Abstract:Plasma proenkephalin peptide F immunoreactivity and catecholamines were examined on separate days in nine healthy males before and after maximal exercise to exhaustion at four intensities [36, 55, 73, and 100% of maximal leg power (MLP)] by use of a computerized cycle ergometer. The mean duration of 36, 55, 73, and 100% MLP was 3.31, 0.781, 0.270, and 0.1 min, respectively. All intensities were greater than those eliciting peak O2 uptake for the individual subjects. Blood samples were obtained before, immediately after exercise, and 5 and 15 min after exercise. Significant (P less than 0.05) increases in plasma peptide F immunoreactivity (i.e., from mean resting value of 0.18 to 0.43 pmol/ml) were observed immediately after exercise at 36% MLP. Significant increases in plasma epinephrine were observed immediately after exercise at 36% MLP (i.e., from mean resting value of 2.22 to 3.11 pmol/ml) and 55% MLP (i.e., from mean resting value of 1.67 to 2.98 pmol/ml) and 15 min after exercise at 100% MLP (i.e., from mean resting value of 1.92 to 3.88 pmol/ml). Significant increases for plasma norepinephrine were observed immediately after exercise (36, 55, 73, and 100% MLP), 5 min after exercise (36, 55, and 73% MLP), and 15 min after exercise (36% MLP). Increases in whole blood lactate were observed at all points after exercise for 36, 55, and 73% MLP and 5 min after exercise for 100% MLP. These data show that brief high-intensity exercise results in differential response patterns of catecholamines and proenkephalin peptide F immunoreactivity.
Local and systemic effects on blood lactate concentration during exercise with small and large muscle groups

Abstract:To evaluate the relationship between lactate release and [lac](art) and to investigate the influence of the catecholamines on the lactate release, 14 healthy men [age 25+/-3 (SE) year] were studied by superimposing cycle on forearm exercise, both at 65% of their maximal power reached in respective incremental tests. Handgrip exercise was performed for 30 min at 65% of peak power. In addition, between the tenth and the 22nd minute, cycling with the same intensity was superimposed. The increase in venous lactate concentration ([lac](ven)) (rest: 1.3+/-0.4 mmol.l(-1); 3rd min: 3.9+/-0.8 mmol.l(-1)) begins with the forearm exercise, whereas arterial lactate concentration ([lac](art)) remains almost unchanged. Once cycling has been added to forearm exercise (COMB), [lac](art) increases with a concomitant increase in [lac](ven) (12th min: [lac](art), 3.2+/-1.3 mmol.l(-1); [lac](ven), 5.7+/-2.2 mmol.l(-1)). A correlation between oxygen tension (P(v)O(2)) and [lac](ven) cannot be detected. There is a significant correlation between [lac](art) and norepinephrine ([NE]) (y=0.25x+1.2; r=0.815; p<0.01) but no correlation between lactate release and epinephrine ([EPI]) at moderate intensity. Our main conclusion is that lactate release from exercising muscles at moderate intensities is neither dependent on P(v)O(2) nor on [EPI] in the blood.
A test of the catecholamines hypothesis for an acute exercise-cognition interaction

Abstract:The purpose of the study was to examine the usage of norepinephrine (NE) and dopamine (DA) in the brain when exercising while simultaneously undertaking cognitive tests. Plasma concentrations of the NE metabolite 3-methoxy 4-hydroxyphenylglycol (MHPG) and the DA metabolite homovanillic acid (HVA) showed a linear increase from rest to exercising at 40% and 80% maximum power output (W.max) while simultaneously undertaking cognitive tasks (random number generation (RNG) and response time). Delta plasma concentrations of MHPG and HVA at each exercise intensity while undertaking cognitive tasks and while exercising without cognitive tasks did not differ. Taking blood samples at 0, 1, 3, and 5 min following cessation of exercise did not affect results. Regression correlations showed that delta MHPG and HVA plasma concentrations at the 1 and 3 min sampling times were strong predictors of delta RNG, response time and movement time. Reaction time at 80% W.max significantly increased, while movement time at 80% W.max significantly decreased. It was concluded that these results provide no support for a direct effect of increased catecholamines concentrations on cognitive performance during exercise. The regression data suggest that there is some relationship between exercise, catecholamines concentrations and cognition.
Functional outcomes following trauma-related lower-extremity amputation.

Abstract:BACKGROUND The principal aims of this study were to examine functional outcomes following trauma-related lower-extremity amputation and to compare outcomes according to the amputation levels. We hypothesized that above-the-knee amputations would result in less favorable outcomes than would through-the-knee or below-the-knee amputations. A secondary aim was to examine the factors, in addition to amputation level, that influence outcome, including the type of soft-tissue coverage, selected patient characteristics, and the technological sophistication of the prosthetic device. METHODS A cohort of 161 patients who had undergone an above-the-ankle amputation at a trauma center within three months following the injury was followed prospectively at three, six, twelve, and twenty-four months after the injury. The Sickness Impact Profile, a self-reported measure of functional status, was used as the principal measure of outcome. Secondary outcomes included pain; degree of independence in transfers, walking, and climbing stairs; self-selected walking speed; and the physician's satisfaction with the clinical, functional, and cosmetic recovery of the limb. Longitudinal multivariate regression techniques were used to determine whether outcomes differed according to the level of amputation after we controlled for covariates. RESULTS There was no significant difference in the scores on the Sickness Impact Profile between the patients treated with above-the-knee and those treated with below-the-knee amputation. However, patients with a below-the-knee amputation performed better than did patients with an above-the-knee amputation on the timed test for walking speed (p = 0.04). Patients with a through-the-knee amputation had worse regression-adjusted Sickness Impact Profile scores (p = 0.05) and slower self-selected walking speeds (p = 0.004) than did patients with either a below-the-knee or an above-the-knee amputation. Differences according to the level of amputation were most pronounced for physical function. In general, physicians were less satisfied with the clinical, cosmetic, and functional recovery of the patients with a through-the-knee amputation. Except for problems encountered with insufficient gastrocnemius coverage of the stump in many patients with a through-the-knee amputation, neither the soft-tissue coverage nor the technological sophistication of the prosthesis correlated with outcome. CONCLUSIONS Severe disability accompanies above-the-ankle lower-extremity amputation following trauma, regardless of the level of amputation. Clinicians should critically evaluate the need for a through-the-knee amputation in patients with a traumatic injury. The results of this study also underscore the need for controlled studies that examine the relationship between the type and fit of prosthetic devices and functional outcomes.
Fourteen-day optimized levofloxacin-based therapy versus classical quadruple therapy for Helicobacter pylori treatment failures: a randomized clinical trial

Abstract:Abstract Objective. To test the efficacy of lansoprazole, bismuth, levofloxacin, and amoxicillin therapy compared to bismuth metronidazole tetracycline (BMT) quadruple therapy for second-line treatment of Helicobacter pylori infection. Methods. A total of 284 patients who failed prior H. pylori eradication were randomized to receive 14-day regimens containing lansoprazole 30 mg twice a day (b.i.d.), bismuth subcitrate 240 mg b.i.d., and either amoxcillin, 1 g b.i.d. and levofloxacin 500 mg once daily (qd) (levofloxacin/bismuth therapy) or metronidazole 400 mg four times daily (q.i.d.) and tetracycline, 500 mg q.i.d. (BMT quadruple therapy). Endoscopy and culture were performed before treatment. Antimicrobial susceptibility was by agar dilution. H. pylori status was determined 6 weeks after the end of therapy using a 13C-urea breath test. Results. The metronidazole, levofloxacin, tetracycline, and amoxicillin resistance rates were 85.3%, 40.2%, 1.1%, and 0.5%, respectively. The intention-to-treat and per-protocol (PP) eradication rates were 83% (95% confidence interval [CI]: 75.9–88.3%) and 88.1% (95% CI: 81.2–92.4%) (p = 0.22) for levofloxacin-bismuth (levo-bismuth) versus BMT quadruple, respectively, and PP rates were 85.4% (95% CI: 78.5–90.3%) and 90.6% (95% CI: 84.6–94.5%) (p = 0.18). Moderate and severe side effects were significantly higher with BMT quadruple than levo-bismuth (22.4% vs. 5%, p < 0.001) and higher in women (28.4%) than men (10.4%) in BMT quadruple therapy group (p = 0.015). Conclusion. Increasing fluoroquinolone resistance has undermined levo-bismuth quadruple therapy making BMT quadruple therapy a better choice empiric second-line therapy for H. pylori infection. However, compliance was significantly higher with levo-bismuth quadruple therapy, especially among women.

Abstract:The government orders of April 1996 were devoted to the second generation Regional Health Organisation Plans (SROS II) as health planning levers. One of the main issues at stake in their development was the need to favour an overall management of the regional public health priorities and to involve the system's clients in them. In order to better understand the opinions of the clients themselves and the opinions of the health planning professionals on the participation of the clients in SROS II, a national sample was selected in order to take into account the diversity of regional experiences, the different institutional linkages and the unity of both the place and the problem at hand. Two series of 42 interviews conducted at 6 month intervals were recorded and a thematic analysis according to a validated interview grid was carried out by the Analytical Laboratory for Social and Health Policy (LAPSS) at the National School of Public Health (ENSP). This survey shows that the first attempt to include the participation of the system's clients in the health planning efforts by the SROS II was appreciated and recognised as legitimate and useful, yet suffered from a lack of legibility and of a clear definition of its role. Other participation mechanisms remain to be investigated and explored as they could be enlarged in the next plan, in which the clients could become a separate entity of actors as an entirely independent group.
The effect of different intensities of exercise on the excretion of epinephrine and norepinephrine

Abstract:Anticipation of a potentially stressful situation can alter the activity of the sympathetic nervous system. The purpose of this study was to measure the effect of three different intensities of exercise on epinephrine (E) and norepinephrine (NE) excretion when the work test order was randomly assigned and the subjects were not informed of the test order. Six subjects exercised at work loads which averaged 49%, 65%, and 82% Vo2max. Each subject showed an increased rate of NE excretion as the work load increased. The differences among work loads were significant (P less than 0.01). The E response was less consistent. Two subjects showed the greatest rate of excretion at the lowest work load, and one subject showed no increase above his pre-exercise resting value. While NE excretion was closely related to the work load, factors other than exercise appeared to exert considerable influence on the rate of E excretion during exercise.
Greater lactate accumulation does not alter peripheral concentrations of key appetite-regulating neuropeptides

Abstract:The potential mechanisms involved in lactate's role in exercise-induced appetite suppression requires further examination. We used sodium bicarbonate (NaHCO 3 ) supplementation in a double-blind, placebo controlled, randomized crossover design to explore lactate's role on neuropeptide Y (NPY), agouti-related peptide (AgRP), and alpha-melanocyte stimulating hormone (α-MSH) concentrations. Twelve adults (7 males; 24.2±3.4 kg‧m -2 ; 42.18±8.56 mL‧kg -1 ‧min -1 ) completed two identical high-intensity interval training sessions following ingestion of NaHCO 3 (BICARB) or sodium chloride (PLACEBO) pre-exercise. Blood lactate, acylated ghrelin, NPY, AgRP, α-MSH, and appetite perceptions were measured pre-exercise, 0-, 30-, 60-, and 90-min post-exercise. Free-living energy intake (electronic food diaries) was measured the day before, of, and after each experimental session. In BICARB, blood lactate was greater post-exercise (p 0.70) though acylated ghrelin was similar (p=0.075, =0.206) at all time-points post-exercise (p>0.034, d 0.509) and AgRP (p 0.488) had main effects of time increasing following exercise and returning to baseline, with no differences between sessions (NPY: p=0.0.192, =0.149; AgRP: p=0.422, =0.060). α-MSH had no main effect of time (p=0.573, =0.063) or session (p=0.269, =0.110). Appetite perceptions were similar during BICARB and PLACEBO (p=0.007, d=0.28) increasing in both sessions post-exercise (p 0.57). Energy intake had a main effect of day (p=0.025, =0.825), where the experimental session day was greater than the day before (p=0.010, d=0.59) with no other differences between days (p>0.260, d<0.38). The lower accumulation of lactate than our previous work did not generate exercise-induced appetite suppression as there were no differences in acylated ghrelin, appetite perceptions, or peripheral concentrations of neuropeptides.

Abstract:Lactate has been implicated in exercise-induced appetite suppression though little work has explored the mechanisms underpinning its role. Recent work suggests lactate accumulation via exercise and intracerebroventricular injection can alter central appetite regulating pathways, though a supraphysiological dose of lactate was administered centrally and there was no assessment of peripheral appetite markers. Therefore, we examined how physiologically relevant lactate accumulation via exercise or intraperitoneal injection altered central and peripheral appetite signaling pathways and whether the lactate dehydrogenase inhibitor oxamate could blunt any exercise effect. Forty 10-week-old C57BL/6 J male mice ( n = 10/group) were assigned to either: 1) sedentary (SED + SAL; saline); 2) exercise (EX+SAL; saline); 3) exercise with oxamate (EX+OX; 750 mg‧kg −1 body mass); or 4) lactate (SED + LAC; 1.0 g‧kg −1 body mass). Blood, stomach, and hypothalamus samples were collected ~2 h post-exercise/injection. Though oxamate blunted exercise-induced lactate accumulation compared to the EX+SAL condition ( P = 0.044, d = 0.73), there were no differences in circulating acylated ghrelin or stomach ghrelin O-acyltransferase content between groups ( P > 0.213, ηp2 <0.125). There were also no differences in hypothalamic content for neuropeptide Y, proopiomelanocortin, agouti-related peptide, and alpha melanocyte-stimulating hormone ( P > 0.150, ηp2 <0.170). Exercise did increase phosphorylated-total signal transducer and activator of transcription 3 (pSTAT3) compared to EX+OX ( p = 0.065, d = 1.23) but there were no differences in other markers of lactate signaling: phosphorylated-total adenosine monophosphate activated protein kinase, and protein kinase b ( P > 0.121, ηp2 <0.160). Our results suggest that lactate accumulation due to exercise or peripheral injection does not alter central or peripheral appetite signaling when measured 2 h post-exercise/injection, though pSTAT3 was blunted with oxamate.

Abstract:Exercise suppresses appetite in an intensity‐dependent manner partly due to changes in peripheral appetite‐regulating hormones. While the underlying mechanisms are unclear, lactate release from active musculature may be important as its binding to gastric cell lines inhibits the release of the orexigenic hormone ghrelin. The contraction‐induced myokine interleukin‐6 (IL‐6) may also be involved, as it stimulates the production and release of anorexigenic factors such as glucagon‐like peptide‐1 (GLP‐1) and peptide YY (PYY) from intestinal cells. If these mechanisms contribute to the intensity‐dependent suppression of appetite following exercise, post‐exercise increases in lactate and IL‐6 should coincide with changes in ghrelin, GLP‐1, and PYY. To test this hypothesis, the current study examined changes in blood lactate, IL‐6, and appetite‐regulating hormones in response to running at various intensities. Eight males performed four experimental sessions: 1) Moderate‐intensity continuous training (MICT, 30 min at 65% VO 2max ); 2) High‐intensity continuous training (HICT, 30 min at 85% VO 2max ); 3) Sprint interval training (SIT, 4 × 30 sec maximal efforts, 4 min rest); 4) Control (CTRL, no exercise). Lactate, IL‐6, ghrelin, GLP‐1, and PYY concentrations were measured pre‐, post‐, 30 min post‐, and 90 min post‐exercise. Appetite perceptions were assessed at the same time‐points using a visual analog scale. Exercise suppressed ghrelin and appetite in an intensity‐dependent manner (HICT and SIT vs. CTRL, p <0.001), with SIT resulting in a greater (SIT vs. MICT at 30 min post‐exercise, p <0.020) and more prolonged (SIT vs. all other sessions at 90 min post‐exercise, p <0.005) response. GLP‐1 increased immediately after MICT ( p <0.001 vs. all other sessions) and at 30 min post‐exercise after HICT ( p <0.002 vs. all sessions) and SIT ( p =0.001 vs. CTRL). PYY increased immediately post‐exercise ( p <0.001 vs. CTRL) though more so after HICT versus MICT ( p =0.027). Exercise‐induced changes in appetite‐regulating hormones correlated significantly ( p <0.009) with appetite perceptions (ghrelin: r= 0.40; GLP‐1: r= −0.31; PYY: r =−0.36). Post‐exercise increases in blood lactate correlated negatively ( p <0.036) with ghrelin (MICT: r =−0.43; HICT: r =−0.50; SIT: r =−0.45) and appetite (MICT: r =−0.43; HICT: r =−0.47; SIT: r =−0.45). Changes in IL‐6 correlated with GLP‐1 following SIT only ( r =0.48, p =0.018), though no relationship between IL‐6 and PYY was observed. These findings support an intensity‐dependent paradigm for appetite regulation following exercise, which appears to be closely associated with reductions in ghrelin though increases in GLP‐1 and PYY also contribute. The results also highlight the potential involvement of lactate and IL‐6 in mediating changes in appetite‐regulating hormones, particularly after intense exercise. Abstract:To evaluate current environmental contamination and contributions from internal and external exposure due to the accident at the Chernobyl Nuclear Power Plant (CNPP) and nuclear tests at the Semipalatinsk Nuclear Testing Site (SNTS), concentrations of artificial radionuclides in edible mushrooms, soils and stones from each area were analyzed by gamma spectrometry. Annual effective doses were calculated for each area from the cesium contamination. Calculated internal effective doses of (137)Cs due to ingestion of mushrooms were 1.8 × 10(-1) mSv/year (y) in Gomel city (around CNPP), 1.7 × 10(-1) mSv/y in Korosten city (around CNPP), 2.8 × 10(-4) mSv/y in Semipalatinsk city, and 1.3 × 10(-4) mSv/y in Nagasaki. Calculated external effective doses of (137)Cs were 3.4 × 10(-2) mSv/y in Gomel city, 6.2 × 10(-2) mSv/y in Korosten city, 2.0 × 10(-4) mSv/y in Semipalatinsk city, and 1.3 × 10(-4) mSv/y in Nagasaki. Distribution of radionuclides in stones collected beside Lake Balapan (in SNTS) were (241)Am (49.4 ± 1.4 Bq/kg), (137)Cs (406.3 ± 1.7 Bq/kg), (58)Co (3.2 ± 0.5 Bq/kg), and (60)Co (125.9 ± 1.1 and 126.1 ± 1.1 Bq/kg). The present study revealed that dose rates from internal and external exposure around CNPP were not sufficiently low and radiation exposure potency still exists even though current levels are below the public dose limit of 1 mSv/y (ICRP1991). Moreover, parts of the SNTS area may be still contaminated by artificial radionuclides derived from nuclear tests. Long-term follow-up of environmental monitoring around CNPP and SNTS, as well as evaluation of health effects in the population residing around these areas, may contribute to radiation safety with a reduction of unnecessary exposure of residents.

Abstract:It is unknown whether changes in lactate concentration produced by different situations (e.g., glycogen depletion or heat) modify fat oxidation. If confirmed, we could determine a dose–response relationship between lactate and fat. The aim of this study was to determine whether changes in lactate concentration (due to glycogen depletion or heat) alter fat oxidation during exercise. 11 males and eight females performed an incremental exercise test under three situations: control, glycogen depletion, and heat. At rest, in the last minute of each step and immediately post‐exhaustion, lactate was analyzed and fat oxidation was estimated by indirect calorimetry. Lactate concentration was inversely associated with fat oxidation in the three aforementioned situations (r > 0.88 and p 0.05). In conclusion, lactatemia is strongly and inversely associated with fat oxidation under the three different situations. Furthermore, the lowest lactate concentrations were accompanied by the highest fat oxidations in the glycogen depletion situation, whereas the highest lactate concentrations were accompanied by the lowest fat oxidations in the heat situation.

Abstract:The catecholamines hypothesis for the acute exercise-cognition interaction in humans fails to adequately explain the interaction between peripherally circulating catecholamines and brain concentrations; how different exercise intensities×durations affect different cognitive tasks; and how brain catecholamines, glucocorticoids, BDNF and 5-hydroxytryptamine interact. A review of the animal literature was able to clarify many of the issues. Rodent studies showed that facilitation of cognition during short to moderate duration (SMD), moderate exercise could be accounted for by activation of the locus coeruleus via feedback from stretch reflexes, baroreceptors and, post-catecholamines threshold, β-adrenoceptors on the vagus nerve. SMD, moderate exercise facilitates all types of task by stimulation of the reticular system by norepinephrine (NE) but central executive tasks are further facilitated by activation of α2A-adrenoceptors and D1-dopaminergic receptors in the prefrontal cortex, which increases the signal to 'noise' ratio. During long-duration, moderate exercise and heavy exercise, brain concentrations of glucocorticoids and 5-hydroxytryptamine, the latter in moderate exercise only, also increase. This further increases catecholamines release. This results in increased activation of D1-receptors and α1-adrenoceptors, in the prefrontal cortex, which dampens all neural activity, thus inhibiting central executive performance. However, activation of β- and α1-adrenoceptors can positively affect signal detection in the sensory cortices, hence performance of perception/attention and autonomous tasks can be facilitated. Animal studies also show that during long-duration, moderate exercise and heavy exercise, NE activation of β-adrenoceptors releases cAMP, which modulates the signaling and trafficking of the BDNF receptor Trk B, which facilitates long-term potentiation.

Abstract:To examine the influence of an increase in central blood volume with head-out water immersion (WI) on the sympathoadrenal response to graded dynamic exercise, nine healthy men underwent upright leg cycle exercise on land and with WI. Plasma norepinephrine and epinephrine concentrations were used as indexes of overall sympathoadrenal activity. Oxygen consumption (VO2), heart rate, systolic blood pressure, and plasma concentrations of norepinephrine, epinephrine, and lactate were determined at work loads corresponding to approximately 40, 60, 80, and 100% peak VO2. Peak VO2 did not differ on land and with WI. Plasma norepinephrine concentration was reduced (P less than 0.05) at 80 and 100% peak VO2 with WI and on land, respectively. Plasma epinephrine and lactate concentrations were similar on land and with WI at the three submaximal work stages, but both were reduced (P less than 0.05) at peak exertion with WI. Heart rate was lower (P less than 0.05) at the three highest work intensities with WI. These results suggest that the central shift in blood volume with WI reduces the sympathoadrenal response to high-intensity dynamic exercise.

Abstract:Mutations that influence lin-12 activity in Caenorhabditis elegans may identify conserved factors that regulate the activity of lin-12/Notch proteins. We describe genetic evidence indicating that sel-10 is a negative regulator of lin-12/Notch-mediated signaling in C. elegans. Sequence analysis shows that SEL-10 is a member of the CDC4 family of proteins and has a potential human ortholog. Coimmunoprecipitation data indicate that C. elegans SEL-10 complexes with LIN-12 and with murine Notch4. We propose that SEL-10 promotes the ubiquitin-mediated turnover of LIN-12/Notch proteins, and discuss potential roles for the regulation of lin-12/Notch activity by sel-10 in cell fate decisions and tumorigenesis.
Optima Localization in Scheduling Multi-Processor Jobs

Abstract:The paper considers a multi-processor tasks scheduling problem with dedicated processors. The main target is determining the tight optima localization intervals for different sub-problems of the basic problem. Based on the ideas of a computer-aided technique developed in the paper by Sevastyanov and Chernykh [15] for shop scheduling problems, we elaborated a similar method for a different-type problem (see above). For small values of the number of processors (from 3 to 5) a computer program coped with the main target. The extremum values of the optimum (in terms of a standard lower bound taken for 1) computed for several sub-problems of the basic problem are listed in Table 1 (page 7). In parallel with those sub-problems, so called prime-versions of those problems were investigated, in which only jobs requiring more than one processor are allowed. An interesting phenomenon was discovered: nearly all (but one) prime-sub-problems appeared to be polynomial-time solvable (with optima coinciding with their lower bounds – such instances and classes of instances were called in [10, 11] normal). As a by-product of those results, a family of lineartime approximation algorithms was designed (for the sub-problems listed in Table 1) with ratio performance guarantees taken from Table 1.

Abstract:Background: The study investigated the concentrations of plasma catecholamine, adrenaline (A) and noradrenaline (NA), and the adrenal medulla responsiveness to the sympathetic nervous activity in sprinters (S), endurance runners (E) and untrained subjects (U) during a supramaximal exercise (the Wingate-test). Methods: A group of 19 men took part in the tests: 6 S (20.5+/-0.7 years), 6 E (21.0+/-1.0 years) and 7 U (20.9+/-0.4 years). The maximal power (Wmax) and the mean power (W) were determined from the Wingate-test on a cycle ergometer. The plasma lactate, A and NA were analysed at rest (La0, A0 and NA0), immediately at the end of the exercise (Amax and NA(max)) and after 5 min recovery (La(max), A5 and NA5). The disappearance of A and NA was judged by the difference between the maximal values and those determined after 5 min recovery (Amax-A5 and NA(max)-NA5) and the ratio A/NA was considered as an index of the adrenal medulla responsiveness to the sympathetic nervous activity. Results: During the Wingate-test S exhibited higher performances and higher La(max) than the two other groups. At the end of the Wingate-test the NA(max) values were similar in the three groups whereas the Amax values were significantly higher in S than in E and U (8.00+/-0.5 nmol x l(-1) in S vs 3.47+/-0.30 nmol x l(-1) and 3.29+/-1.14 nmol x l(-1) respectively in E and U). This leads to a higher Amax/NA(max) ratio for S compared to the other two groups (0.77+/-0.10 in S vs 0.23+/-0.03 and 0.28+/-0.05, respectively in E and U). As the disappearance of A (Amax-A5) was significantly higher in S (6.80+/-0.47 nmol x l(-1) in S vs 2.64+/-0.19 nmol x l(-1) and 1.64+/-1.37 nmol x l(-1), respectively in E and U), the higher values of Amax in S could be explained by an increase of the adrenal medullary secretory capacity in this group. Conclusions: It was concluded that essentially short term and intense exercises as sprint ones or interval-training may alter the adrenal medulla responsiveness to supramaximal exercise but not long duration exercises.

Abstract:Neuropeptide Y 1-36 (NPY1-36) is a vasoconstrictor peptide co-secreted with norepinephrine (NE) by nerve endings during sympathetic activation. NPY1-36 potentiates NE action post-synaptically through the stimulation of the Y1 receptor, whereas its metabolite NPY3-36 resulting from DPP4 action activates Y2 presynaptic receptors, inhibiting NE and acetylcholine secretion. The secretions of NPY1-36 and NPY3-36 in response to sympathetic nervous system activation have not been studied due to the lack of analytical techniques available to distinguish them. We determined in healthy volunteers NPY1-36, NPY3-36 and catecholamine kinetics and how these neurotransmitters modulate the physiological stress response during and after moderate- and heavy-intensity exercises. Six healthy males participated in this randomized, double-blind, saxagliptin vs placebo crossover study. The volunteers performed an orthostatic test, a 30-min exercise at moderate intensity and a 15-min exercise at heavy intensity each followed by 50 min of recovery in two separate sessions with saxagliptin or placebo. Oxygen consumption (V̇O2), ventilation and heart rate were continuously recorded. NE, epinephrine, NPY1-36 and NPY3-36 were quantified by tandem mass spectrometry. We found that exercise triggers NPY1-36 and NE secretion in an intensity-dependent manner and that NE returns faster to the baseline concentration than NPY1-36 after exercise. NPY3-36 rises during recovery parallel to the decline of NPY1-36. Saxagliptin reverses the NPY1-36/NPY3-36 ratio but does not affect hemodynamics, nor NPY1-36 and catecholamine concentrations. We found that NPY1-36 half-life is considerably shorter than previously established with immunoassays. NPY1-36 and NE secretions are finely regulated to prevent an excessive physiological Y1 stimulating response to submaximal exercise.
