Abstract:Background: The local management of Ewing sarcoma family of tumors (ESFT) often centers on the surgical resectability of the primary lesion and physician biases regarding differences in the morbidity between primary surgical and radiotherapeutic management. Methods: The authors retrospectively reviewed the records of 33 patients with localized ESFT who underwent surgery and received systemic chemotherapy at St. Jude Children's Research Hospital (Memphis, TN). Two multiagent systemic chemotherapy regimens were used: 14 patients received vincristine, doxorubicin, cyclophosphamide, and actinomycin D (VACA), and 19 received VACA in combination with ifosphamide and etoposide. The primary tumor was surgically resected via a wide, local excision (n = 32) or a marginal excision (n = 1)performed either at diagnosis or after 3-5 months of systemic chemotherapy. Clinical outcome and prognostic factors for disease control were reported in the current study. Results: The median follow-up for patients was 9.9 years. The 5-year and 10-year survival rates were 84.5% and 75.8%, respectively. At 5 years, the cumulative incidence of local disease recurrence was 12.5%, and the event-free survival (EFS) rate was 71.7%. The same values were found at 10 years. The site of tumor origin was a significant predictor of EFS. The survival rate of patients whose tumors arose in bone was 78.6%, and the survival rate of patients whose tumors originated in soft tissue was 25.0% (P = 0.028). No other factors investigated were predictive of outcome. Conclusions: Local disease control and overall outcome for patients with ESFT managed by multiagent systemic therapy and surgery was excellent. Local disease control rates remained near 90% at 10-year follow-up. Patients with extraosseous primary sites of disease may fare less well with this approach to therapy.
Definitive irradiation in multidisciplinary management of localized Ewing sarcoma family of tumors in pediatric patients: outcome and prognostic factors

Abstract:Purpose: To assess the effect of radiation dose on local tumor control of the Ewing sarcoma family of tumors in 79 patients with localized disease treated at a single institution. Methods and materials: Thirty-seven patients received vincristine, actinomycin D, cyclophosphamide, and doxorubicin, and 42 received vincristine, actinomycin D, and cyclophosphamide, with alternating cycles of ifosfamide and etoposide; all underwent definitive radiotherapy (median dose, 37.5 Gy) with either low-dose (<40 Gy) or standard dose (> or =40 Gy) radiation delivered according to the protocol. We calculated the cumulative incidence of local treatment failure, disease recurrence, and overall survival and analyzed the effect of known prognostic factors and radiation dose. Results: The cumulative incidence of local treatment failure at 10 years was 30.4% and that of disease recurrence was 40%. The overall survival rate was 64.5%. Patient age > or =14 years and tumor size > or =8 cm were adverse prognostic factors for local treatment failure; patient age > or =14 years was also associated with worse survival. Although the radiation dose alone did not predict for local treatment failure, the cumulative incidence of local failure at 10 years was 19% when tumors <8 cm were treated with <40 Gy, and no patient treated with standard doses (> or =40 Gy) developed local recurrence (p = 0.084). Conclusion: Tumor size and patient age predict for local tumor control in patients with Ewing sarcoma family of tumors treated with systemic therapy and definitive radiotherapy. Patients treated with reduced-dose radiotherapy experienced unacceptably high rates of local recurrence.

Abstract:Fifty-two previously untreated patients with localized Ewing's sarcoma of bone were treated with nonintensive chemotherapy in combination with surgery or radiation therapy (RT). RT was delivered to limited volumes in a dose dependent on the initial response to induction chemotherapy (30 to 35 Gy v 50 to 55 Gy). Fifty of the 52 patients achieved complete or partial responses with induction chemotherapy, with one nonresponding patient rendered free of tumor with surgery. Fifty patients were evaluable for local control of tumor and overall response to protocol therapy. Seventeen relapses have occurred; three metastatic, four local plus metastatic, and ten local. Two factors predicted worse disease-free survival: high WBC count (P = .03) and size of primary tumor (P = .05). Of the 14 local recurrences, 12 occurred in 28 patients who presented with primary tumors greater than 8 cm in size while only two of 22 patients with lesions less than 8 cm had local recurrence. The Kaplan-Meier estimate of disease-free survival at 3 years is 82% for those with small lesions and 64% for those with larger lesions. Site of primary was of no prognostic value (P = .27). The 5-year survival estimate for all patients is 80% (median time on study, 3.3 years).
Prognostic factors for local and distant control in Ewing sarcoma family of tumors

Abstract:Background: Advances in the treatment of Ewing sarcoma family of tumors (ESFT) are the result of improvements in systemic and local therapies. The individual contributions of each treatment component cannot be analyzed separately; improvements in local and systemic control can influence each other. Patients and methods: We reviewed the records of 220 patients treated on institutional protocols from 1979 to 2004. Factors predictive of local and distant recurrence were analyzed. Results: The median age at diagnosis was 13.7 years. Ninety-five patients relapsed at a median of 1.6 years. The 5-year overall survival estimate was 63.5% +/- 3.5%. The estimated 5-year cumulative incidence (CI) of local failure was 25.1% +/- 3.0%. Local failure was associated with treatment era (P < 0.001), tumor size (P = 0.037) and type of local control (P = 0.021). Systemic treatment intensification improved local control. The estimated 5-year CI of distant recurrence was 22.5% +/- 2.9%. Patients with localized disease (P < 0.001), smaller tumors (P = 0.018) and those who received surgery +/- radiation for local control (P = 0.023) had lower CI of distant failure. Conclusions: Successful treatment of ESFT requires optimal systemic and local therapy. Both treatment modalities are intertwined and the control of both local and distant disease is the result of the combined approach.

Abstract:In two studies the authors want to confirm experimentally what is assumed in every day ENT-practice: otitis and sinusitis are not infrequent complications of swimming. The first study, involving 60 children with chronic non-specific lung diseases shows that swimming in a sea water pool increases the nasal airway resistance during two days. In the second study, performed in 32 normal children swimming in a chlorous pool, a negative relationship is found between the number of days after the swimming and the increased nasal airway resistance. Probably other factors also play a part in this phenomenon. Due to the too small number of children showing an otitis media with effusion, the influence of swimming on the function of the Eustachian tube could not be determined.
Radiation therapy dose escalation achieves high rates of local control with tolerable toxicity profile in pediatric and young adult patients with Ewing sarcoma

Abstract:Background Local control for patients with Ewing sarcoma (EWS) who present with large tumors are suboptimal when treated with standard radiation therapy (RT) doses of 54–55.8 Gy. The purpose of this study is to determine local control and toxicity of dose‐escalated RT for tumors ≥8 cm (greatest diameter at diagnosis) in pediatric and young adult patients with EWS. Methods Eligible patients ≤30 years old with newly diagnosed EWS ≥8 cm treated with definitive conformal or intensity modulated photon, or proton radiation therapy techniques were included. All patients in the study received dose‐escalated RT doses. Outcomes included overall survival (OS), event‐free survival (EFS), local failure rates, and toxicity. Results Thirty‐two patients were included, 20 patients presented with metastatic disease and 12 patients with localized disease. The median RT dose was 64.8 Gy (range, 59.4–69.4 Gy) with variability of doses to protect normal surrounding tissues. All patients received systemic chemotherapy. The 5‐year OS and EFS for the cohort was 64.2% and 42%, respectively. The 5‐year cumulative incidence of local failure was 6.6%. There were two combined local and distant failures with no isolated local failures. Twenty‐nine patients experienced short term toxicity, 90% of those being radiation dermatitis. Twenty‐seven patients experienced long‐term toxicity, with only one experiencing grade 4 toxicity, a secondary malignancy after therapy. Conclusion This study demonstrates that definitive RT for pediatric and young adult patients with EWS ≥8 cm provides high rates of local control, while maintaining a tolerable toxicity profile.

Abstract:BACKGROUND AND OBJECTIVES: There is continuing debate regarding the ideal modality for local control of the primary tumor for patients with Ewing's sarcoma. The primary aim of this study is to investigate the impact of the method of local control on overall survival in patients with Ewing's sarcoma.METHODS: The National Cancer Data Base was used to identify patients <40 years of age with high-grade Ewing's sarcoma of bone. A Kaplan-Meier survival analysis was performed at 2, 5, and 10 years. Factors with a level of significance of P < 0.1 at the 5-year time point were included in a multivariate Cox proportional hazards model.RESULTS: Diminished 5-year survival was noted for patients with metastatic disease, local control with radiation alone, age ≥18 years, tumor size >8 cm, and male sex while controlling for tumor site. Surgery alone was consistently the method of local control that resulted in the highest overall survival.CONCLUSIONS: Surgery alone resulted in the best overall survival for patients with Ewing's sarcoma of bone. The results of this investigation provide support to the approach of surgical resection with negative margins when possible.

Abstract:Limited information exists on the clinical behavior of the Ewing sarcoma family of tumors (ESFT) of the kidney. We reviewed the records of 30 patients (aged 8-69 years) with ESFT of the kidney seen at our institution between 1990 and 2013. We analyzed the event-free survival (EFS) and overall survival (OS) for associations with patient demographics, disease group, tumor size, tumor thrombus, and treatment. Six patients (20%) had tumors confined to the kidney (Group I), seven (23.3%) had local tumor extension beyond the kidney (Group II), and 17 (56.7%) had distant metastasis at diagnosis (Group III). Twenty-five (83.3%) patients underwent radical (19 upfront, five delayed) or partial (one upfront) nephrectomy, 25 (83.3%) chemotherapy and four (13.3%) radiotherapy. The 4-year EFS and OS were 43% (95% CI, 26-61%) and 63% (95% CI, 46-81%), respectively. EFS and OS were significantly associated with disease group and chemotherapy (p < 0.039). The presence of tumor thrombus in renal vein and/or inferior vena cava was associated with worse EFS (p = 0.053). Patients with disease confined to the kidney treated with nephrectomy and adjuvant chemotherapy have favorable outcomes. Local tumor extension beyond the kidney, tumor thrombus, and distant metastasis are unfavorable factors that warrant intensification or novel approaches of therapy.
The treatment and prognosis of primary spinal Ewing′s sarcoma family tumor

Abstract:Objectives: To investigate the clinical features, treatment and prognosis of patient with primary spinal Ewing′s sarcoma family tumor (ESFT). Methods: 28 patients were retrospectively investigated between May 1997 and June 2010, 18 patients(17 males and 11 females) with Ewing′s sarcoma and 10 patients with peripheral malignant neuroectodermal tumor (PNET). The age of patients ranged from 5 to 45 years with an average of 19 years. The tumor involved cervical vertebrae(4 cases), thoracic vertebrae(5 cases), lumbar verte-brae (6 cases), thoracic-lumbar segments (1 case) and sacral vertebrae (12 cases). The treatment included surgery, chemotherapy and/or radiotherapy. A five-drug regimen of chemotherapy (vincristine, doxorubicin, cy-clophosphamide, ifosfamide and etoposide) was administered to all patients. Surgery was performed on 27 pa-tients, surgery followed by radiotherapy on 22. 7 cases underwent en bloc surgery. Results: The follow-up period was from 3 to 12 years with an average of 6.7 years. One patient died of tumor progression during chemotherapy. Twenty patients(20/23, 86.9%) obtained satisfied outcomes with nerurofunction improved for 1-2 Frankel grade. Nine (33.3%) patients developed isolated recurrence and 10 (37.1%) patients developed lung metastasis. Only 3 patients developed multiple bone metastasis. The treatment after relapse included adminis-tration of second line chemotherapy(13 cases) and combined surgery(6 cases). 11 patients died at final fol-low-up. The 2-year and 5-year survival rate was 74.3% and 53.3%, respectively. Conclusions: The spinal Ewing′s sarcoma family tumor is a highly malignant tumor with poor prognosis. The local control and chemotherapy is necessary for improving the prognosis of the ESFT. Surgery plays an important role especially for the cases in which the tumor can be resected. En bloc resection is useful to decrease the recurrence. Ra-diotherapy is applicable for the patient whose tumor can not be resected.
Improved Local Control by Surgery and Paclitaxel-Based Chemoradiation for Esophageal Squamous Cell Carcinoma: Results of a Retrospective Non-Randomized Study.

Abstract:Background and Objectives: To investigate the impact of using paclitaxel in chemoradiation on locally advanced esophageal squamous cell carcinoma (SCC) treated with or without surgery.Methods: Of 127 patients with AJCC stages II-III esophageal SCC undergoing definitive chemoradiation (DefCRT, n = 44) or ncoadjuvant chemoradiation plus surgery (NeoCRT + S, n = 83), 57 received chemotherapy with paclitaxel and cisplatin (TP), and 70 received 5-fluorouracil and cisplatin (PF). Three-year local progression-free survival (LPFS), distant metastasis-free survival, overall survival, and prognostic factors were retrospectively analyzed.Results: The median survival was 30 months. Pathological complete response rate was 41 % and 27% with TP and PF, respectively (P = 0. 19). NeoCRT + S achieved significantly higher LPFS than DefCRT (71 % vs. 39%, P < 0.001). Patients receiving TP had significantly higher LPFS than PF (74% vs. 48%, P = 0.04). Local control was similar between DefCRT with TP and NeoCRT + S. Distant metastasis-free survival and overall survival were not different between treatment modalities or chemotherapy regimens. In multivariate analysis, surgery (HR 0.30, P < 0.00 1), TP regimen (HR 0.38, P = 0.007), and mediastinal lymphadenopathy (HR 2.37, P = 0.008) were independent factors for LPFS.Conclusions: Both surgery and the use of paclitaxel-based chemoradiation may improve local disease control. Future randomized trials should integrate paclitaxel into definitive chemoradiation.
Comprehensive radiotherapy for pediatric Ewing Sarcoma: Outcomes of a prospective proton study

Abstract:Background and purpose Patients with Ewing Sarcoma (EWS) are treated with multimodality therapy which includes radiation therapy (RT) as an option for local control. We report on the efficacy after proton radiation therapy (PRT) to the primary site for localized and metastatic EWS. Materials and methods Forty-two children with EWS (33 localized, 9 metastatic) treated between 2007 and 2020 were enrolled on 2 prospective registry protocols for pediatric patients undergoing PRT. PRT was delivered by passive scatter (74 %), pencil-beam scanning (12 %) or mixed technique (14 %). Treated sites included the spine (45 %), pelvis/sacrum (26 %), skull/cranium (14 %), extraosseous (10 %), and chest wall (5 %). Median radiation dose was 54 Gy-RBE (range 39.6–55.8 Gy-RBE). Patients with metastatic disease received consolidative RT to metastatic sites (4 at the time of PRT to the primary site, 5 after completion of chemotherapy). Median follow-up time was 47 months after PRT. Results The 4-year local control (LC), progression-free survival (PFS), and overall survival (OS) rates were 83 %, 71 %, and 86 %, respectively. All local failures (n = 6) were in-field failures. Tumor size ≥ 8 cm predicted for inferior 4-year LC (69 % vs 95 %, p = 0.04). 4-year PFS and OS rates were not statistically different in patients with localized versus metastatic disease (72 % vs 67 %, p = 0.70; 89 % vs 78 %, p = 0.38, respectively). Conclusion In conclusion, LC for pediatric patients with EWS treated with PRT was comparable to that of historical patients who received photon-RT. Tumor size ≥ 8 cm predicted increased risk of local failure. Patients with metastatic disease, including non-pulmonary only metastases, received radiation therapy to all metastatic sites and had favorable survival outcomes.

[Analysis of 41 cases of non-metastatic Ewing's sarcoma in children].

Abstract:OBJECTIVES:To summarize the clinical characteristics, treatment outcomes, and prognostic factors of children with non-metastatic Ewing's sarcoma (ES). METHODS:A retrospective analysis was conducted on the clinical data of 41 children with non-metastatic ES diagnosed and treated at the Shanghai Children's Medical Center, Shanghai Jiao Tong University School of Medicine from January 2010 to December 2018. All patients underwent chemotherapy based on the RMS-2009 protocol of the center, and local treatment such as surgery and/or radiotherapy was performed according to risk grouping. The Kaplan-Meier method was used to calculate the overall survival (OS) and event-free survival (EFS) rates. Univariate prognostic analysis was performed using the log-rank test, and multivariate analysis was conducted with Cox regression. RESULTS:Of the 41 children, 21 were male and 20 were female. The median age at diagnosis was 7.7 years (range: 1.2-14.6 years). The median follow-up time for patients with event-free survival was 68.1 months (range: 8.1-151.7 months). As of the last follow-up, 33 patients were in complete remission, and the overall 5-year EFS and OS rates were (78±6)% and (82±6)%, respectively. Univariate analysis by the log-rank test showed that a tumor diameter ≥8 cm, time from diagnosis to start of local treatment ≥16 weeks, and incomplete surgical resection were associated with poor prognosis (P<0.05). Multivariate Cox regression analysis indicated that incomplete surgical resection (HR=8.381, 95%CI: 1.681-41.801, P=0.010) was an independent risk factor for poor prognosis in children with ES. Secondary tumors occurred in 2 cases. CONCLUSIONS:A comprehensive treatment strategy incorporating chemotherapy, surgery, and radiotherapy can improve the prognosis of children with ES. Poor prognosis is associated with an initial tumor diameter ≥8 cm, while complete surgical resection and early initiation of local treatment can improve outcomes.

Abstract:Ewing sarcoma (ES) is an aggressive sarcoma of bone and soft tissue occurring at any age with a peak incidence in adolescents and young adults. The treatment of ES relies on a multidisciplinary approach, coupling risk-adapted intensive neoadjuvant and adjuvant chemotherapies with surgery and/or radiotherapy for control of the primary site and possible metastatic disease. The optimization of ES multimodality therapeutic strategies has resulted from the efforts of several national and international groups in Europe and North America and from cooperation between pediatric and medical oncologists. Successive first-line trials addressed the efficacy of various cyclic combinations of drugs incorporating doxorubicin, vincristine, cyclophosphamide, ifosfamide, etoposide, and dactinomycin and identified prognostic factors now used to tailor therapies. The role of high-dose chemotherapy is still debated. Current 5-year overall survival for patients with localized disease is 65% to 75%. Patients with metastases have a 5-year overall survival < 30%, except for those with isolated pulmonary metastasis (approximately 50%). Patients with recurrence have a dismal prognosis. The many insights into the biology of the EWS-FLI1 protein in the initiation and progression of ES remain to be translated into novel therapeutic strategies. Current options and future approaches will be discussed.

Abstract:Background: Despite improved therapies, 30-40% of patients with Ewing tumors (ET) experience recurrence and have a poor prognosis. The authors analyzed factors prognostic of survival in patients with recurrent ET. Methods: The authors assessed the relation between postrecurrence survival (PRS) and demographic, disease, and treatment factors in 71 patients who experienced recurrent ET after treatment on one of three consecutive institutional protocols. Results: Thirty-four patients (47.9%) had distant recurrence, 25 patients (35.2%) had local recurrence, and 12 patients (16.9%) had both distant and local recurrence at a median of 1.7 years after diagnosis. The probability of 5-year PRS (+/- 1 standard error) was 17.7%+/-4.5%. Recurrence > or = 2 years after diagnosis predicted a significantly better outcome (5-year PRS, 34.9%+/-8.5%) compared with earlier recurrence (5.0%+/-2.8%; P < 0.001). Patients who had both local and distant recurrence fared worse (5-year PRS, 12.5%+/-8.3%) compared with patients who had local recurrence alone (21.7%+/-7.8%) or distant recurrence alone (17.6+/-6.1%). Among patients with local recurrence alone, those who underwent salvage with radical surgery had significantly higher 5-year PRS estimates (31.4%+/-11.6%) compared with the other patients (9.1%+/-6.1%; P = 0.023). Pulmonary irradiation significantly improved the outcomes of patients with isolated pulmonary recurrence (5-year PRS estimate, 30.3%+/-12.5% vs. 16.7%+/-10.8%, respectively; P = 0.018). Conclusions: Although outcomes are generally poor after patients experience recurrence of ET, certain patient groups differ appreciably in their likelihood of survival. Patients who experience recurrence > or = 2 years after diagnosis and patients who have local recurrence that can be treated with radical surgery and intensive chemotherapy have the most favorable outcomes.

Abstract:Congenital syphilis is an infection transmitted from mother to fetus and can present with early but variable cutaneous manifestations. In rare situations, a bullous eruption known as pemphigus syphiliticus may develop. We present an unusual case of broad desquamation of the extremities in a newborn infant who was found to have congenital syphilis. Pemphigus syphiliticus should be considered in the differential diagnosis of neonatal bullous eruptions and erosions.
Postoperative Intensity Modulated Radiation Therapy Improved Survival of Lymph Node-Positive or Stage III Squamous Cell Carcinoma Esophagus

Abstract:To retrospectively analyze the effect of postoperative intensity-modulated radiation therapy (IMRT) on the recurrence and survival in patients with lymph node positive or Stage III thoracic esophageal squamous cell carcinoma (TESCC), and evaluate the role of postoperative IMRT in the TESCC therapy. From January 2004 to June 2009, 535 patients with TESCC underwent radical resection with or without postoperative IMRT were eligible for analysis. Inclusion criteria: (1) having undergone radical resection (R0); (2) a diagnosis of squamous cell carcinoma confirmed by pathological studies; (3) Karnofsky performance status scale ≥70; (4) Preoperative neoadjuvant therapy was not proceeded; (5) postoperative pathologic stage was IIb or III. According to UICC02 stage classification, 122 patients were stage IIb and 413 patients were stage III. Patients were grouped to surgery alone (n = 345) and surgery plus postoperative IMRT (n = 190). The median total dose in postoperative IMRT was 60 Gy. The Kaplan-Meier method was used to calculate the survival rates, and the log-rank test was used for univariate analysis. The chi-square test was used to analyze the sites of failure of esophageal cancer with different treatments. The 1-, 3-, and 5-year overall survival (OS) and disease-free survival (DFS) for all the patients were 79.5%, 42.7%, 32.8%, and 67.2%, 38.0%, 32.5%, and the median OS and DFS were 28.6 months and 21.5 months, respectively. For patients with stage IIb and stage III, the 5-year OS rates were 45.3% and 29.2%, respectively (p = 0.001). The 5-year OS in the patients with lymph node positivity was 28.5% in the group receiving surgery alone and 38.9% in the group undergoing surgery plus postoperative IMRT (p = 0.000). The 5-year OS rates in patients with stage III was 24.3% in the surgery group and 38.0% in the surgery plus postoperative IMRT group, respectively (p = 0.001). Use of postoperative IMRT resulted in significant decrease in intrathoracic recurrence from 33.6% to 13.3% (p = 0.000) as well as supraclavicular recurrence from 13.4% to 6.1% (p = 0.012), Similarly, use of postoperative IMRT obviously delayed the median local recurrence from 10.7 months to 20.7 months and the median system metastases from 8.8 months to 14.5 months. While systemic metastases increased from 21.6% to 30.4% (p = 0.032) by postoperative IMRT treatment. Postoperative IMRT reduced local recurrence and improved survival for patients with lymph node positive or stage III TESCC, which would provide rationale in establishing patient selection criteria for the postoperative IMRT for TESCC after surgery.

Abstract:The aim of the present study was to evaluate the antitumor activity of drugs in phase II clinical trials for recurrent or refractory EWS. A systematic review was performed using clinical trials from four data sources: i) ClinicalTrials.gov; ii) PubMed; iii) Clinicaltrialsregister.eu; and iv) American Society of Clinical Oncology. The search terms included: '(Ewing sarcoma OR Ewing family of tumors) AND (phase II OR phase I/II)'. Overall, 465 trials were identified and 64 were included in the present study, of which, 37 had published results. The highest objective response rate came from irinotecan-based chemotherapy. Currently, the majority of targeted therapy has failed to demonstrate any activity except for regorafenib. Trials using anti-angiogenesis small molecular tyrosine kinase inhibitors (aaTKIs) are currently ongoing with promising early results. For immunotherapy, anti-insulin like growth factor 1 receptor antibody demonstrated disappointing activity. The best outcome came from irinotecan-based regimens. Targeted therapy with aaTKIs is worthy of further investigation, with immunotherapy is not recommended for off-label use.
Ewing's Sarcoma of the Pelvis: Treatment Results of 31 Patients

Abstract:OBJECTIVE:To define oncologic and functional outcomes of multidisciplinary treatment methods combined chemotherapy, surgery and radiotherapy, patients with Ewing's sarcoma of the pelvis who were treated at our center were analyzed.METHODS:Thirty-one patients with Ewing's sarcoma of the pelvis were eligible for this analysis. Primary lung metastases occurred in ten patients who were classified as stage III according to the system of Musculoskeletal Tumor Society (MSTS). All the patients received multidisciplinary treatment methods combined chemotherapy, surgery and radiotherapy. Twenty-eight patients underwent limb-sparing surgery, three patients underwent hemipelvectomy. The complications, including radiation or surgery-related complications and mechanical failures were recorded, besides the functional results were evaluated according to the MSTS 93 criteria. Kaplan-Meier survival analysis and Cox multivariate analysis were used as statistical methods.RESULTS:The 5-year overall survival (OS) rate for all the patients was 42.3%. The 5-year OS for patients with small tumor (< 10 cm) had a significant better prognosis than patients with large tumor (≥ 10 cm, 58.2% vs. 26.0%, χ(2) = 4.382, P < 0.05). The 5-year OS for patients with surgical stage IIb was significantly better than for those with surgical stage III (50.2% vs. 30.3%, χ(2) = 4.521, P < 0.05). The 5-year OS for patients with wide or radical surgery had a better prognosis than patients with marginal, intralesional surgery or no surgery (60.0% vs. 29.4%, χ(2) = 4.851, P < 0.05). In multivariate analysis, surgical stage III (RR = 2.480, 95%CI: 0.857 - 7.173) and resection margin (RR = 2.576, 95%CI: 0.474 - 14.011) were independent prognostic factors. The mean MSTS 93 score for all the patients was 63.3%. The complication rate in patients treated with surgery was 29.0% (9/31).CONCLUSIONS:Patients with Ewing'sarcoma of the pelvis can benefit from multidisciplinary treatment in terms of improved survival, acceptable functional results and reasonable complications. Surgical staging and resection margin are independent prognostic factors.
Individual risk evaluation for local recurrence and distant metastasis in Ewing sarcoma: A multistate model

Abstract:BackgroundWe investigated the effects of surgical margins, histological response, and radiotherapy on local recurrence (LR), distant metastasis (DM), and survival in Ewing sarcoma. ProcedureDisease evolution was retrospectively studied in 982 patients with Ewing sarcoma undergoing surgery after chemotherapy using a multistate model with initial state surgery, intermediate states LR, pulmonary metastasis (DMpulm), other DM ± LR (DMother), and final state death. Effect of risk factors was estimated using Cox proportional hazard models. ResultsThe median follow‐up was 7.6 years (95% CI, 7.2‐8.0). Risk factors for LR are pelvic location, HR 2.04 (1.10‐3.80), marginal/intralesional resection, HR 2.28 (1.25‐4.16), and radiotherapy, HR 0.52 (0.28‐0.95); for DMpulm the risk factors are <90% necrosis, HR 2.13 (1.13‐4.00), and previous pulmonary metastasis, HR 4.90 (2.28‐8.52); for DMother are 90% to 99% necrosis, HR 1.56 (1.09‐2.23), <90% necrosis, HR 2.66 (1.87‐3.79), previous bone/other metastasis, HR 3.08 (2.03‐4.70); and risk factors for death without LR/DM are pulmonary metastasis, HR 8.08 (4.01‐16.29), bone/other metastasis, HR 10.23 (4.90‐21.36), and <90% necrosis, HR 6.35 (3.18‐12.69). Early LR (0‐24 months) negatively influences survival, HR 3.79 (1.34‐10.76). Once DMpulm/DMother arise only previous bone/other metastasis remain prognostic for death, HR 1.74 (1.10‐2.75). ConclusionDisease extent and histological response are risk factors for progression to DM or death. Tumor site and surgical margins are risk factors for LR. If disease progression occurs, previous risk factors lose their relevance. In case of isolated LR, time to recurrence is important for decision‐making. Radiotherapy seems protective for LR especially in pelvic/axial. Low percentages of LR in extremity tumors and associated toxicity question the need for radiotherapy in extremity Ewing sarcoma.
