Abstract:Cardiac conduction velocity (CV) is a critical electrophysiological characteristic of the myocardium, representing the speed at which electrical pulses propagate through cardiac tissue. It can be delineated into longitudinal, transverse, and normal components in the myocardium. The CV and its anisotropy ratio are crucial to both normal electrical conduction and myocardial contraction, as well as pathological conditions where it increases the risk of conduction block and reentry. This comprehensive review synthesizes longitudinal and transverse CV values from clinical and experimental studies of human infarct hearts, including findings from the isthmus and outer loop, alongside data derived from animal models. Additionally, we explore the anisotropic ratio of conductivities assessed through both animal and computational models. The review culminates with a synthesis of scientific evidence that guides the selection of CV and its corresponding conductivity in cardiac modeling, particularly emphasizing its application in patient-specific cardiac arrhythmia modeling.

Abstract:Bioimpedance measurements are a non-invasive method to determine the composition of organic tissue. For measuring the complex bioimpedance between two electrodes, an alternating current with a constant amplitude is injected into the tissue. The developed voltage drop is used to calculate the real and imaginary part of the impedance under test. Measurements in the past indicated that it could be possible that the beating of the heart has an effect on the measured phase shift of the impedance under test. In this work a hardware system is developed, capable of measuring changes of the bioimpedance phase shift with high resolution. Two different measurement methods are used. The first method is an analog circuit, which has been developed in a previous project. The other method uses the integrated circuit AFE4300 (Texas Instruments) [1]. The results of both methods are transmitted with a microcontroller (Xmega256A3BU from Atmel Corp.) via the USB interface to a host PC. On the host PC the visualization of the measurements and the control of the embedded system is achieved with a developed LabView program. The work includes the development of the hardware and software, as well as a comparison of the accuracy of the two measurement methods. The result of a verification of the systems is that both measurement methods have an effective resolution of about 0.3°.

Abstract:Microconductance catheters have been successfully applied to measure left ventricular (LV) function in the mouse to assess cardiac or pharmacological interventions for a number of years. New complex admittance methods produce an estimate of the parallel admittance of cardiac muscle that can be used to correct the measurement in real time. This contrasts with existing conductance technologies that require in vivo calibration using a bolus of hypertonic saline. Here, we report the application of this emerging technology in the context of myocardial infarction and LV remodelling. Using a combination of high-resolution ultrasound and LV conductance catheters, we compared measures of LV function using an admittance system and a traditional conductance-derived pressure-volume (PV) system. We subjected C57BL/6 mice to focal myocardial ischaemia-reperfusion by transient ligation of the left anterior descending coronary artery and assessed cardiac function with different systems to determine the reliability and accuracy of these methods to distinguish between normal and dysfunctional ventricle. We demonstrate that the admittance PV system, in our hands, provides a straightforward solution for assessing LV function in mice. Using this technique in combination with other established methods, we measured LV dysfunction following coronary artery occlusion and reperfusion, which can be ameliorated using a known preconditioning agent (CORM-3), and found that functional read-outs are representative of other methods. We have found that, especially in diseased tissue, LV pressure-volume loops derived from complex admittance provide a reproducible and reliable method of determining LV function without the need for technically challenging calibration. Our data suggest that admittance records accurate/physiological LV cavity volumes when compared with other invasive methods in the same animal. This emerging technology is both effective and reproducible for measuring LV function and dysfunction in the mouse, without the need for complicated interventions to calibrate the measurements or training in a new technology. This may mark the way towards a fast and accurate assessment of murine cardiac function in normal animals and disease models.

Abstract:Monitoring cardiac mechanical dyssynchrony of a patient has a pivotal role in the cardiovascular disease and cardiac resynchronization therapy (CRT), however current approaches highly demand on the operator's skill and experience. In addition, the clinical instrument is limited by high standard operation conditions and hardware and maintenance costs, which leads to restricted deployment in daily life monitoring. To solve the problems, we propose a code division multi-point near-field cardiac RF sensing system that the mechanical movements of the heart chambers can be directly modulated to the radio frequency signal, from which the signals obtained through processing and analysis can not only locate the P-wave, QRS-waves, and T-wave in the electrocardiogram (ECG), but also they are used to analyze atrioventricular mechanical dyssynchrony and interventricular mechanical dyssynchrony. Furthermore, to reduce the crosstalk between signals from different channels and improve digital signal dynamic range, code division is applied for the multiple sensing points access. The RF sensing system can be readily designed with a high sampling rate, 50 kSps in this work, to recover the high frequency mechanical dynamics of the heart. The system demonstrated in this work provides 4-channel cardiac monitoring on the heart chambers with a high dynamic range, temporal resolution of 20 mu s, and frequency response of 25 kHz. This novel method of monitoring the mechanical movement of the heartbeat provides a new candidate to further explore the cardiovascular disease and CRT treatment.

Abstract:Electromechanical imaging is a novel technique for the noninvasive mapping of electrical conduction waves in the left ventricle through the combination of ECG gating, high frame rate and RF-based displacement estimation techniques. In this paper, we identify and separate the electromechanical waves from the hemodynamically induced waves and determine the dependence of the wave direction and velocity at different pacing scenarios. In vivo imaging (30 MHz) was performed on anesthetized wildtype mice (n=12) at high frame rates (Vevo 770, Visualsonics, Inc.) in order to better explore the electromechanical coupling within the heart muscle. The acquisition was triggered on the mouse electrocardiogram (ECG) and yielded a high frame rate of 8000 fps. RF frames from long-axis views were digitized at 160 MHz. Axial, frame-to-frame displacements were estimated using 1D cross-correlation (window size of 240 microns, overlapping at 90%).Three pacing protocols were applied in each mouse: 1) sinus rhythm (SR) (natural pacing), 2) right-atrial (RA) pacing and 3) right-ventricular (RV) pacing. Pacing was achieved using a nine-electrode catheter and catheterization through the right side of the heart, with each separately activated for varying the pacing location. Throughout the entire cardiac cycle, several waves were shown on the electromechanical images that propagated transmurally and/or from base to apex (septum) or apex to base (posterior wall). Through comparison of the cine-loop images obtained at different pacing protocols, we were able to identify and separate the electrically induced, or contraction, wave from the hemodynamic (or, blood-wall coupling) waves. The contraction wave was best observed along the posterior wall starting at the S-wave of the ECG, which occurs after Purkinje and during myocardial activation. Only the contraction wave changed direction when the pacing origin changed, i.e., it propagated from apex to base at SR and RA pacing and from base to apex at RV pacing. This reversal in the wave propagation direction was found to be consistent in all mice scanned and the wave velocities were found to be within the reported conduction wave range with statistically significant differences between SR/RA pacing (0.8496 +/-0.2214 m/s and 0.8379 +/- 0.1967 m/s), respectively and RV pacing (0.5213 +/- 0.3125 m/s). This pacing study demonstrates that electromechanical imaging may constitute the sole noninvasive method for conduction mapping of the entire left ventricle and thus diagnosis and treatment of dyssynchrony, arrhythmia or other conduction abnormalities.

Abstract:We continue our study of the use of fabricated data in the investigation of speech recognition algorithms. After reviewing the basic data generation algorithm and some earlier results involving the recognition of fabricated conversational speech data, we go on to describe some new and intriguing experiments concerning on the one hand, training acoustic models from simulated speech data, and on the other, recognizing fabricated data with different amounts of context in the language model. Among other things, we conclude that standard training algorithms are remarkably good at recovering the underlying structure in acoustic models when they are given 30 hours of data generated from those models. We also propose an alternative to perplexity for measuring the quality of a language model — the word error rate on fabricated data — that takes into account the inherent acoustic con-fusability of words.

First, in vivo imaging (30MHz) was performed on anesthetized, wild-type mice (n=12) at high frame rates in order to take advantage of the transient electromechanical coupling occurring in the myocardium. The RF signal acquisition in a long-axis echocardiographic view was gated between consecutive R-wave peaks of the mouse electrocardiogram (ECG) and yielded an ultra-high RF frame rate of 8000frames/s (fps). The ultrasound RF signals in each frame were digitized at 160MHz. Axial, frame-to-frame displacements were estimated using 1D cross-correlation (window size of 240μm, overlap of 90%). Three pacing protocols were sequentially applied in each mouse: (1) sinus rhythm (SR), (2) right-atrial (RA) pacing and (3) right-ventricular (RV) pacing. Pacing was performed using an eight-electrode catheter placed into the right side of the heart with the capability of pacing from any adjacent bipole. During a cardiac cycle, several waves were depicted on the electromechanical wave images that propagated transmurally and/or from base to apex, or apex to base, depending on the type of pacing and the cardiac phase. Through comparison between the ciné-loops and their corresponding ECG obtained at different pacing protocols, we were able to identify and separate the electrically induced, or contraction, waves from the hemodynamic (or, blood-wall coupling) waves. In all cases, the contraction wave was best observed along the posterior wall starting at the S-wave of the ECG, which occurs after Purkinje fiber, and during myocardial, activation. The contraction wave was identified based on the fact that it changed direction only when the pacing origin changed, i.e., it propagated from the apex to the base at SR and RA pacing and from base to apex at RV pacing. This reversal in the wave propagation direction was found to be consistent in all mice scanned and the wave velocity values fell within the previously reported conduction wave range with statistically significant differences between SR/RA pacing (0.85±0.22m/s and 0.84±0.20m/s, respectively) and RV pacing (−0.52±0.31m/s; p<0.0001). This study thus shows that imaging the electromechanical function of the heart noninvasively is feasible. It may therefore constitute a unique noninvasive method for conduction wave mapping of the entire left ventricle. Such a technology can be extended to 3D mapping and/or used for early detection of dyssynchrony, arrhythmias, left-bundle branch block, or other conduction abnormalities as well as diagnosis and treatment thereof.

Abstract:When heart is in ventricular fibrillation or atrial fibrillation, its electrical activities are very complex not only on spatial distribution but also on temporal variation. It is very difficult to find out its rules by traditional points by points mapping method. The 64/176-channel epicardial mapping system developed by Fudan University can be used to study the complex arrhythmias and has acquired some interesting results from the animal trials.The system includes a multi-electrodes patch, 64-channel ECG (electrocardiogram) signal amplifier, 80-channel A/D converter and a PC (personal computer). The electrode patch contains 8*8 array of electrodes with 2mm spacing. These 64 electrodes can be formed to 64 unipolar leads and 112 bipolar leads (totally 176 leads) for an electrocardiogram (ECG). Each electrode on the patch and the referenced electrode (which position is defined during the surgery) compose a unipolar lead. A pair of adjacent electrodes (include x-direction and y-direction) on the patch composes a bipolar lead. This unipolar leads and bipolar leads combined mapping technique make the independent channels of the ECG detecting greatly increased than the electrode quantity. The technique not only reduces the hardware and software expends but also increases the detecting precision. It uses finite hardware resources to detect depolarization wave from any way, any direction wherever in the whole mapping area.The system also can provide several techniques to analyze the depolarization wave signals from different sides, such as temporal, spatial and statistical.

Abstract:Isolated adult cardiac ventricular myocytes have been a useful model for cardiovascular research for more than 20 years. With the recent advances in cellular physiology and transgenic techniques, direct measurement of isolated ventricular myocyte mechanics is becoming an increasingly important technique in cardiac physiology that provides fundamental information on excitation-contraction coupling of the heart, either in drug intervention or pathological states. The goal of this article is to describe the isolation of ventricular myocytes from both rats and mice, and the use of real-time beat-to-beat simultaneous recording of both myocyte contraction and intracellular Ca2+ transient.

Abstract:Intracardiac wireless communication is crucial for the development of multi-chamber leadless cardiac pacemakers (LCP). However, the time-varying characteristics of intracardiac channel pose major challenges. As such, mastering the dynamic conduction properties of the intracardiac channel and modeling the equivalent time-varying channel are imperative for realizing LCP multi-chamber pacing. In this paper, we present a limiting volume variational approach based on the electrical properties of cardiac tissues and trends in chamber volume variation. This approach was used to establish a quasi-static and a continuous time-varying equivalent circuit model of an intracardiac channel. An equivalence analysis was conducted on the model, and a discrete time-varying equivalent circuit phantom grounded on the cardiac cycle was subsequently established. Moreover, an ex vivo cardiac experimental platform was developed for verification. Results indicate that in the frequency domain, the congruence between phantom and ex vivo experimental outcomes is as high as 94.3%, affirming the reliability of the equivalent circuit model. In the time domain, the correlation is up to 75.3%, corroborating its effectiveness. The proposed time-varying equivalent circuit model exhibits stable and standardized dynamic attributes, serving as a powerful tool for addressing time-varying challenges and simplifying in vivo or ex vivo experiments.

Abstract:The heart is a mechanical pump that is electrically driven. We have previously shown that the contractility of the cardiac muscle can reliably be used in order to assess the extent of ischemia. In this paper, in vivo imaging (35 MHz) was performed on anesthetized mice before and after left anterior descending (LAD) coronary artery ligation at high frame rates (Vevo 770, Visualsonics, Inc.) in order to better explore the electromechanical coupling within the heart muscle. The acquisition was triggered on the mouse electrocardiogram (ECG) and yielded a high frame rate of 8 kHz. RF signals from long-axis views were digitized and stored in real-time at a similar sampling rate for off-line processing. Axial displacements were estimated using 1D cross-correlation (window size of 60 microns, overlapping at 90%) and displayed at 8 kHz. The mice were initially imaged during sinus rhythm (i.e., at the natural pacing of the heart (100 ms period)), right-atrial pacing (at 100 ms) and right-ventricular pacing (at 100 ms). Pacing was achieved using catheterization through the right side of the heart, where the catheter carried nine electrodes that could be separately activated for varying the pacing location. In some of the scans, the catheter was within the imaging field-of-view and allowed for imaging of the pacing wave during ventricular pacing. The most pronounced wave propagating during sinus rhythm and atrial pacing was the contraction wave, or wave originating at the isovolumic contraction phase, that propagated along the longitudinal direction of the myocardium initiating radial thickening in its path. The contraction wave started at the apex, right at the QRS peak, and then propagated along the septum first and then the posterior wall. So, the direction of propagation was counterclockwise in a long-axis view. A relaxation wave was also detected propagating across the septum, i.e., from the endo- to the epicardial level. Right-atrial pacing, i.e., similar to sinus rhythm,- induced the same counter-clockwise pattern as in the sinus rhythm during contraction. Right-ventricular pacing induced a reverse direction on the contraction wave that started from the tip of the catheter (close to the base) with two waves propagating from base to apex, one along the septum and one along the posterior wall. In summary, strong contraction waves were imaged at 8 kHz frame rate in the septum (from base to apex) and the posterior wall (from apex to base) that changed direction or speed according to the pacing scheme induced. These preliminary results indicate that through electromechanical coupling, motion estimation at high frame rates may yield a novel non-invasive method for conduction mapping of the live myocardium and diagnosis of related diseases
Probe Contact Force Monitoring during Conductivity Measurements of the Left Atrial Appendage to Support the Design of Novel Diagnostic and Therapeutic Procedures

Abstract:The electrical properties of many biological tissues are freely available from the INRC and the IT'IS databases. However, particularly in lower frequency ranges, few studies have investigated the optimal measurement protocol or the key confounders that need to be controlled, monitored, and reported. However, preliminary work suggests that the contact force of the measurement probe on the tissue sample can affect the measurements. The aim of this paper is to investigate the conductivity change due to the probe contact force in detail. Twenty ex vivo bovine heart samples are used, and conductivity measurements are taken in the Left Atrial Appendage, a common target for medical device developments. The conductivity measurements reported in this work (between 0.14 S/m and 0.24 S/m) align with the literature. The average conductivity is observed to change by -21% as the contact force increases from 2 N to 10 N. In contrast, in conditions where the fluid concentration in the measurement area is expected to be lower, very small changes are observed (less than 2.5%). These results suggest that the LAA conductivity is affected by the contact force due to the fluid concentration in the tissue. This work suggests that contact force should be controlled for in all future experiments.

Abstract:A key challenge in cardiology is the non-invasive imaging of electric current propagation occurring in the cardiovascular system at an intra-cardiac scale. A promising approach for directly mapping the current dynamics is to monitor the associated stray magnetic field. However, in this magnetic field approach, the spatial resolution deteriorates significantly as the standoff distance between the target and the sensor increases. Existing sensors usually remain relatively far from the target and provide only centimetre-scale resolution because their operating temperature is not biocompatible. Here we demonstrate millimetre-scale magnetocardiography of living rats using a solid-state quantum sensor based on nitrogen-vacancy centres in diamond. The essence of the method is a millimetre proximity from the sensor to heart surface, which enhances the cardiac magnetic field to greater than nanoteslas and allows the mapping of these signals with intra-cardiac resolution. From the acquired magnetic images, we also estimate the source electric current vector, flowing from the right atria base via the Purkinje fibre bundle to the left ventricular apex. Our results establish the solid-state quantum sensor's capability to probe cardiac magnetic signals from mammalian animals and reveal their intra-cardiac electrodynamics. This technique will enable the study of the origin and progression of myriad cardiac arrhythmias including flutter, fibrillation, and tachycardia.

Abstract:Background: Understanding the dynamics of conduction velocity (CV) and voltage amplitude (VA) is crucial in cardiac electrophysiology, particularly for substrate-based catheter ablations targeting slow conduction zones and low voltage areas. This study utilizes ultra-high-density mapping to investigate the impact of heart rate and pacing location on changes in the wavefront direction, CV, and VA of healthy pig hearts. Methods: We conducted in vivo electrophysiological studies on four healthy juvenile pigs, involving various pacing locations and heart rates. High-resolution electroanatomic mapping was performed during intrinsic normal sinus rhythm (NSR) and electrical pacing. The study encompassed detailed analyses at three levels: entire heart cavities, subregions, and localized 5-mm-diameter circular areas. Linear mixed-effects models were used to analyze the influence of heart rate and pacing location on CV and VA in different regions. Results: An increase in heart rate correlated with an increase in conduction velocity and a decrease in voltage amplitude. Pacing influenced conduction velocity and voltage amplitude. Pacing also influenced conduction velocity and voltage amplitude, with varying effects observed based on the pacing location within different heart cavities. Pacing from the right atrium (RA) decreased CV in all heart cavities. The overall CV and VA changes in the whole heart cavities were not uniformly reflected in all subregions and subregional CV and VA changes were not always reflected in the overall analysis. Overall, there was a notable variability in absolute CV and VA changes attributed to pacing. Conclusions: Heart rate and pacing location influence CV and VA within healthy juvenile pig hearts. Subregion analysis suggests that specific regions of the heart cavities are more susceptible to pacing. High-resolution mapping aids in detecting regional changes, emphasizing the substantial physiological variations in CV and VA.

Abstract:OBJECTIVE : Using a combination of high-frequency ultrasonography, ECG gating, and RF-based motion estimation, our laboratory has developed a novel technique for the noninvasive mapping of contraction waves in the intact murine left ventricle. In this study, we sought to measure the velocity and direction of contraction waves in the murine heart during native conduction and pacing. METHODS : Twelve wild-type mice were anesthetized with IP pentobarbital, and an octapolar pacing catheter introduced into the right ventricle (RV) through the external jugular vein. In vivo ultrasound RF frames (30 MHz) were acquired at 8000 fps (Vevo 770, Visualsonics, Inc.), using ECG gating. Axial frame-to-frame motion were estimated using a speckle tracking method. Images were obtained in the parasternal long axis view under three conditions; sinus rhythm (SR), right atrial (RA) pacing, and RV pacing. Contraction was analyzed using ciné-loop images. RESULTS : Distinct contraction patterns were noted during isovolumic contraction (IC) ejection under all conditions. In SR, during IC, convergent motion of the septal wall (SW) and posterior wall (PW) began during the latter half of the QRS at the apex and spread basally, causing divergent movement at the base. At the onset of ejection, medial contraction spread to the base. Similar results were obtained with RA pacing. During RV pacing, the IC phase began with convergent motion of the basal SW and PW that spread apically, causing divergent motion of the apical SW and PW. During ejection, convergent motion of the apical SW and PW developed which spread to the basal SW and PW. This difference between SR/RA pacing and RV pacing was found in all mice (n=12). We found a significantly slower contraction wave velocity during RV pacing (0.5213 +/- 0.3125 m/s) than during SR/RA pacing (0.8496 ± 0.2214 and 0.8379 ± 0.1967 m/s, respectively) (p<0.01), in line with previously reported results. CONCLUSION : This study demonstrates the ability of this modality to noninvasively map the velocity and direction of contraction waves. Using this technique, we demonstrate the difference in motion patterns between native conduction and ventricular pacing. This technique has potential applications in the noninvasive assessment of ventricular dyssynchrony.

Abstract:The sensing of left ventricular (LV) activity is fundamental in the diagnosis and monitoring of cardiovascular health in high-risk patients after cardiac surgery to achieve better short- and long-term outcome. Conventional approaches rely on noninvasive measurements even if, in the latest years, invasive microelectromechanical systems (MEMS) sensors have emerged as a valuable approach for precise and continuous monitoring of cardiac activity. The main challenges in designing cardiac MEMS sensors are represented by miniaturization, biocompatibility, and long-term stability. Here, we present a MEMS piezoresistive cardiac sensor capable of continuous monitoring of LV activity over time following epicardial implantation with a pericardial patch graft in adult minipigs. In acute and chronic scenarios, the sensor was able to compute heart rate with a root mean square error lower than 2 BPM. Early after up to 1 month of implantation, the device was able to record the heart activity during the most important phases of the cardiac cycle (systole and diastole peaks). The sensor signal waveform, in addition, closely reflected the typical waveforms of pressure signal obtained via intraventricular catheters, offering a safer alternative to heart catheterization. Furthermore, histological analysis of the LV implantation site following sensor retrieval revealed no evidence of myocardial fibrosis. Our results suggest that the epicardial LV implantation of an MEMS sensor is a suitable and reliable approach for direct continuous monitoring of cardiac activity. This work envisions the use of this sensor as a cardiac sensing device in closed-loop applications for patients undergoing heart surgery.

Abstract:Quantitative microPET imaging provides the capability of performing biochemical and physiological investigations. The collection of multiple blood samples over time is necessary for quantitative analysis. The procedure is challenging because the amount of blood volume is small in rats and mice. To overcome this issue, an automatic blood sampling system was developed using a novel microfluidic chip design to take serial blood samples (<0.25 muL per sample; 18 blood samples in current design) at precise times from rats and mice. In this work, we utilize a National Instruments DAQPad 6507 digital output computer interface to control the sampling on the chip via mini-pneumatic valves. The system provides a user-friendly GUI programmed by LabVIEW to setup the sampling information, which includes mouse ID, tracer selection and sampling volume and time sequences. It can be easily reconfigured to utilize new microfluidic chip designs as well as changes in the sampling protocol. Real-time sampling status and the output of sampling information is continually displayed. Our initial implementation of this design can automatically collect ~0.185 muL blood samples within less than 2 second intervals from a small rodent.
