surveys were carried out in Chinese ICUs from January 2013 to March 2014. The nurses' demographic information, knowledge of the guidelines, and barriers to adherence were assessed by a validated questionnaire and then analyzed statistically.ResultsThe questionnaires were completed by 455 ICU nurses from 4 provinces of China. The mean score was 8.17 of 20, and higher scores were significantly associated with province, years of experience, and years of ICU experience. Forty-nine (10.7%) nurses had not heard of the guidelines, whereas 231 (50.7%) nurses heard of the guidelines but did not receive training for them. Trained nurses' scores were higher than untrained nurses' scores. The three main barriers to compliance with the guidelines were an unfamiliarity with them, an excessive workload due to a shortage of nurses, and a lack of training.ConclusionsICU nurses' knowledge of the updated guidelines is quite low, which could be a potential risk factor for patient safety. Multidisciplinary interventions and continuous educational programs are needed to improve the knowledge of and adherence to the guidelines.

Abstract:Background: The neonatal intensive care unit is one of the most infection-prone areas in the hospital. The laryngoscopeis identified as a potential vector for cross-infection. So, assessing knowledge and performance of NICU nurses isbaseline to improve well-being of patients. Aim: This study aimed to assess the nurses' knowledge and performanceregarding infection control measures in processing of laryngoscope of neonatal intensive care unit. Method: Adescriptive cross-sectional study design was utilized in this study. Setting: study was conducted at Mansoura SpecializedHospital at Mansoura city on a convenient sample of (35) nurses working in the NICU. Tools: Three tools were used tocollect data as the following: nurses’ demographic and occupational characteristics questionnaire sheet, nurse’sknowledge about infection control measures toward laryngoscope processing and observational checklist. Results: Theoverall total knowledge score revealed that (60%) of nurses showed a poor score level of knowledge with a mean of14.4(4.33). A few percentages of nurses had competent perform hand hygiene, wear personal protective equipment,sharp and waste disposal (2.9%, 0% and 0% respectively). Moreover, there was significant relation between nurses'educational level and total knowledge score. While there was no relation between nurses' occupational characteristics andtotal performance score related infection control measure in laryngoscopy. Conclusion: Nurses had a poor score level ofknowledge in addition, inadequate performance regarding infection control measures in processing of laryngoscope.Recommendation: Continuous educational programs to keep nurses in neonatal intensive care units updated with theevidence based practices and carry interventions for reducing infection among children.

Abstract:The objective of the research was to establish the relationship between the level of knowledge and the level of practices of nurses on the prevention of infections associated with vascular catheters in the neonatal intensive care unit at the Hospital Belén de Trujillo (HBT) and the Regional Teaching Hospital of Trujillo (HRDT) 2020. The sample universe consisted of 40 nurses who work in the Neonatal Intensive Care Unit of the HBT and HRDT, who met the inclusion criteria. The instruments used were the Questionnaire to measure the Level of Knowledge on prevention of vascular catheter-associated infection (CMNCPICV) and the Scale for measuring practices on prevention of vascular catheter-associated infections (EMPPICV). The results showed that 50 percent of nurses had a high level of knowledge, 35 percent a medium level, and 15 percent a low level of knowledge. Regarding the level of practices on prevention of infections associated with vascular catheters, 62.5 percent were adequate while 37.5 were inadequate. A highly significant relationship was found between the level of knowledge and the level of nursing practice on the prevention of infections associated with vascular catheters (p= 0.000).

Abstract:Objective: To explore the knowledge and beliefs of intensive care nurses about urinary catheter securement. Method and setting: A self-report survey tool comprising 19 questions (15 closed, 4 open-ended) was developed through consultation with a panel of experts and then disseminated electronically to nurse members of two societies for critical care medicine and nursing in Spain. Responses were analysed by calculating descriptive statistics and through qualitative analysis. Main outcome measures: Nurses' self-reported knowledge and beliefs about urinary catheter securement in the intensive care patient. Results: Responses of 333 nurses were analysed. Most respondents considered that urinary catheter securement was beneficial for patients (n = 261, 78.4 %) and that it reduced the risk of both meatal pressure injury (n = 266, 79.9 %) and accidental tugging (n = 304, 91.3 %). Regarding the securement procedure, less than a quarter of nurses mentioned the use of barrier film to protect the skin at the securement site (n = 62, 18.6 %) or the need to leave slack in the tube to avoid tugging (n = 45, 13.5 %). Qualitative analysis of responses to open-ended questions revealed that some nurses believed that catheter securement was contraindicated due to the risk of skin injury, or that it interfered with some aspects of basic care, thus increasing their workload. Conclusions: There appears to be a lack of knowledge among Spanish intensive care nurses about recommended procedures for securing a urinary catheter. Clear and concise guidelines for the intensive care setting are required to ensure safe and standardised practice without creating additional burden for nurses. Implications for clinical practice: Lack of knowledge among intensive care nurses about how to secure urinary catheters may pose certain risks to patients, therefore specific training in how to secure a urinary catheter is needed.

Abstract:Abstract Background Healthcare-associated infections (HAIs) are important indicators of healthcare quality. The aseptic non touch technique (ANTT), a standardised aseptic technique, is a crucial preventative measure against HAIs. However, there is insufficient evidence currently available regarding ANTT awareness and proficiency among nurses. This study aimed to investigate the status of ANTT proficiency in a cohort of nurses and analyse the associated risk factors, with the ultimate goal of providing a reference for promoting the application of ANTT in clinical practice. Methods The study population was sampled from nurses working in a tertiary hospital in Zhejiang Province, China, in January 2024. A cross-sectional survey was conducted using a self-designed questionnaire to assess ANTT knowledge. Multivariate linear regression analysis was used to analyse the risk factors influencing ANTT knowledge in nurses. Results A total of 458 nurses were included in this study. The cohort had an overall score of 49.7% correct responses on th e ANTT knowledge questionnaire, with a mean score of 12.4 ± 2.4 out of 25, and 30.1% of the nurses felt that they did not need training related to ANTT. Multivariate linear regression analysis indicated that professional title, educational level, and the need for ANTT training were independently associated with ANTT knowledge in nurses. Nurses with Master’s degrees and associated chief nurses were observed to have higher levels of ANTT knowledge compared to the rest. Conclusions ANTT knowledge is insufficient among nurses in China, and its importance is not widely recognised. Continuous efforts are required to strengthen this knowledge. Constructing sustained, multiform, and stratified training programmes may represent an effective method to strengthen ANTT knowledge among nurses and promote its clinical use.

Abstract:Abstract Introduction. Due to frequent contact with sick people, the hands of healthcare professionals are the most popular way of transmitting pathogens from patient to patient and from hospital to patient. Hand hygiene is crucial when it comes to reducing HCAI (healthcare-associated infections). Aim. The aim is to analyze the professional knowledge of nurses about nosocomial infections transmitted through contact. Its main assumption is the evaluation of this knowledge in the context of preventing the spread of nosocomial infections. Material and method. A proprietary questionnaire consisting of questions was used to assess the knowledge of medical personnel. The survey contains 25 questions based on the principle of “true/false”. The reliability of the scale was measured and assessed with the Cronbach’s alpha test at the level of 68.4%. Results. Factors adversely affecting the general level of knowledge in the field of hand hygiene turned out to be older age (p=0.012), longer work experience (p=0.023) and the lack of higher education (p=0.048).

Abstract:AIM: To evaluate healthcare workers' knowledge, attitudes and practices level of prevention and management of catheter-associated urinary tract infection.DESIGN: A mixed-methods systematic review.METHODS: Searches were conducted in CINAHL, Cochrane Library, EMBASE, Medline, PubMed and Web of Science databases. Limited literatures published in English before 20 June 2021. Data were analysed and synthesized using thematic analysis by two authors.RESULTS: Thirty-four articles were included. Healthcare workers' unbalanced varied knowledge level, positive attitudes, undesirable practices of catheter-associated urinary tract infection's prevention and control were identified. Barriers of healthcare workers' knowledge, attitudes and practices level of infection prevention included heavy workload, understaffing, physician variability in indwelling urinary catheter (IUC) practice by diagnosis, nursing variability in IUC placement technique, poor relationship and nurse's poor documentation. Leadership, better education, teamwork, technique training and information technology support, advocacy for nurse-driven protocol and IUC removal reminder were considered as facilitators.

Abstract:Background: Hospital-acquired infections are recognized as a major risk factor in healthcare settings. Hand hygiene as the first stage of infection control plays an important role in reducing these infections. This study assesses the knowledge, attitudes, and practice regarding hand hygiene among nurses in intensive care units (ICUs) of Basrah City and Dhi Qar City, Iraq. Methods: This descriptive cross-sectional study targeted 231 ICU nursing staff working in hospitals of Al-Basra and Dhi-Qar in Iraq in 2022. The subjects were recruited through the census. To assess the nurses’ hand hygiene knowledge, attitude, and practice, electronic questionnaires were administered via WhatsApp. The collected data were analyzed using descriptive statistics (absolute and relative frequency, Mean±SD) and inferential statistics (independent t-test, chi-square test, and Fisher test). The data analysis was performed using the SPSS software, version 22. Meanwhile, a significance level of <0.05 was considered for all statistical tests. Results: The mean scores (ranges) of nurses’ hand hygiene knowledge, attitude, and practice were 4.74±1.82 (1-10), 77.25±7.89 (55-95), and 49.80±12.23 (21-70), respectively. There was a significant relationship between hand hygiene knowledge and gender, with female nurses’ knowledge being higher compared to male nurses (P=0.03). Furthermore, nurses with less than 5 years of practical experience in the ICU had a higher mean score of attitudes toward hand hygiene compared to nurses with 5-10 years of practical experience (P=0.015). Additionally, nurses with less than 5 years of experience in the ICU showed a higher mean score of hand hygiene practice compared to nurses with more than 10 years of practical experience (P=0.028). Conclusion: According to the findings, the knowledge, attitude, and practice of ICU nurses about hand hygiene were inappropriate. It is necessary to implement educational programs to improve the knowledge, attitude, and practice of Iraqi ICU nurses in this area.

Abstract:Objective: To understand healthcare worker (HCW) perceptions surrounding Staphylococcus aureus transmission and prevention in the neonatal intensive care unit (NICU). Design: Qualitative case study with focus groups. Setting: A level IV, 150-bed NICU at a Midwestern academic medical center that conducts active surveillance and decolonization of S. aureus–positive patients. Participants: NICU HCWs, including bedside nurses, nurse managers, therapy services personnel, pediatric nurse practitioners, clinical fellows, and attending neonatologists. Methods: Semistructured focus group interviews, assembled by occupation, were conducted by 2 study team members. Interviews were video recorded and transcribed. Deductive coding and thematic analyses were performed using NVivo software. Results: In total, 38 HCWs participated in 10 focus groups (1–12 participants each), lasting 40–90 minutes. Four main themes emerged: (1) Methicillin-resistant S. aureus (MRSA) and methicillin-susceptible S. aureus (MSSA) are inconsistently described as high risk. (2) Infection prevention interventions are burdensome. (3) Multiple sources of transmission are recognized. (4) opportunities exist to advance infection prevention. HCWs perceived MSSA to be less clinically relevant than MRSA. Participants expressed a desire to see published data supporting infection prevention interventions, including contact precautions, environmental cleaning, and patient decolonization. These practices were identified to be considerable burdens. HCWs perceived families to be the main source of S. aureus in the NICU, and they suggested opportunities for families to play a larger role in infection prevention. Conclusions: These data highlight opportunities for HCW and parental education, research, and reevaluating interventions aimed at improving infection prevention efforts to reduce the burden of S. aureus in NICU settings.

Abstract:We report a case of classic Wegener's granulomatosis. Direct immunofluorescent study showed finely granular deposits of IgG and IgM in some of the alveolar walls, and of IgM in the maxillary sinus arteries. Electron-microscopic study of the lung and maxillary sinus showed intravascular fibrin, but failed to demonstrate electron-dense deposits in the blood vessel walls. Our patient also had circulating cryoglobulins, consisting of IgG, IgM, Clq, and C3, and evidence of circulating immune complexes as demonstrated by the Clq-binding test. These findings suggest that circulating immune complexes may play an important role in the pathogenesis of the respiratory lesions of Wegener's granulomatosis.
